    زمینه و هدف
    با توجه به اهمیت هیپرلاکتاتمی پس از جراحی، تاثیر مایع درمانی در میزان آن و چالش های متفاوت در مورد محلول های کریستالوئیدی و کلوئیدی در مایع درمانی، در این مطالعه بر آن شدیم تا با مقایسه تغییرات سطح لاکتات سرمی بیماران تحت آپاندکتومی، اثربخشی سرم های هماکسل و رینگر را بر سطح لاکتات خون را بررسی نمائیم.
    مواد و روش ها
    168 بیمار که کاندید عمل جراحی آپاندکتومی بودند پس از اخذ رضایت آگاهانه ب طور تصادفی در دو گروه استفاده کننده از سرم هماکسل و یا رینگر قرار داده شدند و از بیماران قبل و یک ساعت بعد از عمل 2 میلی لیتر نمونه خون گرفته شد و میزان لاکتات سرمی در آن مورد بررسی قرار گرفت. اطلاعات جمع آوری شده با استفاده از نرم افزار SPSS مورد تجزیه و تحلیل قرار گرفتند.
    یافته ها
    نتایج نشان داد که سرم رینگر به طور معنی داری (0/0001=P) سطح لاکتات سرمی بعد از عمل را کاهش داده ولی سرم هماکسل باعث افزایش سطح لاکتات بعد از عمل شده است (0/0001=P).
    نتیجه گیری
    با توجه به نتایج مطالعه حاضر می توان گفت که استفاده از سرم رینگر برای مایع درمانی بیماران تحت آپاندکتومی برای کاهش سطح لاکتات پس از جراحی نسبت به سرم هماکسل ترجیح دارد.

    زمینه و هدف
    عمل جراحی یک عامل اضطراب آور است که باعث ایجاد واکنش فیزیولوژیک در بدن می شود. در نتیجه پاسخ فیزیولوژیکی بدن، تنفس، ضربان قلب و فشار خون افزایش می یابد. مطالعه حاضر با هدف بررسی تاثیر موسیقی بر برخی شاخص های فیزیولوژیک بیماران قبل از اعمال جراحی انجام شد.
    مواد و روش ها
    این پژوهش یک مطالعه مداخله ای از نوع کارآزمایی بالینی تصادفی شده شاهددار می باشد که بر روی 60 بیمار تحت عمل های جراحی بی حسی نخاعی در بیمارستان های وابسته به دانشگاه علوم پزشکی ایلام در سال 1392 انجام گردید. شرایط ورود به مطالعه شامل تمایل بیمار جهت شرکت در مطالعه، عمل های غیر اورژانسی، قرار داشتن در گروه سنی 20 تا 40 سال، عمل با بی حسی نخاعی برای اولین بار و همچنین داشتن ثبات همودینامیک (فشار خون سیستولیک بیشتر از 90 میلی متر جیوه، عدم وجود دیس ریتمی های خطرناک و ضربان قلب بین (60 تا100) بود(3،4). در گروه مورد علائم حیاتی بیمار که شامل فشارخون سیستولیک، فشار خون دیاستولیک، تعداد نبض، تعداد تنفس و همچنین میزان اشباع خون شریانی می باشد قبل از مداخله ارزیابی و ثبت شده و بعد بیمار توسط متخصص بیهوشی تحت بی حسی نخاعی قرار گرفته و بلافاصله آوای بدون کلام توسط دستگاه پخش موسیقی ام پی تری پلیر و گوشی مخصوص (هدفون) یک بار مصرف به مدت 10 دقیقه برای بیمار پخش گردید و بلافاصله بعد از اتمام این زمان دوباره علائم حیاتی و میزان اشباع خون شریانی بیمار ارزیابی و ثبت گردید. برای گروه شاهد همین اندازه گیری ها بدون پخش موسیقی انجام گردید.
    یافته ها
    مقایسه میانگین شاخص های فیزیولوژیک قبل و بعد از مداخله، اختلاف معنی داری را نشان داد به طوری که پخش آوای بدون کلام بر میانگین تعداد نبض، تعداد تنفس، فشار خون سیستولیک، فشارخون دیاستولیک، تاثیر چشم گیری داشت (0/50>P))، اما بر روی درصد اشباع اکسیژن خون شریانی تاثیری مشاهده نشد (0/50>P)
    نتیجه گیری
    شنیدن آوای بدون کلام، بر روی فشارخون سیستولیک و دیاستولیک و ضربان قلب افراد اثر چشمگیری داشت، اما بر روی درصد اشباع اکسیژن خون شریانی تاثیری نداشت.

    زمینه و هدف
    اعمال جراحی ارتوپدی به دلیل ماهیت جراحی روی استخوان و مفاصل با درد پس از عمل بالایی همراهند. استفاده از مخدر های وریدی، عضلانی، داخل نخاعی و اپیدورال همگی از روش های کنترل درد پس از عمل هستند که دارای عوارض جانبی متعددی می باشند. تجویز منیزیوم داخل نخاعی با بلوک گیرنده آن «متیل دی آسپارتات» باعث کاهش درد پس از عمل می شود، بدون آنکه با افزایش عوارض جانبی همراه باشد. هدف از انجام این مطالعه تعیین تاثیر اضافه کردن سولفات منیزیوم به لیدوکائین- اپی نفرین داخل نخاعی در مدت بی دردی پس از عمل، در جراحی های ارتوپدی اندام تحتانی است.
    مواد و روش ها
    این مطالعه به صورت کارآزمایی بالینی دوسوکور در بیماران کاندید عمل ارتوپدی اندام تحتانی انجام شده است. برای این منظور 156 بیمار به دو گروه تقسیم شدند، که یک گروه سولفات منیزیوم همراه با لیدوکائین-اپی نفرین و گروه دیگر لیدوکائین-اپی نفرین همراه با آب مقطر به عنوان پلاسبو دریافت نمود. مدت زمان بی دردی بر اساس فاصله زمانی درخواست مسکن توسط بیمار از زمان تزریق دارو، درد بیماران بر اساس مقیاس دیداری درد و میزان پتیدین مصرفی در دو گروه محاسبه شد. اطلاعات توسط نرم افزار آماری SPSS 12 آنالیز گردید.
    یافته ها
    در مطالعه ما میانگین سنی بیماران در دو گروه درمانی و کنترل به ترتیب 7/5±34 و 1/8±9/31 سال بود. میانگین مدت زمان بی دردی در گروه درمانی، 143.7±29.7 دقیقه و در گروه کنترل، 100.9±18.1 دقیقه بود که مدت زمان بی دردی به طور معنی داری در گروه سولفات منیزیوم طولانی تر از گروه کنترل بوده است. میزان درد بیماران بر اساس مقیاس دیداری درد در گروه سولفات منیزیوم 36/0±9/3 و در گروه کنترل 5.65±0.35 بود، که به طور معنی داری در گروه سولفات منیزیوم، کمتر از گروه کنترل بوده است (0/000=P). میانگین پتیدین مصرفی در گروه سولفات منیزیوم 24/6 ±15 میلی گرم و در گروه کنترل 33±6.27 میلی گرم بود که از نظر آماری معنی دار بود (0/000=P.
    نتیجه گیری
    سولفات منیزیوم همراه با لیدوکائین موجب کاهش درد پس از عمل بیشتری در مقایسه با لیدوکائین تنها می شود.

    زمینه و هدف
    روش اداره چند وجهی برای بهبود درد پس از عمل پیشنهاد می شود. پره گابالین ب عنوان یک آنالوگ گاما آمینو بوتیریک اسید نشان داده شده است که اثرات تسکینی بر درد پس از عمل دارد. این مطالعه تصادفی شده دوسو کور اثر پیش درمانی با پره گابالین را بر درد و مصرف مخدر پس از عمل در بیماران تحت اعمال جراحی الکتیو ارتوپدی مورد ارزیابی قرار داده است.
    مواد و روش ها
    60 بیمار با کلاس انجمن بیهوشی امریکا 1 یا 2 که کاندید اعمال جراحی الکتیو ارتوپدی تحت بی حسی نخاعی بودند، به صورت تصادفی در 2 گروه قرار گرفتند. گروه اول، 300 میلی گرم پره گابالین و گروه 2 کپسول دارونما دریافت می کردند. پمپ کنترل کننده بی دردی با کنترل بیمار آماده شده با سوفنتانیل جهت درد پس از عمل به هر 2 گروه متصل می شد. نمره بندی عددی درد، مصرف مخدر و عوارض جانبی نظیر تهوع به مدت 12 ساعت پس از عمل ثبت گردید. (0/05> P) معنی دار تلقی می شد.
    یافته ها
    مقیاس دیداری درد در ریکاوری، 6 و 12 ساعت پس از عمل در گروه پره گابالین به میزان قابل ملاحظه ای کمتر از گروه دارونما بود (0/05> P). مصرف پتیدین هم به شکل معنی داری در گروه پره گابالین کمتر بود (0/05> P)
    نتیجه گیری
    تجویز تک دوز خوراکی 300 میلی گرم پره گابالین یکساعت قبل از اعمال جراحی انتخابی ارتوپدی تحت بی حسی نخاعی، درد و مصرف مخدر پس از عمل را کاهش می دهد.

    زمینه و هدف
    بررسی و مدیریت درد یک موضوع مهم در بخش ویژه می باشد. با این وجود، اسناد محدودی در مورد بررسی و کنترل درد در بیماران غیرهوشیار وجود دارد. هدف از این مطالعه بررسی تاثیر یک برنامه مدیریت درد بر کنترل درد در بیماران با کاهش سطح هوشیاری بستری در بخش های ویژه می باشد.
    مواد و روش ها
    پژوهش حاضر یک کارآزمایی بالینی دو گروهی قبل و بعد و به صورت مرحله به مرحله است. در این مطالعه 50 بیمار بستری در بخش مراقبت های ویژه به روش نمونه گیری آسان انتخاب و بطور تصادفی در دو گروه قرار گرفتند. برنامه مدیریت درد به مدت 48 ساعت یا تا زمانیکه سطح هوشیاری بیمار (مقیاس گلاسکو کما) به بالاتر از 8 برسد، اجرا شد. برای گروه کنترل، اقدامات روتین بخش انجام شد و برای گروه مورد، نمره شدت درد آنها با استفاده از ابزار مقیاس درد غیر کلامی تعیین، و بعد براساس نمره محاسبه شده، مسکن انتخابی (مورفین یا فنتانیل) برای آنها طبق برنامه انفوزیون شد. در هر دو گروه با استفاده از ابزار مقیاس درد غیر کلامی، نمرات شدت درد، قبل و بعد از اقدام درمانی ثبت شد. اطلاعات با نرم افزار 16spss مورد تجزیه و تحلیل قرار گرفت. برای تجزیه و تحلیل داده ها، آزمون های تی مستقل، مجذور کای و آنالیز متغیر با مشاهدات تکراری مورد استفاده قرار گرفت.
    یافته ها
    نتایج نشان داد که میانگین نمره شدت درد در گروه کنترل به نسبت گروه آزمون بعد از مداخله، بعد از مرحله اول، بیشتر بوده است و میانگین تغییر نمرات شدت درد در گروه آزمون با استفاده از آزمون تی مستقل ب طور معنی داری کمتر از گروه کنترل می باشد (0/4= P).
    نتیجه گیری
    یافته های این مطالعه نشان داد که با ب کارگیری برنامه مدیریت درد، می توان درد این گونه بیماران را بطور مناسبی کنترل کرد.

    زمینه و هدف
    گاز بیهوشی نایتروس اکساید رایج ترین گاز بیهوشی مورد استفاده در اتاق های عمل است که به عنوان ماده بیهوشی دهنده و ضد درد ضعیف ب کار می رود. سطح کورتیزول سرم بعنوان یکی از نشانگرهای استرس بوده و بالا بودن این فاکتور می تواند حاکی از ایجاد حالات استرسی و عصبی بشمار آید. هدف از مطالعه بررسی ارتباط غلظت عوامل بیهوشی (نیتروس اکساید) بر مقدار کورتیزول ادرار کارکنان اتاق عمل می باشد.
    مواد و روش ها
    این مطالعه از نوع توصیفی مقطعی و همبستگی می باشد. جامعه پژوهش را پرسنل اتاق عمل (30نفر) و پرسنل بخش اداری (30نفر) بیمارستان بعثت همدان تشکیل دادند. ابتدا غلظت گاز نیتروس اکساید در اتاق های عمل بیمارستان بعثت در طی یک هفته کاری در نوبت صبح و بعدازظهر اندازه گیری شد. جهت بررسی میزان تاثیر استنشاق و جذب عوامل شیمیایی مضر بر فاکتو رهای نوروبیولوژیک نیز در آغاز و پایان هر روز کاری (بمدت 5 روز) از هر گروه آزمون و کنترل نمونه ادرار گرفته شد و اندازه گیری سطح کورتیزول انجام شد.
    یافته ها
    غلظت عوامل بیهوشی و ضد عفونی کننده در فضای اتاق عمل 388.26 ± 101.4 پی پی ام بود. سطح کورتیزول در نمونه ادرار پرسنل شاغل در اتاق عمل، قبل و بعد از یک روز بمدت 5 روز 648.4± 2.74 پی پی ام بود. میانگین سطح کورتیزول در نمونه ادرار پرسنل شاغل در بخش اداری (ب عنوان گروه کنترل)، قبل و بعد از یک روز بمدت 5 روز 579.2 ± 3.1 بود (0/05>P). از طرفی یافته های پ‍ژوهش نشان می دهد بین میانگین تراکم گاز نایتروس اکساید محیط تنفسی کارکنان اتاق های عمل و ریکاوری بر حسب 3 زمان نمونه برداری در یک روز با توجه به آزمون آماری آنووا (0/05> P) اختلاف معنی داری (0/028=P) وجود دارد. ضمنا در مقایسه میانگین و انحراف معیار کورتیزول آزاد ادرار بر حسب نانو گرم بر میلی لیتر بین دو گروه شاهد و کنترل با توجه به آزمون تی (0/599= P) اختلاف معنی داری مشاهده نگردید(P< 0/05).
    نتیجه گیری
    در نهایت یافته های پ‍ژوهش طبق آزمون ضریب همبستگی پیرسون نشان میدهد که ارتباط مستقیم و مثبت و ناقصی بین غلظت گاز نیتروس اکساید در فضای اتاق عمل و کورتیزول آزاد ادرار (نانو گرم بر میلیلیتر) پرسنل اتاق عمل وجود دارد (r=0/875) (P=0/004). این بدین معنی است که با افزایش میزان غلظت گاز نیتروس اکساید در فضای اتاق عمل میزان کورتیزول آزاد ادرار پرسنل اتاق عمل افزایش می یابد (P< 0/05).

    مقدمه
    تهوع و استفراغ بعد از عمل جراحی عارضه نسبتا شایعی است که باعث ناخوشنودی بیمار، افزایش زمان بستری و افزایش هزینه ها و در موارد شدید باعث دهیدراتاسیون، اختلال الکترولیتی، آسپیراسیون، پنومونی و حتی باز شدن بخیه های محل جراحی می شود. انتخاب وسیله ای که برای اداره راه هوایی استفاده می شود، ب دلیل ایجاد تحریک در بخش های مختلف راه های هوایی، ممکن است تهوع و استفراغ بعد از عمل را تحت تاثیر قرار دهد. لذا برآن شدیم تا میزان بروز تهوع و استفراغ در استفاده از ماسک راه هوایی حنجره ای و انتوباسیون تراشه در بیماران تحت جراحی استرابیسم، را با هم مقایسه نماییم.
    مواد وروش ها
    160 بیمار کاندید عمل جراحی استرابیسم تحت بیهوشی عمومی در مطالعه قرار گرفتند. بیماران به طور تصادفی در دو گروه انتوباسیون با لوله تراشه و تعبیه ماسک راه هوایی حنجره ای قرار گرفتند و سپس شدت تهوع و بروز استفراغ در ریکاوری، 6 و 18 ساعت بعدازعمل طبق معیار کلامی مورد ارزیابی قرار گرفتند.
    یافته ها
    دو گروه از لحاظ اطلاعات دموگرافیک (سن، وزن، جنسیت، علائم همودینامیک (ب جز ضربان قلب در زمان های قبل از القای بیهوشی، دقیقه 5 و 15 بعد از القای بیهوشی)، طول مدت جراحی، تعداد عضلات تحت جراحی و تعداد چشم های تحت جراحی با یکدیگر یکسان بودند) بروز تهوع و استفراغ بعد از عمل در ریکاوری، 6 ساعت و 18 ساعت بعد از عمل در دو گروه یکسان بود.
    نتیجه گیری
    طبق نتایج این مطالعه استفاده از لوله تراشه و یا ماسک راه هوایی حنجره ای در اداره راه هوایی، تاثیری بر بروز تهوع و استفراغ بعد از عمل ندارند.

    زمینه و هدف

    با توجه به اهمیت بی دردی بیماران حین و پس از اعمال جراحی ارتوپدی، تحقیق حاضر با هدف مقایسه اثرات بی دردی و مصرف داروی مخدر آپوتل (استامینوفن تزریقی) پس از عمل شکستگی های شفت ساعد طراحی شده است.

    مواد و روش ها

    تعداد 160 بیمار مبتلا به شکستگی شفت ساعد به طور کاملا تصادفی به دو گروه آپوتل و دارونما تقسیم می شوند. گروه آپوتل: 1 گرم آپوتل در 200 میلی لیتر نرمال سالین و در گروه دارونما: تنها 200 میلی لیتر نرمال سالین 20 دقیقه قبل عمل استفاده شد. مقدار مخدر مصرفی حین عمل و طی 24 ساعت بعد و نیز مدت زمان درخواست اولین مسکن بعد از عمل ثبت می گردد. از معیار مقیاس دیداری درد جهت تعیین نمره درد بیماران بعد از عمل استفاده شد.

    یافته ها

    میانگین مصرف مخدر حین عمل (فتنانیل بر حسب میکروگرم) در گروه آپوتل 51/42-/+2/3 و در دارونما 70/10-/+3/6بود که با توجه به (0/5>P) اختلاف معنی داری بین 2 گروه دیده شد.این میانگین در گروه آپوتل کمتر بود.
    در هیچ یک از زمان های ریکاوری، 4، 12 و 24 ساعت بعد از عمل اختلاف معنی داری در نمره درد دیده نشد و با توجه به (0/05< P) لذا نمره درد در 2 گروه تقریبا یکسان بود. زمان درخواست اولین مسکن در گروه آپوتل 254/71-/+28/2 دقیقه و در گروه دارونما 198/28/-+34/2دقیقه بود که با توجه به (0/05> P) ب طور معنی داری زمان درخواست اولین مسکن در گروه آپوتل طولانی تر بود.

    نتیجه گیری

    آپوتل ب عنوان پیش دارو در کاهش مصرف مقدار مصرفی مخدر حین عمل و افزایش طول مدت درخواست مسکن بعد از عمل موثر است.

    زمینه و هدف
    آسپیراسیون ریوی محتویات معده ای یکی از مهمترین عوارض بیهوشی عمومی باقی مانده است. پیشگیری و اداره این عارضه یکی از دغدغه های متخصصین بیهوشی می باشد. لذا در این مقاله سعی شده است تا علاوه بر معرفی مورد، دستورالعمل های جامع برای پیشگیری و رویارویی با این عارضه مورد بحث قرار گیرد.
    معرفی بیمار: این مقاله گزارش آسپیراسیون شیر در حین القاء بیهوشی در عمل جراحی الکتیو یک پسر 6 ساله است که به علت ارائه اطلاعات نادرست درباره ناشتا بودن بیمار توسط والدین اتفاق افتاده است. پس از اقدامات لازم و تخلیه شیر آسپیره شده، عمل جراحی ادامه می یابد. بیمار بدون مشکل اکستیوب و با اشباع اکسیژنی 95% در هوای اتاق به ریکاوری منتقل شد. بیمار پس از دو روز بستری در بخش مراقبت های ویژه اطفال، با حال عمومی پایدار به بخش اطفال انتقال یافت.

    Aims and
    Background
    Considering the importance of postoperative hyperlactatemia, rehydration impact on lactate amount, and different challenges in using crystalloid and colloid solutions for rehydration, we decided to investigate the effect of Ringer solution and Hemaxel on blood lactate level with comparing the changes before and after the operation, in patients undergoing appendectomy.Methods and Materials: 168 patients scheduled for appendectomy, were randomly divided into two groups (using Ringer solution or Hemaxel) after obtaining their consent. A blood sample of 2ml was obtained before and 1 hour after the surgery and the amount of lactate level was measured. The data was analyzed by SPSS.
    Findings
    The results showed that Ringer solution reduced the post-operative serum lactate level significant, while Hemaxel increased it(p=0.0001).
    Conclusions
    According to this study, we can perhaps say that Ringer solution is preferred to Hemaxel in order to reduce lactate level after surgery in patients undergoing appendectomy.

    Aims and
    Background
    Surgery is a factor of anxiety which creates a physiological reaction in body. As a result of this physiological response of the body, breathing, heart rate and blood pressure would increase. The present study was done to evaluate the effect of wordless music on some physiological indices and anxiety of patients.
    Materials And Methods
    This research is a randomized controlled clinical trial, which has been based on 60 patients undergoing spinal anesthesia for surgery in hospitals affiliated to Ilam University of medical sciences, during the year 1392. Including criteria were: the patient's willingness to participate in the study, non-emergency surgery, being between 20 to 40 years old, undergoing spinal anesthesia for the first time, as well as having hemodynamic stability (systolic blood pressure greater than 90 mm Hg, absence of any dangerous form of heart rate and heart rhythm whatsoever between 60-100). In both groups, vital signs including diastolic blood pressure, systolic blood pressure, pulse rate, respiratory rate, and arterial blood O2 saturation were recorded. After the baseline assessment patients underwent spinal anesthesia by the anesthesiologist, and immediately the wordless song was played for the case group with mp3 player and disposable headphone for 10 minutes. Immediately afterwards, again the arterial blood saturation level and the vital signs were assessed. The same action was taken for the control group without playing the music.
    Findings
    Comparison between physiological indices before and after the intervention in the two groups showed significant differences; so that broadcasting the song without words had beneficial effect on the average number of pulse pressure, systolic and diastolic blood pressure,and respiratory rate (p<0.05). However there was no significant difference considering the percentage of arterial blood saturation (p>0.05).
    Conclusion
    Hearing wordless song had a significant effect on systolic and diastolic blood pressures, heart rate, and respiratory rate; but It did not affect the percentage of arterial oxygen saturation.

    Background
    Due to the nature of the orthopaedic surgery, affecting bone and joint, it is associated with a high rate of postoperative pain. Using intravenous, intramuscular, intrathecal and epidural opioids are methods for postoperative pain control for which the side effects are numerous. However, Intrathecal administration of magnesium with blocking NMDA receptors reduces postoperative pain without increasing side effects. The aim of this study was to investigate the effect of adding magnesium sulfate to Intrathecal lidocaine- epinephrine mixture on duration of postoperative analgesia in lower limb orthopaedic surgeries.
    Materials And Methods
    This double blind study was performed on patients who were candidate for lower extremity orthopaedic surgeries. For this purpose, 156 patients were randomly divided into two groups. One group received magnesium sulfate with lidocaine – epinephrine mixture and other group received the same drugs with distilled water as placebo. The duration of postoperative pain was determined according to patient's request for analgesics. Patient's pain degree was evaluated according to visual analogue score (VAS), and Pethedine consumption was also calculated. Data were analyzed by SPSS software.
    Findings
    Mean age of our patients in magnesium and control groups were 34.5 ±7.5 and 31.9±8.1 years old, respectively. The Mean (±SD) of postoperative analgesia duration in the magnesium group was 143.7±29.7min and it was 100.9±18.1 min in the control group, which shows a significant difference between two groups (p <0. 000). Patients’ pain score according to VAS in magnesium group was 3.9±o.36 and it was 5.65±0.35 in the control one. This was also significantly different between the two groups (p=0.000). Also mean Pethedine consumption was 15±6.24 mg and 33±6.27mg in the magnesium and control groups, which was again statistically significant (p=0.000).
    Conclusions
    Magnesium sulfate combined with lidocaine produces a reduction in postoperative pain in comparison with lidocaine alone.

    Background
    Nowadays, multimodal approach is used to control post-operative pain. Pregabalin has been shown to have analgesic effects on postoperative pain as a Gaba-Amino-Buteric (GABA) analogue. This prospective, randomized, double-blind, controlled study has evaluated the beneficial effect of Pregabalin pretreatment on postoperative pain and opioid consumption in patints undergoing orthopedic elective surgery.
    Materials And Methods
    Sixty ASA 1-2 patients, scheduled for orthopedic elective surgery under spinal anesthesia, were randomly divided into two groups. Group 1 received 300 mg Pregabalin one hour before the operation and Group 2 was given Placebo capsule. Patient-controlled intravenous analgesia (PCIA) device, prepared with Sufentanil, was connected to both groups for postoperative analgesia. Postoperative pain scores according to visual analogue scale (VAS), opioid consumption, and side-effects like nausea were recorded for 12 hours after surgery. P value<0.05 was considered significant.
    Findings
    According to visual analogue scale (VAS), pain scores were significantly lower in Pregabalin group at post anesthesia care unit (PACU), 6 hours and 12 hours post-operatively(p<0.05). Also pethidine consumption was significantly less than the placebo group in Pregabalin group (p<0.05).
    Conclusion
    Administering a single dose of 300 mg Pregabalin orally, and 1 hour before elective orthopedic surgery under spinal anesthesia, significantly reduces postoperative pain and opioid consumption.

    Background
    Evaluation and management of pain is an important issue in the intensive care unit and Limited research there is on clinical decision making in relation to the management of pain in patients with decreased level of consciousness. The purpose of this study was to determine the effect of a pain management program on pain management In patients with decreased level of consciousness of patients in intensive care units.Methods and Materials: This study is a clinical trial across the two groups before and after and stage to stage design. In this study, 50 hospitalized patients with decreased level of consciousness in the intensive care unit selected sampling and randomly divided into two groups. Pain management program was implemented the patient's level of consciousness, to reach more than 8 (GCS), as or planned until 48 hours. For the control group implemented routine unit. With used of the adult non-verbal pain scale collected the pain intensity scores in two group in before and after of pain therapy.Data were analyzed by SPSS (16) software. To describe the data, analytic statistics (independent t-tests, chi-square and ANOVA with repeated) were used.
    Finding
    Results showed that the mean pain intensity scores (after one stage) was significantly more in the control group than the experimental group after of implemented program, and the mean change in pain intensity scores in the experimental group using independent t-test Was significantly lower than control group (p=0/04).
    Conclusion
    This study showed that with applying a pain management program can the pain controlled properly for patients with loss of consciousness after general surgery.

    Background
    The most common used anesthetic gas in operating rooms is nitrous oxide which is a poor anesthetic agent. Moreover, the level of serum cortisol is resumed as an indicator of stress. As a result an increase of serum cortisol could indicate stress and nervous condition. The aim of this study was to evaluate the relationship between the concentrations of anesthetic agent, named nitrous oxide, on urinary cortisol levels of the operating room's personnel.
    Materials And Methods
    This study was a cross-sectional and correlational one. The Study population were the operating room's personnel (n=30) and the Official personnel (n=30) in Besat Hospital of Hamedan. The concentration of nitrous oxide was measured in operating room for a week in the morning and afternoon. To investigate the effect of the inhalation and absorption of harmful chemical agents on neurobiological factors, urine samples were taken at the beginning and at the end of each day continuously for 5 days from the test and control groups. To reach the goal, Cortisol level in urine samples was measured.
    Findings
    The concentration of anesthetic and antiseptic agents in the operating room space was 388.26 ± 101.4 ppm. Mean Cortisol level in urine samples of the operating room personnel(test group), at the beginning and the end of the day, for 5 days was 648.4± 2.74 ng/ml. Mean Urine Level of cortisol in the official personnel (control group), collected in the same manner was 579.2 ± 3.1ng/ml. Our findings showed that there is a significant difference between the mean concentration of the breathing gas nitrous oxide in operating room's and recovery workers at three-time sampling a day according to ANOVA test (P<0.05) (P=0.028). T test showed no significant difference in urinary free cortisol (ng per ml) between the two groups (P=0.559).
    Conclusions
    Pearson correlation showed that there is a direct positive and incomplete relation between the concentration of nitrous oxide in the operating room space the and urinary free cortisol (ng/ml) of the operating room's personnel (r=0.857) (P=0.004). This means that increasing the concentration of nitrous oxide in the operating room space, increased urinary free cortisol levels. (P<0.05)

    Background
    Nausea and vomiting after surgery is a common complication that can cause unpleasant feeling, increased hospitalization time, and increased cost. Even in some cases it leads to severe dehydration, electrolyte imbalance, aspiration, pneumonia, and even the opening of the surgical site which has been sutured. Selecting the device for airway management may affect postoperative nausea and vomiting, in part due to the irritation of the airway. Therefore, we decided to compare two methods of airway management in patients undergoing strabismus surgery regarding the incidence of postoperative nausea and vomiting.
    Materials And Methods
    The study population underwent strabismus surgery with general anesthesia. Patients were randomly divided into two groups with either endotracheal intubation or laryngeal mask airway insertion. The severity of nausea and vomiting was assessed in the recovery room, 6, and 18 hours after the surgery. Nausea was measured according to the Verbal Rating Scale (VRS) criteria.
    Findings
    160 patients were enrolled and divided into two groups of 80. The groups were identical in terms of demographic data (age, weight, gender, symptoms, hemodynamics (except for heart rate before induction, 5 min and 15 min after induction), duration of surgery, number of muscles under surgery and number of eyes under surgery.
    Conclusion
    Incidence of PONV in the recovery room, 6 hours and 18 hours after surgery were similar in both groups.

    Aims and

    Background

    Regarding the importance of providing analgesia for patients during and after orthopedic surgeries, the present study has been designed to evaluate the analgesic effect of Intravenous Acetaminophen(Apotel) and its effect on opioid consumption in patients undergoing radius shaft fracture surgery.

    Materials And Methods

    In this research, 160 patients with radius shaft fracture who met the inclusion criteria were randomly divided into two groups using random number table: Apotel (A) and placebo (P). The Apotel group received 1mg paracetamol in 200cc N/S, 20 min before surgery; while the placebo group received 200cc N/S only. The mean dose of opioid consumption during surgery and 24 hours after the surgery, and also the mean interval to first analgesic request have been recorded. Visual analogue scale (VAS) was used for measuring pain score of patients after surgery.

    Findings

    The mean dose of opioid consumption during surgery (fentanyl based on micrograms) in Apotel group was 51/42-/+2/3 and it was 70/10-/+3/6 in the placebo group. Considering the p.value <0.05 to be significant, there has been a significant difference between the two group regarding opioid consumption during surgery. This amount has been clearly less in Apotel group. Pain scores of two groups were determined, which showed no significant difference between the groups in recovery period, 4 hours, 12 hours, and 24 hours after surgery. Regarding p value which was >0.05, pain score in two group has been almost the same.Interval to first analgesic request in Apotel and placebo groups were 254/71-/+28/2 198/28/-+34/2 min, respectively. Considering p value which was<0.05, the interval to first analgesic request in Apotel group has been significantly longer.

    Conclusion

    Premedication with intravenous paracetamol (Apotel) provided rapid and effective analgesia and was well tolerated. At the same time it reduced opioid consumption during and after orthopedic surgery.

    Background
    Pulmonary aspiration of gastric contents is one of the most important complications of general anesthesia. Prevention and specially management of this complication are still both challenging subjects for all anesthesiologists. In this case report beside case presentation we will discuss about comprehensive guidelines for prevention and management of this event.Case report: This study presents a case of milk aspiration in a 6-year-old boy during induction of anesthesia for an elective surgery, which occurred because of incorrect parent-reported information about fasting time of the child. The operation has been continued after appropriate interventions and drainage of the milk. After the operation, he was extubated in the operating room and then was moved to the recovery room with O2 saturation of 95% on room air. After two day's stay in the pediatric intensive care unit (PICU), the patient was transferred to the pediatric ward with stable physiologic conditions.
