    برای روشن شدن تاثیر ساختار داخلی مولکول ها در اثر سره، ما به بررسی اعمال گرادیان دما به یک تک ذره شناور در سیال می پردازیم. تک ذره مورد بررسی با دو ریز کره که با فنر نرمی به یکدیگر متصل شده اند، یا به صورت جداگانه در دام دو انبرک نوری قرار گرفته اند، مدل شده است. ما نیرویی که این مجموعه دوکره ای در حضور گرادیان دما، به سیال پیرامون خود وارد می کند را محاسبه کرده، و با استفاده از آن ضریب سره مربوط به این مدل دو ذره ای را به دست می آوریم. در گام بعدی، وابستگی گرانروی سیال به دما را در محاسبات وارد کرده، و اثر این تصحیح را بر نتایج خود بررسی می کنیم.

    To study the effect of internal structure of molecules on the Soret effect, we probe consequences of exerting a temperature gradient to a suspension of one single particle inside the fluid. The particle is modeled as two microspheres, loosely connected with a spring between them, or each trapped with an optical tweezers. For the later case, we calculate the net force, that the two beads system exerts on its surrounding fluid, due to the a temperature gradient. This result, lets us calculate the corresponding Soret coefficient of the two beads system. We further correct these calculations, considering the temperature dependence of the surrounding fluid’s viscosity.

    Chronic eosinophilic pneumonia (CEP) is a rare idiopathic interstitial lung disease, predominantly observed in females. Eosinophilia is present in most cases, and alveolar eosinophilia is a diagnostic criterion in more than 40% of bronchoalveolar lavage (BAL) samples. The current study reported a 27-year-old male patient, non-smoker, with a history of uncontrolled asthma, presented to the emergency room with a complaint of cough, fever, and moderate dyspnea. A 30% eosinophilia was reported in his peripheral blood sample. A chest-X ray examination showed an upper and middle lobe consolidation, especially in the left lung. Broad-spectrum antibiotics were then started with a presumptive diagnosis of pneumonia, but no improvements were evident. The chest computed tomography scan showed air space opacities with septal thickening and predominant involvement of upper and middle lobes. Flexible bronchoscopy was performed, and the BAL sample analysis showed eosinophil infiltration, while negative culture. No parasites were identified. Transbronchial biopsies demonstrated eosinophil accumulation in alveoli and interstitium

    Conclusion

    Early recognition, diagnosis, and prompt treatment with corticosteroids are the main therapeutic approaches to CEP.

    Sesame food allergy (SFA); especially anaphylaxis, is a life-threatening condition. The accurate diagnosis of SFA is done by skin prick test (SPT), skin prick to prick (SPP) or specific IgE (sIgE) and is confirmed by oral food challenge (OFC). Since there are few studies evaluating and comparing the utility of these methods for diagnosis of sesame anaphylaxis in adult patients, we aimed to compare OFC with diagnostic tests, including SPT, SPP, and sesames IgE; using ImmunoCAP considering the sensitivity and specificity issues in patients with sesame anaphylaxis. Twenty patients with sesame anaphylaxis were diagnosed based on OFC. Then SPT, SPP, and sIgE were evaluated. Sixteen patients had positive OFC; while 4 patients had negative results. Out of 16 OFC+ patients, 7 patients were SPT+, 15 patients were SPP+, and 2 patients had detectable sIgE. A positive SPT indicated 44% sensitivity and 50% specificity. A positive SPP showed 87.5% sensitivity and 75% specificity. A positive ImmunoCAP test demonstrated 12.5% sensitivity and 75% specificity. The AUC of SPP was significant for the diagnosis of sesame anaphylaxis (p=0.038). In conclusion, when the OFC is not possible, the SPP test with natural sesame seed may be applicable in patients with a convincing history instead of the artificial or commercial extracts of sesame used for SPT. Positive SPP is a good alternative diagnostic method for patients with sesame anaphylaxis. Also, the poor sensitivity of SPT and sIgE may indicate the poor discriminative capability of these tests.

    Introduction
    Netherton Syndrome (NS) is a rare hereditary autosomal recessive disorder with ichthyosiform cutaneous lesions, specific hair shaft defect, and atopic diathesis. The incidence of NS is estimated to be approximately 1 in 200000. The objective of this case report is to present NS in a patient with severe eczema atopic dermatitis-like eruption.
    Case Presentation
    A 41-month-old boy was referred to the clinic of Allergy and Immunology, Hazrat Rasoul Hospital with generalized erythema, and scaling cutaneous lesions. The patient underwent clinical examinations and laboratory analysis. His laboratory data revealed only an elevated IgE level with a leukocyte count of 7800/μL containing 10% eosinophil. His hair shaft indicated classic trichorrhexis invaginata (Bamboo hair). Based on the clinical and laboratory findings, he was diagnosed with NS. A brief review was also done related to this case.
    Conclusion
    This case was reported because of the severity of the disorder and other differential diagnoses in severe and refractory atopic dermatitis-like eruptions. The importance of the case is related to other differential diagnoses, especially with early onset disorders at neonatal age.

    Hyper IgE syndromes are groups of primary immunodeficiency diseases which present with a series of symptoms including recurrent infections accompanied by elevated serum IgE level and some atopic features. Both autosomal dominant and recessive mutations may cause hyper IgE syndrome. The autosomal dominant forms are mutations in signal transducer and activator of transcription3 (STAT3), ERBB2 and CARD11. The recessive forms are mutations in dedicator of cytokinesis8 (DOCK8) , phosphoglucomutase3 (PGM3) , thyrosin kinase2 (TYK2) and interleukin-6 ST. There are some features that help distinguish among different types of hyper IgE syndrome. In autosomal dominant form, connective tissue, skeletal and vascular abnormalities are prominent. In autosomal recessive form, viral infections, malignancies and neurological disorders are more prominent. The definite diagnosis is made by mutation analysis.

    هدف
    یکی از علل کاهش تعادل، افت احساس در کف پا گزارش گردیده است؛ بنابراین، از کفی های دارای سطوح بافت دار برای افزایش حس کف پا و بهبود تعادل استفاده می شود. بیشتر مطالعات قبلی، بر روی کف پا متمرکز بوده و این تاثیرات روی نواحی دورسوم پا موردتوجه قرار نگرفته است. هدف از این مطالعه، مقایسه اثر سطوح بافت دار در قسمت های مختلف داخل کفش بر تعادل و حس عمقی مجموعه پاست.
    روش بررسی
    16 زن و 14 مرد دارای سابقه زمین خوردن در یک سال گذشته، به صورت تصادفی تحت چهار حالت مداخله حسی (بافت در کف پا، بافت در زبانه و یقه کفش، کفی پلاسبو و حالت کنترل) قرار گرفتند و حس درک موقعیت مفصل مچ پا و آزمون های تعادل عملکردی شامل: دسترسی به قدام، برخاستن و رفتن زمان دار در آنها اندازه گیری شد. برای مقایسه داده های ثبت شده در حالات مختلف مداخله از آزمون آماری تحلیل واریانس با اندازه های مکرر استفاده شد.
    یافته ها
    خطای حس موقعیت مفصل مچ پا در مداخله به شکل کفی بافت دار در زیر پا کاهش یافت (P<0.01). همچنین زمان انجام آزمایش برخاستن و رفتن هنگام استفاده از سطح بافت دار روی دورسوم، نشان از کاهش داشت (P<0.01).
    نتیجه گیری
    نتایج این مطالعه بر کاربرد سطوح بافت دار در داخل کفش و افزایش محتوای حس پیکری و بهبود وضعیت تعادل عملکردی و حس درک موقعیت مفصل پا تاکید دارد.

    Objective
    The plantar sensory impairments has an impact on the balance. Textured insoles are being used to improve foot sensation and balance. The majority of the literature has only focused on the intervention applied to the plantar surface; no adequate attention paid toward other area of the foot such as dorsum surface. This study was to compare the effect of textured insoles on ankle proprioception and balance.
    Materials and Methods
    16 females, 14 males with the history of falling during the last year were recruited in this study. They were randomly tested in four different conditions: textured insole, textured collar and tongue, placebo insole, and no insole. Ankle joint position sense and balance tests including “forward reach test” and “timed up and go” were carried out by all participants. All data were recorded and analyzed using “repeated measure ANOVA”.
    Results
    Participants did the ankle joint position sense with smaller errors while using textured insole (P<0.01). The recorded time showed a significant reduction when textured insole used over the dorsal surface and around the ankle, in timed up and go test (P<0.01).
    Conclusion
    It has been postulated that using the textured insole in different instep area of a shoe could improve the somatosensory sensation, functional balance and proprioception.

    Background
    Diarrhea is a leading cause of mortality and morbidity during the early life period especially in developing countries. Micronutrients deficiencies have been proposed either as a risk factor or a consequence of diarrhea. Association studies highlight the relation of vitamins and minerals’ deficiencies with acute diarrhea. In this regard we aimed to evaluate the status of vitamins A and D, and zinc serum levels in children with acute diarrhea.
    Methods
    In this cross sectional study performed in a referral teaching hospital, we measured and compared baseline vitamin A, 25-hydroxy vitamin D (25(OH)D), and zinc serum levels in 2 children admitted with acute diarrhea and 25 other children who were admitted for undergoing elective surgeries.
    Results
    25-(OH)D levels were significantly lower in the diarrhea group (p=0.03). We were unable to demonstrate a significant difference in the levels of vitamin A and zinc between the case and control groups (p= 0.14 and p=0.07, respectively).
    Conclusion
    We observed lower serum 25(OH)D levels in children with acute diarrhea. Whether this finding indicates a premorbid risk factor or simply a consequence of diarrhea needs further studies. Regardless of the cause and effect relationship, supplementation with vitamin D in acute diarrhea remains as a plausible consideration.
