صفحات 1-8مقدمهکاردیومیوپاتی از جمله عوارض ناشی از دیابت است. به نظر می رسد هایپرگلیسمی طولانی مدت موجب افزایش بیان ژن گیرنده RAGE و در نتیجه فعالیت آبشار سیگنالینگی مسیرهای پاتوژنز در قلب رت های دیابتی نوع دو می شود، بنابراین فعالیت های بدنی ممکن است بتواند از طریق اثرگذاری بر هایپرگلایسمی در کاهش کاردیومیوپاتی موثر باشد. لذا هدف از پژوهش حاضر بررسی تاثیر 12 هفته تمرین مقاومتی بر بیان ژن RAGE VCAM، ICAM، در قلب رت های دیابتی شده با STZ بود.روش ها16 سر رت نر نژاد ویستار با میانگین وزن20 ± 200 گرم به صورت تصادفی انتخاب و در دو گروه دیابتی مقاومتی (تعداد 8)، دیابتی کنترل (تعداد 8) تقسیم شدند. گروه تمرین مقاومتی، 12 هفته تمرین مقاومتی را اجرا کردند و 48 ساعت پس از پایان آخرین جلسه ی تمرینی، رت ها بی هوش و قربانی شدند. سپس قلب آن ها خارج و با استفاده از روش Real time – PCR میزان بیان ژن RAGE VCAM، ICAM، طن چپ قلب اندازه گیری شد.یافته هانتایج نشان داد بیان ژن RAGE در بطن چپ قلب گروه تمرین مقاومتی نسبت به گروه کنترل کاهش معنی داری یافت، بیان ژن ICAM ،VCAM در گروه تمرین مقاومتی کمتر از گروه کنترل بود که این کاهش از نظر آماری معنی دار نبود.نتیجه گیریبا توجه به کاهش معنی دار در بیان ژن RAGE و الگوی کاهشی اما غیر معنی دار در ژن ICAM VCAM،در بطن چپ رت های دیابتی که تمرین مقاومتی نموده اند، به نظر می رسد تمرین مقاومتی در شرایط دیابت یک روش تاثیرگذار بر کاهش فعالیت آبشار سیگنالینگی مسیرهای پاتوژنز در قلب رت های دیابتی نوع دو می باشد.کلیدواژگان: تمرین مقاومتی، RAGE VCAM، ICAM

صفحات 9-16مقدمهدیابت نوع دو یک اختلال متابولیکی است که با افزایش قند خون می تواند به اعصاب آسیب بزند. در این میان اندام های بسیاری تحت تاثیر قرار می گیرند، به ویژه پا که آسیب منجر به درد و در نهایت از دست دادن حس لامسه در این اندام می گردد که این عوامل، شرایط را برای ابتلا به زخم پای دیابتی مساعد می سازند. پلی مورفیسم (Thr399Ile) از ژن (TLR4) Toll Like Receptor4 سبب نقص در عملکرد پروتئین TLR4 که یک پروتئین مهم تنظیم کننده ی التهاب زخم است می گردد. این مطالعات با هدف بررسی پارامترهای تاثیر گذار بر عدم تعادل برنامه ی ضد التهاب و در نتیجه مزمن شدن التهاب و عدم بهبود زخم بنا گردیده است و در صورت تایید همبستگی پلی مورفیسم (Thr399Ile) با زخم پای دیابتی می توان از آن به عنوان مارکر زیستی پیش آگاهی دهنده استفاده کرد.روش هااین مطالعه از نوع مورد - شاهد است. پلی مورفیسم (Thr399Ile) از ژن TLR4 در 100 بیمار دیابتی دارای زخم در گروه مورد و 120 بیمار دیابتی فاقد زخم در گروه شاهد با استفاده از تکنیک Tetra ARMS PCR تعیین ژنوتیپ گردید.یافته هاآزمون Chi-square بین دو گروه شاهد و مورد، از نظر آماری یک اختلاف معنی دار را در فراوانی ژنوتیپ های (CC)، (TT) و(CT) متعلق به پلی مورفیسم (Thr399Ile) نشان داد (P=0.021).نتیجه گیرییک ارتباط معنی دار بین پلی مورفیسم (Thr399Ile) از ژن (TLR4) و خطر ابتلا به زخم پای دیابتی در بین افراد مبتلا به دیابت نوع دو وجود دارد.کلیدواژگان: TLR4، پلی مورفیسم، زخم پای دیابتی

صفحات 17-38مقدمهتشخیص به موقع بیماری دیابت به طور چشم گیری صدمات و آسیب های ناشی از این بیماری را در جامعه کاهش می دهد. دیابت بیماری است که علاوه بر پیشگیری، نیاز به مراقبت های فراوانی از قبیل پیش بینی صحیح میزان نوسانات سطح قند خون دارد. از مهم ترین عوارض این بیماری می توان به بیهوشی، کما و حتی مرگ اشاره کرد. امروزه در این بیماران، تعیین صحیح دز انسولین براساس تجربه و دانش پزشکان در کنار تعامل بیماران با آن ها مشخص می شود، هر چند که وجود خطاهای انسانی اجتناب ناپذیر است.روش هادر این تحقیق 124 بیمار و 188 فرد سالم مشکوک به بیماری براساس 21 ویژگی (7 ویژگی به منظور تشخیص، 14 ویژگی به منظور پیش بینی دز انسولین) مورد مطالعه و بررسی قرار گرفتند، سپس سیستمی ارائه شد که ابتدا شناسایی یا تشخیص بیماری را انجام دهد، بعد برای افراد بیمار، مهم ترین داروی این بیماران، یعنی دز انسولین مشخص کند. سیستم پیشنهادی دارای دو مرحله (تشخیص بیماری و پیش بینی) و چندین زیر سیستم می باشد. در مرحله ی تشخیص بیماری، زیر سیستم هایی از قبیل سیستم فازی (Fuzzy) به منظور برآورد صحیح پیشرفت بیماری در بیماران، درخت تصمیم گیری (D-T) به منظور تهیه قوانین در سیستم فازی (فرآیند نگاشت فضای ویژگی (افراد) به خروجی (نتیجه ی تشخیص)) استفاده شده است. همچنین در مرحله ی پیش بینی دز انسولین از الگوریتم های کاوشی (BPSO) به منظور انتخاب بهترین ویژگی ها، الگوریتم های طبقه بندی (SVM) به منظور طبقه بندی ویژگی های موثر از غیر موثر و سیستم های انطباقی مصنوعی فازی – عصبی (ANFIS) برای پیش بینی نهایی داروی بیماران استفاده شده است.یافته هاسیستم پیشنهادی براساس بهترین ویژگی ها در بانک داده تهیه شده در قالب ترکیب و تعامل موفق شد به دقت 1/95% دست یابد، که البته در هنگام مقایسه با سایر روش های معمول به سرعت قابل توجه و عملکرد مناسب آن و البته دقت بالای آن پی می بریم.نتیجه گیرینتایج به دست آمده به میزان قابل توجهی نسبت به تحقیقات قبلی بهبود یافت. همچنین در مقایسه با نتایج پزشکان، نشان دهنده ی عملکرد خوب در صحت پیش بینی سری زمانی غلظت قند خون است، چرا که سیستم پیشنهادی موفق شد، سطح قند خون تا 48 ساعت آینده را پیش بینی نماید.کلیدواژگان: دیابت، تشخیص، پیش بینی، سیستم های فازی، الگوریتم طبقه بندی، سیستم فازی عصبی انطباقی

صفحات 39-48مقدمهخودکارآمدی با بهبود رفتارهای خودمراقبتی موجب ارتقاء کیفیت زندگی بیماران می گردد. مطالعه ی حاضر با هدف تعیین عوامل مرتبط با خودکارآمدی سازگاری درک شده در بیماران مبتلا به دیابت نوع دو اجرا گردید.روش هااین مطالعه ی مقطعی، از نوع توصیفی – تحلیلی بود که به مدت 9 ماه در سال 1394 انجام شد. 536 نفراز بیماران مبتلا به دیابت نوع دو مراجعه کننده به مراکز بهداشتی- درمانی تابعه ی دانشگاه علوم پزشکی تهران به صورت تصادفی انتخاب و از طریق پرسش نامه روایی و پایای خودکارآمدی سازگاری درک شده، وضعیت خودکارآمدی سازگاری و عوامل مرتبط با آن، در آن ها مورد بررسی قرار گرفت. برای آنالیز داده ها از آزمون های آماری تی تست، من - ویتنی، آنالیز واریانس و نرم افزار آماری SPSS نسخه ی 23 استفاده شد.یافته هامیان سن (013/0P=)، تحصیلات (036/0P=)، درآمد (002/0P=) و میزان HbA1c (049/0P=) با خودکارآمدی سازگاری درک شده کل رابطه ی آماری معنادار وجود داشت. اما متغیرهای جنس، شغل، مدت زمان ابتلا، وضعیت تاهل و سابقه ی خانوادگی و سابقه ی بیماری مزمن با هیچ کدام از ابعاد خودکارآمدی سازگاری درک شده ارتباط آماری معنی داری نداشتند.نتیجه گیریمداخلات مبتنی بر نظریه های سازگاری جهت افزایش خودکارآمدی سازگاری درک شده بیماران دیابتی بیشتر بر روی بیمارانی که دارای شرایط اقتصادی- اجتماعی پایین، سن بالا و با کنترل نامطلوب دیابت پیشنهاد می گردد.کلیدواژگان: دیابت نوع دو، خودکارآمدی سازگاری درک شده، عامل پیشگویی کننده

صفحات 49-62مقدمهغلبه بر موانع مرتبط با مراقبت از دیابت به عنوان زیرساختی منطقی در راستای توانمندسازی بیماران مبتلا در مدیریت زندگی با دیابت و شناخت عوامل موثر بر این موانع به شمار می آید. این مطالعه با هدف تعیین تاثیر مداخله ی آموزشی مبتنی بر مدل بزنف بر موانع آگاهی، سبک زندگی، سازگاری و حمایتی در زندگی با دیابت در بیماران مبتلا به دیابت نوع دو و عوامل موثر بر آن طراحی و اجرا شد.روش هااین مطالعه یک مداخله ی نیمه تجربی دو گروهی بود که روی 168 نفر از بیماران مبتلا به دیابت نوع دو تحت پوشش مراکز بهداشتی درمانی دانشگاه علوم پزشکی شهید بهشتی طی ماه های مرداد و شهریور سال 94 انجام شد. قبل از اجرای مداخله، پرسش نامه موانع زندگی با دیابت و پرسش نامه براساس سازه های الگوی بزنف تکمیل گردید که بعد از انجام پیش آزمون با تخصیص تصادفی به دو گروه مداخله و مقایسه با روش بلوک های چهارتایی تصادفی شده انتخاب شدند و در دو گروه مداخله و کنترل تحت بررسی قرار گرفتند. بیماران مبتلا به دیابت گروه مداخله، تحت شش جلسه ی آموزشی 45 تا 60 دقیقه ای به عنوان مداخله آموزشی قرار گرفتند و آموزش جلسه ی پنجم را به همراه خانواده خود و کادر درمانی مربوطه دریافت کردند. طراحی برنامه ی آموزشی براساس سازه های مدل بزنف (آگاهی، نگرش، نرم های انتزاعی، عوامل قادر کننده و قصد رفتاری) و محتوی آموزشی براساس موانع زندگی با دیابت (آگاهی و باور، سبک زندگی، حمایتی و سازگاری) تدوین گردید. داده ها با استفاده از نرم افزار SPSS نسخه 20 و آزمون های آماری T مستقل و T زوجی تجزیه، ویلکاکسون، مک نمار و من یو ویتنی تحلیل شدند. سطح معناداری در مطالعه، کمتر از 5% در نظر گرفته شد.یافته هانتایج نشان داد، بین نمره ی دریافتی مربوط به مولفه های آگاهی، سبک زندگی، سازگاری و حمایتی موانع زندگی با دیابت و آگاهی، نگرش، نرم های انتزاعی، عوامل قادر کننده و قصد رفتاری متغیرهای اجزای مدل بزنف در گروه مورد قبل و بعد از مداخله آموزشی اختلاف معناداری وجود داشت (05/0P<).نتیجه گیریآموزش بیماران دیابتی نوع دو بر اساس الگوی بزنف به عنوان الگوی آموزشی موثر می تواند به کاهش مولفه های آگاهی، سبک زندگی، حمایتی و سازگاری موانع زندگی با دیابت منجر شود.کلیدواژگان: دیابت نوع دو، موانع زندگی با دیابت، آگاهی و باورها، سبک زندگی، سازگاری و حمایتی، مدل بزنف

صفحات 63-76مقدمههدف مطالعه ی حاضر بررسی تاثیر سه برنامه ی تمرینات هوازی- مقاومتی با شدت های مختلف بر کنترل متابولیک و سطوح ویسفاتین در مردان مبتلا به دیابت نوع دو بود.روش ها43 بیمار مبتلا به دیابت نوع دو به صورت تصادفی در 4 گروه (سه گروه تمرینات ترکیبی 1، 2 ،3 و گروه کنترل) تقسیم شدند. گروه های تمرینی به مدت 12 هفته، 3 جلسه در هفته تمرینات ترکیبی هوازی- مقاومتی را با شدت های مشخص (گروه 1: مقاومتی60-50% یک تکرار بیشینه- هوازی 80-70 % ضربان قلب بیشینه، گروه 2: مقاومتی70-60% یک تکرار بیشینه- هوازی70-60% ضربان قلب بیشینه و گروه 3: مقاومتی80-70% یک تکرار بیشینه- هوازی 60-50% ضربان قلب بیشینه) اجرا کردند. فرآیند خونگیری به منظور تعیین سطوح ویسفاتین، انسولین، گلوکز ناشتا و هموگلوبین گلیکوزیله قبل و پس از 12 هفته، اجرا شد.یافته هاپس از 12 هفته تمرین سطوح ویسفاتین و مقادیر هموگلوبین گلیکوزیله در هرسه گروه تمرینی و گلوکز ناشتا در گروه 2 تمرینات ترکیبی نسبت به گروه کنترل به طور معنی داری کاهش یافت (05/0>P). در هر سه گروه تمرینی حداکثر اکسیژن مصرفی به طور معناداری افزایش یافت (05/0>P). تفاوت معنی داری در وزن، نمایه ی توده ی بدنی، درصد چربی، سطوح انسولین و مقاومت به انسولین بین گروه ها مشاهده نشد.نتیجه گیری12 هفته تمرینات ترکیبی با شدت های اجرا شده به واسطه ی کاهش سطوح ویسفاتین و هموگلوبین گلیکوزیله و بهبود توان هوازی می تواند در بیماران مبتلا به دیابت نوع دو مفید باشد. علاوه بر فواید فوق، تمرینات ترکیبی با شدت70-60 % یک تکرار بیشینه، 70-60 % ضربان قلب بیشینه به واسطه ی کاهش گلوکز ناشتا می تواند مزیت های مثبت بیشتری را نصیب این بیماران نماید.کلیدواژگان: تمرین ترکیبی، هوازی مقاومتی، ویسفاتین، دیابت نوع دو

Pages 1-8BackgroundCardiomyopathy is a side effect caused by diabetes. Prolonged hyperglycemia gives rise to an increase in the expression of the receiver gene RAGE subsequently triggering pathogenesis cardiac signaling pathways in the heart of rats with type II diabetes. The present paper aims to examine how a 12 week Resistance training on gene expressions RAGE, ICAM, VCAM in the heart of diabetic rats with STZ.Methods16 male Wistar rats with weight mean ranging from 200 ± 20 g were randomly assigned to two groups of Resistance diabetes (n = 8) and control diabetes (n = 8) and were kept under lab circumstances. A 12 week Resistance training was administered with the experimental group and 48 hours after the end of the last training session the rats were made unconscious and examined. Their hearts were, afterwards, cut out and the extent of gene expressions RAGE, ICAM, VCAM in the left ventricular heart was measured using Real time-PCR method.ResultsThe results indicated there was a significant difference between left ventricular heart of the Resistance diabetes and that of control diabetes in terms of gene expression RAGE, yet no significant difference was detected between the two groups in terms of gene expressions ICAM, VCAM.ConclusionAccording to the results, in seems that Resistance trainings effectively reduce gene expressions RAGE and reduction pattern but non-significant in the Gene ICAM, VCAM in left ventricular heart of diabetic rats and therefore can be considered an effective way in reducing pathogenesis cardiac signaling pathways in the heart of rats with type II diabetes.Keywords: Resistance training, RAGE, ICAM, VCAM

Pages 9-16BackgroundType 2 diabetes is a metabolic disorder characterized by high blood sugar levels that can damage nerves. Many organs are affected ¡especially the foot that leading to loss of sensation. These factors make favorable conditions for the development of diabetic foot ulcers. Polymorphisms (Thr399Ile) of Toll Like Receptor4 (TLR4) gene due to malfunction of TLR4 protein which plays an important role in immunity. The purpose of this study was to determine the parameters which are affecting the imbalance resulting in chronic inflammation and wound healing. By showing the relationship between single nucleotide polymorphism (Thr399Ile) of TLR4 gene with diabetic foot ulcer we can identify a biomarker for prediction of diabetic foot ulcer.MethodsThis is a case-control study. Single nucleotide polymorphisms of TLR4 gene were genotyped by hit Tetra ARMS PCR technique. In this study, 100 and 120 diabetic patients with and without foot ulcer were selected as the cases and controls, respectively.ResultsThe Chi-square test revealed significant difference in frequency of TT, CC and TC alleles of (Thr399Ile) between case and control groups (P=0.021).ConclusionAccording to this study, there is a relationship between single nucleotide polymorphisms (Thr399Ile) of TLR4 gene with diabetic foot ulcer in type 2 diabetes patientsKeywords: rbp4, Polymorphism, Diabetic foot ulcer

Pages 17-38BackgroundOn time diabetes diagnosis dramatically reduces the many injuries and damage in the community. Diabetes is a disease that requires a lot of care in addition to prevention, such as prediction the correct level of blood sugar fluctuations. The most important complications of such disease are anesthesia, coma and even death at final. Today, in these patients, determining the correct dose of insulin is based on the experience and knowledge of physicians along with the interaction of patients with them, although human error is inevitable.MethodsThis study includes 124 patients and 188 healthy suspects were examined based on 21 features which hold by 7 features for diagnosis and 14 features for predicting insulin dose. The proposed system was presented to identify or diagnose the disease at first, and finally the correct doses of insulin for patients have been determine. The proposed system has two stages (which include diagnosis and prediction) and several subsystems. In the diagnosis phase, some sub systems such as the Fuzzy system for the purpose of accurately estimating the disease progression in patients and the decision tree (DT) for the preparation of rules in the fuzzy system (the process of mapping the attribute space (individuals) to the output (the diagnostic result)) have used. Also, in the prediction phase of insulin dose, the BPSO algorithms are used to select the best features. Classification algorithms (SVMs) are used to categorize effective to non-effective and adaptive artificial neuropsychological (ANFIS) systems for ultimate patient prediction have used.ResultsThe proposed system, based on the best features in the provided data base in the form of the combination and interaction, succeeded to achieve a 95.1% precision, of course due to comparing by other commonly used methods and its performance the proposed method have high precision.ConclusionThe results were significantly improved compared to previous studies. Also, in comparison with the results of physicians, it is indicative of good performance in predicting the accuracy of the time series of glucose concentration because the proposed system succeeded in predicting blood sugar levels for up to 48 hours.Keywords: Diabetes, Diagnosis, Prediction, Fuzzy Systems, Classification Algorithm, Adaptive Neuro Fuzzy Inference System

Pages 39-48BackgroundImproving self-care behaviors happened by self-efficacy which result in better quality of life. Current study aimed at assessing the perceived coping self-efficacy predictors among patients with type 2 diabetes referees to health centers affiliated to Tehran University of Medical Sciences.MethodsIn this cross sectional study which was descriptive and analytical one, perceived coping self-efficacy and its effective factors assessed among 536 type 2 diabetic patients referee to Tehran University of medical Sciences using random sampling based on 11-likert scale questionnaire. This questionnaire included 11 items in demographic variables and 24 items in perceived coping self-efficacy. Collected data analyzed by SPSS version 23 using MANCOVA.ResultsStudy findings revealed that age (P=0.013), level of education (PConclusionPlanning theory based intervention programs to improve coping self-efficacy among diabetic patients in low socioeconomic status population and poor diabetes control is recommended.Keywords: Type 2 diabetes_Perceived Coping Self-efficacy_Predictor factor

Pages 49-62BackgroundOvercoming barriers associated with diabetes is considered a logical infrastructure to empower diabetic patients in management of living with diabetes and identify factors which affect these barriers. This study aimed to determine the effect of an educational intervention based on the BASNEF model on barriers to awareness, lifestyle, adaptation, and support in living with diabetes in patients with type 2 diabetes and it's related factors.MethodsThis was a quasi-experimental study which. Performed among 168 patients with type 2 diabetes referred to health centers of Beheshti University of Medical Sciences from August to September 2014. Before the educational intervention, questionnaires of life barriers and the BASNEF structures were completed. After pre-test, the patients were randomly allocated to the case and control groups based on randomizing four block design. Only in the intervention group patients received six education sessions of 45 to 60 minutes that fifth education session with their families, and related staff. The educational program was designed based on the BASNEF constructs (Knowledge, attitude, subjective norm, enabling factors and behavioral intention), and educational content was developed based on the barriers to living with diabetes (awareness, lifestyle, adaptation, and protection). Data analyzed Spss stafware version 20 utelized to data analysis using statistical tests such as independent sample t-test, paired t test, Wilcoxon, McNemar test, and Mann-Whitney UResultsOur findings showed that there was a significant difference between a score received on components of barriers to of living with diabetes (awareness, lifestyle, adaptation, and protection) and variable of the BASNEF model (knowledge, attitude, subjective norm, enabling factors and behavioral intention) before and after the educational intervention the significant level was set less than 0.05 (PConclusionEducation in type 2 diabetic patients based on BASNEF, as a model for effective education, can lead to dimensions barriers reduction in (awareness, lifestyle, adaptation, and protection).Keywords: Type 2 diabetes_Barriers to living with diabetes (knowledge_belief_lifestyle_adaptation_supporting)_BASNEF Model

Pages 63-76BackgroundThe purpose of this study was to compare effects of three combined aerobic-resistance exercise training protocols with different intensities on metabolic control and Visfatin levels in men with type 2 diabetes.Methods43 male patients with type 2 diabetes were randomly divided into 4 groups (three combined aerobic-resistance exercise training groups 1, 2, 3 and control). Training groups performed combined aerobic-resistance exercises for 12weeks, 3 times a week with given intensities )group 1:resistance 50-60% one repetition maximum- aerobic 70-80%maximum heart rate, group 2:resistance 60-70% one repetition maximum- aerobic 60-70% maximum heart rate and group 3:resistance 70-80% one repetition maximum- aerobic 50-60% maximum heart rate). Blood sampling to determine the levels of Visfatin, insulin, HbA1c, fasting glucose before and after 12weeks, were conducted.ResultsVisfatin levels and HbA1c in all training groups and fasting glucose in 2 combined training groups significantly decreased compared with control group (PConclusion12 weeks of combined aerobic-resistance exercise training protocols with were executed intensities through decreasing Visfatin and HbA1c levels and improving aerobic capacity could be helpful in patient with type 2diabetes. In addition to the above benefits, combined exercise training protocols of with intensity of 60-70% maximum heart rateintensity 60-70% repetition maximum due to reducing fasting blood glucose could give greater benefits to patients.Keywords: Combined training_Aerobic-resistance_Visfatin_Type 2 diabetes
