زمینه و هدف
یکی از شایع ترین مشکلات روانی در بین بیماران مبتلا به وزوز گوش، اضطراب است. لذا پژوهش حاضر با هدف مقایسه اثربخشی درمان ترکیبی فراشناخت درمانی با تحریک الکتریکی فراجمجمه ای بر کاهش اضطراب مبتلایان به وزوز گوش مزمن صورت گرفت.
روش پژوهش:
طرح پژوهش، نیمه آزمایشی با طرح پیش آزمون – پس آزمون با گروه کنترل بود و جامعه آماری شامل کلیه بیماران دارای مشکل وزوز گوش مراجعه کننده به مرکز ارزیابی شنوایی رضوان افشار در شهر اراک در سال 1401 بود. حجم نمونه آماری، 30 نفر (15 نفر گروه آزمایشی و 15 نفر گروه کنترل) از جامعه آماری مذکور بود که به شیوه نمونه گیری در دسترس انتخاب و به شیوه تصادفی در دو گروه کنترل و آزمایش جایدهی شدند. گروه آزمایش در طی 8 جلسه ابتدا به مدت 20 دقیقه تحت تحریک الکتریکی فراجمجمه ای و بلافاصله به مدت 60 دقیقه تحت مداخله فراشناخت درمانی قرار گرفتند و گروه کنترل تنها درمان معمول خود (تحریک الکتریکی فراجمجمه ای) را دریافت کردند. در این پژوهش، برای جمع آوری داده ها از پرسشنامه اضطراب بک استفاده شد. جهت تجزیه و تحلیل آماری داده های بدست آمده، از آزمون تحلیل کواریانس تک متغیره استفاده شد.
یافته هامیانگین نمرات اضطراب در مراحل قبل و بعد از مداخله در گروه آزمایش به ترتیب 13/11 و 20/7 و در گروه کنترل به ترتیب 40/11 و 53/8 بود. نتایج تحلیل کوواریانس نشان داد که میانگین اضطراب در گروه آزمایش، پس از مداخله ترکیبی فراشناخت درمانی با تحریک الکتریکی فراجمجمه ای در مقایسه با گروه کنترل به طور معناداری کاهش پیدا کرده است (01/0<p).
نتیجه گیرینتایج این پژوهش حاکی از آن است که مداخله ترکیبی فراشناخت درمانی با تحریک الکتریکی فراجمجمه ای به طور معناداری سبب بهبود اضطراب شده است، بنابراین استفاده از این مداخله برای مبتلایان به وزوز گوش مزمن پیشنهاد می شود.

Therefore, the present study was conducted with the aim of comparing the effectiveness of combined metacognitive therapy with transcranial electrical stimulation on reducing anxiety in chronic tinnitus sufferers.
MethodsThe research design was semi-experimental with a pre-test-post-test design with a control group, and the statistical population included all patients with tinnitus who referred to Rizvan Afshar Hearing Evaluation Center in Arak city in 1401. The size of the statistical sample was 30 people (15 people in the experimental group and 15 people in the control group) from the mentioned statistical population, who were selected by available sampling method and randomly placed in two control and experimental groups. During 8 sessions, the experimental group was first subjected to transcranial electrical stimulation for 20 minutes and immediately underwent metacognitive therapeutic intervention for 60 minutes, and the control group received only their usual treatment (transcranial electrical stimulation). In this research, Beck's anxiety questionnaire was used to collect data. In order to statistically analyze the obtained data, univariate analysis of covariance test was used.
ResultsThe mean of scores of anxiety before and after the intervention in the experimental group were 11.13 and 7.20, respectively, and in the control group, 11.40 and 8.53, respectively. The results of covariance analysis showed that the mean anxiety in the experimental group has decreased significantly (P<0.01) after the combined intervention of metacognitive therapy with transcranial electrical stimulation in comparison with the control group.
ConclusionThe results of this research indicate that the combined intervention of metacognitive therapy with transcranial electrical stimulation has significantly improved anxiety, so the use of this intervention is recommended for chronic tinnitus sufferers

ObjectiveOccupational noise is one of the major causes of hearing loss and tinnitus. In the present study, the effect of occupational noise-induced tinnitus on auditory attention was investigated.MethodA sample of 42 man, all workers, ages 40 to 56, participated in the study in three groups of workers with hearing loss, hearing loss and tinnitus, and normal hearing. Workers in two first groups had a long history of working in environments with excessive noise, and third group was matched control workers with normal hearing. For all subjects the Mini-Mental State Examination (MMSE) and the Bergen dichotic listening test were performed in identical conditions.ResultsIn all three conditions of attention, including non-forced, forced right, and forced left attentions, right ear advantage (REA) was lower in tinnitus and hearing loss group than the two other groups. The difference between the tinnitus and hearing loss groups and the control group was significant in the non-forced attention condition (p<0.047). Furthermore, both groups had poorer result in the MSSE test compared to the control group (p<0.025).ConclusionOur study revealed effect of noiseinduced tinnitus on selective and divided auditory attention.
