صفحات 7-19زمینه و هدففاکتور هستهای کاپای B (NF-κB)، به عنوان یک عامل رونویسی اصلی، در پیشرفت و توسعه التهاب عروق ریز ناشی از هیپرگلیسمی نقش مهمی ایفا میکند. هیپرگلیسمی باعث فعال شدن NF-κB و به دنبال آن افزایش سیتوکین های پیش برنده التهاب همچون TNF-α، IL-6 و IL-1β و گسترش التهاب می شود. مطالعات جدید نقش microRNA-146a (miR-146a) را در پاتوژنز دیابت، از طریق یک حلقه فیدبک منفی وابسته به NF-κB نشان داده اند. اگرچه مطالعات بی شماری بیانگر تغییر غلظت پلاسمایی miR-146a طی هایپرگلایسمی بوده اند، اما هنوز سلول های منشاء این تغییر مشخص نیستند. بدین منظور این مطالعه تجربی- مداخلهای برای ارزیابی نقش NF-κB بر میزان بیان ژن miR-146a در سلولهای اندوتلیال عروقی بندناف انسان (HUVECs) در شرایط هیپرگلیسمی طراحی شد.روش کارسلولهای HUVEC در محیط رشد سلول اندوتلیال در شرایط گلوکز طبیعی (5 میلی مول در لیتر) و شرایط هیپرگلیسمی (25 میلی مول در لیتر) در پلیت 6 چاهکی به مدت 24 ساعت کشت داده شدند. اضافهشدن 30 میکرومول در لیتر JSH-23 به عنوان بازدارنده انتقال NF-κB به هسته، 30 دقیقه قبل از ایجاد هیپرگلیسمی صورت گرفت. جهت اندازه گیری میزان بیان miR-146a و NF-κB روش quantitative Real Time PCR استفاده شد. میزان فعالیت NF-κB با روش ELISA سنجیده شد.یافته هاهیپرگلیسمی میزان فعالیت و میزان بیان mRNAی NF-κB را در سلولهای HUVEC به طور معنی داری افزایش داد. همچنین میزان بیان miR-146a در گروه هیپرگلیسمی در مقایسه با گروه کنترل، به طور محسوسی افزایش یافت. از طرف دیگر، JSH-23 باعث جلوگیری از افزایش miR-146a و نیز افزایش بیان mRNAی NF-κB در سلول های هیپرگلیسمی گردید.نتیجه گیریاین یافته نشان میدهد که NF-κB بیان ژن miR-146a را در فاز اولیه هیپرگلیسمی در سلولهای HUVEC افزایش میدهد.کلیدواژگان: فاکتور هسته ای کاپای B، هیپرگلیسمی، سلول های اندوتلیال عروقی بند ناف انسان، microRNA-146a

صفحات 20-29زمینه و هدفبا توجه به شیوع روزافزون NAFLD، تمرکز بر استراتژیهای مختلف برای پیشگیری و مدیریت آن ضروری است. هدف این پژوهش بررسی تاثیر دویدن روی تردمیل و رژیم غذایی کمکالری بر مقادیر ویسفاتین و hs-CRP در افراد مبتلا به NAFLD بود.روش کاردر این مطالعه نیمهتجربی 55 فرد مبتلا به NAFLD انتخاب شدند و به طور تصادفی در یکی از چهار گروه تمرین (14 نفر)، رژیم غذایی (14 نفر)، تمرین و رژیم غذایی (14 نفر) و کنترل (13 نفر) قرار گرفتند. برنامه تمرینی شامل هشت هفته تمرین با شدت 75-55 درصد حداکثر ضربان قلب، 45 دقیقه و سه روز در هفته بود. رژیم غذایی با کسر 500 کیلوکالری از ثبت غذایی سه روزه بیماران طراحی شد. آزمودنیهای گروه ترکیبی هر دو نوع مداخله را به کار بردند. دادهها با استفاده از آزمون تحلیل کوواریانس و آزمون تعقیبی بونفرونی در سطح معنیداری 0/05>p تجزیه و تحلیل شدند.یافته هامقادیر ویسفاتین در گروه تمرین (0/01= p)، در گروه ترکیبی (0/001=p ) و مقادیر hs-CRP در گروه تمرین (0/003= p) و گروه ترکیبی (0/008p=) کاهش معناداری را نشان داد. شاخص توده بدنی آزمودنیها نیز بر اثر تمرین (0/003p=) و مداخله ترکیبی (0/001p=) کاهش معناداری یافت.نتیجه گیریدویدن روی تردمیل و رژیم غذایی کمکالری بصورت همزمان باعث کاهش بیشتر مقادیر پلاسمایی ویسفاتین و hs-CRP در بیماران مرد مبتلا به NAFLD میشود. بنابراین برای کاهش عوامل التهابی وابسته به NAFLD مداخله درمانی مفیدی هستندکلیدواژگان: فعالیت ورزشی، محدودیت کالری، التهاب، بیماری کبد چرب غیرالکلی

صفحات 30-40زمینه و هدفدر سالهای اخیر نشان داده شده است که اسینتو باکتر بومانی یکی از عوامل ایجادکننده عفونت بیمارستانی شامل پنومونی، باکترمی، عفونتهای دستگاه ادراری، عفونتهای زخم و مننژیت میباشد. این میکروارگانیسم میتواند در محیط بیمارستان زنده بماند و به سرعت به بسیاری از آنتی بیوتیکها مقاوم می شود. ژنوتایپ مولکولی می تواند اطلاعات ما را در مورد گسترش سویه های اسینتو باکتر بومانی از یک بیمارستان به بیمارستان دیگر و همچنین مقاومت دارویی آنها افزایش دهد. هدف از این مطالعه تعیین شیوع مقاومت آنتی بیوتیکی و نیز روابط فیلوژنیک سویههای اسینتوباکتر بومانی در بیمارستانهای آموزشی شهرکرد بود.روش کاردر این مطالعه، میزان مقاومت آنتی بیوتیکی سویه های اسینتوباکتر بومانی جدا شده از نمونه های بالینی مختلف )ادرار، خون و خلط( نسبت به سیپروفلوکساسین، اوفلوکساسین، نوروفلوکساسین، سفتازیدیم، جنتامیسین، آمیکاسین، ایمی پنم، مروپنم، سفی پیم، پایپریسیلین- تازوباکتام، آمپی سیلین- سولباکتام، توبرامایسین و آزترونام با روش انتشار دیسک (Kirby-Bauer) اندازه گیری شد. در نهایت ژنوتیپ سویه های اسینتوباکتر بومانی با استفاده از روش REP-PCR بررسی شد.یافته هادر این مطالعه، 50 نمونه از بیماران به عنوان اسینتوباکتر بومانی شناسایی شدند که میزان مقاومت به دارو در آنها مورد ارزیابی قرار گرفت. تمام سویه ها به سفتازیدیم و سفپیم مقاوم بودند و همچنین میزان بالای مقاومت به آزترونام، نورفلوکساسین، سیپروفلوکساسین، آمیکاسین، ایمی پنم، جنتامایسین و آمپی سیلین- سولباکتام مشاهده شد. از سوی دیگر، نتایج مطالعه حاضر نه گروه ژنوتیپ در میان گونههای اسینتوباکتر بومانی بر اساس روش REP-PCR را نشان داد.نتیجه گیریبا توجه به شیوع بالای مقاومت آنتی بیوتیکی در سویه های جدا شده اسینتو باکتر بومانی و همچنین شباهت ژنوتیپ های مختلف و پراکنش این ژنوتیپ ها در بخش های مختلف بیمارستان های شهرکرد، اجرای برنامه های کنترل عفونت در بخش های مختلف بیمارستان لازم است.کلیدواژگان: اسینتوباکتر بومانی، مقاومت آنتی بیوتیکی، ژنوتایپینگ، REP-PCR

صفحات 41-52زمینه و هدفبیماری انسدادی مزمن ریه (COPD) یک بیماری است که با انسداد غیر قابل برگشت راه هوایی و التهاب سیستمیک مشخص میشود و با عوارض ریوی و خارج ریوی همراه است. پرسشنامه تشخیصی بیماری انسدادی مزمن ریه (CDQ) پرسشنامه ای اعتبار بخشی شده میباشد که جهت شناسایی بیماران مبتلا به COPD کاربرد دارد. هدف این مطالعه بررسی رابطه بین شدت انسداد راه هوایی و امتیاز CDQ در بیماران COPD بود.روش کاردر این مطالعه 100 بیمار مبتلا به COPD با سابقه مصرف سیگار و بر اساس معیارهای انجمن توراکس آمریکا (ATS) شامل بیماران با سرفه، دفع خلط و تنگی نفس مزمن همراه با یافته های اسپیرومتری به صورت انسداد غیر قابل برگشت 70% FEV1/FVC< وارد مطالعه شدند. شدت بیماری بر اساس معیارهای GOLD طبقه بندی گردید. پس از اخذ رضایت، اطلاعات دموگرافیک بیماران، و متغیرهای مورد مطالعه شامل سابقه مصرف سیگار، تعداد دفعات تشدید بیماری در طی یکسال گذشته، میزان اشباع خون شریانی (Spo2) و داده های اسپیرومتری ثبت گردید. سپس پرسشنامه CDQ و شدت تنگی نفس بر اساس معیار MMRC تکمیل گردید. همچنین رابطه بین امتیاز CDQ با مقادیر FEV1% و سایر متغیرهای بالینی بیماران مورد بررسی قرار گرفت.یافته هاهمه بیماران مورد مطالعه مرد بودند. بین امتیاز پرسشنامه CDQ با میزان FEV1% (0/001<p)، GOLD stage (0/001<p) ، میزان اشباع خون شریانی (Spo2) ( 0/001<p)، تعداد حملات تشدید بیماری در طی یکسال (0/001<p)، و شدت تنگی نفس بر اساس MMRC (0/001<p ) ارتباط معنی داری وجود داشت.نتیجه گیریبراساس یافته های این مطالعه از پرسشنامه CDQ میتوان جهت ارزیابی محدودیت جریان راه هوایی، شدت تنگی نفس و تعداد حملات تشدید بیماری COPD استفاده نمود. لذا توصیه میشود که این پرسشنامه در مراکز بهداشتی- درمانی اولیه جهت شناسایی و ارجاع بیماران COPD استفاده گردد.کلیدواژگان: بیماری انسدادی مزمن ریه، اسپیرومتری، پرسشنامه CDQ، محدودیت جریان راه هوایی

صفحات 53-60زمینه و هدفبیماری انسدادی ریوی مزمن (COPD) با التهاب سیستمیک بروز می کند و با محدودیت غیرقابل برگشت جریان هوا همراه است. لپتین سایتوکاینی با اثر پیش التهابی هست. در مورد تاثیر مکمل اسید لینولئیک مزدوج (CLA) بر غلظت سرمی لپتین در بیماران COPD مطالعه ای صورت نگرفته است، این بررسی با هدف تاثیر CLA بر سطح لپتین سرم و عملکرد ریه و کیفیت زندگی در بیماران COPD انجام شده است.روش کارمطالعه به روش مداخله ای و بر روی 90 بیمار COPD صورت گرفت. بیماران به صورت تصادفی به دو گروه 45 نفر (CLA و دارونما) تقسیم شدند. پس از اخذ رضایت نامه کتبی از بیماران و ثبت فاکتورهای دموگرافیک، اسپیرومتری انجام و امتیاز COPD Assessment Test برای بیماران محاسبه شده و نمونه خون ناشتا جهت آزمایش لپتین سرم اخذ گردید. سپس مکمل CLA به میزان 3/2 گرم روزانه به مدت 6 هفته برای گروه CLA و پلاسبو برای گروه شاهد تجویز شد. بعد از 6 هفته اسپیرومتری، امتیاز CAT و نمونه گیری برای هر دو گروه تکرار و نتایج حاصله، مورد تجزیه و تحلیل قرار گرفت.یافته هادر گروه CLA، کاهش معنی داری در سطح سرمی لپتین و امتیاز (0/05CAT (p< به عنوان شاخص کیفیت زندگی و افزایش معنی داری در مقادیر (0/05 FEV1 (p<به عنوان تست عملکرد ریه بعد از مداخله مشاهده گردید در حالیکه این تفاوت بین دو گروه معنی دار نبود.نتیجه گیریمصرف مکمل CLA ممکن است از طریق تعدیل سطوح سرمی لپتین باعث ارتقاء کیفیت زندگی بیماران COPD گردد.کلیدواژگان: اسید لینولئیک مزدوج، لپتین، COPD، CAT، عملکرد ریه، کیفیت زندگی

صفحات 61-70زمینه و هدفداروهای خط اول درمان لیشمانیوز ترکیبات پنج ظرفیتی آنتیموان هستند که دارای عوارض جانبی متعدد از جمله سمیت دارویی هستند. از طرفی مقاومت به این داروها در سراسر جهان رو به افزایش میباشد. داروهایی مانند آمفوتریسین ب و پنتامیدین که در خط دوم درمان قرار میگیرند، نیز دارای عوارض جانبی بوده یا هزینه بالایی را به بیمار تحمیل میکنند. با توجه به اثرات سمی پاراکوات، این مطالعه با هدف تاثیر آن روی زنده مانی پروماستیگوتهای لیشمانیا ماژور و سلولهای HUVEC انجام گرفت.روش کارپروماستیگوتهای انگل لیشمانیا به تعداد 106×2/5 در هر چاهک از پلیتهای 96 خانه ای تحت مواجهه با غلظتهای مختلف پاراکوات قرار گرفتند. تیمار به مدت 48 ساعت و در دمای C° 24 انجام شد. سپس آزمایش MTT برای ارزیابی میزان تاثیر پاراکوات انجام گردید. جذب نوری با استفاده از دستگاه خوانشگر پلیت در طول موج 570 نانومتر قرائت گردید. از رنگ آمیزی تریپان بلو جهت بررسی تعداد پروماستیگوتهای زنده پس از تیمار با پاراکوات استفاده شد. همچنین، تاثیر غلظت های مختلف پاراکوات بر بقا سلول های HUVEC در شرایط کشت بررسی گردید.یافته هانتایج تست MTT نشان داده که غلظتهای افزایشی پاراکوات به صورت معنیداری میتواند بقا انگلهای پروماستیگوت را در مقایسه با گروه کنترل تحت تاثیر قرار دهد. در این مطالعه، غلظت IC50 پاراکوات بر پروماستگوتهای انگل لیشمانیا به میزان 272/46 میکروگرم بر میلی لیتر براورد گردید. یافته های رنگ آمیزی تریپان بلو با نتایج روش MTT مطابقت داشت. علاوه بر این، ما متوجه شدیم که HUVEC ها حساس به پاراکوات هستند (IC50 = 188.99 μg/ml).نتیجه گیریپاراکوات اثرات مهاری قوی روی پروماستیگوتهای لیشمانیا ماژور و سلول های اندوتلیال انسانی دارد. آزمایشات بیشتر در زمینه تاثیر پاراکوات روی زخمهای حاصل از لیشمانیا ماژور در مدلهای حیوانی و اثرات جانبی آن در استفاده موضعی این دارو لازم میباشد.کلیدواژگان: لیشمانیا ماژور، پاراکوات، روش MTT

صفحات 71-79زمینه و هدفبیماری های مزمن، از جمله مهم ترین عوامل مرگ و میر محسوب می شوند. هدف پژوهش حاضر بررسی رابطه کمال گرایی و استرس با رضایت از زندگی در بیماران قلبی- عروقی شهر اردبیل می باشد.روش کارروش پژوهش توصیفی از نوع همبستگی بود. جامعه آماری پژوهش عبارت بودند از بیماران قلبی- عروقی شهر اردبیل در نیمه اول سال1396 که برای دریافت خدمات مراجعه کرده بودند. نمونه پژوهش شامل 60 بیمار قلبی- عروقی بود که با روش نمونه گیری تصادفی ساده انتخاب شدند. برای جمع آوری داده ها از پرسشنامه های مقیاس کمال گرایی، مقیاس استرس و مقیاس رضایت از زندگی استفاده شد. داده ها با استفاده از آمار توصیفی و رگرسیون چندمتغیره توسط نرم افزار spss-19 تحلیل گردید.یافته هانتایج نشان داد که بین کمال گرایی و رضایت از زندگی رابطه معنی دار منفی وجود دارد. همچنین بین استرس و رضایت از زندگی رابطه معنی دار منفی بدست آمد (0/01>(p. به عبارت دیگر، با افزایش استرس، رضایت از زندگی کاهش می یابد. بین مولفه های کمال گرایی و استرس با رضایت از زندگی رابطه چندگانه وجود داشت (0/01>(p.نتیجه گیرینتایج این مطالعه نشان داد کاهش کمال گرایی و استرس افراد به بالا بردن رضایت افراد از زندگی کمک می کند و همچنین در فرایند درمان موثر است.کلیدواژگان: کمال گرایی، استرس، رضایت از زندگی، بیماران قلبی- عروقی

صفحات 80-87زمینه و هدفکلامیدیا تراکوماتیس یک باکتری گرم منفی است و عفونت کلامیدیایی یکی از عفونتهای شایع منتقله از راه تماس جنسی است که قابل درمان میباشد. با توجه به نقش اساسی کلامیدیا در زمینه ناباروری، مطالعه فراوانی موارد بدون علامت بیماری ارزشمند خواهد بود. هدف از این مطالعه، تعیین فراوانی کلامیدیا تراکوماتیس در نمونه اندوسرویکس زنان نابارور به روش PCR میباشد.روش کاراین مطالعه از نوع توصیفی- تحلیلی، مقطعی بود. 135 خانم 40-20 ساله مراجعه کننده با شکایت نازایی به بیمارستان الزهرا (س) و مطب های خصوصی رشت به طور تصادفی انتخاب شدند. نمونه اندوسرویکس با استفاده از سواپ استریل تهیه و در نمونه PBS به آزمایشگاه منتقل شد. سپس PCR روی نمونه ها انجام شد. جواب حاصل از PCR همراه با اطلاعات موجود در چک لیست ها با استفاده از نرم افزار SPSS-16مورد آنالیز آماری قرار گرفت.یافته هادر 19/3درصد زنان نازا PCR از نظر کلامیدیا مثبت بود. ارتباط معنی داری بین وضعیت ابتلا به کلامیدیا با سن، نوع باروری، انسداد در سالپنگوگرافی، سابقه نازایی در خویشاوندان و مدت نازائی وجود نداشت.نتیجه گیریبا توجه به فراوانی به دست آمده میتوان گفت عفونت کلامیدیایی، عفونتی شایع در جامعه مورد مطالعه بود. به منظور کاهش عوارض بیماری در جامعه، غربالگری کلامیدیا میتواند به عنوان بخشی از برنامه های بهداشتی کشور باشد.کلیدواژگان: کلامیدیا تراکوماتیس، ناباروری زنان، PCR

صفحات 88-99زمینه و هدفمطالعه حاضر با هدف مقایسه بین نیازهای اساسی روان شناختی و سطوح نظریه ذهن در مادران کودکان دارای اختلال نافرمانی مقابله ای و اختلال بیش فعالی/ نقص توجه به روش مقطعی (علی- مقایسه ای) انجام شد.روش کاردر این پژوهش 70 نفر از مادران کودکان دارای اختلال نافرمانی مقابله ای و اختلال بیش فعالی/ نقص توجه که در سال 1396به درمانگاه روانپزشکی بیمارستان فاطمی اردبیل مراجعه کرده بودند به شیوه در دسترس انتخاب شدند. ابزارهای مورد استفاده در این پژوهش عبارت بودند از: پرسشنامه تئوری ذهن، نیازهای اساسی روانشناختی و پرسشنامه علائم مرضی کودکان (فرم والدین CSI-4). داده های جمع آوری شده وارد نرم افزارSPSS-21 شده و جهت تحلیل داده ها نیز از روش تحلیل واریانس چند متغیری (MANOVA) استفاده شد.یافته هایافته های تحلیل واریانس نشان داد بین نظریه ذهن (0/697F=، 0/407>p)، نیازهای اساسی روانشناختی از جمله خودمختاری (0/283F=، 0/597>p)، شایستگی (0/393F=، 0/336>p) و ارتباط (0/760F=، 0/387>p) در بین دو گروه مادران کودکان دارای اختلال نافرمانی مقابله ای و اختلال بیش فعالی/ نقص توجه تفاوت معناداری وجود ندارد.نتیجه گیریاین نتایج اهمیت توجه به سطوح نظریه ذهن و نیازهای روانشناختی مادران این نوع کودکان را در بهبود وضعیت تحصیلی، شغلی و ارتباط اجتماعی و رشد شناختی و ارزیابی هیجانی کودک برجسته می سازد.کلیدواژگان: نیازهای اساسی روانشناختی، سطوح نظریه ذهن، اختلال نافرمانی مقابله ای (ODD) و اختلال بیش فعالی، نقص توجه (ADHD)

صفحات 100-109زمینه و هدفامروزه مقاومت به آنتی بیوتیکها و افزایش هزینه های درمانی از مهمترین موضوعات جهانی نظام سلامت در ارتباط با عفونت های پیچیده باکتریایی میباشند. ایمی پنم یک آنتی بیوتیک بتالاکتام از دسته کارباپنم ها میباشد که به علت طیف اثر وسیع به فراوانی در بخشهای بیمارستانی کشور ما مورد استفاده قرار میگیرد. بررسی الگوی مصرف دارو یک برنامه رسمی و سازمان یافته است که به جمع بندی اطلاعات به منظور شناخت و بهبود عوارض دارویی و کاهش هزینه های درمانی میپردازد. هدف این مطالعه بررسی الگوی تجویز و مصرف ایمی پنم در بیمارستان آموزشی امام خمینی اردبیل طی پاییز سال 1397 بود.روش کاراین مطالعه توصیفی- مقطعی و آینده نگر بوده و طی آن اطلاعات دموگرافیک، دوز دارو، تعدیل دوز در نارسایی کلیوی و سایر داروهای تجویز شده از پرونده 110 بیمار بستری تحت درمان با ایمی پنم استخراج شده و بر اساس منابع مورد بررسی قرار گرفت.یافته ها64 درصد بیماران در اولین روز بستری و 75/5درصد بیماران به صورت تجربی ایمی پنم دریافت کرده بودند درحالیکه تست کشت میکروبی فقط برای 24/5 درصد بیماران درخواست شده بود. سنجش کراتینین برای اکثر بیماران انجام شده بود در حالی که تعدیل دوز دارو برای 25/5 درصد بیماران با نجویز منطقی صورت گرفته بود.نتیجه گیریتجویز ایمی پنم به صورت تجربی و بلافاصله در زمان شروع بستری از نتایج مهم این تحقیق بود. توجه به نوع عفونت و حساسیت به آنتی بیوتیک بر اساس منابع پزشکی در هنگام تجویز ایمی پنم در بیمارستان اکیدا توصیه میگردد.کلیدواژگان: ایمی پنم، ارزیابی الگوی مصرف دارو، مقاومت دارویی میکروبی

صفحات 110-116زمینه و هدفژن MEFV نقش برجسته ای در بیماری خود التهابیFMF دارد. این بیماری بیشتر در ساکنین مناطق مدیترانه ای دیده میشود. با در نظر گرفتن نقش این ژن در بسیاری از بیماری های روماتیسمی و حتی غیر روماتیسمی، شناسایی واریاسیونهای مختلف این موتاسیون در جمعیت نرمال این مناطق هدف این مطالعه بود.روش کارتعداد 224 نفر از افراد فاقد بیماری FMF وارد مطالعه شدند. نمونه خون این افراد از نظر 12 واریاسیون شایع ژن MEFV غربالگری گردید. شرکت کنندگان پرسشنامه ای شامل داده های مورد نیاز را تکمیل نموده و از نظر علائم و یافته های بالینی بیماری در خود و بستگان درجه یک مورد ارزیابی قرار گرفتند. جهت تحلیل آماری از آزمونهای کای دو و تی مستقل در نرم افزار SPSS-24 استفاده گردید.یافته هااز 224 موردی که وارد مطالعه شدند، 113 مورد (50/4%) مرد و 111 مورد (49/6%) زن بودند. افراد بررسیشده 57 مورد (25%) جهش داشتند که 28 مورد مرد (49%) و 29 مورد زن (51%) بودند. بیشترین موتاسیون یافتشده به ترتیب E148Q با 41 مورد (18/3%)، سپس P369S با 7 مورد (3/1%) و V726A 5 مورد (2/2%) بود. از جهش A744S 3 مورد (1/3%) و از F479L و M694V و R761H هر کدام 2 مورد (0/8%) همچنین از جهش K695R تنها یک مورد (4/0%) دیده شد و نهایتا جهشهای M680I(G/C)،M680I(G/A)، I692del، M694I دیده نشد. جهش های هتروزیگوت ترکیبی E148Q/P369S, E148Q/V726A, E148Q/P369S, P369S / F479L در جمعیت نرمال بدون هیچ ارتباطی با بیماری FMD مشاهده شد.نتیجه گیری25 درصد از جمعیت نرمال شمالغرب ایران واریاسیونهای ژن MEFV بصورت هتروزیگوت دارند که شایعترین آن موتاسیون E148Q میباشد که میتواند یک واریانت نرمال جامعه در فقدان علایم بالینی در نظر گرفته شود. همچنین جهشهای M680I(G/C)،M680I(G/A)، I692del و M694I در جمعیت نرمال دیده نشدند، لذا در صورت مشاهده حتی به صورت هتروزیگوت میتوانند یک واریاسیون بیمارگونه تلقی شوند.کلیدواژگان: موتاسیون MEFV، ژن FMF، شمالغرب ایران، اردبیل

Pages 7-19Background & objectivesNuclear Factor kappa B (NF-κB), a master switch transcription factor, plays a critical role in the progression and development of hyperglycemia-induced microangiopathy. Hyperglycemia activates NF-κB, and subsequently increases pro-inflammatory cytokines such as TNF-α, IL-6 and IL-1β leading to development of inflammation. Some new studies have revealed the involvement of microRNA-146a (miR-146a) in the pathogenesis of diabetic complications through an NF-κB-dependent negative feedback loop manner. Despite numerous reports indicating changes of plasma miR-146a during hyperglycemia, the origin of this change remains unclear. This study was designed to evaluate the role of NF-κB on the miR-146a gene expression level in human umbilical vein endothelial cells (HUVECs) during a hyperglycemic condition.MethodsHUVECs were cultured in normal glucose (5 mmol/L), and hyperglycemic (25 mmol/L) endothelial cell growth medium in the six well plates for 24 h. JSH-23 (30 μmol/L), as an inhibitor of NF-κB translocation to the nucleus, was added to the culture medium, 30 min before induction of hyperglycemia. Quantitative Real Time PCR was performed to measure the expression levels of miR-146a and mRNA NF-κB. NF-κB activity was measured by Elisa.ResultsHyperglycemia markedly increased the NF-κB activity and mRNA level in HUVECs. The expression of miR-146a significantly increased in hyperglycemic group compared to the normoglycemic group. On the other hand, JSH-23 prevented from miR-146a increment in hyperglycemic group and also it increased the mRNA expression level of NF-κB in this group.ConclusionThis result shows that NF-κB increases the gene expression of miRNA-146a in the early phase of hyperglycemia in HUVECs.Keywords: Nuclear Factor- κB, Hyperglycemia, HUVECs, miR-146a

Pages 20-29Background & objectiveswith Regard to the increasing prevalence of nonalcoholic fatty liver disease (NAFLD), focusing on various strategies for its prevention and management seem necessary. The purpose of this study was to investigate the effect of running on treadmill and low calorie diet on visfatin and hs-CRP values in people with NAFLD.MethodsIn this quasi-experimental Study, 55 NAFLD patients were selected and randomly assigned to one of four groups: training (n=14), diet (n=14), training plus diet (n=14) and control (n=13). The training program included eight weeks running on a treadmill with maximum heart rate intensity of 55-75% for 45 minutes three times a week. A low calorie diet with an energy deficit of 500 calories of daily energy intake was calculated from 3-day food records of the patients. Combined group subjects also used both types of intervention. Data were analyzed using covariance analysis and Bonferron's post hoc test at a significance level of p<0.05.ResultsVisfatin values in the training group (p=0.01), in the combined group (p=0.001) and also hs-CRP values in the training group (p=0.003) and the combined group (p=0.008) decreased significantly. The body mass index of subjects was significantly decreased by training (p=0.003) and combined intervention (p=0.001).ConclusionRunning on treadmill and a low calorie diet simultaneously reduce the plasma levels of visfatin and hs-CRP in male patients with NAFLD. Pages 30-40Background & objectivesIn recent years, Acinetobacter baumannii has been shown to be associated with several nosocomial infections, including pneumonia, bacteraemia, urinary tract infections, wound infection and meningitis. This organism can survive in the hospital environment and rapidly develops resistance to many antibiotics. The molecular genotyping can increase our knowledge about the spread of A. baumannii strains from one hospital to another and their drug resistance. Therefore, this study aimed to determine the prevalence of antibiotic resistance profile as well as phylogenetic relationships of A. baumannii strains in Shahrekord teaching hospitals.MethodsIn this study, antibacterial susceptibility patterns of A. baumannii strains isolated from different clinical specimens (urine, blood, sputum) to amikacin, ampicillin/sulbactam, aztreonam, cefepime, ceftazidime, ciprofloxacin, gentamycin, imipenem, meropenem, norfloxacin, ofloxacin, piperacillin/tazobactam, tobramycin were tested using disk diffusion )Kirby-Bauer( method. Finally, genotyping of A. baumannii strains was performed using REP-PCR method.ResultsDuring this study, 50 samples of patients were identified as A. baumannii) 71%(, and their drug resistance rates were assessed. All A. baumannii strains were resistant to ceftazidime and cefepime and also a high rate of resistance to aztreonam, norfloxacin, ciprofloxacin, amikacin, imipenem, gentamycin, and ampicillin-sulbactam were observed. On the other hand, our results demonstrated nine genotype groups among A. baumannii strains based on REP-PCR method.ConclusionDue to the high prevalence of antibiotic resistance among isolated A. baumannii strains, similarities between different genotypes and the dispersion of these genotypes in different parts of Shahrekord hospitals, the implementation of infection control programs in different parts of the hospital is necessary.Keywords: Acinetobacter baumannii, Antibiotic Resistance, Genotyping, REP-PCR

Pages 41-52Background & objectivesChronic obstructive pulmonary disease (COPD) is characterized by irreversible airflow limitation and systemic inflammation which is also associated with pulmonary and extra-pulmonary complications. COPD Diagnostic Questionnaire (CDQ) is a valid questionnaire that is used to identify patients with COPD. The purpose of this study was to determine the relationship between the severity of airflow limitation and the CDQ score in COPD patients.MethodsIn this study, 100 patients with COPD and smoking history were included. The diagnosis of COPD was established based on American Thoracic Society (ATS) guidelines: cough and expulsion of phlegm, chronic dyspnea, obstructive spirometric pattern (forced expiratory volume in one second (FEV1)/forced vital capacity (FVC) <70%). Disease severity was classified according to GOLD criteria. After obtaining consent, demographic data, history of smoking, the exacerbation rate in the past year, arterial oxygen saturation (Spo2) and spirometer variables were recorded. Then, CDQ and Modified Medical Research Council (MMRC) dyspnea score were completed. Also, the associations of the CDQ score with FEV1% predicted and clinical parameters of patients were tested.ResultsAll studied patients were male. There was a significant relationship between CDQ score with FEV1% (p<0.001), GOLD stage (p<0.01), Spo2 (p<0.01), exacerbation rate (p<0.01) and severity of dyspnea according to MMRC dyspnea scale (p<0.01).ConclusionThe CDQ can be used to determine airflow limitation, severity of dyspnea and exacerbation rate in COPD patients. Therefore, it is recommended that this questionnaire can be used in primary health centers for identifying and referring of COPD patients.Keywords: COPD, PFT, CDQ, Airflow Limitation

Pages 53-60Background & objectivesChronic Obstructive Pulmonary Disease (COPD) is accompanied by systemic inflammations and is characterized by irreversible airflow limitations. Leptin is a cytokine with pre-inflammatory effect. However, there have been no studies on the effect of conjugated linoleic acid (CLA) on the serum leptin concentration in COPD patients. Therefore, the present study aimed to explore the effect of CLA on the serum leptin level, lung function and quality of life in COPD patients.MethodsThis interventional study was conducted on 90 COPD patients. The patients were randomly divided into two groups (supplement and placebo) with 45 patients per group. After obtaining written consent from the patients and recording their demographic characteristics, the spirometry was performed and COPD assessment test (CAT) score was calculated. Moreover, a fasting blood sample was collected from each of them in order to analyze their serum leptin concentration. After that, the patients in the supplement group were administered with 3.2 g/day of CLA for 6 weeks. The patients in another group received placebo. After the intervention, spirometry, CAT score calculation and blood sampling were repeated for all of the patients and the obtained results were analyzed.Resultsafter the intervention, a significant decrease both in the serum leptin level and in CAT score as a quality life marker was observed in the supplement group (p<0.05). Moreover, the FEV1 levels as a lung function test, increased significantly in the supplement group after the intervention (p<0.05). Although, the difference between the two groups was not significant.ConclusionThe use of CLA supplement can improve COPD patients’ quality of life through decreasing the serum level of leptin in their blood.Keywords: CLA, Leptin, COPD, CAT, Pulmonary Function, Quality of Life

Pages 61-70Background & objectivesPentavalent antimonials are the first-line drugs for treatment of leishmaniasis, which have multiple side effects such as drug toxicity. Moreover, parasite resistance to these drugs is rising around the world. Second-line drugs, including Amphotericin B and pantamidine have also side effects and expensive for patients. According to the cytotoxic effects of paraquat, this study was conducted to evaluate the effect of paraquat on Leishmania major promastigotes and HUVECs viability.MethodsA number of 2.5×106 of Leishmania major promastigotes were treated in each well of 96 well plates with different concentrations of paraquat. Cells were incubated for 48 hours in 24 °C. MTT test was performed for evaluating paraquat impact on promastigotes. The absorbance was measured using a microplate reader at 570 nm. The trypan blue staining assay was performed to evaluate the number of viable Leishmania major promastigotes following paraquat treatment. Furthermore, the effect of paraquat concentrations on HUVECs viability was evaluated under the cell culture condition.ResultsThe results of the MTT test showed that increasing concentrations of paraquat could significantly reduce the viability and the number of Leishmania major promastigotes in comparison to control group (p<0.05). In this study, the IC50 for Leishmania major promastigotes was calculated as 272.46 µg/ml. Trypan blue results were in line with the finding of MTT assay. Moreover, we found that HUVECs were susceptible to paraquat (IC50=188.99 µg/ml).ConclusionParaquat has a strong inhibitory effect on Leishmania major promastigotes and human endothelial cells. Although more comprehensive studies on the effects of the topical use of paraquat on Leishmania major lesions in animal model and its side effects are necessary.Keywords: Leishmania major, Paraquat, MTT Assay

The purpose of this study was to investigate the relationship between perfectionism and stress with life satisfaction in cardiovascular patients.MethodsThe research method is descriptive correlational. The statistical population of this study included patients who came to receive the services in Ardabil in the first half of 1396. The sample consisted of 60 cardiovascular patients who were selected by simple random sampling. Data was collected using questionnaires of perfectionism scale, stress scale and life satisfaction scale. Data were analyzed by descriptive statistics and multiple regression analysis using spss-19 software.ResultsThe results showed that there is a significant negative relationship between perfectionism and life satisfaction. Also, there was a significant negative relationship between stress and life Satisfaction (p<0.01). In other words, with increasing stress, life satisfaction decreases. There is a multiplicity relationship between perfectionism and stress with life satisfaction (p<0.01).ConclusionThe results of this study showed that reducing the perfectionism and stress of individuals helps to increase the satisfaction of people from life and also effective in the process of treatment.Keywords: Perfectionism, Stress, Life Satisfaction, Cardiovascular Patients

Pages 80-87Background & objectives: Chlamydia trachomatis is a gram negative bacterium and chlamydia infection, as a curable infection, is one of the most common sexually transmitted diseases (STD). With regard to the essential role of chlamydia in infertility, the study of the prevalence of asymptomatic cases is precious. The aim of this study was to determine of the prevalence of chlamydia trachomatis in endocervical samples in infertile women with PCR method.MethodsIn this cross sectional descriptive-analytical study, a total of 135 women between 20-40 years old with chief complaint of infertility that referred to Alzahra-Rasht hospital and private clinics were randomly selected. The endocervical specimen was prepared using a sterile swab and was transferred to the laboratory in PBS for performing PCR. . The results of PCR and collected data from checklists were statistically analyzed using SPSS16.ResultsChlamydia trachomatis was detected in 19.3% of infertile women. There were no statistically significant differences between PCR results and the patient's age, type of infertility, obstruction in salpingography, family history and duration of infertility.ConclusionThe results of this study revealed that chlamydia infection has a high prevalence and in order to reduce the complications of this disease, screening tests can be used as a part of the country's health programs.Keywords: Chlamydia trachomatis, Infertile Women, PCR

Pages 88-99Background & objectives: This study was conducted to compare the basic psychological needs and different levels of Theory of mind between mothers of oppositional defiant disorder and hyperactivity / attention deficit disorder childrenMethodsThe statistical population of the study included all mothers of childern with oppositional defiant disorder and hyperactivity/attention deficit disorder in psychiatric clinics of Fatemi hospital of Ardabil in 1396-1397. These mothers were randomly assigned in to two groups (35 people in group ODD & 35 people in group ADHD). The instruments used in this study were: Theory of mind, basic psychological needs and Children's Symptoms Questionnaire (CSI-4 Parent Form). The collected data were entered into SPSS-21 software and analyzed by data analysis method. Multivariate variance (MANOVA) was used.ResultsThe results of the analysis of variance finding show that is no significant difference between mental theory (p>0.407, F=0.697) of basic psychological needs such as competency (p>0.336, F=0.393) self-determination (p>0.597, F=0.283) and communication (p>03.387, F=0.760) between two groups of mothers of children with oppositional defiant disorder and hyperactivity disorder/attention deficit.ConclusionThese results show that the importance of paying attention to the basic psychological needs and the theory of the minds of mothers of these children in improving the educational status of social communication and emotional assessments and cognitive development of the child.Keywords: basic psychological needs, theory of mind, Mothers children with hyperactivity, Attention deficit disorder, oppositional defiant disorder

Pages 100-109Background &objectivesNowadays, bacterial resistance and the increase in the therapeutic costs are considered as the most important global concerns of medical care system regarding complicated infections. Imipenem is a member of the carbapenem class of beta-lactam antibiotics prescribed mostly in our hospitals because of its broad activity against bacterial infections. Drug Utilization Evaluation (DUE) process is an official, ongoing and systemic program that collects information in order to identify and improve the adverse effect of drugs and the cost of medicalization. The objective of this study was to evaluate the administration and use of imipenem in the educational Imam Khomeini hospital in Ardabil in 2018.MethodsIn this prospective, descriptive, cross- sectional study, 110 hospitalized patients, who received imipenem from September to December of 2018, were included in this study. Patient's demographic data, dosing, dosage adjustment in renal failure and other co-prescribed antimicrobial drugs were extracted from current medical file of hospitalized patients and evaluated with medical guidelines.Results64% of patients received imipenem in the first day of hospitalization and 75.5% of patients were empirically received imipenem while antibiogram test was ordered for only 24.5% of patients. Serum creatinine were ordered for most of the patients, but correct dose regimens for patients who get non-empiric antibiotic therapy were only 25.5%.ConclusionHigh rate of empiric prescription without considering the result of antibiogram test and immediate initiation of antimicrobial therapy at the time of admission were the most important aspects of irrational use of imipenem observed in this study. Paying more attention to sampling, culturing and sensitivity test and prescription of imipenem based on specific guidelines are recommended.Keywords: Imipenem, Drug Utilization Evaluation, Microbial Drug Resistant

Pages 110-116Background & objectivesMEFV gene has a major role in Familial Mediterranean Fever (FMF) as an auto-inflammatory disorder. FMF is most often seen in the people of the Mediterranean area. Considering the significant role of the MEFV gene in many rheumatologic diseases and even non-rheumatologic disorders, it is necessary to identify different variations of these mutations in the healthy and normal population of this area.Methods224 healthy people entered into this study. The blood samples were screened for the 12 most common MEFV gene variants according to manufacturer’s instructions. (FMF Strip Assay, Vienna lab, Vienna, Austria)
They filled a questionnaire containing the required information. All patients were initially evaluated for the FMF symptoms and signs in themselves and their first-degree relatives based on clinical criteria. Chi-squared test and t-test were employed for statistical analysis using SPSS ver.24.ResultsAmong 224 cases, 113 cases (50.4%) were male, and 111 cases (49.6%) were female. MEFV mutations were detected in 57 patients (25%) of them, 28 cases were male (49.1%) and 29 cases were female (50.9%). The most frequent mutations were E148Q (18.3%, 41cases), followed by P369S (3.1%, 7 cases), V726A (2.2%, 5cases), A744S (1.3%, 3cases), F479L, M694V and R761H (0.8%, each 2 cases), and eventually K695R (0.4%) respectively. Some mutations such as M694I, M680I (G/C), M680I (G/A), I692del were not seen in these samples. There were compound heterozygous mutations of E148Q/P369S, E148Q/V726A, E148Q/P369S, and P369S / F479L in normal population without any findings in favor of FMF.ConclusionTwenty-five percent of the normal population of the northwest of Iran carrying a heterozygous variant of the MEFV gene, E148Q (18.3%) as a most common mutation, which can be considered as a normal variant in the healthy population. The presence of M694I, M680I (G/C), M680I (G/A) and I692del mutations in the normal population can be interpreted with cautiously, while particular compound heterozygous mutations can be considered as normal variants.Keywords: MEFV mutations, FMF Gene, Northwest of Iran, Ardabil
