هر مورد سختزایی، مشکلی بالینی محسوب میشود که در صورت به کارگیری روشی صحیح میتوان آن را رفع نمود. تشخیص و درمان سختزایی نیازمند درک صحیح روند زایمان طبیعی و حساسیت نسبت به رفاه مادر و فرزند است. سختی تولد زمانی رخ میدهد که یا زورهای زایمانی ناکافی هستند، یا مجرای زایمانی اندازه و شکل مناسبی ندارد، یا قطر جنین نمیتواند از طریق مجرای زایمانی عبور کند، چون یا خیلی بزرگ است یا نحوه قرارگیری آن از عبور طبیعی آن جلوگیری به عمل میآورد. رخداد سختزایی در تلیسهها 3 تا 10 برابر رخداد آن در گاوهای چند بار زایش)چه گاوهای گوشتی و چه شیری(است. عوامل خطر سختزایی شامل عدم تناسب میان جنین و مادر، قرارگیری غیر طبیعی جنین، استنوز فرج، واژن و یا سرویکس، پیچ خوردگی رحم هستند. پیشگیری از سختزایی به ویژه در تلیسههای زایش اول، عنصر مهمی در برنامه تولید مثلی موفق در همه مزارع پرورش گاو شیری یا گوشتی محسوب میشود و عوامل زیادی میتوانند در رخداد سختزایی نقش داشته باشند. حتی در زمانهایی که برنامههایی جهت محدود نمودن خطر سختزایی و رساندن آن به سطح قابل قبول وجود دارند (کمتر از 15٪ برای تلیسه ها و کمتر از 5٪ برای گاوها)، همچنان مواردی از سختزایی در گلههای گاو گوشتی و شیری رخ میدهند. یکی از مهمترین نیازمندیها برای مدیریت ایدهآل سختزایی این است که دامدار بداند چه زمانی مداخله کند یا تقاضای کمک داشته باشد.

سختزایی با منشاء مادری شامل نقص در مجرای زایمان و نقص در زورهای زایمانی است. نقص مجرای زایمان میتواند به علت تنگی لگن، نقص در اتساع سرویکس، شل شدن ناقص بخش خلفی واژن و فرج و سایر اختلالات فیزیکی که موجب انسداد میشوند مانند بقایای مجاری پارامزونفریک باشد. زورهای زایمانی در اثر ترکیب انقباضات میومتر و زورهای ناشی از انقباض عضلات شکمی با گلوت بسته رخ میدهد. از آنجایی که عضلات شکمی تا زمان رانده شدن جنین و پردههای جنینی به داخل مجرای لگنی توسط انقباضات میومتر و تحریک گیرندههای اعصاب حسی لگن، نقشی ایفا نمیکنند، منطقی است که ابتدا نقایص زورهای خارج کننده میومتر در نظر گرفته شوند. این حالت میتواند به شکل خود به خودی یا به شکل وابسته دیده شود که به ترتیب اینرسی اولیه و ثانویه رحم نام میگیرند.

هدف از روندهای دستکاری در گونههای تک قلوزا باید همیشه این باشد که قبل از تلاش برای زایمان، اطمینان یابیم که جنین از نظر نحوه قرارگیری، طبیعی است. اصلاح نقایص در وضعیت ، موقعیت و نحوه قرارگیری اندامها و سر تنها از طریق دستکاری داخل رحمی جنین حاصل میشود. هل دادن جنین به داخل رحم برای تغییر دادن نحوه قرارگیری آن الزامی است. پس از تشخیص علت سختزایی و تصمیم در مورد برنامه عملکردی، شخص ماما باید در نظر بگیرد که آیا تجهیزات در دسترس کافی هستند و آیا دامپزشکان یا سایر کمکها در دسترس هستند. در موارد شدید سخت زایی، دامپزشک همیشه باید به دنبال کمک دامپزشک متخصص باشد و در نظر بگیرد که آیا انتقال دام به مکانی با تجهیزات بیمارستانی به این شرط که حیوان در شرایط با ثباتی برای جا به جایی قرار داشته باشد، کمک کننده است یا خیر. دستکاری مامایی از راه واژن باید تا حد امکان تمیز انجام گیرد. غیر ممکن است که بتوان آن را در شرایط کاملا آسپتیک انجام داد چون به شکل اجتناب ناپذیری کمی آلودگی وجود خواهد داشت.

جراحی سزارین یا لاپاروهیستروتومی یکی از قدیمیترین روشهای جراحی در طب دامپزشکی است. در حیوانات مزرعه، بهخصوص دام های گوشتی، این روش نسبتا رایج است و هنگامیکه زایمان معمول امکانپذیر نباشد، انجام میشود. سختزایی مهمترین دلیل انجام جراحی سزارین است. حفظ سلامت و بقای گاو و گوساله و همچنین حفظ قدرت باروری از مهمترین اهداف سزارین در گاو هستند. هدف این مقاله، اشاره ای به تاریخچه ی این جراحی و نیز تشریح دلایل انجام جراحی سزارین، تصمیمگیری برای جراحی، نحوه ی آمادهسازی بیمار، رهیافتهای مختلف جراحی سزارین و انتخاب آن و همچنین مراقبتهای پس از عمل و عوارض احتمالی پس از این جراحی در گاو است.

آسیب های کانال زایمانی منجر به خسارات مالی قابل ملاحظه ای در گله های گاو شیری میشوند و غالبا به دلیل آسیب مامایی حین زایمان رخ می دهند. پارگی های کانال زایمانی به درجات مختلف رخ می دهند که از یک تا سه بسته به شدت جراحت تقسیم بندی می شوند، پارگی درجه سوم میان دوراه معمولا به دلیل سخت زایی و فیستول های رکتوواژن معمولا به دلایل اصلاح اشتباه پارگی درجه سوم میان دوراه، آبسه ها، سخت زایی و سایر رخدادهای مامایی رخ می دهند. هر دو این عوارض منجر به تجمع مدفوع در کانال زایمانی و بدنبال آن کاهش نرخ آبستنی می شوند. هدف اصلی جراحی های کانال زایمانی و قسمت خارجی دستگاه تناسلی بازگرداندن توانایی آبستنی موفق و اصلاح آسیب های مرتبط با موارد سخت زایی است. در این نگاشته تلاش شده است که با تاکید بر اهمیت آسیب های کانال زایمانی در گاو، به علل و مدیریت و درمان این اختلالات بپردازیم.

دوره حوالی تولد پرخطرترین دوره در زندگی همه حیوانات است. مرگ و میر دوره حوالی تولد گوساله در 48 ساعت اول زندگی بین 3 تا 3/10 درصد برآورد شده است. بیش از 60 درصد از تولیدکنندگان گزارش کردهاند که بیشتر مرگ و میر گوسالههای آنها در بدو تولد اتفاق میافتد. علل اصلی مرگ و میر دوره حوالی تولد به ترتیب اهمیت، اسیدوز تنفسی و متابولیک، ترومای حین زایمان، هیپوگلوبولینمی، عفونت و کمبودهای مادرزادی و امفالوفلبیت هستند. حدود 90 درصد گوسالههایی که در دوره حوالی تولد میمیرند زنده متولد می شوند و بسیاری از این تلفات قابل پیشگیری است. مدیریت گوسالههای تازه به دنیا آمده از طریق اجرای پروتکلهای ساده که راهبردهای مناسبی را درسطح گله و روشهای صحیحی که بصورت انفرادی ارایه میدهند به بهترین شکل قابل دستیابی هستند. این پروتکل ها مدیریت گاوها درحوالی زایش، در زمان زایش (پایش راحتی زایمان و تشخیص و مداخله در سختزایی) و مدیریت گوساله تازه به دنیا آمده را پوشش می دهند. بنابراین بسیار ضروری است که ارزیابی سریع و مراقبت از گوساله جهت اطمینان از سلامت و زندهمانی و جلوگیری از خسارات اقتصادی ناشی از مرگ در حوالی تولد صورت بگیرد.

Each case of dystocia is a clinical problem that may be solved if a correct procedure is followed. The diagnosis and treatment of dystocia requires a good understanding of normal parturition and sensitivity to the welfare of both dam and offspring. Difficult birth will occur when the expulsive forces are insufficient, when the birth canal is of inadequate size and shape, or when the presenting diameter of the fetus is unable to pass through the normal birth canal because it is too large or its disposition prevents it from doing so. The incidence of dystocia in heifers has been calculated to be anywhere from three to ten times that of multiparous cows, whether in beef or dairy cattle. Risk factors for dystocia are fetal-dam disparity, abnormal fetal position, vulval, vaginal, or cervical stenosis and uterine torsion. Preventing dystocia, particularly in first-calf heifers, is a very important component of a successful reproductive program on any dairy or beef operation and there are numerous factors have been involved in the incidence of dystocia. Even when plans are in place to limit dystocia risk to an acceptable level (<15% for heifers, <5% for cows), there are still cases of dystocia in beef and dairy herds. One of the most important fundamentals for optimum dystocia management is for the owner to know when to intervene or call for assistance.

Maternal dystocia includes defects of the birth canal and defects of the expulsive forces. Defects of the birth canal would be due to the pelvic constriction, failure of cervical dilation, incomplete relaxation of the caudal vagina and vulva and other physical abnormalities causing obstruction such as remnants of the paramesonephric ducts. The expulsive force of labour is due to a combination of myometrial contractions and straining induced by the contraction of the abdominal muscles with a closed glottis. Because the abdominal muscles do not come into play until the myometrium has forced the fetus and fetal membranes into the pelvic canal and stimulated the pelvic sensory nerve receptors, it is logical to consider first the expulsive deficiencies that may arise in the myometrium. These may occur spontaneously or dependently and are called, respectively, primary and secondary uterine inertia.

Correction of defects of presentation, position, and posture can be achieved only by intrauterine manipulation of the fetus. Retropulsion of the fetus into the uterus is essential for repositioning it. After diagnosing the cause of dystocia and deciding on a plan of action, the obstetrician should consider whether the available facilities are appropriate, and whether the equipment is adequate to carry out the treatment successfully. In severe forms of dystocia, the veterinarian should always seek the assistance of a professional colleague and consider whether it might be appropriate to transport the animal to a place with hospital facilities, provided that the animal is in a fit state to travel. Obstetrical manipulation per vaginam should be performed as cleanly as possible. It is impossible to perform them aseptically because there will inevitably be some contamination.

Cesarean operation is one of the oldest surgical procedures in veterinary medicine. In farm animals practice, especially in meat producing animals, this technique is a relatively frequent and performed when normal parturition is not possible. Dystocia is the most important reason for cesarean procedure and survival of the cow and the calf, and maintenance of fertility are the main aims of cesarean in cattle. In this article, the history of this technique has been briefly mentioned and the aim of this article is to describe the indications of cesarean, decision making for surgery, preparaton of the patient, different cesarian surgical approaches, as well as post-operative cares and complications in cattle.

Injuries of the birth canal in cows (perineal lacerations, fistula…) cause significant economic loss in dairy herds. These injuries usually are associated with obstetric trauma during labor. Third degree perineal lacerations and fistula formation usually happen because of dystocia, improper repairs, abscesses and forced extractions and lead to fecal contamination of the birth canal and reduced fertility. The main reason for the birth canal and external part of the reproductive tract surgery is to restore fertility of the animal and repair the injuries associated with dystocia. In this review the causes, management, and treatment of these disorders by emphasizing on the importance of birth canal injuries in cows were discussed.

The perinatal period is the most perilous in the life of all animals. Perinatal calf mortality in the first 48 hours of life has been estimated to be between 3 and 10.3%. More than 60% of producers have reported that most of their calf mortality occurs at birth. The main causes of perinatal morbidity and mortality are , in descending order of importance , combined respiratory and metabolic acidosis , parturient trauma, hypoglobulinemia, congenital infections and deficiencies, and omphalophlebitis. Some 90% of calves that die in the perinatal period were alive at the start of calving, and so much of this loss is preventable. Management of the newborn dairy calf is best achieved through implementation of simple protocols that document the correct strategies to be followed at the herd level and the correct procedures to be performed at the individual animal level. These protocols cover management of the prepartum cow, management of calving (monitoring of eutocia and detection and management of dystocia) , and newborn calf care. Therefore, it is imperative to provide immediate evaluation and care of newborn calves to ensure calf survival and health and prevent economic losses associated with perinatal mortality.
