    مقدمه
    فولادسازی یکی از صنایع مهم هر کشور می باشد. آلودگی صدا در صنایع فولادسازی به علت وجود تجهیزات مولد صدای زیان آور همچون کوره قوس الکتریکی به عنوان یک معضل بهداشتی محسوب می شود. شناخت الگوی مواجهه می تواند به اهداف کنترل صدای شغلی کمک نماید. هدف از انجام این مطالعه ارزیابی صدا در واحدهای مختلف یک صنعت فولاد به منظور تعیین الگوی مواجهه کارگران آن بوده است.
    روش کار
    این مطالعه توصیفی - تحلیلی و مقطعی در واحدهای مختلف یک صنعت فولاد با توجه به اطلاعات اولیه از نظر وجود آلودگی صدا انجام گردید. توزیع صدای محیطی مطابق با استاندارد ISO 9612 و با استفاده از دستگاه تراز سنج Casella –Cel مدل 450 در شبکه A و در سرعت Slow اندازه گیری و داده ها جهت تهیه نقشه صوتی در نرم افزار Surfer V.10 وارد گردید. آنالیز فرکانس یک اکتاوباند همزمان با اندازه گیری محیطی در شبکه C انجام گردید. داده های آنالیز فرکانس جهت تهیه نمودار وارد نرم افزار Excel 2010 گردید. در فاز سوم مطالعه از دستگاه دزیمتر مدل TES 1345 برای اندازه گیری مواجهه فردی استفاده گردید.
    یافته ها
    نتایج ارزیابی صدای محیطی نشان داد 56% از مجموع ایستگاه های اندازه گیری در محدوده خطر بود. بیش ترین تراز صدا مطابق با نقشه صوتی در محوطه کوره قوس الکتریکی و کوره پاتیلی به ترتیب برابر با dB(A) 2/112 و dB(A) 97 ثبت شد. میانگین دز دریافتی در 3 گروه شغلی نظافتچی صنعتی، ذوب چی و ریخته گری به ترتیب برابر با 2133 %، 514%، 577% بود. فرکانس غالب صدای واحد کوره قوس الکتریکی و کوره پاتیلی به ترتیب در دو فرکانس مرکزی Hz 125 و Hz250 ثبت گردید.
    نتیجه گیری
    با توجه به نتایج ارزیابی مواجهه فردی با صدای شغلی واحدهای مورد مطالعه این صنعت معلوم گردید که اغلب شاغلین در معرض صدای پیوسته با فرکانس غالب کمتر از 500 هرتز قرار داشته اند و تجهیزات حفاظت فردی نیز در این محدوده کارایی چندانی ندارد و با توجه به دز دریافتی صدا بیش از 500% و عدم امکان کاهش زمان مواجهه، طرح کنترل فنی باید در اولویت اقدامات صنعت قرار گیرد.

    مقدمه
    بنزن، تولوئن، اتیل بنزن و زایلن (BTEX) از ترکیبات مهم خانواده آلاینده های آلی فرار هستند که استفاده گسترده ای در فرایندهای صنعتی دارند. این ترکیبات در گروه های مختلف مواد سرطانزا طبقه بندی شده و دارای سمیت عصبی هستند، بنابراین پایش و ارزیابی کیفیت هوای محیط های کاری جهت تعیین غلظت این آلاینده ها مسئله ای ضروری به نظر می رسد.
    روش کار
    به منظور اندازه گیری غلظت ترکیبات BTEX در هوای محیط کار سالن هیدرومتالورژی مجتمع مس سرچشمه، 6 ایستگاه نمونه برداری انتخاب گردید. با استفاده از روش شماره 1501 سازمان NIOSH، نمونه برداری و آنالیز ترکیبات BTEX صورت پذیرفت. نمونه برداری با استفاده از لوله های جاذب کربن فعال انجام و آنالیز ترکیبات با استفاده از گازکروماتوگرافی مجهز به آشکارساز یونش شعله ای انجام پذیرفت.
    یافته ها
    بیشترین غلظت اندازه گیری شده مربوط به ترکیب اتیل بنزن با غلظت 8/0 پی پی ام در ایستگاه نمونه برداری شماره 1 بود. غلظت بنزن در این واحد 05/0 پی پی ام گزارش گردید. بالاترین غلظت ترکیب تولوئن در ایستگاه نمونه برداری شماره 4 (15/0 پی پی ام) ثبت گردید. کمترین غلظت تولوئن در ایستگاه نمونه برداری شماره 6 نیز 02/0 پی پی ام گزارش گردید.
    نتیجه گیری
    ترکیبات BTEX موجود در هوای داخل محیط های کاری بر کیفیت هوای محیط کار موثر هستند. نتایج به دست آمده از اندازه گیری غلظت ترکیبات BTEX در ایستگاه های نمونه برداری حاکی از غلظت های پایین این ترکیبات در هوای داخل سالن هیدرومتالورژی مجتمع مس سرچشمه بود.

    مقدمه
    ارزیابی ریسک فرایندهای خطرناک یکی از اولویت های مدیریت ریسک می باشد. یکی از روش های مورد استفاده در ارزیابی ریسک، آنالیز لایه های حفاظتی (LOPA) است. به دلیل ناکافی بودن و یا عدم قطعیت اطلاعات موجود در مورد نرخ نقص (PFD) لایه های حفاظتی، ارزیابی ریسک به روش سنتی LOPA همواره دارای خطا می باشد. لذا هدف از این مطالعه، استفاده ازتئوری فازی جهت ارزیابی ایمنی مخازن شارژ آمونیاک با استفاده از روش آنالیز لایه های حفاظتی می باشد.
    روش کار
    ابتدا با استفاده از نظر متخصصین، نرخ نقص لایه های حفاظتی به صورت فازی بدست آمد. سپس با استفاده از عملگرهای فازی، امکان فازی به احتمال فازی تبدیل شده و در نهایت به نرخ نقص قطعی تبدیل شد. در مرحله بعد با استفاده از سیستم منطق فازی شدت، شدت رویداد به صورت فازی محاسبه شده و در انتها با استفاده از ماتریس ریسک، شاخص ریسک فازی محاسبه گردید.
    یافته ها
    در سناریوی آزاد شدن آمونیاک، شدت، احتمال و ریسک محاسبه شده با روش کلاسیک و فازی LOPA به ترتیب برابر با (احتمال: کم، شدت: بالا، و ریسک: (قابل تحمل- غیر قابل قبول)) در روش کلاسیک و احتمال فازی = 66/2، شدت فازی= 99/3 و ریسک فازی = 79/3 (8/0 غیر قابل قبول، 2/0 قابل تحمل- غیر قابل قبول) در روش فازی تعیین گردید. هم چنین پس از اضافه شدن لایه حفاظتی، شاخص ریسک فازی از 79/3 (8/0 غیر قابل قبول، 2/0 قابل تحمل- غیر قابل قبول) به 9/1 (1/0 قابل قبول، 8/0 قابل قبول- قابل تحمل، 1/0 قابل تحمل) تغییر یافت.
    نتیجه گیری
    در شرایط عدم قطعیت و یا موجود نبودن اطلاعات مر بوط به احتمال و شدت سناریوی ریسک، می توان با استفاده از نظرات متخصصین در قالب متغیرهای زبانی و روابط مجموعه های فازی، تا حد امکان خطای محاسباتی را در ارزیابی ریسک کاهش داد.

    مقدمه
    خستگی ناحیه شانه از مهمترین فاکتورهای آغازگر اختلالات اسکلتی-عضلانی (Musculoskeletal Disorders: MSDs) در اندام فوقانی است و در بین قالی بافان، شایع ترین MSDها در این ناحیه به وقوع می پیوندد. هدف از این مطالعه یافتن تاثیر قالی بافی در ایستگاه کاری پیشنهادی بر خستگی عضله ذوزنقه ای فوقانی (Upper Trapezius: UTr) طی یک چرخه وظایف شغلی است.
    روش کار
    در این مطالعه مقطعی 9 زن و 3 مرد مشارکت داشتند. طی 80 دقیقه قالی بافی در یک چرخه از وظایف قالی بافی، امواج الکترومایوگرافی (Electromyography: EMG) عضله UTr راست و چپ به طور پیوسته ثبت شد. پس از پردازش امواج، پارامترهای RMS و MPF به عنوان پارامترهای دامنه و طیف فرکانس امواج تعیین گردیدند. برای آنالیز آماری از روش مدل سری های زمانی استفاده و نتایج در دسته بندی های نمودار جاسا قرار گرفتند.
    یافته ها
    بر اساس روش جاسا در UTr راست، درصد افرادی که خستگی عضلانی، افزایش تولید نیرو، کاهش تولید نیرو و بازیابی را تجربه کرده بودند، به ترتیب برابر با 58%، 16%، 8% و 8% بودند. همچنین نقطه جاسا برای یک شرکت کننده بر روی محور x واقع شد. به همین ترتیب 50%، 25%، 8% و 16% قالی بافان در UTr چپ خود خستگی عضلانی، افزایش تولید نیرو، کاهش تولید نیرو و بازیابی را تجربه کرده بودند.
    نتیجه گیری
    به طور خلاصه یافته های این مطالعه نشان داد بیشتر قالی بافان در ایستگاه کاری پیشنهادی، خستگی عضلانی را در عضله UTr تجربه کردند. اهمیت کاربردی این نتایج در طراحی بهینه و ارگونومیک ایستگاه های کاری قالی بافی مورد توجه می باشد. مطالعات آینده، باید بر دیگر عضلات و در بازه های زمانی بزرگ تر تمرکز کنند.

    مقدمه
    رانندگان تاکسی به طور مکرر با ارتعاش تمام بدن مواجهه دارند که موجب کمردرد، اختلالات گوارشی می گردد. به دلیل وابستگی این نوع ارتعاش از وضعیت اجزاء خودرو، این مطالعه باهدف تاثیر مشخصه های تایر بر کاهش مقدار ارتعاش وارده بر تمام بدن راننده تاکسی پژو 405 انجام گرفت.
    روش کار
    در این مطالعه تجربی، درحالی که میزان باد لاستیک، دور موتور، شیب جاده، تعداد سرنشین، سرعت خودرو، نوع فنرها و کمک فنرها ثابت می باشد، مقدار مشخصه های ارتعاش تمام مطابق با استاندارد ISO2631-1 در هر یک از حالت های: لاستیک بدون تیوب ثابت و سیال درون آن متغیر (هوای معمولی یا گاز نیتروژن) و همچنین نوع باد لاستیک ثابت و وضعیت وجود تیوب متغیر (تیوب دار یا بدون تیوب) در جاده آسفالت اندازه گیری و محاسبه گردید. سپس تاثیر تغییرات با استفاده از آزمون آماری مناسب بررسی گردید.
    یافته ها
    بعد از جایگزینی نیتروژن به جای هوای معمولی و لاستیک بدون تیوپ به جای تیوب دار، میانگین شتاب rms در محور Z (محور غالب)، شتاب rms معادل 8 ساعته A(8) و فاکتور قله کاهش یافت (05/0 > P). همچنین میانگین مقدار A(8) از ناحیه خطرات بهداشتی (9/0 > m/s2) به ناحیه احتیاط (45/0-9/0 m/s2) کاهش یافت و مقدار آن (8/0 m/s2) می باشد.
    نتیجه گیری
    مقدار ارتعاش وارده بر تمام بدن راننده تاکسی پژو 405 به وضعیت وجود تیوب و نوع باد لاستیک حساس می باشد و می توانیم با تغییر این مشخصه های تایر، میزان ارتعاش تمام بدن را به کمتر از حد بالای خطرات بهداشتی (9/0 m/s2) کاهش دهیم.

    مقدمه
    خطاهای انسانی علت بسیاری از حوادث اعم از صنعتی و پزشکی می باشند، به همین دلیل پیدا کردن رهیافت هایی برای شناسایی و کاهش آنها بسیار حائز اهمیت است. از آنجائیکه تاکنون در زمینه خطاهای دندانپزشکی مطالعات اندکی انجام شده است، لذا تصمیم بر آن شد تحقیقی با هدف شناسایی و ارزیابی خطاهای انسانی در دستیاران تخصصی اندودونتیکس دانشکده دندانپزشکی دانشگاه علوم پزشکی کرمان با استفاده از تکنیک SHERPA انجام شود.
    روش کار
    این تحقیق کیفی و بصورت مقطعی در سال 1393 انجام شد. اطلاعات با استفاده از مشاهده مستقیم فعالیت ها و مصاحبه با دستیاران تخصصی اندودونتیکس جمع آوری گردید. در مجموع 10 وظیفه بحرانی که در آنها احتمال ایجاد آسیب به بیماران زیاد بود، شناسایی شدند. پس از تجزیه و تحلیل سلسه مراتبی وظایف (HTA)، شناسایی خطاها به روش SHERPA انجام و برگه های کار مربوطه تکمیل گردید.
    یافته ها
    با تجزیه و تحلیل برگه های کار SHERPA، 90 خطا شناسایی شد که از این تعداد، 7/67 % خطاها از نوع عملکردی، 3/ % خطاها از نوع بازدید، 8/8 % خطاها از نوع انتخابی، 5/5 % خطاها از نوع بازیابی، 4/4 % خطاها از نوع ارتباطی بودند. در نتیجه بیشترین نوع خطاها از نوع عملکردی، کمترین آنها از نوع ارتباطی بود.
    نتیجه گیری
    نتایج حاصل از این تحقیق نشان داد که بیشترین درصد فراوانی خطاها و همچنین بیشترین درصد سطح ریسک مربوط به خطاهای عملکردی با سطح ریسک نامطلوب می باشد، لذا برای کاهش وقوع این قبیل خطاها و محدود کردن پیامدهای آنها، لازم است راهکارهای کنترلی از جمله آموزش های ادواری روش صحیح انجام کار، تهیه چک لیست های نظارتی، تدوین دستورالعمل و ایجاد یک سیستم گزارش دهی منظم و استاندارد، عملیاتی ارائه گردد. با توجه به نتایج بدست آمده از این مطالعه، کنترل خطاهای بازیابی که بیشترین درصد خطاهای با سطح ریسک نامطلوب، و خطاهای عملکردی که بیشترین درصد فراوانی کل خطاها را به خود اختصاص دادند باید در اولویت قرار گیرند.

    زمینه و هدف
    آلودگی صوتی بر فیزیولوژیی بدن، کارایی و بر روی سیستم شنوایی تاثیر منفی می گذارد. هدف از این مطالعه ارزیابی عملکرد روش غربالگری صدا جهت برآورد ریسک صدا در کارگاه های کوچک سطح شهر همدان می باشد.
    روش بررسی
    این پژوهش یک مطالعه مقطعی- تحلیلی می باشد که در 22 کارگاه بالای 20 نفر در همدان انجام پذیرفت. ابتدا غربالگری صدا توسط فرم غربالگری انجام شد. در مرحله بعد اندازه گیری صدا با روش شبکه ای منظم از صدا سنج مدلSL 4001 استفاده گردید. تراز معادل صوت برای هرکدام از گروه ها بر اساس استاندارد ISO 9612 اندازه گیری گردید. جهت آنالیز داده ها از آزمون های پیرسون و رگرسیون خطی استفاده شد.
    یافته ها
    تراز فشار صوت کارگاه های مورد مطالعه 277/7 ± 12/80 دسی بل و میانگین نمره حاصل از فرم غربالگری 964/8± 50/60 بدست آمد. بین نمره فرم غربالگری و اندازه گیری تراز فشار صوت رابطه معنی داری وجود داشت (046/0 p<). درصد کارگران در معرض خطر و در محدوده ایمن به ترتیب 32/29 و 63/3 بدست آمد، بیشترین درصد مربوط به کارگران در محدوده احتیاط 04/67 بود.
    نتیجه گیری
    نتایج نشان داد که انطباق مناسبی در نتایج ارزیابی صدای کارگاه های بالای 20 نفر با دو روش اندازه گیری تراز فشار صوت تراز سنج صوت و استفاده از فرم غربالگری صدا وجود داشته و استفاده از این روش جهت شناسایی کارگاه های که احتمال آلودگی صوتی در آنها وجود دارد موثر است.

    مقدمه
    مواجهه شغلی با میدان های مغناطیسی با فرکانس بی نهایت کم در راهبران قطار بخشی جدایی ناپذیر از این وظیفه بوده و همین امر نگرانی هایی را از نظر سلامت شغل راهبری به وجود آورده است. مطالعه حاضر باهدف بررسی مواجهه شغلی راهبران متروی تهران با میدان های مغناطیسی در فرکانس بی نهایت کم طراحی شد.
    روش کار
    به منظور سنجش مواجهه راهبران، از دو نوع قطار AC و DC داخل شهری و AC بین شهری از هر خط شامل خط 1، 2، 4 و 5 نمونه ای به طور تصادفی انتخاب شد و پس از ایستگاه بندی، اندازه گیری چگالی شار مغناطیسی با فرکانس بی نهایت کم به وسیله دستگاه سه جهته TES– 1394 و با توجه به استاندارد 1994-644 IEEE std انجام شد. سپس مواجهه راهبران در هر یک از قطارها با حدود مواجهه شغلی کشوری مقایسه گردید.
    یافته ها
    بیش ترین میانگین مواجهه در قطارهای AC بین شهری (μT 5/2 ± 1/1) و کمترین مقدار مربوط به قطارهای μT) DC 2/31 ± 0/0) بود. حداکثر مقدار مواجهه در راهبران قطارهای AC خط 5 (μT 9) و حداقل مقدار مربوط به همین خط (μT 01/0) بود. حداکثر مقدار مواجهه در خطوط داخل شهری در قطار AC خط 1 (μT 4/5) و حداقل آن مربوط به همین خط و قطار (μT 08/0) بود.
    نتیجه گیری
    نتایج حاصل از مطالعه نشان داد که در این مطالعه، همچون مطالعات مختلف در دیگر کشورها، هیچ یک از حالات مواجهه در قطارهای مختلف، میزان مواجهه از حدود مجاز مواجهه کشوری و بین المللی فراتر نرفته بود. هرچند این موضوع نباید مبنایی بر بی خطر بودن این میدان ها قرار بگیرد.

    Introduction
    Steel industries are one of the major needs of the country. Noise pollution in steel production operations is an occupational hazard because of special equipment such as electric arc furnaces. The aim of this study was to evaluate noise pollution in various units of a steel company for determining the occupational exposure pattern.
    Methods
    This was a cross-sectional and analytical noise pollution study in various units of a steel company in Iran (Hamadan state). The sound pressure levels were measured in accordance with the ISO-9612 standard and using the Casella-Cel model 450 sound level meter on “A” frequency weighing and the “slow” mode for time response. The frequency analysis in one octave-band was done with measures on “C” frequency weighing. We used the Surfer V.10 software for interpolation and noise maps producing. Microsoft Office Excel 2010 software was used for graph drawing. Noise dosimetry was performed using model TES-1345 dosimeter.
    Results
    The results of this study showed that 56% of the total measurement stations were in the danger zone [above 85 dB(A)]. The maximum sound levels in the areas of electric arc furnaces and ladle furnace were recorded 112.2 dB(A) and 97 dB(A), respectively. The averages of the noise dose in three occupational groups of industrial cleaner, melting and casting were 2133%, 514%, and 577%, respectively. The dominant frequencies recorded in two central frequencies were 125 Hz and 250 Hz, respectively.
    Conclusions
    According to the results, occupational exposure to noise in all the units indicated a dominant frequency of less than 500 Hz and could not be protected by hearing protection devices. The noise doses were over 500% and the time exposure shifts could not be reduced. Therefore, performing technical control plans are of priority in this industry.

    Introduction
    Benzene, Toluene, Ethylbenzene and Xylenes (BTEX) are important parts of the volatile organic compounds (VOCs) family, which are widely used in many industries. These have been classified as carcinogenic compounds and the neurotoxic effects of BTEX have been reported; so, a screening evaluation of workplace air quality for determining BTEX may seem necessary.
    Methods
    In order to measure the workplace concentrations of BTEX in the hydrometallurgical process, six sampling stations were selected. Volatile organic compounds were analyzed using the method 1501 of the National Institute for Occupational Safety and Health (NIOSH). Activated carbon was used for sampling and samples were analyzed by gas chromatography with flame ionization detection (GC-FID).
    Results
    The highest measured levels were observed for ethylbenzene at sampling station 1 (0.8 ppm). The concentration of benzene in this sampling location was 0.05 ppm. The highest toluene concentration was reported at sampling station 4 (0.15 ppm) and the lowest toluene concentration was 0.02 ppm at sampling station 6.
    Conclusions
    Indoor air quality in workplaces can be affected by the presence of BTEX. The results of measurements of BTEX showed low concentration levels of these compounds in the studied workplace.

    Introduction
    Risk assessment of hazardous processes is the priority of risk management. Layer of protection analysis (LOPA) is one of the most popular methods used for risk assessment. Due to the insufficient information or uncertainty in failure rates (PFD) of protective layers, risk assessment based on the conventional LOPA can result in error in calculations. In this study, we tried to use the fuzzy set theory to evaluate the ammonia receiving tank safety, using the LOPA method.
    Methods
    Initially, the fuzzy failure rate of protective layers were calculated using the subjective opinions of professionals. Then, by applying the fuzzy operators, fuzzy possibilities transformed to fuzzy probabilities and subsequently they were deffuzified to crisp failure rate. Afterwards, using the severity fuzzy logic, severity of the outcome event was calculated in the fuzzy form, and subsequently, fuzzy risk index was calculated using the fuzzy matrix.
    Results
    In the ammonia release scenario, calculated severity, probability and risk levels were determined as P: Low, S: High, and R: TNA, and PF = -2.66, SF = 3.99, RF = 3.79 (0.2 TNA, 0.8 NA) for classic and fuzzy LOPA methods, respectively. In addition, after inserting additional layers of protection, the fuzzy risk index reduced from 3.79 (0.2 TNA, 0.8 NA) to 1.92 (0.1 A, 0.8 TA, 0.1 TNA).
    Conclusions
    In the condition of uncertainty and lack of information relating to probability and severity of risk scenarios, the experts’ opinions can be used in forms of linguistic variables and fuzzy relations to reduce calculation errors in risk assessment as much as possible.

    Introduction
    This study aimed to investigate the effect of carpet weaving at a proposed workstation on Upper Trapezius (UTr) fatigue during a task cycle. Fatigue in the shoulder is one of the most important precursors for upper limb musculoskeletal disorders. One of the most prevalent musculoskeletal disorders between carpet weavers is disorder of the shoulder region.
    Methods
    This cross-sectional study, included eight females and three males. During an 80-minute cycle of carpet weaving, Electromyography (EMG) signals of right and left UTr were recorded by the surface EMG, continuously. After raw signals were processed, MPF and RMS were considered as EMG amplitude and frequency parameters. Time series model and JASA methods were used to assess and classify the EMG parameter changes during the working time.
    Results
    According to the JASA method, 58%, 16%, 8% and 8% of the participants experienced fatigue, force increase, force decrease and recovery, respectively in the right UTr. Also, 50%, 25%, 8% and 16% of the participants experienced fatigue, force increase, force decrease and recovery, respectively in the left UTr.
    Conclusions
    For the major portion of the weavers, dominant status in Left and right UTr was fatigue, at the proposed workstation during a carpet weaving task cycle. The results of the study provide detailed information for optimal design of workstations. Further studies should focus on fatigue in various muscles and time periods for designing an appropriate and ergonomics carpet weaving workstation.

    Introduction
    Taxi drivers’ exposure to repeat whole-body vibrations can cause back pain and digestive disorders. Since this type of vibration depends on the car components, this study was carried out to determine the influence of tire characteristics on the amount of whole-body vibrations transmitted to the Peugeot 405 taxi drivers.
    Methods
    In this experimental study, vibration characteristics were measured according to the ISO2631-1 with each of the statuses: tubeless tires fixed and fluid in it (normal air or nitrogen) and also the fluid in the tires fixed with tubes or tubeless on asphalt-paved road. Other variables including tire pressure, engine speed, road gradient, number of passengers, springs, and shock absorbers were kept constant. Then the effect of changes was analyzed using an appropriate statistical test.
    Results
    After changing nitrogen to normal air and tubeless tires to tube, the average of RMS in Z-axis, eight-hour equivalent acceleration A(8) and crest factor were reduced (P 0.9 m/s2) to caution zone (0.45-0.9 m/s2) with a value of 0.8 m/s2.
    Conclusions
    The amount of vibration transmitted to the whole body is sensitive to existence of tubes and tires inflation so that we can reduce the amount of whole-body vibration to lower than the upper limit of the health risk by changing the characteristics of the tire.

    Introduction
    Human errors are the cause of many accidents, including industrial and medical, therefore finding out an approach for identifying and reducing them is very important. Since no study has been done about human errors in the dental field, this study aimed to identify and assess human errors in postgraduate endodontic students of Kerman University of Medical Sciences by using the SHERPA Method.
    Methods
    This cross-sectional study was performed during year 2014. Data was collected using task observation and interviewing postgraduate endodontic students. Overall, 10 critical tasks, which were most likely to cause harm to patients were determined. Next, Hierarchical Task Analysis (HTA) was conducted and human errors in each task were identified by the Systematic Human Error Reduction Prediction Approach (SHERPA) technique worksheets.
    Results
    After analyzing the SHERPA worksheets, 90 human errors were identified including (67.7%) action errors, (13.3%) checking errors, (8.8%) selection errors, (5.5%) retrieval errors and (4.4%) communication errors. As a result, most of them were action errors and less of them were communication errors.
    Conclusions
    The results of the study showed that the highest percentage of errors and the highest level of risk were associated with action errors, therefore, to reduce the occurrence of such errors and limit their consequences, control measures including periodical training of work procedures, providing work check-lists, development of guidelines and establishment of a systematic and standardized reporting system, should be put in place. Regarding the results of this study, the control of recovery errors with the highest percentage of undesirable risk and action errors with the highest frequency of errors should be in the priority of control.

    Introduction
    noise pollution has a negative effect on physiological parameters, efficiency and auditory system. The aim of this study was to evaluate the performance of a sound screening method for estimating sound risk in small workshops of Hamadan city.
    Methods
    This cross-sectional study was conducted in 22 plants with more than 20 workers. Noise screening was done using a screening form. Next, noise level was measured at all the plants by SL 4001 sound level meter and regular grid method. Data were analyzed using Pearson’s test and linear regression by SPSS version 16 software.
    Results
    The mean (SD) scores of sound pressure level and screening form were 80.12 dB (7.277) and 60.50 (8.964) in the 22 understudy plants, respectively. The average measured workshop area and volume were 1448 and 10300, respectively. Pearson’s test revealed a significant relationship between the results of the two methods, (P > 0.046). Also the percentage of workers at risk (greater than 85 dB) was 32.29 and the percentage of workers within safe limit (less than 85 dB) was 63.3. While the largest percentage of workers in the caution zone (82 dB) was 67.04.
    Conclusions
    A correlation was shown between the results of the two methods. The use of this method to identify workshops with the possibility of noise pollution is effective.

    Introduction
    Occupational exposure to Extremely Low Frequency Magnetic Fields (ELF-MFs) in train drivers is an integral part of the driving task and creates concern about driving jobs. The present study was designed to investigate the occupational exposure of Tehran train drivers to extremely low frequency magnetic fields.
    Methods
    In order to measure the driver’s exposure, from each line, a random sample in AC and DC type trains was selected and measurements were done according to the IEEE std 644-1994 using a triple axis TES-394 device. Train drivers were then compared with national occupational exposure limit guidelines.
    Results
    The maximum and minimum mean exposure was found in AC external city trains (1.2±1.5 μT) and DC internal city trains (0.31±0.2 μT), respectively. The maximum and minimum exposure was 9 μT and 0.08 μT in AC trains of line 5, respectively. In the internal train line, maximum and minimum values were 5.4 μT and 0.08 μT in AC trains.
    Conclusions
    In none of the exposure scenarios in different trains, the exposure exceeded the national or international occupational exposure limit guidelines. However, this should not be the basis of safety in these fields.
