Background
Type 2 diabetes is a group of heterogeneous disorders characterized by various degrees of insulin resistance, impaired insulin secretion, and increased glucose production. Sugar substitute are known as a food additive that provides a sweet taste like sugar without excess energy. Some sugar substitutes are naturally produced, and others are synthesized. Those resources which are not natural are generally called artificial sweeteners.
ObjectiveThe present study was carried out to assess the effects of artificial sweeteners in diabetic patients by considering the level of blood sugar before and after intervention.
MethodsA total of 30 patients were included in this clinical trial. The patients were asked not to use the artificial sweeteners for 3 months and then were asked to consume diabetic especial diet containing artificial sweeteners such as sugar, biscuit and candy under the supervision of food specialist for the next three months.
ResultsThe average Fasting blood glucose of diabetic patients increased from (121.03±25.3 mg/dl) to (152 ± 42 mg/dl) at the end of second trimester. Also, two hours postprandial glucose test showed a significant increase from (164.4±44.4 mg/dL) to (222±72 mg/dL). Interestingly, the average of HbA1C levels increased since (6.89 ± 0.9%) to (7.40 ±1.2%) at the end of second trimesters.
ConclusionThe results indicated that consuming food gradients containing artificial sweeteners may increase blood sugar in diabetic patients.

BackgroundAs there is no standard aphasia screening tool for Azeri language yet, the aim of this study was to develop an aphasia screening test with acceptable validity and reliability.MethodsThe present study was conducted in two phases. In the first phase, by literature search, the screening test was designed and to obtain validity it was peer reviewed by expert panel. After collecting experts ratings and comments, appropriate modifications were applied. For test-retest reliability in the second phase, edited test was administered in 32 patients with brain injuries, then the retest was performed two weeks later.ResultsThe developed test had eight subscales including: A) picture description, B) syntax, C) linguistic reasoning, D) descriptive naming, E) perception of minimal pairs, F) comprehensive vocabulary, G) expressive vocabulary, H) verbal fluency. Each section had five questions except verbal fluency which had 3 items. Content validity ratio (CVR) according to Lawshes approach, was 82% for the whole test. Intraclass correlation for all subscales were more than 0.8. Cronbachs alpha coefficient for internal reliability was 0.901.ConclusionThis aphasia screening test seems to have acceptable psychometric properties. مجله دانشکده پزشکی دانشگاه علوم پزشکی مشهد، سال پنجاه و هفتم شماره 9 (پیاپی 133، اسفند 1393)، صص 962 -968مقدمهکمبود ویتامین D یک مشکل مهم بهداشتی جامعه است که در افراد مسن شایع تر است و ممکن است در افراد مسن ساکن خانه سالمندان نیز بیشتر باشد. هدف از این مطالعه بررسی میزان شیوع کمبود ویتامینD در افراد مسن ساکن خانه سالمندان و مقایسه آن با افراد مسن خارج خانه سالمندان می باشد.روش کاردر یک مطالعه موردی- شاهدی که در سال 1387-1388 در شهر تبریز انجام شد، 140 فرد مسن ساکن خانه سالمندان و 140 فرد مسن خارج از خانه سالمندان وارد مطالعه شدند. سطوح سرمی 25(OH)D، هورمون پاراتورمون (PTH)، آلکالین فسفاتاز (Alp)،کلسیم (Ca)، فسفر(P)، آلبومین (Alb) در این افراد اندازه گیری شدند. از روش های تی، کای دو، آنوا وهمبستگی برای تجزیه وتحلیل آماری فرضیات استفاده شد.نتایجمیانگین سطح سرمی 25(OH)D در افراد ساکن خانه سالمندان ng/ml 65/16±10/16و در افراد خارج از خانه سالمندان ng/ml 78/24± 62/39 بود (001/0= p. شیوع کمبود ویتامین D (کمتراز 20 ng/ml) در گروه ساکن خانه سالمندان 75% و در گروه خارج خانه سالمندان 1/20% افراد را شامل می شد. سطح سرمی ویتامین D با سطح سرمی PTH در دو گروه مورد و شاهد و با سطح Alp در گروه مورد ارتباط معکوس داشت (001/0 = p).
نتیجه گیریدر این مطالعه شیوع بالایی از کمبود ویتامین D در افراد مسن ساکن خانه سالمندان نشان داده شد که میزان آن در مقایسه با افراد همسن و همجنس خارج از خانه سالمندان بیشتر بود. توصیه می شود از روش هایی همانند غنی سازی مواد غذایی با ویتامین D و یا مکمل های ویتامینD در گروه های در معرض خطر بیشتر استفاده شود.

So we conducted a study to evaluate the prevalence of Vit D deficiency in elderly nursing home residents and compared it with the other elderly population.Materials And MethodsIn a cross-sectional study 140 aged nursing home residents (G1) & 140 elderly in the community (G2) were enrolled. Serum 25(OH) D, parathyroid hormone (PTH), Alkaline phosphatase (alp), Calcium, Phosphor, Albumin were measured in all subjects. We used t test, chi square test, ANOVA and correlation for statistical analysis of hypothesis.Resultsmean serum 25(OH) Vit D was 16.1±16.64 ng/ml in G1 and 39.62 ± 24.78 in G2 (p= 0.000) Vit D level less than 20 IU/ml was found in 75% of G1 and 24% of G2. There was a significant correlation between serum 25(OH)D and PTH in both groups, and between 25(OH)D and Alp in cases.(P=.001)ConclusionIn this study we showed that Vit D deficiency among the elderly in nursing home residents was higher than the adjusted population out of nursing homes.
