زمینه و هدف
یکی از بیماری های مهم جلدی انسان و حیوانات، درماتوفیتوزیس است که مقاومت آن به درمان های رایج رو به افزایش است. این مطالعه به منظور تعیین میزان فعالیت برون تنی ضدقارچی عصاره های اترنفت، دی کلرومتان، اتیل استات، اتانول و هیدرواتانول اندام هوایی درمنه های خراسانی (Artemisia khorassanica)، شرقی (Artemisia scopari) و معمولی (Artemisia vulgaris) علیه قارچ های شایع عامل بیماری درماتوفیتوزیس شامل تریکوفیتون روبروم (Trichophyton rubrum)، تریکوفیتون وروکوزوم (Trichophyton verrucosum)، اپیدرموفیتون فلوکوزوم (Epidermophyton floccosum) و میکروسپوروم کانیس (Microsporum canis) انجام شد.
روش بررسیاین مطالعه توصیفی روی عصاره های مختلف اندام هوایی درمنه های خراسانی، شرقی و معمولی علیه جدایه های قارچی Trichophyton rubrum، Trichophyton verrucosum، Microsporum canis و Epidermophyton floccosum انجام شد. اندام های هوایی سه گونه درمنه خراسانی، درمنه شرقی و درمنه معمولی با استفاده از پنج حلال اتر نفت، دی کلرومتان، اتیل استات، اتانول و هیدرواتانول (50%) عصاره گیری و عصاره های حاصل برای اثرات ضددرماتوفیتی علیه Trichophyton rubrum، Trichophyton verrucosum، Microsporum canis و Epidermophyton floccosum غربالگری شدند. سپس آزمایش حداقل غلظت بازدارندگی (MIC) براساس روش رقیق سازی در آگار انجام شد. فعال ترین عصاره ها در آزمون های مقدماتی فیتوشیمیایی مورد بررسی قرار گرفتند.
یافته هادر غربالگری اولیه، میکروسپوروم کانیس و تریکوفیتون وروکوزوم به ترتیب حساسیت (86.66%) و مقاومت (100%) بیشتری را به عصاره های مورد مطالعه نشان دادند. عصاره های حاصل از درمنه شرقی، از گسترده ترین دامنه فعالیت برخوردار بودند. موثرترین عصاره های به کار رفته در آزمایش ها نیز با MIC برابر با 78.12 میکروگرم بر میلی لیتر از همین گیاه بودند. عصاره های هیدرواتانولی تمام گونه های گیاهی کمترین فعالیت ضددرماتوفیتی را بروز دادند. نتایج مطالعات مقدماتی فیتوشیمیایی، حضور مشترک ترپنوییدها را در تمام عصاره های اتر نفتی و دی کلرومتانی گونه های گیاهی نشان داد.
نتیجه گیریبرخی ترکیبات چربی دوست موجود در عصاره های مختلف به ویژه عصاره های اتر نفتی و دی کلرومتانی درمنه شرقی فعالیت قابل توجه برون تنی ضددرماتوفیتی دارد.

This study aimed to determine the antifungal activity of petroleum ether, dichloromethane, ethyl acetate, ethanol, and hydroethanol extracts from the aerial parts of Artemisia khorassanica, Artemisia scoparia, and Artemisia vulgaris against common dermatophytic fungi, including Trichophyton rubrum, Trichophyton verrucosum, Epidermophyton floccosum, and Microsporum canis.
MethodsThis descriptive study examined the antifungal activity of various extracts from the aerial parts of Artemisia khorassanica, Artemisia scoparia, and Artemisia vulgaris against fungal isolates of Trichophyton rubrum, Trichophyton verrucosum, Microsporum canis, and Epidermophyton floccosum. The aerial parts of the three Artemisia species were extracted using five solvents: petroleum ether, dichloromethane, ethyl acetate, ethanol, and 50% hydroethanol. The extracts were screened for antidermatophytic effects against the fungi mentioned above, and the minimum inhibitory concentration (MIC) was determined using the agar dilution method. The most active extracts underwent preliminary phytochemical testing.
ResultsIn the initial screening, Microsporum canis and Trichophyton verrucosum showed the highest sensitivity (86.66%) and resistance (100%) to the tested extracts, respectively. Extracts from Artemisia scoparia exhibited the broadest range of activity. The most effective extracts, with an MIC of 78.12 µg/ml, were also from this plant. Hydroethanol extracts from all plant species demonstrated the least antidermatophytic activity. Preliminary phytochemical studies revealed the presence of terpenoids in all petroleum ether and dichloromethane extracts of the plant species.
ConclusionSome lipophilic compounds, particularly in the petroleum ether and dichloromethane extracts of Artemisia scoparia, exhibit significant in vitro antidermatophytic activity.

There are many fungal agents that can infect and clean the skin. The aim of this study was to observe the antifungal effect of Carvacrol and 1.8 cineole on fungi. In this descriptive-cross-sectional study, Carvacrol and 1.8 cineol essential oils were supplied by the microbial collection of the Scientific and inisitue Research Organization of Iran. The fungal strains Candida albicans, Aspergillus flavus, Trichophyton rubrum, and Epidermophyton flucosome were used in this study. Agar diffusion and macrodilution broth were used. Statistical tests were used to investigate the antimicrobial effect of Carvacrol essential oil and 1.8 cineole. Combination effect of carvacrol and 1,8 cineol against fungal species by disc diffusion method showed that these compounds had the greatest effect on Candida and carvacrol was a more effective than 1,8 cineol. A synergistic study of carvacrol with fluconazole, ketoconazole, and terbinafine showed stronger effects against fungal species. The amount of MIC and MFC obtained from carvacrol against Candida albicans, Aspergillus flavus, Trichophyton rubrum and Epidermophyton flucosome was 250, 125 – 62/5 - 125 and 62/5 and 125,62/5 ppm and this amount for 1,8 cineole, respectively, is 15, 7/8 - 15, and 7/8 -31, 7/8 - 31 and 7/8 ppm. In order to the side effects and high toxicity of common antifungals, it is recommended to use herbal compounds in combination with these antifungals and by conducting clinical studies, toxic effects, pharmacological and its side effects should also be considered.

Background and Objectives
Monitoring of contagious diseases is important to advance our knowledge of their epidemiology and to enable more impressive investigation and prevention efforts. This study aimed to examine antifungal drug susceptibility and molecular analysis of clinical isolates of Trichophyton rubrum and Trichophyton mentagrophytes in humans and cattle.
Materials and MethodsA total of 400 patients and 500 cattle were evaluated in this study. Dermatophytosis was confirmed in cases by direct microscopy and culture methods. Antifungal drug susceptibility profiles, MIC50, and MIC90 of isolates were determined using the broth microdilution method. Multiplex-PCR, RAPD PCR, and sequencing methods were used for the genetic analysis of virulence genes and the ITS1 and ITS2 regions, respectively.
ResultsA total of 175 patients and 120 cattle were diagnosed with dermatophytosis. Dermatophytes showed a remarkable rate (30%) of terbinafine resistance. T. mentagrophytes showed lower susceptibility than T. rubrum (MIC50=16 μg/mL). Strains harboring Mep1, Mep2, and Mep4 genes had the highest frequency among all genotypes. A RAPD-PCR dendrogram divided T. mentagrophytes and T. rubrum strains into three and six groups, respectively.
ConclusionA notable rate of resistance to terbinafine in isolated dermatophytes was reported in this study. Examination of RAPD-PCR results showed that T. rubrum strains had higher genetic diversity than T. mentagrophytes. Genetic monitoring of dermatophytes must be considered an important factor in providing fungal infection prevention and treatment approaches.

The Fungalysin gene is an important factor in the incidence of dermatophytosis disease. The aim of this study was to investigate the presence of mep1 and mep2-generating Fungalysin genes using the multiplex PCR method. 150 scabs and nails were sampled from patients with symptoms of dermatophytosis. Samples containing Trichophyton rubrum were identified and isolated using a red pigment production test in DTM medium, without urea production, without hair piercing and sorbitol adsorption. Culture slide was performed to identify the microscopic structure. Then DNA extraction was performed and several PCR experiments were performed in these strains using specific primers of Fungalysin gene. Out of 150 samples isolated from patients, 30 Trichophyton strains were isolated based on morphological,biochemical and microbial tests. Among these, 20 strains had mep2 gene and 1 strain had mep1 gene. Results shows that the frequency of mep2 genes is higher than mep1 and this abundance is involved in causing more pathogenicity of Trichophyton rubrum and is one of the major causes of treatment-resistant infections in medical centers. Now, with the help of the Multiplex PCR method, it is possible to detect the presence of pathogenic genes in the shortest time with high characteristics and sensitivity, and with appropriate and timely treatment, prevented the function of treatment resistance strains, recurrence of the disease and prolongation of the treatment period.Keywords: Trichophyton rubrum, Dermatophytosis, Fungalysin, mep2, mep1}

Background
Nail abnormalities comprise a significant proportion of dermatological disorders. Nail is a window that provides vital clues for any underlying, dermatological, and systemic diseases. Any cutaneous assessment is incomplete without examining nails despite which it is a fairly neglected appendage.
ObjectivesThis study was carried out to document the clinical pattern, determine the etiology, and identify risk factors for various nail changes.
MethodsThis cross-sectional observational study was conducted on 200 consecutive patients presenting with nail changes to the outpatient department of dermatology and venereal diseases of a tertiary care hospital. The patients were enrolled after obtaining informed consent. A case record form was designed for every patient, including demographic data, nail characteristics, associated dermatological and systemic disease, and investigation details.
ResultsThe mean age of the patients was 37.98 ± 16.79 years with a male preponderance. Occupation (housewife and manual laborer) was observed to be a significant risk factor (P < 0.001) for onychomycosis. The chief complaint related to nails was cosmetic in 49.5% of the cases. Etiology-wise nail changes secondary to dermatological diseases (44%) were the most common, followed by infections (36%); however, among dermatological diseases, psoriasis (42.2%) followed by lichen planus (16.6%) predominated. Although nail pitting was noticed in various conditions, there was a significant association between nail pitting and psoriasis (P < 0.001). Onychomycosis showed a significant association (P < 0.001) with nail discoloration and onycholysis. Distal lateral subungual onychomycosis (61.4%) was the most frequently observed pattern; nevertheless, the most common causative agent was dermatophyte Trichophyton rubrum (58.26%). A nail biopsy was performed on 15 cases, with conclusive findings in 12 patients.
ConclusionsNails should be considered an integral part of the complete dermatological evaluation. The meticulous examination of nails facilitates the early diagnosis of hitherto undetected cutaneous/systemic disease. The nail unit deserves greater attention for better understanding and utilization as a diagnostic tool.

مقدمه
درماتوفیتوزیس عارضه قارچی بافت های کراتینیزه پوست، مو و ناخن است که درنتیجه استقرار درماتوفیت در نسوج روی می دهد. اقدام به تایپینگ و افتراق زیرگونه ها و مطالعه ارتباط احتمالی گونه های خاص با اشکال بالینی جدایه تریکوفیتون، با استفاده از روش RAPD-PCR، با کمک پرایمرهای تصادفی است که ابزار سودمندی برای تیپ بندی و تمایز گونه های قارچی به شمار می رود. هدف از این پژوهش، بررسی پلی مورفیسم ژنومی تریکوفیتون روبروم جداشده از منابع کراتینیزه با روش RAPD_PCR است.
مواد و روش ها:
در این مطالعه، 60 نمونه درماتوفیتوزیس از بیماران پوستی در سنین 12 تا 45 سال، از بیمارستان خاتم الانبیا استان تهران و بیمارستان قدس استان مرکزی جداسازی شدند. نمونه ها در محیط سابورو دکستروز آگار یا محیط میکروبیوتیک آگار تلقیح گردیدند. درمجموع، 30 نمونه مثبت (+) به دست آمد. برای استخراج DNA تریکوفیتون روبروم، از کیت اختصاصی استفاده شد. آزمون PCR با استفاده از پرایمرهای اختصاصی صورت گرفت و از روش RAPD_ PCRبرای بررسی ژنومی آن ها استفاده گردید. بررسی پلی مورفیسم ژنوم، شاخص ژنتیکی ارزشمندی در ارزیابی ساختار ژنوم تریکوفیتون روبروم است.
یافته های پژوهش:
از غربالگری نمونه های بالینی آزمایشگاه که بر اساس ویژگی های مورفولوژیک، میکروسکوپی و تست های بیوشیمیایی تعیین هویت شدند، درمجموع 30 جدایه تریکوفیتون روبروم جدا گردید. این سویه ها در 9 گروه مجزا دسته بندی شد که هرکدام از سویه ها صفات فنوتیپی و ژنوتیپی مشترک دارند. در میان کلاسترهای مشخص شده، بیشترین فراوانی مربوط به کلاستر 9 است که 12 ایزوله با ویژگی های خویشاوندی مشترک را در برمی گیرد.
بحث و نتیجه گیری:
بررسی پلی مورفیسم ژنومی تریکوفیتون روبروم جداشده از نمونه های درماتوفیتی افراد مبتلا به درماتوفیتوزیس در منطقه جغرافیایی تهران، با تکنیک RAPD_ PCR ، به وسیله پرایمر اختصاصی پرداخته شد که در این مطالعه، بعد مسافت و متغیر گروه سنی تاثیر بسزایی داشتند. تنوع ژنتیکی می تواند در انتقال و بیماری زایی تریکوفیتون روبروم نقش ویژه ای داشته باشد.

Attempts for tapping and discriminating the subtypes, as well as studying the possible association of specific species with clinical forms of trichophyton isolates using RAPD-PCR with random primers is a useful tool for typing and differentiating the fungal species. This study aimed to investigate the genomic polymorphism of Trichophyton rubrum isolated from keratinized sources using the RAPD-PCR method.
Materials & MethodsIn this study, 60 dermatophytosis specimens were isolated from dermatophilic patients aged 12 to 45 years referred to Khatam-al-Anbia Hospital in Tehran province and Qods Hospital in Markazi province, Iran. The samples were inoculated in Sabourodextrose agar or microbial agar medium. A total of 30 positive samples were obtained, and specific kits were used to extract trichophyton rubrum DNA. The PCR was performed using specific primers, and the RAPD-PCR method was employed for genomic analysis. Investigation of genomic polymorphism is a valuable genetic marker in the evaluation of genome structure of Trichophyton rubrum.
FindingsA total of 30 isolates of Trichophyton rubrum were isolated after screening the clinical specimens that were identified based on morphological, microscopic, and biochemical tests. These strains were divided into nine genomic groups, some of which were found in several groups, indicating the genotypic similarity of the fungi of the same species at different locations. Among the identified clusters, cluster nine obtained the highest frequency, which included 12 isolates with common kinship characteristics.
Discussions & ConclusionsThis study investigated the genomic polymorphism of Trichophyton rubrum isolated from dermatophytes specimens of patients with dermatophytosis in Tehran, Iran, using the RAPD-PCR technique with a primer. According to the results, distance and age group had a significant effect. Furthermore, genetic diversity can play a special role in the transmission and pathogenicity of this microorganism.

خانواده ABC ترانسپورترها در قارچ ها علاوه بر ایفای نقش در تقسیم سلولی و نگهداری حجم سلول در خروج متابولیت های طبیعی و مقاومت دارویی، نیز نقش مهمی دارد. هدف از انجام این تحقیق جداسازی مولکولی و همسانه سازی ژن انتقالی پس زننده (Efflux) کاست ژنومی (ABC) تریکوفایتون روبروم خواهد بود.
روش بررسیدر این مطالعه 60 نمونه درماتوفیتوزیس از بیماران جداسازی و با روش های میکروسکوپی ، شیمیایی و مولکولی تایید گردیدند. پس از تایید تریکوفیتون در مرحله بعدی آزمون PCR با استفاده از پرایمرهای اختصاصی مربوط به کاست ژنی ABC صورت گرفت. قطعه ژنی ABC در وکتور PTG19 کلون و تایید گردید. بیان کاست ژنومیABC با روش Real-Time PCR سنجیده شد.
یافته هااز 60 نمونه جداسازی شده در مجموع 12 ایزوله تریکوفیتون روبروم جداسازی شد. از 12سویه تریکوفایتون جدا شده، 6 سویه حامل ژن ABC بودند.توسط آزمون PCR Colony حضور ژن مورد نظر تایید و در نهایت بیان و تکثیر ژن ABC کلون شده در باکتری اشریشیاکلی از طریق Real time PCR تایید شد.
نتیجه گیرییکی از پایدار ترین راه های مقاومت به آنتی بیوتیک ها در میکروارگانیسم بیان افلوکس پمپ ها می باشد. در این راستا محققان ترکیبات مهارکننده افلوکس پمپ ها را تولید کردند و از ترکیب آن ها با آنتی بیوتیک ها در درمان عفونت ها استفاده می کنند؛ اما به دلیل سمیت اکثر این ترکیبات در انسان استفاده گسترده از آن ها ممنوع می باشد از این رو چالش جدید،یافتن ترکیبات مهارکننده جدید با سمیت کمتر است .

The purpose of this study is to investigate the molecular separation and cloning of the ATP-binding Cassette (ABC) from Trichophyton rubrum.
MethodsSixty samples of dermatophytosis were isolated from the patients. Isolates were identified by chemical and molecular tests. Afterward, PCR was performed using specific primers for the ABC gene. PCR product was cloned in the PTG19 vector. Real-Time PCR was performed to evaluate the expression level of ABC gene.
ResultsA total of 12 isolates of Trichophyton Rubrum were identified. Six isolates were positive for ABC gene. The expression of the ABC gene cloned in E. coli XL1blue was confirmed by Real time PCR.
ConclusionExpression of efflux pumps considered a way of resistance to antibiotics in microorganisms. In this regard, the researchers have developed the efflux pump inhibitor compounds and used them in combination with antibiotics to combat infections. However, because of the toxicity of most of these compounds in humans, their extensive use is prohibited. Therefore, finding new inhibitory compounds with little or no toxicity is challenging.

زمینه و هدفخانواده ژنومی سوبتیلیزین (Subtilisins; SUBs) ترایکوفایتون روبروم، نقش مهمی در تخریب بافت کراتین در جهت تولید یک منبع مغذی و حدت زایی، ایفاء می کند. این مطالعه با هدف تعیین ژن های سوبتیلیزین (7-1) در گونه ترایکوفایتون روبروم جدا شده از بیماران مبتلا به درماتوفیتوزیس ساکن شهر تهران در سال 1396 انجام شده است.مواد و روش هامطالعه توصیفی حاضر، بر روی 32 بیمار مبتلا به عفونت قارچی مراجعه کننده به دانشگاه علوم پزشکی تهران از تیر ماه تا شهریور ماه سال1396 انجام گرفت. بر اساس مشاهدات میکروسکوپی و ماکروسکوپی، گونه ترایکوفایتون روبروم جداسازی و شناسایی شد. DNA درماتوفیت استخراج و سپس ژن های SUB1، SUB2،SUB3 ، SUB4، SUB5، SUB6 و SUB7 با روش مولکولی واکنش زنجیره ای پلیمراز (PCR) با پرایمرهای اختصاصی تکثیر گردیدند. درصد فراوانی نسبی هر یک از هفت توالی ژنومی نیز محاسبه گردید.یافته ها7 گونه ترایکوفایتون روبروم از نمونه بالینی پوست (5 مورد) و ناخن (2 مورد) جداسازی شدند. خانواده سوبتیلیزین ((SUB1-7با فراوانی 57 درصد گزارش شد. در حالی که ژن SUB5 (100 درصد) در تمام ایزوله ها مشاهده شد، ژن SUB6 با کم ترین فراوانی (4 از 7 مورد) و حضور هفت ژن سوبتیلیزین در گونه های جدا شده از بافت ناخن، گزارش شد.نتیجه گیریحضور و عدم حضور ژن های سوبتیلیزین در DNA ایزوله های ترایکوفایتون روبروم، احتمالا حاکی از اهمیت هر یک از ژن ها در مراحل مختلف عفونت (اتصال، تهاجم و التهاب) درماتوفیت و نوع بافت ناخن و پوست می باشد.کلید واژگان: ژن های سوبتیلیزین, درماتوفیتوزیس, ترایکوفایتون روبروم, پوست, ناخن, تهران}Background and ObjectivesThe subtilisin genes (SUBs) of Trichophyton rubrum play an important role in destroying the keratin to produce a nutritional source and virulence. This study was carried out to determine the presence of subtilisin genes in T. rubrum isolated from patients with dermatophytosis in Tehran in 2017.Materials and MethodsThis descriptive study was performed on 32 patients with fungal infections referring to Tehran University of Medical Sciences from July to September 2017. Based on microscopic and macroscopic observations, T. rubrum was isolated and identified. Dermatophyte DNA was extracted and SUB1, SUB2, SUB3, SUB4, SUB5, SUB6 and SUB7 genes of T. rubrum were amplified by polymerase chain reaction (PCR) using specific primers. The relative frequency of the seven genomic sequences was also calculated.ResultsSeven species of T. rubrum were isolated from the clinical specimen of skin (5 cases) and nail (2 cases). The presence of subtilisin family (SUB1-7) was reported with a frequency of 57%. While SUB5 gene was observed in all isolates (100%), the SUB6 gene with the lowest frequency (4 out of 7 cases) and SUB1-7 genes presence in isolates from nail samples were reported.ConclusionThe presence and absence of subtilisin genes in T. rubrum DNA may indicate the importance of genes in different stages of dermatophyte infection (adherence, invasion and inflammation) and the type of nail and skin structure.Keywords: Subtilisin genes, Dermatophytosis, Trichophyton rubrum, Skin, Nail, Tehran}

BackgroundSkin infections with dermatophytes (dermatophytosis) are common human fungal infections, the most common cause of which is Trichophyton rubrum. Antimicrobial peptides (AMPs), which have potential anti-microbial effects, are affected by epidermal growth factor receptor (EGFR) gene. Elevated expression of this gene in skin cells activates AMPs and prevents cloning of dermatophytes in keratinocytes. However, mRNA of EGFR gene is muted with increased expression of microRNAs (miRNAs), in particular miR-212. Therefore, EGFR inhibition may have a negative impact on AMP localization in dermatophyte-infected keratinocytes.ObjectivesThis study aimed to determine the changes in the expression of miR-212 and EGFR genes in cutaneous tissues affected by T. rubrum compared to their healthy margins.MethodsThe number of samples in this study was estimated to be 72. The fungus was cultivated on Sabouraud Dextrose Agar medium. Isolation and optimization of total RNA and synthesis and optimization of cDNA for the EGFR and miR-212 genes were performed. Amplification of these target genes was performed using real-time polymerase chain reaction (RT-PCR). In data aggregation and analysis, changes in expression of the target genes were calculated via 2-ΔΔCt ratio. P value was considered < 0.05.ResultsIn samples infected with T. rubrum, miR-212 significantly reduced the expression of EGFR gene, and in these samples, the expression of miR-212 gene was eight times higher compared to the expression of the EGFR gene. In control samples, the expression of miR-212 was much lower than that of the EGFR gene.ConclusionsBy enhancing the expression of miR-212 in this study, the function of EGFR gene mRNA was turned off, leading to the reduction of AMPs, and in turn, the colonization of T. rubrum and creation of dermatophytosis on the skin.Keywords: Dermatophytosis, Trichophyton rubrum, AMP, EGFR, miR-212}

BackgroundDue to long-term treatment of dermatophytic lesions, such as tinea unguium and using antifungal drugs, which cause side effects, it is essential to investigate new antifungal drugs with greater absorption and fewer side effects, such as nano-drugs. Trichophyton rubrum and Microsporum canis are important dermatophyte ýspecies. To investigate new strategy treatment, researchers have tried to find a new drug with extensive therapeutic effects and short-duration treatment. In this context, nano-drugs have gained great interest in improving the efficacy of antifungal properties compared to commercial drugs.ObjectivesThe aim of this study was to compare the effects of free terbinafine and nano-liposomal terbinafine on growth of clinical isolates of T. rubrum and M. canis.MethodsIn this study, 120 isolates of T. rubrum and M. canis were separated from dermatophytosis lesions. Diagnosis of the strains was based on the morphological structure and molecular identification. Nano-liposomal terbinafine was prepared and analyzed. Antifungal susceptibility testing was performed by broth micro dilution CLSI M 38-A method to compare inhibitory growth effects of nano-liposomal characteristics of terbinafine and free terbinafine.ResultsThe results showed that the Minimum Inhibitory Concentration (MIC) values of terbinafine against T. rubrum and M. canis strains were 0.0625 to 1 μ/mL and 0.0313 to 0.5 μ/mL, respectively. Also, the MIC values of nano-liposomal terbinafine against T. rubrum and M. canis were 0.0156 to 0.25 μ/mL and 0.0078 to 0.125 μ/mL, respectively.ConclusionsComparison of the antifungal effects of nano-liposomal terbinafine and free terbinafine against T. rubrum and M. canis isolates showed that the nanopartilces of terbinafine had a greater antifungal activity against the studied isolates and can be used as an alternative agent for dermatophytosis treatments.Keywords: Antifungal Activity, Nanoparticles, Terbinafine, Microsporum canis, Trichophyton rubrum}

زمینه و هدفترایکوفایتون روبروم معمول ترین پاتوژن مولد بیماری درماتوفیتوزیس است. فلوکونازول دارویی از خانواده آزول ها است که جهت درمان تجویز می شود. اخیرا روش های تایپینگ مولکولی برای پاسخ دادن به سوالات اپیدمیولوژیکی و مشکلات عود مجدد بیماری گسترش زیادی یافته اند. این مطالعه روی 22 ایزوله ترایکوفایتون روبروم که به طور اتفاقی از بیماران مبتلا جدا شده بود، انجام گرفت. هدف مطالعه بررسی رابطه الگوی ژنتیکی و حساسیت به داروی فلوکونازول در ایزوله های کلینیکی ترایکوفایتون روبروم بود.روش کارابتدا جنس و گونه قارچ های جداشده از بیماران با روش های ماکروسکوپی و میکروسکوپی تایید گردید. سپس با استفاده از محیط کشت های حاوی مقدار معین دارو مقاومت و حساسیت ایزوله ها نسبت به دارو تعیین شد. در مرحله بعد DNA قارچ ها استخراج گردید و RAPD-PCR (تکثیر تصادفی قطعات پلی مرفیک) با استفاده از 3 پرایمر با توالی تصادفی انجام گرفت.یافته هاهر پرایمر الگوی تکثیریافته متفاوتی را ایجاد کرد و با هر 3 پرایمر تفاوت هایی در الگوی ژنتیکی نمونه های حساس و مقاوم دیده شد ولی هیچ باندی که 100% اختصاصیت را نشان دهد، یافت نشد.نتیجه گیری12 ایزوله حساس که در غلظت 50 میکروگرم در میلی لیتر دارو رشد نکردند، در غلظت های پایینتر دارو نیز رشد آنها به میزان چشمگیری در مقایسه با پلیت شاهد محدود شد. 10 ایزوله مقاوم که در غلظت 50 میکروگرم در میلی لیتر دارو رشد کردند، در غلظت های پایین تر دارو نیز از خود مقاومت نشان دادند. تفاوت هایی در الگوی ژنتیکی نمونه های حساس و مقاوم وجود دارد. آنالیز RAPD جهت تایپینگ مولکولی ایزوله های ترایکوفایتون روبروم کاملا مناسب نشان داد.

One of the drugs prescribed for fungal infections is fluconazole which belongs to Azoles group of antifungal agents. Recently molecular typing methods have been developed for answering the epidemiological questions and disease recurrence problems. Current study has been conducted on 22 isolates of Trichophyton rubrum obtained from patients randomly. Our aim was the investigation of correlation between genetic pattern and sensitivity to Fluconazole in clinical isolates of Trichophyton rubrum.MethodsFirstly the genus and species of isolated fungi from patients have been confirmed by macroscopic and microscopic methods. Then, the resistance and sensitivity of isolates against drug have been determined using culture medium containing defined amount of drug. In next step fungal DNA has been extracted by RAPD-PCR (random amplified polymorphic DNA) with random sequences of 3 primers.ResultsEach primer produced different amplified pattern, and differences have been observed in genetic pattern of resistant and sensitive samples using each 3 primers, but there was no bond with 100% specificity.ConclusionThe 12 sensitive isolates which didn’t grow in 50µg/ml concentration of drug, also had limited growth at the lower concentration of drug. Ten resistant isolates which grew in 50µg/ml of drug, also showed resistant to lower concentration of drug. There are differences in genetic pattern of resistant and sensitive samples. Objective(s)Trichophyton rubrum (T. rubrum) is one of the most common dermatophytes worldwide. This fungus invaded skin appendages of humans and animals. Recently, resistance to antifungal drugs as well as appearance of side effects due to indication of these kinds of antibiotics has been reported. Besides, using some plant extracts have been indicated in herbal medicine as an alternative treatment of these fungal infections. The aim of this study was to investigate the effects of Garlic (Allium sativum) and pure allicin on the growth of hypha in T. rubrum using Electron miscroscopy.Materials And MethodsThis study was carried out to observe the morphological changes of T. rubrum treated with allicin as well as aqueous garlic extract using scanning electron microscopy (SEM) and transmission electron microscopy (TEM).ResultsSEM surveys, showed that hypha treated with allicin has rough and granular like surface, abnormal and irregularly-shape. However, hypha treated with garlic extract had rough and fluffy surface and also irregularly-shape. TEM studies also found that hypha treated with allicin displays disintegration of cytoplasm, breaking down in cell membrane and the cell wall, and collapsing of hypha, meanwhile hypha treated with garlic extract exhibiting degradation and dissolution of cytoplasm components, demolition of cell wall and cell membrane, and hypha appeared to break.ConclusionThe present study revealed that pure allicin (6.25 µg/ml and 12.5 µg/ml) is more efficient in inhibition of the growth in hyphal cells compare to the garlic extract (2 mg/ml and 4 mg/ml) and they could be used as alternatives in treatment of dermatophytosis.Keywords: Allicin, Dermatophytosis, Electron microscopy, Garlic (allium sativum) extract, Trichophyton rubrum}

هدفدرماتوفیتوزیس یکی از رایج ترین بیماری های عفونی قارچی است که در حال حاضر به صورت جهان گیر درآمده و از مشکلات عمده بهداشتی و درمانی در شهرها و روستاها به حساب می آید. اهداف مشخص این مطالعه شامل اهداف آرمانی، کلی، اهداف ویژه و کاربردی است که عبارتست از تشخیص صحیح درماتوفیتوزیس ناخن، مقایسه حساسیت و دقت PCR با روش های معمول مستقیم و کشت برای شناسایی عوامل درماتوفیتوزیس ناخن و بررسی ارزیابی تریکوفیتون روبروم در بیماران مشکوک به درماتوفیتوزیس ناخن.
مواد و روش هااین مطالعه یک بررسی توصیفی- تجربی بوده که روی نمونه بالینی ناخن 71 نفر از بیماران مشکوک به درماتوفیتوزیس ناخن صورت گرفت. نمونه های بالینی یا تراشه های ناخن مشکوک، به سه بخش تقسیم شد که روی هر قسمت آزمایش مستقیم، کشت و مولکولی انجام شد. در آزمایش مولکولی از آغازگرهای یونیورسال قارچ (ITS1 و ITS4) و اختصاصی ترایکوفایتون روبروم (T-rub) استفاده شد.
نتایجدر مطالعه حاضر برای اولین بار در ایران در ظرف کمتر از 5 ساعت و با استفاده از برنامه و روش اصلاح شده، استخراج DNA از بافت مستقیم ناخن آلوده صورت گرفت. همچنین مقایسه نتایج روش های آزمایشگاهی معمول مثل آزمایش مستقیم و کشت با روش PCR حساسیت و دقت بسیار زیاد آن را نسبت به روش های یاد شده به اثبات می رساند.
نتیجه گیریاین روش می تواند با سرعت و دقتی که دارد جایگزین بسیار مناسب و دقیق روش های معمول کشت و آزمایش مستقیم باشد.

This study aims to evaluate PCR sensitivity and accuracy in the detection of nail dermatophytosis compared to conventional direct and culture detection methods، and performs an assessment of Trichophyton rubrum in patients suspected of having nail dermatophytosis.MethodsThis experiment was a descriptive-experimental study carried out on 71 nail samples obtained from patients with suspected nail dermatophytosis. All clinical samples of nails or chips were divided into three sections and each section underwent direct examination، culture and molecular tests. In the molecular test، we used fungal rRNA universal primers (ITS1 and ITS4) and Trichophyton rubrum-specific primers.ResultsIn this study، for the first time in Iran and based on a modified protocol، DNA was directly extracted from tissues of infected nails in less than five hours. Additionally a comparison of the results obtained from routine laboratory methods such as direct examination and culture with PCR verified the high sensitivity and accuracy of PCR compared to the other studied methods.ConclusionPCR، as a rapid، accurate method، can be a good replacement for conventional culture and direct examination.Keywords: Onychomycosis, Dermatophytosis, Trichophyton rubrum, PCR} -
زمینه و اهدافT.rubrum) Trichophyton rubrum) شایع ترین عامل کچلی انسان در سراسر دنیا می باشد. درمان این بیماری کامل نیست و پس از قطع دارو عود کننده است. اخیرا زیرواحد G از آنزیم V-ATPase به-عنوان هدف برای داروها مورد توجه قرار گرفته. هدف مطالعه کلون کردن ژن کدکننده زیرواحد G آنزیم V-ATPase قارچ T.rubrum در باکتری E.coli به منظور نگهداری و مطالعه بر روی پروتئین حاصل از آن بود.روش بررسیDNA و RNA از قارچ درماتوفیت T.rubrum جدا شد، سپس توسط PCR و RT-PCR تکثیر یافت و محصول RT-PCR در پلاسمید T–pTG19 الحاق شد. پلاسمید نوترکیب با روش شوک حرارتی به داخل E.coli وارد شد. باکتری کشت داده شد و کلنی های مشکوک جدا شدند و مجدادا کشت داده شدند. سپس PCR طبق روش قبل انجام شد.
یافته هاوزن مولکولی DNA استخراج شده حدود 750bp و cDNA تقریبا 700bp بود. با کشت E.coli بر روی (Luria agar (LB Agar کلنی های آبی و سفید دیده شد. PCR کردن DNA حاصل از کلنی های مشکوک نشان داد 5 کلون از 8 کلون دارای ژن مورد نظر بود.نتیجه گیریبه نظر می رسد که این ژن دارای اینترون است. کلنی های سفید رنگ به احتمال زیاد دارای ژن مورد نظر می باشد. نتایج حاصل از PCR در آخرین مرحله نشان دهنده کلون شدن ژن مورد نظر است.

Therapy of this disease is not complete and the disease may relapse after stopping medicine consumption. Recently it is suggested that V-ATPase may work as a target for antifungal drugs development. In this study we cloned G subunit of V-ATPase gene from T.rubrum into E.coli to produce the relevant protein in high density, save it and then study it.Material And MethodsDNA and RNA was extracted from Trichophyton rubrum. Then, the gene was replicated using PCR & RT-PCR. We inserted cDNA in pTG19-T and cloned it in E.coli white using hit shock method. Then PCR was done with the same protocol.ResultsDNA has 750 bp and cDNA has 700 bp. When E. coli was cultured on Luria agar we found both blue and white colonies and by doing PCR it was shown that the 5 colony of E.coli had this gene.ConclusionIt seems the gene V-ATPase has intron. It is also suggests that the white colonies have probably the gene. Background
Trichophyton rubrum is one of the major causes of dermatophytosis both in the world and in Iran. Trichophyton rubrum invades keratinized tissues (hair, skin and nails) of humans and animals. Dermatophytosis is recurrent and drug-resistant. Furthe more, it often needs expensive treatmens. In a search about new treatment modalities alongside conventional drug therapies in order to achieve a better control for dermatophytosis, we studied the effect of Low Level Laser Therapy(LLLT) using diode laser (685 and 830 nm) on Trichophyton rubrum in an in-vitro setting.
Material and MethodsIn this in-vitro study,at first we identified T.rubrum using standard methods. Then equal amounts of T.rubrum were cultured on Sabouraud Dextrose Agar with Chloramphenicol and Cyclohexamide (Scc) plates. Afterwards,the plates were irradiated in 9 group using Diode laser(BTL 2000) 685 nm (25 mW; 3,5,10,20 J/Cm2)and 830 nm (200mW;3,5,10,30,50 J/Cm2). In each group 12 plates were irradiated. Also 12 plates served as control group. Then plates were examined for inhibition of growth, macroscopic and microscopic morphology, minimum inhibitory concentrations (MIC) and genomic changes following exposure.
ResultsThe results of this study indicated that the fungal growth were observed in all groups, but LLLT is effective on quality of growth of Trichophyton rubrum in a dose dependent manner. All these changes were obvious in laser groups irradiated by 685 nm and 830 nm at a fluencies ≥ 10 J/cm2. The most changes were observed at the fluence 20 J/CM2 (685nm) and 30 J/CM2 (830 nm). (p<0/001).
ConclusionsIn this study, LLLT didn’t inhibit the fungal growth, but changes in the quality of growth of Trichophyton rubrum imply that LLLT h effects on the T.rubrum independently, and these changes probably occur on the protein synthesis level and gene expressions.

درماتوفیت ها گروهی از قارچ های کراتین دوست هستند که گروه وسیعی از بیماری های پوست و ضمایم آن مانند ناخن و مو را ایجاد می کنند. عوامل قارچی شامل 3 دسته میکروسپوروم، ترایکوفایتون و اپیدرموفایتون بوده که در بین آن ها جنس ترایکوفایتون از اهمیت خاصی برخوردار می باشد و در حدود 24 گونه آن تاکنون شناخته شده است. علاوه بر آن بررسی های مولکولی روی تمایز استرین های گونه ترایکوفایتون روبروم در نمونه های جمع آوری شده در ناخن های یکسان از بیماران مختلف مبتلا به اونیکومایکوزیس، نشان دهنده وجود چندین استرین متفاوت در این گونه بوده است. در این مطالعه نمونه های ناخن از 10 بیمار مختلف مبتلا به اونیکومایکوزیس کشت داده شد و پلیت های کشت که دارای 5 کلنی بودند انتخاب شدند و اسید دی اکسی ریبونوکلئیک(D.N.A) آن ها به طور جداگانه استخراج شد و نتایج با توجه به مراحل مختلف روش مولکولی واکنش زنجیره ای پلیمراز(PCR) و براساس خصوصیت اختلاف در تعداد تکرار ژن های تولید کننده اسیدریبونوکلئیک ریبوزومی (r R.N.A) در منطقه غیرژنی تجزیه و تحلیل شد. در این مطالعه 10 نمونه از ضایعات ناخن مبتلا به اونیکومایکوزیس در اثر ترایکوفایتون روبروم بررسی شد که تنها در 6 نمونه، تعداد 2 استرین مختلف یا بیش تر در یک واریته از گونه ترایکوفایتون روبروم مشاهده گردید. براساس یافته های این پژوهش پیش بینی می شود که در تعداد زیادی از عفونت های قارچی ناخن توسط ترایکوفایتون روبروم، استرین های گوناگون(Multiple strain) وجود داشته باشد. وجود چنین مساله ای در مطالعات اپیدمیولوژیک مربوط به بیماری های قارچی از اهمیت خاصی برخوردار می باشد.

The genus trichophyton is particularly important and complex which comprises at least 24 recognised species. Additionally, molecular strain typing of trichophyton rubrum isolated within single nail specimens from patients with onychomycosis indicates involvement of multiple strains. In this study, nail specimens from patients with onychomycosis were cultured and 10 specimens from different patients with 5 colonies per culture plate were selected. Deoxyribonucleic acid(DNA) was extracted from these isolates and subjected to a PCR-based typing method that analysed variations in numbers of repeats in the non-transcribed spacer(NTS) region of the ribosomal ribonucleic acid(rRNA) gene repeats. In 6 out of 10 specimens, there were 2 or more strain types. This suggests that in the majority of cases of fungal nail infections by trichophyton rubrum, multiple strains are involved.
