    زمینه و هدف
    کالپروتکتین، S100 A8 و S100 A9 در فرآیندهای مهمی نظیر پیام رسانی و تنظیم پاسخ های التهابی نقش دارند. در این مطالعه، بیان S100 A8 و S100 A9 بصورت نوترکیب، خالص سازی و بررسی ساختار آنها انجام گردید.
    روش کار
    در این مطالعه تجربی از pET15b به عنوان حامل توالی کد کننده ژنهای S100 A8، S100 A9 انسانی و از (E.coli BL21 (DE3 به عنوان میزبان استفاده گردید. بیان ژن و فرآیند خالص سازی از طریق SDS-PAGE مورد ارزیابی قرارگرفت. خالص سازی پروتئینها با استفاده از رزین Ni-NTA و بر اساس میل ترکیبی آن به His-Tag پروتئین های نوترکیب انجام گرفت. ساختار سوم پروتئین ها با طیف سنج فلورسانس بررسی شد.
    یافته ها
    زیر واحدهای مذکور 4-3 ساعت بعد از القاء و دمای 37 درجه سانتی گراد بیان بالایی را نشان دادند. درصد بالایی از پروتئین S100A9 در فاز اینکلوژن بادی نشان داده شد، در حالیکه زیر واحد S100A8 عمدتا بصورت محلول بیان گردید. S100A8 و S100A9 در غلظت mM 100 ایمیدازول خالص سازی گردید. در بررسی طیف سنجی نشان داده شد که اسید آمینه تریپتوفان در ساختار های داخلی قرار گرفته و کمتر در معرض محیط آبی است.
    نتیجه گیری
    در این مطالعه زیر واحدهای S100A8 و S100A9 بصورت نوترکیب بیان شده و تخلیص گردید. همچنین، ساختار آنها مورد تایید قرار گرفت.

    زمینه و هدف
    سکته مغزی سومین عامل مرگ و معلولیت در جوامع صنعتی است. پیشنهاد شده است که سیستم رنین- آنژیوتانسین ممکن است در مکانیسمهای پاتولوژیک سکته مغزی نقش داشته باشد. هدف از این مطالعه بررسی اثرات درمانی مهار گیرنده های AT1 آنژیوتانسین دو بر ضایعه ایسکمیک مغزی می باشد. این مطالعه با استفاده از مدل آزمایشگاهی ایسکمی موضعی مغز در موش صحرایی با فشار خون طبیعی انجام شده است.
    روش کار
    در این تحقیق تعداد 48 عدد موش صحرایی نر از نژاد Sprague - Dawley درچهارگروه مورد مطالعه قرار گرفتند.گروه شاهد، گروه کنترل ایسکمیک و و دو گروه ایسکمیک که داروی کندسارتان را (با دوزهای 1/0 میلی گرم بر کیلوگرم یا 5/0 میلی گرم بر کیلوگرم) در آغاز مرحله برقراری مجدد جریان خون پس از وقوع ایسکمی دریافت کردند. ایسکمی موضعی با استفاده از تکنیک انسداد شریان میانی مغز توسط فیلامان مخصوص در نیمکره راست مغز حیوانات به مدت یک ساعت ایجاد شد. در این حیوانات 24 ساعت پس از پایان ایسکمی اختلالات حرکتی با استفاده از آزمون نورولوژیک ویژه ای ارزیابی شد وحجم ضایعه مغزی ایجاد شده با استفاده از تکنیک رنگ آمیزی با تری فنیل تترازولیوم کلرایدتعیین شد.
    یافته ها
    حیوانات گروه شاهد عملکرد حرکتی طبیعی داشتند (نمره آزمون نورولوژیک 1) و آسیب ایسکمیک در نواحی قشری و زیر قشری مغز آنها مشاهده نشد. وقوع ایسکمی در گروه کنترل سبب ایجاد حجم ضایعه قابل ملاحظه ای در قشر مغز (15 ± 253 میلیمترمکعب) و نواحی زیر قشری (7 ± 92 میلیمتر مکعب) شد و حیوانات این گروه اختلالات حرکتی شدیدی داشتند. مهار گیرنده های AT1 آنژیوتانسین دو با داروی کندسارتان (با دوزهای 1/0 میلی گرم بر کیلوگرم یا 5/0 میلی گرم بر کیلوگرم) اختلالات حرکتی ناشی از ایسکمی را کاهش داد و حجم ضایعه مغزی را در نواحی قشری و زیر قشری مغز بطور معنی داری کاهش داد.
    نتیجه گیری
    یافته های این مطالعه نشان می دهد که آنژیوتانسین دو از طریق تحریک گیرنده های AT1 در ایجاد ضایعه ایسکمیک مغزی نقش دارد. و مهار این گیرنده ها می تواند حجم ضایعه ایسکمیک مغزی را کاهش داده و اختلالات حرکتی ناشی از سکته مغزی را بهبود ببخشد.

    زمینه و هدف
    استرپتوکوک های گروه B (GBS) از مهم ترین علل عفونت های نوزادان و مادران می باشد. این گروه از باکتری ها هنوز نسبت به پنی سیلین، آمپی سیلین و سفالوسپورین های نسل اول حساس هستند. با این وجود مقاومت در برابر دیگر آنتی بیوتیک ها مانند اریترومایسین و کلیندامایسین رایج است. این مطالعه با هدف بررسی اثر ضد میکروبی آلیسین بر GBS در شرایط آزمایشگاهی انجام گرفت.
    روش کار
    عصاره سیر تهیه و آلیسین با استفاده از روش HPLC نیمه کمی خالص سازی شد. حداقل غلظت بازدارندگی (MIC) و حداقل غلظت کشندگی (MBC) آلیسین برای 52 ایزوله GBS کلونیزه کننده جدا شده از دستگاه تناسلی در خانم های حامله به روش میکرودایلوشن در محیط کشت تاد هویت براث تعیین شد.
    نتایج
    MIC آلیسین برای سویه های مورد مطالعه 64-128 میکرو گرم در میلی لیتر (90= 128 MIC) و MBC آن 128-512 میکرو گرم در میلی لیتر (90= 256 MBC) بود.
    نتیجه گیری
    نتایج این مطالعه نشان داد که آلیسین می تواند رشد GBS را در شرایط آزمایشگاهی مهار نماید. لذا با مطالعات بیشتر، احتمالا می توان در آینده از آن برای مقابله با عفونت های ناشی از این باکتری استفاده نمود.

    زمینه و هدف
    ترایکوفایتون روبروم معمول ترین پاتوژن مولد بیماری درماتوفیتوزیس است. یکی از داروهایی که به طور وسیعی جهت درمان تجویز می شود، تربینافین از گروه آلیل آمین ها می باشد. اخیرا روش های تایپینگ مولکولی برای پاسخ دادن به سوالات اپیدمیولوژیکی و مشکلات عود مجدد بیماری گسترش زیادی یافته اند. این مطالعه روی 22 ایزوله ترایکوفایتون روبروم که به طور اتفاقی از بیماران مبتلا جدا شده بود، انجام گرفت. هدف مطالعه بررسی رابطه الگوی ژنتیکی و حساسیت به داروی تربینافین در ایزوله های کلینیکی ترایکوفایتون روبروم بود.
    روش کار
    ابتدا جنس و گونه قارچهای جداشده از بیماران با روش های ماکروسکوپی و میکروسکوپی تایید گردید. سپس با استفاده از محیط کشت های حاوی مقدار معین دارو مقاومت و حساسیت ایزوله ها نسبت به دارو تعیین شد.در مرحله بعد DNA قارچ ها استخراج گردید و RAPD-PCR (تکثیر تصادفی قطعات پلی مرفیک) با استفاده از 3 پرایمر با توالی تصادفی انجام گرفت.
    یافته ها
    هر پرایمر الگوی تکثیریافته متفاوتی را ایجاد کرد و با هر 3 پرایمر تفاوتهایی در الگوی ژنتیکی نمونه های حساس و مقاوم دیده شد ولی هیچ باندی که 100% اختصاصیت را نشان دهد یافت نشد.
    نتیجه گیری
    12ایزوله حساس که در غلظت 1/0 میکروگرم در میلی لیتر دارو رشد نکردند،در غلظت های پایین تر دارو نیز رشد آن ها به میزان چشمگیری در مقایسه با پلیت شاهد محدود شد.10 ایزوله مقاوم که در غلظت 1/0میکروگرم در میلی لیتر دارو رشد کردند، در غلظت های پایین تر دارو نیز از خود مقاومت نشان دادند.
    آنالیز RAPD جهت تایپینگ مولکولی ایزوله های ترایکوفایتون روبروم کاملا مناسب نشان داد.

    زمینه و هدف
    شناسایی سریع بیمارانی که دارای ایزوله های مایکوباکتریوم توبرکلوزیس مقاوم به دارو هستند امری ضروری برای درمان این بیماران می باشد. متاسفانه مقاومت نسبت به دو داروی ایزونیازید و ریفامپین در سال های اخیر مطرح شده است. هدف از مطالعه حاضر شناسایی سریع ایزوله های کلینیکی مایکوباکتریوم توبرکلوزیس مقاوم با استفاده از روش های مولکولی بوده است. مطالعه حاضر به منظور مقایسه بین دو روش Real-time PCR بر اساس Taqman assay و HRM assay در تشخیص موتاسیون های موجود در ژن های rpoB، inhA و katG در ایزوله های کلینیکی مایکوباکتریوم توبرکلوزیس انجام گرفت.
    روش کار
    مطالعه حاضر در مرکز تحقیقات مایکوباکتریولوژی و در طی سال های 92-91 صورت گرفت. تست حساسیت دارویی به ایزونیازید و ریفامپین با روش پروپورشنال و بر روی 83 نمونه انجام گرفت. سپس روش های مالتیپلکس PCR و Real-time PCR بر روی DNA استخراج شده انجام شد. Real-time PCR بر اساس روش های Taqman و HRM انجام شد و موتاسیون های موجود در ژن های rpoB، inhA و katG مورد بررسی قرار گرفت.
    یافته ها
    بر اساس روش پروپورشنال و مالتیپلکس PCR، 47 مورد مقاوم به ریفامپین و 35 مورد مقاوم به ایزونیازید بودند. 30 مورد مقاوم به هر دو داروی ایزونیازید و ریفامپین بودند. تشابه روش Real-time PCR بر اساس Taqman در تشخیص مقاومت 88% و در تشخیص حساسیت 84% بود. از طرفی تشابه روش Real-time PCR بر اساس HRM در تشخیص مقاومت 96% و در تشخیص حساسیت 30% بود. در روش Taqman در مقایسه با مالتیپلکس PCR، حساسیت 84% و اختصاصیت 88% به دست آمد. در روش HRM نیز حساسیت 30% و اختصاصیت 96% به دست آمد.
    نتیجه گیری
    نتیجه مطالعه انجام گرفته نشان داد روش Real-time PCR بر اساس Taqman نسبت به روش HRM در تشخیص مقاومت دارویی مایکوباکتریوم توبرکلوزیس، از حساسیت بیشتری برخوردار بوده است و روشی سریع، مقرون به صرفه (اقتصادی)، دقیق و با حساسیت و اختصاصیت بالا می باشد.

    زمینه و هدف
    زایمان از جمله وقایع مهمی است که اکثریت قریب به اتفاق زنان در زندگی تجربه کرده و درد جز اجتناب ناپذیر آن است که در صورت عدم کنترل منجر به طولانی شدن زایمان و بروز صدمات متعدد در مادر و جنین می گردد لذا دستیابی به تدابیری جهت تسکین درد و کنترل طول مدت زایمان خصوصا با استفاده از روش های غیرتهاجمی نظیر طب فشاری ضروری به نظر می رسد. این پژوهش به منظور تعیین تاثیر فشار در نقاط SP6 و LI4 بر شدت درد و طول مدت زایمان در زنان نخست زا انجام گرفته است.
    روش کار
    این مطالعه کارآزمایی بالینی کنترل شده تصادفی بر روی 84 نفر از زنان نخست زا، در بیمارستان های دولتی شهر اردبیل انجام شد. شرکت کنندگان به روش تصادفی توسط بلوک بندی 4 و 6 تایی به دو گروه تقسیم شدند. مداخله بصورت اعمال فشار در نقاط SP6 و LI4 در دیلاتاسیون های مختلف بود. شدت درد قبل و بعد از مداخله با استفاده از مقیاس بینایی سنجش درد و طول مدت فاز فعال و مرحله دوم زایمان از طریق معاینه واژینال محاسبه و در چک لیست ثبت شد. برای تعیین تاثیر فشار بر شدت درد و طول مدت زایمان از آزمون های آماری تی زوج، تی مستقل و کای دو در نرم افزار SPSS / Ver. 16 استفاده شد.
    یافته ها
    میانگین شدت درد بعد از هر بار مداخله در گروه آزمون کاهش یافت و میانگین شدت درد بعد از مداخله در گروه آزمون در هر چهار دیلاتاسیون مورد بررسی به طور معنی دار کمتر از گروه کنترل بود (p < 0/001)، همچنین میانگین طول مدت فاز فعال و مرحله دوم زایمان در گروه آزمون به طور معنی دار کمتر از گروه کنترل بودند (p< 0/001، p= 0/008).
    نتیجه گیری
    فشار بر نقاط SP6 و LI4 در دیلاتاسیون های مختلف باعث کاهش شدت درد و طول مدت فاز فعال و مرحله دوم زایمان می گردد. لذا این روش آسان، غیر تهاجمی و بی خطر می تواند بطور موثری برای مددجویان در بالین و همچنین در حیطه های آموزشی و پژوهشی مورد استفاده قرارگیرد.

    زمینه و هدف
    نارسایی مزمن کلیوی که در حال حاضر تحت عنوان بیماری مزمن کلیوی نامیده می شود به دلیل تخریب نفرونهای کلیه ایجاد و منجر به اورمی می گردد. فشار خون بالا، دیابت ملیتوس و گلومرونفریت از علل شایع ایجاد این نقص می باشند. این وضعیت می تواند سبب ایجاد تظاهرات دهانی متعدد گردد. هدف از انجام این مطالعه مقایسه تظاهرات دهانی در بیماران دیابتی و غیر دیابتی تحت همودیالیز بود.
    روش کار
    در این مطالعه توصیفی- تحلیلی تعداد 95 نفر از بیماران تحت همو دیالیز بیمارستان رازی شهر رشت مورد بررسی قرار گرفتند. بیماران به دو گروه دیابتی (32 نفر) و غیر دیابتی (63 نفر) تقسیم شدند. معاینه حفره دهان با دستکش و آینه یکبار مصرف صورت گرفت. علائم و نشانه های دهانی بیماران نظیر خشکی دهان، تغییر مزه، درد مخاطی، بوی اورمیک، زبان باردار، پتشی و پورپورا، زخم مخاطی، رنگ پریدگی مخاط دهان، اروژن دندانی و عفونت کاندیدا در پرسشنامه بیماران ثبت گردید. جهت تجزیه و تحلیل اطلاعات از نرم افزار آماری SPSS15 و آزمون تی تست ومجذور کای استفاده گردید.
    یافته ها
    60% (57 نفر) از افراد مورد مطالعه مرد و 40% (38 نفر) را زنان تشکیل می دادند. میانگین سنی بیماران 48 (محدوده سنی 20-76) سال بود. شایعترین علامت دهانی در دو گروه خشکی دهان بود. شایعترین نشانه دهانی در دو گروه رنگ پریدگی مخاط، بوی اورمیک و زبان باردار بود. همچنین ایندکس DMFT به طور معنی داری در گروه دیابتی (63/7 ± 3/17) بیشتر از گروه غیر دیابتی بود (26/8 ± 4/12). ارتباط آماری معنی داری بین مدت زمان انجام دیالیز و تعداد جلسات انجام دیالیز در هفته و تظاهرات دهانی عینی و ذهنی میان دو گروه مشاهده نگردید.
    نتیجه گیری
    تحقیق حاضر نشان داد که تظاهرات دهانی در بیماران دیابتی تحت همودیالیز از گروه غیر دیابتی بیشتر بود هرچند این افزایش از نظر آماری معنی دار نبود، همچنین میزان DMFT در گروه دیابتی بیشتر از گروه غیر دیابتی بود. بنابراین به علت اضافه شدن عوارض دهانی دیابت در افراد با نارسایی مزمن کلیه این افراد مستلزم رعایت بهداشت دهان و دندان و مراقبت بهداشتی بیشتر می باشند.

    زمینه و هدف
    اختلال تنفسی حین خواب یک بیماری قابل پیشگیری و نسبتا شایع که در صورت عدم درمان، منجر به خواب آلودگی، اختلالات شناختی و افسردگی، بیماری های قلبی و عروقی و فشار خون می شود. شیوع آن در بالغین دو تا چهار درصد است. موثرترین راه درمان این اختلال، استفاده از دستگاه سی پاپ می باشد و غالبا بعنوان اولین انتخاب درمانی بکار می رود. برای درمان بیماران با دستگاه سی پاپ، لازم است فشار آن حین انجام پلی سومنوگرافی تنظیم گردد. هدف از این مطالعه بررسی متغیرها و پارامترهای تاثیر گذار در تعیین فشار سی پاپ می باشد.
    روش کار
    از میان بیماران مراجعه کننده به کلینیک خواب، افرادی که نیاز به دستگاه سی پاپ داشتند انتخاب گردیدند. تیتراسیون بر اساس پروتکل آکادمی خواب آمریکا صورت گرفت. مقایسه میان پارامترهای پلی سومنوگرافی با فشار سی پاپ صورت گرفت. داده ها توسط شاخصهای آمار مرکزی و نیز توزیع فروانی و همچنین توسط آزمون های T مستقل، ضریب همبستگی پیرسون و نیز کای اسکوئر در سطح معنی داری 05/0> P با استفاده از نرم افزار نسخه 18 SPSS تجزیه و تحلیل گردید.
    یافته ها
    از 125 بیمار مراجعه کننده با تشخیص اختلال تنفسی حین خواب، 112 نفر وارد مطالعه شدند. سن متوسط12 ± 07/55 سال و از این تعداد 20/59% مرد بودند. اندکس آپنه هیپوپنه 62/43 و فشار متوسط سی پاپ 50/12 سانتی متر آب بود. ارتباط معناداری مابین فشار سی پاپ با اندکس آپنه هیپوپنه (028/0= p)، اندکس بیداری (011/0= p)، شاخص توده بدنی (041/0= p) و اندکس افت اشباع اکسیژن شریانی (022/0= p) مشاهده شد ولی با سن ارتباط معناداری یافت نشد.
    نتیجه گیری
    بر اساس این مطالعه، روشی برای پیش بینی بهترین فشار سی پاپ در بیماران اختلال تنفسی حین خواب بدست آمد.

    زمینه و هدف
    شواهد اپیدمیولوژیکی و آزمایشگاهی حاکی از وجود استرس اکسیداتیو در بین مبتلایان به سندرم داون است. برخی از مطالعات نشان می دهند که رژیم های غذایی غنی از میوه جات و سبزیجات باعث کاهش قابل توجه شاخص های استرس اکسیداتیو می گردد. این بررسی با هدف بسامد مصرف میوه جات و سبزیجات و تاثیر آن بر نمایه های استرس اکسیداتیو در کودکان مبتلا به سندرم داون انجام گرفت.
    روش کار
    در این بررسی اطلاعات مربوط به الگوی مصرف میوه جات و سبزیجات بوسیله پرسشنامه و با سوال از والدین گردآوری گردید و غلظتهای مالون دی آلدیید (MDA) در نمونه های سرم و 8 - هیدروکسی- 2-داکسی گوانوزین (8-OHdG) در نمونه های ادرار به عنوان شاخص های استرس اکسیداتیو مورد اندازه گیری قرار گرفتند. از آزمونهای آماری تی استیودنت و آنالیز واریانس یک طرفه برای آنالیز داده ها استفاده گردید.
    یافته ها
    نتایج مطالعه نشان داد که میانگین بسامد مصرف هفتگی میوه جات در بین کودکان مورد مطالعه مبتلا به سندرم داون 6/4 وعده (سروینگ) و سبزیجات 74/0 وعده (سروینگ) بود. هیچگونه رابطه معنی داری بین میزان مصرف میوه جات و سبزیجات و زیست شاخص های استرس اکسیداتیو در کودکان مورد مطالعه مشاهده نگردید. بر اساس همین یافته 9/65% از این کودکان هیچ نوع سبزی را مصرف نمی کردند و در بین بقیه نیز میزان مصرف کمتر از یک وعده در روز بود. همبستگی معنی داری بین بسامد مصرف میوه جات با مصرف سبزیجات وجود داشت (r= 0/212; p< 0/04).
    نتیجه گیری
    در این مطالعه رابطه معنی داری بین شاخص های استرس اکسیداتیو و میزان مصرف میوه جات و سبزیجات در کودکان مبتلا به سندرم داون مشاهده نگردید. این عدم ارتباط می تواند ناشی از مصرف کم میوه جات و سبزیجات توسط کودکان مبتلا به سندرم داون باشد.

    زمینه و هدف
    دیابت یک بیماری مزمنی است که با اختلال در متابولیسم پروتئین، چربی و کربوهیدرات مشخص می شود. بیشتر داروهای پائین آورنده قند خون که برای درمان دیابت مصرف می شوند، در دراز مدت عوارض جانبی دارند ازسویی دیگر، افزایش استرس اکسیداتیو در پیشرفت بیماری دیابت موثر است. هدف این مطالعه ارزیابی تاثیر عصاره الکلی افوربیا سیپاریسیاس برسطح سرمی گلوکز وآنزیمهای آنتی اکسیدان در رتهای نر دیابتی شده می باشد.

    در این مطالعه از 40 سر موش صحرایی نر نژاد ویستار با وزن20 ± 250 گرم در 4 گروه بطور تصادفی استفاده شد. 1) گروه کنترل (حیوانات سالم دست نخورده)، 2) گروه دیابتی (ایجاد دیابت با داروی استرپتوزوتوسین)، 3) گروه تیمار 1 (دریافت عصاره با دوز 500 میلی گرم بر کیلو گرم) و 4)گروه تیمار 2 (دریافت عصاره با دوز 250 میلی گرم بر کیلو گرم). حیوانات گروه های تیمار 1 و 2 پس از ایجاد دیابت به مدت 21 روز عصاره را از طریق گاواژ دریافت کردند. در پایان دوره تجویز عصاره میزان گلوکز و سطح آنزیمهای آنتی اکسیدان SOD، MDA، TAC و GPX در سرم اندازه گیری شد.
    یافته ها
    یافته ها نشان داد که میانگین سطح سرمی گلوکز در گروه تیمار1 در مقایسه با گروه دیابتی بطور معنی داری پائین تر است (p < 0/05). همچنین مشخص شد که سطح MDA در سرم حیوانات دیابتی در مقایسه با گروه کنترل بطور معنی داری بالاتر بود (p < 0/05). علاوه بر این، در گروه تیمار 1 سطح TAC و نیز آنزیمهای GPX و SOD در مقایسه با گروه دیابتی شده بطور معنی داری بالاتر بود (p < 0/05).
    نتیجه گیری
    این مطالعه نشان داد که تجویز خوراکی عصاره الکلی افوربیا سیپاریسیاس در موشهای صحرایی دیابتی شده با استرپتوزوتوسین اثرات ضد دیابتی وآنتی اکسیدانی دارد.

    زمینه و هدف
    سندرم آیزن منگر همراه با حاملگی باعث افزایش عوارض و مرگ و میر مادران می شود. بنابراین در اکثر موارد توصیه به ختم حاملگی می شود.
    معرفی بیمار: خانم 29 ساله با سن حاملگی 7 هفته به علت سابقه سندرم آیزن منگر ناشی از نقص سپتوم دهلیزی (Atrial septal defect) کاندید سقط درمانی شد. بی حسی داخل نخاعی با 10 میلی گرم بوپیواکایین در وضعیت نشسته در فضای L4-L5 انجام شد. بیمار پس از 2 دقیقه در وضعیت طاقباز قرار گرفت. جراحی وی حدود یک ساعت به طول انجامید. در طول عمل و ریکاوری فشار خون و ضربان قلب تغییرات بارزی نداشت.
    نتیجه گیری
    بیحسی داخل نخاعی با بوپیواکایین برای سقط درمانی در بیماران مبتلا به آیزن منگر توصیه می شود.

    Background and Objectives
    Calprotectin, S100A8 and S100A9 are involved in important processes including cell signaling and regulation of inflammatory responses. In this study, recombinant expression, purification and structural characterization of S100A8 and S100A9 were accomplished.
    Methods
    In this experimental study, pET15b was used as vector of human S100A8 and S100A9 coding sequences, hosted by E.coli BL21 (DE3). Gene expression and purification attempts were evaluated using SDS-PAGE. Protein purification was accomplished using Ni-NTA resin based on its affinity for His-tag present on recombinant proteins. Tertiary structure of proteins were evaluated using spectrofluorimetry.
    Results
    The subunits were over expressed 3-4 hours following induction at 37 °C. S100A9 was expressed mainly as inclusion body while S100A8 was found to be expressed mainly as a soluble protein. Purification of S100A8 and S100A9 was achieved at 100 mM imidazole. Spectroscopic studies showed that the amino acid tryptophan is in the internal structures and is less exposed to the aqueous environment.
    Conclusion
    In this study, a recombinant S100A8 and S100A9 subunits were expressed and purified and also their structures were confirmed.

    Background and Objectives
    Ischemic stroke remains the third leading cause of invalidism and death in industrialized countries. It is suggested that renin–angiotensin system (RAS) may contribute in stroke related pathogenic mechanisms and involve in the ischemic brain damage. This study designed to investigate the role of angiotensin II (Ang II) in conjunction with AT1 receptors in treatment of the brain injuries following transient focal cerebral ischemia in rats.
    Methods
    Forty eight male Sprague-Dawley rats were studied in four groups. Sham group, ischemic control group and two ischemic groups that received candesartan (0.1mg/kg, or 0.5mg/kg) at the beginning of reperfusion period. Transient focal cerebral ischemia was induced by 60 minutes occlusion of the middle cerebral artery, followed by 24 hours reperfusion. At the end of the reperfusion period, neurological deficit score (NDS) was performed. Total cortical and striatal infarct volumes were determined using triphenyltetrazolium chloride (TTC) staining technique.
    Results
    Animals in sham operated group had normal motor function and no ischemic lesions were observed in cortical or striatal regions. Occurring ischemia in ischemic control group that received vehicle produced considerable infarction in cortex (253±15mm3) and striatum (92±7mm3), as well as these animals had sever impaired motor dysfunctions. Blocking of AT1 receptors with candesartan (0.1mg/kg or 0.5mg/kg) improved neurological outcome and significantly lowered cortical and striatal infarct volumes relative to ischemic control group.
    Conclusion
    The findings of the present study indicated that stimulation of AT1 receptors by Ang II involved in ischemia/reperfusion injuries and blocking of AT1 receptors can decrease ischemic brain injury and improve neurological outcome.

    Background And Objectives
    Group B streptococci (GBS) are the major cause of neonatal and maternal infections. They are susceptible to penicillin, ampicillin and first-generation cephalosporins. However, resistance to other antibiotics such as erythromycin and clindamycin is common among GBS strains. The aim of this study was to evaluate the antimicrobial activity of allicin against colonizing GBS strains in vitro.
    Methods
    Garlic extract was prepared and allicin was purified using semi-preparative HPLC procedure. Minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) of allicin were determined for 52 GBS strains using microdilution method in Todd Hewitt broth medium.
    Results
    MIC of allicin was 64-128 µg/mL (MIC90 = 128) and MBC of allicin was 128 to 512µg/mL (MBC90= 256) for GBS strains.
    Conclusion
    The results of this study showed that allicin can inhibit growth of GBS in vitro. Further studies on allicin might be worthy of evaluation as a therapeutic agent in treatment of GBS infections.

    Background and Objectives
    Trichophyton rubrum is one of the most common pathogeniccause of dermatophytosis. One of the medicines which has been prescribed widely for fungal infections is terbinafine which belongs to allylamines group of antifungal agents. Recently molecular typing methods have been developed for answering the epidemiological questions and disease recurrence problems. Current study has been conducted on 22 isolates of Trichophyton rubrum obtained from patients randomly. Our aim was the investigation of correlation between genetic pattern and sensitivity to Terbinafine in clinical isolates of Trichophyton rubrum.
    Methods
    Firstly the genus and species of isolated fungi from patients have been confirmed by macroscopic and microscopic methods, then, the resistance and sensitivity of isolates against medicine have been determined using culture medium containing defined amount of medicine. In next step fungal DNA has been extracted by RAPD-PCR (random amplified polymorphic DNA) with random sequences of 3 primers.
    Results
    Each primer produced different amplified pattern, and using each 3 primers differences have been observed in genetic pattern of resistant and sensitive samples using each 3 primers, but there was no bond with 100% specificity.
    Conclusion
    The 12 sensitive isolates which didn’t grow in 0.1 mg concentration of medicine, also had limited growth at the low concentration of medicine..Ten resistant isolates which grew in 0.1mg/ml of medicine, in lower concentration of medicine were resisted. RAPD analysis for molecular typing of Trichophyton rubrum seems to be completely suitable.

    Background and Objectives
    The rapid identification of patients carrying resistant Mycobacterium tuberculosis (M.TB) isolates is important for effective tuberculosis therapy. Unfortunately, during the recent years considerable numbers of isolates showed resistant to Rifampin (RIF) and Isoniazid (INH). The aim of this study was to rapidly identify resistant MTB isolates using molecular method. For this reason, the comparison between real-time PCR based on Taqman and HRM AssayS in detection of rpoB, inhA and katG genes mutation in clinical isolates were performed and analyzed.
    Methods
    The study carried out on Mycobacteriology Research Center (MRC) from 2012-2013. Classical susceptibility testing i.e., proportional method against INH and RIF was performed on eighty three M.TB isolates. Thereafter, multiplex and real-time PCR were performed on extracted DNA sample. The real-time PCR was based on Taqman and HRM assays. Mutation in genes rpoB, inhA and katG were detected.
    Results
    In overall, based on proportional and multiplex PCR method, 47 and 35 isolates were resistant to RIF and INH, respectively. Thirty of strains were resistant to both RIF and INH. The agreement of real-time PCR using Taqman was 88% for resistant and 84% for susceptible isolates, whereas the agreement of HRM was 96% and 30%, respectively. The sensitivity and specificity of Taqman in comparison to multiplex were 84% and 88%, respectively. In addition, the sensitivity and specificity of HRM were 30% and 96%, respectively.
    Conclusion
    Results documented that real-time PCR based on Taqman assay is more sensitive than HRM assay. Additionally, real-time PCR based on Taqman assay is a rapid, accurate and cost effective method in detection of Mycobacterium tuberculosis resistance.

    Background and Objectives
    Delivery is one of the most important events in a woman's life, and pain is an inevitable part of childbirth. The non-controlled pain leads to prolonged labor and multiple injuries to mother and fetus. This research was done to determine the effect of acupressure at the SP6 and LI4 points on the duration and pain of the labor in nulliparous women.
    Methods
    This randomized controlled clinical trial was conducted on 84 nulliparous women who were attended hospital in Ardabil. The participants were divided into two intervention and control groups using randomized blocking. Pressure has been done on SP6- LI4 points in intervention group. Pain intensity was measured before and after the intervention using visual analog scales and length of active phase and second stage were recorded through vaginal exam in the checklist. Determination of the effect of pressure on the intensity of labor pain and delivery length performed using paired t-test, Student’s t-test and chi-square by SPSS v.16.
    Results
    After the intervention the mean level of pain intensity in the intervention group was decreased. Also, there was a significant decrease in mean intensity of pain after each intervention in the intervention group with different dilations (4, 6, 8, and 10 cm) (p<0.001).The mean duration of active phase and second stage in intervention group was significantly less than the control group. (p<0.001, p=0.008).
    Conclusion
    Since acupressure at the SP6- LI4 point in different dilatations leads to the reduction in labor pain intensity and delivery duration, therefore, this method can be used to reduce the pain and duration of labor.

    Background and Objectives
    Chronic renal failure (CRF), also known as chronic kidney disease, caused by devastated nephron mass of the kidney results in uremia. Hypertension, diabetes mellitus and glomerulonephritis are common etiologic factors of CRF. This condition causes miscellaneous oral manifestations especially in diabetic patients. The aim of this study was to comparison oral manifestations of diabetic and non-diabetic uremic patients undergoing hemodialysis.
    Methods
    A total of 95 patients who undergoing hemodialysis in Razi hospital in Rasht city participated in this descriptive analytical study. Patients were divided into two diabetic and non- diabetic groups. Oral cavity examinations were done by latex gloves and single use mirror. Objective and subjective oral manifestations such as xerostomia, bad taste, mucosal pain, uremic odor, coating tongue, petechial, purpura, pale oral mucosa, ulcer, dental erosion and candida infection were recorded in questionnaire. After gathering of information, the data were analyzed by SPSS 15 software using t-test and chi square statistical test.
    Results
    About 60% of patients (57 person) were men and 40 % (38 person) were women. The mean age of patients was 48 years (range of 20 -76 years). Common subjective oral manifestation in both groups was xerostomia and most common objective oral manifestations were pale oral mucosa, uremic odor and coating tongue respectively. The DMFT index in diabetic group was significantly higher (17.3±7.63) than non-diabetic patients (12.4±8.26). There was no significant statistical correlation between the time of dialysis, number of dialysis appointment during the week and objective and subjective oral manifestations in two groups.
    Conclusion
    Although, the present study has shown an increase in oral manifestations in diabetic patients undergoing hemodialysis relative to non-diabetic group, but this increase was not statistically significant. On the other hand results of our study have indicated significantly higher DMFT in diabetic group in comparison with the non-diabetic group. So patients with diabetes mellitus undergoing hemodialysis need to more oral and dental health care.

    Background and Objectives
    Obstructive sleep apnea is a preventable and prevalent major health hazard with serious health consequences including excessive daytime sleepiness, cognitive disturbances, depression, cardiovascular diseases and hypertension. Obstructive sleep apnea is a disorder affecting 2 to 4% of the adult population. The continuous positive airway pressur e (CPAP) i s the most efficacious therapy and is often the first option for these patients. The pressure titration during laboratory polysomnography is required for treatment by CPAP.
    Methods
    The patients with obstructive sleep apnea requiring continuous positive airway pressure treatment were selected. CPAP titration was done according to American Academy of Sleep Medicine protocol. Comparison among continuous positive airway pressure with polysomnographic parameters was performed and analyzed with Pearson correlation coefficient. For analysis of qualitative parameters, we used chi-square and then checked with SPSS version 18 software.
    Results
    From 125 patients with obstructive sleep apnea, there were 112 cases with inclusion criteria. Mean age of participants was 55.07 ± 12, male frequency was 59.2%, apnea hypopnea index was 43.62 and mean continuous positive airway pressure was 12.50. There was significant relationship among the pressure of continuous positive airway pressure with apnea hypopnea index (P=0.028), arousal index (P=0.011), body mass index (P=0.041) and O2 desaturation index (P=0.022), although age was not significantly related.
    Conclusion
    In accordance to this data, we found out a prediction equation for optimal CPAP in our patients

    Background and Objectives
    Epidemiological and experimental evidences suggest that oxidative stress is elevated in children with Down syndrome (DS). Some studies show that consumption of diets with a high content of fruits and vegetables results in a significant reduction in markers of oxidative stress. We investigated the frequency of fruits and vegetables intake in children with DS and the influence of variations in the level of this consumption on biomarkers of oxidative stress.
    Methods
    Frequency of consumption of fruit and vegetables by children with DS were recorded by interview with the children’s parents. Serum malondialdehyde (MDA) and urinary 8-hydroxy-2'-deoxyguanosine (8-OHdG) were also measured as biomarkers of oxidative stress. Data analysed using ANOVA and Student’s t-tests.
    Results
    Mean consumption of fruits and vegetables was 4.6 and 0.74 servings per week, respectively. No significant relationships were found between consumption of fruits and vegetables with markers of oxidative stress. Based on our findings, the 65.9 % of the children did not eat any vegetables and the rest of the children consumed less than one serving per day. There was a significant correlation between the frequency of fruits and vegetable consumption (r=0.212; p <0.04).
    Conclusion
    There wasn’t any significant relationship between fruits and vegetables consumption and indicators of oxidative stress in children with DS. This lack of correlation could be due to low consumption of fruits and vegetables in children with DS.

    Background and Objectives
    Diabetes is a chronic disease characterized by derangement in protein, fat and carbohydrate metabolism. Most of hypoglycemic agent used in medicine for diabetes treatment are reported to have side effects for long run. On the other hand, increase in oxidative stress is effective in the genesis of diabetes. Therefore, the purpose of this study was to assess the effect of alcoholic extract of Euphorbia cyparissias on serum glucose and antioxidant enzymes level in diabetic male rats.
    Methods
    In this study, 40 adult male Wistar rats weighting 250±20 grams divided into four groups randomly: control, diabetic animals, treatment 1 and treatment 2 which received 500 and 250 mg/kg of E.cyparissias extract for 21 days by gastric gavage, respectively. At the end of the treatment, level of antioxidant enzymes including TAC, MDA, SOD and GPX and also blood glucose were determined in animal's serum.
    Results
    The blood glucose levels were significantly lowered in the group of treatment 1 compared to diabetic group (p<0.05). Results showed that MDA level of serum was increased significantly in diabetic group in comparison with control group (p<0.05). Furthermore, the TAC, SOD and GPX level were increased significantly in the group of treatment 1 compared to diabetic group (p<0.05).
    Conclusion
    This study showed that oral administration of E.cyparissias extract has antidiabetic and antioxidant activity in streptozotocin induced diabetic rats.

    Background and Objectives
    Eisenmenger syndrome associated with pregnancy increases the maternal morbidity and mortality. Therefore, in the most of such patients the termination of pregnancy is recommended. Case report: A 29 -year-old woman at the 7 th week of gestational age was candidate for therapeutic abortion with history of Eisenmenger syndrome due to atrial septal defect (ASD). Spinal a nesthesia was performed in sitting position at the L4-5 interspace with 10 mg of bupivacaine. The p atient was laid in the supine position after 2 minutes. The surgery lasted about an hour. Blood pressure and heart rate did not change significantly during operation.
    Conclusion
    Modified s pinal anesthesia with bupivacaine is recommended for therapeutic abortion in patients with Eisenmenger syndrome.
