مقدمه
بیماران مبتلا به اسکیزوفرنی مصرف کننده ی داروهای آنتی سایکوتیک، بسیار مستعد اختلالات متابولیک نظیر افزایش وزن، دیس لیپیدمی و مقاومت به انسولین می باشند. هدف از این مطالعه، مقایسه ی اثرات داروهای آنتی سایکوتیک آتییپک و سنتی بر روی سطح چربی خون بود.
روش هادر این مطالعه، 128 بیمار دچار اسکیزوفرنی به دو گروه تقسیم شدند. یک گروه، داروی آنتی سایکوتیک سنتی و دیگری داروی آنتی سایکوتیک آتیپیک دریافت کردند. هیچ کدام از دو گروه، طی سال گذشته هیچ داروی آنتی سایکوتیکی دریافت نکرده بودند. برای هر یک از شرکت کنندگان، اطلاعات دموگرافیک و پرسش نامه ی غذایی تکمیل شد. سطوح پروفایل چربی های خون و آپولیپوپروتئین A و B ار طریق نمونه گیری خون پس از 12 ساعت ناشتا، از ورید آنته کوبیتال، اندازه گیری شد. آزمون Student-t برای مقایسه ی دو گروه مورد استفاده قرار گرفت.
یافته هادر دو گروه، تفاوت معنی داری از نظر سن، جنس، مدت زمان بیماری و مصرف دارو و سن در زمان شروع بیماری وجود نداشت. بیماران گروه آتیپیک، از داروهای کلوزاپین و رسپریدون (9/46 درصد) بیشتر از الانزاپین استفاده کردند. در حالی که در گروه سنتی، 3/81 درصد از بیماران داروهای فتوتیازین مصرف کردند. مقایسه ی سطح چربی خون در دو گروه نشان داد که کلسترول تام (01/0 = P)، لیپوپروتئین با چگالی کم (Low-density lipoprotein یا LDL) (03/0 = P) و آپولیپوپروتئین B (01/0 > P) در گروه سنتی به طور واضحی بالاتر از گروه آتیپیک بود.
نتیجه گیریدر هر دو گروه مصرف کننده ی داروی آتیپیک و سنتی، و به خصوص در گروه سنتی، سطوح چربی خون بالا رفته بود. بنابراین، اثرات داروهای آنتی سایکوتیک روی چربی خون باید به صورت دوره ای (متناوب) چک شود.

Methods128 patients with schizophrenia were divided in two groups. One of the groups received one type of atypical antipsychotic drugs while the other got one type of conventional ones. None of the groups had used any other antipsychotic drugs during the past year. Demographic data and food frequency questionnaire were completed by each participant. Serum triglyceride، total cholesterol (TC)، high-density lipoprotein (HDL) and low-density lipoprotein (LDL) cholesterols، and apolipoprotein A and B (Apo A and B) were recorded. Student’s t test was used for statistical analysis.
FindingsThe two groups were similar in age، gender، duration of illness، period of drug consumption، and age at onset of illness. Patients in the atypical group used clozapine and risperidone (46. 9%) more than olanzapine; while in the conventional group، 81. 3% of patients used phenothiazines. Our data showed a significantly higher mean in TC (P = 0. 01)، LDL (P = 0. 03)، and Apo B (P = 0. 01) in conventional group.
ConclusionIn patients with schizophrenia، lipid profile was higher in both atypical and conventional antipsychotic users، especially in conventional drug users.

Student’s t-test was used to compare atypical and conventional groups.ResultsThere was no significant difference in age, gender, duration of illness, period of drug consumption, and age at onset of illness in the two groups. Patients in the atypical group used clozapine and risperidone (46.9%) more than olanzapine. In the conventional group 81.3% of patients used phenothiazines. Comparison between lipid profile in the conventional and atypical groups showed a significantly higher mean in TC (P = 0.01), LDL (P = 0.03), and Apo B (P = 0.01) in conventional group than the atypical group.ConclusionIn schizophrenic patients, the level of lipid profile had been increased in both atypical and conventional antipsychotic users, especially conventional users, so the effect of antipsychotic drugs should be investigated periodically.Keywords: Atypical Antipsychotic, Conventional Antipsychotic, Lipid Profile}

مقدمه
داروهای ضد سایکوزیس اساس درمان اسکیزوفرنی را تشکیل می دهند. درمان با این داروها با اختلال در کنترل قند خون همراه است.گزارشاتی مبنی بر افزایش وزن و اختلال در تحمل گلوکز به دنبال درمان با داروهای ضد سایکوزیس غیر تیپیک ارائه شده است. هدف از این مقایسه تاثیر درمانهای مختلف ضد دیابت بر کنترل قند خون در بیماران مبتلا به اسکیزوفرنی است.
روش کاراین مطالعه کارآزمایی بالینی بر 180 بیمار در بیمارستان ابن سینا مشهد درسال 1387 انجام شد. بیماران در دو گروه قرار گرفتند،گروه اول تحت درمان با داروهای ضد سایکوزیس تیپیک (تعداد=43)و گروه دوم تحت درمان با داروهای ضد سایکوز غیر تیپیک (تعداد=38). اطلاعات به دست آمده با استفاده از نرم افزار SPSS و آزمون تی و آنوا مقایسه و تجزیه و تحلیل شد.
نتایجتفاوت معنی دار مرزی در قند ساعت اول بین دو گروه تحت درمان با داروهای تیپیک و داروهای غیر تیپیک وجود داشت (به ترتیب متوسط12/132 میلیگرم بر دسی لیتر در مقابل 76/151میلیگرم بر دسی لیتر، 07/0=p). تفاوت معنی دار مرزی در مقایسه قند ساعت اول در تست تحمل گلوکز بین دو گروه تحت درمان با داروهای تیپیک و تحت درمان با کلوزاپین دیده شد(به ترتیب متوسط قند خون 12/132، 67/153و06/0=p). در مقایسه داروهای ضد سایکوزیس تیپیک با ریسپریدون این اختلاف وجود نداشت (به ترتیب متوسط 12/132 در مقابل 41/149و23/0=p). مقادیر قند خون ناشتا و قند خون ساعت دوم در این دو گروه تفاوت معنی دار نداشت.
نتیجه گیریبیماران مبتلا به اسکیزوفرنی که تحت درمان با داروهای ضد سایکوزیس غیر تیپیک هستند بیشتر مستعد دیابت شیرین می باشند. همچنین این خطر درافرادی که کلوزاپین مصرف می کنند در مقایسه با ریسپریدون بیشتر است.

Glucoregulatory abnormalities have been associated with the use of antipsychotic medications. There are some reports of weight gain and glucose intolerance in patients with schizophrenia receiving atypical antipsychotic medications. The purpose of this study is to compare the effect of various antipsychotic therapies on glycemic control in patients with schizophrenia.
Materials And MethodsEighty-one patients with schizophrenia were enrolled in the study and underwent glucose-tolerance test. Patients were divided in to two groups، with one group (n=43) receiving typical and the other group (n=38) receiving atypical medication. Demographical and historical data were collected with a questionnaire.
ResultsThere was a marginally significant difference in the first hour glucose level between typical and atypical drugs (mean 132. 12 vs. 151. 76، respectively، p=0. 07). Also، marginal significant difference was present comparing the first hour glucose level between typical antipsychotics and clozapine consumers (mean 132. 12 vs. 153. 67، p=0. 06). This difference was not present when typical antipsychotics were compared with risperidone (mean 132. 12 vs. 149. 41، p=0. 230). Fasting blood sugar and second- hour glucose tolerance results did not show a statistically significant difference. The increased GTT of the first hour blood sugar in patients who received clozapin may indicate the early stages of insulin resistance in this group.
ConclusionThis study suggests that patients under atypical antipsychotic treatment are more vulnerable to diabetes mellitus (DM). Across atypical medications; we observed a greater glycemic effect from clozapin compared to risperidone.
