پیش زمینه و هدف:
یکی از نیازهای اساسی و مهم بیماران تحت همودیالیز، دسترسی عروقی است. دانشجویان پرستاری به عنوان اعضای تیم سلامت و پرستاران آینده نقش اساسی در مدیریت ایمنی بیماران تحت همودیالیز دارند و بنابراین، آموزش آن ها در جهت حمایت از ایمنی بیمار اهمیت دارد. امروزه آموزش مبتنی بر شواهد باهدف استفاده صریح و عقلانی از بهترین شواهد رایج، آگاهی کافی و لازم در زمینه مدیریت ایمن و موثر از بیماران بر طبق دستورالعمل های مبتنی بر شواهد را برای دانشجویان پرستاری فراهم می آورد. لذا پژوهش حاضر باهدف تعیین تاثیر آموزش مبتنی بر شواهد بر آگاهی دانشجویان پرستاری در مراقبت از دسترسی های عروقی همودیالیز در سال 1400 طراحی و اجرا گردید.
مواد و روش هادر این مطالعه نیمه تجربی تعداد 65 نفر از دانشجویان سال چهارم پرستاری دانشکده پرستاری و مامایی ارومیه انتخاب و به دو گروه مداخله و کنترل تقسیم شدند. سپس در گروه مداخله، آموزش مبتنی بر شواهد در 4 جلسه 5/1 تا 2 ساعت برای دانشجویان ارایه شد ولی گروه کنترل آموزشی دریافت نکرد. اطلاعات با پرسشنامه اطلاعات جمعیت شناختی و پرسشنامه آگاهی از دسترسی های عروقی همودیالیز جمع آوری و با استفاده از نرم افزار SPSS نسخه 16 تجزیه وتحلیل و مقایسه شد.
یافته هانتایج این مطالعه نشان داد نمرات آگاهی دانشجویان پرستاری قبل از مداخله در بین دو گروه اختلاف معناداری باهم نداشتند (749/0=p). بااین حال، نمرات آگاهی بعد از مداخله در دو گروه اختلاف معناداری باهم داشتند (0001/0=p). از طرف دیگر، نمرات آگاهی دانشجویان پرستاری از دسترسی های عروقی همودیالیز در ابتدا و انتهای مطالعه در گروه کنترل اختلاف معناداری باهم نداشتند (107/0=p). همچنین نمرات قبل و بعد آگاهی دانشجویان در گروه مداخله اختلاف معناداری باهم داشتند (0001/0=p).
بحث و نتیجه گیریبا توجه به یافته های پژوهش می توان نتیجه گرفت که اجرای آموزش مبتنی بر شواهد بر روی آگاهی دانشجویان پرستاری از دسترسی های عروقی موثر بود. لذا پیشنهاد می شود بر اهمیت دانش دستورالعمل های مبتنی بر شواهد به منظور ترویج انتقال دانش نظری به بالین برای دانشجویان پرستاری بیش ازپیش تاکید گردد.

Nursing students as members of the health team and future nurses, play an essential role in the safety management of hemodialysis patients and therefore, their training is important to support patient safety. Today, evidence-based education with the objective of explicit and rational use of the best common evidence provides sufficient and necessary knowledge in the field of safe and effective management of patients according to evidence-based guidelines for nursing students. Therefore, the present study was designed and implemented with the aim of determining the effect of evidence-based education on the knowledge of nursing students in the care of hemodialysis vascular access in 2021.
Materials & MethodsIn this experimental study, 65 fourth-year nursing students of Urmia College of Nursing and Midwifery were selected and divided into two intervention and control groups. Then, in the intervention group, evidence-based training was provided to students in 4 sessions each for 1.5 to 2 hours, but the control group did not receive any training. Information was collected with demographic information and hemodialysis vascular access questionnaires, and analyzed and compared using SPSS version 16 software.
ResultsThe results of this study showed that the knowledge scores of nursing students before the intervention did not differ significantly between the two groups (p=0.749). However, the scores of awareness after the intervention were significantly different in the two groups (p=0.0001). On the other hand, there was no significant difference in the scores of knowledge of nursing students about hemodialysis vascular access at the beginning and end of the study in the control group (p=0.107). Also, the scores before and after the knowledge of the students in the intervention group were significantly different (p=0.0001).
ConclusionAccording to the findings of the research, it can be concluded that the implementation of evidence-based education was effective on the awareness of vascular access among nursing students. Therefore, it is suggested to emphasize the importance of knowledge of evidence-based guidelines in order to promote the transfer of theoretical knowledge to the bedside for nursing students.

Background
Intravenous cannulation is needed in about 90% of patients admitted to hospitals to administer medications or replace fluids, which is difficult to perform in obese patients with diabetes.
ObjectivesThe present study aimed to assess the effect of using near-infrared vein finder technology on the success rate of cannulation in obese patients with diabetes.
MethodsThis randomized controlled trial was conducted on 92 eligible patients with type 2 diabetes and obesity at Al-Rusafa hospitals in Baghdad, Iraq, in September 2021. The subjects were divided into the intervention (n = 46) and control (n = 46) groups. However, cannulation using near-infrared vein finder technology was only performed for the intervention group. The data were collected using a demographic and clinical data questionnaire and a chronometer.
ResultsThe success rates of cannulation for the first time were 60.9% and 15.2% in the intervention and control groups, respectively. The mean values of procedural time were 53.2 ± 28.9 and 94.3 ± 41.5 seconds in the intervention and control groups, respectively. The independent effects of patients’ body mass index (P = 0.002) and skin color (P = 0.040) on procedural time were significant.
ConclusionsThe use of near-infrared vein finder technology in obese diabetic patients reduces procedural time and venipuncture attempts.

Background
This study aimed to determine changes in microvascular endothelial function with upper arm arteriovenous fistula (AVF) creation and maturation in patients with chronic kidney disease (CKD).
Materials and MethodsThis prospective cross?sectional study was performed at Hospital Universiti Sains Malaysia, a tertiary hospital in Malaysia. Forty CKD patients (stage 4–5) who were scheduled for elective AVF creation over the upper extremity for maintenance hemodialysis were recruited using conveniencesampling method. Microvascular endothelial?dependent vasodilation was measured using laser Doppler flowmetry and the process of iontophoresis preoperatively and postoperatively at weeks 2 and 6. Fistula maturation was assessed at week 6.
ResultsThirty?two patients had successful AVF maturation. Endothelial?dependent vasodilation (acetylcholine (Ach)%) was higher (246.48 [standard deviation (SD) 209.38] vs. 104.95 [SD 43.29], P = 0.001) while systolic blood pressure was lower (142.25 [SD 21.50] vs. 162.25 [SD 13.26], P = 0.017) in this group as compared to unsuccessful AVF group. No significant changes were seen in overall microvascular endothelial?dependent vasodilation during the 6?week study period (day 0, 246.48 [SD 209.38]; week 2, 201.14 [SD 198.19]; and week 6, 203.53 [SD 145.89]).
ConclusionUpper arm AVF creation does not affect microvascular endothelial function up to 6 weeks post operation and may not contribute to the success of AVF maturation. However, the lower microvascular endothelial?dependent vasodilation and higher systolic blood pressure in unsuccessful AVF subjects need to be further studied.

Background
Access to arteries is one of the most appropriate approaches for helping chronic kidney diseases.
ObjectivesThe aim of the present study was to evaluate the survey method of vascular surgery on the onset of ischemic limb syndrome following vascular access in dialysis patients.
MethodsIn this cross-sectional study, the population study was patients with end-stage renal disease (ESRD) referring to Ahvaz Golestan Hospital in 2018. Control of limbs temperature and comparison with the other side, before and after surgery, feeling cold by the patient, control of organs pulse and comparison with the other side, pain of organs comparing to the other side, pain during the dialysis, paresthesia, and movement failure and during dialysis, and organs atrophy-ulcer were measured.
ResultsIn the present study, 294 patients, including 139 women (46.94%) and 156 men (53.06%), were studied. Ischemic steal syndrome (ISS) was observed in 20 patients, that 13 cases of them had benefited from fistula approach and 7 patients used graft approach. The most common observed ISS was related to brachiocephalic fistula approach (AVF-Brachiocephalic) and then graft in arm. In non-steal syndrome group, brachiocephalic fistula (AVF-Brachiocephalic) had the most common application. In the present study, there was no significant correlation between access method and underlying diseases of high blood pressure, diabetes, and cardiac ischemic diseases.
ConclusionsOne of themost important results of the present study was the highest existence of steal syndrome in brachiocephalic approach, which is confirmed by other studies.

Background
Local vascular complications in pediatrics undergoing congenital cardiac catheterization are consistently reported to be the commonest regardless of the access type and side.
MethodsThis is a retrospective study with data collection of multidetector computed tomography studies performed between 2016 and 2019 from 3 large cardiac centers in our country.
ResultsTotally, 190 patients who required both arterial and venous access sites were randomization into Group I or the planned ipsilateral group (the planned insertion of both arterial and venous sheaths in the same limb), Group II or the planned contralateral group (the planned insertion of arterial and venous sheaths in different limbs), and Group III or the unplanned group (unplanned vascular access sidedness after the failure of initial randomization). The incidence and types of local vascular complications during the hospital stay were recorded.Patients with the unplanned vascular access site had a higher incidence of local vascular complications, longer hospital stays, with higher needs for heparin and thrombolytic therapy than patients with the planned vascular access site. Patients with a systematically planned contralateral access site showed a lower incidence of arterial thrombosis, delayed capillary refilling time, and arteriovenous fistula, as well as lower needs for heparin and thrombolytic administration than patients with the planned ipsilateral vascular access site.
ConclusionsSystematic planned contralateral vascular access in patients who undergo congenital heart disease catheterization requiring both arterial and venous sheaths is associated with a lower incidence of vascular complications, especially in patients weighing less than 10 kg. زمینه و هدف
دسترسی عروقی با عملکرد مناسب به عنوان راه حیاتی برای بیماران همودیالیزی مطرح است. دسترسی های عروقی شامل فیستول شریانی- وریدی، کاتتر موقت یا دایم ورید مرکزی است. شاخص کفایت دیالیز به دلیل تاثیر عمیقی که بر میزان ابتلاء و مرگ و میر بیماران همودیالیزی دارد، به عنوان یکی از مولفه های مهم و ضروری در نظر گرفته می شود. عوامل مختلفی نظیر نوع دسترسی عروقی می توانند روی کفایت دیالیز تاثیرگذار باشند. مطالعه حاضر با هدف بررسی ارتباط نوع دسترسی عروقی با معیار کفایت دیالیز در بیماران همودیالیزی انجام شد.
روش کارهمه بیماران بخش همودیالیز بیمارستان توحید سنندج به روش سرشماری انتخاب شدند. بیماران بر اساس نوع دسترسی عروقی در دو گروه کاتتر دایم ورید مرکزی و فیستول شریانی- وریدی تقسیم شدند. برای سنجش کفایت دیالیز از معیار KT/V استفاده شد. تست های آزمایشگاهی شامل میزان هموگلوبین، فسفر، آلبومین و هورمون پاراتیرویید ثبت شد. در پایان متغیرهای زمینه ای مورد ارزیابی قرار گرفت.
یافته ها:
بیماران در دو گروه فیستول شریانی- وریدی(74نفر) و کاتتر دایم(69 نفر) قرار گرفتند. میانگین معیار KT/V در کل بیماران 28/0±32/1 بود. از نظر کفایت دیالیز دو گروه با هم اختلاف معنی داری نداشتند (P=0.32). هم چنین در مقایسه ی تست های آزمایشگاهی نتایج نشان داد که دو گروه با هم اختلاف معنی داری ندارند (P≥0.05).
نتیجه گیری:
مطالعه حاضر نشان داد که دیالیز در اکثریت بیماران دارای دسترسی عروق دایم (فیستول شریانی- وریدی یا کاتتر دایم ورید مرکزی) از کیفیت و کارآیی مطلوبی برخوردار است، لذا بدون نگرانی از این روش ها در اندیکاسیون ها و شرایط خاص می توان از هر کدام از آنها استفاده کرد. اما از نظر بهبودی در تست های آزمایشگاهی برای روشن تر شدن قضیه نیاز به مطالعات بیشتری است.

Vascular access includes Arterio-Venous Fistula (AVF) and temporary or permanent central venous catheter. Dialysis adequacy is an essential element in the management of chronic Hemodialysis (HD) treatment as the adequacy of the dose has a profound effect on the patientchr('39')s morbidity and mortality. Various factors affect the adequacy of dialysis including vascular access. The aim of present study was to investigate the relationship between vascular access type and dialysis adequacy in HM patients.
MethodsAll patients undergoing HM referring to the dialysis ward of Tohid Hospital in Sanandaj city were selected by census method. Patients were divided into two groups according to vascular access type (AVF or permanent CVC). The Kt/V was used as the gold standard for measuring the adequacy of dialysis. Laboratory parameters including hemoglobin, phosphorus, albumin and parathyroid hormone were recorded. Finally, socio-demographic information and treatment characteristics were collected.
ResultsThe patients were divided into two groups: arterial-venous fistula (n=74) and permanent catheter (n=69). The mean KT/V in patients was 1.32±0.28. There was no significant difference in dialysis adequacy between the two groups. Also, the laboratory parameters in the two groups were not significantly different.
ConclusionFindings revealed that dialysis adequacy in patients with permanent vascular access (arteriovenous fistula or permanent central venous catheter ) is highly efficient. Therefore, these methods can be used in their indications. In terms of improving laboratory tests, further studies are needed to clarify this issue.

Background
Caring for patients undergoing hemodialysis is very critical because of the disease’s complications. In the context of hemodialysis, the essential duty of nurses is to reduce the inflammation resulting from the vascular access.
MethodsThis clinical trial was conducted on 101 hemodialysis patients in Montaserie and Imam Reza hospitals, Mashhad, Iran, for one month. Sampling was done by non-probability convenience method. Using random allocation, patients were placed into three groups including those who received a combination of alcohol and betadine to disinfect the vascular access site (n = 37; henceforth, group 1), those who underwent a concurrent application of alcohol after betadine (n = 33; henceforth, group 2), and the control group (n = 31). In group 1, the vascular access site was disinfected with betadine solution and alcohol mixed with a ratio of 2:1 before the dialysis initiated. In group 2, alcohol and betadine were used separately for disinfection. In the control group, the routine method of disinfection was performed using betadine, while antiseptic precautions were taken into account. During 12 sessions in four weeks, the vascular access sites of patients were evaluated for inflammation criteria postulated by the Iranian Nurses Association. Data were analyzed by SPSS16 software (significance level of P < 0.05).
ResultsMean inflammation severity was lower in group 1 than in group 2 (P = 0.039). No significant difference was observed between group 2 and the control group (P = 0.079).
ConclusionsThe betadine and alcohol combination can be used in similar studies because this combination is simple, inexpensive, and most importantly, effective to reduce the inflammation of the vascular access site in hemodialysis patients.

IntroductionThe mechanical circulation support used in treatment of low cardiac output at most is the intra-aortic balloon pump (IABP). Its usage fields are the complications occurring due to ischemic heart disease, disrupted left ventricle function, and the low cardiac output syndrome occurring during coronary artery by-pass surgery.MethodsDuring 28 years from 1985 to 2013, IABP support has been implemented to 3135 patients in our cardiac surgery operating theater and intensive care unit. The mean age of the patients was 61.4 ± 13.2 years (16-82). 2506 patients (80%) were the ones whom the cardiac surgery has been implemented. IABP support has been provided for 629 (20%) patients for medical treatment. We utilized IABP most frequently in coronary artery patients (70%). The first choice for placing the balloon catheter is the femoral artery in 3093 cases (98.7%).ResultsThe most frequently observed balloon complication was the lower extremity ischemia in 383 cases (12.2%).The leg ischemia was statistically significantly more frequent in patients with sheath (P=0.004). The extremity ischemia has developed in 4 of 12 patients with balloon placed from upper extremity. The local bleeding and balloon rupture were more frequent in patients whom the balloon has been placed without sheath. The mortality due to IABP has occurred in only 5 patients.ConclusionDespite increase in IABP usage frequency rapidly, the complications due to catheter are still seen. زمینه و هدفایجاد یک راه دستیابی عروقی مطمئن، مهمترین عامل برای انجام همودیالیز موثراست. فیستول شریانی وریدی، شیوه ای ایده آل برای دستیابی عروقی طولانی مدت می باشد. در این مطالعه پیامدهای تعبیه زودهنگام فیستول با تعبیه دیرهنگام آن مقایسه می شود.روش بررسیاین یک مطالعه مداخله ای مقایسه ای است که برروی 168بیمار، از بین بیماران مرحله پایانی بیماری کلیه ارجاع شده از طرف نفرولوژیست به درمانگاه جراحی عروق بیمارستان گلستان اهواز جهت فیستول گذاری در سال 1394، دردو گروه 84 نفری، بیماران دیالیز نشده و بیماران تحت دیالیز با کاتترورید مرکزی انجام شده است. اطلاعات جامعی شامل داده های دموگرافیک، و اطلاعات مربوط به بیماری و عمل جراحی جمع آوری گردید.یافته هامتوسط مدت پیگیری ( 2 ± 16) هفته بود. میزان بروز عوارض در گروه دیرهنگام بیشتر بود(42 (50%) vs 22(26.1%) ، p< 0.01 ) ، بویژه عوارضی مانند عدم بلوغ و ترومبوز درگروه دیرهنگام شیوع بیشتری داشتند . به ترتیب (34.5 % & 21.4 %) و .( 8.3 % & 6%) p< 0.01 . غالب موارد فوت در سالمندان و مبتلایان به بیماری های زمینه ای همراه بود.نتیجه گیریایجاد دسترسی عروقی زود هنگام، احتمالا بجز در سالمندان مبتلا به بیماری های عمده، همراه با افزایش موفقیت در استفاده از فیستول و اصلاح موفق عوارض آن بدون نیاز به تعبیه کاتتر می باشد و موجب بهبود کیفیت زندگی در بیماران مرحله انتهایی بیماری کلیوی می شود.کلید واژگان: فیستول شریانی وریدی, همودیالیز, دستیابی عروقی}Background And ObjectiveCreating a reliable vascular access (VA) is the most important determinant for effective hemodialysis (HD). The ideal long-term VA is arterio-venous (AV) fistula. This study was undertaken to compare and determine the impact of timing of the AV fistula creation on its outcomes.
Subjects andMethodsIn this interventional-comparative study a total of 168 ESRD patients referred by nephrologist for VA creation, to the vascular surgery clinic of Ahvaz Golestan Hospital from 1 Nov. to 31 May 2015-2016 were included in two 84-memberd groups. The late group in whom HD has been already started by double-lumen (DL) catheter and the early group in whom HD has not been started yet. they were followed prospectively and a complete data about their HD and VA were gathered.ResultsThe mean duration of follow up was 16 ± 2 weeks. The incidence of complications was higher in late group (42 (50%) vs 22 (26.1%), PConclusionEarly placement of VA before HD start, except perhaps in elderly with multiple comorbidities, is reasonable and is associated with higher success rates. زمینه و هدفهمودیالیز به عنوان یک درمان حیاتی نیازمند مسیر دسترسی به عروق مناسب است. کوتاهی در مراقبت از این مسیر منجر به از بین رفتن کارایی آن در مدت زمانی کوتاه می شود. مطالعه حاضر با هدف تعیین فراوانی مراقبت های پرستاری از مسیر دسترسی به عروق بیماران تحت همودیالیز انجام شد.مواد و روش هامطالعه ی مقطعی حاضر به روش تمام شماری بر روی پرستاران (20 نفر) و بیماران (120 نفر) تحت همودیالیز مراجعه کننده به بخش همودیالیز بیمارستان شهید بهشتی بابل در سال 1394 انجام شد. پژوهشگر چگونگی مراقبت های پرستاری از مسیر دسترسی به عروق بیماران را با استفاده از چک لیست محقق ساخته مورد بررسی قرار داد. تجزیه و تحلیل داده ها با استفاده از آمار توصیفی تحت نرم افزار آماری 16 SPSS انجام شد.یافته ها: از 14 مراقبت، پوشیدن دستکش و ماسک، تزریق بلوس به موقع هپارین، شستشوی لاین ها، کنترل مکرر فشارخون، شستشوی کامل لاین ها، برگشت کامل خون به بدن در پایان همودیالیز و پوشاندن استریل محل ورود سوزن در 100 درصد موارد اجرا شده بود. مراقبت های مربوط به رعایت دور مناسب دستگاه در هنگام شروع دیالیز (81/66%)، رعایت فاصله سوزن با فیستول (75/83%)، نگه داشتن صحیح محل سوزن بعد از خروج آن (75%)، رعایت فاصله کافی بین سوزن ها (54/17%)، کنترل فشارخون در پایان دیالیز (50%) و تغییر دادن محل سوزن در بین دو جلسه ی همودیالیز (27/5%) در حین مراقبت رعایت نشده بودند.نتیجه گیرییافته ها نشان داد از میان مراقبت های پرستاری برخی مراقبت های مربوط به شروع و پایان دیالیز مورد بی توجهی پرستاران قرار گرفته اند. پیشنهاد می شود برنامه هایی برای آموزش مداوم و بازآموزی پرستاران در زمینه مراقبت از محل دسترسی عروقی بیماران همودیالیزی تدوین شود.کلید واژگان: دسترسی عروقی, همودیالیز, مراقبت پرستاری}Background And ObjectiveHemodialysis is a critical treatment method, which depends on the vascular access route. Lack of care for access route could diminish its efficiency in a short while. This study aimed to evaluate the frequency of nursing care for vascular access route in hemodialysis patients.Materials And MethodThis cross-sectional study was conducted on 20 nurses and 120 hemodialysis patients, who referred to the Hemodialysis Department of Shahid Beheshti Hospital, Babol, Iran, in 2015. We evaluated the method of nursing care for vascular access route using a researcher-made checklist. Data analysis was performed in SPSS, version 16, using descriptive statistics.ResultsIn total, 14 cases of nursing care were evaluated, in 100% of which all the caring steps, including wearing gloves and a mask, early bolus injections of heparin, washing lines, frequent monitoring of blood pressure, rinsing the lines, returning the blood to the body after hemodialysis, and sterile covering of the puncture site, were observed. However, some of the other techniques, such as care related to appropriate pump speed when starting dialysis machine (81.66), observance of distance of fistula needles (75.83%), right placement of needle after exit (75%), pressing the injection site (54.17%), blood pressure monitoring at the end of dialysis (50%), and change of the needle site between two sessions (27.5%), were not adhered to by the nurses.ConclusionAccording to the results of this study, some of nursing care steps necessary in the beginning and end of hemodialysis were not accurately followed by the nurses. IntroductionCare for vascular access of patients undergoing hemodialysis is a critical issue. Inflammation and subsequent infection are the major factors which threaten patients'' health and diminish effectiveness of hemodialysis. Therefore, this study aimed to evaluate the severity and incidence of inflammation of vascular access in hemodialysis patients.Materials And MethodsThis cross-sectional study was conducted on 90 patients undergoing hemodialysis in Emam Reza and Montazerie Hospitals in Mashhad, June, 2014. Evaluation of inflammation severity over the course of one month (12 hemodialysis sessions) was performed by means of an inflammation tool designed by the Board of Nursing. Data were analyzed using SPSS version 16, and performing descriptive and Chi-square tests.ResultsThe mean and standard deviation of incidence of inflammation in the first session of hemodialysis was 3.2±1.3 cases. The mean and standard deviation of the intensity of inflammation was 12.5±4.7.ConclusionSince inflammation of vascular access in hemodialysis patients impairs their safety and health improvement, necessary measures to reduce this complication must be taken.Keywords: Health promotion of hemodialysis patients, Inflammation during hemodialysis, Vascular access}

BackgroundArteriovenous Fistula (AVF) is the ideal method of vascular access for patients on maintenance hemodialysis (HD). Therefore it is an important part of treatment in HD. There are several observations that indicate the role of inflammation in failure of AVF. The aim of this study was to evaluate the hematologic and inflammatory biomarkers in early AVF failure.MethodsThis case-control study included 110 ESRD patients, whom were undergone AVF creation, divided in two groups. About 700 radius-cephalic AVF were created during these two years. We found 55 cases with AVF failure. In this study, we compared those 55 failures with 5 functional AVF which were selected using randomized sampling from the rest of patients according to age, gender, and AVF location. Levels of serum C-reactive protein (CRP) were checked in both groups to evaluate the relation between AVF failure and CRP level before surgery.ResultsThe mean±SD age of the patients was 49.7±17.28 years. CRP was positive in 34 patients (61.8%) with unsuccessful fistula function, while only 4 (7.3%) of those with successful AVF had positive CRP and the rest had negative CRP. The difference between the two groups of patients was strongly significant (p<0.001). Statistically, there was not any significant difference between the average of age (p: 0.580) of patients in the control and experimental groups. However, the gender (p: 0.832) discrepancies was not meaningful between the groups.ConclusionAVF thrombosis is one of the main complications after AVF creation. مقدمه
روش های رایج برای توقف خونریزی به علت اعمال فشار طولانی در بیماران با زمان هموستاز طولانی می تواند منجر به تشکیل لخته و ترومبوز دسترس عروقی گردد. عوامل هموستاتیک موضعی نقش موثری در کاهش زمان هموستاز و در نتیجه پیشگیری از این عوارض دارد.
هدفمقایسه اثر پودر سلوکس و پانسمان فشاری معمول بر زمان هموستاز محل دسترس عروقی بیماران همودیالیز.
روشدر این کارآزمایی بالینی یک گروهی با طرح قبل و بعدی که در سال های 92-93 انجام شد؛ 30 بیمار دیالیزی در بیمارستان های امام رضا(ع) و منتصریه مشهد و شهیدمدرس کاشمر که از طریق فیستول شریانی- وریدی تحت درمان همودیالیز قرار داشتند به روش نمونه گیری هدفمند انتخاب شدند. محل پانکچرهای فیستول شریانی- وریدی بیماران در سه هفته منقطع با استفاده از پانسمان پودر سلوکس درمان شد. زمان هموستاز محل هر یک از پانکچرها در هر جلسه اندازه گیری شد. داده ها با استفاده از آزمون های اندازه های تکراری، فریدمن و تصحیح بون فرونی با نرم افزار SPSS نسخه 5/11 تحلیل شد.
یافته هامیانگین سنی بیماران تحت مطالعه 3/14±2/54 سال و 16 نفر (3/53 درصد) مذکر بودند. زمان هموستاز محل دسترس عروقی بیماران 9/1±8/9 دقیقه بود. تفاوت معنی داری در زمان هموستاز بین روش استفاده از یک گرم پودر سلوکس در مقایسه با روش پانسمان معمول وجود داشت (001/0p<). تفاوت معنی داری نیز در زمان هموستاز بین روش استفاده از نیم گرم پودر سلوکی در مقایسه با روش پانسمان معمول وجود داشت (001/0p<).
نتیجه گیریاستفاده از پودر سلوکس باعث کاهش زمان هموستاز محل دسترس عروقی بیماران همودیالیز می گردد. بنابراین استفاده از آن برای بیمارانی که هموستاز محل دسترس عروقی آن ها با تاخیر صورت می گیرد توصیه می شود

MethodsThis one-group، before-after، clinical-trial was conducted on 30 dialysis patients who had been selected by purposive sampling and underwent hemodialysis through arteriovenous fistula in Imam Reza and Montaserieh hospitals of Mashhad and Modarres hospital of Kashmar in 2013- 2014. The arteriovenous fistula puncture sites were treated with Celox powder dressing in three intermittent weeks. Time to hemostasis in each puncture site was calculated. Data were analyzed by repeated measures، Bonferroni correction، Friedman tests using SPSS version 11. 5.
ResultsThe mean age of patients was 54. 2 ±14. 3، and most of them (53. 3 percent) were male. Time to hemostasis in the vascular access sites of patients was 9. 8±1. 9 minutes. There was a significant difference in the hemostasis time between using 1 gram Celox powder and conventional dressing، (p<0/001). There was also a significant difference in time to hemostasis between utilizing 0. 5 gram Celox powder and conventional dressing (p<0/001).
ConclusionUsing Celox powder reduces time to hemostasis in vascular access site in hemodialysis patient. Therefore، its use in patients with delayed vascular access site hemostasis is recommended.

مقدمهالتهاب در محل دستیابی عروقی بیماران همودیالیز باعث عفونت می گردد و دومین علت مرگ این بیماران است؛ اما پروتکل خاصی برای ضدعفونی قبل از دستیابی عروقی بیماران همودیالیز موجود نمی باشد.هدفتعیین تاثیر ترکیب الکل و بتادین بر شدت التهاب محل دستیابی عروقی بیماران همودیالیز.روشتعداد 69 بیمار تحت همودیالیز در این کارآزمایی بالینی تصادفی در تیر 1393 در بیمارستان های منتصریه و امام رضا(ع) در شهر مشهد در مدت یک ماه به صورت تخصیص تصادفی در دو گروه ضدعفونی با ترکیب الکل و بتادین و شاهد قرار گرفتند. در گروه مداخله، محل دستیابی عروقی با محلول بتادین به نسبت 2 و الکل با نسبت 1 توسط پژوهشگر قبل از شروع همودیالیز ضدعفونی گردید. در گروه شاهد از روش رایج بخش که استفاده از بتادین است، برای ضدعفونی محل دستیابی عروقی بیماران استفاده شد. محل دستیابی عروقی بیماران مذکور طی 12 جلسه با استفاده از معیار التهاب انجمن پرستاری ایران ارزیابی شد. داده ها توسط نرم افزار SPSS نسخه 5/11 و آزمون های من ویتنی و مجذورکای تحلیل گردید.یافته هامیانگین سن در گروه مداخله 6/16±8/46 و شاهد 6/1±4/54 سال بود. میانگین کل شدت التهاب در گروه مداخله (2/0±2/0) از شاهد (4/0±1/1) کمتر بود (05/0>p).نتیجه گیریترکیب الکل و بتادین به عنوان یک ضدعفونی کننده موثر در کاهش التهاب و در نتیجه عفونت محل دستیابی عروقی بیماران همودیالیز می باشد و پرستاران می توانند از این محلول در محل دستیابی عروقی آن ها استفاده کنند.

However، there is no specific protocol available for sterilization before vascular access in such patients.Aimto determine the effect of the combination of alcohol and Betadine on the severity of inflammation in the vascular access site in hemodialysis patientsMethodsIn this Randomized Controlled Trial in July 2014، 68 patients undergoing hemodialysis and hospitalized in Imam Reza and Montasereye hospitals of Mashhad، were randomly assigned in two groups of control and combination of alcohol and Betadine group for one month. In the intervention group، vascular access site was disinfected with Betadine solution and alcohol in 2:1 ratio by the researcher before starting dialysis. The common method of the ward، which was povidone iodine alone، was used to disinfect the vascular access site in control group. The vascular access site of the patients was evaluated by the Iranian Nurses Association''s inflammatory criteria during 12 sessions. Data were analyzed by SPSS version 11. 5 using Mann-Whitney and chi-square tests.ResultsThe mean of age in the intervention group was 46. 8±16. 6، and in control group was 54. 4±1. 6. Mean severity of inflammation in the intervention group (0. 2±0. 2) was lower than that in control group (1. 1±0. 4) (p<0/05).ConclusionThe combination of alcohol and Betadine as a disinfectant is effective in reducing the inflammation and consequently the infection in the vascular access site in hemodialysis patients، and nurses can use this antiseptic for vascular access.Keywords: Vascular access, hemodialysis, Inflammation, Antiseptic} -
زمینه و هدفافزایش کیفیت همودیالیز به کاهش عوارض محل دستیابی عروقی این بیماران بستگی دارد. کاهش التهاب محل دستیابی عروقی از وظایف مهم پرستار است. هدف از این مطالعه، بررسی مقایسه ای بروز التهاب در دو گروه استفاده کننده از روش ضد عفونی ترکیب الکل- بتادین و روش الکل بعد از بتادین در محل دستیابی عروقی بیماران تحت همودیالیز بود.مواد و روش هااین کارآزمایی بالینی سه گروه بر روی بیماران بستری در بخش همودیالیز بیمارستان های تحت پوشش دانشگاه علوم پزشکی مشهد در سال 1393 انجام گرفت. تعداد 100 بیمار به صورت در دسترس انتخاب و به روش تخصیص تصادفی از نوع طبقه ای در سه گروه کنترل (31 نفر)، ضد عفونی با ترکیب الکل- بتادین (37 نفر) و ضد عفونی با الکل بعد از بتادین (32 نفر) قرار گرفتند. در گروه مداخله یک، محل دستیابی عروقی با محلول ترکیبی الکل- بتادین و در گروه مداخله دو، محل دستیابی عروقی با محلول جداگانه الکل و بتادین ضد عفونی گردید. در گروه کنترل، از روش رایج بخش برای شستشوی محل دستیابی عروقی (بتادین) استفاده شد. طی یک دوره یک ماهه شامل 12 جلسه همودیالیز محل دستیابی عروقی بیماران با استفاده از معیار فلبیت (Phlebitis) انجمن پرستاری ایران مورد ارزیابی قرار گرفت. داده ها توسط نرم افزار SPSS نسخه 16 و آزمون های آماری کای دو، آنالیز واریانس یک طرفه و تست دقیق فیشر تجزیه و تحلیل شد.یافته هامیزان بروز التهاب به ترتیب درگروه ترکیب الکل- بتادین 46 درصد، درگروه الکل بعد از بتادین 87/9 درصد و در گروه کنترل 100 درصد بود. بروز التهاب در گروه ترکیبی الکل- بتادین بطور معنی داری کمتر از گروه الکل بعد از بتادین بود (0/001 > P). اما تفاوت معنی داری بین گروه الکل بعد از بتادین با گروه کنترل وجود نداشت.نتیجه گیریترکیب الکل و بتادین به عنوان یک روش ساده و ارزان در کاهش بروز التهاب محل دستیابی عروقی بیماران تحت همودیالیز موثر می باشد. و می توان از آن به عنوان روش شستشوی محل دستیابی به عروق در بخش همودیالیز استفاده کرد.

One of the crucial roles of nurses in hemodialysis wards is reducing inflammation and infection of the vascular access. This study was conducted to compare the incidence of inflammation around the vascular access area in patients undergoing hemodialysis between two antiseptic methods of alcohol after povidone-iodine and the combination of alcohol and povidone-iodine.Materials And MethodThis clinical trial was performed under the supervision of Mashhad University of Medical Sciences, Iran, after gaining ethical committee approval in 2014. In the present study, 100 participants were selected by convenience sampling method and randomly divided into three groups of combination of alcohol and povidone-iodine (n = 37), alcohol after using povidone-iodine (n = 32), and control group (n = 31). In the intervention groups 1 and 2, vascular access was disinfected using a combination of alcohol and povidone-iodine and alcohol after povidone-iodine, respectively. In the control group, vascular access was disinfected using the method of the related ward. Patients were fully observed for phlebitis occurrence for 12 hemodialysis sessions (1 month). Vascular access was controlled using the Iranian Nurses Association's phlebitis criteria. Data were analyzed using chi-square, ANOVA test, and Fisher's exact test in SPSS version 16.ResultsThe incidence rate of inflammation in the combination of alcohol and povidone-iodine, alcohol after povidone-iodine, and control groups, respectively, were 46%, 87.9%, and 100%. The incidence rate of inflammation was significantly lower in the combination of alcohol and povidone-iodine compared to the alcohol after povidone-iodine group (P < 0.001). However, no significant differences existed between the alcohol after povidone-iodine and control group.ConclusionThe combination of alcohol and povidone-iodine is a simple, inexpensive, and most importantly effective method of reducing the incidence of vascular access inflammation in patients undergoing hemodialysis. BackgroundVascular access complications are a major cause of excessive morbidity and mortality in the dialysis population. Moreover, there is not sufficient research regarding the factors correlated with vascular access complications among hemodialysis patients. This study aimed to evaluate the vascular access complications and their related factors such as nursing techniques and self-care in hemodialysis patients.Materials And MethodsA cross-sectional study was performed on 110 patients undergoing hemodialysis in Isfahan Aliasghar hospital during 9 months from July 2010 to March 2011. The data collection tools were a demographic questionnaire and three checklists designed to assess the complications of vascular access and care techniques. Data were collected by observations and interviews with the patients. Fisher’s exact test, chi-square test, Pearson correlation coefficient, and descriptive statistics were used to analyze the data through SPSS 16.ResultsAmong 110 subjects, there were 63 male and 47 female subjects. Subjects’ mean age was 55.88 (15.51) years. There were 72 patients undergoing hemodialysis through arteriovenous fistula and 38 through permanent intra-jugular catheters. Insufficient blood flow in the catheter was the most common complication in patients with jugular catheters. Also, aneurysm was the most prevalent complication in patients with arteriovenous fistula. Low self-care of patients and needling into the aneurysm were correlated with aneurysm size. Presence of underlying diseases was related to ischemia.ConclusionsNursing techniques and self-care of patients were correlated with the occurrence of complications. Background And ObjectivesArteriovenous fistula is a popular vascular access method for surgical treatmentof hemodialysis patients. The method, however, is associated with a high rate of early failure varying in the rangeof 20-60%. Predicting early Arteriovenous fistula failure and its risk factors can help reduce its incidence, its hospitalizationrate, and associated costs. In this study, we examined performance of data mining in the predictionof early AVF failure and identification of its risk factors.MethodsThe data of 193 patients who underwent homodialysis in Hasheminejad Kidney Center were explored. Eightcommon attributes of the patients including age, sex, hypertension level, Diabetes Mellitus state, hemoglobin level,smoking behavior, location of Arteriovenous fistula, and thrombosis state were used in the machine learning process.Two learning operators including W-Simple Cart and WJ48 tree were used in data mining process.FindingsSmoking was identified as a factor influencing the relationship between the outcome of vascular accesssurgery and hemoglobin level. Prediction accuracy varied within the range of 69.15-85.11%.ConclusionsAccording to our results smoking is a crucial risk factor for early Arteriovenous fistula failure, evenat normal levels of hemoglobin. سابقه و هدفاستفاده از فیستول شریانی- وریدی باعث پیشرفت های بزرگی در اداره بیماران نیازمند به دیالیز مکرر شده است. با توجه به محدودیت محل های ایجاد این فیستول ها در هر فرد و برای جلوگیری از صرف هزینه ها و مراجعات مکرر بیمارستانی، میزان کارآیی فیستول های شریانی- وریدی و شناخت عوامل موثر بر آن از اهمیت زیادی برخوردار است.مواد و روش هادر یک بررسی 5 ساله 72 بیمار مبتلا به نارسایی مزمن کلیه که مورد عمل جراحی فیستول شریانی- وریدی برای همودیالیز قرار گرفته بودند، به مدت یک سال پیگیری شدند. مدت کارکرد هر فیستول برحسب ماه، محل فیستول، نوع آناستوموز، مدت نارسایی کلیه، سابقه دیابت، هیپرتانسیون و سابقه کاتتر موقتی برای همودیالیز، سن و جنس مشخص گردید و اطلاعات با استفاده از مدل های رگرسیون مورد تجزیه و تحلیل قرار گرفت.یافته هامطالعه نشان داد که کارآیی یکساله فیستول های شریانی- وریدی 6/80 درصد بوده و در صورت سن بالای 65 سال، سابقه نارسایی مزمن کلیه بیش از 20 ماه و فیستول در ناحیه مچ، کارآیی یک ساله به طور معنی داری پایین تر است (001/0، 002/0، 031/0 =p). به علاوه فاکتورهای جنس، سابقه دیابت، سابقه هیپرتانسیون، سابقه کاتتر موقتی برای همودیالیز تاثیر قابل توجهی بر میزان کارآیی یک ساله ندارند.استنتاجبا توجه به کارآیی بالای فیستول های شریانی- وریدی هنوز این فیستول ها بهترین راه دستیابی عروقی جهت همودیالیز مکرر می باشند و در صورت سن بالای 65 سال و یا سابقه طولانی نارسایی مزمن کلیه بهتر است از ابتدا فیستول در ناحیه کوبیتال گذاشته شود. علاوه بر آن با توجه به اثر نارسایی مزمن کلیه طولانی مدت، بر میزان کارآیی فیستول، بهتر است در نارسایی مزمن کلیه برای گذاشتن فیستول سریع تر تصمیم گیری و اقدام شود.

Patency rate of these fistulas and the factors influencing them is of great importance to reduce the costs and rate of repeated hospitalization.Materials And MethodsIn a five-year study 72 patients who had native AVF were followed for one year. Function time of each fistula, site, type of anastomosis, age, sex, history of diabetes, hypertension, temporary catheter for dialysis, and duration of renal failure (RF) were determined.ResultsOne- year patency rate of AVF was 80.6% which was found significantly lower among those aged more than 65 years, cases with history of RF more than 20 months, and among patients who had wrist fistulas (P=0.001, 0.002, 0.031, respectively). However, the patency rate was not affected by other factors.ConclusionBecause of high patency rate, native AVF is yet the best way of vascular access for repeated hemodialysis. To achieve an optimal outcome with AVF we recommend placing them preferentially in the upper arm in those older than 65 years of age and in patients with prolonged history of RF. زمینه و هدفهمودیالیز مهمترین روش درمانی در بیماران مرحله انتهائی بیماری کلیوی (ESRD) می باشد و فیستول های شریانی وریدی هنوز ایده آل ترین روش دیالیز در بیماران کلیوی هستند. بنابراین دستیابی های عروقی از مهم ترین چالش های درمان در این بیماران می باشد. در این میان مطالعات محدودی در مورد تاثیر التهاب روی شکست عملکرد فیستول انجام شده است. هدف از مطالعه ما بررسی ارتباط مارکرهای التهابی با شکست زودرس فیستول است.مواد و روش هاهشتاد و هفت بیمار مرحله انتهایی بیماری کلیوی که تحت جراحی فیستول قرار گرفته بودند در مطالعه وارد شدند و به صورت مطالعه مقطعی مورد آنالیز قرار گرفتند. بیماران طی یک ماه، برای شکست زودرس و عوارض پیگیری شدند. سن، جنس، عوارض، نوع و محل قرارگیری فیستول، غلظت هموگلوبین، ESRوCRP در این مطالعه مورد بررسی قرار گرفتند. نتایج براساس میانگین و انحراف معیار با استفاده از نرم افزار SPSS 18 و استفاده از فرمولهایt-test Mann Whitney، و Chi-Square به دست آمد. در این مطالعهP-value کمتر از 05/0 معنی دار به حساب می آمد.یافته هامیانگین سن بیماران در این مطالعه 8/16±9/55 بود که 73% آن ها مرد بودند. 93% فیستول ها در دست چپ در ناحیه رادیوسفالیک بودند. شایع ترین عارضه فیستول، ترومبوز (8%) بود. رابطه معنی دار بین CRP مثبت و شکست فیستول در این مطالعه به دست آمد. اما ارتباط آماری معنی داری بین ESR با شکست فیستول بدست نیامد، همچنین ارتباط بین هموگلوبین با شکست فیستول معنی دار نبود.نتیجه گیریترومبوز یکی از مهم ترین حادثه های فیستول های شریانی وریدی است که باعث شکست درمان و دیالیز می شود. توصیه می گردد قبل از تعبیه فیستول شریانی وریدی توسط جراح، CRP جهت جلوگیری از شکست بررسی شود.

The aim of this review is to discuss the practical use of duplex ultrasonography in the setting of dialysis vascular access, based on the current evidence and own experience. After explaining the necessary tools and practical considerations, the role of ultrasonography in the placement of dialysis catheters will be described.
