Neonatal hypoxia-ischemia (HI) is one of the most important causes of neurological disorders in children. Various studies suggest that maternal exercise during pregnancy has a beneficial impact on the health status of offspring infants. In this study, the effect of maternal treadmill exercise during pregnancy on neurological and molecular changes induced by HI in newborn rats was investigated.
Materials and MethodsIn this experiment, 24 pregnant female rats were divided into two groups; the first group was subjected to treadmill exercise for six weeks. The treadmill exercise program was initiated by running for 17 min at 5–10 m/min at 0 inclination in the first week, followed by running for 21 min at 5–25 m/min at 5° inclination in the second week, running for 25 min at 5–30 m/min at 10° inclination in the third and fourth weeks, running for 25 min at 5–15 m/min at 10° inclination in the fifth and sixth weeks. The second group was left untreated and did not perform the exercise. Newborn rats were assigned to four groups; (1) control, (2) control+exercise, (3) HI, and (4) HI+exercise. HI was developed in the offspring on the 8th postnatal day. One week following the induction of HI, the Garcia test was carried out. The histological morphology of neonates was assessed, and the expression levels of caspase-1 and NLRP3 were evaluated.
ResultsThe data showed that maternal exercise during pregnancy significantly improved neural cell death (P<0.001) and the Garcia score (P<0.05), while it attenuated the expression levels of Caspase-1 (P<0.001) and NLRP3 (P<0.05) genes in newborn rats induced by HI.
ConclusionThese results demonstrated that maternal treadmill exercise during pregnancy could reverse the neurological deficits, as well as the expression levels of caspase-1 and NLRP3 genes, which occur in neonatal hypoxia-ischemia.

مقدمه
فلج مغزی، یکی از علل شایع شکایت قصور پزشکی در حوزهی مامایی است. تصور میشود که پس از ابلاغ و اجرای بخشنامهی ترویج زایمان طبیعی در سال 1393، شکایات مرتبط با آسیب مغزی نوزاد و فلج مغزی ناشی از آسیبهای هیپوکسیک ایسکمیک حین Labor افزایش یافته است. از این رو، مطالعهی حاضر با هدف مقایسهی پروندههای شکایت قبل و بعد از این تاریخ انجام شد.
روشهااین مطالعه مقطعی بر روی 117 پرونده شکایتهای با موضوع فلج مغزی مطرحشده در اداره کمیسیون پزشکی استان تهران از ابتدای سال 1390 تا انتهای سال 1396 که به صورت سرشماری انتخاب شدند، انجام شده است. دادهها با استفاده از فرم ثبت اطلاعات از پروندهها استخراج گردید و با استفاده از نرم افزار SPSS واکاوی شد.
یافتههازمان تولد تعداد 64 مورد (7/54 درصد) از موارد شکایت در رابطه با فلج مغزی قبل از ابلاغ بخشنامه و 53 مورد (3/45 درصد) بعد از آن بوده است. از 64 مورد متولدین قبل از ابلاغ، تعداد 33 مورد شکایت خود را قبل از این تاریخ ثبت کردهاند و سایر موارد (31 مورد) بعد از تاریخ ابلاغ بخشنامه طرح شکایت کردهاند و شکایات پس از ابلاغ به 84 مورد رسیده است. با وجود این، تعداد متولدین در این بازهی زمانی 53 مورد بوده است و زمان تولد با زمان شکایت تفاوت آماری معنیداری داشت (001/0 > P).
نتیجهگیریبا وجود افزایش تعداد شکایت با موضوع فلج مغزی در طی سالهای اخیر، غالب موارد فلج مغزی مربوط به متولدین سالهای قبل از ابلاغ بخشنامه است و ابلاغ بخشنامهی ترویج زایمان طبیعی، منجر به افزایش شکایت با موضوع فلج مغزی در طی سالهای مورد مطالعه نشده است.

It is assumed that after the announcement and implementation of ministerial circular promoting vaginal childbirth in 2014, complaints related to neonatal brain damage and cerebral palsy due to hypoxic ischemic injuries during labor and delivery have increased. So in this study, this claim was investigated by comparing the complaints before and after this time.
MethodsThis was a cross-sectional study on 117 complaints on cerebral palsy selected by census in the Tehran Medical Commission, Iran, from 2011 to 2017. Data were extracted from the files using a data registry form, and analyzed using SPSS software.
ResultsOut of 117 complaints related to cerebral palsy, 64 cases (54.7%) were born before announcement of circular and 53 (45.3%) after it. Out of 64 cases born before it, 33 cases complained before and the rest (31 cases) after its announcement; so all the complaints after its announcement reached to 84 while number of cases born in this period were just 53. There was statistically significant difference between the time of birth and the time of complaints (P < 0.001).
ConclusionDespite the increase in the number of complaints on cerebral palsy in recent years, the majority of these cases are related to those born in the years before the announcement. So announcement of this circular has not caused excess in complaints on cerebral palsy during this period of time
