    قبل از استقرار و توسعه رشته تخصصی طب اورژانس در جهان، وضعیت ارائه خدمات در بخش های اورژانس بیمارستانی به شکلی دیگر بود. در آمریکا قبل از ایجاد بخش اورژانس به شکل تخصصی، آن را یک مکان دست دوم و بی اهمیت در نظر می گرفتند. جایی که در آن صف های طولانی انتظار و تشخیص های اشتباه امری طبیعی محسوب می شد. معمولا این بخش توسط پرستاران اداره می شد و آنها بر اساس برآورد اولیه شان از پزشکان درخواست کمک می کردند. تیم پزشکی اورژانس از یک بیمارستان تا بیمارستان دیگر تفاوت فاحش داشت. در بیمارستان های غیر دانشگاهی، معمولا پزشکان اورژانس کسانی بودند که تازه شروع به کار کرده و یا سال-های آخر کار خود را می گذراندند و در بیمارستان های دانشگاهی بخش اورژانس منطقه کارورزان با کشیک های 24 ساعته در نظر گرفته می شد و رزیدنت های سال های بالاتر در رشته های مختلف تخصصی تا حد امکان از بخش اورژانس دوری می کردند.

    مقدمه
    تروما از مهمترین دلایل مراجعه کودکان به بخش اورژانس و شایعترین علت ناتوانی و مرگ و میر در آنها محسوب می شود. آگاهی از اطلاعات اپیدمیولوژیک تروما در این گروه سنی به برنامه ریزی دقیق جهت پیشگیری از این حوادث کمک می کند. لذا این مطالعه با هدف بررسی اپیدمیولوژی تروما در کودکان مراجعه کننده به بخش اورژانس انجام شد.
    روش کار
    در یک مطالعه مقطعی گذشته نگر به روش سرشماری، پرونده کودکان مراجعه کننده به واحد ترومای بخش اورژانس بیمارستان امام حسین، تهران، ایران در سال های 90 و 91 مورد بررسی قرار گرفتند. اطلاعات مربوط به سن، جنس، زمان بروز و مکانیسم تروما، محل آناتومیک تروما، اقدامات انجام شده و پیامدها جمع آوری شد و با استفاده از نرم افزار آماری SPSS 16 مورد آنالیز توصیفی قرار گرفتند.
    یافته ها
    اطلاعات موجود در پرونده بالینی 547 کودک به روش سرشماری جمع آوری و مورد بررسی قرار گرفت (3/69 درصد پسر). 169 (9/30 درصد) مورد در فصل تابستان و 322 (9/58 درصد) مورد بین ساعت 16 تا 24 مراجعه کرده بودند. حوادث رانندگی با 286 (3/52 درصد) مورد شایع ترین مکانیسم و ناحیه سر و گردن با 217 (7/39 درصد) مورد شایع ترین محل تروما بودند. در نهایت 537 (2/98 درصد) کودک با حال عمومی خوب از بیمارستان ترخیص شدند و 10 (8/1 درصد) مورد ترومای منجر به مرگ گزارش گردید. بیشترین ترومای منجر به مرگ ترومای قفسه سینه و شکم (3/14 درصد) بود و رابطه معنی داری بین مکانیسم تروما (845/0 = p) و زمان بروز (895/0 = p) حادثه با مرگ و میر مشاهده نشد.
    نتیجه گیری
    بر اساس نتایج مطالعه حاضر، بیشترین موارد تروما زیر 14 سال مراجعه کننده به بخش اورژانس بیمارستان امام حسین مربوط به پسر بچه ها، فصل تابستان، ساعت 16 تا 24 و با مکانیسم تصادفات رانندگی بوده است. 8/1 درصد موارد تروما با مرگ و میر همراه بودند و بیشترین ترومای منجر به مرگ مربوط به ترومای قفسه سینه و شکم بود.

    مقدمه
    افزایش غلظت اکسیدان ها یا افت آنتی اکسیدانی منجر به از بین رفتن تعادل اکسیدان-آنتی اکسیدانی و به وجود آمدن حالتی به نام استرس اکسیداتیو می شود. به نظر می رسد جبران کمبود آنتی اکسیدانی در شرایط مذکور بتواند در کاهش ضایعات ناشی از بیماری و تسریع روند بهبود کمک کننده باشد. لذا مطالعه حاضر با هدف ارزیابی تغییرات سطح سرمی آنزیم آنتی اکسیدان پاراکسوناز-1 در بیماران دچار سوختگی حرارتی طراحی شده است.
    روش کار
    مطالعه موردی-شاهدی آینده نگر حاضر از تاریخ اول تیر 1393 به مدت 3 ماه بر روی بیماران دچار آسیب سوختگی حرارتی بستری شده در بخش اورژانس بیمارستان مطهری که حداکثر 4 ساعت از سوختگی آنها گذشته بود انجام شد. گروه شاهد از میان افراد سالم انتخاب شدند. چک لیستی شامل اطلاعات دموگرافیک، سابقه بیماری های قبلی، نتایج آزمایشات و اقدامات پاراکلینیک و سطح سرمی آنزیم پاراکسوناز برای دو گروه مورد مطالعه تکمیل گردید. در نهایت با استفاده از نرم افزار آماری SPSS 19 اقدام به آنالیز آماری و مقایسه نتایج بین دو گروه شد.
    یافته ها
    33 بیمار مبتلا به سوختگی حرارتی با 53 فرد سالم به عنوان گروه شاهد مورد مقایسه قرار گرفتند. میانگین سنی در گروه کنترل برابر 09/9±61/36 سال و در گروه دچار سوختگی 52/11±69/37 سال بود (632/0=p). میانگین درصد سوختگی بیماران مورد مطالعه 37/23 ± 76/30 (100-3) درصد بود. دو گروه در مورد ویژگی های پایه با هم اختلاف معنی داری نداشتند (05/0 > p). میانگین سطح سرمی آنزیم پاراکسوناز در گروه مورد و شاهد به ترتیب 80/136 ± 52/228 و 08/149 ± 65/350 واحد بین المللی در لیتر بود (001/0p <). ارتباط معنی داری بین سطح فعالیت پاراکسونازی بیماران دچار سوختگی و درصد سوختگی مشاهده نشد (532/0 = p). نتایج آنالیز زیر گروه ها حاکی از تاثیر مخدوش کننده معنی دار سطح آلبومین و کلسترول با دانسیته بالا در سطح فعالیت آنزیم پاراکسوناز-1 بود.
    نتیجه گیری
    بر اساس نتایج مطالعه حاضر به نظر می رسد که علی رغم پایین بودن سطح فعالیت آنتی اکسیدانی پاراکسوناز-1 در بیماران دچار سوختگی حرارتی، با توجه به پایین بودن سطح آلبومین و کلسترول با دانسیته بالا در این بیماران نمی توان سوختگی را به عنوان تنها عامل دخیل در این امر دانست.

    مقدمه
    اولویت بندی بیماران برای ارایه خدمات بر اساس شدت آسیب و انجام بهترین اقدام درمانی برای بیشترین افراد در کمترین زمان را تریاژ می گویند. هرچند سابقه راه اندازی تریاژ بیمارستانی در ایران به چند سال اخیر بر می گردد ولی امروزه تقریبا تمام اورژانس های بیمارستانی در کشور دارای این واحد در ساختار خود هستند. مسوولیت انجام تریاژ بر عهده پرستاران گذاشته شده است ولی اطلاعات اندکی از میزان دانش و عملکرد ایشان درباره تریاژ در دسترس می باشد. لذا این مطالعه با هدف تعیین میزان آگاهی و عملکرد پرستاران بخش اورژانس درباره تریاژ بیمارستانی انجام گرفته است.
    روش کار
    این مطالعه از نوع مقطعی آینده نگر می باشد که به منظور بررسی میزان دانش و عملکرد پرستاران در زمینه تریاز بیمارستانی صورت گرفت. نمونه گیری به روش تصادفی ساده و از پرستاران بخش اورژانس بیمارستانهای آموزشی استان یزد انتخاب گردید. جمع آوری داده ها با استفاده از پرسشنامه ای که روایی و پایایی آن قبلا مورد تایید قرار گرفته بود صورت گرفت. این پرسشنامه در سه قسمت مشتمل بر 11 سوال دموگرافیک، 15 سوال ارزیابی دانش و 10 سوال ارزیابی عملکرد بود. داده ها با استفاده از آزمون های آماری توصیفی و استنباطی و نرم افزار آماری SPSS 16.0 مورد تجزیه و تحلیل قرارگرفتند.
    یافته ها
    در مجموع 45 پرسشنامه تکمیل و جمع آوری شد. 2/62 درصد نمونه ها مرد با میانگین سنی 74/5 ± 92/35 سال و 8/37 درصد زن با میانگین سنی 37/7 ± 82/30 سال بودند. میانگین سنی کلیه پرستاران مورد مطالعه نیز 80/6 ± 34 سال بود. میانگین سابقه کار پرستاران مورد مطالعه به طور کل 89/6 ± 07/10 سال و میانگین سابقه کاری ایشان در بخش اورژانس 70/4 ± 31/5 سال بود. بررسی نتایج بخش دوم پرسشنامه که مربوط به میزان دانش پرستاران بود نشان داد که 07/42 درصد پاسخ های داده شده به این بخش صحیح بوده و میانگین نمرات نمونه های مورد پژوهش 96/1 ± 33/6 بود. همچنین بررسی نتایج میزان عملکرد پرستاران نیز نشان داد که 44/50 درصد پاسخ های داده شده به این بخش صحیح بوده و میانگین نمرات کسب شده در این زمینه 47/1 ± 5 بود. در مجموع 33/45 درصد پاسخ های پرستاران شرکت کننده در پژوهش به آزمون مربوط به سناریو ها صحیح بود. میانگین نمره کل (مجموع بخش میزان دانش و عملکرد) شرکت کنندگان نیز معادل 50/2 ± 33/11 بدست آمد. آنالیز آماری تنها ارتباط معنی داری را بین شیفت کاری و نمره مربوط به میزان دانش پرستاران در زمینه تریاژ (003/0p =) نشان داد.
    نتیجه گیری
    به نظر می آید میزان آشنایی و احاطه پرستاران شاغل در بخش های اورژانس مراکز تحت پوشش دانشگاه علوم پزشکی شهید صدوقی یزد در خصوص تریاژ در سطح کمتر از متوسط قرار دارد. در این راستا برگزاری دوره های باز آموزی مخصوص تریاژ برای پرسنل به منظور ارایه خدمات با کیفیت بالا پیشنهاد می شود.

    مقدمه
    در سراسر جهان آسیب های ناشی از تصادفات وسایل موتوری یک معضل در حال رشد برای سلامت جمعیت عمومی محسوب می شوند. سالانه تعداد افراد زیادی به علت تصادفات جاده ای شدیدا زخمی می شوند و یا جان خود را از دست می دهند. لذا بر آن شدیم تا با طراحی مطالعه حاضر به بررسی دموگرافیک شکستگی های ناحیه سر و گردن ناشی از تصادفات موتورسیکلت بپردازیم.
    روش کار
    در این مطالعه مقطعی گذشته نگر تمامی بیمارانی که در طول سال 1392 به بخش اورژانس ارجاع شدند و دچار آسیب های ناحیه سر و گردن ناشی از تصادف با موتورسیکلت بودند به صورت سرشماری وارد گردیدند. چک لیستی شامل اطلاعات دموگرافیک، مکانیسم تروما، زمان وقوع تروما، راکب یا سرنشین بودن، کاربری وسیله نقلیه، استفاده از تجهیزات ایمنی، شکستگی های ناحیه سر و گردن، آسیب های همراه، یافته های تصویربرداری و پیامد نهایی از طریق مراجعه به پرونده های بالینی برای تمامی بیماران تکمیل و مورد آنالیز توصیفی قرار گرفت.
    یافته ها
    386 بیمار با میانگین سنی بیماران 79/10 ± 4/28 سال مورد ارزیابی قرار گرفتند (4/84 درصد مرد). اکثر بیماران در محدوده سنی کمتر از 30 سال قرار داشتند (001/0p =). سه ماهه اول سال با 152 (4/39 درصد) مورد بیشترین آمار تصادفات از جهت توزیع فصلی را به خود اختصاص داد. 223 (8/57 درصد) مورد از تصادفات در طی روز افتاده بود (037/0p =). تنها 191 (5/49 درصد) نفر از بیماران از کلاه ایمنی استفاده کرده بودند. شایع ترین شکستگی مربوط به مهره های گردنی با 61 (8/15 درصد) مورد بود. در نهایت 207 (6/53 درصد) مورد بعد از انجام اقدامات تشخیصی و درمانی مقتضی از اورژانس ترخیص شدند. 175 (4/45 درصد) مورد در بخش های مختلف بیمارستان برای اقدامات درمانی بیشتر بستری و بعد از مدتی با حال عمومی خوب ترخیص گردیدند. 4 (1 درصد) مورد هم مرگ و میر ثبت شد.
    نتیجه گیری
    نتایج این مطالعه اپیدمیولوژیک نشان می دهد، شیوع شکستگی های ناحیه سر و گردن در تصادف با موتورسیکلت در افراد با سن پایین، جنس مذکر و عدم استفاده از تجهیزات ایمنی بیشتر است. شایعترین محل شکستگی های ناحیه سر و گردن به ترتیب مربوط به شکستگی مهره های گردن، جمجمه و در نهایت استخوان های صورت بود.

    مقدمه
    به موازات افزایش جمعیت سالمندان در جوامع، مشکلات پزشکی این گروه سنی نیز افزایش می یابد. از جمله این مشکلات می توان به حوادث قابل پیشگیری اشاره نمود. هر ساله بیش از 5 میلیون نفر در نتیجه این گونه آسیب ها جان خود را از دست می دهند. لذا مطالعه حاضر با هدف بررسی علل و پیامد حوادث قابل پیشگیری در سالمندان طراحی شده است.
    روش کار
    پژوهش حاضر یک مطالعه مقطعی گذشته نگر بر روی سالمندان بستری شده در بیمارستان طی دو سال می باشد. اطلاعات مورد نیاز از طریق سرشماری و با استفاده از چک لیستی مشتمل بر اطلاعات دموگرافیک، علت حادثه، طول مدت بستری، تعداد دفعات مراجعه و پیامد نهایی جمع آوری و با استفاده از نرم افزار آماری SPSS 16 مورد آنالیز توصیفی قرار گرفت.
    یافته ها
    در مجموع اطلاعات 2336 بیمار با میانگین سنی 02/9 ± 24/72 سال مورد ارزیابی قرار گرفت (7/50 درصد زن). فراوان ترین علل حوادث به ترتیب عبارت بودند از سقوط 1033 (2/44 درصد) مورد، مسمومیت عمدی 382 (4/16درصد) مورد و مسمومیت تصادفی 360 (4/15 درصد) مورد. متوسط طول مدت بستری 70/7 ± 5 روز بود. تعداد سالمندان فوت شده در طی زمان مورد مطالعه 144 (2/6 درصد) نفر بود. از جهت نوع حادثه (001/0 > p) و پیامد نهایی (001/0 > p) تفاوت معنی داری بین دو جنس مشاهده گردید.
    نتیجه گیری
    بر اساس یافته های مطالعه حاضر سقوط، مسمومیت عمدی و مسمومیت تصادفی به ترتیب شایعترین علل بروز حوادث قابل پیشگیری در سالمندان مورد مطالعه بودند. اکثریت موارد بعد از بهبودی نسبی از بیمارستان ترخیص شده بودند و سه عامل فوق الذکر شیوع بالاتری در زنان داشتند. بهبودی کامل درمسمومیت ها، عدم بهبودی کامل در موارد سقوط و فوت در مسمومیت ها و سوختگی بیشترین فراوانی ها را داشتند.

    مقدمه
    با توجه به شیوع تروما در جامعه و عوارض و هزینه های آن، امروزه تروما یکی از مشکلات اساسی تهدید کننده سلامت جامعه محسوب می شود. آگاهی از اطلاعات اپیدمیولوژیک بیماران می تواند در برنامه ریزی جهت کاستن از بار بهداشتی این معضل کمک کننده باشد. لذا این پژوهش به منظور بررسی اپیدمیولوژی ترومای اندام فوقانی در مصدومان مراجعه کننده به بخش اورژانس بیمارستان امام حسین صورت گرفته است.
    روش کار
    پژوهش مقطعی حاضر با نمونه گیری سرشماری و بر روی کلیه مصدومان مراجعه کننده با ترومای اندام فوقانی مراجعه کننده به بخش اورژانس بیمارستان امام حسین، تهران، ایران، در سال 1391 انجام شد. اطلاعات مورد نظر با استفاده از چک لیستی حاوی سوالاتی پیرامون اطلاعات دموگرافیک (سن، جنس)، مکانیسم تروما، زمان تروما، نوع آسیب و مداخلات درمانی جمع آوری گردید. داده های حاصل به کمک برنامه آماری SPSS 19.0 مورد آنالیز آماری قرار گرفتند.
    یافته ها
    751 بیمار با میانگین سنی 6/7 ± 27 مورد بررسی قرار گرفتند. 616 (82 درصد) بیمار مرد بودند. محدوده سنی 20 تا 30 سال با 248 (33 درصد) مورد بیشترین موارد را به خود اختصاص دادند. بیشترین موارد در طول فصل بهار با 205 (3/27 درصد) مورد رخ داده بود. اکثریت موارد شامل 339 (1/45 درصد) مورد در بین ساعت 8 صبح الی 16 بعداز ظهر به وقوع پیوسته بودند. آسیب ناشی از تصادف با موتورسیکلت با 185 (6/24 درصد) مورد شایعترین مکانیسم در موارد تحت بررسی بود. از مجموع موارد بررسی شده، 575 (6/76 درصد) بیمار ترومای ایزوله اندام فوقانی داشتند و 176 (4/23 درصد) بیمار دچار ترومای متعدد شده بودند که درگیری اندام فوقانی را نیز شامل می شد. شایعترین آسیب های نیازمند مداخله جراحی به ترتیب عبارت بودند از: شکستگی دیستال رادیوس با 143 (34 درصد) مورد، آسیب های بافت نرم با 75 (8/17 درصد) مورد و شکستگی ایزوله اولنا با 46 (9/10 درصد) مورد. 421 (1/56 درصد) مورد مصدومان تحت انجام عمل جراحی قرار گرفتند..
    نتیجه گیری
    بنابر نتایج حاصل از این مطالعه بیشترین موارد ترومای اندام فوقانی در مراجعین به اورژانس مورد مطالعه شامل مردان بین 20-30 سال، در فصل بهار و در ساعت 8 تا 16 بود. شایعترین مکانیسم عامل بروز سانحه تصادف راکبین موتورسیکلت بود و شکستگی دیستال رادیوس شایعترین آسیب نیازمند مداخله جراحی بود.

    پنومومدیاستن خودبخودی عارضه نادری است که غالبا سیر خوش خیم و خود محدود شونده ای دارد. تظاهر غالب آن درد رترو استرنال ناگهانی می باشد که می تواند به دنبال مانور والسالوا، استفراغ، سرفه های مکرر، ورزش سنگین، استشاق ماری جوانا، تروما و اینتوباسیون اتفاق بیفتد. با توجه به شیوع اندک و اهمیت در نظر داشتن پنومومدیاستن خودبخودی در تشخیص افتراقی بیماران مراجعه کننده به بخش اورژانس با درد قفسه سینه حاد به معرفی یک بیمار 17 ساله با شکایت درد قفسه سینه و تشخیص نهایی پنومومدیاستن خود به خودی خواهیم پرداخت.

    پزشکان اورژانس هر روزه با سوالات متعددی در زمینه مدیریت صحیح بیماران و انتخاب مناسبترین تست آزمایشگاهی یا روش تصویربرداری مواجه هستند. آگاهی در مورد ویژگی های غربالگری ابزارهای تشخیصی مورد استفاده در این بخش نقش مهمی در پاسخ به این سوالات بازی می کند. در مقالات گذشته از سری مقالات آموزشی مجله اورژانس به بررسی ویژگی های مذکور پرداختیم. در نوشته حاضر به نحوه استفاده از این ویژگی های غربالگری در انتخاب ابزارهای تشخیصی صحیح خواهیم پرداخت.

    Before establishment of emergency medicine specialty around the world, emergency departments (ED) were deemed unimportant and long waiting time and misdiagnosis were common. However, the need for specialty in this regard was sensed during the 1960s and 1970s and it was established in the 80s and 90s. In Iran, establishment of emergency medicine in 2001 has revolutionized patient care. Although emergency physicians face some challenges, the most important thing is carrying out their responsibilities. An emergency physician should be responsible and reliable and people have a high expectation of them. In this letter, authors tried to briefly review the history of emergency medicine establishment in Iran and look into the future by discussing some challenges of this specialty.

    Introduction
    Trauma is one of the most important reasons that children visit emergency departments (ED) and is the most common cause of disability and mortality among them. Having epidemiological information on trauma in this age range, aids in accurate planning for prevention of these events. Therefore, the present study aimed to evaluate the epidemiology of trauma in pediatric trauma patients presented to ED.
    Methods
    In a retrospective cross-sectional study using census method, the profiles of children presented to the trauma unit in ED of Imam Hossein Hospital, Tehran, Iran during 2011 and 2012 were evaluated. Data regarding age, sex, time and mechanism of trauma, anatomic site of trauma, measures taken, and outcome were gathered and descriptively analyzed using SPSS 16.
    Results
    Data from the clinical profiles of 547 children were gathered using census method and evaluated (69.3% male). 169 (30.9%) cases had occurred in summer and 322 (58.9%) had visited between 4 PM and 12 AM. Car accidents with 286 (52.3%) cases were the most common mechanism, and head and neck area was the most common anatomic site of trauma. Finally, 537 (98.2%) children were discharged with a good general condition and 10 (1.8%) died. Most common traumas leading to death were chest and abdominal traumas (14.3%) and no significant relationship was detected between mortality and trauma mechanism (p = 0.845) or timing (p = 0.895).
    Conclusion
    Based on the findings of the present study, most trauma cases under the age of 14 presented to ED of Imam Hossein Hospital, Tehran, Iran, were boys, in summer, between 4 PM and 12 AM, and car accident-related. 1.8% of traumas led to mortality, the most common causes being chest and abdominal traumas.

    Introduction
    An increase in oxidant concentration or a drop in anti-oxidants leads to an imbalance in oxidant/anti-oxidant equilibrium and a condition called oxidative stress. It seems that recovery from anti-oxidant decrease in this situation can help reduce the effects of the disease and accelerating recovery. Therefore, the present study was designed aiming to evaluate changes in serum paraoxonase-1 levels in thermal burn patients.
    Methods
    This prospective case-control study was carried out from July to October 2014 on thermal burn patient hospitalized in the emergency department of Motahari Hospital, within 4 hours of burning. The control group were healthy people. A checklist consisting of demographic data, medical history, test results, paraclinical measures taken, and serum paraoxonase-1 level was filled for both groups. Finally, SPSS version 19 was used for statistical analysis and comparing the results between the 2 groups.
    Results
    33 thermal burn patients were compared with 53 healthy people as the control group. Mean age was 36.61 ± 9.09 years in control group and 37.69 ± 11.52 years in burn group (p = 0.632). Mean burn percentage in patients was 30.76 ± 23.37 (3 – 100). The 2 groups were not significantly different regarding baseline characteristics (p > 0.05).     Conclusion
    Based on the results of the present study, it seems that although the level of paraoxonase-1 anti-oxidative activity in thermal burn patients is low, since albumin and high-density cholesterol levels are low, burn might not be the only cause.

    Introduction
    Determining patients’ treatment priority based on the severity of their condition, and choosing the best treatment for the highest possible number of patients in the shortest time is called triage. Although triage has only been used in Iranian hospitals since a few years ago, almost all hospital emergency services over the country currently have this unit. Nurses are responsible for triaging patients, but there is little data on their knowledge and quality of practice in this regard. Therefore, this study was aimed to assess the knowledge level and practice of emergency department (ED) nurses regarding hospital triage.
    Methods
    This study is a prospective cross-sectional one, carried out with the aim of evaluating the knowledge level and practice of nurses regarding hospital triage. Simple random sampling was used to select ED nurses from teaching hospitals in Yazd province. Data were gathered using a questionnaire, validity and reliability of which had been confirmed before. The questionnaire comprised of 3 parts, 11 demographic questions, 15 questions evaluating knowledge, and 10 assessing practice. Data were analyzed using SPSS 16.0 and descriptive and inferential statistical tests.
    Results
    A total of 45 questionnaires were filled and gathered. 62.2% of cases were male with the mean age of 35.92 ± 5.74 years, and 37.8% were female with the mean age of 30.82 ± 7.37 years. Mean age of all participants was 34 ± 6.80 years. Mean work experience of the studied nurses was 10.07 ± 6.89 years and their mean work experience in ED was 5.31 ± 4.70 years. Evaluation of the second part, regarding their knowledge, revealed that 42.07% of the answers to the questions were correct and their mean score was 6.33 ± 1.96. In addition, their practice assessment showed that 50.44% of the answers given in this part were right and their mean score in this regard was 5 ± 1.47. In total, 45.33% of the answers given by the participants were correct regarding the scenarios. Mean overall score (sum of the scores obtained from knowledge and practice parts) for the participants was 11.33 ± 2.50. Statistical analysis found a significant correlation only between the working shift and the knowledge score (p = 0.003).
    Conclusion
    It seems that the knowledge and experience of the nurses working in EDs of centers under supervision of Shahid Sadoughi University of Medical Sciences, Yazd, Iran, regarding triage, is below average. Consequently, it is suggested to hold special retraining courses in the field of triage for the staff in order to improve the quality of care.

    Introduction
    Injuries resulting from motor-vehicle accidents are a growing burden to health all over the world. Each year, many people get severely injured or die due to road traffic accidents. Therefore, we aimed to evaluate the demographics of head and neck fractures caused by motorcycle accidents.
    Methods
    In the present retrospective cross-sectional study, all the patients who were referred to emergency department, during the time from March 2013 to March 2014, with head and neck injuries due to motorcycle accidents were included using census sampling. A checklist consisting of demographic data, trauma mechanism, time of trauma, being the driver or the passenger, using safety equipment, head and neck fractures, accompanying injuries, imaging findings, and final outcome was filled for all patients using their medical files, and the data were used for descriptive analysis.
    Results
    386 patients with the mean age of 28.4 ± 10.79 years were evaluated (84.4% male). Most of the patients were under 30 years old (p = 0.001). Spring, with 152 (39.4%) cases, had the highest rate of accidents among the seasons. 223 (57.8%) accidents happened in the daytime (p = 0.037). Only 191 (49.5%) patients had used helmets. The most frequent fracture was cervical vertebrae fracture with 61 (15.8%) cases. Finally, 207 (53.6%) of the patients were discharged from emergency department after diagnostic and therapeutic measures were taken. 175 (45.4%) cases were hospitalized in other departments for further treatments and were later discharged with good general condition. 4 (1%) cases of mortality were recorded.
    Conclusion
    The results of the present epidemiologic study reveal that the prevalence of head and neck fractures in motorcycle accidents is higher in young people, male sex, and those who do not use safety equipment. The most common site for head and neck fractures are cervical vertebrae, skull, and face bones, respectively.

    Introduction
    With the growing population of the elderly in societies, the medical problems of this age range also increases. Preventable accidents are among these problems. More than 5 million people die as the result of these injuries each year. Therefore, the present study was done aiming to evaluate the causes and outcomes of preventable accidents in the elderly.
    Methods
    The present study is a retrospective cross-sectional one, performed on elderly patients admitted to a hospital during 2 years. Required data were gathered using census method via a checklist consisting of demographic data, cause of the accident, length of hospital stay, number of times revisiting the hospital, and final outcome. Data were then descriptively analyzed using SPSS 16 statistical software.
    Results
    2336 patients with the mean age of 72.24 ± 9.02 years were evaluated (50.7% female). The most common causes of accidents were falling with 1033 (44.2%) cases, intentional poisoning with 382 (16.4%) cases, and unintentional poisoning with 360 (15.4%) cases, respectively.     Conclusion
    Based on the findings of the present study, falling, intentional poisoning and unintentional poisoning, respectively, were the most common causes of preventable accidents in the studied elderly population. Most cases were discharged after partial recovery and the 3 afore-mentioned causes had higher prevalence in women. The most frequent outcomes were complete recovery in poisoning, partial recovery in falling, and death in poisoning and burn.

    Introduction
    Considering its high prevalence and cost as well as frequent complications, trauma is one of the major health problems in Iran. Knowing the epidemiologic characteristics of these patients could be helpful in arrangement for decreasing the burden of disease. This study was aimed to evaluate the epidemiology of upper extremity trauma among patients visiting trauma unit of emergency department (ED).
    Methods
    The present cross-sectional study was done using census method on all patients with upper extremity trauma visiting the ED of Imam Hossein Hospital, Tehran, Iran in 2012. Required data were gathered using a checklist consisting of questions regarding demographic data (age, sex), trauma mechanism, time of trauma, type of trauma, and therapeutic measures taken. Gathered data were statistically analyzed via SPSS version 19.0.
    Results
    751 patients with the mean age of 27 ± 7.6 years were evaluated. 616 (82%) patients were male and the highest frequency belonged to the age range of 20 – 30 years with 248 (33%) cases. 339 (45.1%) accidents had occurred between 8 AM and 4 PM. Injury caused by motorcycle accidents was the most common mechanism of trauma among those evaluated with 185 (24.6%) cases. 575 (76.6%) cases had an isolated extremity trauma and 176 (23.4%) had multiple trauma that included extremity trauma. The most prevalent injuries that needed surgical intervention were distal radius fracture with 143 (34%) cases, soft tissue injury with 75 (17.8%) cases, and isolated ulna fracture with 46 (10.9%) cases.
    Conclusion
    Based on the results of this study, the most common cases of upper extremity trauma occurred in men, age range of 20 – 30 years, spring, and between 8 AM and 4 PM. The most common trauma mechanism was motorcycle accident and distal radius fracture was the most prevalent injury that needed surgical intervention.

    Spontaneous pneumomediastinum is a rare condition, which is usually benign and self-limiting. Its main presentation is sudden retrosternal pain that may occur following valsalva maneuver, vomiting, frequent cough, heavy exercise, marijuana inhalation, trauma, and intubation. Given the low prevalence and importance of paying attention to spontaneous pneumomediastinum in screening patients presented to the emergency department with acute chest pain, here we will introduce a 17-year-old patient with chief complaint of chest pain and final diagnosis of spontaneous pneumomediastinum.

    Emergency physicians face numerous questions regarding proper management of patients and selection of the best laboratory test or imaging every day. Knowledge on screening performance characteristics of the diagnostic tools used in this department plays an important role in finding the answers to these questions. We assessed these characteristics in the previous parts of educational article series in Emergency journal. In the present manuscript, we will describe how to use these screening characteristics for selecting the right diagnostic tools.
