صفحات 73-79مقدمهپنومونی وابسته به ونتیلاتور (Ventilator-associated pneumonia یا VAP)، از جمله شایع ترین علل مرگ و میر بیماران بستری در بخش مراقبت های ویژه (Intensive care unit یا ICU) است. مطالعات اندکی در زمینه ی انواع VAP در بیمارانی که به علت جراحی های اعصاب تحت ونتیلاسیون قرار گرفته اند، انجام شده است؛ در حالی که احتمال VAP در این دسته از بیماران به دلیل خطر آسپیراسیون اوروفارنژیال (Oropharyngeal aspiration یا OPA) به دنبال آسیب مغزی زیاد است. هدف از انجام پژوهش حاضر، بررسی اتیولوژی انواع VAP زودرس و دیررس در بیماران تحت جراحی های اعصاب بود.روش هااین مطالعه ی مقطعی، بر روی 82 بیمار مبتلا به VAP به دنبال انجام جراحی های اعصاب صورت گرفت. اطلاعات بیماران با استفاده از چک لیستی شامل اطلاعات دموگرافیک، نوع VAP (زودرس/ دیررس)، امتیاز Acute Physiology and Chronic Health Evaluation II (APACHE II)، نمره ی Clinical Pulmonary Infection Score (CPIS)، علت جراحی مغز و اعصاب، سابقه ی بیماری گذشته، مصرف دخانیات، نتایج اسمیر و کشت و آنتی بیوگرام بیماران ثبت گردید و مورد تجزیه و تحلیل قرار گرفت.یافته هاارتباط معنی داری بین نوع VAP با سن (560/0 = P)، جنسیت (270/0 = P)، علت جراحی (960/0 = P) و سابقه ی بیماری (930/0 = P) وجود نداشت، اما ارتباط معنی داری بین نوع VAP با تعداد روز بستری (034/0 = P)، مصرف دخانیات (001/0 > P)، امتیاز APACHE بدو ورود (001/0 > P) و نمره ی CPIS (001/0 > P) مشاهده شد. تعداد روز بستری [040/0 = P، 04/35-1/21 = Confidence interval 95% (CI95%)، 71/4 = Odds ratio (OR)] و مصرف دخانیات (015/0 = P، 01/14-1/1 = CI95%، 07/1 = OR) به طور مستقل با ابتلا به VAP مرتبط بود. ارتباط معنی داری بین نوع VAP با رنگ آمیزی گرم (330/0 = P) و آنتی بیوگرام (140/0 = P) نمونه ها یافت نشد.نتیجه گیریبر اساس نتایج مطالعه ی حاضر، تعداد روز بستری، مرگ و میر، مصرف دخانیات، امتیاز APACHE و نمره ی CPIS به صورت معنی داری در VAP دیررس بالاتر می باشد. همچنین، تعداد روز بستری و مصرف دخانیات در ابتلا به VAP بدون در نظر گرفتن نوع آن، موثر بود. الگوی میکروبی بیماران ارتباطی با ابتلا به VAP دیررس/ زودرس نداشت.کلیدواژگان: پنومونی وابسته به ونتیلاتور، جراحی های اعصاب، بخش مراقبت های ویژه

صفحات 80-85مقدمهامروزه بهترین راهبرد برای کنترل، مدیریت و پیش گیری از مسمومیت ها، شناسایی الگوهای مسمومیت و آموزش آن ها می باشد. با توجه به اهمیت این موضوع و جهت پیش گیری از مسمومیت ها، این مطالعه با هدف ارزیابی نگرش و دانش مردم اصفهان نسبت به عوامل مسمومیت زا در هفته ی مسمومیت انجام شد.روش ها280 نفر به صورت تصادفی در طی هفته ی پیش گیری از مسمومیت ها وارد این مطالعه ی مقطعی شدند و پرسش نامه ی محقق ساخته را کامل کردند. پرسش نامه، شامل سه بخش اطلاعات جمعیت شناختی، آگاهی سنجی و نگرش سنجی بود. روایی و پایایی این پرسش نامه مورد تایید قرار گرفت (784/0 = Cronbach's alpha).یافته ها51 درصد افراد مورد مطالعه مرد و 49 درصد زن بودند و میانگین سنی آنان 37 سال بود. میانگین آگاهی افراد شرکت کننده نسبت به عوامل مسمومیت زا، برابر با 10/2 ± 57/6 درصد و میانگین نگرش برابر با 00/9 ± 69/32 درصد و میانگین آگاهی و نگرش در دو جنس، برابر بود. رابطه ی مثبتی میان سن با میانگین آگاهی وجود داشت. متاهلین نسبت به مجردها، افراد با تحصیلات بالاتر نسبت به افراد با تحصیلات کمتر و دارندگان شغل اداری نسبت به سایر مشاغل، از میانگین دانش و نگرش بیشتری در زمینه ی مسمومیت برخوردار بودند.نتیجه گیریمیانگین آگاهی مردم اصفهان در زمینه ی مسمومیت ها در حد متوسط و سطح نگرش آنان نسبت به برنامه های پیش گیری از مسمومیت در سطح خوب برآورد شد. با افزایش آگاهی و سطح نگرش افراد نسبت به موارد مسمومیت زا، انتظار می رود راهبردهای پیش گیری ارتقا یابند.کلیدواژگان: آگاهی، نگرش، مسمومیت

صفحات 86-92مقدمههدف از انجام مطالعه ی حاضر، ارزیابی اثرات آسیب حاد کلیه و نقش استروژن در این رابطه، بر یادگیری و حافظه ی اجتنابی غیر فعال در Ratهای ماده ی اورکتومی شده، بود.روش هاحیوانات مورد آزمایش، 40 Rat ماده ی اورکتومی شده از نژاد Wistar بودند که به چهار گروه Sham- حلال (Sham-vehicle یا Sham-V)، Sham- استروژن (Sham-estrogen یا Sham-E)، ایسکمی- رپرفیوژن- حلال (Renal ischemia reperfusion-vehicle یا RIR-V) و ایسکمی- رپرفیوژن- استروژن (Renal ischemia reperfusion-estrogen یا RIR-E)، تقسیم شدند. آسیب حاد کلیه با استفاده از مدل ایسکمی- رپرفیوژن القا گردید. استروژن زیرجلدی (200 میکروگرم/کیلوگرم) هر هفته یک بار به مدت 4 هفته تزریق گردید. سپس، یادگیری و حافظه ی اجتنابی ارزیابی شد.یافته هاآسیب حاد کلیه در Rat های اورکتومی شده، میانگین زمان تاخیر ورود به اتاقک تاریک را به صورت معنی داری کاهش داد (010/0 > P). تزریق استروژن، اگر چه در پی ایجاد آسیب حاد کلیوی منجر به افزایش سطوح کراتینین و اوره ی خون گردید، اما تفاوت معنی داری در تاخیر ورود به اتاقک تاریک در گروه ایسکمی- رپرفیوژن در مقایسه با گروه حلال نشان نداد (100/0 = P).نتیجه گیریآسیب حاد کلیه در Ratهای ماده ی اورکتومی شده، می تواند بر حافظه ی اجتنابی غیر فعال اثرات مخرب داشته باشد. با این وجود، استروژن در دز مورد مطالعه نتوانست از اختلال حافظه ی اجتنابی غیر فعال ناشی از آسیب حاد کلیه در Ratهای ماده ی اورکتومی شده، جلوگیری نماید.کلیدواژگان: آسیب حاد کلیه، اورکتومی، حافظه، استروژن

صفحات 93-100مقدمهژن ویژه ی توقف رشد 2 (Growth arrest-specific 2 یا GAS2) در فعالیت های سلولی متعددی مانند چرخه ی سلولی، مرگ برنامه ریزی شده و تکثیر سلولی دخالت دارد و ممکن است به طور بالقوه در گسترش سرطان نقش داشته باشد. با این حال، ارتباط GAS2 با پیشرفت و پیش آگهی سرطان کولورکتال مشخص نیست و نیاز به بررسی دارد. بنابراین، مطالعه ی حاضر با هدف بررسی رابطه ی بین بیان GAS2 بافت توموری با گسترش و پیش آگهی سرطان کولورکتال انجام شد.روش هادر این مطالعه ی مورد- شاهدی، نمونه های جراحی بافت توموری و بافت سالم مجاور آن، از 40 بیمار مبتلا به سرطان کولورکتال در انستیتو کانسر بیمارستان امام خمینی تهران جمع آوری گردید و میزان نسبی GAS2 بافت ها با روش Real-time polymerase chain reaction (Real-time PCR) کمی سنجش گردید. ارتباط میزان بیان GAS2 در بافت توموری با پارامترهای بالینی- آسیب شناختی و میزان بقای کلی بیماران مشخص گردید.یافته هامیزان نسبی GAS2 در بافت های توموری نسبت به بافت های سالم مجاور افزایش قابل توجهی داشت [(40/3-17/1) 96/1 در مقابل (31/1-00/1) 10/1؛ 001/0 > P]. به علاوه، میزان نسبی GAS2 در بافت های توموری ارتباط معنی داری با ویژگی های آسیب شناختی سرطان نظیر مرحله بندی Tumor spread، Lymph node و Metastasis (TNM) (010/0 = P)، درجه (010/0 = P)، اندازه (030/0 = P) و تهاجم تومور به سیستم لنفاوی (030/0 = P) و عروقی (040/0 = P) و همین طور کاهش بقای کلی بیماران (040/0 = P) نشان داد.نتیجه گیریافزایش بیان GAS2 در سرطان کولورکتال با شاخص های پیشرفت سرطان و پیش آگهی ضعیف بیماری ارتباط دارد و بنابراین، ممکن است به عنوان یک نشانگر زیستی پیش آگهی در سرطان کولورکتال عمل کند.کلیدواژگان: پروتئین Growth arrest-specific 2 انسانی، مرحله بندی سرطان، پیش آگهی

صفحات 101-107مقدمهسلول های بنیادی مزانشیمی (Mesenchymal stem cells یا MSCs)، سلول های بنیادی چند توانه ای هستند که تمایز آن ها به کندروسیت ها در سال های اخیر مورد توجه قرار گرفته است. پلاسمای غنی از پلاکت (Platelet-rich plasma یا PRP)، به دلیل غنی بودن از فاکتورهای رشد متعدد و عامل هایپراسمولاریتی، به دلیل نزدیک کردن محیط تمایزی به محیط اصلی کندروسیت ها، می تواند بر این تمایز موثر باشد. با توجه به ایجاد عامل نامطلوب هایپرتروفی و استخوانی شدن، پژوهش حاضر با هدف دستیابی به یک محیط بهینه ی تمایز غضروفی بر MSCs انسانی انجام شد.روش هاMSCs بافت چربی انسانی استخراج و به رده ی سلولی غضروف تمایز داده شد. اثر محیط های پایه حاوی فاکتور رشد تغییر دهنده ی بتا (Transforming growth factor beta یا TGF-β)، PRP، هایپراسمولاریتی، محیط پایه + هایپراسمولاریتی و PRP + هایپراسمولاریتی بر تمایز غضروفی با استفاده از رنگ آمیزی های اختصاصی شامل Alcian blue، هماتوکسیلین ائوزین (Hematoxylin and Eosin یا H&E) و ایمونوسیتوشیمی کلاژن نوع 2 و 10 بررسی گردید. شاخص های بیوشیمیایی شامل فعالیت Alkaline phosphatase (ALP) و رسوب کلسیم و بررسی بیان ژن های Col2، Sox9، Smad2 و Aggrecan نیز با استفاده از روش Real-time polymerase chain reaction (RT-PCR) مورد سنجش قرار گرفت.یافته هاMSCs بافت چربی انسانی با دارا بودن قدرت تمایز به رده ی سلولی چربی و استخوان، ویژگی های سلول های غضروفی را در همه ی گروه های تمایزی نشان داد. فعالیت ALP و میزان رسوب کلسیم در گروه تمایزی پایه + هایپراسمولاریتی نسبت به سایر گروه ها کمتر بود.نتیجه گیریگروه تمایزی پایه + هایپراسمولاریتی می تواند محیط بهتری جهت تمایز غضروفی باشد.کلیدواژگان: سلول های بنیادی مزانشیمی، تمایز سلولی، پلاسمای غنی از پلاکت، کندروسیت

Pages 73-79BackgroundVentilator-associated pneumonia (VAP) is among the most common etiologies of mortality in intensive care unit (ICU). Studies about VAP among patients under mechanical ventilation admitted for neurosurgery procedures are limited while probability of VAP among these patients is high, because of oropharyngeal aspiration following brain injury. In the current study, the etiology of early- and late-onset VAP was assessed among the patients admitted for neurosurgical procedures.MethodsCurrent cross-sectional study was conducted on 82 patients with VAP following neurosurgical procedure. Information about patients including demographics, early/late-onset VAP, the score of Acute Physiology and Chronic Health Evaluation II (APACHE II), Clinical Pulmonary Infection Score (CPIS), underlying reason of neurosurgical procedure, medical history, smoking history, and smear/culture, and antibiograms were recorded and analyzed.FindingsVAP type was not in association with age (P = 0.560), gender (P = 0.270), etiology of neurosurgery (P = 0.960), and medical history (P = 0.930), but was statistically in association with admission duration (P = 0.034), smoking (P < 0.001), APACHE II score (P < 0.001), and CPIS score (P < 0.001). Duration of admission [odds ratio (OR) = 4.71; confidence interval of 95% (95%CI): 1.04-21.35; P = 0.040) and smoking (OR = 1.07; 95%CI: 1.01-1.14; P = 0.015) were independently in association with VAP. There was no association between type of VAP and Gram staining (P = 0.330) and antibiogram (P = 0.140).ConclusionBased on the current study, duration of admission, mortality, smoking, APACHE II score, and CPIS score were significantly higher among late-onset VAP. Moreover, admission duration and smoking was independently in association with VAP regardless of its type. Pages 80-85BackgroundNowadays, the best strategy to control, manage, and prevent poisoning is identification of poisoning patterns, as well as education. Considering the importance of this issue and to prevent poisoning, this study was conducted to evaluate the attitude and knowledge about intoxications factors in general population in Isfahan City, Iran, during the poisoning prevention week.MethodsIn this cross-sectional study, 280 participants were randomly selected during the poisoning prevention week, and completed the researcher-made questionnaire. The questionnaire consisted of three sections (demographic information, knowledge, and attitude). The validity and reliability of was approved (Cronbach’s alpha = 0.784).Findings51% of the participants were men, and the mean age was 37 years. The mean scores of knowledge and attitude of the participants about the intoxications factors were 6.57 ± 2.10, 32.69 ± 9.00, respectively. This mean did not show significant difference between both genders. There was a positive correlation between age and knowledge. Higher knowledge and attitudes were observed in married participants, those with higher education, and those with office occupations when compared with others.ConclusionThe mean knowledge of people in Isfahan City was moderate, and their mean attitude was good about intoxications factors. In future, by increasing the knowledge and attitudes of general population about poisoning, prevention strategies will improve.Keywords: Knowledge, Attitudes, Poisoning

Pages 86-92BackgroundThe aim of this study was to evaluate the effects of acute kidney injury (AKI) and mediatory role of the estrogen on passive avoidance memory in ovariectomized rats.MethodsFrothy ovariectomized Wistar rats were subjected to sham-vehicle, sham-estrogen, renal ischemia reperfusion (RIR)-vehicle, and RIR-estrogen groups. AKI was induced by ischemia reperfusion. Estrogen (200 μg/kg in 0.2 ml/100 g body weight) was injected subcutaneously once a week for four weeks. Then, passive avoidance memory was assessed.FindingsAKI resulted a significant decrease in the mean latency of entering the dark chamber (P < 0.010). Although, estrogen therapy following AKI led to elevated blood urea nitrogen (BUN) and creatinine, but, there was no significant difference in the delay in entering the dark chamber in RIR-estrogen group compared to RIR-vehicle group (P = 0.100).ConclusionThe results showed that AKI could have a deleterious effect on passive avoidance memory. However, the studied dose of estrogen cannot prevent passive avoidance memory impairment induced by AKI.Keywords: Acute kidney injury, Ovariectomy, Memory, Estrogen

Pages 93-100BackgroundGrowth arrest-specific 2 (GAS2) gene is implicated in a variety of cellular functions such as cell cycle, apoptosis, and proliferation, and may be potentially involved in cancer progression. However, whether GAS2 is associated with colorectal cancer (CRC) progression and prognosis remains to be uncovered. Thus, this study investigated the association of GAS2 expression in tumor with CRC progression and prognosis.MethodsIn the case-control study, surgical tumor and adjacent normal tissues from 40 patients with CRC were collected at Cancer Institute of Imam Khomeini Hospital in Tehran, Iran, and relative expression level of GAS2 in the tissues was assayed using quantitative real-time polymerase chain reaction method. The correlation of tumor GAS2 expression with the clinicopathological features and overall survival rate of patients was determined.FindingsThe relative expression level of GAS2 in tumor tissues was significantly elevated compared to the adjacent normal tissues [1.96 (1.17-3.40) vs. 1.10 (1.00-1.31), P < 0.001]. Moreover, the expression levels of GAS2 in tumor tissues were significantly associated with clinicopathological features of cancer including tumor stage in Tumor spread, Lymph node spread, and Metastasis (TNM) staging (P = 0.010), grade (P = 0.010), size (P = 0.030), and lymphatic (P = 0.030) and vascular invasion (P = 0.040) as well as the decreased overall survival (P = 0.040).ConclusionElevated expression of GAS2 in CRC is associated with cancer progression indices and poor prognosis; hence it may be served as a prognostic biomarker in CRC.Keywords: Growth arrest-specific 2 protein_human_Cancer staging_Prognosis

Pages 101-107BackgroundMesenchymal stem cells (MSCs), are multipotent stem cells that their differentiation to chondrocytes is considered in recent years. Platelet-rich plasma (PRP), because of rich of several growth factors, and also hyperosmolarity, because of stimulating differentiation culture medium to original medium of chondrocytes, can affect this differentiation. Considering hypertrophy and ossification as resulted adverse factors, this study aimed to achieve an optimal medium for chondrogenic differentiation of human MSCs.MethodsMSCs were extracted from adipose tissue, and differentiated to chondrocyte line. The effect of base medium containing transforming growth factor beta (TGF-β), PRP, hyperosmolarity, base + hyperosmolarity and PRP + hyperosmolarity on chondrogenic differentiation was assessed using special staining methods such as Alcian blue, hematoxylin and eosin (H&E), and collagen type II and X immunocytochemistry, measurement of biochemical indexes including alkaline phosphatase (ALP) activity and deposit of calcium, and study of expression of Col2, Smad2, Sox9, and Aggrecan genes using real-time polymerase chain reaction (RT-PCR).FindingsHuman adipose tissue derived MSCs, with differentiation ability to adipocyte and osteocyte lines, were presented characterizations of chondrocytes in all differentiation groups. In base + hyperosmolarity group, ALP activity and deposit of calcium were less than other groups.ConclusionBase + hyperosmolarity differentiation group can be a better culture medium for chondrogenic differentiation.Keywords: Mesenchymal stem cells, Cell differentiation, Platelet-Rich Plasma, Chondrocytes
