صفحات 816-824مقدمهبازگشت جریان خون خود به خودی هدف اولیه احیاء در ایست قلبی است. کیفیت احیای قلبی ریوی(CPR[1]) عامل مهم تعیین کننده بازگشت جریان خون خود به خودی بیمار و پیامد احیاء است. بازخورد در احیاء بر بهبود کیفیت CPR موثر بوده اما تاثیر آن بر پیامد بیمار نامشخص است. مطالعه حاضر با هدف تعیین«اثر بازخورد کاپنوگرافی حین CPR بر بازگشت جریان خون خودبخودی ([2]ROSC)» انجام شد.روش کاراین کارآزمایی بالینی بر روی 130 بیمار ایست قلبی در دو گروه احیاء به روش معمول و احیاء با بازخورد کاپنوگرافی توسط یک تیم احیاء مجزا در بیمارستان قائم (عج) مشهد انجام شد. بازگشت جریان خون خود به خودی با کاپنوگراف سیار با روش جریان جانبی بررسی شد. ابزار جمع آوری داده ها شامل فرم های ارزیابی اولیه بیمار، ثبت فرایند احیاء و اطلاعات دموگرافیک بود که روایی و پایایی آن تایید شد.نتایجاز 130 بیمار شرکت کننده اکثریت آنان (60 %) مرد بودند. میانگین سنی بیماران 7/15 ±2/57 سال و شایع ترین ریتم قلبی اولیه، آسیستول(3/%52) بود. در بیش از 50% موارد، مدت زمان رسیدن تیم بر بالین بیمار کمتر از یک دقیقه بود. میزان بازگشت جریان خون خود به خودی بیماران در گروه کنترل 6/44% و در گروه احیاء با کاپنوگرافی 1/63 % بود که تفاوت چشمگیری بین دو گروه وجود داشت (035/0< p).نتیجه گیریبازخورد کاپنوگرافی در حین احیاء میزان بازگشت جریان خون خود به خودی و کیفیت احیاء را ارتقاء می دهد، لذا استفاده از آن در تمامی موارد احیاء بیمارستانی توصیه می شود.کلیدواژگان: احیاء قلبی، بازخورد کاپنوگرافی، بازگشت جریان خون خود به خودی

صفحات 825-835مقدمهبا توجه به اینکه یک شاخص با صحت تشخیصی بالا می تواند بیماران آماده برای جداسازی از ونتیلاتور را به درستی شناسایی کند و از بروز جداسازی زودرس و عوارض آن جلوگیری نماید این مقاله با هدف مقایسه دو مورد شاخص CROP و RSBI و بررسی ارزش تشخیصی آن ها در پیش بینی نتیجه جداسازی از ونتیلاتور انجام شد.روش کارمطالعه حاضر به روش آینده نگر یک سو کور بر روی 80 بیمار بستری در سه آی سی یو(داخلی-عمومی–سکته های مغزی) بیمارستان قائم(عج) مشهد از مهر سال 1394 تا اردیبهشت سال 1395انجام شد. واحدهای پژوهش بعد از احراز شرایط ورود به مطالعه در یک گروه مورد بررسی قرارگرفتند. همه بیماران آماده برای جداسازی از ونتیلاتور به مدت 30 دقیقه تحت تهویه با مد حمایت فشاری با حمایت حداقل قرار گرفتند. در صورت ثبات علایم حیاتی در طی این 30 دقیقه پارامترهای مورد نیاز برای محاسبه شاخص ها بوسیله ونتیلاتور اندازه گیری و گازهای خون شریانی اندازه گیری و ثبت شد. سپس تمرین تنفس خود به خودی به مدت 120 دقیقه با قطعه تی انجام شد. در نهایت بر اساس نتیجه جداسازی و پیش بینی شاخص ها، قدرت تشخیصی شاخص ها مورد ارزیابی قرار گرفت.نتایجتعداد 57 مورد جداسازی موفق و 23 مورد جداسازی ناموفق وجود داشت. حساسیت شاخص های CROP و RSBI به ترتیب 85% و 2/98% و ویژگی آن ها به ترتیب 75% و 26% بود. صحت تشخیصی شاخص CROP (2/85 %) ز شاخص RSBI بیشتر بود.نتیجه گیریشاخص CROP دارای بیش ترین صحت تشخیصی بود و حساسیت و ویژگی آن بالا بود. نقطه برش شاخص جداسازی RSBI به همه بیماران با اتیولوژی های مختلف قابل تعمیم نمی باشد.کلیدواژگان: بخش های مراقبت ویژه، جداسازی از ونتیلاتور، شاخص های جداسازی از ونتیلاتور

صفحات 836-846مقدمهشباهت زیاد ویژگی های بالینی اختلال شخصیت مرزی و دوقطبی-II علاوه بر مسائل تشخیصی، مجادلات علمی فراوانی را درباره رابطه میان این دو اختلال به وجودآورده است. مطالعات مختلفی به بررسی شباهت ها و تفاوت های این دو اختلال در حوزه های مختلف علامت شناسی، آسیب شناسی و سبب شناسی پرداخته اند. بر این اساس هدف از مطالعه حاضر، بررسی و مقایسه کارکردهای اجرایی در بیماران مبتلا به اختلال شخصیت مرزی و اختلال دوقطبیII به منظور شناخت بیشتر از رابطه میان دو اختلال است.روش کار25 بیمار مبتلا به اختلال شخصیت مرزی و 27 بیمار مبتلا به اختلال دوقطبیII ، با روش نمونه گیری در دسترس از بین بیماران مراجعه کننده به بیمارستان کامکار-عرب نیای شهر قم انتخاب شدند این دو گروه با یکدیگر و با 20 نفر از افراد سالم با استفاده از فرم کامپیوتری آزمون دسته بندی کارت های ویسکانسینکه ابزاری برای ارزیابی کارکردهای اجرایی است، مورد مقایسه قرار گرفتند. داده های بدست آمده با استفاده از روش های آماری تی مستقل، خی دو و آنووا تجزیه و تحلیل شدند.نتایجیافته های پژوهش نشان داد که دو گروه بالینی در تمامی کارکردهای اجرایی عملکرد ضعیف تری نسبت به گروه کنترل سالم داشتند. بیماران مبتلا به اختلال شخصیت مرزی ضعیف ترین کارکرد را در حوزه انعطاف پذیری شناختی بدست آوردند. بیماران مبتلا به اختلال دوقطبی پایین ترین عملکرد را در ناتوانی از دریافت بازخورد نشان دادند. بین دو گروه بالینی تفاوت معناداری در هیچ یک از مولفه های کارکرد اجرایی مشاهده نشد.نتیجه گیریاین نتایج، حمایت تجربی در تایید یافته های پیشین فراهم آورد که حاکی از کارکردهای اجرایی مختل در این دو گروه از بیماران است.از سوی دیگر هرچند تفاوت معناداری بین دو گروه بالینی در کارکردهای اجرایی مشاهده نشد اما مطالعات گسترده برای روشن تر شدن رابطه میان دو اختلال ضرورت دارد.کلیدواژگان: اختلال شخصیت مرزی، اختلال دوقطبی II، کارکردهای اجرایی، اختلالات طیف دوقطبی

صفحات 847-852مقدمهکافئین یکی از پر مصرف ترین مواد محرک در بین ورزشکاران است که می تواند با افزایش هوشیاری و عملکرد همراه باشد. هدف از این پژوهش پاسخ به این سوال مهم است که آیا مصرف کافئین بر میزان فشار خون شناگران تاثیر دارد؟روش کاردر مطالعه حاضر 16شناگران جوان به صورت تصادفی انتخاب و براساس سن و شاخص توده بدنی در دو گروه کافئین و دارونما قرار گرفتند. شناگران جوان یک ساعت قبل از شنا 5 میلی گرم به ازای هر کیلوگرم وزن بدن کافئین یا دارونما در قالب کپسول های ژلاتینی مصرف کردند. فشار خون آزمودنی ها در سه مرحله، قبل از مصرف کافئین یا دارونما، یک ساعت پس از مصرف و بلافاصله بعد از پایان شنای 100 متر کرال سینه اندازه گیری شد. داده ها با استفاده از آزمون آماری تی مستقل و تی زوجی مورد تجزیه و تحلیل قرار گرفتند.نتایجنتایج نشان داد که فشار خون سیستولی و دیاستولی و میانگین فشار خون شریانی با مصرف کافئین در حالت استراحت افزایش یافت(05/0>p)، اما در پایان فعالیت تفاوت بین کافئین و دارونما معنی دار نبود(05/0< p).نتیجه گیریبا توجه به یافته ها، مصرف کافئین در هنگام استراحت باعث افزایش فشار خون افراد می گردد و به نظر نمی رسد مصرف این ماده قبل از شنا عامل خطرناکی بشمار آید.کلیدواژگان: کافئین، فشار خون، شناگران جوان

صفحات 853-863مقدمهبا توجه به شیوع بالای بیماری های مزمن، یافتن درمان های مکمل می تواند یکی از راهکارهای مقابله با این بیماری ها باشد. مطالعات متعددی نشان داده اند که مصرف عسل می تواند اثرات مفیدی بر روی عوامل پیش گویی کننده بیماری های قلبی عروقی داشته باشد. مطالعه ی حاضر با هدف بررسی اثر مصرف عسل در مقایسه با سوکروز بر روی فشار خون و قند خون ناشتا در افراد جوان صورت پذیرفت.روش کار60 فرد سالم 18 تا 30 سال به مدت یک ماه وارد این کار آزمایی بالینی دوسو کور شدند. شرکت کنندگان به طور تصادفی به دو گروه دریافت کننده ی عسل و سوکروز تخصیص داده شدند که گروه عسل 70 گرم عسل را در روز و گروه سوکروز همین مقدار شکر را به صورت روزانه به مدت یک ماه مصرف کردند. فشار خون شرکت کنندگان در ابتدا و انتهای مطالعه اندازه گیری شد و قند خون ناشتای تمامی افراد قبل از شروع مطالعه و در پایان مداخله از شرکت کنندگان گرفته شد.نتایجدر ابتدای این مطالعه، قند خون ناشتا، فشار خون سیستولیک و دیاستولیک تفاوت معناداری بین افراد شرکت کننده در مطالعه نداشت (3/0> p). در پایان مطالعه، بر خلاف سوکروز، مصرف عسل توانست موجب کاهش قند خون ناشتا در افراد جوان سالم شود (001/0< p). اما دریافت شکر یا سوکروز بر فشار خون سیستولیک یا دیاستولیک اثر معنی داری نداشتند (4/0> p). در همه این بررسی ها، اثر متغیر های مخدوش گر مثل سن، فعالیت فیزیکی و دریافت های غذایی تعدیل شدند.نتیجه گیریدریافت عسل طبیعی می تواند باعث کاهش قند خون ناشتا در افراد سالم شود ولی در دوز 70 گرم روزانه تاثیر معنی داری بر فشار خون سیستولیک یا دیاستولیک ندارد. مطالعات بیشتری برای تایید یافته های ما مورد نیاز است.کلیدواژگان: عسل، فشار خون، قند خون ناشتا

صفحات 864-876مقدمهبا توجه به اهمیت نشانگان منفی افسردگی و عواطف منفی در بهزیستی روانی، هدف از مطالعه حاضر تعیین اثربخشی الگوی درمانی کاهش استرس براساس ذهن آگاهی بر عواطف مثبت و منفی وکاهش نشانگان افسردگی در جمعیت غیربالینی است.روش کارپژوهش حاضر از نوع شبه آزمایشی است. جامعه ی آماری شامل تمام دانشجویان دختر مقطع کارشناسی در تمامی دانشکده های دانشگاه شاهد تهران می باشد. پس از انتخاب آزمودنی های واجد شرایط، افراد به صورت تصادفی در دو گروه درمان کاهش استرس براساس ذهن آگاهی(11 نفر) و گروه کنترل (9 نفر) قرار گرفتند. اعضای گروه آزمایش به مدت دو ماه (هشت جلسه) در بازده زمانی آبان تا دی ماه در دوره ی درمانی که در دانشکده انسانی دانشگاه شاهد تهران برگزار شد، شرکت کردند، در حالی که کنترل هیچ نوع درمانی دریافت نکردند. پرسش نامه ی افسردگی بک (II) و مقیاس های عاطفه مثبت و منفی پاناس قبل و بعد از دوره درمانی به آزمودنی های هر دو گروه داده شد. داده های حاصل با استفاده از آزمون مقایسه دو گروه یومن ویتنی و آزمون ویلکاکسون و از طریق نرم افزار SPSS تحلیل گردید.نتایجالگوی درمانی کاهش استرس براساس ذهن آگاهی منجر به کاهش سطح نشانگان افسردگی و عاطفه ی منفی در گروه آزمایش نسبت به گروه کنترل شد اما در سطح عاطفه ی مثبت شرکت کنندگان تغییری ایجاد نکرد. براین اساس می توان این الگوی درمانی را با رعایت احتیاط های بالینی در قشر دانشجویی و جمعیت غیر بالینی به کار گرفت.نتیجه گیریبراساس نتایج، درمان کاهش استرس براساس ذهن آگاهی در کاهش نشانگان افسردگی و عاطفه ی منفی جمعیت غیربالینی موثر می باشد.کلیدواژگان: افسردگی، عاطفه منفی، عاطفه مثبت، ذهن آگاهی، الگوی درمانی کاهش استرس براساس ذهن آگاهی

بررسی ضریب پایایی دومین آزمون صلاحیت های بالینی دانشجویان پزشکی مشهد با استفاده از تئوری تعمیم پذیریصفحات 877-887مقدمهآزمون آسکی یکی از موثرترین روش ها برای ارزشیابی مهارت ها می باشد که بدلیل دشواری در اجرای آزمون آسکی سنجش کیفیت آن، امری حیاتی است. یک گام مهم در سنجش کیفیت هر آزمون، تعیین ضریب پایایی آن آزمون می باشد، لذا این پژوهش با هدف تعیین ضریب پایایی دومین آزمون صلاحیت های بالینی مشهد با استفاده از تئوری تعمیم پذیری انجام گردیده است.روش کاردومین آزمون صلاحیت های بالینی مشتمل بر 15ایستگاه موازی بود. 75 دانشجو و 30 ممتحن در این آزمون حضور داشتند. چک لیست های آزمون به تفکیک ایستگاه و ممتحن جمع آوری و در بسته نرم افزاری اکسل ثبت گردید. مولفه های واریانس، مجموع مربعات و میانگین مربعات با استفاده از نرم افزارSPSS نسخه 16 و آزمون ANOVA محاسبه گردید. سپس واریانس ایستگاه ها، دانشجویان و ممتحنین و همچنین ضرایب تعمیم پذیری و پایایی آزمون با استفاده از نرم افزارEDUG به دست آمد.همچنین برای بررسی رابطه بین جنسیت و نمرات از آزمون های آماری Mann-Whitney Uو chi squareاستفاده گردید.نتایجضریب پایایی آزمون 98/0 محاسبه گردید و مولفه تعامل دانشجویان با ممتحنین و ایستگاه ها بیشترین سهم را در ایجاد خطای اندازه گیری دارا بود. بین جنسیت دانشجویان و نمرات آن ها ارتباط معنا داری مشاهده نشد، همچنین بین جنسیت ممتحنین و نمرات دانشجویان نیز ارتباط معناداری به دست نیامد.نتیجه گیریدومین آزمون صلاحیت های بالینی مشهد، پایایی بسیارخوبی داشت و با توجه به نتایج حاصله می توان گفت، استفاده از تئوری تعمیم پذیری برای برآورد ضریب پایایی و مشخص نمودن خطاهای اندازه گیری روش مناسبی است اما پژوهش های بیشتری باید انجام شود تا بتوان با قطعیت بیشتری ادعا نمود که استفاده از تئوری تعمیم پذیری، مزایای بیشتری نسبت به تئوری کلاسیک دارد.کلیدواژگان: رینگر، رینگر لاکتات، اسیدیته خون، جراحی الکتیو

صفحات 888-899مقدمهنگهداری حجم کافی خون گردشی و پرفیوژن بافتی و بالانس الکترولیت ها در طی جراحی ها مهم می باشد و ترکیبات مختلفی شامل محلول های کریستالوئید و کلوئیدی به این منظور پیشنهاد شده اند. مطالعه حاضر با هدف بررسی و مقایسه اثر دو روش مایع درمانی با رینگرلاکتات و رینگر بر اسیدیته خون و عوارض پس از عمل انجام شد.روش کارمطالعه حاضر یک کارآزمایی بالینی در سال96 بر روی86 بیمارکاندید اعمال جراحی الکتیو از نوع ماژور که در کلاس بیهوشی ASA I و II بودند و مشکلات قلبی ریوی و کلیوی نداشتند انجام شد. مایع درمانی در حین عمل، در یک گروه(43نفر) به کمک محلول رینگر لاکتات و در گروه دیگر(43نفر) با رینگرانجام شد. قبل از جراحی، جهت انجام آزمایش هموگلوبین، هماتوکریت، آلبومین و الکترولیت ها نمونه خون وریدی گرفته شد. در پایان بیهوشی بیماران از نظر تاخیر در بیدار شدن و تهوع و استفراغ بررسی شدند.نتایجبعد از مایع درمانی در گروه رینگر لاکتات میزان هموگلوبین و هماتوکریت به طورمعناداری کاهش یافت و میزان آلبومین افزایش معناداری داشت و در گروه رینگر تفاوت معناداری در میزان آن ها وجود نداشت پتاسیم و کلسیم در گروه رینگر لاکتات و کلر در گروه رینگر به طور معناداری افزایش یافتند. بیکربنات و PHدر گروه رینگر لاکتات افزایش و در گروه رینگر به طور معناداری کاهش یافته بود. سدیم در دو گروه قبل و بعد از مایع درمانی اختلاف معناداری نداشت. مقدار خونریزی حین عمل و مدت زمان بستری پس از عمل در گروه رینگر بیشتر بود که از نظر آماری معنادار نبود. تهوع و استفراغ در بیماران گروه رینگر بیشتر بود که از نظر آماری معنادار نبود. تاخیر در بیداری در گروه رینگر به طور معناداری بیشتر بود.نتیجه گیریجایگزینی حجم مایعات با کریستالوئیدهای رینگر و رینگرلاکتات در جراحی های الکتیو اختلاف معنی داری در میزان PH و عوارض بعد از عمل ایجاد می کند و اسیدیته خون و تاخیر در بیداری در تجویز رینگر نسبت به رینگر لاکتات بیشتر بود.کلیدواژگان: رینگر، رینگر لاکتات، اسیدیته خون، جراحی الکتیو

This study was to investigated the Effect of Capnography Feedback During CPR on Return of Spontaneous Circulation ".
Subjects &MethodsThis clinical trial study was performed on 130 cardiac arrest patients in two group by conventional CPR and with the use of Capnography feedback by a Separate CPR team at Ghaem Hospital in Mashhad. Rate of return of spontaneous circulation of the patients were evaluated. portable sidestream capnography was used.ResultsIn this study, the majority (60%) of 130 patients were male.The mean age of patients was 57.2 ± 15.7 year and the most common initial cardiac rhythm was asystole (52.3 %). In more than 50% of cases the arrival time of cpr team was less than a minute, return of spontaneous circulation rate in the control group was 44.6 % and in the Capnography group 63.1%, which was significantly different (P =0/035).ConclusionBased on our findings, Capnography feedback during resuscitation improves quality of cpr and rate of return of spontaneous circulation, therefore, its use is recommended in all cases of hospital resuscitation.Keywords: Cardiopulmonary resuscitation, Capnography feedback, return of spontaneous circulation -
Pages 825-835BackgroundUsage of indicators to predict the outcome of weaning from the ventilator is a controversial issue. An index with high diagnostic accuracy can properly identify the Patients ready for weaning from mechanical ventilation and prevents premature weaning and its complications. Two of the introduced weaning indicators are RSBI and CROP but their predictive value in predicting the outcome of weaning from mechanical ventilation is different or contradictory.ObjectivesThis study was to Compare RSBI and CROP in predicting the outcome of weaning from mechanical ventilation in patients admitted to ICU.Materials And MethodsThis prospective, single-blind study was performed on 80 adult patients who were on mechanical ventilation for more than 24 hours in 3 ICUs (Internal-General-stroke) of Mashhad Ghaem hospital from October 2015 to May 2016. All the patients ready for weaning were identified and studied in one group. All of them were ventilated with PSV mode with minimum support for 30 mins. If the patients vital signs were stable during those 30 mins, we measured the required respiratory variables and arterial blood gas values to calculate CROP and RSBI. Then the SBT was performed, with a T-piece for 120 mins. finally the diagnostic accuracy of each index was evaluated according to the weaning outcome and prediction of indexes.ResultsThere were 57 successful and 23 unsuccessful weaning cases. The specificities were CROP 0.85 and RSBI 98.2. The sensitivities were CROP 0.75 and RSBI 0.26. The diagnostic accuracy were CROP 85.2 and RSBI 77.5. The new threshold value obtained in this study was very different for RSBI.ConclusionsAccording to the analysis performed by the cut-off points obtained in previous studies, CROP index has the highest diagnostic accuracy, sensitivity and specificity were both high. The cut-off point of RSBI previously reported for non-homogeneous patients population is not extensible to all with different etiologies.Keywords: weaning from mechanical ventilation, predicting weaning outcome, predictores of weaning outcome

Pages 836-846IntroductionHuge similarities between clinical characteristics of borderline personality disorder and bipolarII disorder has caused many conversations in terms of the relationships between two disorders.Many studies have investigated the similarities and differences of these two disorders in the field of symptomology, pathology and etiology. On this base, the goal of the current research was to investigate and compare the executive function of borderline and bipolar II disorders patients to know more about the relationship between these two disorders.
Subjects &MethodsA total of 25 patients with borderline personality disorder and 27 patients with bipolar-II disorder, who were referred to the Kamkar-Arabnia hospital in Qom, were selected through convenience sampling method. These two groups were compared together and to a healthy control group, by Wisconsin Card Sorting Test, that is used to measure the executive functions. Collected data were analyzed by using independent t-test, X2 and ANOVA.ResultsResults showed that two clinical groups had poorer performance in all of the executive functions in comparison with healthy controls. Patients with borderline personality disorder showed poorest performance on cognitive flexibility domain. Patients with bipolar II had lowest performance on inability to receive feedback. There was no significant difference between the two clinical groups in none of the executive functions domains.ConclusionThese results provides empirical supportfor previous findings, which have reported executive dysfunctions, in both groups of patients. Although, there is no significant difference between the two clinical groups in executive functions in this study, more extensive studies are needed for clarification of relationship between the disorders.Keywords: Borderline personality disorder, Bipolar II, Executive functions, Bipolar spectrum disorders -
Pages 847-852IntroductionCaffeine is one of the most commonly used stimulants among athletes, which can be accompanied by increased vigilance and performance. The purpose of this research was to answer the question whether caffeine consum ption affect the blood pressure in swimmers?
Subjects &MethodsIn this study, 16 young swimmers selected randomly in two groups based on age and body mass index, caffeine and placebo. Young swimmers swam an hour before the 5 mg caffeine per kg of body weight or placebo was ingested in the form of gelatin capsules. Blood pressure of subjects in three stages, before taking caffeine or placebo, one hour after use and immediately after swimming the 100m front crawl was measured. Data, using independent t-test and paired t-test, were analyzed.ResultsThe results showed that systolic and diastolic blood pressure and mean arterial blood pressure increased with caffeine intake at rest (p0/05).ConclusionAccording to the findings, caffeine increases blood pressure at rest and do not seem to be taking this matter before swim, considered to be a risk factor.Keywords: blood pressure, Caffeine, Young Swimmers -
Pages 853-863IntroductionGiven the high prevalence of chronic diseases, complementary treatments can be one of the ways to decrease the prevalence of these diseases. It has been shown that honey consumption can have beneficial effects on cardiovascular disease risk factors. The current study aimed to investigate the effect of honey intake compared with sucrose on blood pressure and fasting blood glucose among young healthy subjects.
Subjects &MethodsThere were 60 healthy subjects, aged 18 to 30 years that were enrolled in this double blind randomized trial for one month. Participants were assigned randomly into two groups: honey (received 70 gram honey per day) and sucrose (received 70 gram sucrose per day). We measured Systolic and diastolic blood pressure and collected fasting blood glucose from all subjects in the beginning and end of study.ResultsIn the present study, there was no difference on the baseline FBS, SBP and DBP between the two groups (P > 0.3). Results showed that honey intake can decrease the FBS in healthy young individuals, but sucrose consumption increased it. In addition, The effect of sucrose on FBS was move significant than honey (PConclusionIt is our conclusion that honey consumption decreases the FBS, but has no significant effect on SBP and DBP. Pages 864-876IntroductionDue to the importance of negative depression syndrome and negative affects in psychological well-being, the aim of the present study was to determineý ýthe effectiveness of mindfulness-based stress reduction model on Positive and Negative Affects and reducing the Depression syndrome in a non-clinical population.MethodologyThe present research was a quasi-experimental study. The statistical population included all female undergraduate students studying in all faculties of Shahed University of Tehran. Having selecting eligible participants, they were categorized into two groups randomly: mindfulness-based stress reduction group (11 people) and control group (9 people). Experimental group participated in the course which was hold at the faculty of Humanities of Shahed University for two months from November to January (eight sessions) , while the control group did not receive training at all. The two groups received two questionnaires before and after their training, i.e., Beck Depression Inventory (II) and Positive and Negative Affect scale (PANAS). The acquired data were analyzed using U Mann Whitney and Wilcoxon signed-rank tests via SPSS software.
Resuls: Using therapy pattern of mindfulness-based stress reduction results in reducing Depression syndrome and negative affects levels in experimental group compared to control group but do not make change iný ýPositive affectý ýlevel participants. Pages 877-887IntroductionOSCE is a combination of simulation tasks to assess medical students'' skills in diagnosis and treatment of diseases. it is vital to measure the quality of an OSCE. One of the important steps in assessing the quality of a test is to determine it''s reliability. This study aimed to determine the reliability coefficient of the second clinical skills test in Mashhad by using generalizability theory.
Subjects &MethodsThe OSCE was conducted in two days and consists of 15 parallel stations. 75 students and 30 examiners participated in this test. Check lists collected and recorded in Excel. Variances and mean squares (MS) calculated by using SPSS. Variance for stations¡ students and examiners and generalizability and reliability coefficients calculated by using EDUG. Mann-Whitney U and Chi square tests were used for assessing relation between students gender and score¡ raters gender and score and relation between students gender and raters gender.ResultsThere was no significant relationship between gender of students and their scores¡ gender of examiners and students scores¡ and between gender of students and gender of examiners. Reliability coefficient was calculated 0/98 and interaction between students¡ examiners and station component had the largest effect on creating measurement error.Conclusionthis exam had a good reliability coefficient and according to results we can say¡ using Generalizability Theory for estimating the reliability coefficient¡ measurement error¡ and to identify impact of any facets on measurement error is a logical way. Pages 888-899IntroductionIt is important to keep a sufficient amount of circulating blood and tissue perfusion during surgery. Different compounds including crystalloid and colloidal solutions are proposed for this purpose. There is a lot of evidence that there is a clinically difference in the use of various crystalloids. In this study, the effect of two methods of fluid therapy with Ringers Lactate and Ringers solution on acid-base balance and postoperative complications in patients candidate for elective operation was studied.
Subjects &MethodsThis clinical trial study was performed on 86 patients undergoing major anesthetic surgery in ASA class I and II and had no heart and kidney problems. The fluid therapy was performed with a ringer's lactate solution in one group (n = 43) and in the other group with ringer solution (n = 43). Before starting surgery, blood samples were taken for hemoglobin, hematocrit, albumin and electrolytes, and arterial blood samples for analysis of arterial blood gas. At the end of the anesthesia, the patient was transferred to the recovery and was evaluated for recovery in the wake delay and other complications such as nausea and vomiting.ResultsIn this study, hemoglobin and hematocrit were significantly reduced in ringer's lactate groups after fluid therapy, and the amount of albumin after fluid therapy was reduced in the ringer's lactate group.There was no significant difference in sodium levels in both groups before and after fluid therapy. However, the amount of potassium and calcium significantly increased after the liquid therapy in the ringer's lactate group and the amount of chlorine in the ringer group significantly increased. Bicarbonate and PH were increased after fluid therapy in ringer's lactate group and decreased significantly in ringer group. Also, the amount of bleeding during operation and duration of postoperative hospitalization was higher in ringer group, but this difference was not statistically significant. Nausea and vomiting were reported more in the ringer group than in the ringer's lactate group, but this difference was not statistically significant. Also, the delay in waking in the ringer group was significantly higher than ringer's lactate group.ConclusionAccording to the findings of this study, it seems that in elective surgery, the replacement of crystalloids ringer and ringer's lactate, there is a significant difference in PH and postoperative complications following the use of these serums. The blood acidity following fluid therapy with ringer was higher in comparison to fluid therapy with ringer's lactate, and the delay in awakening in this group was reported high.Keywords: Ringer, Ringer Lactate, Blood acidity, Elective surgery
