Introduction
Atypical infections are often considered as a potential etiology for men with chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). We aimed to describe the prevalence of atypical infections in this patient population to inform clinical management for male patients complaining of pelvic pain.
MethodsWe retrospectively reviewed patients at a single center from January 2016 to January 2019. We included patients with CP/CPPS Type III diagnosed with pelvic or genital pain in the absence of bacterial infection. All patients underwent an atypical infection panel. The primary outcome measure was the presence of any atypical infection.
ResultsIn total, 345 patients met the inclusion criteria. Of those, 9/345 (2.6%) had an atypical infection (5 mycoplasma and 4 ureaplasma). The mean age of patients with positive atypical infections was 34 compared to the overall study population (44 years, P=0.01). Two patients with atypical infections were also followed for infertility. Urinalysis was available for 6 of the 9 patients with positive atypical infection: 50% (3 out of 6) were normal and 50% (3 out of 6) had >5 WBC/hpf. Symptoms resolved in 66% (2 out of 3) of the patients with positive atypical infection with available follow-up data.
ConclusionAtypical infectious agents were uncommon causes of CP/CPPS. Screening for atypical microbes such as chlamydia, ureaplasma, or mycoplasma may not be necessary for male patients complaining of pelvic or genital pain.

Chronic prostatitis/Chronic pelvic pain syndromes (CP/CPPS) are a widespread pathology with unknown etiology without a proved treatment algorithm. Neurologic, endocrine and immune systems, and oxidative stress, infections are ranked in the physiopathology.Anogenital distance (AGD) as a marker for the degree of antenatal exposure of androgens that link to some disor-ders of androgen-sensitive tissues especially of urogenital system. In this study, we aimed a construct a hypothesis that improper development of perineum and pelvic bottom due to the insufficient embryologic androgen exposure, which can be detected by reduced AGD, can form histologic/clinic CP in adulthood through the physical forces that resulted in stretched prostate via chronic hypoxia induced oxidative stress and failed immune mechanisms. AGD, unlike the previous published ones, suggested as a real physical scale to detect narrowed pelvic bottom other than an endocrine related biomarker.

BackgroundInnovative thermobalancing therapy (TT) with Dr Allen’s therapeutic device (DATD) treats affected organs by the energy of one’s own body. 10 years empirical evidence has shown that the use of DATD helps to recover from chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) and benign prostatic hyperplasia (BPH), and to dissolve kidney stones. Moreover, the cost of DATD compares favourably with the cost of conventional treatments.ObjectivesThe purpose of this study is to point to the introduction of TT from DATD to the Primary Health Care system. Methods and Results: Two clinical trials observed dynamics of urinary symptoms by International Prostate Symptom Score (IPSS), quality of life (QoL), ultrasound prostate volume (PV mL) and uroflowmetry (maximum flow rate, Qmax mL/s) in men with BPH and chronic prostatitis. These trials have explored that TT with DATD reduced: prostate volume from 45 mL to 31 mL (P < 0.001) and, consequently, lower urinary tract symptoms, from 14.2 to 4.9 (P < 0.001) in the treatment group 124 patients with BPH; and pain score, from 10.38 to 3.58 (P < 0.001), and decrease of prostate volume from 31 mL to 27 mL (P < 0.001) in the treatment group of 45 men with CP/CPPS. The QoL improved in both treatment groups significantly. In the control groups: in 124 men with BPH in watchful waiting, and in 45 men with chronic prostatitis on standard treatment, no significant difference in parameters was observed. 10 years empirical evidence showed that DATD treats both kidneys simultaneously, dissolving kidney stones, despite their size and type.ConclusionsThe outcomes of clinical studies and empirical evidence demonstrated the high effectiveness of TT, which was prescribed as mono-therapy. This therapy is side effects free. It is easy to use Dr Allen’s device, which is a class 1 medical device that doesn’t require involvement of a notified body. BackgroundMedications, alternative and complementary treatments for type-III chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) are used frequently. The aim of this article is to define thermobalancing therapy as an independent treatment for internal diseases, such as CP/CPPS.MethodsThe effect of thermobalancing therapy (TT) by using Dr. Allens therapeutic device-(DATD) on patients with CP/CPPS was investigated. National institute of health chronic prostatitis symptom index (NIH-CPSI) scores, prostatic volume (PV), and maximum urinary flow rate (Qmax) were measured in one group of 45 patients who underwent TT and a control group that did not have TT. These all parameters were compared between groups.ResultsNo significant difference was found at baseline evaluation in treatment and control groups with regard to age, NIH-CPSI score, PV or Qmax. In the treatment group pain score decreased and quality of life (QoL) improved significantly, whereas in the control group no changes. TT reduced PV and increased Qmax significantly, whereas in the control group TT did not elicit significant changes in PV and Qmax.ConclusionsThe study has explored that TT with DATD as monotherapy for CP/CPPS patients: (i) reduces pain dramatically and improves QoL; (ii) reduces PV and increases Qmax. None of the patients who received TT suffered side effects and the cost of TT compares favourably with the cost of conventional treatment. BackgroundThe optimal treatment is not possible yet for chronic prostatitis due to the unknown etiology of the diseases. We aimed to investigate the association of Helicobacter pylori infection with chronic prostatitis.MethodsIn this prospective case-control study that conducted in Imam Hospital Complex affiliated to Tehran University of Medical Sciences in Tehran, Iran from 2014 to 2015, patients with diagnosis of chronic prostatitis according to the criteria of National Institute of Health (NIH) were enrolled. Control group constituted of consecutive healthy patients.Blood samples were obtained for each patient and control and evaluated for serum levels of anti H. pylori IgG, A. Data analysis was carried out using SPSS, version 18. Values of PResultsMean ± SD age of patients was 59.5 ± 3.08 yr in the case group and 56.88 ± 3.20 yr in the control group with no significant difference (P>0.05). Mean ± SD levels of anti H. pylori IgG and IgA in the control group were 9.36 ± 7.45 U/ml and 6.25 ± 7.29 U/ml, respectively compared with 20.94 ± 16.98 U/ml and 18.63 ± 15.65 U/ml in the case group, respectively both of which revealed statistically significant (PConclusionChronic prostatitis is associated with H. pylori infection. Both anti H. pylori IgG and IgA are increased in patients with chronic prostatitis.
