    زمینه و هدف
    بسیاری از بیماران با بدخیمی های دستگاه گوارش در مرحله ای به درمان جراحی می رسند که مدتها است علامت دار بوده اند و برخی از آنان در مرحله غیر قابل علاج هستند. لازم است که تمامی عوامل ممکن در ایجاد مشکل فوق بررسی و راهکارهای کاربردی آن مشخص شود.
    روش بررسی
    در یک بررسی آینده نگر بیماران مبتلا به سرطانهای مری، معده و کولورکتال که برای اولین جراحی مراجعه کرده بودند، بررسی شدند و از شرح حال، مدارک تشخیصی، پرونده بخش جراحی و گزارش پاتولوژی بیماران، اطلاعات لازم جهت بررسی میزان تاخیر در تشخیص و درمان و عوامل موثر در آنها استخراج شد.
    یافته ها
    اطلاعات 109 بیمار مورد بررسی قرار گرفت که 42 نفر (38.5%) بدخیمی مری، 35 نفر (32.1%) بدخیمی معده و 32 نفر (29.4%) بدخیی کولورکتال داشتند. متوسط فاصله زمانی از علامت دار شدن بیماری تا بستری 348 روز و از علامت دار شدن بیماری تا انجام جراحی 367 روز بوده است. اجزای عمده این تاخیر به ترتیب عبارتند از: عدم ارجاع به موقع توسط پزشک عمومی (124 روز)، درمان طبی نادرست توسط متخصصان (100 روز)، آندوسکوپی نامناسب (58 روز)، تاخیر به علت مراجعه دیر بیمار (41 روز). درمان طبی نابجای پزشکان عمومی در بدخیمی های معده در مقایسه با بدخیمی های مری به طور معنی داری بیشتر بود (p-value=0.01). آندوسکوپی نادرست در بیماران با بدخیمی های مری و معده تاخیر بیشتری نسبت به بدخیمی کولورکتال ایجاد کرده بود (p-value=0.016). نیاز به تکرار آندوسکوپی نادرست در بیماران با سرطان مری، معده و کورکتال به ترتیب 43%، 51% و 22% بوده است (p-value=0.040).
    نتیجه گیری
    به نظر می رسد تاخیر عمده در درمان سرطانهای دستگاه گوارش مرتبط با درمانهایی است که به صورت سرپایی در سطح جامعه توسط پزشکان انجام می شود. تخصیص قسمتی از برنامه های آموزش مداوم پزشکان، به ویژه پزشکان عمومی، به سرطان های دستگاه گوارش برای تاکید بر علایم بیماری و شک بالینی، اندیکاسیون بررسی های تشخیصی یا ارجاع، انواع آزمونهای تشخیصی و ارزش هر یک، از راهکارهای دارای اولویت می باشند.

    زمینه و هدف
    ریفلاکس معده به مری (GER) با عبور محتویات معده به داخل مری و بیماری ریفلاکس معده به مری (GERD) با علایم و مشکلات حاصل از برگشت محتویات معده مشخص می شود و از شایعترین مشکلات در کودکان است.
    در این مطالعه، آزمونهایی که به طور معمول برای تشخیص ریفلاکس معده به مری استفاده می شوند. برای دستیابی به یک آزمون حساس و اختصاصی که درجه تهاجمی کمتری نیز داشته باشد، مورد بررسی قرار می گیرند.
    روش بررسی
    این مطالعه آینده نگر مقطعی را برای مقایسه دقت بلع باریم و سونوگرافی در تشخیص ریفلاکس معده به مری در مقایسه با pH متری انجام دادیم. پنجاه کودک (یک ماهه تا 15 ساله) با علایم بیماری ریفلاکس معده به مری به مطالعه وارد شدند. پس از کسب رضایت نامه از والدین برای بلع باریم، سونوگرافی و pH متری 24 ساعته، هر اقدام توسط یک پزشک متخصص، که هیچ یک از نتایج دیگری اطلاع نداشتند، انجام شد.
    یافته ها
    در بیمارانی که نتیجه pH متری آنها مثبت بود، نتایج 90% موارد سونوگرافی (p=0.004) و 50% موارد بلع باریم (p=0.02) مثبت به دست آمد. میزان منفی کاذب در بلع باریم 50% و در سونوگرافی 10% بود. میزان مثبت واقعی در سونوگرافی 90% و در بلع باریم 50% و میزان منفی واقعی در سونوگرافی 40% و در بلع باریم 35% بود. میزان حساسیت برای سونوگرافی 90% و برای بلع باریم 50% و میزان ویژگی برای سونوگرافی 40% و برای بلع باریم 35% به دست آمد. میزان پیشگویی مثبت (positive perdictive value-PPV) برای سونوگرافی 69% و برای بلع باریم 53% بود و میزان پیشگویی منفی (negative predictive value-NPV) برای سونوگرافی 72% و برای بلع باریم 31% بود.
    نتیجه گیری
    مطالعه ما نشان داد که سونوگرافی از حساسیت و ویژگی و میزان پیشگویی مثبت و منفی بالاتری نسبت به بلع باریم برخوردار است و با توجه به هزینه کمتر و بی خطر بودن انجام آن برای کودکان، می توان آن را در خط اول تشخیص برای بیماران مشکوک به برگشت محتویات معده به مری قرار دارد و توصیه می شود از pH متری 24 ساعته برای پیگیری بیماران مقاوم به درمان استفاده گردد.

    زمینه و هدف
    در جامعه ما 1.3 درصد افراد، دارای آنتی بادی ضد HBc در سرم خون می باشند. هدف اصلی این مطالعه مشخص کردن میزان سروکاتورژن آنتی بادی ضد HBs، پس از واکسیناسیون، در افرادی است که به صورت پایدار، تنها دارای آنتی بادی ضد HBc هستند.
    روش بررسی
    در این مطالعه نیمه تجربی، 94 نفر از مراجعه کنندگان به سازمان انتقال خون تهران که تنها دارای آنتی بادی ضد HBc بودند، به روش غیر احتمالی آسان، طی سالهای 1381 تا 1383 مورد مطالعه قرار گرفتند. این افراد تحت واکسیانسیون هپاتیت B نو ترکیب در سه نوبت و به فاصله 1 و 6 ماه پس از ترزیق اول قرار گرفتند. ایمنی زایی با تعیین تیتر HBsAb (بیشتر از 10mIU: ایمن) ارزیابی شد.
    یافته ها
    میزان پاسخ به واکسیانسیون هپاتیت B پس از واکسیانسیون نوبت اول، دوم و سوم، به ترتیب 27.7%، 61.7% و 75.5% بود. پاسخ کلی به واکسیانسیون در 75 نفر (79.8%) مشاهده شد. 19 نفر (20.2%) حتی پس از سه بار واکسیانسیون نیز دارای سطح ایمنی از anti-HBs در سرم خود نشدند. مثبت بودن آنتی بادی anti-HBs، وجود سابقه حجامت و سابقه قبلی مثبت بودن HBsAg، احتمال پاسخ به واکسیانسیون را به ترتیب به طور معنی داری 6.4 برابر (p=0.008)، 3.1 برابر (p=0.039) و 5.6 برابر (p=0.002) کاهش می داد.
    نتیجه گیری
    به طور کلی، اگرچه در مطالعات مختلف در مقایسه با مطالعه ما تفاوتهایی در میزان پاسخ به واکسیانسیون پس از تزریق اول، دوم و سوم دیده می شود، درصد پاسخ کلی به واکسیانسیون بسیار نزدیک به اکثر مطالعات مشابه است.

    زمینه و هدف
    هر چند بیماری ریفلاکس معده به مری (gastroesophageal reflux disease-GERD) از بیماری های شایع دستگاه گوارش است، علل موثر در ایجاد آن شناخته شده نیست. اطلاعات کمی در زمینه ارتباط اختلالات روان با GERD وجود دارد. از آنجا که تاکنون در ایران، ارتباط وضعیت روان و ریفلاکس بررسی نشده است، نقش عوامل روانی در بیماری مطالعه شد.
    روش بررسی
    1016 نفر به صورت تصادفی انتخاب و به مطالعه دعوت شدند، با افرادی که شرکت در مطالعه را پذیرفتند مصاحبه حضوری انجام شد (1000 نفر با محدوده سنی 40-18 سال). کسانی که در طی 12 ماه گذشته، حداقل یک بار سوزش پشت جناغ سینه و یا برگشت اسید را داشتند، به عنوان مورد و گروهی که هرگز این علایم را نداشتند، به عنوان شاهد در نظر گرفته شدند. در بررسی وضعیت روان از پرسشنامه (general health questionnaire) GHQ-28، که اختلالات غیر سایکوتیک روان را ارزیابی می کند، استفاده شد.
    یافته ها
    شیوع ریفلاکس هفتگی، 12.3% بود. اختلالات روان تنی (OR: 3.50; %95 CI: 2.17-5.66)، اضطراب (OR: 4.02; %95 CI: 2.51-6.43)، اختلالات عملکرد اجتماعی (OR: 1.95; %95 CI: 1.21-3.17)، افسردگی (OR: 3.29; %95 CI: 1.95-5.50)، افزایش سن و چاقی با بروز بیشتر GERD همراه بودند.
    نتیجه گیری
    شیوع GERD در این منطقه، بیش از اغلب کشورهای آسیایی و قابل مقایسه با کشورهای غربی است. نتایج حاصل از این مطالعه همراهی معنی دار اختلالات روان با علایم ریفلاکس را نشان می دهند. ممکن است عومل روانی - اجتماعی نقش مهمی در ایجاد علایم GERD داشته باشند.

    به نظر می رسد همزمانی آرتریت تاکایاسوو بیماری کرون پدیده ای نادر باشد. بنابر دانسته های ما، تاکنون در ایران هیچ موردی از ابتلای همزمان به هر دو بیماری گزارش نشده است. در گزارش موردی، دختر ایرانی 22 ساله ای معرفی می شود که مورد شناخته شده بیماری کرون بوده و متعاقبا به آرتریت تا کایاسو مبتلا شده است.

    پانکراتیت خود ایمنی اختلال نادری است که امکان دارد به صورت انسداد مجاری صفراوی مشترک نمایان شود. بیمار، آقای 75 ساله ای بود که به علت دل دردهای مکرر و زردی همراه با افزایش بیلی روبین و آلکالن فسفاتاز سرمی و مشاهده هتروژنسیته سر پانکراس در سی تی اسکن و اتساع مجرای صفراوی داخل و خارج کبدی، بیماری وی سرطان سرپانکراس تشخیص داده شد و استنت مجرای صفراوی مشترک گذاشته شد.
    بیمار به علت ادامه دردهای مکرر مراجعه کرد، ولی با توجه به طبیعی شدن بیلی روبین و آلکالن فسفاتاز سرمی در سیر بیماری، شاخصهای طبیعی توموری و افزایش قابل توجه IgG4 و آنتی بادی ضد هسته ای برای بیمار آندوسونوگرافی انجام شد و از هتروژنسیته سرپانکراس نمونه برداری به عمل آمد. در پاتولوژی نمونه های به دست آمده از سرپانکراس، فیبروز همراه با آنفیلتراسیون لنفوپلاسماسیتیک بدون شواهدی از بدخیمی گزارش شد.

    شناخت پروتئینهای بافت سرطانی و متمایز کردن آنها از بافت معمولی و خروج آنها از سلول به خون، یکی از رشته های مهم تحقیقات در سالهای اخیر است که با نام «تحقیقات پروتئومیکس» (شناخت پروتئینهای بافت) خوانده می شود و نتیجه این تحقیقات، تشخیص زود هنگام و قبل از بروز علایم سرطان پیشرفته به کمک شناخت شاخصهای تومور در خون است.

    Background
    Many patients with gastrointestinal malignancies undergo false treatments and incorrect diagnostic studies before diagnosis. This study considered all possible causes to propose prior solutions.
    Materials And Methods
    Patients with gastrointestinal malignancies (esophageal, gastric, and colorectal) who were admitted for the first surgery were assessed. The delay of diagnosis and treatment and responsible factors were analyzed from patient's history by interview and reviewing paraclinical data, patient's records, and pathology report.
    Results
    109 patients were assessed with esophageal cancer (42 patients), gastric cancer (35 patients), and colorectal cancers (32 patients). Mean duration "from being symptomatic to admission" was 348 days and the mean duration "from being symptomatic to surgery" was 366 days. The most important parts of the delay were: "not being referred by general practitioners" (124 days), "incorrect medical treatment by specialists (100 days), inadequate endoscopy (58 days) and ignoring symptoms by patients (41 days). The mean duration of incorrect medical treatment by general practitioners in gastric cancer was more in compare to esophageal cancers (p-value= 0.01). Inadequate endoscopy resulted in more delay in esophageal and gastric than colorectal cancers (p-value= 0.016). Need to repeat endoscopy in patients with esophagus, gastric and colorectal cancer were 43%, 51%, and 22% respectively (p-value= 0.040).
    Conclusion
    The most important part of delay in surgical treatment of gastrointestinal malignancies was related to incorrect outpatient treatments by general practitioners and specialists. It seems that the best solution to reduce time of diagnosis in gastrointestinal malignancies is continuing education for physicians, especially general practitioners.

    Background
    Gastroesophageal reflux (GER), defined as passage of gastric contents into the esophagus, and GER disease (GERD), defined as symptoms or complications of GER, are common pediatric problems. The value of commonly used diagnostic tests for GER is examined to define the most sensitive and specific test that is the least invasive as well.
    Materials And Methods
    We conducted this cross sectional prospective study to compare the accuracy of barium meal (BM) and sonography (US) in diagnosing GER with the gold standard test, PH monitoring (pH-m). Fifty children (1 mo- 15 yr) with GERD symptoms were enrolled. After filling the informed by their parents, BM, US and 24 hr PH-m were done by one physician who were blinded to each others.
    Results
    Patients with positive pH-m had Positive US and BM 90% (p = 0.004) and 50% (0.02) respectively and in those with negative pH-m the negative rate of us was 60% (p = 0.004) and negative rate of BM was 65% (P = 0.02). The rates of false positive and false negative were 60% and 10% in US and 65% and 50% in barium meal respectively. True positive and negative results were 90% and 40% for US and 50% and 35% for BM, respectively. The sensitivity and specificity rates were 90% and 40% for US and 50% and 35% for BM. The positive and negative predictive values were 69% and 72% for US and 53% and 31% for BM respectively.
    Conclusion
    Ultrasonography is much more sensitive and specific than barium meal in diagnosing GER with higher positive and negative predictive value. US is a safe, reliable and cost- effective procedure, so it could be used as the primary mode in diagnosing GER and use 24 hr pHm for follow up of resistant to treat patients.

    Background
    1.3% of people in Iran are positive for anti-HBc antibody.The objective of this study is to determine the rate of seroconversion in those with persistent isolated anti-HBc after vaccination.
    Materials And Methods
    In a quasi-experimental study, 94 persons with isolated anti-HBc antibody who were referred to Tehran Blood Transfusion Organization between 2002 and 2004 were selected through a convenient sampling. They were given recombinant hepatitis B vaccine on 0, 1 and 6 months. Immunization was defined by anti-HBs antibody titer |≥|10 mIU/mL, 1-2 months after administration of the last dose.
    Results
    Response to HB vaccination consisted of 28%, 62% and 76% after each injection, respectively. The total response rate was 80% (n=75). Nineteen (20%) persons were negative for anti-HBs even after three doses of HB vaccination. Factors associated with decreased response to vaccination were positive anti-HBe (OR=6.4, P=0.008), history of bloodletting (OR=3.1, P=0.039) and prior history of being HBsAg positive (OR=5.6, P=0.002).
    Conclusion
    Although there were some differences between the response rates after each vaccine injections observed in our study and similar ones, the total response was nearly the same.

    Background
    Although gastroesophageal reflux disease (GERD) is a common disorder, its etiologies are still largely unknown. There are limited data on the association between psychological status and GERD in the world, in general, and in Iran, in particular. This study was conducted to determine the role of psychological factors in causing GERD.
    Materials And Methods
    1,016 randomly-selected subjects were invited for a face-to-face interview. 1,000 persons participated in this study (age range: 18-40 years). Those who reported heartburn and/or regurgitation, at least once a week, during the last 12 months were defined as GERD patients. Controls were persons who did not complian of those symptoms. The psychosocial status was assessed by a reliable general health questionnaire for Iranian, GHQ-28.
    Results
    The prevalence of "at least weekly GERD" was 12.3%. Somatic disorders (OR: 3.50; 95% CI: 2.17-5.66), anxiety (OR: 4.02; 95% CI: 2.51-6.43), social disorders (OR: 1.95; 95% CI: 1.21-3.17), and depression (OR: 3.29; 95% CI: 1.95-5.50), increasing age and obesity were associated with developing GERD.
    Conclusion
    The prevalence of GERD in northern Iran is more than that in most Asian countries and is comparable to Western countries. The result of this study indicates significant association between psychiatric disorders with reflux symptoms. The psychosocial factors may play important roles in the production of GERD symptoms.

    Autoimmune pancreatitis (AIP) is a rare disorder that may be present as common bile duct stricture. The patient was a seventy-year-old man with presentations as recurrent abdominal pain and jaundice, elevated Billirobin (Bil) and elevated Alkaline Phosphatase (ALKP) in association with the presence of hetrogenesity of the pancreatic head and dilated intra and extra hepatic bile ducts in CT. Such clinical features confirmed the diagnosis of cancer the head of pancreas, therefore, was taken common bile duct stent for him. The patient came back with continuation recurrent abdominal pain and normal (Bil, ALKP and tumor markers) but remarkable elevated IgG4 and anti nuclear antibody. The patient was biopsied of hetrogenesity of the Pancreatic head by Endoscopic Ultra Sonographic (EUS)-guided. In the pathology of the pancreatic head, was observed fibrosis with Lymphoplasmacitic infiltration without the presences of malignancy. Finally, the patient was treated with the diagnosis of autoimmune Pancreatitis and was gone out bile duct stent.

    Background
    There is evidence of a possible etiological role of human papillomaviruses (HPVs) in the development of colorectal cancer. Loss of p53 tumor suppressor gene function has been found in many malignancies and it can occur in a variety of ways, including gene mutation and interaction with the E6 protein of oncogenic HPVs. The objective of this study was to determine the prevalence of HPV infection and p53 overexprssion in colorectal cancer (CRC) tissue samples and its association with tumor histopathologic characteristics.
    Materials And Methods
    60 tissue sections from patients with CRC were investigated by immunocytochemistry techniques for aberrant expression of p53 using the streptavidin-biotin-peroxidase method with monoclonal antibodies. The HPV status was also analyzed using type-specific primers for HPV16/18 by polymerase chain reaction (PCR).
    Results
    Overall, 21 (35%) of 60 patients were found positive for HPV DNA; HPV 18 was detected in 19 (32%) and HPV16 in 11 (18%) of 60 samples. An abnormal expression of tumor-suppressor protein p53 was observed in 29 (48%) samples. p53 overexpression was observed in 15 (71%) of 21 HPV-positive and in 14 (36%) of 39 HPV-negative patients (p=0.009). Similar significant difference was found in p53 overexpression in HPV18-positive patients (p=0.007) but not in those positive for HPV 16 (p=0.261). HPV DNA presentation was not significantly associated with histopathologic characteristics including tumor stage (p=0.509), grade (p=0.668), peri-neural invasion (p=0.265) and lympho-vascular invasion (p=0.275).
    Conclusions
    p53 inactivation caused by HPV infection may play a role in the pathogenesis of colorectal cancer. There is no association between HPV infection and histopathologic characteristics.

    Takayasu's arteritis and Crohn's disease co morbidity seems to be rare. To our knowledge, no patient with Crohn - Takayasu syndrome has been reported yet from our region. Herein we present a 22 year old Iranian woman previously diagnosed with Crohn's disease with subsequent development of Takayasu's arteritis.
