    Background and Objective

    Fetal macrosomia increases the risk of perinatal mortality and morbidity. The aim of this study was to determine the prevalence of macrosomia and its relationship with related factors in neonates born in the hospitals affiliated to Mashhad University of Medical Sciences.

    Methods

    This cross-sectional study was performed on 97569 neonates using the data obtained from the neonateschr('39') screening program in Mashhad. Their information was extracted from Sina electronics system (electronic medical records software) and analyzed using SPSS 16. The prevalence of macrosomia and its related factors are revealed by tables.

    Findings

    In this study, the prevalence of macrosomia was estimated at 6.6 % which was higher in male neonates than females. The mean birth weight was higher in male infants than female ones. It occurred 8.25 times higher in singletons than multiplets. The prevalence of macrosomia was increased by maternal age. Most of the neonates were born by cesarean section, and in most of them, the parents were not relatives. The prevalence of macrosomia gradually increased in parents living in densely populated cities and its trend gently decreased from the beginning to the end of the year.

    Conclusion

    The prevalence of macrosomia in infants in Mashhad was 6.6% and was correlated with maternal age during pregnancy, infantchr('39')s gender, number of fetuses, mode of delivery, parental consanguinity, parental residence places and birth season.

    Idiopathic thrombocytopenic purpura is a common disease in children and young adults, as well as being the most common cause of acute thrombocytopenia in patients with good general health status.
    The case is a 23-year-old woman with no history of disease or any type of temperament disorder. She was referred to the haematology and oncology specialists in October 2014, following a sudden drop in platelet count, and complaints regarding numerous ecchymosis lesions in the upper and lower limbs. She was admitted to the Imam Reza Hospital in Tabriz. Due to the repeated reduction of platelet count in the patient, the oncologist ordered splenectomy, which is the removal of spleen. In December 2014, she was brought to the traditional medicine clinic. Her platelet count was 17,000. The patient received health and nutritional considerations. Pharmaceutical measures, including administering medical herbs and combination drugs, were taken with the patient’s temperament and the disease status driving the course. Several practices, including cupping, massage, blood-letting, meditation, and yoga, and eventually rose water incense, were part of a compiled programme for the patient. In subsequent sessions, the platelet level considerably increased and after a month, was almost close to the normal level. After 40 days of treatment, the platelet count reached the normal level, and finally in July 2015, with full recovery of the patient, the corticosteroids and herbal medicines were completely stopped. As is evident from the monthly monitoring of the patient until September 2016, she showed no clinical or laboratory symptoms explaining thrombocytopenia, and her platelet count was at normal level. Due to the absence of signs of ecchymosis and normal level of blood platelets in paraclinical testing, it seems that the combination of modern and Iranian traditional medicine can pave the way for the treatment of a number of diseases. Hence, further studies are required in order to find a cure for different diseases, according to the doctrines of Iranian effective medicine.

    Distal fractures of tibia along with fibula and ankle dislocation is one of the orthopedic problems seen in the area of fracture treatment. These fractures and dislocations can cause various complications such as non-union, bad union, ischemia caused by vascular damage, skin necrosis, and soft tissue damage of fracture, in addition to compartment syndrome, and venous congestion. The patient in this case study is a 12-year boy who suffered from right lower limb trauma due to a motorcycle accident. The patient underwent orthopedic surgery for internal fixation of bones and right ankle stability. After the surgery, the patient's right leg was swollen with severe venous congestion. The patient did not respond to conventional treatments, and so, the physician ordered leech therapy- according to a letter written by the physician to the traditional medicine clinic. The leech therapy was for the treatment of venous congestion in the patient’s right leg. Due to a severe trauma, major surgery, and post-operative venous congestion, the patient suffered from the melancholy in the surgical area, and the bottom of foot, and consequently, the whole body. The patient's venous congestion and severe swelling were significantly decreased after four stages of leech therapy on the basis of Iranian Traditional Medicine in two weeks with 10 leeches, and significant recovery was recorded. After two months of treatment by his physician, the patient underwent a successful surgery for a skin graft on anterior leg, and then was fully recovered. According to the process of patient’s recovery and significant treatment of patient’s leg edema and congestion after leech therapy, it seems that combining the Iranian modern and traditional medicines can be a solution to such diseases.

    Leukemia is the most common malignant neoplasm during the childhood and constitutes about 31% of malignancies in under-15-year-old children.
    We report a 3-year-old boy with acute lymphoblastic leukemia and a history of cancer complications and 9 months treatment process. After 2 months of unsuccessful treatment in a hospital in Bojnurd city, the patient was referred to Dr. Sheikh Hospital of Mashhad to undergo chemotherapy and different and frequent diagnostic methods. Due to lack of response to treatment, repeated relapses and lack of adequate financial resources and inability to pay the costs of repeated chemotherapy and radiotherapies he patient returned to Bojnurd after 9 months. With complaint of severe side effects and complications of drug treatments, the patient referred to Bojnurd traditional medicine center for treatment in September 2008. During the examination, the patient appeared pale with weak pulse, cold and swollen body. The patient was very weak and barely able to speak. After entering the traditional medicine clinic, the patient was controlled for his health and nutrition. In this case, the pharmaceutical measures were according to his temperament and disease status including the medicinal herbs and combined drugs. Manual actions were used for patient including full cupping therapy of back every night until the end of the therapy, Hijama (bloodletting) and massage therapy. The recovery appeared in the whole body from the fourth week. Since the patient's physical force and general status became much better, the routine tests were recommended of which hemoglobin and hematocrit test results were significantly better. After 8 months, all of the tests were normal and there was no relapse in clinical and laboratory tests until 2016.
    Given the patient's recovery process with an approach to his physical power, mental status, cancer control, and significant recovery of acute lymphoblastic leukemia and complications caused by cancer and chemotherapy, it seems that the combination of modern and traditional medicine can pave the way for most of the common diseases, and thus it is essential to conduct widespread assessments of different diseases based on teachings of Iranian Effective Medicine.

    Introduction
    Diabetic foot ulcer is among serious problems for diabetic patients and its treatment is very complicated and requires great care. Inappropriate responses to various medical and surgical treatments have made this complication problematic.
    Case Presentation
    The patient was a 58-year-old man with diabetes for more than twenty five years and hospitalized with complaint of diabetic foot ulcers, uncontrolled blood sugar, and sepsis at Shohadaye Gonbad Kavus hospital in June 2015. The patient's foot ulcer did not respond to conventional treatments, and amputation order was made by his treating orthopedist. The patient referred to traditional medicine clinic for counselling. The patient had two wounds on the right foot in Zone 1 (Right 11 X and 1 XR) with a diameter of approximately 5 cm by 6 cm and Zone (Right 9 Y and 6, 7, 10 X) with a diameter of approximately 12 cm by 5 cm which was equivalent to STAGE D/GRADE III according to clinical examination by traditional medicine physician based on the standards of Texas University Diabetic Wound Classification System, and equivalent to GRADE 4 based on the standards of Wagner Comprehensive System for Wound Diabetic Foot. The patient's health and nutrition was under control. Pharmaceutical measures in this patient were including the herbal medicines (including both uni- and multi-component medicines), and conventional antihyperglycemic drugs. The manual actions were including full back cupping every night until the end of therapy, massage therapy, and leech therapy inside and around the wound with 10 to 12 fine, medium and big leeches in 16 sessions every 2 to 4 days. The diabetic foot ulcer healed completely, the patient's physical and mental states improved, and his blood sugar was controlled after 60 days of treatment
    Conclusion
    It seems that combining the modern and traditional medicine can treat most of the common diseases especially diabetic foot ulcer and it is essential to perform a wide range of assessments and studies in different diseases based on the Iranian effective medicine.
