زمینه و هدف
ارزشیابی اقتصادی مداخلات بهداشتی با مقایسه هزینه ها و منافع مداخلات بهداشتی منجر به تخصیص بهینه منابع و افزایش کارایی تخصیصی نظام سلامت و با بهبود عدالت و افزایش دسترسی به خدمات سلامت، منجر به افزایش اثربخشی نظام سلامت می شود. هدف این پژوهش ارزشیابی انتقادی مطالعات منتشر شده تحلیل هزینه -اثربخشی مداخلات بهداشتی در جهان است.
روش کاراین پژوهش با روش مرور انتقادی انجام شد. کلیه مقالات مرتبط با تحلیل هزینه- اثربخشی مداخلات بهداشتی منتشر شده از سال 1975تا 2018 میلادی در هفت پایگاه الکترونیکیCochrane Database of Systematic Review، Web of science، Scopus، PubMed، Science Direct، Springer Link و SID و موتور جستجویGoogle scholar با استفاده از کلید واژه های مناسب جمع آوری و ارزشیابی کیفیتی شدند. تعداد 173 مطالعه تحلیل هزینه-اثربخشی مداخلات بهداشتی شرایط ورود به این پژوهش را داشتند.
نتایجمطالعات هزینه-اثربخشی مداخلات بهداشتی در شش گروه مداخلات واکسیناسیون، آموزش، تغذیه ، پیشگیری از بیماری های مقاربتی، پیشگیری از بیمارهای زنان و زایمان و کنترل بیماری های دارای ناقل انجام و منجر به شناسایی مداخلات بهداشتی هزینه-اثربخش شدند. با وجود این، مطالعات تحلیل هزینه-اثربخشی به ویژه در کشورهای در حال توسعه با محدودیت های پژوهشی متعددی مواجه هستند. پژوهشگران مطالعات ارزشیابی اقتصادی باید دقت بیشتری به ویژه در انتخاب درست دیدگاه و افق زمانی ارزشیابی اقتصادی، روش پژوهش، حجم نمونه، نحوه محاسبه هزینه ها و نتایج مداخله بهداشتی، ابزار مورد استفاده برای سنجش اثربخشی، روش و ساختار مدل مورد استفاد برای تحلیل هزینه-اثربخشی و تحلیل حساسیت و اعتبار یافته های مطالعه به عمل آورند.
نتیجه گیریمطالعات هزینه-اثربخشی و هزینه- مطلوبیت مداخلات بهداشتی به ویژه مداخلات واکسیناسیون بیماری ها در دو دهه گذشته افزایش چشمگیری داشتند. دستورالعمل ها و راهنماهای ارزشیابی اقتصادی باید بازنگری شوند و آموزش های لازم در اختیار پژوهشگران قرار گیرد تا کیفیت این مطالعات افزایش یابد.

The purpose of this study was to critically evaluate the Cost Effectiveness Analysis (CEA) studies on health interventions worldwide.
Materials and MethodsA critical review of the published CEA studies on health interventions was conducted. Seven databases including PubMed, Cochrane Database of Systematic Reviews, Web of science, Science Direct, Scopus, Springer Link, and SID were searched between 1975 and 2018, using appropriate keywords. The retrieved articles were evaluated using the Drummond (2005) quality assessment checklist. Overall, 173 articles met the entry criteria and were included in this study.
ResultsCEA of health-related interventions were classified into six categories, namely, studies on immunization, education, nutrition, sexually transmitted diseases prevention, gynecological diseases prevention and vector-borne diseases and, eventually, cost-effective interventions were identified. Further analysis of the data showed that the methods used in health intervention CEA studies are very heterogeneous and lack sufficient scientific quality especially in developing countries. Researchers working in this area should pay more attention when designing studies and follow valid guidelines for CEA, particularly as regards research methods, sample size, CEA model, cost and benefit calculations, determining effectiveness, timeframe and, finally, analysis of the sensitivity and validity the research data.
ConclusionThe number of cost effectiveness analysis and cost utility studies has increased greatly during the last two decades. In order to improve the quality of these studies it is essential to revise the guidelines and procedures for economic evaluation of health interventions and train and update researchers in this area.

مجله دانشکده بهداشت و انستیتو تحقیقات بهداشتی، سال چهاردهم شماره 4 (پیاپی 56، زمستان 1395)، صص 97 -109زمینه و هدفارزشیابی اقتصادی مداخلات بهداشتی برای تخصیص بهینه منابع در بخش سلامت ضروری است. هدف این پژوهش مرور انتقادی مطالعات تحلیل هزینه - منفعت مداخلات بهداشتی در 40 سال گذشته است.روش کارمطالعه حاضر با جستجو در هفت پایگاه اطلاعاتیWeb of science Science Direct، Scopus، PubMed، Springer، Link Cochrane Database of Systematic Reviews، و SID با استفاده از کلید واژه های مناسب برای یافتن مقالات تحلیل هزینه-منفعت مداخلات بهداشتی منتشر شده در سال های 1975 تا 2015 میلادی انجام شد. مقالات بدست آمده پس از ارزشیابی کیفیت با استفاده از یک ابزار معتبر، تحلیل شدند.نتایجتعداد 33 مطالعه تحلیل هزینه-منفعت مداخلات بهداشتی در فاصله زمانی تعیین شده بدست آمد. به ترتیب 39، 37، 21 و 3 درصد از مطالعات در قاره های آسیا، آمریکا، اروپا و آفریقا انجام شده بود. بیشتر مطالعات (79%) درباره مداخلات مرتبط با واکسن، آموزش، تغذیه سالم و استفاده از فن آوری های بهداشتی بود. مداخلات مختلف واکسیناسیون، آزمایش های تشخیصی و غربالگری، تغذیه مناسب، بهداشت دهان و دندان، آموزش سلامت روان و استفاده از تجهیزات ایمنی هنگام رانندگی دارای منافع بالینی و اقتصادی بسیار زیاد هستند.نتیجه گیریمطالعات تحلیل هزینه - منفعت مداخلات بهداشتی در حال افزایش است. این مداخلات می توانند موجب استفاده بهینه از منابع محدود بخش سلامت به منظور ارتقای سلامت مردم شوند.کلید واژگان: ارزشیابی اقتصادی, تحلیل هزینه-منفعت, مداخلات بهداشتی, مرور انتقادی}Background And AimEconomic evaluation of health interventions is critical for efficient allocation of resources. The aim of this study was to critically review empirical studies on the cost-benefit of health interventions published over the last 40 years.Materials And MethodsThis study was carried out by searching seven databases (PubMed, Cochrane Database of Systematic Reviews, Web of science, Science Direct, Scopus, Springer Link, and SID) using appropriate keywords to find articles on the cost-benefit of health interventions published during 1975-2015. All the relatedretrieved articles were analyzed after quality assessment using a valid check list.ResultsA total of 33studies on the cost-benefit of health interventions during the last 40 years were included in the review; 39%, 37%, 21% and 3%of the studies had been conducted in Asia, America, Europe and Africa, respectively. The subjects dealt with in most ofthe studies (79%) were related to immunization, education, healthy nutrition and use of health technologies. Further analysis of the data showed mental health education, healthy nutrition, use of safety equipment while driving, vaccination, diagnostic and screening tests, and oral and dental healthto have considerable clinical and economic benefits.ConclusionThe number of studies on the cost-benefit analysis of health interventions is increasing.
