The survey of patient safety culture in Razi Psychiatric Center in Tehran

Introduction
Safety is one of the main factors in providing quality health care in health care providers. Safety culture in an organization is the most important determinants of patients’ safety. This study aimed to determine patients’ safety culture in Razi Psychiatric Center in Tehran.
Materials and Methods
This is a descriptive study that was conducted in 2014 with participation of 164 employees of the Razi Psychiatric Center. Data collection was carried out using the Persian version of the standard questionnaire “Hospital Survey on Patient Safety Culture’’. Validity and reliability of the questionnaire was confirmed base on the previous studies. Data was analyzed by SPSS/19.
Findings
Average percentage of total positive response of the patient safety culture was 42%. The highest percentage of positive responses to feedback and communication on errors was 62%, organizational learning and continuous improvement was 59%. The lowest percentage of no punitive response to errors and communication openness were 8% and 32% respectively. There was no difference in scores between units of patients’ safety culture (P=0.359). In total 29.8% of respondents on patients’ safety in units were weak and 50 % of participants did not report any events.
Conclusion
The findings revealed that patients’ safety culture in this center was weak and requires improvement. Communication openness and no punitive response to errors is essential to enhance the safety culture. To better promote the patients’ safety culture, planning an education programs is suggested.
