    Disease (dis+ ease) is a term denoting to disorder (dis+ order) of function or structure in a living organism. It means feeling of inconvenience. Well known and specific disorders are human-made classifications for better understanding, teaching, and optimization of management. In most instances classic descriptions of diseases are clear-cut definitions with widely accepted well demarcated margins.From this point of view any one may imagine specific and classic disorders as separate islands apparently apart from each other from a “topdown” view (Figure, top).

    Cystic partially differentiated nephroblastoma is a rare tumor of the kidney usually affecting infants. It should be distinguished from Wilms’ tumor with multicystic areas and multilocular cystic nephroma. Multilocular cystic nephroma is a benign tumor whereas Wilms’ tumor with multicystic areas is at the malignant end of the range of classification of such tumours.Cystic partially differentiated nephroblastoma may undergo local recurrence but there is no report of metastasis. In this paper we report the cases of two young boys aged fewer than three years presented with painless tumefaction on left side flank. The diagnosis of partially differentiated cystic nephroblastoma was retained on histological evidences.

    Erythema nodosum is the most common cause of inflammatory nodules secondary to septal panniculitis in the body.It may be observed as an isolated finding or in the context of a systemic disorder. We described herein a young lady with EN- like lesions on her shins finally diagnosed as gram-positive cocci multiloculated subcutaneous abscesses.

    A 15 years-old girl with diabetes mellitus after a major stress expressed that a group of viruses invaded her gastrointestinal tract and a clump adherent damaged these organs (acute stress disorder and then nihilistic delusion). No evidence of HIV infection was found, but she believed that she was carrier for viruses (aids neurosis). She was resistant and sensitive to pharmacotherapy and psychotherapy, but response to Electroconvulsive therapy ECT (six sessions) was favorable. Stress management helped her physicians to control her blood sugar.

    Creutzfeldt-Jakob disease (CJD) is a rare disease that belongs to the category of transmissible spongiform encephalopathies. The condition is invariably fatal and progresses with severe dementia with psychiatric signs and symptoms, as well as cortical, subcortical and cerebellar signs. We present a case of a woman aged 54 years, who developed CJD without risk factors for the disease.The usual manners of transmission could not be confirmed for this patient, who died ten months after the onset of symptoms. CJD is also a serious health care concern, since patients who are still asymptomatic may be donating blood for transfusions or for blood products.

    Vesicorectal fistula and urethrorectal fistula are uncommon complications of urological endoscopic surgery. Several cases of iatrogenic urethrorectal fistulas due to urologic procedures have been reported.This is the first report of a case of vesicorectal fistula occurring during endoscopic resection of a bladder lesion. Because of the persistence of the fistula after a conservative follow-up period, a single stage primary repair with omental flap interposition with no colostomy was performed. In the follow up of the patient, no fistula or bladder mass were observed. A single stage primary repair with the interposition of an omental flap is usually a safe and sufficient method of treatment

    Acinic cell carcinoma is a rare salivary gland tumor. The variant with lymphoid stroma is even rarer and has distinct histological features. We describe the case of a forty three year-old woman who presented with a left retroauricular mass of 5 years duration; magnetic resonance imaging revealed a well-circumscribed mass of 1.5 cm suggestive of pleomorphic adenoma.The patient underwent inferior polar parotidectomy. Frozen section analysis was performed and the interpretation of the sample suggested a metastatic intraparotid lymph node. The definitive final diagnosis of acinic cell carcinoma with lymphoid stroma was made on the basis of histological and histochemical findings, after a review of the literature.

    Giant hydrocele is rarely seen. These patients usually present as a painless swelling a case of giant hydrocele in 26 year old male is reported. Due to social embracement, patient was reluctant for get examining scrotal area. Presentation was as a painless scrotal swelling. Hydrocelectomy was done. The brief report reflects neglect and ignorance of patient in seeking medical care.

    Endometrial stromal sarcoma (ESS) is a rare low-grade malignancy affecting mainly the uterus. Primary extrauterine occurrence of this tumor, involving most commonly the ovary, is exceedingly uncommon, and is retained after excluding an occult or misdiagnosed EES of the uterus.A 79 year-old-woman, with a previous history of hysterectomy for prolapse and leiomyoma, presented with a bilateral ovarian mass. Morphological characteristics suggested ESS with extraovarian spread, peritoneal implants and endometriotic foci. The sections of the previous "leiomyoma" revealed ESS with prominent foci of epithelial-like structures. We discuss through this case clinicopathologic features of ESS, with emphasis on its unpredictable outcome.
