    Background
    An appropriate anesthesia duration with minimal side effects and prolonged postoperative analgesia are the ideal characteristics of an intrathecal drug used during spinal anesthesia. Neostigmine and magnesium sulphate have been used as spinal anesthetic additives with narcotics and local anesthetics..
    Objectives
    This study aimed to assess the analgesic properties of intrathecal neostigmine and magnesium sulphate by adding them to intrathecal bupivacaine-fentanyl..
    Methods
    : In total, 210 patients undergoing tibial fracture surgery were enrolled in a double-blinded clinical trial study. Patients were randomly allocated to one of three groups: group F received 10 mg of bupivacaine and 25 µg of fentanyl as intrathecal drug for spinal anesthesia, group N received 150 µg of neostigmine added to 10 mg of bupivacaine and 25 µg of fentanyl, and group M received 50 mg of magnesium sulphate added to 10 mg of bupivacaine and 25 µg of fentanyl. Analgesia duration, motor blockade scores, postoperative pain scores 6 and 12 hours after surgery, postoperative voiding time, and the incidence of hypotension, bradycardia, respiratory depression, and nausea and vomiting were recorded..
    Results
    Group M showed significantly longer analgesia duration (330.76 ± 80.98 minutes) than group F (280.98 ± 60.33 minutes). The pain scores in group M 6 hours (NRS: 2.44 ± 0.98) and 12 hours (NRS: 4.10 ± 0.88) after surgery were significantly lower than those of the other two groups. Before discharge from recovery, motor blockade scores and voiding time were not significantly different between the three groups. Hypotension (40%), bradycardia (25%), and nausea and vomiting (70%) were more obvious among group N patients. Respiratory depression did not occur in any patients..
    Conclusions
    The addition of 50 mg of magnesium sulfate to a bupivacaine–fentanyl solution for intrathecal anesthesia improved the efficacy and duration of the analgesia without any significant side effects. The addition of 150 µg of neostigmine increased the incidence of hypotension, bradycardia, and nausea and vomiting. Moreover, neostigmine failed to prolong analgesia duration..

    Objectives
    The aim of the present study was to compare the analgesic effects of paracetamol and ketorolac in VATS patients..
    Methods
    This was a double-blinded randomized clinical trial conducted on 70 patients undergoing lobectomy or segmentectomy due to lung masses, using video-assisted methods. The patients were randomly divided into two groups (each n = 35): the ketorolac (K) group and the paracetamol (P) group. The K group received ketorolac 30 mg IV stat at the end of surgery and then a 90 mg/24 h infusion. The P group received paracetamol 1 g IV stat at the end of surgery and then a 3 g/24 h infusion. Pain scores were recorded during recovery and 2, 4, 8, 12, and 24 hours after drug administration. Pain scores, total doses of rescue analgesics, and patient satisfaction levels were compared between the groups..
    Results
    There was no significant difference between the K and P groups in pain scores in any of the evaluations. Seventeen (48.6 %) and 9 (25.7 %) patients in the K and P groups, respectively, did not require any rescue analgesia (P = 0.047). The mean doses of rescue analgesia in the K and P groups were 3.129 ± 4.27 mg and 4.38 ± 3.69 mg, respectively, which were similar (P = 0.144). There was no significant difference between the groups in satisfaction scores (P = 0.175)..
    Conclusions
    Paracetamol 1 g stat 3 g/24 h infusion is as effective as ketorolac 30 mg stat 90 mg/24 h infusion in post-VATS pain management, with good tolerability and a low incidence of adverse effects..

    Background
    The goal of this study was to evaluate the impact of intubation skill training involving the use of mannequins on novice anesthesiology residents in a knowledge, attitudes, and practices designed study in which three different types of evaluation were implemented..
    Methods
    All first-year anesthesiology residents (24) of Sina Hospital, affiliated to the Tehran University of Medical Sciences, were invited to participate in an intubating skills training course. The program comprised two theoretical and three practical sessions, lasting a total of 16 hours over four days. Faculty assessment of residents’ practices was carried out using the questionnaire results, measured using a Likert scale, as the primary outcome. An improvement in the theoretical knowledge of the novice anesthesiology residents (using the Likert scale) and their attitudes towards the educational course in general (via a multiple choice question examination), were also evaluated..
    Results
    The mean score following faculty assessment of the residents’ practical skills was 4.6 out of 5.0 (92%) [standard deviation (SD) of 0.13]. The mean score with respect to the attitudes of the residents was 4.8 out of 5.0 (96%) (SD of 0.16). The overall mean theoretical score of the residents improved significantly upon completion of the training program (P = 0.001)..
    Conclusions
    Our results suggest that the personnel in the five participating faculties were highly satisfied with the practical performance of the residents, who were found to hold good attitudes towards the program as a whole..

    Objectives
    The aim of the present study was to assess and compare the sociodemographic characteristics and clinical features of patients referring to a university hospital’s pain clinic with chronic ( ≥ 12 weeks) and subacute pain (
    Methods
    In this cross-sectional study, 426 patients were included. Demographic variables including education level, marital and employment status, and risk factors such as obesity, diabetes mellitus, hypertension, cigarette smoking, and opium addiction were recorded. Also, sites of pain, pain quality and associated symptoms, and pain severity were assessed using a numerical rating scale. Each one of these variables was compared between the chronic and subacute pain groups..
    Results
    Of the 426 studied patients, 292 (69%) had chronic pain and 134 (31%) reported subacute pain. Patients with chronic pain were older and had higher body mass indices. Additionally, self-employment was less frequent among the chronic pain group. The patients with chronic pain had a higher prevalence of addiction. The most commonly reported site of pain in all patients was the lower back (62.4%), followed by pain in the leg and foot (39.9%), knee (24.4%), and hip (18.8%). There were no statistically significant differences in pain sites between the two groups, except for knee pain, which was more common among the chronic pain group. The patients with chronic pain had a higher incidence of obscure and persistent pain, while those with subacute pain experienced more night pain..
    Conclusions
    About one-third of the patients referring to the pain clinic had subacute pain. The patients with chronic pain were older and more obese, had a higher prevalence of addiction, had more cases of knee pain, and reported more instances of obscure and persistent pain than those with subacute pain.

    Background
    One of the main challenges in anesthesiology is difficult intubation. There are many anatomical parameters for evaluating the feasibility of tracheal intubation; one that can reliably predict a difficult intubation is the Cormack-Lehane classification obtained during direct laryngoscopy. This is an invasive procedure that cannot be performed in an awake patient or for pre-anesthetic airway assessments in patients with no prior history of tracheal intubation.     Objectives
    The aim of this study was to compare and correlate the ultrasound view of the larynx with the Mallampati classification before anesthesia and the Cormack-Lehane classification during direct laryngoscopy under general anesthesia..
    Methods
    This cross-sectional descriptive-analytic study included 53 ASA class I - III patients aged 18 - 70 years who were scheduled for tracheal intubation under general anesthesia. Before anesthesia, an oblique transverse ultrasound view of the airway was obtained; in addition, the total time taken to achieve the final plane, the depth of the pre-epiglottic space, and the distance from the epiglottis to the mid-point between the vocal cords were all recorded. The ultrasound measurements were then compared with the Mallampati class on the preoperative evaluation and with the Cormack-Lehane grade during direct laryngoscopy under general anesthesia..
    Results
    It was observed that correlations between the pre-epiglottic space (PE) and Cormack-Lehane grades I, II, and III were weak. Correlations between the distance from the epiglottis to the vocal cords (E-VC) and Cormack-Lehane grades I, II, and III were also weak. The PE/E-VC ratio for correlations between the sonographic view and laryngoscopy had 87.5% sensitivity and 30% specificity. There was no correlation between Mallampati class and the PE/E-VC ratio (P = 0.566)..
    Conclusions
    Our study revealed weak correlation between PE/E-VC and Cormack-Lehane grade, with 87% sensitivity and 30% specificity. Therefore, we concluded that sonographic measurement criteria are not accurate in airway evaluations before anesthesia..

    Background

    Patient and
    Methods
    This was a prospective, double blinded randomized study conducted in a tertiary hospital in Malaysia. Patients (n = 64) were randomly allocated to receive 2 Hertz EA and compared to a control group. EA was started intraoperatively till the end of the surgery (mean duration of surgery was 149.06 ± 42.64 minutes) under general anaesthesia. Postoperative numerical rating scale (NRS), the incidence of nausea, vomiting and usage of rescue antiemetics were recorded at 30 minutes, 2, 4, and 24 hours, respectively. The total morphine demand and usage from the patient-controlled analgesia Morphine (PCAM) were also recorded in the first 24 hours postoperatively..
    Results
    The mean NRS was 2.75 (SD = 2.34) at 30 minutes and 2.25 (SD = 1.80) at 2 hours postoperatively in the EA group that was significantly lower than the mean NRS in the control group as 4.50 (SD = 2.37) at 30 minutes and 3.88 (SD = 2.21) at 2 hours. The mean PCA morphine demand was 27.28 (SD = 21.61) times pressed in the EA group and 55.25 (SD = 46.85) times pressed in the control group within 24 hours postoperatively, which showed a significant reduction in the EA group than the control group. Similarly, total morphine requirement was significantly lower in the EA group with the value of 21.38 (SD = 14.38) mg compared to the control group with the value of 33.94 (SD = 20.24) mg within 24 hours postoperatively. Incidence of postoperative nausea also significantly reduced in the EA group at 30 minutes (15.6%) compared to the control group (46.9%)..
    Conclusions
    It can be concluded that subjects receiving EA intraoperatively experienced less pain and PONV. Hence, it is plausible that EA has an opioid-sparing effect and can reduce PONV.

    Methods
    In a crossover study, patients under mechanical ventilation in neurosurgical ICU were enrolled. The patients alternatively experienced two types of ventilations for 30 minutes (adaptive support ventilation and synchronized intermittent mandatory ventilation). The respiratory parameters (tidal volume, respiratory rate, airway pressure, lung compliance, end-tidal carbon dioxide, peripheral oxygenation and respiratory dead space), hemodynamic variables, every 10 minutes and arterial blood gas analysis at the end of each 30 minutes were recorded.     Fifty four patients who were scheduled for ultra-sound guided intra-articular injection were enrolled in the study. The patients were randomly allocated to groups P (platelet rich plasma) and PS (platelet rich plasma and Somatropin). Group P and PS were injected with 5 mL of platelet rich plasma, and 4 IU growth hormone (Somatropin) added to platelet rich plasma, respectively. Intra-articular injection was performed in two steps; the onset of study and one month after.     Background
    Chronic knee pain from osteoarthritis (OA) is common in the aging and the obese population. Radiofrequency ablation of the genicular nerves has been introduced as a potential surgery-sparing treatment for chronic knee pain from OA, yet only two outcome studies have been published and optimal patient selection for this procedure has not been established..

    We describe a standardized protocol for selecting patients for cooled radiofrequency ablation (C-RFA) of the genicular nerves, as well as the clinical outcomes of four patients ages 63-65 years..
    Methods
    The threshold for selection based on diagnostic genicular nerve block was ≥ 80% pain reduction. Following successful block, C-RFA of the genicular nerves was performed.     Twenty diabetic patients were randomly allocated to two groups (n = 10).     There was no statistically significant difference between the blood glucose levels in groups A and B up to 24 hours postoperatively (P = 0.088). A statistically significant difference in the blood glucose level was not demonstrated 24 hours after surgery in comparison with the baseline level in Group A.     Background
    Pressure and waveform at the catheter tip are continuously monitored during catheterization of pulmonary artery to ensure accurate catheter placement. We present a case in which pulmonary venous blood was unexpectedly collected from the pulmonary artery catheter despite pulmonary artery pressure and waveform detection at the catheter tip, and describe the measures taken to correct the catheter placement..
    Case Presentation
    A 74-year-old male underwent mitral valve plasty for cardiac failure caused by mitral valve regurgitation. Preoperative transthoracic echocardiography showed no septal shunt. The pulmonary artery was catheterized through a sheath introducer in the right jugular vein, and the balloon was inflated after insertion of a 15-cm catheter. The catheter was advanced until a pulmonary artery waveform was detected and the pulmonary artery wedge pressure was 21 mmHg at end-expiration. The balloon was deflated and the catheter tip was pulled back 3 cm. Pulmonary artery waveforms and appropriate a and v waves were detected, and transesophageal echocardiography confirmed the location of the catheter tip in the right pulmonary artery. The first collected blood sample had an oxygen partial pressure of 358.8 mmHg, carbon dioxide partial pressure of 20.1 mmHg, and oxygen saturation of 99%, indicating pulmonary venous blood. The pulmonary artery catheter was pulled back 5 cm, but a second blood sample showed the same results. The catheter was pulled back a further 6 cm while the location of the catheter tip was monitored on X-ray fluoroscopy. Blood gas testing through the catheter tip showed oxygen saturation of 84.4 % and oxygen partial pressure of 41.6 mmHg. Surgery was performed uneventfully. Postoperative chest radiographs showed proper placement of the pulmonary artery catheter, but radiographs on postoperative day 1 showed over-insertion, although the insertion length was unchanged. The catheter was removed. The patient was discharged 2 months postoperatively..
    Conclusions
    Our case highlights the fact that the tip of the pulmonary artery catheter can easily advance into a peripheral branch of the pulmonary artery and cause pulmonary venous blood to be sampled instead of pulmonary arterial blood. A variety of monitoring techniques are needed to confirm accurate catheter placement..
