    Introduction

    Kidney transplantation is the treatment of choice in the majority of end-stage renal disease (ESRD) patients. However, most of the incident ESRD patients are not given the necessary information regarding kidney transplantation. The aim of this study was to evaluate awareness and knowledge about kidney transplantation in ESRD patients who were on dialysis.

    Methods

    In this cross-sectional study, a total of 300 ESRD patients who underwent hemodialysis or peritoneal dialysis and could be eligible for kidney transplant, were included. A questionnaire with 15 multiple choice questions (MCQs) was designed to collect the data. SPSS version 16 was used for data analysis and a P value less than .05 was considered statistically significant.

    Results

    Two hundred thirty- four patients participated in the study (response rate of 78%). Among them 58.1% were male with the mean age (SD) of 52.5 (12.1) years. The majority of the patients (94.0%) were on hemodialysis. About 87.6% wanted to receive kidney transplant; but despite the desire of the patients, this method was initially offered to about 11.5 % of the patients as a possible method of renal replacement therapy. Patients who had desire to receive kidney transplantation were significantly younger, male, married, employed, and had high level of education (P < .05).

    Conclusion

    Although most of the patients wanted to receive a kidney transplant, it was initially offered to a small population. Age, gender, marital status, employment condition, and level of education were significant factors for the patient’s tendency to receive a kidney transplant.

    Introduction

    The incidence of ESRD is increasing dramatically and the majority of patients are commenced to hemodialysis (HD) or peritoneal dialysis (PD) due to the long waiting time for renal transplantation. PD has comparable outcomes with HD but many barriers limit its utilization. Obesity is considered among the barriers and this was attributed to its related complications.

    Case Report

    A 50-year-old male patient with ESRD presented to our hospital for PD. He was extremely obese (BMI = 44.2 kg/m2). The case was discussed between the nephrology, surgical and nursing team, and the decision was made to proceed towards PD.

    Conclusion

    Obesity should not impede the beneficial effects of PD. The obstacles of obesity, which we faced; could be overcome with the collaboration between a highly qualified multidisciplinary team.

    Implication for health policy/practice/research/medical education

    In December 2019, an outbreak of coronavirus disease (COVID-19) due to the novel SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) virus began in China and spread rapidly worldwide. After China, Italy is the country with the highest number of cases so far. There are only limited data on COVID-19 in patients affected by chronic kidney disease. Herein, we report the outcomes of two patients with COVID-19 pneumonia who required renal replacement therapy. It is unknown whether hemodialysis patients represent a distinct group of patients with certain characteristics that may make them susceptible to infection or severe disease. In this letter, we describe our clinical experience with COVID-19 patients.

    Introduction: Vitamin D deficiency is a common problem in end-stage renal disease patients under hemodialysis. Both active and nutritional vitamin D supplementation have been recommended for its treatment. In this study we aimed to evaluate the effects of treatment with ergocalciferol on bone metabolism indexes in hemodialysis patients. Materials and Methods: In a randomized controlled trial, 40 hemodialysis patients were randomly allocated to the intervention (n = 20) and placebo (n = 20) groups. During the study, 4 patients in the placebo and 1 in the intervention group were excluded. Patients received calcitriol, 0.25 mg/d, with ergocalciferol, 50 000 IU, or placebo weekly for 3 months. Serum levels of 25-hydroxyvitamin D, calcium, parathyroid hormone, and alkaline phosphatase were measured before and after treatment. Results: 25-hydroxyvitamin D levels were significantly improved in the intervention group (12.00 ± 4.90 ng/mL versus 29.89 ± 9.48 ng/mL, P < .001), but the placebo group had no improvement (14.23 ± 7.62 ng/mL versus 13.87 ± 8.04 ng/mL, P > .05). There was no significant changes in serum calcium, parathyroid hormone, or alkaline phosphatase levels in each group. Eight patients (42.1%) in the intervention compared to zero cases in the placebo group had normal 25-hydroxyvitamin D levels after treatment (P = .004). No cases of hypercalcemia were seen in the studied patients. Conclusions: Treatment with ergocalciferol could significantly improve vitamin D deficiency with no significant effects of serum calcium or parathyroid hormone levels.
