    مقدمه
    رژیم غذایی با چگالی انرژی بالا زمینه ساز چاقی و دیابت نوع 2 است، اما بررسی های انجام شده در این زمینه معدود می باشد. هدف پژوهش حاضر، بررسی ارتباط چگالی انرژی رژیم غذایی و خطر بروز دیابت نوع 2 بود.
    مواد و روش ها
    مطالعه ی حاضر به صورت مورد شاهدی لانه گزیده روی 178 فرد دیابتی که بعد از 9 سال پی گیری، در مطالعه ی قند و لیپید تهران به این بیماری مبتلا شدند، انجام گرفت. برای هر مورد 3 کنترل به صورت تصادفی از افراد سالم انتخاب شد. چگالی انرژی، از تقسیم کل کالری حاصل از مواد غذایی و نوشیدنی ها بر مقدار ماده مصرفی تعیین گردید.
    یافته ها
    میانگین سنی زنان 5/6±43 سال و مردان 12±5/44 سال بود. رژیم غذایی با چگالی انرژی بالا، با دریافت بالای کالری، چربی، کربوهیدرات و گوشت فراوری شده ارتباط مستقیم، و با دریافت پایین سبزی و میوه ارتباط معکوس داشت (05/0P<). چگالی انرژی رژیم غذایی در زنان، پس از تعدیل برای عوامل خطر بروز دیابت نوع 2 و کل انرژی دریافتی (28/2OR=، 02/0P=) و کل چربی دریافتی (15/2OR=، 03/0P=) با خطر دیابت نوع 2 ارتباط داشت، در مردان بعد از تعدیل برای عوامل خطر دیابت و کالری و چربی دریافتی این ارتباط دیده نشد.
    نتیجه گیری
    در بررسی حاضر ارتباطی بین خطر دیابت نوع 2 و چگالی انرژی رژیم غذایی در مردان دیده نشد، اما در زنان بعد از تعدیل عوامل مداخله گر ارتباط مثبت میان خطر دیابت نوع 2 و چگالی انرژی رژیم غذایی مشاهده نگردید.

    مقدمه
    استرس اکسیداتیو نقش مهمی در بیماری زایی و عوارض دیابت دارد. از طرفی مصرف حبوبات می تواند اثرات مفیدی بر بهبود شاخص های استرس اکسیداتیو در بیماران مبتلا به دیابت داشته باشد. هدف پژوهش حاضر، تعیین اثر دریافت حبوبات غیر سویا روی شاخص های استرس اکسیداتیو، مانند LDL اکسید شده (ox-LDL)، مالون دی آلدئید (MDA) و ظرفیت تام آنتی اکسیدانی (TAC) در این بیماران بود.
    مواد و روش ها
    در این کارآزمایی بالینی تصادفی متقاطع، 24 فرد مبتلا به دیابت نوع 2 و دارای اضافه وزن، در دامنه ی سنی80-50 سال، انتخاب شدند. افراد به طور تصادفی به دو گروه دریافت کننده ی رژیم غذایی بر پایه ی تغییرات درمانی شیوه ی زندگی (کنترل) و یا دریافت کننده ی رژیم غذایی بر پایه ی تغییرات درمانی شیوه ی زندگی به همراه حبوبات (جایگزینی2 واحد حبوبات در سه روز هفته به جای واحد گوشت دریافتی) تقسیم شدند. طول هر یک از رژیم ها 8 هفته بود که با یک دوره شستشوی 4 هفته ای از هم جدا می شدند. ابتدا و انتهای هر دو مرحله ی دریافت رژیم غذایی گلوکز ناشتای سرم، ox-LDL، MDA و TAC بیماران اندازه گیری شد.
    یافته ها
    رژیم مداخله با کاهش ox-LDL (5/0±1/3- در برابر 4/0±7/0-، 003/0P=)، MDA (2/0±1/1- در برابر 1/0±3/0- 0، 006/0P=) و قند خون ناشتا (7/6±7/28- در برابر 5/5±5/19-، 001/0 P <) همراه بود، اما تفاوت معنی داری در تغییرات TAC بین دو گروه مشاهده نگردید.
    نتیجه گیری
    رژیم غذایی TLC بر مبنای دریافت حبوبات غیر سویا شاخص های استرس اکسیداتیو و قند خون را در بیماران دیابتی بهبود می بخشد.

    مقدمه
    آدیپونکتین پروتئین اختصاصی مترشحه از بافت چربی است که می تواند روی روند پیشرفت دیابت تاثیرگذار باشد. هدف پژوهش حاضر، بررسی تغییرات سطح سرمی آدیپونکتین و تغییرات سطح سرمی لیپید در موش های نر دیابتی مورد درمان با عصاره الکلی و آبی 3 گونه گیاه دارویی زرشک، عناب و زعفران بود.
    مواد و روش ها
    در مطالعه ی تجربی حاضر، موش های صحرایی به وسیله ی تزریق درون صفاقی استرپتوزوسین دیابتی شدند. موش های دیابتی در گروه های مختلف تقسیم شده و به مدت 21 روز تحت درمان با عصاره آبی و الکلی گیاهان مورد نظر با دو دوز 25 و 100 میلی گرم بر کیلوگرم وزن بدن قرار گرفتند. سطح سرمی آدیپونکتین، قند خون ناشتا، تری گلیسرید و کلسترول اندازه گیری شد و یافته ها با استفاده از آزمون آماری یک طرفه آنووا مورد ارزیابی قرار گرفت.
    یافته ها
    سطح سرمی قند خون ناشتا، تری گلیسرید و VLDL در تمام گروه های دریافت کننده ی عصاره کاهش معنی داری را نشان داد (05/0P<). سطح سرمی کلسترول HDL تنها در گروه دریافت کننده ی عصاره ی الکلی عناب نسبت به گروه دیابتی کنترل تفاوت معنی داری نشان داد. سطح سرمی آدیپونکتین در تمام گروه های دریافت کننده ی عصاره افزایش معنی داری داشت (05/0P<).
    نتیجه گیری
    با توجه به نقش آدیپونکتین در تنظیم سوخت و ساز چربی، و با در نظر گرفتن این موضوع که آدیپونکتین در تمام گروه های دریافت کننده ی عصاره افزایش معنی داری نسبت به گروه دیابتی کنترل داشت، می توان تنظیم قند و چربی در گروه های دیابتی دریافت کننده ی عصاره را به این هورمون نسبت داد.

    مقدمه

    تاکنون تاثیر جوانه ی عدس روی مبتلایان به دیابت نوع 2 دچار چاقی و اضافه وزن مورد بررسی قرار نگرفته، هم چنین مصرف جوانه غلات و حبوبات در طب سنتی ایران بسیار توصیه گردیده است. در بررسی حاضر تاثیر جوانه عدس روی چربی های خون و قند خون ناشتا در مبتلایان به دیابت نوع 2 مبتلا به چاقی و اضافه وزن مورد بررسی قرار گرفت.

    مواد و روش ها

    39 بیمار به طور تصادفی در دو گروه جوانه عدس و کنترل قرار گرفتند. تمام آزمودنی ها داروهای خود را مصرف می کردند و افراد در گروه جوانه عدس در کنار داروهای خود، روزانه 60 گرم جوانه عدس به مدت 8 هفته مصرف نمودند. در ابتدا و انتهای پژوهش چربی های خون (کلسترول تام، تری گلیسرید، کلسترول-LDL و کلسترول-HDL) و قند خون ناشتا اندازه گیری شد.

    یافته ها

    نمایه ی آتروژنیک بعد از 8 هفته در افراد گروه جوانه عدس کاهش یافت (07/0P<). میزان افزایش کلسترول-HDL و کلسترول-LDL در گروه جوانه عدس در مدت 8 هفته مداخله معنی دار بود. در پایان بررسی 22% افزایش در میزان تری گلیسرید افراد گروه کنترل دیده شد، در حالی که میزان تری گلیسرید در افراد گروه جوانه عدس 5/4% کاهش داشت. این یافته ها تاثیرات جوانه عدس را در پیشگیری از بیماری های قلبی تاکید می نماید.

    نتیجه گیری

    یافته ها نشان داد مصرف جوانه عدس با تاثیرات مطلوبی که روی ترکیب چربی های خون دارد، می تواند خطر بیماری های قلبی را در افراد مبتلا به دیابت نوع 2 مبتلا به چاقی و اضافه وزن کاهش دهد.

    مقدمه
    پژوهش حاضر با هدف بررسی برهم کنش پلی مورفیسم (C>G) rs5128از ژن APOC3 و چربی های دریافتی در ارتباط با اجزا سندرم متابولیک در بزرگسالان انجام شد.
    مواد و روش ها
    پژوهش حاضر به روش مورد شاهدی لانه گزیده در قالب مطالعه ی قند و لیپید تهران صورت گرفت. هر یک از 755 مورد (سن: سال 18≤) به صورت فردی با شاهد خود از نظر سن و جنس همسان سازی گردید. دریافت های تغذیه ای از راه تکمیل پرسش نامه روا و پایا بسامد خوراک به دست آمد. تعیین ژنوتیپ با استفاده از روش PCR-RFLP صورت پذیرفت.
    یافته ها
    میانگین سنی مردان و زنان در گروه مورد و شاهد اختلافی نداشت. فراوانی الل C 81% بود که در گروه مورد و شاهد تفاوت نداشت. تحت مدل ژنتیک غالب، در زنان با ژنوتیپ CG+GG با کلسترول دریافتی ≥208 میلی گرم در روز، خطر کلسترول HDL پایین، 93/1 برابر در مقایسه با افراد با کلسترول دریافتی>208 بود. در مردان با ژنوتیپ CG+GG و دریافت اسید چرب اشباع (SFA)≥ 8/9% از انرژی، نسبت شانس فشار خون دیاستولی بالا (46/1-1)15/2 برابر در مقایسه با افراد با دریافت >SFA8/9% از انرژی بود. خطر فشارخون دیاستولی بالا با افزایش دریافت اسیدچرب با یک باند دوگانه (MUFA) به بالاتر از 4/9% انرژی در حاملین الل G در مقایسه با ژنوتیپ CC افزایش یافت.
    نتیجه گیری
    پژوهش حاضر بیان گر برهم کنش چربی های دریافتی و پلی مورفیسمrs5128 با عوامل خطر سندرم متابولیک می باشد؛ افراد حامل الل G و با دریافت بالاتر کلسترول، SFA و MUFA، شانس خطر بالاتری برای کلسترول HDL پایین و فشارخون بالا در مقایسه با افراد هموزیگوت C داشتند.

    مقدمه
    فشار خون بالا یکی از مشکلات مهم سلامتی در کشورهای توسعه یافته و در حال توسعه می باشد. در ایران به طور تقریبی یک چهارم بزرگسالان مبتلا به فشار خون بالا هستند. عوامل مختلفی از جمله الگوی غذایی می توانند بر بروز فشار خون بالا تاثیرگذار باشند. هدف پژوهش حاضر تعیین ارتباط فشار خون بالا و الگوهای غذایی بود.
    مواد و روش ها
    مطالعه ی مقطعی حاضر روی 973 نفر از ساکنین شهر خرم آباد انجام گرفت. ارزیابی دریافت های غذایی با استفاده از یک پرسش نامه بسامد خوراک (201 آیتمی) نیمه کمی صورت گرفت. فشار خون بالا، بر مبنای فشارخون سیستولی بیش از 140 و یا فشارخون دیاستولی بیش از 90 میلی متر جیوه تعریف گردید. به منظور شناسایی الگوهای غذایی از تحلیل عاملی استفاده شد. هم چنین، برای بررسی ارتباط ابتلا به فشار خون بالا و الگوهای غذایی از رگرسیون لجستیک استفاده گردید.
    یافته ها
    الگوهای غذایی غربی، سالم و سنتی، سه الگوی غالب شناسایی شدند. افرادی که در بالاترین پنجک الگوی غذایی غربی بودند شانس بیشتری برای ابتلا به فشار خون بالا داشتند (نسبت شانس 2 و فاصله اطمینان 95%:04/1 تا 85/3؛ 037/0P=)، در حالی که افرادی که در پنجک پنجم الگوی غذایی سالم بودند نسبت به افراد پنجک اول؛ قبل از تعدیل اثر نمایه ی توده ی بدن؛ 44% (56/0: 34/0 تا 93/0؛ 043/0P=) شانس پایین تری برای ابتلا به فشار خون بالا داشتند. اما این ارتباط بعد از تعدیل اثر BMI معنی دار نبود (06/0P=)؛ بین الگوی غذایی سنتی و فشار خون بالا ارتباط معنی داری مشاهده نشد (71/0: 37/0 تا 35/1؛ 29/0P=).
    نتیجه گیری
    الگوی غذایی شبه غربی ارتباط مستقیمی با خطر ابتلا به فشار خون بالا دارد. برای نشان دادن ارتباط دقیق الگوهای غذایی و فشار خون بالا مطالعات آینده نگر مورد نیاز می باشد.

    مقدمه
    در ایران بیماری های تیرویید به ویژه در خانم ها، بسیار شایع است. زنانی که مبتلا به کم کاری یا پرکاری شدید تیرویید هستند با نازایی، سقط های مکرر و عوارض نامطلوب دوران بارداری روبرو می باشند. پژوهش حاضر با هدف تعیین شیوع اختلالات عملکردی تیرویید در زنان باردار مراجعه کننده به درمانگاه های شهر کرمانشاه در سال های 91-1390 انجام گرفت.
    مواد و روش ها
    مطالعه ی حاضر به روش مقطعی انجام شد. افراد مورد بررسی خانم های دارای حاملگی زیر 16 هفته بودند که برای انجام آزمایش های روتین بارداری مراجعه کرده بودند. نمونه گیری به روش خوشه ایدومرحله ای انجام گرفت، 1200 نمونه مورد بررسی قرار گرفت. نمونه خون برای انجام آزمایش سطح TSH و FT4 گرفته شد. تحلیل های آماری بر اساس آزمون هم بستگی مجذور کای انجام گرفت.
    یافته ها
    تعداد 248 (7/20%) نفر از افراد مورد مطالعه مبتلا به اختلال عملکرد تیرویید بودند، از این تعداد 194 نفر (17/16%) به هیپرتیروییدی و 54 نفر (5/4%) به هیپوتیروییدی مبتلا بودند. بیشترین میزان ابتلا به هیپرتیروییدی (9/27%) و هیپوتیروییدی (3/23%) در زنان دارای بارداری چهارم مشاهده شد. رابطه ی معنی داری بین سابقه ی خانوادگی ابتلا به اختلال عملکرد تیرویید، سابقه ی سقط خود به خودی، سابقه ی ناباروری و سابقه ی زایمان زودرس با ابتلا به اختلال عملکرد تیرویید در زنان باردار مشاهده گردید.
    نتیجه گیری
    با توجه به بالا بودن شیوع اختلال عملکرد تیرویید، و هم چنین ارتباط بین برخی عوامل خطر شناسایی شده، بهتر است که آزمایش های غربالگری تیرویید در اولین ویزیت دوران بارداری کمینه در زنان باردار دارای عوامل خطرساز عنوان شده انجام گردد.

    مقدمه

    راشیتیسم در استخوان های در حال رشد به علت اختلال در رسوب کلسیم (ca) و فسفر (p) ایجاد می شود. راشیتیسم مقاوم یا وابسته به ویتامینD نشانگر کمبود شدید ویتامینD می باشد و 2 نوع با توارث اتوزومال مغلوب دارد: تیپ یک که ناشی از نقص در آنزیم 1- الفا هیدروکسیلاز است. تیپ دو VDDR2)) که ناشی از جهش در گیرنده ی درون سلولی ویتامین بوده و با علایم اختصاصی مانند آلوپسی و کوتاهی قد نیز همراه است.
    معرفی بیماران: در این مقاله به معرفی 3 مورد از این بیماری در یک فامیل پرداخته شده است. دو خواهر 5 و 5/1 ساله و دختر عموی 5 ساله ی ایشان با علایم بدشکلی اندام ها، مچ های پهن، تشنج، اختلال در رویش دندان ها، آلوپسی، کوتاهی قد، شکستگی های متعدد استخوانی به درمانگاه غدد و متابولیسم کودکان در بیمارستان امام رضا مشهد مراجعه کردند و برخلاف دریافت درمان دارویی کافی، بهبودی نداشتند و زمانی که به علت اختلال رشد تحت درمان با هورمون رشد قرار گرفتند، علایم راشیتیسم نیز بهبود قابل توجهی پیدا کرد.

    نتیجه گیری

    درمان راشیتیسم وابسته به ویتامین D تیپ 2 نیاز به مقادیر زیاد ویتامین D فعال و کلسیم دارد و برخلاف مصرف مقدار زیاد دارو در بیشتر موارد پاسخ درمانی مناسب نیست. در این بررسی اثربخشی استفاده از مصرف هورمون رشد برای بهبود رشد قدی و بیماری زمینه ای مشاهده گردید.

    Introduction
    Dietary energy-dense foods predispose to obesity and may be associated with risk of type 2 diabetes; therefore, the aim of our study was to investigate the association between dietary energy density (DED) and the risk of type 2 diabetes.
    Materials And Methods
    In this matched nested case-control study, 178 new onset cases of diabetes were selected from among participants of the Tehran Lipid and Glucose Study, and followed for over 9 years. Each case was randomly matched with 3 controls from among participants who had not developed diabetes. Dietary intakes were determined using a valid and reliable food frequency questionnaire. DED was calculated as energy intakes (kcal) from foods and beverages divided by the weight (grams) of foods.
    Results
    The mean ages of participants were 43±6.5 and 44.5±12 years, in women and men respectively. Individuals with higher DED had higher calories, total fat, carbohydrate and processed meat intakes and lower fruit and vegetable intakes in men and women (P<0.05). Energy density was positively associated with incident diabetes in women, after adjustment for diabetes risk factors and total calorie intake (OR: 2.28, P trend=0.02) or total fat intake (OR: 2.15, P trend=0.03), an association which was not significant in men.
    Conclusion
    In this nested case-control study, there was positive association between DED and risk of type 2 diabetes in women.

    Introduction
    Oxidative stress plays a key role in the pathogenesis of late diabetic complications, the markers of which could be improved by the beneficial effects of legumes.
    Materials And Methods
    In this randomized crossover trial, 24 overweight subjects, aged 50-80 years, with type 2 diabetes, were selected. We aimed to determine the effects of non-soybean legumes consumption on oxidative stress markers such as ox-LDL (oxidative LDL), MDA (Malonedialdehyde) and TAC (total antioxidant capacity) in type 2 diabetic patients. The subjects were randomly assigned into two groups, receiving two diets – the TLC (controls) or the TLC diet with legumes (receiving 2 servings of legumes instead of meat three days of the week in the TLC diet). Duration of each diet was 8 weeks with a 4 week wash-out period. At the beginning and end of each dietary period, fasting plasma glucose, ox-LDL, MDA, TAC and weight were measured.
    Results
    After the nutritional intervention, diabetic patients, those individuals who followed the legume-based TLC diet had significant reduction in ox- LDL (-3.1±0.5 vs. -0.7±0.4; P=0.003) and MDA (-1.1±0.2 vs. -0.3±0.1; P=0.006), but no change was observed in the TAC and BMI of the case and control groups.
    Conclusions
    The legume-based TLC diet improves oxidative stress markers in diabetic patients, compared to the legume-free TLC diet.

    Introduction
    Adiponectin is a protein hormone secreted from adipose tissue that can affect diabetes progression. The aim of this study was to investigate the changes in serum levels of adiponectin in diabetic rats treated with alcoholic and aqueous extracts of three medicinal plants, jujube (Ziziphus jujuba), barberry (Berberis vulgaris) and saffron (Crocus sativus).
    Materials And Methods
    In this study, streptozocin-induced diabetic rats were treated with two doses of aqueous and alcoholic extracts of the herbs, 25 and 100 mg/kg respectively, for 21 days. After the treatment period, the levels of adiponectin, fasting blood glucose, triglycerides and cholesterol were measured and the results were analyzed using one way ANOVA test.
    Results
    In all the diabetic groups receiving extracts, serum levels of fasting blood glucose, triglycerides and VLDL (very low density lipoprotein), showed a significant decrease (P<0.05). Serum level of HDL (high density lipoprotein) increased significantly only in the group receiving jujube (P<0.05), whereas adiponectin level was significantly increased in all the treated groups.
    Conclusion
    Given the role of adiponectin in regulating lipid metabolism, and the increased level of adiponectin in the treated diabetic groups, it can be said that the extracts regulate glucose and lipid metabolism by changes in adiponectin level. Further studies are recommended to confirm these findings.

    Introduction

    The effects of lentil sprouts (LS) on lipid profiles and FBS among overweight and obese patients with type 2 diabetes have not yet been examined. Considering the importance of legumes and cereal sprouts in traditional Iranian medicine, we conducted this trial to investigate the effects of LS on fasting blood glucose, and the atherogenic index of plasma (AIP), in overweight and obese patients with type 2 diabetes.

    Materials And Methods

    Thirty-nine patients were randomly divided into two groups. For all individuals throughout the 8 weeks of the study period, the same drug treatment was given, with the intervention group patients consuming 60 g lentil sprouts in addition daily. Total cholesterol, triglycerides (TG), low-density lipoprotein (LDL-C), high density lipoprotein (HDL-C), fasting blood glucose (FBS) and atherogenic index of plasma (AIP) were measured at baseline and 8 weeks after intervention.

    Results

    AIP decreased (P<0.07) in the LS group and their levels of HDL-C and LDL-C increased compared with baseline levels, whereas in the control group no significant change was seen. In this group a decrease in TG of 4.5% was seen, whereas this increase in the control group was 22%.

    Conclusions

    Consumption of LS as supplementary treatment in type 2 diabetes could have favorable effects on lipid profiles, fasting blood glucose and reduce risk of coronary vascular diseases.

    Introduction
    The aim of this study was to evaluate the interaction between dietary fatty acids and the genetic variant of APOC3 rs5128 3238C>G in relation to metabolic syndrome (MetS) components in adults.
    Materials And Methods
    In this matched nested case-control study, 755 MetS subjects and 755 controls were selected from among participants of the Tehran Lipid and Glucose Study. Dietary intake was determined using a valid and reliable food frequency questionnaire. APOC3 was genotyped by the conventional polymerase chain reaction and restriction fragment length polymorphism.
    Results
    Mean ages of men and women were not different in cases and controls. The frequency of C allele was 81%, which did not differ in cases and controls or in men and women. Compared to CC genotype, low HDL-C risk was increased in women with the CG+GG genotypes and with cholesterol intakes ≥208 mg/day (OR: 1.93). In men with the CG+GG genotypes and saturated fatty acid (SFA) intakes ≥9.8% of energy, OR of high diastolic blood pressure (BP) was 2.15(1-1.46), compared to individuals with SFA intake <9.8% of energy and CC genotype. Compared to the CC genotype, the risk of high diastolic BP was higher in men carrying the G allele and consuming mono-unsaturated fatty acid (MUFA) intakes ≥9.4% of energy.
    Conclusions
    Results demonstrate a nutri-genetic interaction between rs5128 and fat intakes in relation to components of MetS; individuals with G allele carriers and higher intakes of cholesterol, MUFA or SFA had higher risk of low HDL-C and hypertension than the CC genotype.

    Introduction
    Hypertension is a common public health problem, both in the developed and the developing countries. In Iran approximately one in every four adults has high blood pressure. Many factors, e.g dietary pattern, can affect the incidence of high blood pressure. The aim of this study is to determine the relationship between hypertension and dietary patterns.
    Materials And Methods
    In this cross-sectional study, the 973 participants were residents of Khorramabad. Food intake assessments were measured by a validated semi quantitative 201-item food frequency questionnaire (FFQ). Hypertension was identified as systolic blood pressure>140 mm/Hg and/or diastolic blood pressure>90 mm/Hg. Dietary patterns were identified by factor analyses. Multivariate logistic regression was used to determine the associations of dietary patterns and hypertension.
    Results
    The three major dietary patterns identified were the Traditional Dietary pattern (TDP), the Healthy Dietary Pattern (HDP), and the Western Dietary Pattern (WDP). Subjects in the highest quintile of the WDP scores had a greater odds ratio for hypertension (odds ratio: 2; 95% CI: 1.04-3.85; P=0.037) than those in the lowest quintile. Before BMI adjustment, compared with those in the lowest quintile, subjects in the highest quintile of HDP scores had lower odds for hypertension (0.56; 0.34-0.93; P=0.043), a relationship however that was not significant after BMI adjustment (P=0.06). TDP consumption and hypertension were not significantly associated (0.71; 0.37-1.35; P=0.29).
    Conclusion
    The WDP had a direct association with the risk of hypertension. More longitudinal studies are needed to clarify the exact association between dietary patterns and hypertension.

    Introduction
    Thyroid diseases, especially among women, are very common, and most women with severe hypo- or hyperthyroidism are faced with infertility, recurrent miscarriage and poor outcome pregnancy. This study aimed to determine the prevalence of thyroid dysfunction in pregnant women referring to clinics in Kermanshah, between 2011-2012.
    Materials And Methods
    This cross-sectional study was conducted on a group of women with pregnancies under 16 weeks gestation, admitted for routine tests. Two-stage cluster sampling was conducted and 1200 samples were analyzed. Statiscal analysis was performed using the Chi 2 Correlation Test.
    Results
    Of subjects, 248(20.7%) were diagnosed with thyroid dysfunction; 194 patients (16.17%) with hyperthyroidism and 54 (4.5%) patients with hypothyroidism. The highest prevalences of hyperthyroidism (27.9 %) and hyporthyroidism (23.3%) were observed in pregnant women with their 4th gravidity. Significant relationships between variables such as family history of thyroid dysfunction, spontaneous abortion, infertility and preterm delivery in pregnant women with thyroid dysfunction were observed.
    Conclusions
    Considering the high prevalence of thyroid dysfunction and relationships between some of the identified risk factors, it is recommended that screening tests for thyroid function in pregnant women at the first prenatal visit be conducted, at least in those with known risk factors of this condition.

    Introduction

    Rickets, which occurs during bone growth development, is a result of severe vitamin D deficiency, that has autosolamal subtypes, i.e. A- type 1- (VDDR-I), due to a defect of the 1-alfa-hydroxilase enzyme and B) type -2(VDDR-II), due to a mutation in the intracellular vitamin –D receptor (VDR), associated with some specific symptoms like alopecia (global head hair loss) and short stature. Diagnosis was established based on physical examination, laboratory findings and radiological examination.

    Case Presentation

    We report here 3 cases with vitamin D-dependent rickets, type-II, all members of the same family (2 sisters, aged 1.5, 5 years and their 5 year-old cousin) who had referred to the endocrine and metabolic center in the Imam–Reza hospital in Mashhad, presenting with limb deformities, wide wrist, seizures, disorder in dental growth, alopecia, and several bone fractures. Despite various treatments, they had not recovered. However it was notable that after receiving growth hormone therapy for growth failure, rickets symptoms improved dramatically in clinical, laboratory, and radiological parameters. Using growth hormone therapy to improve height, we observed improvements in the underlying disease of siblings with VDDR-type-II.

    Conclusion

    Growth hormone therapy may be advisable to treat VDDR-type II symptoms.
