صفحات 22-27پیشزمینهدرمان با استفاده از تکنیک گچ گیری پونستی روش استاندارد درمان کلاب فوت است. هدف از این مطالعه، بررسی تاثیر استفاده از تکرار روش پونستی در مواردی است که پس از اقدامات قبلی اصلاح نشده است.مواد و روش هااین مطالعه آینده نگر بر روی بیماران بزرگتر از 3 ماه مبتلا به کلاب فوت که دفرمیتی آنها با گچ گیری های قبلی اصلاح نشده بودند و برای عمل جراحی به کلینیک های ما ارجاع شده بودند انجام شد. نمرات اولیه و نهایی «پیرانی» در یک پیگیری متوسط 5/4 ساله مقایسه شدند. نمرات عملکردی، میزان عود، و روش های اضافی مورد نیاز برای اصلاح نیز بررسی گردیدند.یافته ها79 پا از 53 بیمار در سنین 3 ماهگی تا 9 سالگی مورد بررسی قرار گرفتند که میانگین نمرات پیرانی آنها 4/4 بود و قبلا به طور میانگین 8/7 (5/2±) بار گچگیری شده بودند و 9 مورد تنوتومی آشیل داشتند. به طور متوسط 3/4 (7/1±) (محدوده: 2 تا 8) بار گچ گیری و 32 مورد بلند کردن تاندون آشیل (ATL) مورد نیاز بود تا اصلاح کامل در 77 پا (98% موارد) صورت گیرد. عود دفرمیتی در 42 پا (53%) دیده شد که منجر به اصلاح 89% موارد در پیگیری نهایی شد. با این حال، نمره عملکردی در پایان، در 93% موارد خوب یا عالی بود.نتیجه گیریاستفاده مجدد از تکنیک پونستی در موارد مقاوم به درمان، از میزان موفقیت بالایی، حتی در مراحل دیررس برخوردار است.کلیدواژگان: کلاب فوت، دفرمیتی مادرزادی پا، درمان، گچ گیری

صفحات 28-38پیشزمینهاستئوآرتریت به عنوان بیماری دژنراتیو مفصلی شناخنه می شود. داروهای سنتتیک در دسترس برای درمان استئوآرتریت، عوارض جانبی خطرناک و اثربخشی کم دارند. بنابراین، استفاده از داروهای گیاهی، جزء مهم طب مکمل و جایگزین است، سلول های بنیادی مزانشیمی از جایگاه ویژه ای در درمان بیماری ها برخوردار می باشند. برگ گواوا به عنوان آنتی اکسیدان، ضد التهاب و ضد درد استفاده می شود. هدف از این مطالعه، بررسی تاثیر تزریق داخل مفصلی عصاره هیدرو الکلی برگ گواوا همراه با سلول بنیادی در زانوی رت می باشد.مواد و روش هاابتدا 90 سر رت نر انتخاب و برای ایجاد استئوآرتریت در مفصل زانو، کلاژناز نوع 2 تزریق شد. سپس به 9 گروه تقسیم شدند. که شامل: شم، کنترل، درمان با هایالگان، درمان با سلول های بنیادی، درمان با پلاسمای غنی از پلاکت، درمان با عصاره برگ گواوا، درمان با عصاره برگ گواوا و پلاسمای غنی از پلاکت، درمان با عصاره برگ گواوا و سلول بنیادی و درمان با عصاره برگ گواوا، سلول بنیادی و پلاسمای غنی از پلاکت می باشد. پس از 5 ماه از درمان، نتایج توسط رادیولوژیست و پاتولوژیست مورد ارزیابی قرار گرفت.یافته هاارزیابی رادیوگرافی و هیستوپاتولوژیکی، ترمیم غضروف مفصلی در گروه عصاره برگ گواوا همراه با سلول بنیادی و پلاسمای غنی از پلاکت را بهتر از گروه کنترل نشان داد (P<0.05).نتیجه گیریبه نظر می رسد عصاره هیدروالکلی برگ گواوا را به دلیل دارا بودن ترکیبات پلی فنولی زیاد، می توان به عنوان طب مکمل و جایگزین مناسب در استئوآرتریت پیشنهاد کردکلیدواژگان: استئوآرتریت، پسیدیوم گواوا، پلاسمای غنی از پلاکت، سلول های بنیادی سینوویوم، رت

صفحات 39-45پیشزمینهشکستگی های کلاویکل بدنبال فیکساسیون داخلی دارای عوارض مختلفی است. هدف اصلی مطالعه حاضر بررسی شیوع عوارض شکستگی های کلاویکل بدنبال فیکساسیون داخلی است. همچنین بررسی شیوع عفونت، نارضایتی محل، نان یونیون و مال یونیون در شکستگی کلاویکل بدنبال فیکساسیون داخلی می باشد.مواد و روش هاپژوهش از نوع عرضی بود. نمونه آماری 52 بیمار بود که با شکستگی های کلاویکل که تحت درمان جراحی با روش جااندازی باز و فیکساسیون داخلی بودند. به منظور تعیین دامنه حرکت، قدرت و درد شانه بیماران از امتیاز شانه کانستانت استفاده شد و برای معاینه و بررسی یونیون از طریق گرافی صورت گرفت.یافته هااز 52 بیمار 12 نفر زن و 40 نفر مرد بود. میانگین امتیاز شانه کانستانت 2±84 ( 83-84 CI 95%) بود. یک سوم بیماران از هوک استفاده کرده بودند. بیماران بیشتر از ظاهر زخم (96%)، بی حسی محل پوست جراحی شده (80%) و درد (1%/73 ) شکایت داشتند. میزان عفونت در بیماران بسیار پایین بود.نتیجه گیریعارضه ای که بیشتر شایع است که شامل: درد متوسط، نارضایتی از ظاهر زخم و بی حسی موضعی است که می توان گفت نسبتا امری طبیعی است و میتوان بیان کرد در بیشتر بیماران دیده می شود. البته باید این موضوع را نیز در نظر گرفت که تفاوت های تکنیک های جراحی جراحان، مسائل فرهنگی، جنسیتی و سن بیماران در این امر می تواند تاثیر گذار باشد.کلیدواژگان: شکستگی های کلاویکل، جااندازی باز، فیکساسیون داخلی، درمان جراحی، عوارض

صفحات 46-53پیشزمینهیکی از روش های درمان دررفتگی های مفصل آکرومیوکلاویکولار (ACJ)، تکنیک Weaver-Dunn است که تا کنون modification های مختلفی از آن ارائه شده است. با توجه به مشکلات روش های قبلی و ارزش حفظ لیگامان کوراکوآکرومیال (CAL)، ما در این مطالعه به معرفی یک modification جدید از تکنیک Weaver-Dunn پرداختیم. همچنین نتایج اولیه این روش بررسی شد.مواد و روش ها15 بیمار دچار دررفتگی حاد ACJ در مطالعه بررسی شدند. در این روش نیمی از بخش مدیال CAL از آکرومیون جدا میشود. لیگامان جدا شده از سوراخی که در کلاویکل ایجاد شده، عبور داده و روی خود گره زده شود. قبل و پس از جراحی، CC distance هر دو سمت اندازه گیری و مقایسه شد. در ویزیت نهایی، دو پرسشنامه Constant و UCLA تکمیل شد. شدت درد با استفاده از visual analogue scale (VAS) تعیین شد. بیماران بطور میانگین 6/2±2/4 سال پیگیری شدند.یافته هاCC distance قبل از عمل بطور معناداری در سمت آسیب دیده بیشتر بود (mm 6/1±5/19 در برابر mm 5/0±1/7؛ 001/0>p). در ویزیت نهایی CC distance سمت آسیب دیده بطور قابل توجهی کاهش یافت و اختلاف اندک آن با سمت سالم، معنادار نبود (mm 9/0±2/8 در برابر mm 5/0±1/7؛ 318/0=p). در ویزیت نهایی، میانگین Constant score سمت جراحی شده برابر 2/13±3/93 و سمت سالم 8/10±1/95 بود که اختلاف بین دو سمت از نظر آماری معنادار نبود (118/0=p). میانگین UCLA score برابر 3/3±6/32 و میانگین درد بر اساس VAS برابر 8/0±4/1 بود.نتیجه گیریدرمان دررفتگی های حاد ACJ با این تکنیک با نتایج مطلوبی همراه است. با این روش، ثبات مفصل حفظ و عملکرد آن بازگردانده می شود.کلیدواژگان: مفصل آکرومیوکلاویکولار، دررفتگی، لیگامان کوراکوآکرومیال، تکنیک Weaver-Dunn

صفحات 54-62پیشزمینهامروزه در ارتوپدی دامپزشکی و انسانی برای تحریک التیام شکستگی ها، سرعت بخشیدن به اتصال مفصلی و ترمیم نقیصه های استخوانی از پیوند استخوانی استفاده می شود. به این منظور محققین مدام در حال تحقیق در انتخاب بهترین جایگزین ترمیم استخوان هستند.هدف از این مطالعه بررسی اثرات ترانکرون و هیدروکسی آپاتیت بر روند ترمیم استخوان در مدل حیوانی خرگوش می باشد.مواد و روش هاابتدا 20 قطعه خرگوش تهیه شد و به چهار گروه پنج تایی تقسیم شدند. یک گروه تزریقات ترانکرون به محل نقیصه استخوانی در روزهای 3، 7، و 10 بعد از عمل صورت گرفت و یک گروه کنترل خالی بود و گروه دیگر در محل نقیصه هیدروکسی آپاتیت جایگذاری شد و گروه دیگر ترکیب هیدروکسی آپاتیت و ترانکرون بود. رادیوگراف ها از دست خرگوش ها بعد از عمل و در روزهای 14، 28 ،42 و 56 در نمای جانبی تهیه شد و نمونه هیستوپاتولوژی هم در آخر دوره از استخوان زند زبرین در محل نقیصه گرفته شد.یافته هااز نظر رادیوگرافی گروه هیدروکسی آپاتیت-ترانکرون بهترین عملکرد رو داشت و گروه کنترل خالی ضعیف ترین عملکرد را دارا بود. از نظر هیستوپاتولوژی هم بهترین عملکرد گروه هیدروکسی آپاتیت-ترانکرون بود و ضعیف ترین عملکرد مرتبط با گروه کنترل خالی بود.نتیجه گیریدر مجموع گروه ترانکرون و ترانکرون هیدروکسی آپاتیت در مقایسه با دو گروه دیگر از نظر ترمیم استخوانی بهتر عمل کرده بودند.کلیدواژگان: ترمیم استخوان، ترانکرون، هیدروکسی آپاتیت، خرگوش، زند زبرین

صفحات 63-70پیشزمینهعمل جراحی تعویض مفصل زانو یک روش درمانی تهاجمی موثر و مقرون به صرفه است که میزان موفقیت آن در کاهش درد و بهبود عملکرد زانو در بیماران مبتلا به آرتریت پیشرفتهزانو بسیار بالاست . اما عواملی وجود دارد که ممکن است موجب مردود شدن این عمل جراحی شود که به دنبال آن بیمار مجبور به جراحی مجدد می گردد. از شایع ترین عوامل عمل جراحی مجدد زانو می توان به مواردی چون عفونت ، عملکرد نامناسب مکانیکی مفصل تعویض شده ، مردود شدن ایمپلنت یا شگستگی آن اشاره کرد که عفونت به عنوان شایع ترین عامل ، از علل اصلی جراحی مجدد زانو محسوب می شود. هدف ما از این پژوهش کشف روش های جدید در راستای کاهش نارسایی های ناشی از ایمپلنت و هم چنین افزایش طول عمر ایمپلنت در جراحی های آرتروپلاستی زانو است.
مواد وروشجستجو در بانک های اطلاعاتی pupmed، science direct و مجله materials برای شناسایی مقالات چاپ شده و مطالعات صورت گرفته بر روی عفونت های ناشی از جراحی ایمپلنت های استخوانی انجام شد. تعداد 76 مقاله از سال های 2003 تا 2016 بررسی شدند که 50 مورد از این مقالات در متن استناد شده.نتایجاز 50 مقاله استناد شده در این پژوهش 41 مقاله تاثیر درمان های غیر آنتی بیوتیکی بر عفونت های ایمپلنت های استخوانی را مورد بررسی قرار داده اند. تعداد 5 مقاله به بررسی دقیق درمان های غیر آنتی بیوتیکی و اثرات آن بر بافت استخوانی به شکل درون تنی پرداخته و 4 مقاله دیگر در ارتباط با ترکیب این عوامل با آنتی بیوتیک ها و افزایش اثر بخشی آن ها بعد از جراحی است.نتیجه گیریاستفاده از مواد غیر آنتی بیوتیکی برای جلوگیری از عفونت های بعد از جراحی زانو و تشکیل بیوفیلم در این ناحیه ،از روش هایی است که برای جلوگیری از مقاومت باکتری ها در برابر آنتی بیوتیک ها پیشنهاد می شود. امروزه در این جراحی ها عموما از سیمان پلی متیل متا اکریلات آغشته به آنتی بیوتیک هایی مثل ونکومایسین و جنتامایسین استفاده می گردد ، که ممکن است با مقاومت از سوی باکتری مواجه شده و بیوفیلم تشکیل شود و نهایتا منجر به عفونت های حاد گردد. در این پژوهش مروری ، به بررسی تکنولوژی هایی پرداخته ایم که می توانند در کنار یکدیگر باعث بهبود در عملکرد ایمپلنت های ارتوپدی شده ، شاید بتوان به وسیله آنها تا حدودی عفونت های ناشی از جراحی مفصل زانو را کاهش داد. همچنین نتایج این گزارشات نشان داده ترکیب تکنولوژی های مختلف به طور هم زمان میتواند باعث ارتقاء مهار عفونت میکروبی شود.کلیدواژگان: عفونت در ایمپلنت های ارتوپدی، درمان های ضد میکروبی، بیو مواد ضد چسبندگی، ضد بیوفیلم، بیومواد ضدمیکروبی

Pages 22-27Aim: Closed treatment using Ponseti casting technique is the standard management for clubfoot deformity. This study is investigating the efficacy of repeat use of Ponseti technique in late cases that had not achieved correction following previous attempts.MethodA prospective study was conducted on clubfeet cases of over 3 months of age who had not achieved correction with previous casting and were referred for surgery to our clinics. The Baseline and final Pirani scores were compared in a 4 .5- year average follow-up. The functional scores, recurrence rates and additional procedures required for correction were also documented.Results79 feet in 53 patients of 3 months to 9 years of age were studied. They had mean Pirani scores of 4.4 and had previously received 7.8 ± 2.5 castings and 9 Achilles tenotomies. An average of 4.3 ± 1.7(28) castings and 32 Achilles tendon lengthening were needed to achieve complete correction in 77 feet (98%). Relapse of deformity was seen in 42 (53%) feet, which dropped the correction rate in the final follow-up to 89%. However, the functional score at the end , was good or excellent in 93% of the cases.DiscussionAccurate re-use of Ponseti technique in cases of previously- failed clubfoot casting has high success rate, even in late referrals.Keywords: Clubfoot, Congenital Foot Deformity, Treatment, Plaster Casts

Therefore, using of herbal medicines is an important component of alternative and complementary medicine, Mesenchymal stem cells have a special site in the treatment of diseases. drugs.Psidium guajava Leaves used as antioxidant, anti-inflammatory antinociceptive and analgesic. The goals of scientific research of this study were to the evaluate the regeneration articular cartilage effect of injection of the hydroalcoholic extract of leave of Psidium Guajava with Stem Cell into the knee joints of rats.MethodsFirstly, ninety male rats were choosen, and collagenase type II was prepared for an intra-articular knee injection, for them to induce OA in the animal models. Then, they were divided into nine treatment groups. (N = 10): (a) Sham, (b) Control, (c) injection of hyaluronic Acid intra-articular, (d) Injections of Stem Cell intra-articular, (e) Injections of PRP, (f): Injections of the extract Psidium guajava leaves (g) Injections of the extract Psidium guajava leave with PRP, (h) Injections of the extract Psidium guajava leaves with Stem Cell (i) Injections of the extract Psidium guajava leaves with Stem Cell and PRP. After 5 months, the results were assessments by a Radiologist and Histopathologist.ResultsRadiological and histopathological findings were in which the improvement of the articular cartilage was better in the leaves with Stem Cell and PRP compared with the control group(PConclusionsIt seems that Psidium guajava leaves can be suggested as an effective complementary and alternative treatment for OA of the knee. Pages 39-45BackgroundClavicle fractures have various complications following open reduction and internal fixation. The main objective of this study was to investigate the prevalence of complications of Clavicle fractures following operative treatment. Also, the prevalence of infection, local dissatisfaction, non:::::union::::: and mal:::::union::::: in the Clavicle fracture after internal fixation was investigated.
Material andMethodsThe study was cross-sectional. A sample of 52 patients was treated with clavicle fractures, which were treated with open resection and internal fixation. In order to determine the range of motion, strength and shoulder pain, patients were selected from Constant Shoulder Score. Radiography used to detect :::::union:::::.Results12 were female and 40 were male of 52 patients. The mean score of the shoulder was 84 ± 2 (95% CI 83-84). One third of the patients used Hook. Patients complained more dysmorphic scar (96%), skin numbness (80%) and pain (73.1%). The rate of infection in patients was very low.ConclusionsThe most common complication is pain, dysmorphic scar and skin numbness that can be said to be relatively normal and can be seen in most patients. Of course, it should also be taken into account that the differences in surgical techniques, surgeries, cultural issues, gender, and age of the patients can be affected.Keywords: Clavicle Fractures, Complication, Open Reduction, Operative Treatment, Internal Fixation -
Pages 46-53BackgroundWeaver-Dunn technique is one the prevalent methods for treating acute acromioclavicular joint (ACJ) dislocation. Until now, several modifications of this technique have been introduced. Regarding the drawbacks of the previous modifications and the importance of presence of an intact coracoacromial ligament (CAL), we introduced a new modification of Weaver-Dunn technique and investigated the preliminary results.Materials And MethodsThere were 15 patients with acute ACJ enrolled in current study. In the modified surgical technique, the medial half of the CAL is dissected from the bone and passed through a hole made in the clavicle bone and tied on itself. Before the operation and at the last visit, CC distance was measured on both sides. At the last visit, Constant score and UCLA score were completed. The pain intensity was measured using visual analogue scale (VAS). The patients were followed for 4.2±2.6 years.ResultsPreoperatively, CC distance of injured side was significantly greater than the other side (19.5±1.6 mm Vs 7.1±0.5 mm; pConclusionTreatment of acute ACJ dislocations using the modified technique was associated with favorable outcomes. Pages 54-62BackgroundNowadays, bone grafting is used in both human and veterinary orthopedics to stimulate fracture healing, accelerate joint ::::union::::, and to restore bone defects. In such procedures, orthopedic surgeons are searching to favorable substitute for autograft bone. The purpose of this study is evaluation of theranekron and hydroxyapatite effects on bone healing in rabbit model.MethodsFirst, 20 rabbits were prepared and they were divided into four groups of five. In the group of theranecron, theranecrone was injected to the bone defected site on days 3, 7, and 10 after surgery and one of the group was left empty as control group and in the third group combination of the theranekron and hydroxyapatite was filled in the defected site and in the final group hydroxyapatite was implanted alone. Radiographs from rabbit's hand after surgery were taken on days 14, 28, 42, 56 in lateral view. At the end of the study, histopathological samples were taken from injured area of radius.ResultsIn radiographical and histopathological evaluations the group of theranekron-hydroxyapatite had the best performance in healing criteria and the control group was the weakest group.Conclusiontheranekron group and theranekron hydroxyapatite groups were better than two other groups in bone healing criteria by radiological and histopathological evaluation.Keywords: bone healing, theranekron, hydroxyapatite, rabbit, radius

Pages 63-70total Knee arthroplasty is an effective and cost-reducing therapeutic approach, which is very useful in relieving pain and improving the performance of patients with advanced knee arthritis, but there are some factors that may lead to the failure of this surgery. From that which patients need for re-surgical repair, the most common causes of knee replacement are included as infection, poor mechanized joint replacement, implant failure, or frailty of implant. Infection is the most common cause of the main cause Knee replacement surgery. The main goal in this study is to explore new procedures to reduce implant-induced defects and also increase the implants life in knee arthroplasty surgery.Methodologysearch was carried out in PubMed, science direct and magazine of materials for identification of published articles and studies on infections caused by bone implants. 76 articles were reviewed from 2003 to 2016 and 50 articles of them were cited in the text.ResultsFrom the 50 papers cited in this study, 41 articles examined the effects of non-antibiotics on infections of bone implants. 5 papers have been reviewed carefully for non-antibiotic treatments and their effects on bone tissue in vitro and 4 are related to combination of these agents with antibiotics and subsequent increase in their efficacy after surgery.ConclusionUsing of non-antibiotic agents to preventing cause infections after knee surgery intended to preventing biofilm formation in this area is one of the methods suggested to prevent antibiotic resistance against antibiotics.
Today, in these surgeries, polyethylmetacrylate cements are commonly used filled with antibiotics such as vancomycin and gentamicin, which unaccustomedly may be exposed by bacteria and biofilms, and ultimately lead to acute infections. Our overview is exploring the technologies that can improve the performance of orthopedic implants, which may reduce the severity of knee joint infections.
