Aim: The aim of this study was to evaluate and compare three available methods for mitochondrial isolation from a human cell line to predict the best method for each probable application.BackgroundOrganelle isolation is gaining importance in experimental laboratory settings. Mitochondrial dysfunction may affect tumorgenesis process. There are some evidences that transplantation of healthy, intact and active mitochondria into cells containing defective mitochondria may reduce the proliferation. Therefore, isolated mitochondria could be considered as an effective tool for assessment and management of mitochondrial related disorders.
Patients andMethodsMitochondrial isolation from the human liver cell line (HepG2) was performed using two commercially available kits, including Qproteome (Qiagen) and MITOISO2 (Sigma-Aldrich), as well as a manual method. Integrity of inner membrane of mitochondria was assessed by JC-1 staining. Activity of isolated mitochondria was evaluated by DCFH-DA staining, and total yield of isolated mitochondria determined by micro-Lowry method. Finally, relative quantification using Real-time PCR was conducted to compare the mtDNA copy number of mitochondria isolated by three different methods.ResultsCompared to other methods, manual kit resulted in higher yields of total amount of mitochondrial protein and mtDNA copy numbers. Isolated mitochondria by Qproteome kit, showed a higher activity. Finally, the integrity of inner-membrane of isolated mitochondria was significantly higher in Qproteome when compared with the other two methods.ConclusionDue to differences in quality, quantity and activity of isolated mitochondria using three techniques discussed here, the method in which best-suited to each research project should be selected according to the distinct features of isolated mitochondria. -
Background
Evidences suggest an association between the prevalence of thyroid peroxidase (anti-TPO) and anti thyroglobulin (anti-Tg) with recurrent abortions and infertility. Iodine deficiency was once endemic in Iran and little data is available about the prevalence of these antibodies in different groups of fertile or infertile individuals.
ObjectiveThis case control study was designed to compare the presence of anti-TPO and anti-Tg in four groups of women to reveal their role in the etiology of recurrent abortion and infertility.
Materials And MethodsFour groups of euthyroid women referring to Avicenna Infertility Clinic in Tehran were selected; 95 cases as fertile controls and 70, 78 and 137 cases with male and female factor infertility and recurrent abortion respectively. TSH, anti-TPO and anti-Tg were evaluated by chemiluminescent immunoassay.
ResultsThe prevalence of the above mentioned autoantibodies in euthyroid controls was about 25% and the percentage of people with an anti-Tg >500 was two times bigger in the abortion group compared to the control group (p<0.05) and the proportion of people with an anti-Tg >500 in younger cases in the abortion group was significantly higher than the rest of the cases (p<0.05). Anti-TPO distribution had no significant differences.
ConclusionThere were no statistically significant differences among four groups. It seems that more comprehensive studies are needed to reach a common conclusion about thyroid autoantibodies in women with recurrent abortions in different groups and different parts of Iran. In addition, dividing the recurrent abortion and infertility groups on the basis of their etiologies could be effective.

زمینه و هدفایزوآنزیم GSTM و GSTP آنزیم گلوتاتیون S-ترانسفراز در سطح اسپرم انسان وجود دارند که در محافظت علیه استرس اکسیداتیو نقش دارد. هدف این مطالعه بررسی پلی مورفیسم ژن GSTP1 و GSTM1 و ارتباط آن با فعالیت آنزیم گلوتاتیون S-ترانسفراز و پارامترهای اسپرمی می باشد.روش بررسیاین مطالعه روی 95 مرد مبتلا به الیگوآستنو تراتواسپرمی و 26 مرد نورمواسپرمی انجام گردید. آنالیز مایع منی بر اساس روش استاندارد WHO برای هر دو گروه انجام گرفت.پس از استخراج DNA ژنومی خون با استفاده از روش Salting out، پلی مورفیسم ژن GSTM1 با استفاده از Multiplex-PCR و پلی مورفیسم ژن GSTP1 به کمک روش PCR-RFLP بررسی گردید. سپس فعالیت آنزیم گلوتاتیون S-ترانسفراز در سطح اسپرم اندازه گیری شد.یافته هافراوانی ژنوتیپ نول GSTM1 در گروه مورد و کنترل به ترتیب 1/52% و 8/53% بود. اختلاف فعالیت آنزیم گلوتاتیون S-ترانسفراز بین دو ژنوتیپ مثبت و نول GSTM1 در دو گروه معنی دار نبود. نتایج پلی مورفیسم ژن GSTP1 نشان داد همه نمونه های مورد بررسی دارای ژنوتیپ Ile/Ile در کدون 105 می باشند. فراوانی ژنوتیپ هموزیگوت Ala/Ala) 114 (، هتروزیگوت (Ala/Val 114)، هموزیگوت (Val/Val114) ژن GSTP1 در گروه الیگوآستنوتراتواسپرمی به ترتیب 1/81% و 9/17% و 1/1% و در گروه نورمو اسپرمی به ترتیب 5/88% و 5/11% و صفر بود.
نتیجه گیریفقدان فعالیت آنزیمی مربوط به ژنوتیپ نول GSTM1 تاثیری بر پارامترهای اسپرمی و میزان فعالیت آنزیمی ندارد که ممکن است به دلیل فعالیت جبرانی سایر ایزوزیم های موجود در سطح اسپرم از جمله GSTP1 باشد که خود می تواند مربوط به افزایش بیان ژن GSTP1 در مواجهه با استرس اکسیداتیو باشد.

ناباروری یکی از مشکلات فردی و اجتماعی است که صرف هزینه های گزافی را جهت درمان طلب کرده و می تواند باعث از هم گسیختگی ثبات خانواده گردد. پیشرفت روش های باروری جایگزین خصوصا در طی دهه اخیر، ایجاد بارداری را در زنان نابارور امکان پذیر ساخته است. گرچه این روش ها در ابتدا برای درمان زنان در سنین پایین و قبل از یائسگی بکار می رفت، اما امروزه استفاده روزافزون از روش هایی مانند اهدای گامت، بارداری را در زنان حتی بعد از سن یائسگی امکانپذیر ساخته است. روش اهدای گامت مسائل بحث انگیز زیادی را مطرح کرده است. یکی از سوالات مهم مطرح، ایجاد یا عدم ایجاد محدودیت سنی برای زنان جهت استفاده از این روش می باشد. با توجه به اهمیت موضوع در این مطالعه به بررسی تاثیر افزایش سن در باروری و روش های جایگزین باروری می پردازیم.
پژوهش حاضر، از نوع مطالعات توصیفی تحلیلی است که با توجه به جوانب مختلف باروری در سنین بالا به مقایسه سیاستها و نگرش های مختلف نیز پرداخته است.
در ابتدا تغییرات توانایی تولیدمثل زنان در طول زمان و اثرات افزایش سن بر روی نتایج حاصل از روش های کمک باروری مورد توجه قرار گرفته و سپس تاثیرات باروری و داشتن فرزند در سنین بالا در ابعاد پزشکی، اجتماعی و روانی بررسی شده و در نهایت با توجه به نتایج و مطالعات انجام گرفته و شرایط جامعه ایران دسترسی زنان مسن به فناوری های نوین باروری مورد ارزیابی قرار گرفته است.
نتایج این مطالعه بیانگر تفاوت سیاستگذاری ها در ارتباط با افزایش سن و نتایج حاصل از روش های باروری شخص ثالث در جوامع مختلف است. این سیاستگذاری ها براساس سطح دسترسی به رفاه اجتماعی در جامعه و سیستم های حمایتی، امید زندگی و شرایط جسمی، اقتصادی و اجتماعی فرد متغیر است، بنظر می رسد در ایران با توجه به شرایط فعلی حداکثر سن استفاده کنندگان از روش های باروری جایگزین تا همان سن باروری طبیعی یعنی 50 سال در نظر گرفته شود و از ورود زنان بالای 50 سال به پروسه درمان ناباروری جلوگیری به عمل آید. سن فرد می تواند با توجه به شرایط جسمی، اقتصادی و اجتماعی وی و پس از انجام مشاوره در صورت داشتن شرایط لازم افزایش یا کاهش یابد. از طرفی لازم است به تمامی زنان مسن استفاده کننده از این روش ها آموزش لازم در رابطه با فرزندآوری در سنین بالا و پیامدهای احتمالی آن برای خود و فرزندان داده شود.کلید واژگان: باروری, باروری جایگزین, سن}Objective(s)Infertility as one of individual and social dilemmas sometimes result in significant expenditures, it also might weaken the family stability. Recent developments in assisted reproductive technologies have enabled the infertile couples to become pregnant. Although these methods were initially used only for women in premenopausal age, gamete donation has brought about the possibility of treating women beyond the age of menopause. In this article an attempt will be made to consider effects of ageing on natural and artificial reproduction. In this descriptive-analytic study we compare different views and policies relevant to various aspects of pregnancy in the aged. First we describe the changes in reproductive ability during the lifespan and the effect of ageing on the results of assisted reproductive technologies. This is discussed from medical, social and psychological points of view. Finally by regarding the different studies and the current status of Iranian society, we evaluate the access of middle-aged women to assisted reproductive technologies.
ConclusionOur study displays the differences in policies defining an age limit and the results of donation in different societies. These policies differ based on the level of access to social welfare, supportive services, life expectancy and the medical, financial and social status of each individual. Regarding Iran’s current social standing, it is emphasized to limit the reproductive treatments to the age of fifty. There is a possibility that some individuals might be included in a younger or older age depending on their medical, financial and social situations and after a thorough and timely counseling. It is also essential to educate all advanced-aged women on the issues related to child-bearing in middle age and the likely consequences.

با شکل گیری و تولد اولین فرزند ناشی از به کارگیری تکنیک لقاح خارج رحمی در سال 1978، درمان ناباروری زوجهای بدون فرزند تا حدودی محقق گردید. توسعه روش های لقاح خارج رحمی، امکان لقاح و تشکیل جنین و باروری زوجینی را فراهم نمود که حتی در شرایط اولیه ظهور این تکنولوژی (IVF)، باروری آنها میسر نبود. در عین حال، در موارد قابل توجهی از ناباروری ها، دستیابی به گامت (تخمک یا اسپرم) سالم زوج، جهت تشکیل جنین امکان پذیر نیست. در این شرایط استفاده از گامت یا جنین جایگزین به عنوان یکی از راه های موفق درمان ناباروری مطرح شده است. استفاده از گامت جایگزین نیازمند اطمینان از سلامت اهدا کننده و دریافت کننده و تطابق خصوصیات و ویژگی های ظاهری، خونی، فرهنگی اجتماعی و رعایت تناسب دینی مذهبی است و انجام آن مستلزم در نظر گرفتن کلیه جوانب و رعایت دستورالعملهای لازم در این زمینه است. از آنجا که با استفاده از این روش ها امکان وقوع حاملگی در زنان بالای چهل سال نیز فراهم می شود، لازم است علاوه بر ارزیابی معمول سلامت، بررسی های بیشتری نیز از وضعیت سلامت جسمی، ریسک فاکتورهای حاملگی و علی الخصوص آمادگی روحی نیز صورت پذیرد. اولین اقدام در درمان ناباروری با استفاده ازگامت جایگزین، آگاهی زوجین نابارور از چگونگی دریافت گامت، تشکیل جنین و شکل گیری عمل لقاح خارج رحمی است، تا نگرانی های حاصل از انجام این عمل و اطمینان از سلامتی آن، ریسکهای مربوط به آن، درصد موفقیت و هزینه های احتمالی مورد بررسی قرار گیرد. به دنبال آن، اطمینان اهدا کننده و دریافت کننده از نگرانی های شرعی و قانونی، محرمانه ماندن هویت دهنده از گیرنده و بالعکس و چگونگی ارزیابی سلامت گامت و جنین حاصل از موارد قابل توجهی است که باید در جلسات مشاوره ارائه گردد. بررسی سلامت اهدا کننده با گرفتن شرح حال و معاینه فیزیکی و آزمایشهای کلینیکی و مشاوره های عفونی، داخلی و ژنتیک تکمیل می شود و از صحت و سلامت روان نیز در مشاوره اطمینان حاصل می شود و هرگونه نگرانی و نقص در موارد فوق که منجر به انتقال از گامت به جنین باشد، موجب حذف اهدا کننده از پروسه اهدا خواهد شد.
خصوصیات گیرنده با دریافت کننده شامل مشخصات فنوتیپی زوجین نظیر قد، وزن، رنگ پوست، چشم، مو، بر اساس درخواست دریافت کننده تطبیق داده می شود. هرچند مطابقت گروه خونی از نظر پزشکی لازم نیست ولی در مواردی تطابق آن ضروری می شود. در بسیاری از موارد دریافت کنندگان علاقه مند به طرح موارد خاصی از مشخصات اهدا کننده نظیر اعتقادات، سطح فرهنگی اجتماعی، ضریب هوشی و تحصیلات هستند که در صورت امکان کلینیکهای اهدا باید همکاری های لازم را معمول دارند.کلید واژگان: گامت, جنین, لقاح خارج رحمی, مشاوره, باروری شخص ثالث}Objective(s)Development of Assisted Reproductive Technologies (ART) and the successful birth of the first child in 1978, new therapeutic options have become available for infertile couples. Development of IVF methods made conception and embryo formation possible for the infertile, something impossible by then. ART has also brought about the possibility of third party involvement in the process of reproduction. This may occur by providing an egg, a sperm, an embryo or a uterus by a third party.
ResultsTo use a donated gamete requires an evaluation and screening of the health of both donors and recipients in order to make any necessary adjustment regarding physical characteristics and specifications, blood group, social and cultural traits and religious requirements. All these ought to be in accordance with relevant rules and regulations. As ART methods enables women over 40 to conceive and bear child, meticulous evaluations of physical health, pregnancy risk factors and in particular emotional readiness of the couples is necessary. The very first step in the treatment of infertility using a substitute gamete is to inform the couple of the method of gamete and embryo supplement and the way the embryo will form in vitro. This may lessen their worries, make the procedure safer, reduce possible ethnic hazards, increase the success rate and finally bring the expenses involved to an efficient level. Other aspects that should be considered, since they may give the couple a sense of confidence are a religious and legal counseling, technique explanation, confidentiality of identities of recipients and donors None of the said aspects should be overlooked. Health evaluation could be undertaken by a history review, physical examination, infection tests, genetics and general medicine consultation.
ConclusionPhonotypical characteristics of the recipient such as height, weight, skin color; hair color will be matched with donor according upon recipient’s request. Although, in most cases, matching blood group is not medically compulsory but it might be required in some cases. In many cases, recipients are interested in raising questions regarding particularities of the donor like their beliefs, social and cultural status, intelligence quota education on which a donation clinic should be cooperate.

همزمان با ظهور فناوری لقاح خارج رحمی در سال 1978، امکان درمان ناباروری برخی از زوجهایی که به هر دلیلی دارای گامت بوده، ولی از داشتن فرزند محروم بودند فراهم شد. بررسی جنبههای مختلف به کارگیری روشهای باروری کمکی جایگزین در ایران، علیالخصوص از نظر شرعی و قانونی یکی از مقدمات اصلی ورود به استفاده از این فناوری است. مفاهیم باروری کمکی، لقاح خارج رحمی، انتقال مستقیم گامت اهدایی به داخل رحم و مقایسه آن با تشکیل جنین حاصل از گامت جایگزین پس از کشت و آمادهسازی آن در آزمایشگاه و سپس انتقال آن به داخل رحم از مفاهیم اساسی و اولیه جهت آشناسازی حوزههای علمیه و فقها و دانشمندان سایر علوم به ویژه حقوق، روانشناسی و جامعه شناسی است. استفاده از روشهای گامت جایگزین در باروری زوجین نابارور زمینه نگرش جدیدی را در فقه و قانون برای استفاده از این روشها در درمان ناباروری فراهم نموده است. استفاده از این روشها در ایران نیازمند بررسیهای اساسی و جدی مطابق با شرایط فرهنگی اجتماعی آن است. سلامت جسمی، روحی و روانی اهدا کننده و دریافت کننده یکی از مهمترین عوامل ارزیابی فرآیند اهدا است. همچنین علاوه بر ارزیابی سلامت جسمی فرد و اطمینان از عدم ابتلای به بیماریهای صعبالعلاج، جلوگیری از انتقال برخی از بیماریهای ژنتیک به جنین حاصل نیز حایز اهمیت است. اگرچه تطابق ویژگیهای خونی، ظاهری، فرهنگی، اجتماعی و حتی رعایت تناسب دینی نیز در این رابطه مطرح است. استفاده زوج نابارور از این تکنیکها در درمان ناباروری، مستلزم اطلاع دقیق از ابعاد متنوع آن است. هر گونه اطلاعی که بتواند در تصمیم زوج نسبت به استفاده از این تکنیکها تغییری حاصل نماید، بایستی قبل از هر گونه اقدام درمانی در مشاورههای متعدد عرضه گردد و پس از پذیرش آگاهانه و رضایت زوج، امکان استفاده از این تکنیکها محقق گردد. به کارگیری این تکنیکها در درمان ناباروری زوج بدون فراهم آمدن مقدمات اولیه ارزیابی سلامت اهدا کننده و دریافت کننده و عدم آگاهسازی دقیق زوج نابارور از ابعاد متنوع فقهی، حقوقی، روانی، اجتماعی و اخلاقی، میتواند زمینهساز آسیبهای اجتماعی جبران ناپذیر بعدی برای زوج و فرزند حاصل باشد و مراکز درمان ناباروری بایستی دقت کافی در این زمینه را مبذول نمایند.

Assessment of applying ART methods in Iran, in particular from jurisprudential point of view, is among the main prerequisites to make such a therapeutic approach to this technology. Concepts like assisted reproductive techniques, IVF, direct transfer of donated gamete into the uterus and its comparison with formation of embryo from donated gamete after its culture, in the laboratory and the consequent embryo transfer to the uterine cavity are among the fundamental and initial concepts for familiarization of religious schools and jurisprudents with the application of donated gametes in treatment of infertile couples, which together have provided a new view point in Shiite jurisprudence in confronting treatment of infertility. The donors' and the recipients' physical and mental health are among the most important factors in assessing the process of donation which will not only increase the level of confidence in preventing transmission of genetic and non genetic diseases to the embryo but also insures a high level of compatibility in blood born, physical, cultural, social and even religious characteristics. Preparing the socio-cultural basement for application of these techniques for treatment of infertile couples calls for a comprehensive introduction of such issues in the society and also in the scientific community. For an infertile couple to volunteer to use these techniques, they need to be well aware of their different dimensions.
ConclusionAny information that can affect the decision of the couple in applying these techniques must be discussed with them in comprehensive and even repeated consultation sessions and only after well educated and informed consent of the couple, application of these techniques may be allowed. It is the task of the infertility clinics to cover these issues with maximum caution.

مطالعات توصیفی مختلف مطرح نموده اند زوجهای نابارور اشکال مختلف استرس روانی – هیجانی را متحمل می شوند که در نهایت می تواند آنها را مستعد افسردگی نماید و همین امر ممکن است احتمال بارور شدن را بیشتر از پیش کاهش دهد. از سوی دیگر دیده شده نمرات شادمانی در میان افرادی که در روند درمان ناباروری شکست می خورند نسبت به افرادی که موفق می شوند به وضوح پایینتر است. تمامی اینها لزوم جنبه های مختلف مشاوره را در تمام مراحل درمانی مطرح می نماید.
در بررسی مقالات غالب مطالعات زنان را در دو گروه نابارور و کنترل قرار داده اند. گروه نابارور، زنانی با مشکلات ناباروری هستند که در انتظار ART به سر می بردند و گروه کنترل زنانی با توزیع سنی مشابه می باشند که حداقل یک فرزند داشته و نیز سابقه ای از ART نداشته اند. این دو گروه از طریق تکمیل پرسشنامه مورد ارزیابی و مقایسه قرار گرفتند.
گروه نابارور میزان بالاتری از احساسات هیجانی منفی را نشان دادند (69%-37%). روابط جنسی و زناشویی زوج در گروه نابارور بیشتر تحت تاثیر قرار گرفته بود.همچنین در زمان مشاوره، رضایتمندی ذهنی در گروه نابارور بخصوص زنان با سابقه درمان ناموفق قبلی اختلاف معناداری را با گروه کنترل نشان می داد که در زمینه خلق افسرده، حافظه و تمرکز، اضطراب و ترس و نیز جذابیت از نگاه دیگران بارزتر بود و اما شیوع اختلال افسردگی در گروه نابارور 25% به دست آمد در حالیکه در گروه کنترل 8/6% بود.
از مطالعات موجود اینطور برآورد می شود که زنان برخوردار از مشکلات باروری بخصوص آنهایی که جهت درمان خود تلاش می نمایند اما موفق نمی شوند، آشفتگی هیجانی پیدا می کنند که این امر ضرورت یک مشاوره حرفه ای را می طلبد که نه تنها بر احساسات و تجارب کنونی بلکه بر سوابق گذشته زوج در خصوص نحوه برخورد با این موضوع توجه می نماید و از آنجا که اثبات نشده واکنشهای روانی جدی در این افراد شایع باشد؛ لذا انجام مشاوره های روانی – اجتماعی مناسب می تواند نقش بسزایی در آماده سازی این افراد پیش از درمان، حین درمان و پس از انجام اقدامات درمانی ایفا نماید.کلید واژگان: ناباروری, استرس روانی, افسردگی, اضطراب, مشاوره}Objective(s)Descriptive reports suggest that couples with fertility problems undergo various forms of psychoemotional stress which may render them susceptible to depression. Such stress may further decrease the likelihood of conception. On the other hand, reported that happiness scores were lower among people who fail to conceive as compared with those who had successfully done so. In review of articles most of studies have categorized women into two infertile and controled groups. Infertile group comprise women awating for ART. The other group consistes of women ( similar age distribution) who are ready had at least one child of their own and had no history of ART. The survey was carried out using questionnaires of the self – administration type.
ResultsThe prevalence of negative emotional feelings was higher in infertile group (37%-69%). Reported more changes interpartner relationship and sexuality. At the time of consultation, the infertiles had less favorable scores than controls on scales for depressed mood, memory/concentration, anxiety/feares and attractiveness. One in 4 of the infertiles had scores indicating depressive disorders as compared with only 6/8% of the controls.
ConclusionIt has emerged from the studies that many of those facing fetility problems may experience turmoil of negative feelings in particular at the time when they are attempting to conceive but realize they are not succeeding. Psychosocial consultation has significant role for preparation of these pre-, during and post ART.

Background
Auto antibodies to zona-pellucida (AZA) seem to be important autoantibodiesimplicated in reproduction, with substantial role in both endocrine and reproductive functions of thehuman ovary. There are some debates on the relation of AZA with infertility, repeated In VitroFertilization (IVF) attempts, and outcome of it.
ObjectiveIn this study, we assessed the presence of AZA in the follicular fluids (FFs) of womenwho underwent intra cytoplasmic sperm injection (ICSI), in relation to etiology of infertility andmultiple puncture of ovaries.
Materials And MethodsIn this prospective study, follicular fluids were evaluated from 96 infertilewomen, (19-40 years old, 31.5±5.1), who were candidates for ICSI based on the etiology ofinfertility. From these 80 women had explained infertility whereas 16 had unexplained infertility.All FFs were evaluated for presence of AZA by ELISA test.
ResultsTwenty patients (20.8%) were positive for AZA in follicular fluid. In patients withunexplained infertility, AZA antibody in follicular fluid, was significantly higher than the groupwith proven etiology of infertility (p=0.001). In addition, 20.4 % of patients who had beenpunctured previously showed AZA in their FFs which is statistically similar to the patients whowere punctured for the first time.
ConclusionsThe high incidence of AZA in infertile women, especially women with unexplainedinfertility has to be considered. Relation of the presence AZA and repeated puncture of ovaries isstill debatable. Determinations of AZA are highly recommended in evaluation of infertile couplesespecially in patient with unexplained infertility.
