صفحات 1-11زمینه و هدفتهوع و استفراغ، شایع ترین شکایت زنان در نیمه اول بارداری می باشد که بر جنبه های مختلف سلامت زنان باردار تاثیر می گذارد. در زمینه نقش عوامل روانی در جهت ایجاد و تشدید تهوع و استفراغ بارداری تحقیقات بسیار کمی انجام شده است. مطالعه اخیر با هدف بررسی ارتباط بروز و شدت تهوع و استفراغ دوران بارداری با اضطراب و افسردگی زنان باردار انجام شده است.روش بررسیپژوهش حاضر یک مطالعه از نوع همبستگی توصیفی است که بر روی 220 زن باردار سالم با سن بارداری 16- 6 هفته مراجعه کننده به واحد مراقبت های دوران بارداری دو مرکز بهداشتی درمانی منتخب شهر تهران انجام شده است. برای سنجش تهوع و استفراغ بارداری، اضطراب و افسردگی به ترتیب از ابزارهای سنجش تهوع و استفراغ بارداری (PUQE)، پرسشنامه اضطراب آشکار و پنهانSpielberger و پرسشنامه افسردگی Edinburgh استفاده شد. برای تجزیه و تحلیل داده ها از نرم افزارSPSS نسخه 16 و آزمون های آماری مجذور کای و ضریب همبستگی اسپیرمن استفاده شد.یافته هافراوانی تهوع و استفراغ بارداری در نمونه های مورد بررسی 6/83 درصد بود. همچنین رابطه مستقیم و معنی داری بین بروز و شدت تهوع و استفراغ بارداری با اضطراب و افسردگی (05/0P<) زنان باردار وجود داشت.
نتیجه گیری کلی: یافته های حاصل از این مطالعه نشان دهنده وجود یک رابطه مستقیم و معنی دار بین بروز و شدت تهوع و استفراغ بارداری با اضطراب و افسردگی در زنان باردار است. بنابراین پیشنهاد می شود، در مراقبت های دوران بارداری، ارزیابی عوامل روانی نیز مورد توجه قرار گیرد.کلیدواژگان: تهوع و استفراغ، بارداری، اضطراب، افسردگی، عوامل روانی -
صفحات 12-22زمینه و هدفکودکان دارای تهویه مکانیکی به دلیل نداشتن رفلکس سرفه، بلع و همچنین بی حرکتی، در معرض خطر ابتلا به پنومونی هستند. بسیاری از راهکارهایی که با هدف پیشگیری از پنومونی ناشی از دستگاه تهویه مکانیکی انجام می شود، از مسئولیت های مستقیم پرستاران در بالین است. این مطالعه با هدف بررسی عملکرد پرستاران در زمینه مراقبت از دهان کودکان دارای تهویه مکانیکی انجام شد.روش بررسیپژوهش حاضر مطالعه توصیفی بود که 100 پرستار شاغل در بخش های مراقبت ویژه کودکان طی سال 1394 به صورت نمونه گیری در دسترس وارد مطالعه شدند. ابزار پژوهش شامل فرم مشخصات جمعیت شناختی و سه نوع چک لیست بررسی عملکرد، بر اساس توصیه های مبتنی بر شواهد موجود در راهنمای بالینی بهداشت دهان در کودکان دارای تهویه مکانیکی بود. عملکرد هر پرستار توسط یکی از سه نوع چک لیست، سه مرتبه و در نوبت های کاری مختلف مشاهده و بررسی شد. نمره عملکرد پرستاران در سه سطح زیر 50 درصد، بین50 تا 9/74درصد و بالای 75 درصد طبقه بندی شد. تجزیه و تحلیل آماری با استفاده از آمار توصیفی صورت گرفت.یافته هابیش از نیمی از پرستاران (67%) میانگین نمره عملکرد زیر 50 درصد، 24درصد میانگین نمره عملکرد 50 درصد تا 9/74 درصد و تنها 9 درصد از شرکت کنندگان میانگین نمره عملکرد بالای 75 درصد را کسب کردند. بالاترین نمره عملکرد پرستاران مربوط به پوشیدن دستکش (%980) و کمترین آن مسواک زدن (0%) بود.
نتیجه گیری کلی: با توجه به عملکرد ضعیف پرستاران در به کارگیری راهنمای بالینی، به نظر می رسد ارائه راهکارهای لازم جهت آموزش پرستاران، فراهم کردن امکانات مناسب اجرای پروتکل های مبتنی بر شواهد در زمینه بهبود کیفیت مراقبت های بالینی و نیز انجام پژوهش هایی جهت بررسی علل عملکرد پایین پرستاران امری ضروری است.کلیدواژگان: عملکرد، پرستاران، دهان، مراقبت، کودکان، تهویه مکانیکی -
صفحات 23-32زمینه و هدفدانشجوی پرستاری زمانی از آموزش احساس رضایت می کند که در طی دوره بالینی درگیر مراقبت از بیمار شود، خود را جزئی از تیم درمان بداند و همچنین تجربه مسئولیت پذیری و استقلال را کسب و در واقع به پیامدهای یادگیری دست پیدا کند. مطالعه حاضر با هدف بررسی رضایت دانشجویان پرستاری از نظارت پرسپتور در آموزش بالینی انجام شد.روش بررسیمطالعه حاضر به صورت مقطعی انجام شد. صد و دوازده دانشجوی کارشناسی پرستاری که برنامه آموزش بالینی پرسپتورشیپ را تجربه کرده بودند، در مطالعه شرکت کردند. جهت ارزیابی رضایت دانشجویان پرستاری از نظارت پرسپتور، از پرسشنامه تسهیل گری های بالینی استفاده شد. داده ها با استفاده از نرم افزار SPSS نسخه 16 و آزمون T-test، ANOVAو ضریب همبستگی پیرسون تحلیل شد.یافته هامیانگین رضایت دانشجویان پرستاری از پرسپتور 85.18 ± 25/100 بود. بین رضایت دانشجویان از نظارت پرسپتور با سن، محل سکونت، ترم تحصیلی، طول دوره کارآموزی/ کارورزی و محل گذراندن کارآموزی و کارورزی ارتباط وجود داشت (05/0P<).
نتیجه گیری کلی: یافته ها گویای رضایت بالای دانشجویان از نظارت پرسپتور در برنامه آموزش بالین بود. بنابراین انجام مطالعات بیشتر و تاثیر این برنامه در ارتقای توانمندی دانشجویان در مطالعات آتی پیشنهاد می شود.کلیدواژگان: پرستاری، پرسپتور، رضایت، دانشجو، آموزش بالین -
صفحات 33-42زمینه و هدفگزارش پرستاری یکی از اسناد مهم پرونده بیماران بوده و بهترین مدرکی است که می تواند فعالیت های 24 ساعته پرستاران را بصورت کامل و روشن بیان نماید. از این رو وجود خطا و اشکال در نگارش آن می تواند تبعاتی برای پرستار بدنبال داشته باشد. پژوهش حاضر با هدف تعیین وضعیت خطاهای گزارش نویسی در پرستاران انجام شد.روش بررسیتحقیق حاضر مطالعه توصیفی- تحلیلی است که جامعه آماری آن شامل کلیه پرستاران (161 نفر) شاغل در بیمارستان شهید بهشتی یاسوج در سال 1393 بودند. ابزار مورد استفاده در پژوهش، پرسش نامه ی محقق ساخته بود که توسط آن مشخصات جمعیت شناختی و خطاهای گزارش نویسی پرستاران در قالب 19 سوال بررسی شد. تجزیه و تحلیل داده ها با استفاده از آمار توصیفی و شاخص های مرکزی و پراکندگی و همچنین آزمون های استنباطی t-test و ANOVA با نرم افزارSPSS نسخه 16 انجام شد.یافته هابر اساس یافته های پژوهش حاضر، 71/85 درصد پرستاران خطای گزارش نویسی داشتند که بیشترین خطا مربوط به نوشتن گزارش نویسی خیلی طولانی با 247 مورد (02/21%) بوده است. میانگین خطای گزارش نویسی برای هر پرستار در یک ماه 22/1 مورد بوده و همچنین بین خطاها و جنس و سابقه کار پرستاران ارتباط معنی داری وجود داشته است .
نتیجه گیری کلی: نتایج نشان داد گزارش های پرستاری دارای وضعیت مناسبی نیستند. به نظر می رسد ایجاد زیرساخت های قانونی و فرهنگی برای گزارش دهی خطاها، تدوین برنامه های آموزشی و پایش مداوم در ارتقای کیفیت گزارش نویسی پرستاران ضروری است.کلیدواژگان: پرستار، خطا، خطای گزارش نویسی، ایمنی بیمار -
صفحات 43-56زمینه و هدفمدیران پرستاری نقش مهمی در اجرای مراقبت مبتنی بر شواهد در محیط های بالینی دارند. لذا سواد اطلاعاتی آنها از پیش نیازهای مهم اجرای این مراقبت می باشد. اهداف این مطالعه بررسی سواد اطلاعاتی مدیران پرستاری و نیازهای آموزشی آنها در زمینه مراقبت مبتنی بر شواهد بود.روش بررسیاین مطالعه توصیفی و مقطعی در چهار بیمارستان آموزشی دانشگاه علوم پزشکی کرمان در سال 1393 انجام شد. داده ها با استفاده از یک پرسشنامه سه بخشی از 70 مدیر پرستاری جمع آوری شد. برای تجزیه و تحلیل داده ها از نرم افزار SPSSنسخه 16 و آمار توصیفی (درصد، میانگین و انحراف معیار) و آمار تحلیلی) آزمون t مستقل و آنالیز واریانس) استفاده گردید.یافته هامشارکت کنندگان برای جستجوی اطلاعات پزشکی بیشتر از منابع چاپی و انسانی استفاده می کردند تا منابع الکترونیک (96/0±97/1). میانگین نمرات استفاده از امکانات مختلف جستجو در بانک های اطلاعاتی و موتور های جستجو بسیار پایین بود (04/1±78/1). درصد کمی از شرکت کنندگان از عملکرد و هدف عملگرهای Boolean و نزدیک یابی آگاهی داشتند؛ بطوری که برای جستجوی اطلاعات در یک پایگاه داده فقط 17 درصد از آنها مناسبترین گزینه را از یک مثال فرضی انتخاب کردند. نیازسنجی آموزشی نشان داد تقاضای مشارکت کنندگان برای دریافت آموزش در حوزه های مراقبت مبتنی بر شواهد در حد متوسط بود (97/0±89/3).
نتیجه گیری کلی: مدیران پرستاری سواد اطلاعاتی لازم برای جستجو و بازیابی اطلاعات از منابع الکترونیکی را ندارند و به ندرت از این منابع روزآمد استفاده می کنند. این امر موجب می شود که آنان نتوانند به خوبی کارکنان پرستاری را به استفاده از دانش و مهارت مورد نیاز و هم چنین تکنیک های نوآورانه جهت ارائه بهترین و موثر ترین مراقبت راهنمایی و تشویق نمایند. پیشنهاد می شود مطالعات آتی نقش کتابداران بالینی، متخصصین انفور ماتیک پزشکی و مربیان پرستاری بالینی را در ارتقای سواد اطلاعاتی ارائه دهندگان مراقبت سلامت بررسی نمایند.کلیدواژگان: مراقبت مبتنی بر شواهد، سواد اطلاعاتی، منابع اطلاعات -
صفحات 57-66زمینه و هدفرضایت شغلی پرستاران جزء بسیار مهمی از زندگی کاری آنان است که می تواند بر امنیت بیمار، روحیه کارکنان، عملکرد و کیفیت مراقبت های پرستاری تاثیر بگذارد. کودکان بیمار، همواره برای برطرف کردن نیازهای خود وابسته به پرستاران هستند و پرستاران نقش عمده ای در کار با کودکان در بیمارستان ها دارند. مطالعه ی حاضر با هدف تعیین میزان و عوامل موثر بر رضایت شغلی پرستاران شاغل در بخش های کودکان بیمارستان های وابسته به دانشگاه علوم پزشکی اهواز در سال 1391 انجام شده است.روش بررسیاین پژوهش مطالعه ای توصیفی و از نظر زمانی مقطعی است که بر روی 80 پرستار شاغل در بخش های کودکان بیمارستان های دانشگاه علوم پزشکی اهواز که به صورت سرشماری وارد مطالعه شدند، انجام گردید. به منظور جمع آوری داده ها در این مطالعه از پرسشنامه رضایت شغلی «مینه سوتا» استفاده شد. تجزیه و تحلیل داده ها از طریق آمار توصیفی و آزمون های آماری تی مستقل و من ویتنی انجام شد. نرم افزار مورد استفاده برای تجزیه و تحلیل داده هاSPSS نسخه 16 بود.یافته هامیزان رضایت شغلی 55 درصد از پرستاران در سطح متوسط و 45 درصد آنها در سطح بالا قرار داشت. میانگین رضایت شغلی پرستاران بخش کودکان 93/10± 86/72 بود. با استفاده از آزمون آماری تی مستقل ارتباط معنی دار آماری بین رضایت شغلی پرستاران و علاقه ی آنان به حرفه پرستاری به دست آمد (05/0>P). همچنین استفاده از آزمون من ویتنی نشان داد که بین اشتغال به کار در چند بیمارستان و رضایت شغلی پرستاران ارتباط معنی دار آماری وجود دارد (05/0>P).
نتیجه گیری کلی: با توجه به یافته های پژوهش حاضر مبنی بر متوسط بودن رضایت شغلی پرستاران بخش کودکان که ناشی از مشکلات مختلف مانند نداشتن علاقه به حرفه پرستاری بوده است، لذا استفاده از راهبردهایی جهت افزایش علاقه و انگیزه پرستاران بخش کودکان توسط مسئولین و مدیران پرستاری ضروری است.کلیدواژگان: رضایت شغلی، پرستاران، کودکان

Pages 1-11Background and AimsNausea and vomiting are common hard symptoms experienced by pregnant women in the first trimester. Nausea and vomiting of pregnancy (NVP) has adverse effects on the quality of a woman's life, social, relationship with family, and her general health. The aim of this study was determining the relationship between during pregnancy with anxiety and depression.
Material &MethodsIt was a descriptive, correlational study. Anxiety and depression scores of 220 women were investigated by using the Spielberger's state-trait anxiety questionnaire and the Edinburgh Depression Inventory. NVP were scored by using the pregnancy unique quantification of emesis and nausea (PUQE). Data was analyzed by SPSSver.16 and Chi-square, and spearman tests.ResultsThe frequency of nausea and vomiting in pregnancy was 83/6%. A significant correlation between incidence of nausea and vomiting of pregnancy and both anxiety and depression score (pConclusionThere is an association between anxiety and depression early in pregnancy and severity of NVP. Pages 12-22Background and AimChildren under mechanical ventilation are at risk of developing pneumonia, due to the lack of cough reflex, swallowing and immobility. Many of the pneumonia preventing strategies related to ventilation are direct responsibilities of clinical nurses. The aim of the study was to evaluate the performance of nurses in the field of oral care in the children under mechanical ventilation.
Method & Materials: The research was a cross-sectional- descriptive study and 100 nurses from pediatric intensive care unit were selected by using convenience sampling, during 2015. The instruments were including the demographic characteristics form and three types of performance checklist related to oral care guideline in the children under mechanical ventilation. The performance of each nurse was observed and examined by one of the three check lists for three times in different shifts. Then the performance of nurses was classified at three levels including less than 50%, between 50% - 74.9% and also 75% and above. Statistical analysis was performed using descriptive statistics.ResultsMore than half of nurses (67%) obtained below 50% of the mean performance score, 24% of them received 50% -74.9% of the mean and only 9% of them reached to 75% mean performance score. The most items related to wearing gloves (0.98) and the lowest was brushing (0%).ConclusionThe nurses performance in oral care was poor, hence it is necessary to design education course for nurses, provide proper facilities of evidence-based protocols in improvement of the quality of clinical care and conduct research about the causes of poor performance in nurses.Keywords: Performance, Nurses, Oral, Care, Children, Mechanical Ventilation -
Pages 23-32Background and AimsStudent satisfaction from preceptor is achieved when the student involves in patient care in clinical setting, he knows himself as part of the treatment team, experiences responsibility and authority, and in fact, encounters with learning outcomes. The aim of this study was to evaluate nursing student's satisfaction with supervision from preceptors during clinical practice.
Material &MethodsIt was a cross-sectional study. One hundred and twelve nursing students who had experienced preceptorship program participated in this study. To evaluate the nursing student satisfaction with supervision from preceptors Nursing Facilitator Clinical Questionnaire was used. Data were analyzed with t-test, ANOVA and Pearson correlation test by using SPSS version 16.ResultsThe mean score of nursing student satisfaction from preceptor was 100.25±18.85. There was correlation between nursing student satisfaction from preceptor and age, living place, academic semester, apprenticeship / internship duration, and apprenticeship / internship location (PConclusionThe study underlines that, the students had high satisfaction with supervision from preceptors. Pages 33-42Background and AimsNursing report is one of the important documents in a patients record and it is also the best tool which can help to evaluate the nurses activities in 24-hour periods. Therefore, committing error in report writing may have consequences for a nurse. This study aims to determine the status of error reporting among nurses.Materials and MethodsThis study is a descriptive-analytic one and the participants were all the nurses (161 Nurse) working at Yasuj Shahid Beheshti hospital of Yasuj, Iran in 2014. The data collecting tool was a researcher-made questionnaire that contains 19 questions about the demographic characteristics of nurses and the reporting error among them. The data was analyzed using descriptive statistics, coefficient of dispersion, t-test and ANOVA and SPSS software version 16.ResultsBased on the findings of study, only 85/71% of the nurses have committed reporting error and writing long reports was the most frequent error (247 cases, 21%). The mean of error reporting for each nurse was 1/22 in a one-month period. There were significant relationships among reporting errors and the sex and work experiences of the nurses.ConclusionThe results showed that nursing reports have a poor quality. It seems that creating legal and cultural infrastructures for error reporting, developing the training programs and continuous monitoring are necessary to improve the quality of nursing reports.Keywords: Nurse, Error, Reporting Error, Patient Safety

Pages 43-56Background and AimsNursing managers play an important role in the implementation of evidence-based practice (EBP) in clinical settings. Hence, their information literacy is an important prerequisite for providing this service. The objectives of this study were to examine information literacy of nursing managers and to determine their training needs in the context of EBP.Materials and MethodsThis descriptive and cross-sectional study was conducted in four hospitals affiliated with Kerman University of Medical Sciences in 2014. Data were collected from 70 nursing managers of these hospitals using a questionnaire. Data were analyzed in SPSS 16 using descriptive (percentage, mean, standard deviation) and analytic statistics (t-test, analysis of variance).ResultsThe participants used more printed and human resources (1.97±0.97) than electronic resources to seek information. The mean scores of using different search features in online databases and search engines was very low (1.78±1.04). A small percentage of participants were aware of the purpose and functions of Boolean and Proximity operators, so that only 17% of them chose the most appropriate option to search information from a database. Educational need assessment showed that participants demand for training in different fields of EBP was at a medium level (3.89±0.97).ConclusionNursing managers do not possess required skills to search and retrieve information from electronic resources and they use these up-to-date resources scarcely. Because of this, they can not guide and encourage nursing staffs to use the required knowledge and skills, as well as innovative techniques to provide a high quality care. It is recommended that future studies evaluate the role of clinical librarians, medical informaticians and clinical nurse educators for promoting information literacy of healthcare providers.Keywords: Evidence Based Practice, Information literacy, Information Resources

Pages 57-66Background and AimsNurse's job satisfaction forms an extremely crucial part of their work-lives which can affect the patient's safety, the staff morale and nursing care quality and functioning. Pediatric patients are dependent on nurses to have had their needs met and nurses have significant role in pediatric wards of hospitals. The purpose of this study was to determine the level and effective factors affecting pediatric nurse's job satisfaction in hospitals affiliated with Ahwaz Jundishapur University of Medical Sciences, Iran.Materials and MethodsThis cross- sectional, descriptive study was conducted in 2012 on 80 nurses working in pediatric wards in hospitals affiliated with Ahwaz Jundishapur University of Medical Sciences that were selected by census. Data were collected by using Minnesota job satisfaction questionnaire and were analyzed by descriptive statistics and T-test and Mann Whitney test utilizing SPSS version 16.ResultsThe job satisfaction level turned out to be the average in 55 percent of the nurses and high in 45 percent of them. The mean score of pediatric nurse's job satisfaction was 72.86± 10.93. The findings showed that the relationship between the nurse's job satisfaction and their interest in nursing profession was significant (PConclusionConsidering the current research findings, different difficulties such as lack of interest in nursing profession have led to the average level of pediatric nurse's job satisfaction.
