صفحات 243-249سابقه و هدفالتراسوند یک روش غیر تهاجمی برای مشاهده تغییرات ضخامت عضلات شکمی می باشد که این تغییر ضخامت به عنوان شاخصی برای فعالیت عضله در نظر گرفته می شود. اندازه گیری صحیح سونوگرافی وابسته به کنترل تمام منابع احتمالی ایجاد خطا حین اندازه گیری ها می باشد. هدف از این مطالعه بررسی سطح پر بودن معده بر روی ضخامت عضلات شکم در وضعیت استراحت و در حین انقباض طی مانور گود کردن شکم می باشد.مواد و روش هاتعداد 30 مرد سالم با دامنه سنی 18 تا 26 سال برای شرکت در این مطالعه داوطلب شدند. ضخامت 4 عضله شکمی در هر دو سمت در وضعیت استراحت (در انتهای بازدم) و طی مانور گود کردن شکم قبل و بعد از صرف غذا توسط دستگاه سونوگرافی با پروب خطی 5/3 مگاهرتز اندازه گیری شد.یافته هامیانگین ضخامت عضلات راست شکمی مورب خارجی و داخلی و عرضی شکم در سمت راست و چپ در حالت استراحت قبل از صرف غذا به ترتیب 8/10 و 7/10، 2/6 و 1/6، 6/8 و 3/8، 1/4 و 1/4 میلی متر و بعد از صرف غذا به ترتیب 3/10 و 3/10، 6/5 و 5/5، 3/7 و 3/7، 4/3 و 6/3 میلی متر بود. میانگین ضخامت عضلات راست شکمی مورب خارجی و داخلی و عرضی شکم در مانور گود کردن شکم قبل از صرف غذا به ترتیب 11 و 8/10، 1/6 و 3/6، 10 و 10، 3/5 و 5/5 میلی متر و بعد از صرف غذا به ترتیب 5/10 و 6/10، 7/5 و 7/5، 8/8 و 7/8، 4/4 و 5/4 میلی متر بود.نتیجه گیرینتایج نشان می دهد که بعد از مصرف غذا ضخامت عضلات شکم در هر دو سمت به طور معنی داری کاهش یافت، بنابراین مصرف غذا باید در اندازه گیری سونوگرافیک ضخامت عضلات شکم مد نظر قرار گیردکلیدواژگان: سونوگرافی، عضلات شکم، خوردن

صفحات 250-257سابقه و هدفبه نظر می رسد که ترس از حرکت نقش بسزایی بر تغییر سطح فعالیت عضلات عمقی تنه افراد مبتلا به کمردرد دارد. این در حالی است که تا به حال مطالعه ای به بررسی ارتباط بین سطح فعالیت عضلات عمقی تنه با شاخص میزان ترس از حرکت نپرداخته است. هدف از مطالعه حاضر بررسی ارتباط بین سطح فعالیت عضلات شکمی و شاخص های درد، ناتوانی و ترس از حرکت طی تکالیف پاسچرال ایستاده در زنان مبتلا به کمردرد مزمن بود.مواد و روش هااز 44 داوطلب مبتلا به کمردرد خواسته شد که بر روی سطوح دستگاه تعادل بایودکس، تعادل خود را حفظ نمایند. به طور هم زمان داده های اولتراسونوگرافی جهت اندازه گیری ضخامت عضلات شکمی ذخیره و به شکل آفلاین پردازش گردید. شدت درد، سطح ناتوانی و ترس از حرکت نیز توسط مقیاس های معتبر مورد ارزیابی قرار گرفت.یافته هاهمبستگی معنی داری بین درصد تغییر ضخامت عضلات سطحی و عمقی شکمی و نمرات مقیاس شدت درد و پرسش نامه ناتوانی در افراد وجود نداشت. در حالی که بین درصد تغییر ضخامت عضلات عمقی شکمی در شرایط سطح پویایی حداکثر و مقیاس ترس از حرکت تمپا همبستگی معنی دار و معکوسی وجود داشت (05/0P<)، اگر چه این همبستگی ضعیف بود (32/0- -36/0-=r).نتیجه گیریبه نظر می رسد که افزایش میزان ترس از حرکت بر کاهش سطح فعالیت عضلات عمقی شکمی افراد کمردرد اثر معنی داری خواهد داشت. وجود این ارتباط اهمیت انجام تمرینات شناختی-رفتاری و کنترل میزان ترس از حرکت افراد مبتلا به کمردرد بر بهبود عمل کرد عضلات عمقی شکمی را نشان می دهدکلیدواژگان: کمردرد، عضلات شکمی، درد، ارزیابی ناتوانی، ترس

صفحات 258-264سابقه و هدفعضلات پلانتارفلکسور نیروی جلوبرنده بدن را در نیمه دوم مرحله ایستایش راه رفتن مهیا می کنند، هرگونه نقص در بازده این عضلات منجر به نیروی جلوبرنده بدن ناکافی در مرحله push off و کاهش سرعت راه رفتن می شود. این مطالعه تاثیر شیب ها و سرعت های مختلف بر زمان شروع فعالیت الکترومیوگرافی عضلات مفصل مچ پا حین راه رفتن در افراد میانسال سالم را بررسی کرده است.مواد و روش ها15 فرد سالم میانسال (زن 5 / مرد 10 ، با میانگین سن 20/7±53/56 و شاخص توده بدنی 70/3±52/28 کیلوگرم بر مترمربع) در این مطالعه شرکت کردند. آزمودنی ها بر روی تردمیل استاندارد با سه سرعت مختلف (سرعت انتخابی، سرعت انتخابی +20%، سرعت انتخابی +40%) و شیب های مثبت مختلف (صفر، سه و شش درجه) راه رفتند و زمان شروع فعالیت عضلات گاستروکنمیوس داخلی، تیبیالیس قدامی در مرحله push off راه رفتن ثبت گردید.یافته هااز مدل خطی مختلط برای آنالیز استفاده شد و زمان شروع فعالیت عضلات گاستروکنمیوس داخلی و تیبیالیس قدامی با افزایش سرعت راه رفتن در سمت غیرغالب تاخیر پیدا کرد (05/0 >p)، ولی شروع فعالیت این عضلات در سمت غالب با افزایش شیب و سرعت تردمیل، تغییری پیدا نکرد. صفحات 265-275سابقه و هدفمعلمان اغلب مدت زمان طولانی با بلندی زیاد در محیط های پرسروصدا و موقعیت های پراضطراب صحبت می کنند، لذا بیش تر در معرض ابتلا به آسیب بافت های صوتی قرار دارند. این مشکلات صوتی ممکن است بر کیفیت آموزشی، وضعیت شغلی، اقتصادی، ارتباطی و عاطفی آنان تاثیر نامطلوب داشته باشد. این پژوهش از طریق مقایسه ادراک معلمان دارای و فاقد دیسفونی از مشکل صوتی خود، به بررسی تاثیر آن بر عمل کرد شغلی، ارتباطی، اجتماعی و عاطفی آن ها می پردازد.مواد و روش هااین مطالعه روی 243 نفر از معلمان مدارس عادی مقطع ابتدایی انجام شده است. آزمودنی ها بر اساس پاسخ به سوال 4 گزینه ای در ارتباط با میزان مشکل صوتی خود (همیشه، اغلب اوقات، گاهی اوقات و به ندرت) به دو گروه دارای مشکل صوتی (همیشه و اغلب اوقات) و فاقد مشکل صوتی (گاهی اوقات و به ندرت) تقسیم شدند.یافته هامطالعه نشان داد مشکلات صوتی با تدریس، نگرش شغلی، توانایی ارتباطی، اجتماعی و عواطف معلمان رابطه معنی داری دارد (05/0P<)، به طوری که وقوع مشکلات صوتی در معلمان شانس تجربه اثرات فوق را افزایش می دهد.نتیجه گیریمعلمان دارای مشکلات صوتی در مقایسه با معلمان فاقد مشکلات صوتی، دارای مشکلات بیش تری در عمل کرد شغلی، ارتباطی، اجتماعی و عاطفی و در نتیجه کیفیت زندگی بودند که باید در ارزیابی و درمان اختلالات صوتی به آن ها توجه شودکلیدواژگان: اختلال صوتی، معلمان، کیفیت زندگی، عوارض شغلی

صفحات 276-285سابقه و هدفاستئوآرتریت زانو شایع ترین بیماری مفصلی می باشد که با تخریب غضروف مفصلی همراه است. درمان های دارویی بیش تر بر ترمیم غضروف مفصلی تاکید دارند. از گلوکزآمین سولفات به عنوان یک ماده محافظ غضروف با نتایج متناقضی استفاده می شود. این مطالعه با هدف بررسی اثرات فونوفورز گلوکزآمین در استوآرتریت زانو انجام شد.مواد و روش هااین مطالعه از نوع کارآزمایی بالینی بر روی 41 بیمار زن 75-45ساله (21 مورد ابتلای یک طرفه و 20 مورد ابتلای دوطرفه) انجام شد. بیماران به صورت تصادفی به سه گروه زیر تقسیم شدند. گروه 1 (15=n) : درمان استاندارد همراه با اولتراسوند خاموش، گروه 2 (12=n): درمان استاندارد همراه با اولتراسوند روشن (مداوم با فرکانسMHz 1 و زمان 5 دقیقه و شدت 3/0 وات بر سانتی مترمربع با افزایش تدریجی در طول جلسات) و گروه 3 (14=(n: درمان استاندارد همراه با فونوفورز گلوکزآمین (با نسبت مساوی 50 درصدی ژل و گلوکزآمین، اولتراسوند روشن مشابه پارامترهای قبل به مدت 30 جلسه و روزانه روی زانوی غالب). درمان استاندارد شامل اشعه مادون قرمز( Infrared)، تحریک عصبی الکتریکی پوستی (Transcutaneous electrical nerve stimulation، TNS) و ورزش (Exercise) بود. درد، دامنه فعال خم شدن زانو، تورم زانو، مدت سفتی صبحگاهی و کیفیت زندگی قبل و پس از درمان اندازه گیری شد.یافته هاداده ها نشان دهنده بهبودی معنی دار شاخص های استئوآرتریت (کاهش بیش تر درد، تورم زانو و مدت سفتی صبحگاهی، افزایش دامنه خم شدن زانو و بهبود کیفیت زندگی) در گروه مداخله در مقایسه با دو گروه دیگر بود (05/0P<).نتیجه گیریبا توجه به اتیولوژی بیماری، انتخاب درمان هایی که بتواند به ترمیم غضروف مفصلی کمک کند درمان موثرتری جهت پیشگیری از روند پیشرفت بیماری است. فونوفورزیس گلوکزآمین می تواند یکی از این گزینه ها باشدکلیدواژگان: فونوفورزیس، درمان با اولتراسوند، التهاب استخوان و مفصل، گلوکزآمین، مفصل زانو

صفحات 286-294سابقه و هدفتغییر گشتاور اینورتورها و اورتورهای مچ پا از جمله ناتوانایی های حسی حرکتی است که همراه با بی ثباتی عمل کردی مچ پا رخ می دهد. با توجه به اثرات مثبت گزارش شده Whole Body Vibration (WBV) بر گشتاور عضلانی، این مطالعه به بررسی اثر WBV بر بهبود نسبت گشتاور کانسنتریک عضلات اورتور و اینورتور در افراد مبتلا به بی ثباتی عمل کردی مچ پا می پردازد.مواد و روش ها30 نفر از بیماران زن مبتلا به بی ثباتی عمل کردی یک طرفه مچ پا به صورت تصادفی به دو گروه آزمایش و کنترل تقسیم شدند. افراد گروه آزمایش به مدت 6 هفته (3 جلسه در هفته) تحت درمان با WBV قرار گرفتند و پس از آن از نظر گشتاور کانسنتریک عضلات اورتور و اینورتور مورد ارزیابی قرار گرفتند. گشتاور کانسنتریک عضلانی توسط دستگاه ایزوکینتیک بایودکس اندازه گیری شد.یافته هاگشتاور کانسنتریک عضلات اورتور و اینورتور افراد مبتلا و افراد گروه کنترل قبل از مداخله تفاوت معنی دار نداشتند (05/0p>). درمان با WBV، گشتاور کانسنتریک عضلات اورتور و اینورتور پای مبتلایان به بی ثباتی عمل کردی مچ پا را بهبود بخشید (05/0p<).نتیجه گیریاعمال 6 هفته (18 جلسه) WBV در افراد مبتلا به بی ثباتی عمل کردی مچ پا توانایی افزایش گشتاور کانسنتریک عضلانی را داردکلیدواژگان: ارتعاش، مچ پا، مفصل مچ پا، ناپایداری مفصل، گشتاور

صفحات 295-301سابقه و هدفاگر چه مشخصه اصلی فلج مغزی اختلال در پوسچر و حرکت می باشد اما اغلب این کودکان دارای مشکلات حسی نیز می باشند. لذا هدف از این مطالعه بررسی ارتباط میان پردازش حسی و مهارت های حرکتی در کودکان فلج مغزی بود.مواد و روش هادر این پژوهش توصیفی-تحلیلی 56 کودک فلج مغزی 5 تا 10 ساله مورد بررسی قرار گرفتند. عمل کرد حرکتی درشت با استفاده از سیستم طبقه بندی عمل کرد حرکتی درشت، عمل کرد دستی کودکان از طریق سیستم طبقه بندی توانایی دستی، شدت اسپاستی سیتی از طریق مقیاس اصلاح شده اشورث و وضعیت حسی کودکان از طریق پرسش نامه وضعیت حسی مورد ارزیابی قرار گرفت.یافته هانتایج به دست آمده از این پژوهش نشان داد که مهارت های عمل کردی درشت و دستی در کودکان فلج مغزی با فاکتورهای تون و تحمل عضلانی پایین، ضعف ثبت حسی و بی تحرکی ارتباط معنادار دارد. هم چنین میان توانایی دستی کودکان فلج مغزی با حساسیت حس دهانی نیز ارتباط معنادار مشاهده شد (01/0p<).نتیجه گیریبر اساس ارتباط معنادار میان مهارت های حرکتی و پردازش حسی در کودکان فلج مغزی، توصیه می شود که در مداخلات درمانی و توانبخشی علاوه بر مهارت های حرکتی به مهارت های حسی این کودکان نیز توجه شودکلیدواژگان: پردازش حسی، مهارت حرکتی، فلج مغزی، کودک

صفحات 302-307سابقه و هدفبیماری مالتیپل اسکلروزیس (MS) یک اختلال نورولوژیک با آسیب در سیستم عصبی مرکزی است که بین سنین 20 تا 45 سالگی آغاز می شود. یکی از مشکلات شایع در بیماران، مشکلات مربوط به صوت و گفتار می باشند و یک علت مهم در کاهش کیفیت زندگی بیمار است. هدف از این مطالعه بررسی شاخص کیفیت زندگی مرتبط با اختلال صوت در بیماران مبتلا به مالتیپل اسکروزیس بود.مواد و روش هااین مطالعه توصیفی-تحلیلی و مقطعی بر روی نمونه های در دسترس مبتلا به MS مراجعه کننده به کلینیک های مغز و اعصاب شهر سمنان انجام شد. از میان مبتلایان به MS، کسانی که نمره EDSS آن ها بین 1 تا 6 و حداقل 3 سال از شروع بیماری آن ها گذشته بود وارد مطالعه شدند و نمونه صوتی مراجعان با استفاده از کشش واکه های واکه /a/ و /i/ را به مدت 3 ثانیه و سه بار با حفظ بلندی و زیر و بمی متعادل ضبط شد و پارامترهای آکوستیک با استفاده از نرم افزار صوتی Praat مورد تجزیه و تحلیل قرار گرفت سپس شاخص معلولیت صدا به بیمار برای تکمیل ارائه می شد.یافته هانتایج این پژوهش نشان داد که همبستگی معنی داری بین پارامترهای آکوستیک صدا و شاخص معلولیت صدا وجود ندارد.نتیجه گیرینتایج این پژوهش نشان داد که اختلال صوت بیماران MS بر کیفیت زندگی این بیمارن تاثیر معنادار ندارد ولی در توان بخشی این بیماران باید این اختلالات ارزیابی و درمان شوندکلیدواژگان: مالتیپل اسکلروزیس، اختلال صوت، کیفیت زندگی

Pages 243-249IntroductionUltrasound imaging is a non-invasive method for observing changes in muscle thickness of the abdominal muscles. Accurate measurements depend on the control of all potential sources of error. In this study we investigated whether the level of stomach fullness might be a source of error not only in the resting thickness but also in contractile thickness (abdominal hallowing maneuver) abdominal muscles.Materials And MethodsThirty healthy male subjects ranging in age from 18 to 26 years volunteered for this study. Ultrasound image of the four abdominal muscles on both sides were obtained at rest (at the end of exhalation) and during abdominal hallowing maneuver before and after food consumption. Abdominal muscle thickness was assessed using a 3.5 MHZ linear head transducer in B-mode condition.ResultsThe mean thickness for right and left rectus abdominis, external oblique, internal oblique, and transversus abdominis muscles at rest before food consumption were 10.8-10.7, 6.2-6.1, 8.6-8.3, 4.1-4.1 mm, respectively and after food consumption were 10.3-10.3, 5.6-5.5, 7.3-7.3, 3.4-3.6 mm, respectively. The mean thickness for right and left rectus abdominis, external oblique, internal oblique, and transversus abdominis muscles during Abdominal Hollowing Maneuver before food consumption were 11-10.8, 6.1-6.3, 10-10. 5.3-5.5 mm, respectively and after food consumption were 10.5-10.6, 5.7-5.7, 8.8-8.7, 4.4-4.5 mm, respectively.ConclusionAfter food consumption the thicknesses of all abdominal muscles in both sides are decreased, and therefore, food consumption must be considered when measuring abdominal muscle thickness using sonographyKeywords: Ultrasonography, Abdominal Muscle, Eating

Pages 250-257IntroductionIt appears that the level of fear of movement changes deep trunk muscle activity in the patients with low back pain (LBP). There is no study to investigate the relationship between deep trunk muscle activity and fear of movement in the patients with LBP. Thus, the purpose of this study was to assess the relationship between abdominal muscle activity and pain, disability and fear of movement during standing postural tasks in females with chronic nonspecific LBP.Materials And MethodsForty four females participated were asked to maintain their balance during standing on the platform stability levels of Biodex Balance System (BBS). Concurrently, ultrasonography (US) data about abdominal muscles thickness measurement were transferred and saved to process offline. The pain intensity, disability and fear of movement were assessed by valid scales and questionnaire.ResultsThere was not significant correlation between abdominal muscle thickness changes and pain and disability intensity (P>0.05), while significant and inverse correlation between deep abdominal muscle thickness changes and fear of movement was observed in the patients (PConclusionIt seems that increases in fear of movement decrease significantly deep abdominal muscles activity in the patients with LBP. Pages 258-264IntroductionPlantarflexor muscles produce propulsive force in the second half of stance phase; deficient motor output from these muscles would lead to inadequate propulsion at push off and decreasing walking speed. This study has examined the effects of walking on a treadmill at varying gradients and speeds on ankle muscle timing in healthy middle-aged adults.Materials And MethodsFifteen healthy middle-aged adults (10M/5F; average age 56.53±7.20 years; body mass index 28.52±3.70 kg/m2) participated in the study. Participants walked on a standard treadmill at three different walking speeds (self-selected, self-selected%, self-selected%) and positive inclines (0°, 3°, and 6°). The onset muscle timing of MedialGastrocnemius (MG), and Tibialis Anterior (TA) muscles recorded at push off phase of the gait.ResultsA linear mixed model (LMM) regression analysis was used to analysis. The onset of MG and TA muscles activity delayed at faster speeds on the nonodominant side (p0.05).ConclusionData showed that there is a bias towards using dominant side to overcome challenging walking conditions in middle-aged adults.Keywords: Electromyography, Exercise Test, Walking

Pages 265-275IntroductionTeachers often speak to high volume for a long time both in the noisy environments and in the stressful situations; therefore, their tissue of vocal cords are exposed to damage. The voice problems may have the adverse impacts on the quality of education, employment, economic and communicative conditions and also on the emotional status. In this study, the effects of voice problems on the teachers job performance communication, social and emotional status were investigated through comparing the teachers knowledge with/without dysphonia from their voice problems.Materials And MethodsThis study was conducted on 243 teachers at primary schools. The participants on the basis of response to the four choice items and in relation to the rate of his/her voice problems (always, often, sometimes and rarely) were divided into two groups with/without voice problems (VP = always and often) and lack of voice ones (NVP = sometimes and rarely). To collect the data, the questionnaire with α = 0.91 was used, and to investigate the relationship between the voice problems and the performance of teachers.ResultsThe findings showed that the voice problems among the teachers had a significant relationship with teaching, job attitude, ability for communication, social ability and their emotions (PConclusionThe teachers with voice problems compared with those without voice ones have more difficulties in job performance, communication, social and emotional status and also in their quality of life. Pages 276-285IntroductionKnee osteoarthritis (OA) is the most common joint disease characterized by deterioration of articular cartilage. Pharmacological treatments mainly emphasize on the regeneration of articular cartilage. Glucosamine sulfate is used as a cartilage protective substance with controversial results. This study was designed to investigate the efficacy of glucosamine phonophoresis on the knee osteoarthritis.Materials And MethodsThis clinical trial was performed on 41 female patients aged 45-75 years (21 unilateral and 20 bilateral cases). Patients randomly assigned into one of three groups. Group 1: This group received standard treatment with ultrasound (US)/off (n=15); Group 2: This group received standard treatment with US/on (n=12) (US was in continuous mode at frequency of 1 MHz and 0.3 watt/cm2 intensity, 5 min duration increasing gradually during the treatment sessions) and Group 3: This group received glucosamine phonophoresis (with equal ratio of %50 gel and glucosamine, and US/on as the same parameters). All treatment sessions were done daily on the dominant knee and for 30 sessions. Standard treatment included daily and on the dominant knee infrared, transcutaneous ekectrical stimulation and exercise. Pain, active knee flexion range of motion, swelling of knee joint, morning stiffness and quality of life (QOL) of all participants were measured before and after the intervention.ResultsThe data showed significant improvement in OA criteria (more decreased pain; swelling and morning stiffness, improvement in knee flexion and QOL index) in the intervention group than other placebo groups (PConclusionConsidering the etiology of disease, administration of treatments aimed on healing of articular cartilage seems to be more effective in the prevention of disease progression. Pages 286-294IntroductionChanges in invertor and evertor muscle torque is one of the sensory-motor disabilities along with functional ankle instability (FAI). According to positive advantages of whole body vibration (WBV) on muscle torque, this study has been conducted to evaluate the effect of WBV on evertor to invertor muscle concentric ratio in people with FAI.Materials And Methods30 female with FAI assigned randomly into experimental and control groups. The experimental group received 6 weeks WBV (3 times per weeks). Then they were evaluated according to their evertor and invertor muscle concent ric torque. Concentric muscle torque was evaluated by Biodex Isokinetic.ResultsThere were no significant differences between concentric torque of evertor and invertor muscles (P> 0.05). The gross motor function was assessed using the gross motor function classification system, manual ability was assessed using manual ability classification system, intensity of spasticity was evaluated using modified ashworth scale and sensory process was assessed by sensory profile questionnaire.ResultsThe results of this study showed that the gross motor function and manual ability of children with cerebral palsy are related to low muscle tone and tolerance, poor sensory registration, and sedentary, significantly. Pages 302-307IntroductionMultiple Sclerosis is (MS) a neurological disorder with damage in central nervous system. This disease begins between ages of 20-40. A common problem in patients is voice disorders that have negative impact on the quality of life of patients. The aim of this study was to examine the quality of life in MS patients with the voice disorder.Materials And MethodsThis is a descriptive-analytic study. Participants included 30 people with multiple sclerosis who referred to Semnan (Iran) clinics of neurology. The patients who were their EDSS score between 1-6 and at least 3 years of onset disease last were enrolled. All Patients were asked to produce the vowel /a/ and /i/ at a comfortable loudness level and a constant pitch at least three seconds in three trials and sample sounds were analyzed for acoustic parameters by Praat software, then voice handicap index (VHI) was presented to all participants to complete.ResultsThe finding of this study indicated that, there is not statistically significant correlation between acoustic parameters and VHI.ConclusionThe results of this study indicated that voice disorders have no significant effects on the quality of life in patients with MS, but these disorders should be detected and treated during the rehabilitation of these patents.Keywords: Multiple Sclerosis, Voice Disorders, Quality of Life
