68Ga-PSMA PET/CT has been routinely utilized in patients with intermediate to high-risk category prostate carcinoma for staging, biochemical recurrence and before planning the PSMA radioligand therapy (RLT). 177Lu-PSMA RLT has also been approved by FDA as a novel treatment modality in metastatic carcinoma prostate patients who have failed to other lines of treatment. The non-target organs like salivary and lacrimal glands have shown to have high physiological PSMA uptake on PSMA PET/CT. Recently, strong uptake of PSMA ligand has also been noted in the dorsal wall of the nasopharynx in the region of torus tubarius on PSMA PET/CT, which has led to the identification of new pair of salivary gland structures called “tubarial salivary glands”. The clinical significance of these distinct anatomical structures lies in the fact these structures might be involved in a variety of immune related, inflammatory disorders, malignancies and could be a probable organ at risk during radiotherapy in case of head and neck malignancies, causing adverse effects to the patient.

Hypercalcemia is a clinical condition characterized by elevated circulating serum calcium levels either due to raised parathyroid hormone in hyperparathyroidism or due to secondary causes of hypercalcemia without elevated parathyroid hormone levels. However, hyperthyroidism may occasionally present with incidentally detected hypercalcemia. We present a case of a 53-year-old woman with a previous history of an underlying thyroid disorder, now presented with features of hypercalcemia and mildly elevated parathyroid hormone levels. Her ultrasonography of the neck was suggestive of an intra-thyroidal parathyroid adenoma and it was localized as a tracer avid lesion within the thyroid gland on dual-phase 99mTc-sestamibi planar scintigraphy with single photon emission computed tomography/ computed tomography (SPECT/CT). However, a subsequent thyroid profile followed by 99mTc- pertechnetate thyroid scintigraphy showed a hot nodule in the thyroid gland which changed the diagnosis to a toxic thyroid adenoma. She was treated with radioactive iodine ablation and thyrotoxicosis resolved and the serum calcium levels normalized on her follow-up.

Asia Oceania Journal of Nuclear Medicine & Biology, Volume:9 Issue: 1, Winter and Spring 2021, PP 67 -70We present a case of metastatic prostate cancer with rare metastases involving the brain and orbit, in addition to liver, skeletal and nodal metastases. The patient had undergone prior hormonal therapy and chemotherapy and had disease progression despite 2 cycles of 177Lu-Prostate specific membrane antigen (177Lu-PSMA) based radioligand therapy. He had a partial response after 2 cycles of 225Ac-PSMA based targeted alpha therapy, as demonstrated on the 68Ga-PSMA PET/CT study. However, the patient had disease progression at the end of 4 cycles of 225Ac-PSMA therapy, evident by rising prostate specific antigen levels and imaging findings. The end of treatment 68Ga-PSMA PET/CT showed additional sites of metastases in the orbit and brain apart from overall disease progression. These are rare sites of distant spread in prostate cancer and require urgent evaluation and local treatment to prevent potential complications. The importance of detection of metastatic sites in closed cavities is because of the requirement for urgent intervention to avoid compression related complications.Keywords: Prostate Specific Membrane Antigen, Prostate cancer, Ocular, CNS, PET, CT

Asia Oceania Journal of Nuclear Medicine & Biology, Volume:8 Issue: 2, Summer and Autumn 2020, PP 132 -135We present the case of a 60-year-old man with metastatic neuroendocrine tumor of the ileum following ileal resection, being evaluated for 177Lu-based peptide receptor radionuclide therapy. 68Ga-DOTANOC PET/CT showed focal increased tracer uptake in the scrotal region without any morphologic changes on the corresponding CT images. Similar increased tracer uptake was seen on post-therapy whole-body imaging following 177Lu-DOTATATE therapy. An USG guided FNA revealed no malignant cells on cytopathologic examination. This case illustrates that focal testicular tracer uptake, may not always be pathological and can represent a normal physiologic variant, similar to the diffuse testicular somatostatin receptor expression as previously reported in literature.Keywords: 68Ga-DOTANOC- PET, CT, Testicular uptake, Physiologic Neuroendocrine tumor, 177Lu-DOTATATE, Somatostatin receptor, DOTATOC
