صفحات 73-83مقدمهاهداء عضو یک تصمیم بسیار سخت برای خانواده ها است. خانواده بیماران مرگ مغزی با مراحل مشکل و آسیب زننده تجربه و درک مرگ مغزی روبرو هستند و همزمان با این بحران با مشکل تصمیم برای اجازه اهداء عضو نیز روبرو می شوند. از آنجا که رسیدن به محتوای درک و ذهن و تجربه افراد با روش کمی معمول کمتر تحقق می یابد (این در حالی است که در اکثر مطالعات برای شناخت مشکلات روان شناختی از روش های کمی و پرسشنامه استفاده می شود) بر آن شدیم که از ترکیب دو روش کمی و کیفی به منظور درک عمیق تر مشکلات روان شناختی خانواده بیماران مرگ مغزی اهدا کننده عضو استفاده نماییم تا بتوانیم گامی کوتاه در کمک به مشکلات بزرگ این افراد برداشته باشیم.مواد و روش هااین مطالعه با روش تلفیق در تحقیق انجام گردید . داده های کمی حاصل ازپرسشنامه ، و دیدگاه خانواده بیماران مرگ مغزی اهدا کننده عضو از طریق مصاحبه مورد بررسی قرار گرفت.یافته هانتایج کیفی در قالب سه مفهوم کلی مجهولات - ترس و تردید- وابستگی و تعالی طبقه بندی شد. هر کدام از این مفاهیم به نمایه های کوچک تری تقسیم شدند. در زیرگروه مجهولات درونمایه های آگاهی، نگرانی، مجهولات، برخورد با افراد و خانواده و ارتباط درمانگر، در زیر گروه ترس و تردید ترس از پشیمانی، احتمال بهبود، عدم تایید دیگران، اعتقادات خاص، افکار وسواسی و در گروه درونمایه تعالی و وابستگی پذیرش واقعیت، نوع دوستی، اعتقادات مذهبی، رشد کردن در شرایط سخت. علایق و توقعات و روابط عاطفی قرار گرفت.
در بررسی نتایج کمی: در مطالعه 224 عضو درجه یک خانواده بیماران مرگ مغزی مورد ارزیابی قرار گرفتند. میانگین سن مشارکت کنندگان در پژوهش 37/13±60/35 سال بود. میانگین سن بیماران 51/21±82/44 سال بود. از بین تمامی ویژگی های دموگرافیک تنها سطح تحصیلات فرد با اختلالات روان شناختی مرتبط بود (05/0p>) و اضطراب فرزندان نسبت به سایر اعضاء خانواده بالاتر بود و اختلاف معنی داری نیز داشت (05/0p>). بیشتر افراد شرکت کننده در مطالعه از سطح بالایی از اضطراب و افسردگی برخوردار بودند. این در حالی بود که افراد با استرس طبیعی بیشتر بودند و بیشتر افراد از نظر نمره کلی پرسشنامه روان شناختی در محدوده طبیعی قرار گرفتند.نتیجه گیرینتایج حاصل از بررسی کمی و کیفی مشکلات روان شناختی خانواده بیماران مرگ مغزی اهدا کننده عضو نشان داد که تجربیات متفاوت خانواده، کمبود آگاهی ، سطوح آسیب زننده اضطراب و افسردگی، حمایت مناسب از خانواده ها در شرایط بحرانی را ضروری می کند، معانی استخراج شده از تجربه خانواده ها نشان داد که تمامی آنها نیازمند همراهی تیم درمان با آنها هستند و در این راه بایستی به آنها کمک شود تا بر مجهولات و ناشناخته های ذهن خود آگاهی پیدا کنند و درک بهتری از این شرایط برایشان فراهم شود و با تقویت روحیه انسان دوستانه در افراد، بروز افکار و عوامل مداخله گر نامناسب در روند پذیرش این بحران کاهش یابد.کلیدواژگان: مرگ مغزی، اهداء عضو، تجربه خانواده، مطالعه کیفی، مطالعه کیفی -
صفحات 84-91مقدمهفرورفتن سوزن در بدن از جمله مشکلاتی است که افراد شاغل در اتاق عمل با آن مواجه هستند که بیشترین احتمال HIV و هپاتیت های C و B منتقله از راه خون، برای پرسنل در آن وجود دارد. هدف از این مطالعه تعیین فراوانی و علل آسیب های ناشی از وسایل نوک تیز آلوده به خون بیمار در کارکنان اتاق عمل بیمارستان های دولتی اهواز است.مواد و روش هااین پژوهش یک مطالعه توصیفی تحلیلی است که بر روی کلیه پرسنل اتاق عمل بیمارستان های امام خمینی، رازی، گلستان و طالقانی اهواز به تعداد 400 نفر انجام گرفت. برای جمع آوری اطلاعات از پرسشنامه محقق ساخته، استفاده شد. تجزیه و تحلیل اطلاعات با استفاده از آزمون های آماری میانگین و انحراف معیار و کای اسکور، در نرم افزار 19spss صورت گرفت.یافته هااز بین 385 نفر مورد مطالعه تعداد 92 نفر(%9/23) هرگز سابقه نیدل استیک شدن را نداشتند، اما 293 نفر (10/76% )1 تا بیش از 5 بار نیدل استیک شده بودند. از دیدگاه کارکنان، عوامل موثر در نیدل استیک شدن عجله کردن (%5/61)، بی احتیاطی همکار (%0/39) و شلوغی بخش (%6/35) است. بیشترین وسیله ای که باعث ایجاد نیدل استیک می شود به ترتیب سوزن بخیه (%4/51)، نیدل سرنگ (%4/37)، تیغ بیستوری (%6/30) است. آزمون آماری کای اسکوئر ارتباط معنی داری بین جنسیت، بیماری روحی، لرزش دست، مشکلات بینایی، سابقه، تعداد شیفت در ماه، تعداد ساعت کار در هفته، مدرک تحصیلی، شغل، فعالیت در یک مرکز درمانی دیگر و تعداد نیدل استیک شدن را نشان داد (05/0>P).نتیجه گیرینتایج این مطالعه نشان داد که بیش از نیمی از کارکنان اتاق عمل با نیدل استیک مواجه هستند. لذا با توجه به عوارض و احتمال ابتلا به بیماری های منتقله از راه خون و بالا بودن میزان صدمات در اتاق عمل، به نظر می رسد تدوین و برگذاری کلاس های آموزشی برای پیشگیری از این مشکلات، ضرورت دارد.کلیدواژگان: کارکنان اتاق عمل، عفونت، آسیب ناشی از سوزن

صفحات 92-97مقدمهیکی از عوارض مهم عمل پرکوتانئوس نفرولیتوتومی، جذب مایع نفروسکوپی و تغییرات الکترولیتی سرم است. هدف از انجام این مطالعه تعیین اثرات آب مقطر طی عمل پرکوتانئوس نفرولیتوتومی بر روی الکترولیت و همودینامیک بیماران بود.مواد و روش هااین مطالعه از نوع کارآزمایی بالینی مداخله ای قبل و بعد بود که روی 90 بیمار PCNL انجام گرفت. برای اطلاع از سطح الکترولیت ها، از بیمار هنگام پذیرش، قبل از نفروسکوپی، هر یک ساعت طی نفروسکوپی، پایان عمل و 24 ساعت بعد نمونه خون گرفته شد و داده ها ثبت گردید.یافته هاحجم مایع مورد استفاده برای نفروسکوپی، با میزان سدیم و پتاسیم سرم پس از شروع نفروسکوپی، یک ساعت بعد و ریکاوری رابطه معنی دار داشت ولی مدت زمان نفروسکوپی در زمان های فوق با سدیم رابطه معنی دار با پتاسیم، ارتباط نداشت. بین حجم و مدت زمان نفروسکوپی با ضربان قلب در شروع نفروسکوپی و دقیقه 10 و با مدت زمان بیهوشی در دقایق شروع، 10، 30 ارتباط معنی دار بود. همچنین حجم مایع شستشو و مدت زمان بیهوشی در دقایق شروع، 20، 30، 40 و زمان شستشو در دقیقه 10 با فشار خون سیستول، ولی حجم مایع و مدت زمان نفروسکوپی در دقایق 30 و 40 و مدت زمان بیهوشی در شروع با فشار خون دیاستول ارتباط معنی دار داشت.نتیجه گیریطبق مطالعه انجام شده با کاهش حجم مایع و مدت زمان نفروسکوپی از هیپوناترمی و هیپوکالمی می توان جلوگیری کرد و ضرورت دارد سطح سرمی سدیم و پتاسیم در شروع نفروسکوپی و ریکاوری بررسی شود.کلیدواژگان: آب مقطر، نفرولیتوتومی از راه پوست، الکترولیت، همودینامیک، نفروسکوپی

صفحات 98-104مقدمهآسیب های تروماتیک مغزی سبب تغییرات قابل توجهی در سطح هوشیاری بیماران می شود. بررسی سطح هوشیاری در بیماران بخش ویژه مشکل است. اولین ابزار ارائه شده برای این کار معیار کمای گلاسکو (GCS) بود اما به دلیل وجود جزء کلامی قابلیت اعتماد به این ابزار کم شده است، اخیرا ابزار FOUR برای بررسی بیماران لوله گذاری شده ارائه شد. این مطالعه با هدف بررسی تاثیر اجراء برنامه تحریکات حسی بر سطح هوشیاری بیماران ضربه مغزی با مقایسه ابزار FOUR و GCS انجام شد.مواد و روش کارمطالعه حاضر یک کارآزمایی بالینی است که روی 60 بیمار ترومایی بخش مراقبت ویژه انجام شد. مداخله شامل ارائه یک برنامه تحریکات حسی منظم توسط پژوهشگر طی 6 روز بود، سطح هوشیاری قبل و بعد از اجراء مداخله با ابزار FOUR و GCS اندازه گیری شد. تجزیه و تحلیل داده ها با استفاده از روش های آمار توصیفی و آزمون های پارامتریک و غیر پارامتریک و با نرم افزارSPSS16 انجام شد.یافته هانتایج نشان داد میانیگن نمرهFOUR قبل و بعد از مداخله در گروه مداخله به طور معناداری تفاوت داشته است اما این تفاوت در مورد نمرهGCS مشاهده نشد، همچنین نمره FOUR و GCS طی 6 روز مداخله افزایش داشته اند که این افزایش در گروه مداخله به طور معناداری از گروه کنترل بیشتر بوده است.نتایجاجراء مداخله تحریک حسی بر سطح هوشیاری بیمار با صدمه مغزی تاثیرات مثبت دارد. همچنین به نظر می رسد معیار FOUR نسبت به GCS در بیماران ترومایی نتایج واضح تری از تغییرات سطح هوشیاری ارائه می دهدو معیار قابل اعتمادتری است.کلیدواژگان: صدمات تروماتیک مغزی، سطح هوشیاری، GCS، FOUR، بخش مراقبت ویژه

صفحات 105-111مقدمهکانولاسیون وریدهای بزرگ مرکزی روش استاندارد کلینیکی برای پایش فشار ورید مرکزی (CVP) است. همچنین کاتتر ورید مرکزی برای اهداف درمانی مختلفی به کار می رود. گایدلاین های کنونی توصیه به قرارگیری نوک کاتتر در اجوف فوقانی (SVC) بالاتر از محدوده سفالیک برگشتگی پریکارد می دهند. هدف این مطالعه تعیین محل دقیق قرارگیری نوک کاتتر ورید مرکزی با مقایسه دو روش مختلف کارگذاری بود.
مواد و روش هل: در این مطالعه مداخله ای، 100بیمار بالغ کاندید عمل جراحی قلب باز CABG که نیاز به کانولاسیون وریدمرکزی داشتند به طور تصادفی به دو گروه تقسیم شدند. گروه اول با روش تخمین عمق کارگذاری کاتتر با روش 15 سانتی متر و گروه دوم با روش C-Length تحت کانولاسیون ورید مرکزی قرار گرفتند. با نرم افزار آماری SPSS و به کارگیری تست های آماری کای اسکوئر و Tبررسی آماری انجام شد و P value<0.05 معنی دار تلقی گردید.یافته هادر گروه 15 سانتی متر 100% کاتترها پایین تر از خط C قرار گرفتند و فاصله نوک کاتتر تا خط C به طور میانگین 10/2±22/4 سانتی متر پایین تر از خط C بود. در گروه C-Length در 26 مورد (52%) کاتتر در پایین خط C قرار گرفت که میانگین فاصله نوک کاتتر تا خط C 5/0±77/0 سانتی متر پایین تر از خط C بود. مقایسه محل قرار گیری نوک کاتتر در دو گروه تفاوت معنی داری را نشان می دهد (P=0.00).نتیجه گیریروش C-Lengthدر مقایسه با روش 15 سانتی متر در تشخیص محل درست نوک کاتتر روش قابل اعتمادتری است و برای جلوگیری از عوارض جای گذاری نامناسب کاتتر روش C-Length روش ارجح تر و ایمن تری است.کلیدواژگان: کاتتریزاسیون، ورید مرکزی، بزرگسال، ونا کاوای فوقانی، رایوگرافی -
صفحات 112-119مقدمهحفظ ثبات قلبی عروقی به دنبال تجویز اتومیدیت سبب شده که این دارو انتخابی مناسب برای بیماران قلبی عروقی باشد. از عوارض اتومیدیت درد محل تزریق است. هدف از مطالعه حاضر مقایسه اثر لیدوکایین و منیزیم سولفات بر درد ناشی از تجویز وریدی اتومیدیت است.مواد و روش هااین مطالعه کارآزمایی بالینی 99 بیمار تحت عمل جراحی الکتیو بجز سزارین در محدوده سنی 60-18 سال به 3 گروه 33 نفره تقسیم شدند. در گروه 1 (شاهد 5 میلی لیتر نرمال سالین)، گروه 2 )30 میلی گرم لیدوکایین 1%( و گروه 3 )48/2 میلی مول منیزیم سولفات( به عنوان پیش دارو تزریق شد، بعد از 3 دقیقه 2/0 میلی گرم / اتومیدیت تزریق شد. بلافاصله درد محل تزریق با مقیاس امبش ثبت شد. داده ها با استفاده از آنالیز واریانس، آزمون توکی و کروسکال والیس با استفاده از نرم افزار SPSS ویرایش نهم تجزیه و تحلیل شدند.یافته هابروز درد در گروه 1 (شاهد) بدون در نظر گرفتن شدت درد 67/66%، در گروه 2 (لیدوکایین) 6/63% و در گروه 3 (منیزیم سولفات) 45/45% بود. میزان درد در گروه 2 (لیدوکایین) از گروه 1 (شاهد) کمتر بود (p=0/03). این میزان در گروه 3 (منیزیم سولفات) نیز در مقایسه با گروه 1 (شاهد) کمتر بود (p=0.015). از نظر میزان درد محل تزریق بین گروه 2 (لیدوکایین) و گروه 3 (منیزیم سولفات) تفاوت معنی داری دیده نشد.(p=0.325)نتیجه گیریبیماران دریافت کننده لیدوکایین و منیزیم سولفات درد کمتری بعد از تزریق اتومیدیت نسبت به گروه شاهد داشتند. همچنین میزان بروز درد در گروه دریافت کننده منیزیم سولفات کمتر از گروه دریافت کننده لیدوکایین بود.کلیدواژگان: درد، اتومیدیت، لیدوکایین، منیزیم سولفات

صفحات 120-125مقدمهدرد بعد از عمل نوعی درد حاد است که درمان آن باعث کاهش موربیدیتی می شود. کاتاراکت شایع ترین علت کوری در کشورهای توسعه یافته است و تنها درمان موثر آن جراحی است. چون چشم عضوی حساس است و اکثر بیماران سالمند هستند وآستانه درد آنها پایین است، لذا کاهش درد ناشی از کاتاراکت حائز اهمیت است.مواد وروش هااین مطالعه بر روی تعداد 48 بیمار زن و مرد با طیف سنی 20 تا 80 سال و ASA 1 و 2 انجام شد. بیماران به طور تصادفی به دو گروه 24 نفره تقسیم شدند، هر 2 گروه تحت بیهوشی عمومی با داروهای یکسان قرار گرفتند ولی در گروه مطالعه بعد از بیهوشی به میزان 1/0 سی سی از لیدوکایین 1% داخل اتاق قدامی چشم تزریق گردید و در گروه شاهد تزریق نشد.
بیماران قبل از شروع عمل و در ریکاوری (زمان صفر)، 1، 2 و 3 ساعت بعد از اتمام عمل ازنظر شدت درد، میزان فشار خون، تعداد ضربان قلب و عوارضی مانند خارش مورد ارزیابی قرار گرفتند.یافته هامیزان درد در گروه مطالعه نسبت به گروه شاهد کمتر بود اما میزان فشار خون و ضربان قلب بین 2 گروه تفاوت چشمگیری نداشت.نتیجه گیریتزریق لیدوکایین داخل اتاق قدامی چشم در کاهش درد ناشی از کاتاراکت موثر است.کلیدواژگان: کاتارکت، لیدوکایین، درد -
صفحات 126-131مقدمهلرز پس از عمل در کنار تهوع و استفراغ پس از عمل یکی از شایع ترین عوارض پس از اعمال جراحی و به ویژه پس از بیهوشی عمومی است. میزان شیوع لرز پس از اعمال غیر کرانیوتومی در 20 تا 60٪ موارد ذکر شده است. لرز پس از کرانیوتومی با توجه به خطر بالاتری که برای بیمار دارد نیاز به درمان های پیشگیری دارد. هنوز میزان شیوع دقیق لرز پس از اعمال جراحی اعصاب به تفصیل بررسی نشده است.هدفتعیین میزان شیوع لرز پس از کرانیوتومی در بیمارانی که تحت جراحی اعصاب کرانیوتومی الکتیو قرار می گیرند.
روش اجرا: تعداد 89 بیمار کاندید جراحی الکتیو کرانیوتومی به صورت توصیفی از نظر شیوع لرز بررسی شدند. بیماران با روش مشابه تحت بیهوشی جنرال قرار گرفتند. اندازه گیری دمای مرکزی از ترمومتر نازوفارنکس و برای دمای محیطی از ترمومتر نوک انگشتان استفاده شد. دمای بیمار و علائم حیاتی قبل از عمل و سپس هر 2 ساعت حین جراحی و بلافاصله پس از پایان عمل ثبت شد. در نهایت در پایان عمل وجود یا عدم وجود لرز ثبت شد.نتایجتعداد 89 بیمار کاندید عمل جراحی کرانیوتومی الکتیو از نظر شیوع لرز پش از عمل بررسی شدند که میانگین سنی4/53±6/14 داشتند و میانگین طول عمل 6/2± 02/5 بود.میزان شیوع لرز روند دمای مرکزی و محیطی کاهش سریع در شروع وکاهش خطی و آهسته در ادامه و رسیدن به یک پلاتو را نشان داد. در این بیماران میانگین دمای مرکزی در ریکاوری در بعد از عمل (56/0±2/34) به طور معنی داری نسبت به قبل از عمل (65/0±1/37) کاهش داشت (p=0.0001). میانگین دمای محیطی بعد از عمل (65/0±6/34) نیز به طور معنی داری نسبت به قبل از عمل (72/0±2/37) کاهش یافت (p=0.0001).نتیجه گیریمیزان شیوع لرز پس از عمل در بیماران بعد از کرانیوتومی 15٪ است. میزان شیوع لرز پس از کرانیوتومی علی رغم افت واضح دمای مرکزی و محیطی در بیماران و طول عمل زیاد کمتر از اعمال جراحی دیگر است.کلیدواژگان: لرز پس از عمل، کرانیوتومی، شیوع - گزارش مورد

صفحات 132-13خانمی 27 ساله با حاملگی 12 هفته و طبقه بندی کلاس 1ASA برای عمل جراحی سر کلاژ گردن رحم در فهرست عمل جراحی قرار داده شده بود. برای بیمار بیهوشی نخاعی انتخاب شد و با سوزن شماره 25 و تزریق 100 میلی گرم (2 میلی لیتر) لیدوکایین پنج درصد در وضعیت نشسته صورت گرفت. بعد از دو روز بیمار دچار درد کمر با انتشار به پای چپ شد. درد شدید و همراه احساس مورمور شدن (پارستزی) بود و برای یک ماه ادامه یافت و سپس به تدریج رفع گردید. معاینه عصبی و نوار عصب و عضله طبیعی بودند.کلیدواژگان: بیهوشی نخاعی، لیدوکایین، درد، پارستزی

Pages 73-83IntroductionOrgan donation is a very difficult decision for families. Families of brain death patients are facing difficult and damaging stages of experiencing and understanding the brain death, and at the same time they also encounter the crisis of decision about organ donation. Quantitative methods and questionnaires have been used for identification of psychological problems in most studies, but perception of peoples mentality and experiences with common quantitative approach is not possible. Thus we decided to combine both quantitative and qualitative methods for deeper understanding of the psychological problems of donors family, to help them in making this difficult decision.Materials And MethodsThis study was performed by combining investigations. Quantitative data and donors family view were evaluated through questionnaire and interviews, respectively.ResultsQualitative results were classified in three general concepts: unknowns-fear, doubt-dependency, and excellence. Each of these concepts was divided into smaller profile. Knowledge, concerns, unknowns, dealing with family and the therapist relationship were in the subgroup of unknowns; fear of regret, the chances of recovery, rejection of others', beliefs, and obsessive thoughts were in the subgroup of doubt and fear; and acceptance of reality, altruism, religious beliefs, growing up in difficult circumstances, interests and expectations, and emotional relationships were in the subgroup of development and dependence. In the quantitative results members of the families of 224 brain death patients were evaluated. The mean age of the participants in this study was 37/13 ± 60/35 years. The mean age of patients was 51/21 ± 82/44 years. Among all the demographic characteristics of individuals only the level of education was associated with psychological disorders (P> 0.05). Childrens anxiety was also significantly higher than the other family members (PConclusionQuantitative and qualitative results of the psychological problems of families of brain-dead organ donors showed that because of different experiences of families, lack of awareness, harmful levels of anxiety and depression, it is necessary to appropriately support families in this crisis situations. All of them are in need of medical team sympathy and in this way we must help them to find an awareness of their unknowns, thus better understanding of these conditions is provided. By strengthening the humanitarian morale of the people, the incidence of inappropriate thoughts and confounding factors in the adoption of this crisis would reduce.Keywords: Brain death, Organ donation, Family experience, Qualitative research, Quantitative research

Pages 84-91BackgroundNeedle stick the body, including the problems faced by those working in the operating room with the greatest likelihood of HIV and hepatitis C and B blood-borne, the staff there so The aim of this study was to determine the frequency and causes of injuries from sharp instruments contaminated by blood in the operating room staff at public hospitals in Ahwaz.Materials And MethodsThis study is a crosssectional study on Imam Khomeini, Razi, Golestan and Taleghani hospital operating room personnel was performed in 400 subjects. For data collection questionnaire was used. Analyze data using mean and standard deviation and Chi square test, the software was 19spss.ResultsOf the 385 patients studied, 92 subject (23.9%) did not have a history of needle stick, but 293 subject (76.10%) were needle stick 1 to more than 5 times. From the employee's perspective, the factors affecting the rush needle stick (61.5%), careless partner (39.0%) and the bustle% (35.6%) are. In order to collect data on electrolyte levels, blood samples were taken on admission, before irrigation, every hour during irrigation, immediately and 24 hours after the procedure, and the data were recorded.ResultsThere was a significant relationship between the irrigation fluid volume and serum sodium and potassium levels after the start of the procedure, one hour later and during recovery; however there was a significant correlation between irrigation time and sodium level but it was not the same with potassium.There was a significant correlation between irrigationfluid volume and time and heart rate before, at 10 minutes after the irrigation and also between irrigation fluid and time and duration of anesthesia before, at 10 and 30 minutes after the irrigation. There was also a significant correlation between irrigation fluid volume and duration of anesthesia immediately before, and at 20, 30, 40 minutes after; as well as between irrigation time and systolic blood pressure 10 minutes after the irrigation . However ,irrigation fluid volume and time were significantly correlated with diastolic blood pressure at 30 and 40 minutes after the procedure and there was a significant correlation between the duration of anesthesia and diastolic blood pressure immediately before the procedure.ConclusionAccording to the present study, hyponatremia and hypokalemia can be avoided by reducing the irrigation fluid volume and time; the serum level of sodium and potassium must be checked immediately before the irrigation and during recovery.Keywords: Water, percutaneous nephrolithotomy, electrolytes, hemodynamics, irrigation

Pages 98-104IntroductionTraumatic brain injury makes singnificant change in the level of consciousness (LOC). Such patients, due to different reason, are at risk of sensory deprivation. It is difficult to assess the LOC of patients in ICU. The first scale to do such a thing is GCS which is of low validity because of verbal component. The FOUR has more information than GCS and seems to be better in intubated patients. The aim of the study was to determine effect of sensory stimulation program on LOC, with the a camparison of FOUR and GCS scales.
Material andMethodThis study is a clinical trial on 60 patients who had brain injury in ICU. The intervention aregular sensory stimulation by the reasarcher which is done on 6 day and 5 times a day and LOC were measured before and after intervention using both FOUR and GCS. The data analyses was done by SPSS 16.0 and using parametric and nonparametric tests and descriptive statistics method.ResultsResults showed that the mean of FOUR was significantly different before and after intervention but this finding was not observed with the GCS scales showed a rise during 6 days of intervention and this increase was significantly higher compared to the control group.ConclusionSensory stimulation program in patients with traumatic brain injury has positive effects. Pages 105-111IntroductionCentral vein catheterization is a standard clinical method for central venous pressure monitoring besides being used for other therapeutic purposes. The present guidelines recommend to place the catheter tip in the superior vena cava above the pericardial cephalic reflection. The aim of this study was to compare the accuracy of two different approaches in locating the tip of the CVC at the suggested vascular zone.
Material andMethodsThis was an interventional study on 100 patients undergoing CABG operation who required a central venous cannulation. They were randomly assigned into two groups. In the first group catheter placement was applied through using the conventional 15 cm method. In the second group a Clength method was applied for measuring the depth of catheter tip insertion from the preoperative chest radiographs. For statistical analysis Chi-square test and T-test were used (SPSS v.22 software).ResultsIn the first group (15 cm)100% of the patients had their catheters placed below the C-line (Carina line) and the average distance between the catheter tip and the C-line was 4.22±2.10 cm. In the second (CLength) group 52% of the catheters were below C-line with an average distance of 0.77±0.5 cm. There was a meaningful difference between the two groups in respect to the catheter location depth and zone placement. The incidence of pain on injection is a side effect of using Etomidate.The purpose of this study is to compare the efficacy of Lidocaine versus Magnesium sulfate on pain associated with intravenous administration of Etomidate .Materials And MethodsIn this clinical trial study, 99 patients undergoing elective surgery aged 18-60 years were divided into 3 groups: group 1 (control) without medication, group 2 received 30 mg of lidocaine 1% and group 3 received 2.48 mMol of magnesium sulfate. After 3 minutes, 0.2mg/kg etomidate was injected. The severity of injection pain was immediately evaluated and recorded using the scale of Ambesh et al. Data were analyzed using ANOVA, Tukey and Kruskal-Wallis tests by SPSS software version 9.0.ResultsThe incidence of pain in group I (control), regardless of the intensity of pain, was 66/67%; in group II (lidocaine) 63/60% and in group III (magnesium sulfate) 45/45%, respectively. Pain in group II (receiving lidocaine) was lower than in group I (control) (p=0.030) and also lower in Group III (receiving magnesium sulfate) as compared to group I (control) (p=0.015). In terms of pain at the injection site after receiving lidocaine and magnesium sulfate, no significant differences were observed between groups II and III (p=0.325).ConclusionPatients receiving lidocaine and magnesium sulfate had less pain on injection with etomidate compared to the control group. Cataract is the most common cause of blindness in developed countries and the only effective treatment is surgery. Because the eye is a sensitive organ and most patients are elderly and with low pain threshold, therefore, it is important to reduce the pain caused by cataract surgery.Materials And MethodsThe study included 48 male and female patients with an age range of 20 to 80 years and ASA1 & 2. Patients were divided randomly to two groups of 24 person, each of the 2 groups were under general anesthesia with equal drugs, in the study group 0/1 cc lidocaine1% intra cameral was injected after anesthesia and the control groups did not receive it. Patients were assessed before surgery and recovery (time zero), 1, 2 and 3 hours after surgery in terms of pain, blood pressure, heart rate and effects such as itching.ResultsThe study group had lower pain than the control group, but blood pressure and heart rate were not significantly different between the 2 groups.ConclusionThe injection into the anterior chamber is effective in reducing pain after cataract surgery.Keywords: Cataract, lidocaine, pain

Pages 126-131IntroductionShivering is an early postoperative complication following neurosurgical procedures. Incidence of postoperative shivering (POS) following non-craniotomy surgeries varies between 20 to 60%. POS after craniotomy is considered high risk and requires preventive therapies. However, incidence of postoperative shivering following craniotomies is not reported in previous reports.
Aim: To determine the incidence of postoperative shivering following craniotomy.Materials And MethodsIn a descriptive study, 89 patients candidate for elective craniotomy were enrolled and followed for incidence of POS. Method of anesthesia were similar in all cases. Patients were covered with 3 layers of surgical drapes. Shivering was evaluated during the postoperative period by observation of shivering movements of more than 10 seconds duration. Core (nasopharynx) and skin temperature (dorsum of middle finger) were recorded. Patients temperature were measured prior to anesthesia and then every 2 hours during and after surgery.ResultsMean age was 53.4±14.6 and duration of surgery was 5.02±2.6. Incidence of post-craniotomy shivering was 15%. Trend of decrease in core and peripheral temperature was fast reduction at the early 2 hour of anesthesia, the slow decrease and then it reached a plateau phase. Core temperature at preoperative time (37.1±0.65) was significantly higher than in postoperative (34.2±0.56) (p=0.0001). Besides, mean peripheral temperature in preoperative period (37.2±0.72) was significantly higher than in postoperative period (34.6±0.65) (p=0.0001).ConclusionIncidence of post-craniotomy shivering was 15% in our patients. Although decrease in core and peripheral temperature were significant during craniotomy, however, the incidence of post-operative shivering is lower than other surgeries.Keywords: Postoperative shivering, Incidence, craniotomy -

Spinal anesthesia was selected and induced with spinal needle No:25 and 100 mg (2ml) lidocaine 5% in sitting position .After 2 days , the patient sensed lumbar pain with radiation in her left foot. The pain was sever and coexisted with paresthesia and continued for 1 month and then suppressed slowly.
