Prevention of Acute Post- Traumatic Bacterial Endophthalmitis with Combination of Intraocular Gentamicin and Clindamycin

Abstract:
To evaluate the effectiveness of combined intraocular injection of gentamicin and clindamycin in the prevention of acute post–traumatic bacterial endophthalmitis following penetrating ocular injuries. A prospective randomized double–blind study was conducted on 60 eyes of 60 patients with penetrating ocular injuries. Following primary repair, the eyes were randomized into two groups: antibiotic-treated group who received a single intracameral or intravitreal injection of 0.1 ml containing 40 μg gentamicin and 45 μg of clindamycin. Placebo injection group received an intracameral or intravitreal injection of 0.1 ml of BSS. The incidence of acute post–traumatic bacterial endophthalmitis was seen in 2 eyes of the control group and none in the treatment group (p>0.05). Although the incidence of bacterial endophthalmitis was not significantly different, but the overall findings of this study suggest that, at the dosage used, combined intraocular injection of gentamicin and clindamycin may be an effective modality in the prevention of acute–traumatic bacterial endophthalmitis without any significant side effects.
