A SURVEY ON PROGNOSTIC FACTORS IN PATIENTS WITH DIABETIC FOOT ULCER IN TALEGHANI HOSPITAL IN URMIA

Diabetic foot ulcer is one of the newest complications of diabetes mellitus and needs effective management if not it leads to disability and lower quality of life due to amputation. The aim of this study was to investigate clinical, laboratory variables and determining prognostic factors for designing a protocol for prevention of development of diabetic foot ulcer and amputation in patients with diabetic foot ulcer. This prospective cohort study was conducted from May 2012-May 2013 in Taleghani Hospital of Urmia University of Medical Sciences. Patients with grade 2 or more diabetic foot ulcer according to meggit/wagner grading were selected. Patients were followed to determine the recovery or amputation of foot for three months. Data collection was through check list and analyzed using SPSS version 16 and the significant relationship was below 0.05. From 72 patients that participated almost 58.3% were male, 47.2% had atherosclerosis, 94.4% of patients had type 2 Diabetes mellitus, 70.8% had ESR>=50 and 35.9% (the most percent) had CRP=3+, 80.6% had leukocytosis <12000, 95.6% had HbA1c>=7, 58.6% had LDL<100, 95.2% had HDL<50, 67.2% had TG<200, 50% using oral agents for blood sugar control. 55.6% of patient's ulcer was treated by ceftriaxone, vancomycin, clindamycin (metronidazole). There was no significant relationship between sex, education, residence, duration of Diabetes mellitus, ESR, Atherosclerosis, leukocytosis>=12000, HbA1c>=7, LDL>=100, HDL<50, drug type to control blood sugar (injection or oral), Diabetes mellitus type and amputation. Instead significant relationship was found between CRP, TG and amputation over 90%.
