مقدمه
نفروپاتی که از عوارض اصلی دیابت شناخته می شود، شایع ترین علت نارسایی برگشت ناپذیر کلیه است. بیماران دیابتی و دیالیزی مستعد برخی از مشکلات دهانی دندانی مانند بیماری پریودنتال، افزایش جرم، التهاب لثه، عفونت کاندیدیایی، خشکی دهان، از دست دادن زودرس دندانها و لقی دندان می باشند. با توجه به شیوع بالای دیابت و افزایش بیماران مبتلا به نارسایی مزمن کلیوی، مطالعه حاضر با هدف ارزیابی توزیع فراوانی علایم و نشانه های دهانی و دندانی این بیماران انجام گرفت.
مواد و روش هااین مطالعه توصیفی مقطعی، بر روی 118بیمار دیابتی در دو گروه مراحل انتهایی بیماری کلیوی ESRD(End Stage Renal disease) و non-ESRD انجام شد. علایم درد یا سوزش در مخاط دهان با مقیاس (Visual Analogue Scale)VAS و احساس خشکی دهان براساس پرسشنامه استاندارد فاکس توسط بیمار در پرسشنامه ثبت گردید. نشانه های دهان که شامل زبان باردار، زخم مخاطی، کاندیدیازیس و اروژن دندان بودند و هم چنین CPITN (community periodontal index of treatment needs) و DMFT توسط محقق ارزیابی و ثبت شد. اطلاعات به دست آمده در نرم افزار آماری SPSS17 و با استفاده از آزمون های T-test، Chi-squre، ANOVA و Mann-Whitney آنالیز شد.
یافته هارنگ پریدگی مخاط (001/0>P)، احساس درد مخاط (007/0=P) و نیز شاخص CPITN (04/0<P) در گروه non-ESRD به طور معنی داری بیشتر از گروه بیماران تحت درمان دیالیز بود. همبستگی مستقیم و معنی داری بین سن با هریک از متغیرهای DMFT و CPITN در هردو گروه مشاهده شد.
نتیجه گیریبرخی تظاهرات دهانی در بیماران کلیوی و دیالیزی بیشتر مشاهده می شود. همانطور که نتایج نشان داد رنگ پریدگی مخاط، احساس درد مخاط و همچنین مشکلات پریودنتال براساس شاخص CPITN در گروه بیماران دیالیزی به طور قابل توجهی کمتر از گروه دیگر بود.

Diabetic and dialysis patients are prone to some oral dental problems, such as periodontal disease, increased plaque, gingivitis, candidiasis, dry mouth, premature loss of teeth. And mobility of teeth. Due to the high prevalence of diabetes and the increase in patients with chronic renal failure, the present study aimed to evaluate the frequency distribution of oral and dental signs and symptoms.
Materials and MethodsThis cross-sectional descriptive study was performed on 118 diabetic patients in ESRD (End Stage Renal disease) and non-ESRD groups. The patients assessed the symptoms of pain or burning sensation in the oral mucosa and dry mouth using the Visual Analogue Scale (VAS) and Fox questionnaire, respectively. Oral symptoms, including coated tongue, mucosal ulcers, candidiasis, and dental erosion, as well as CPITN (community periodontal index of treatment needs) and DMFT (decayed, missing, and filled permanent teeth), were evaluated and recorded by the researcher. Data were analyzed in SPSS software (version17) using T-test, Chi-squre, ANOVA, and Mann-Whitney tests.
ResultsMucosal pallor, mucosal pain sensation, and CPITN index were significantly higher in the non-ESRD group, as compared to those inthe group of dialysis patients with p-values reported as 0, 0, and 0.04, respectively. Age demonstrated a direct and significant correlation with DMFT and CPITN index in both groups.
ConclusionSome oral manifestations are more common in kidney and dialysis patients. As illustrated by the obtained results, mucosal pallor, mucosal pain sensation, as well as periodontal problems according to CPITN index, were significantly lower in the group of dialysis patients than in the other group.

The current study aimed to evaluate the safety profile and efficacy of a cannabis-based sublingual spray, CBDEX10® (containing 100 µg cannabidiol and 10 µg Δ9-tetrahydrocannabinol per puff; CBD/Δ9-THC 10:1), in improving lipid profile and glycemic state of the diabetic patients. Fifty diabetic patients were randomly allocated to the treatment (n = 25; receiving two puffs of CBDEX10® twice daily) or the control groups (n = 25; receiving two puffs of placebo). The primary endpoint of the study was to evaluate the efficacy of the CBDEX10® adjunctive therapy in improving the lipid profile and glycemic state of diabetic patients; the secondary endpoint was to assess the safety profile and tolerability of the spray. A statistically significant decline in total cholesterol [estimated treatment difference (ETD) = −19.73 mg/dL; P < 0.05], triglyceride (ETD = −27.84 mg/dL; P < 0.01), LDL-C (ETD = −5.37 mg/dL; P < 0.01), FBS (ETD = −12 mg/dL; P < 0.01), HbA1c (ETD = −0.21 mg/dL; P < 0.01) and insulin secretion (ETD = -5.21 mIU/L; P < 0.01) was observable in the patients treated with CBDEX10® at the end of the 8-week treatment period. Regarding safety, the mentioned adjunctive regimen was well, and there were no serious or severe adverse effects. Overall, CBDEX10® sublingual spray could be a new therapeutic agent for lipid and glycemic control in diabetic patients.

اهدافدیابت به عنوان یک بیماری متابولیک شایع می تواند پیامدهای روانی مهمی به دنبال داشته باشد و باعث مشکلاتی در زندگی روزمره شود، لذا استفاده از روش های درمانی برای کمک به افراد مبتلا به این بیماری امری ضروری است. هدف این پژوهش، تعیین تاثیر مداخله مبتنی بر پذیرش و تعهد بر ادراک بیماری و سرمایه روان شناختی افراد مبتلا به دیابت نوع دو بود.مواد و روش هادر این پژوهش نیمه تجربی با طرح پیش آزمون- پس آزمون با گروه کنترل در سال 1394، تعداد 34 بیمار مبتلا به دیابت نوع دو در کلینیک تخصصی دیابت بیمارستان شهید دکتر مصطفی چمران شهر فردوس به روش نمونه گیری دردسترس انتخاب شده و به صورت تصادفی به دو گروه کنترل (17 نفر) و آزمایش (17 نفر) تقسیم شدند. ابزار پژوهش پرسش نامه کوتاه ادراک بیماری و پرسش نامه سرمایه روان شناختی لوتانز بود. مداخله مبتنی بر پذیرش و تعهد در 8 جلسه 60دقیقه ای به صورت هفتگی برای گروه آزمایش به اجرا درآمد. داده ها به کمک نرم افزار SPSS 18 و با استفاده از آمار توصیفی و آزمون تحلیل کوواریانس مورد تجزیه و تحلیل قرار گرفتند.یافته هامداخله مبتنی بر پذیرش و تعهد باعث بهبود معنی دار نمرات ادراک بیماری و کلیه زیرمقیاس های آن (0001/0=p) به جز زیرمقیاس کنترل شخصی و همچنین افزایش معنی دار نمرات سرمایه روان شناختی و زیرمقیاس های آن (0001/0=p) در بیماران مبتلا به دیابت نوع دو پس از تعدیل نمره های پیش آزمون شد.نتیجه گیریمداخله مبتنی بر پذیرش و تعهد می تواند ادراک بیماری و سرمایه روان شناختی افراد مبتلا به دیابت نوع دو را به طور قابل ملاحظه ای بهبود بخشد.کلید واژگان: مداخله, درمان مبتنی بر پذیرش و تعهد, دیابت نوع دو}Aims: As a prevalent metabolic disease, diabetes can be followed by severe mental outcomes leading to problems affecting the daily life. The aim of the study was to determine the effects of acceptance and commitment-based intervention on illness perception and psychological capital in persons with type II diabetes.Materials and MethodsIn the controlled pretest-posttest semi-experimental study, 34 patients with type II diabetes were studied in the Diabetes Clinic of Chamran Hospital of Ferdows City in 2015. The subjects, selected via available sampling method, were randomly divided into two groups including control (n=17 persons) and experimental (n=17 persons) groups. Data was collected by short illness perception questionnaire (IPQ) and Luthans psychological capital questionnaire (PCQ). Eight 60-minute acceptance and commitment-base intervention sessions were weekly conducted in experimental group. Data was analyzed by SPSS 18 software using descriptive statistics and covariance analysis test.
Findings: The pretest score having been adjusted, the acceptance and commitment-based intervention significantly increases the scores of illness perception and its sub-scales (p=0.0001) except the personal control sub-scale. In addition, it significantly increases the scores of the psychological capital and its sub-scales (p=0.0001) in patients with type II diabetes.ConclusionThe acceptance and commitment-based intervention can considerably improve the illness perception and the psychological capital in persons with type II diabetes.Keywords: Intervention, Acceptance, Commitment Therapy, Diabetes Melitus, Type II} -
دیابت شیرین، یک بیماری پیچیده است. در بیشتر افراد مبتلا به این بیماری، دیابت باعث ایجاد یک شیوه جدید زندگی می-شود و تمامی روابط فرد را در همه زمینه ها متاثر می کند و حتی نحوه غذا خوردن و رفتارهای فیزیکی فرد را تحت تاثیر قرار می دهد. اگر قند خون در طولانی مدت به طور مداوم بالا باشد، می تواند بر قلب، چشم ها، کلیه ها، اعصاب و سایر قسمت های بدن تاثیر بگذارد. در درمان دیابت، هدف این است که به افراد کمک شود تا همزمان با کنترل میزان قند خون، خطر ابتلا به عوارض جانبی آن در آینده به حداقل برسد. از زمان های قدیم، گیاهان یک منبع مهم دارو بوده اند. تحقیقات انجام شده بر روی گیاهان در چند دهه گذشته، نشان داده است گیاهانی مانند پیاز و شنبلیله که در گذشته برای درمان دیابت استفاده می شده است دارای خاصیت ضد دیابتی می باشند؛ بنابراین در این پژوهش اثر 15 نوع گیاه بر درمان دیابت مورد بررسی قرار گرفته است.

Researches conducted in last few decades on plants mentioned in ancient literature or used traditionally for diabetes have shown anti-diabetic property of some kinde of plants. IntroductionMetabolic syndrome (M.S.) is a common health problem in the world. Consisting of multiple interrelated risk factors, M.S. increases the risk of atherosclerotic cardiovascular disease by 1.5-3 times, and raises the risk for type 2 diabetes as much as 3-5 times. It affects over 26percent of adults. Patients with M.S are known to be at high risk for heart diseases. The aim of this study was to assess the prevalence of metabolic syndrome and find out about the relationship of this syndrome with coronary heart disease in patients who referred to the Heart Center of Hamedan University in 1387 (H.SH).Materials And MethodsA cross sectional study has been carried out on 1064 patients. We examined all the C.H.D patients (a total of 514) as well as 550 patients without CHD disease. The patients referred to the Heart Center over a period of three months. The patients with three or more of the following symptoms (IDF criteria) were considered to have Metabolic Syndrome: abdominal obesity (waist >102 cm for men; >88 cm for women),increased triglycerides (>150 mg/dl); low HDL-C (<40 mg/dl in men; <50 mg/dl in women), elevated blood pressure >=130/>=85 mmHg, and a raised fasting glucose (>=110 mg/dl).ResultsWe have found significant differences between M.S. and I.H.D. The prevalence of M.S. was rated at 32.5 in patients and 21.3 in the others.. Men comprised 53.7% and women 46.3% of the patients with M.S., 13.6% of who were smokers. The findings of this study indicate that the rate of Hypertriglyceridemia was equal to or over 150 mg/dl in 39.1% of the subjects.ConclusionThe prevalence of M.S. in the sample of I.H.D patients was very high. Also, the correlation between M.S and other risk factors was very significant.
