موفقیت بالای لقاح داخل آزمایشگاهی از طریق انتقال جنین با بالاترین قابلیت تکاملی، به دست می آید. حاملگی های چندقلویی حاصل از انتقال همزمان چند جنین در هر سیکل با عوارض متعددی همراه است. بنابراین، انتقال جنین منفرد روشی ترجیحی در درمان با روش های کمک باروری می باشد. به منظور بهبود میزان حاملگی در انقال جنین منفرد، جنین شناسان نیازمند بیومارکرهای مطمین برای کمک انتخاب جنین با بالاترین پتانسیل تکاملی، هستند. تکنولوژی تایم-لبس، جایگزینی غیرتهاجمی برای ارزیابی های میکروسکوپی سنتی می باشد. این روش ارزیابی مداوم و بدون وقفه ای از جنین تا روز انتقال، فراهم می نماید. امروزه، چهار مدل سیستم تایم-لبس به صورت تجاری در مراکز درمانی کمک باروری موجود می باشد. در سال 2010 و 2015 در جهان و ایران به ترتیب، برای اولین بار جنین حاصل از انتقال جنین منفرد با استفاده از تکنولوژی تایم-لبس، متولد شد. در این مطالعه، ما مروری بر استفاده از تکنیک تایم-لبس در مشاهده تکامل جنین و نقش این سیستم در انتخاب جنین منفرد، به عمل آوردیم. یافته-های مطالعه ما اگرچه استفاده معمول از این سیستم را در مراکز درمانی کمک باروری حمایت می کند، اما مطالعات بزرگتری که به خوبی طراحی شده اند، بایستی انجام شود.

فرایند اسپرماتوژنز از لومن لوله های اسپرم ساز در قاعده ی این لوله ها آغاز شده و پس از مهاجرت به غشای پایه مجددا در لومن خاتمه می یابد. ما برآن بودیم تا آثار محافظ کننده ی هرمون های ملاتونین و گرلین را بر فرایند اسپرماتوژنز و به خصوص بر سلول های بنیادی اسپرماتوگونی (SSC) به عنوان دو هرمون به نسبت نوظهور بررسی کنیم. از راه های حفظ باروری در کودکان مبتلا به سرطان که مورد شیمی درمانی و پرتودرمانی قرار می گیرند، استفاده از فرایند انجماد است. این انجماد دارای دو روش انجماد آهسته (cryopreservation) و انجماد سریع (vitrification) است. مداخله ی ملاتونین می تواند موجب حفظ کیفیت اسپرم هایی که انجماد برروی آن ها ایجاد شده بود، گردد؛ همان طوری که گرلین نیز از سلول های جنسی در برابر سمیت های ناشی از آسیب های ایسکمیک و اپوپتوز پاتولوژیک محافظت می کند. مرور حاضر نشان می دهد که مداخله ی in-vitro و in-vivo ملاتونین و گرلین می تواند برحفاظت از باروری در چنین شرایطی موثر باشد و نیز می تواند به منظور بهبود کیفیت اسپرم ها در فن آوری کمک باروری استفاده گردد. پیشنهاد می گردد مقالات پژوهشی اصیل دراین حوزه به سمت مطالعات انسانی و البته با رعایت اخلاق در پژوهش سوق داده شوند.

مطالعات متعدد نقش مولتیمینرال ها و ویتامین ها در پیشگیری از پره اکلامپسی را متغیر گزارش کرده اند.
هدفمطالعه حاضر با هدف بررسی تاثیر مکمل یاری مولتی مینرال-ویتامین D (کلسیم، منیزیم، روی و ویتامین D) و ویتامینهای C و E در پیشگیری از پرهاکلامپسی انجام شده است است.
موارد و روش هامطالعه حاضر یک کار آزمایی بالینی تصادفی شده شاهددار می باشد. جحم نمونه 90 نفر در سه گروه تقسیم شدند: گروه A فروس سولفات (1 قرص در روز) + روزانه یک قرص Claci-care که حاوی mg800 کلسیم، mg 200 منیزیم، mg 8 روی و 400 IU ویتامین D3 می باشد. گروه B، فروس سولفات (1 قرص در روز) + mg250 ویتامین C و mg 55 ویتامین E و گروه شاهد تنها فروس سولفات دریافت می کنند. در نهایت داده ها با استفاده از نرم افزار SPSS-17 آنالیز شدند.
نتایجبروز پره اکلامپسی در گروه A به طور معنی داری پایین تر از گروه شاهد بود (03/0=p)، در حالی که بین گروه B و گروه شاهد (50/0=p)، و همچنین گروه A و B تفاوت معنی داری وجود نداشت(063/0=p). بروز عوارض نوزادی در گروه A به طور معنی داری پایین تر از گروه شاهد بود (01/0=p) بود، در حالی که بین گروه B و کنترل (48/0=p) این تفاوت معنی داری نبود.
نتیجه گیریبا توجه به نتایج، مکمل مولتی مینرال-ویتامین D (کلسیم، منیزیم، روی و ویتامین D) دارای اثر معنی داری در پیشگیری از بروز پره اکلامپسی دارد. علاوه بر این، تجویز مکمل مولتی مینرال-ویتامین D در دوران بارداری می تواند به عنوان یک راه ارزان و مقرون به صرفه برای کاهش بروز پره اکلامپسی در زنان خطر بالای پره اکلامپسی باشد و از عوارض نوزادی نیز جلوگیری کند.

تراتواستنوزواسپرمیا، یکی از اشکال شدید ناباروری در مردان است که علت پاتوفیزیولوژی آن چندان روشن نیست.
هدفمقایسه آنیون سوپراکساید داخل سلولی، HSPA2 و نقص پروتامین در اسپرم مردان نابارور تراتو استنوزواسپرمیا و نورموزواسپرمیا. عقیده بر آن است که این عوامل ممکن است که در ناباروری مردان تراتواستنوزواسپرمیا نقش داشته باشند.
موارد و روش هادر یک مطالعه مورد- شاهدی، 40 نفر تحت بررسی قرار گرفتند: 20 مرد نابارور با علایم تراتواستنو (گروه تحت مطالعه)، و 20 مرد بارور نورموزواسپرمیا (گروه کنترل). بعد از سیمن آنالیز برطبق WHO، ما آنیون سوپراکساید داخل سلولی و HSPA2 را با روش فلوسیتومتری و نقص پروتامین در اسپرم را با تست CMA3، در هر دوگروه مورد ارزیابی و مقایسه قرار دادیم.
نتایجآنیون سوپراکساید داخل سلولی و نقص پروتامین در بیماران تراتواستنو سطح بالاتری را نسبت به افراد نرمال نشان دادند (به ترتیب43/1±69/33 و 69/1±67/36 در مقابل 7/2±35/16 و68/0±71/20 : 05/0p≤)، در حالیکه HSPA2، سطح پایین تری را در گروه بیمار نسبت به افراد نرمال نشان داد (به ترتیب 87/2±86/22 در مقابل 81/1±56/46: 5 0/0p≤). نتایج ما رابطه مثبتی را بین آنیون سوپراکساید داخل سلولی و آپوپتوز و همچنین مورفولوژی غیر نرمال اسپرم نشان دادند، ولیکن بین آنیون سوپراکساید داخل سلولی و زنده مانی اسپرم، تعداد اسپرم و حرکت اسپرم (01/0≥p و 81/0-=r) و غلظت اسپرم (05/0≥p و 052/0-=r) رابطه منفی برقرار بود.
نتیجه گیریسنجش آنیون سوپراکساید داخل سلولی، HSPA2 و نقص پروتامین در ارزیابی ناباروری بیماران تراتواستنوزواسپرمیا می تواند مفید باشد.

مقیاس افسردگی و اضطراب بیمارستانی یک ابزار غربالگری است که برای اندازه گیری میزان اضطراب و افسردگی در جوامع غیر روان پزشکی به کار می رود. این ابزار به طور گسترده ای در بررسی بیمارن نابارور به کار گرفته شده است.
هدفهدف این مطالعه، بررسی ساختار عاملی، تحلیل آیتم و سازگاری درونی این ابزار در بیماران نابارور مراجعه کننده به پژوهشگاه رویان است.
موارد و روش هااین پژوهش مقطعی شامل 651 بیمار (248 مرد و 403 زن) بود که در آن بیماران به یکی از مراکز مرجع ناباروری در تهران مراجعه کرده بودند. پرسشنامه HADS، مقیاس رضایت از زندگی (SWLS) و اطلاعات بالینی و دموگرافیک بیماران مورد بررسی و ثبت شدند. تحلیل عاملی تاییدی برای تعیین ساختار عاملی پرسشنامه HADS به کار گرفته شد. ساختارهای تک، دو و سه عاملی به کار گرفته شده در مطالعات مختلف مورد ارزیابی و مقایسه قرار گرفتند.
نتایجنتایج تحلیل عاملی تاییدی نشان داد که الگوی سه عاملی در مقایسه با سایر الگوها برازش بهتری را نتیجه داد. ضریب آلفای کرونباخ برای دو خرده مقیاس اضطراب و افسردگی به ترتیب برابر با 866/0 و 735/0 بود و به طور معناداری با SWLS همبسته بودند که نشان دهنده اعتبار همگرایی قابل قبولی است.
نتیجه گیریپرسشنامه HADS به عنوان یک ابزار با ساختار عاملی سه گانه می تواند در زمینه ناباروری مورد استفاده قرار بگیرد.

انتخاب بهترین جنین برای انتقال به بیمار در تکنیک های کمک باروری (ART) از اهمیت زیادی برخوردار می باشد. استفاده از بررسی های مورفولوژیک برای این انتخاب اطلاعات اندکی فراهم می آورد تا آنجایی که ارتباط معنی داری بین مورفولوژی جنین و پتانسیل باروری نشان داده نشده است. از طرف دیگر، آنوپلوییدی یکی از فاکتورهای کلیدی است که می تواند بر روی موفقیت باروری انسان در ART موثر باشد.
هدفهدف از این مطالعه بررسی شیوع آنوپلوییدی در جنین های با کیفیت بالا از نظر مورفولوژی از بیماران نابارور در سنین جوانی که به منظور تعیین جنسیت جنین تحت درمان ART قرار گرفته اند.
موارد و روش هاتعداد 97 جنین با کیفیت بالا از 23 بیمار زن زیر 37 سال که قبلا تحت غربالگری جنینی پیش از تولد (PGS) قرار گرفته بودند وارد این مطالعه شدند .بعد ازشستشو، لام های حاوی بلاستومرهای جنینی بیماران مورد آنالیز مجدد کروموزوم های معمول اتوزومی 13، 18 و21 با استفاده از تکنیک (FISH) قرار گرفتند .
نتایجمیزان قابل توجهی از انوپلوییدی در جنین ها در مجموع کروموزوم های مورد ارزیابی در مقایسه با نتایج غربالگری قبلی کروموزوم های جنسی وجود داشت (62/9% در برابر 24/7%، 0/00=p). بیشترین انوپلوییدی اتوزومی مشاهده شده متعلق به تریزومی /مونوزومی کروموزوم 13 بود.
نتیجه گیریمیزان قابل توجهی از ناهنجاریهای تعدادی کروموزوم در جنین های حاصل از لقاح آزمایشگاهی بوجود می آید که یکی از دلایل اصلی شکست درمان می باشد و به نظر می رسد که شاخصه های اصلی مورفولوژیک جنین روش مناسبی برای تشخیص جنین های ژنتیکی ناهنجار نمی باشد.

ویروس پاپیلومای انسانی (HPV) یکی از مشکلات عمده بهداشت عمومی و علل اصلی سرطان دهانه رحم می باشد. شیوع عفونت HPV در کشورهای در حال توسعه با منابع مالی کم بالا است. بنابراین مطالعه در مورد عوامل خطر و ایجاد عفونت و ژنوتایپینگ HPV در زنان می تواند به ایجاد برنامه های غربالگری موثری کمک نماید.
هدفاین مطالعه با هدف تعیین فراوانی نسبی ژنوتیپ های HPV و بررسی ویژگی های اجتماعی جمعیتی وابسته به آن مانند سن، دوره قایدگی، سن ازدواج، سن یایسگی، روش های پیشگیری از بارداری، بیماری زمینه ای، پیشینه بیماری های مقاربتی، پیشینه استفاده از سیگار، سطح تحصیلات، پیشینه مقاربتی و بارداری و وضعیت اقتصادی در زنان مراجعه کننده به یک بیمارستان عمومی در تهران، ایران از سال 93 تا 94 انجام پذیرفت.
موارد و روش هامطالعه ی مقطعی بر روی 400 نفر از زنان متاهل مراجعه کننه به کلینیک زنان یک بیمارستان عمومی در تهران انجام شد. نمونه پاپ اسمیر و معاینه gynecology از تمامی افراد گرفته شد. تشخیص 28نوع ژنوتیپ HPV که بوسیله ی تکنیک Multiplex PCR انجام پذیرفت. بررسی خصوصیات اجتماعی جمعیتی برای هر زن HPV مثبت صورت گرفت.
نتایج28 نوع تیپ مختلف از HPV در میان زنانی که به بیمارستان مراجعه کرده بودند شناسایی شد. عفونت HPV مثبت در 155 زن(75/38%) در سنین بین 17 الی 85 تشخیص داده شد. شایع ترین تیپ ها به ترتیب شامل HPV16 (1/19%) و پس از آن HPV39 (5/12%) و سپس HPV 18 (9/8%) گزارش شدند. بیشترین میزان عفونت HPV در سن 36 (7/7%) مشاهده شد. سطح تحصیلات و وضعیت اقتصادی هر زن نشان داد که بسیاری از زنان HPV مثبت دارای دیپلم دبیرستان (6/34%) و وضعیت اقتصادی متوسط (9/67%) هستند. 9/60% از این زنان خانه دار بودند و 3/67% در پایتخت (تهران) زندگی میکردند تمام فاکتور های خطر HPV16 را به عنوان شایع ترین تیپ ویروس نشان دادند. در میان زنانی که سیگاری بوده و قلیان مصرف میکردند HPV16 در مقایسه با انواع دیگر بیشتر مشاهده شد.
نتیجه گیریتعیین ژنوتیپ HPV و عوامل خطر مربوط به عفونت آن در هر منطقه جغرافیایی می تواند منجر به تولید واکسن موثر علیه ویروس شود. همچنین می تواند در مدیریت بیماری و تشخیص نیوپلازی داخل گردن رحم با حسایت بالا مفید باشد.

High implantation success following in vitro fertilization cycles are achieved via the transfer of embryos with the highest developmental competence. Multiple pregnancies as a result of the transfer of several embryos per cycle accompany with various complication. Thus, single-embryo transfer (SET) is the preferred practice in assisted reproductive technique (ART) treatment. In order to improve the pregnancy rate for SET, embryologists need reliable biomarkers to aid their selection of embryos with the highest developmental potential. Time-lapse technology is a noninvasive alternative conventional microscopic assessment. It provides uninterrupted and continues the survey of embryo development to transfer day. Today, there are four time-lapse systems that are commercially available for ART centers. In world and Iran, the first time lapse babies were born in 2010 and 2015, respectively, conceived by SET. Here, we review the use of time-lapse monitoring in the observation of embryogenesis as well as its role in SET. Although, the findings from our review support common use of time-lapse monitoring in ART centers; but, future large studies assessing this system in well-designed trials are necessary.

Protective effects of melatonin and ghrelin on spermatogenesis: A narrative review of the literaturePages 265-272
Spermatocytogenesis starts from lumens of seminiferous cords and after migration to the basal membrane ends to the lumens again. We attempt to review the protective effects of melatonin and ghrelin on Spermatocytogenesis and in particular on spermatogonial stem cells, as two rather newly-discovered hormones. Testicular freezing prior to chemotherapy and radiotherapy is one of the ways of preserving fertility in children with cancer. The freezing has two methods of slow-freezing (cryopreservation) and rapid-freezing (vitrification). Administration of melatonin can maintain the quality of the germ cells underwent such processes, as well as ghrelin, can protect germ cells from the toxicities secondary to ischemic injuries, and pathologic apoptosis. This review indicates that in vitro or in vivo administration of melatonin or ghrelin, could be effective to preserve fertilization and also they can be used in assisted reproductive technologies to improve the quality of sperms. Future original studies should be propelled toward human studies, of course with observing the ethics.

Several studies have reported the uncertain role of multi-minerals and vitamins in the prevention of preeclampsia.
ObjectiveThe present study aims to investigate the effect of multimineral-vitamin D supplements (calcium, magnesium, zinc and Vitamin D) and vitamins (C E) in the prevention of preeclampsia.
Materials And MethodsIn this randomized clinical trial, 90 pregnant women were divided into three groups: group A received Ferrous sulfate (1 tablet/day) one tablet of Claci-care multimineral-vitamin D containing 800mg calcium, 200mg magnesium, 8mg zinc and 400 IU Vitamin D3 per day; group B received Ferrous sulfate (1 tablet/day) 250 mg vitamin C 55 mg vitamin E; and the controls received only one Ferrous sulfate tablet daily.
ResultsThe incidence of preeclampsia in group A was significantly lower than the control group (p=0.03), while there was no significant difference between group B and controls (p=0.50), as well as groups A and B (p=0.063). The incidence of neonatal complications in the group A was significantly lower than the control group (p=0.01), while there was no significant difference between group B and control (p=0.48).
ConclusionAccording to the results, calcium, magnesium, and zinc supplements have a significant effect on the prevention of preeclampsia. In addition, prescription of multimineral-vitamin D during pregnancy can be a low-cost and affordable way to reduce the incidence of preeclampsia in women who are at high risk of preeclampsia.

Teratoasthenozoospermia (TA) is a severe form of male infertility with no clear etiology.
ObjectiveTo compare the level of intracellular anion superoxide (O2), heat shock protein A2 (HSPA2) and protamine deficiencies in ejaculated spermatozoa between teratoasthenozoospermic and normozoospermic men.
Materials And MethodsIn this case- control study, semen samples of 20 infertile men, with TA (with normal morphology lower than 4%_ and total motility lower than 40% ) as the case group and 20 normozoospermic fertile men as the control group were evaluated for intracellular O2 and HSPA2 by flow cytometry and protamine deficiency by Chromomycin A3 (CMA3) test.
ResultsThe rate of CMA3 spermatozoa in the case group was higher than controls (p=0.001). The percentages of HSPA2 spermatozoa in the cases were significantly lower than controls (p=0.001). Also, intracellular O2 levels in the case group were significantly higher than controls (p=0.001) and had positive correlations with sperm apoptosis (r=0.79, p=0.01) and CMA3 positive sperm (r=0.76, p=0.01), but negative correlations with normal morphology (r=-0.81, p=0.01) and motility (r=-0.81, p=0.01). There was no significant correlation between intracellular O2 and HSPA2 in the case group (r=0.041, p=0.79).
ConclusionWe suggest that the increase in intracellular O2 , decrease in spermatozoa HSPA2, and high percentages of spermatozoa with immature chromatin might be considered as etiologies of infertility in TA patients.

The hospital anxiety and depression scale (HADS) is a common screening tool designed to measure the level of anxiety and depression in different factor structures and has been extensively used in non-psychiatric populations and individuals experiencing fertility problems.
ObjectiveThe aims of this study were to evaluate the factor structure, item analyses, and internal consistency of HADS in Iranian infertile patients.
Materials And MethodsThis cross-sectional study included 651 infertile patients (248 men and 403 women) referred to a referral infertility Center in Tehran, Iran between January 2014 and January 2015. Confirmatory factor analysis was used to determine the underlying factor structure of the HADS among one, two, and threefactor models. Several goodness of fit indices were utilized such as comparative, normed and goodness of fit indices, Akaike information criterion, and the root mean squared error of approximation. In addition to HADS, the Satisfaction with Life Scale questionnaires as well as demographic and clinical information were administered to all patients.
ResultsThe goodness of fit indices through CFAs exposed that three and onefactor model provided the best and worst fit to the total, male and female datasets compared to the other factor structure models for the infertile patients. The Cronbachs alpha for anxiety and depression subscales were 0.866 and 0.753 respectively. The HADS subscales significantly correlated with SWLS, indicating an acceptable convergent validity.
ConclusionThe HADS was found to be a three-factor structure screening instrument in the field of infertility.

Selection of the best embryo for transfer is very important in assisted reproductive technology (ART). Using morphological assessment for this selection demonstrated that the correlation between embryo morphology and implantation potential is relatively weak. On the other hand, aneuploidy is a key genetic factor that can influence human reproductive success in ART.
ObjectiveThe aim of this lab trial study was to evaluate the incidence of aneuploidies in five chromosomes in the morphologically high-quality embryos from young patients undergoing ART for sex selection.
Materials And MethodsA total of 97 high quality embryos from 23 women at the age of 37or younger years that had previously undergone preimplantation genetic screening for sex selection were included in this study. After washing, the slides of blastomeres from embryos of patients were reanalyzed by fluorescence in-situ hybridization for chromosomes 13, 18 and 21.
ResultsThere was a significant rate of aneuploidy determination in the embryos using preimplantation genetic screening for both sex and three evaluated autosomal chromosomes compared to preimplantation genetic screening for only sex chromosomes (62.9% vs. 24.7%, p=0.000). The most frequent detected chromosomal aneuploidy was trisomy or monosomy of chromosome 13.
ConclusionThere is considerable numbers of chromosomal abnormalities in embryos generated in vitro which cause in vitro fertilization failure and it seems that morphological characterization of embryos is not a suitable method for choosing the embryos without these abnormalities.

Human papilloma virus (HPV) is one of the major public health problems and the main causes of cervical cancer. The prevalence HPV infection in developing countries with low financial resources is high.
ObjectiveThis study aimed to determine the relative frequency of HPV genotypesand its sociodemographic characteristics in women referred to a general hospital in Tehran, Iran from 2014-2015.
Materials And MethodsThis cross-sectional study was performed in 400 women with Pap smear samples, referring to to a general hospital in Tehran, Iran from 2014- 2015. The detection of 28 HPV genotypes was performed by using the Multiplex PCR technique. The sociodemographic survey was conducted for each HPV positive woman.
ResultsHPV-positive infection was detected in 155 (38.75%) women aged 17 85 years. HPV 16 (19.1%) was the most prevalent type, followed by HPV 39 (12.5%) and HPV 18 (8.9%). The highest rate of HPV infection was observed at the age of 36 years (7.7%). The level of education and economic situation of each woman were showed most of HPV-positive women had a high school diploma (34.6%) and average economic situation (67,9%). 60.9% of these women were a housewife, and 67.3% lived in the capital .
ConclusionDetermination of HPV genotype and risk factor related to HPV infection in each geographical region can lead to the production of effective vaccines against the HPV virus. It can also be useful for disease management and high sensitivity diagnosis of cervical intraepithelial neoplasia.
