    The progression and recurrence of urothelial carcinoma (UC) are correlated with carcinoma in situ and urothelial dysplasia. It is frequently challenging to distinguish dysplasia and carcinoma in situ from reactive atypia only based on histological characteristics. In daily practices, 2 of the adjunct immunohistochemistry markers (cytokeratin 20 (CK20) and p53) are used in addition to the histology to diagnose carcinoma in situ. This is accomplished by combining histological research results with immunohistochemistry. This systematic review summarizes the current findings on the diagnostic significance of p53 and CK20 as adjunct markers to urine cytology in the detection of UC. A systematic search of the relevant literature was conducted using PubMed, Wiley Online Library, and ScienceDirect databases. After screening for the eligibility criteria, a total of 14 selected articles were reviewed. Data extraction included a total number of samples, specimen samples, type of cells, and outcome parameters (mainly sensitivity and specificity). Urine cytology alone had a sensitivity of 75%-85% and specificity of 66%-95%. CK20 with urine cytology staining showed improved sensitivity and specificity in the range of 77%-94% and 71%-100%, respectively; p53 immunostaining with urine cytology showed a sensitivity of 52%-86% and specificity of 80%-98%. The dual staining in combination with urine cytology showed comparatively higher sensitivity and specificity in the range of 70%-90% and 74%-100%, respectively. This was more evident for high-grade UC (HGUC). Overall, single or dual staining combined with urine cytology was effective in this detection and can be applied as an adjunct marker in urine cytology.

    Background

    Over-expression of CD117 and cytokeratin 20 (CK20) is seen in many malignancies. Given that few studies have been conducted regarding the role of these biomarkers in the etiology of bladder tumours, this study aimed to evaluate the prognostic value of CD117 and CK 20 biomarkers in the benign and malignant tumours of urinary bladder carcinoma.

    Methods

    This case-control study was conducted on 62 bladder tumours (17 benign tumours and 45 malignant tumours). The immunohistochemistry technique was used to assess CK20 and CD117 biomarkers in tumour samples.

    Results

    There is no significant difference between benign and malignant groups in terms of CD117 (p=0.094). No significant difference was observed between expression of CD117 in terms of grade (p=0.184). However, a significant difference was observed between benign and malignant groups in terms of CK20 expression (p=0.022). Moreover, a significant difference was observed between CK20 expression in terms of grade (p=0.009). In addition, a significant difference was observed between the age of patients considering grade (p<0.047).

    Conclusion

    The results of the current study supported the role of CK20 in the carcinogenesis of urinary bladder carcinoma. Moreover, it seems that CK20 expression may be considered as a poor prognostic parameter in transitional bladder carcinoma. Furthermore, these findings indicated that patients with higher age had higher grade. In addition, assessment of CD117 expression did not play a main role in tumour progression. Therefore, it is not recommended for bladder tumours.

    Colorectal cancer remains one of the major cancer- related deaths despite progress in the treatment during past decades. Detection of disease at earlier stages reduces its mortality. The aim of current study was to investigate expression of Cytokeratin 19 (CK19), Cytokeratin 20 (CK20) and Guanylyl Cyclase C (GCC) mRNA in peripheral blood of non- metastatic colorectal cancer patients which may result into introducing of an early detection test. 25 patients with colorectal cancer and 25 healthy controls were recruited. Blood was obtained from all individuals. Expression of CK19 and CK20 and GCC mRNA and 18SrRNA (as reference gene) were determined based on real- time RT-PCR on total RNA from blood.     مقدمه و هدف
    تعیین پیش آگهی سرطان معده، پس از تشخیص آن، مهم ترین مرحله در روند درمان است. سیتوکراتین ها فیلامان های حد واسطی هستند که در اسکلت داخل سلولی بافت های اپی تلیالی یافت می شوند. تحقیق های اخیر بر تعیین ارتباط میان بروزسیتوکراتین های مختلف با درجه و پیش آگهی تومورها، متمرکز شده اند. هدف از این مطالعه، بررسی ارتباط میان بروز سیتوکراتین 20 و سیتوکراتین 7 در مبتلایان به کارسینوم معده با عوامل موثر در تعیین پیش آگهی است.
    مواد و روش ها
    مطالعه به صورت مقطعی انجام شد. بروز سیتوکراتین 20 و سیتوکراتین 7 روی 31 نمونه آدنوکارسینوم معده با درجه تمایز متفاوت به شیوه ایمنوهیستوشیمیایی بررسی شد؛ ارتباط میان بروز سیتوکراتین 20 و سیتوکراتین 7 با عوامل موثر در پیش آگهی بیماران، شامل درجه تمایزیافتگی بافت سرطان معده، درگیری گره های لنفاوی و عمق تهاجم تومور تعیین شد. اطلاعات حاصل با آزمون های آماری کای-اسکوار و اسپیرمن تجزیه وتحلیل آماری شدند.
    نتایج
    میان بروز سیتوکراتین 20و سیتوکراتین 7 با درجه تمایزیافتگی بافتی و درگیری گره های لنفاوی در سرطان معده، ارتباط آماری معکوس وجوددارد. اگرچه میان میزان تهاجم بافتی در سرطان معده با بروز سیتوکراتین 7 ارتباط آماری معناداری وجودداشت، ارتباطی میان بروزسیتوکراتین 20 و تهاجم بافتی دیده نشد.
    نتیجه گیری
    کاهش بروزسیتوکراتین 20 و سیتوکراتین 7 با کاهش درجه تمایزیافتگی بافتی و افزایش درگیری غدد لنفاوی ارتباط دارد.

    Background And Objective
    One important step in the management of gastric cancer is determination of its prognosis. Cytokeratins are intermediate filaments that mainly exist in cytoskeletal component of epithelial cells. Recent researches have focused on the relationship between the expression of different cytokeratins with differentiation and outcomes of tumors. This study was done for determination of the relationship between expression of cytokeratin 20 and cytokeratin 7 with prognostic factors in the adenocarcinoma of the stomach.
    Materials And Methods
    The study was a cross-sectional one. In this research, 31 archival samples of gastric adenocarcinoma with different histopathologic differentiation were immunohistochemically examined for analyzing the expression of cytokeratin 20 and cytokeratin 7. The relationship between cytokeratin 20 and cytokeratin 7 expression with prognostic factors including histopathologic differentiation, lymph node involvement and depth of invasion were assessed. Obtained data were analyzed by Chi-square and Spearman statistical tests.
    Results
    A negative correlation between cytokeratin 20 and cytokeratin 7 expressions with histopathologic differentiation and lymph node involvement was observed. There was a significant correlation between cytokeratin 7 expressions and tumor thickness. The correlation between cytokeratin 20 expression and tumor thickness was not significant.
    Conclusion
    Expression of cytokeratin 20 and cytokeratin7 was negatively related to histopathologic differentiation and lymph node involvement.

    Background And Aims
    Immunohistochemical tests are one of the most important tests, which are under study to determine the prognosis of the cancers such as transitional cell carcinoma. By the time, flexible cystoscopy and urine cytology are the routine tests for following up the patients with transitional cell carcinoma, which are both operator dependent. On the other hand, cystoscopy is an invasive method, and urine cytology is a method with low sensitivity. The aim of this study was to determine CK20 in patients with transitional cell carcinoma of bladder and its relation with prognostic factors, which are the stage and the grade of the tumor.
    Materials And Methods
    Our study was done in Mostafa Khomeini Hospital from 2007 to 2010 on the 2001 to 2009 stored information files, included 53 patients diagnosed as transitional cell carcinoma of bladder (TCC) of different stages and grades that were underwent total cystectomy. Immunohistochemical staining was performed on tissue sections with specific CK20 antibody. Then the samples were studied by light microscope so positive and negative cases were identified.
    Results
    According to statistical analysis there were significant reverse relationship between CK20 and stage, and significant reverse relationship between CK20 and grade (P= 0.000).
    Conclusion
    Immunohistochemical study of patients with transitional cell carcinoma of bladder in order to identify CK20 can be a useful method to determine the prognosis of these patients.
