    زمینه و هدف
    پژوهش حاضر با هدف تعیین نقش آلکسی تیمیا و بدتنظیمی هیجان در کیفیت زندگی مبتلایان به درد مزمن انجام گردید. مرد) مبتلا به انواع دردهای 60 زن و 90 نفر (150
    مواد و روش ها
    پژوهش حاضر توصیفی از نوع همبستگی است. نمونه پژوهش شامل بودند که به روش نمونه گیری دردسترس انتخاب 1396 مزمن مراجعه کننده به کلینیک درد گیلان شهر رشت طی فروردین تا خرداد شدند و مقیاس آلکسی تیمیا تورنتو، مقیاس بدتنظیمی هیجان گراتز و رومر و مقیاس کیفیت زندگی را تکمیل نمودند. داده ها با استفاده از روش ضریب همبستگی پیرسون و رگرسیون گام به گام تحلیل گردید.
    نتایج و
    یافته ها
    نتایج نشان داد ارتباط معکوس و معناداری میان آلکسی تیمیا و بدتنظیمی هیجان با کیفیت زندگی وجود درصد از تغییرات کیفیت زندگی مبتلایان به درد مزمن را 32) و دشواری در شناسایی احساسات و بدتنظیمی هیجان حدود p>0/01دارد( .)p>0/01 پیش بینی می کنند(
    بحث و نتیجه گیری
    آلکسی تیمیا و بدتنظیمی هیجان درکیفیت زندگی مبتلایان به درد مزمن نقش دارند و لزوم توجه به فرایندهای هیجانی جهت ارتقای سطح کیفیت زندگی مبتلایان می تواند حائز همیت باشد.

    زمینه و هدف
    درد ناگهانی پهلو یا تیرکشیدن پهلو یک درد شناخته شده در میان بسیاری از رشته های ورزشی است. هدف از تحقیق حاضر مقایسه میزان جابه جایی عمودی بدن و زوایای گردن، تنه، ران، زانو و مچ پا هنگام دویدن در بین افراد مبتلا به درد پهلو و سالم بود. نفر سالم) 13 نفر مبتلا به درد پهلو و 13
    مواد و روش ها
    بیست و شش نفر از دانشجویان تربیت بدنی پسر دانشگاه فردوسی مشهد ( به صورت در-دسترس و هدفمند انتخاب، و در دو گروه مبتلا به درد پهلو و سالم قرار گرفتند. پس از تشکیل مدل حرکتی با مارکر، Motionجابجایی عمودی بدن و زوایای گردن، تنه، ران، زانو و مچ پا در هر دو گروه طی دویدن روی تردمیل با استفاده از سیستم اندازه گیری و مقایسه شد. جهت تجزیه داده ها از آزمون آماری تی مستقل استفاده شد.     یافته ها
    تفاوت معنی داری در زوایای گردن ( .)P<0/05 به طور معناداری در مقایسه با گروه مبتلا به درد پهلو بیشتر بود. اما تفاوتی در زوایای زانو و مچ پا بین دو گروه مشاهده نشد ( .)P=0/02 بعلاوه نتایج نشان داد که جابه جایی عمودی بدن گروه دارای درد پهلو در مقایسه با گروه سالم به طور معنادار بیشتر بود (
    نتیجه گیری
    افزایش جابه جایی عمودی بدن و کاهش زاویه گردن، تنه و مفصل ران ممکن است عواملی اثر گذار در بروز درد پهلو باشند. بنابراین با اصلاح این عوامل در افراد مبتلا به درد پهلو شاید بتوان درد آنها را تسکین بخشید.

    نتیجه گیری
    هنگام راه رفتن توزیع فشار کف پایی تحت تاثیر بیماری کمردرد قرار می گیرد. بنابراین بررسی الگوی توزیع فشار کف پایی و تجویز کفی مناسب در طراحی روش های درمانی و مدیریت درد بیماران کمردرد مزمن باید مد نظر قرار گیرد.

    زمینه و هدف
    یکی از مشکلات شایع در اعمال جراحی، درد بعد از عمل می باشد که جهت تخفیف و کنترل آن تاکنون روش های متعددی بکار گرفته شده است. در عین حال درد پس از عمل در کودکانی که تحت عمل جراحی قرار می گیرند از اهمیت ویژه ای برخوردار است زیرا از یک سو تحمل درد درکودکان کمتر بوده و از طرف دیگر مصرف مخدر نیز جهت کاهش درد در این گروه سنی محدودیت دارد. لذا از آنجایی که استامینوفن به عنوان یک داروی کم عارضه در سطح گسترده ای جهت کاهش درد مورد استفاده قرار می گیرد، این مطالعه با هدف مقایسه تاثیر استامینوفن و مورفین بر کاهش درد بعد عمل در کودکان تحت عمل آپاندکتومی به انجام رسید. نفره توزیع شده و نیم ساعت قبل 40 کودک کاندید عمل آپاندکتومی در دو گروه 80
    نتایج و
    یافته ها
    در یک مطالعه کارآزمایی بالینی، دقیقه تزریق و در 15 رقیق شده بود در طی N/S استامینوفن که در صد سی سی سرم 15 mg/kgاز شروع جراحی، در گروه استامینوفن مورفین وریدی توسط مجری تهیه شده تزریق شده و شدت درد در ریکاوری و دیگر عوارض از جمله تهوع 0.01mg/kgگروه مورفین نیز spss و استفراغ بعد عمل و پارامترهای همودینامیک در طی مدت عمل در دو گروه تعیین و مقایسه گردید. داده های مطالعه با نرم افزار مورد تجزیه و تحلیل قرار گرفت. 10/4±0/9 در زمان ورود به ریکاوری در دو گروه استامینوفن و مورفین به ترتیب Cheopsis
    نتیجه گیری
    میانگین شدت درد طبق معیار ریکاوری شدت درد در دو گروه مذکور به ترتیب 15). در دقیقه p >0/001 بوده و اختلاف معنی دار بین دو گروه دیده شد ( 8/93±2و ). در زمان خروج از ریکاوری شدت درد در دو گروه p >0/001 بوده و تفاوت معنی دار بین دو گروه وجود داشت ( 8/1±1/72 و 10/58±1/06 ). بر حسب آزمون آنالیز واریانس با p >0/001 بوده و اختلاف معنی دار بین دو گروه وجود داشت ( 8/03±1/8 و 10/28±1/2فوق به ترتیب .)p >0/001 تکرار مشاهدات، تغییرات شدت درد در طی مدت اقامت بیماران در ریکاوری اختلاف معنی دار داشت (
    بحث: برابر نتایج مطالعه ما تزریق استامینوفن در قبل از عمل اپاندکتومی لاپاراسکوپیک، با کاهش شدت درد بعد از عمل، کم شدن زمان اقامت در ریکاوری و بروز کمتر عوارض دیگری همچون تهوع واستفراغ بعد عمل و بیقراری در کودکان تحت عمل آپاندکتومی می باشد و از انجایی که مصرف مخدر در کودکان محدودیت مصرف دارد، استامینوفن می تواند جایگزین مناسبی برای کنترل درد بعد از عمل آپاندکتومی در کودکان باشد(هر چند مورفین در کنترل درد تاثیر بیشتری داشته است )و استفاده از آن، مشروط به عدم وجود کنترا اندیکاسیون مصرف توصیه می شود.

    زمینه و هدف
    احساس درد، یک تجربه نامطلوب به دنبال آسیب های بافتی است. درد پس از عمل جراحی یکی از موضوعات اصلی در مراقبت های بعد از عمل می باشد و نقش مهمی در تسریع بهبود وضعیت عمومی بیماران جراحی ایفا می کند. کنترل مناسب درد پس از جراحی از نظر جلوگیری از تاکی کاردی، افزایش فشارخون، ایسکمی میوکارد، کاهش تهویه آلوئولار و بهبود ضعیف زخم ها، اهمیت ویژه ای دارد. لذا در این مطالعه کارایی و عوارض دو داروی شیاف مورفین و شیاف دیکلوفناک در مقایسه با پتدین تزریقی به عنوان یک روش ایمن در کنترل درد پس از جراحی آپاندکتومی و هرنی اینگوئینال مورد بررسی قرار گرفته است.
    مواد و روش ها
    این مطالعه از نوع کار آزمایی بالینی بوده و بیماران مورد مطالعه، بیماران مراجعه کننده به بیمارستان دکتر فاطمی نفر که با روش 75 سال بودند. حجم نمونه 18 -70 با آپاندیسیت و هرنی اینگوئینال در گروه سنی 1392شهرستان اردبیل در سال میلیگرمی پس از جراحی و گروه دوم 10 نفر وارد مطالعه شدند. گروه اول، شیاف مورفین 25 گروه 3تخصیص تصادفی سیستماتیک در میلیگرم استفاده گردید. 50 میلیگرمی پس از جراحی و درگروه سوم نیز برای کنترل درد از پتیدین عضلانی 100شیاف دیکلوفناک و 3/68 ،4/40 ،4/56 به ترتیب برابر 24-12-6-2
    یافته ها
    میانگین نمرات آنالوگ بصری درد در گروه دریافت کننده دیکلوفناک درساعات ،6/68 ،6/52 و در گروه کنترل 2/44 و 3/12 ،3/84 ،4/36، میانگین همین نمرات درگروه دریافت کننده شیاف مورفین به ترتیب برابر 3/36 اختلاف معنی داری میان کاهش درد بیماران در هر سه گروه مورد بررسی وجود داشت.     نتیجه گیری
    شیاف مورفین و دیکلوفناک، هر دو دارو توانستند میزان بی دردی معنی داری نسبت به گروه دریافت کننده پتدین تزریقی در کنترل درد پس از جراحی آپاندکتومی و هرنی اینگوئینال ایجاد کنند.

    کمردرد مزمن غیر اختصاصی شایع ترین نوع کمردرد مزمن می باشد و دستیابی به یک برنامه ی درمانی و توانبخشی برای بهبود سریع تر بیماران مبتلا به کمردرد مزمن همواره مورد توجه بوده است. لذا هدف از پژوهش حاضر، ارزیابی تاثیر یک دوره تمرینات بر کنترل درد، تعادل پویا و حس عمقی کمری - لگنی بیماران مبتلا به کمردرد مزمن غیر اختصاصی بود. نفر از 30
    مواد و روش ها
    مطالعه حاضر از نوع نیمه تجربی با طرح پیش آزمون - پس آزمون با گروه کنترل بود. به همین منظور، نفری انتخاب شدند. گروه تجربی به مدت 15گروه مساوی 2 بیماران مرد مبتلا به کمر درد مزمن غیر اختصاصی به روش تصادفی در هشت هفته، هر هفته سه جلسه، تمرینات ثبات مرکزی را انجام دادند. به منظور ارزیابی کنترل درد، تعادل پویا و حس عمقی به ترتیب از معیار دیداری درد، مقیاس تعادل برگ و کونیامتر دستی استفاده گردید. جهت تجزیه و تحلیل آماری از آزمون تی زوجی و مستقل استفاده گردید. ). در P=0/001
    یافته ها
    در یافته ها بهبودی معنی داری در کنترل درد، تعادل پویا و حس عمقی گروه تمرینات ثبات مرکزی مشاهده شد ( حالی که این تغییرات در گروه کنترل معنی دار نبود.
    نتیجه گیری
    نتایج پژوهش حاضر نشان داد که هشت هفته تمرین ثبات مرکزی بر کنترل درد، تعادل پویا و حس عمقی کمری - لگنی بیماران مبتلا به کمر درد مزمن غیر اختصاصی تاثیر معنی داری دارد. لذا به درمانگران توصیه می شود در تدوین برنامه های تمرینی خود، تمرینات ثبات مرکزی را مورد توجه ویژه قرار دهند.

    نتایج پژوهش نشان می دهد خروجی شبکه برای تشخیص حالت بیداری، بیهوشی سبک و بیهوشی عمومی به ترتیب به رسیده است. بنابراین مدل پیشنهادی در طبقه بندی حالت های مختلف بیهوشی به درصد 99/32 و 98/62 ،93/98ت درصدهای صح رسیده است. 97/3ت متوسط صح

    کلره هانتینگتلون یلک اختلال ارثی نادر سیستم عصبی است که به صورت اتوزوم غالب به ارث می رسد. این اختلال با حرکات کره پیشرونده، دمانس و اختلال روانپزشکی مشخص می شود. در این بیماران پاسلخ غیرطبیعی به بسیاری از داروهای بیهوشی گزارش شده است. هدف اصلی در بیهوشی این بیماران، حفاظت از راه هوایی و ایجاد ریکاوری سریع و ایمن می باشد. در این مطالعه موردی یک ساله مبتلا به کره هانتیگتون که تحت عمل جراحی که سیستکتومی الکتیو تحت بیهوشی عمومی قرار گرفت معرفی می شود. 30 آقای سال قبل با افزایش ترشحات بزاقی و حرکات پیشرفته کره در 5 ساله مورد شناخته شده کره هانتینگتون از 30
    معرفی بیمار: بیمار آقای اندامها تحت درمان دارویی که جهت انجام کوله سیستکتومی الکتیو مراجعه نموده بود. پس از ویزیت قبل از عمل و انجام اقدامات لازم بیمار تحت بیهوشی عمومی با انتوباسیون داخل تراشه قرار گرفت و جرای بیمار بدون عارضه انجام شد. بیمار بعد از جراحی با حال عمومی خوب منتقل ریکاوری و بعد از سه روز مرخص شد.

    Aim and
    Background
    This study was performed with the purpose to determine the role of alexithymia and emotion dysregulation in quality of life in patients with chronic pain.
    Materials And Methods
    This research is descriptive and correlational. The research sample consisted of 150 patients(90female and 60male) with chronic pain who referred to Gilan pain clinic in march-June 2017. They were selected by available sampling method. Toronto’s Alexithymia scale, Gratz and Roemer emotion dysregulation Scale and Quality of Life Scale were used for their evaluation. Data was analyzed by using Pearson correlation coefficient and stepwise regression.

    Aims and
    Background
    Exercise-related transient abdominal pain (ETAP) or side stitch is a well known injury in many sport activities. The purpose of this study was to compare vertical displacement of the body and body angles during running in people with side stitch and healthy people.
    Materials And Methods
    Twenty six subjects (male students) in Ferdowsi University of Mashhad were selected through non-probability inconvenient sampling and based on the pain during running, they were divided in to two groups; stitch group and control group (13 subjects with stitch and 13 healthy subjects). After the formation of the marker motion model, vertical displacement and angles of the neck, trunk, hip, knee and ankle in both groups were measured during treadmill running by using Motion Analysis system and theyvwere compared. To analyze the data, independent t-test was used.
    Findings: Significant differences in Neck angles (P = 0.04), trunk (P = 0.01) and hip (P = 0.02) were observed between the two groups and these angles were significantly higher in control group than stitch group. But no difference between the two groups was observed at angles of knee and ankle (P>0.05). The results also have shown that vertical displacement in stitch group was significantly higher than control group (P = 0.02).
    Conclusion
    Increased vertical displacement of the body and reduced angle of the neck, trunk and hips may be factors affecting the incidence of stitch. Therefore, by modifying these factors in people with stitch, perhaps we can reduce their pain

    Background
    The aim of this study was to compare peak pressure and time to peak pressure during normal walking between females with idiopathic chronic low back pain (LBP) and healthy controls.
    Materials And Methods
    10 females with chronic LBP and 11 healthy female participated in this study. LBP was measured using Quebec questionnaire. Plantar pressure was measured using emed platform during barefoot walking. After dividing foot into 10 parts, peak pressures and time to peak pressures were calculated. Repeated measure and MANOVA were used for statistical analysis and significance level was set at 0/05.
    Findings: This study showed that peak pressure of LBP subjects were significantly greater than healthy subjects on right (p=0/001) and left (p=0/001) medial midfoot parts; and they were significantly smaller than healthy subjects on left lateral midfoot (p=0/03), right hallux (0/04) and left 3rd, 4th and 5th finger (p=0/003) parts. Also, LPB patients showed significantly increased time to peak pressure in the parts of medial (0=0/002) and lateral heel (p=0/003), medial (0=0/005) and lateral midfoot (0=0/01) in the right foot and in the meidal (0=0/002) and lateral heel (p=0/004), medial (0=0/002) and lateral midfoot (p=0/003), second metatarsal (0=0/002) and 3rd, 4th and 5th metatarsals (p=0/001) in the left foot.
    Conclusion
    plantar pressure is affected by LBP ; So the assessment of plantar pressure distribution and prescribing the appropriate insole should be considered in treatment planning and pain management of chronic LBP patients.

    Aims and
    Background
    Post operative pain is the most common problem after surgeries. In the meanwhile, post operative pain in children is very important because tolerance to pain in children is lower; also use of opioid in children is limited. As using acetaminophen is increasing for pain relief, this study aimed to compare the effect of Acetaminophen with Morphine in decreasing post opearative pain in children undergoing laparoscopic apandectomy.
    Materials And Methods
    In a clinical trial 80 children, candidate for apandectomy, were selected and randomly divided into two groups. The first group recevied 15 mg/kg acetaminophen in 100 ml normal saline and the second group received 0.01 mg/kg morphine intravenously and post operative pain and other complications such as nausea and vomitting and hemodynamic parameters during surgery and recovery were compared between the two groups. Data were analyzed by SPSS soft ware.
    Findings: According to Cheopsis score the mean post operative pain in the enterance to recovery in acetaminophen and morphine groups were 10.4±0.9 and 8.93±2, respectively and the difference between the two groups was statisticallysignificant (P
    Conclusion
    According to the results of the study, injection of acetaminophen before laparascopic apandectomy led to decreased post operative pain, decreased length of stay in recovery and decreased other post operative events such as nausea and vomitting. Therefore acetaminophen is a suitable replacement for control of post operative pain in children under laparoscopic apandectomy and this recommended except in the case of contraindication

Postoperative pain control is one of the essential issues in postoperative care, and plays an important role in quick healing of general status of surgery patients. Appropriate control of the postoperative pain is particularly important in preventing tachycardia, hypertension, myocardial ischemia, decreased alveolar ventilation and poor wound healing. Therefore, in this study the efficacy and adverse effects of morphine and diclofenac suppositories are compared to injectable pethedine in controlling postoperative pain in appendectomy and inguinal hernia.
    Material and
    Methods
    This study is a clinical trial. The patients were among the ones admitted to Doctor Fatemi Hospital of Ardabil city with appendicitis and inguinal hernia, aged between 18 to 70 years old during 1392. 75 individuals, chosen in 3 groups of 25 patients by Systematic random allocation were chosen.The first group received 10 mg of morphine suppository after surgery, the second group received 100 mg of Diclofenac suppository after surgery, and in the third group 50 mg of intra muscular pethedine was used to control pain.
    Findings: The average scores of pain )VAS( in the Diclofenac receiving group at 2- 6-12- 24 hours after surgery were 4/56, 4/40, 3.68 and 3/36, respectively. The average scores in the morphine suppository group were 4/36, 3/84, 3/12, and 2/44, respectively; the scores were 6/52, 6/68, 6/28 and 5/84 in the control group )pethedine group(. According to ANOVAS test, there was significant difference among patients’ pain relief in the three studied groups.
    Conclusion
    Both morphine and diclofenac suppositories, cause significant pain relief in comparison to the group receiving IM pethedine to control pain after appendectomy and inguinal hernia surgery.

    Background
    Non-specific chronic low back pain is the most common type of chronic low back pain. Rehabilitation to achieve a more rapid improvement in patients with chronic low back pain has always been taken into account. The aim of this study was to evaluate the effect of 8 weeks core stabilization on pain control, Dynamic balance and proprioception of lumbo pelvic part in subjects with non-specific chronic low back pain.
    Materials And Methods
    The present study was a quasi - experimental study with a pretest-posttest design and a control group. For this purpose, 30 male patients with non-specific chronic low back were selected by convenience sampling in 2 equal groups of 15. The experimental group performed core stability exercise for 8 weeks, every week in three sessions. In order to assess pain control, Dynamic balance and proprioception of subjects, Visual analog scale(VAS), Berg Balance Scale and manual goniometer were used. The data was analyzed using a paired t-test and independent t-test to determine the statistical significance (P≤0/05).
    Findings: The research findings showed a significant improvement in pain control, Dynamic balance and proprioception of core stabilization group (P=0/001); whereas these changes were not significant in the control group.
    Conclusion
    The results showed that the effect of eight week core stabilization on pain control, Dynamic balance and proprioception (lumbo pelvic) of subjects with non-specific chronic low back pain was significant. We recommend therapists to include Core stabilization in preparing their programs in order to enhance these variables.

    Background
    This study develops a computational framework for the classification of different anesthesia states, including awake, moderate anesthesia, and general anesthesia, using electroencephalography (EEG) signals and peripheral parameters.
    Materials And Methods
    The proposed method proposes data gathering; preprocessing; a new labeling process of EEG signal; appropriate selection of window length by genetic algorithm; feature extraction by Hjorth parameters, approximate entropy, Petrosian fractal dimension, Hurst exponent, largest Lyapunov exponent, Lempel-Ziv complexity, correlation dimension, and Daubechies wavelet coefficients; feature normalization; feature selection by non-negative sparse principal component analysis; and classification by radial basis function (RBF) neural network. Correct labeling process of EEG signals is performed by an expert opinion and also qualitative and quantitative analysis of the extracted parameters from peripheral nerve stimulator, pulse oximetry, blood pressure, and the time of drug injection.
    Findings: The results indicate that the proposed method would classify different anesthesia states including awake, moderate anesthesia, and general anesthesia, with the accuracy of 93.98%, 98.62, and 97.3, respectively. Therefore, the proposed method can classify different anesthesia states with the average accuracy of 97.3%.
    Conclusion
    Finally, the proposed method provided a good representation of the brain behavior in different anesthesia states.

    Background
    Huntington’s chorea )HC( is a rare, autosomal, dominant hereditary disorder of the nervous system. This disease is characterized by progressive chorea movement, dementia and psychiatric disorders. In these patients, an abnormal response to many anesthetic drugs has been observed. The primary goal of general anesthesia in these patients is airway protection and a fast and safe recovery. In this study, a 30 year old male with huntington chorea undergoing elective cholecystectomy with general anesthesia is introduced.
    case report: The patient was a 30 year old man, known case of Huntington’s chorea from 5 years ago with increased salivary secretions and advanced limb chorea movements underdrug treatment, candidate for elective cholecystectomy. After pre operative visit and necessary implementation, the patient underwent general anesthesia with endotracheal intubation and surgery was performed successfully without complications. After surgery the patient was transferred to recovery room with good general appearance and was discharged after three days.
