آسیب کروماتین و DNA اسپرم ممکن است در بیضه، مجراهای تناسلی و همچنین پس از انزال رخ دهد. مکانیزم تغییر در ساختار کروماتین و آپوپتوز نافرجام و استرس اکسیداتیو ناشی از گونه های اکسیژن فعال (ROS) می باشد. عوامل داخل بیضه ای، پس از بیضه و عوامل خارجی با افزایش سطح آسیب به DNA اسپرم انسان می تواند احتمال باروری را تحت تاثیر قرار دهد. شیوه زندگی، محیط زیست، پزشکی و عوامل درمانزاد ممکن است به عنوان عوامل تخریب کننده در نظر گرفته شوند که باعث اختلالات اندوکرین می شوند. در نتیجه، این تغییرات ممکن است با تاثیر مخرب بر کروماتین /DNA در سلول های زایا، در میان نسل ها انتقال پیدا کنند و باعث با عواقب فنوتیپی گردند. میزان هیستون باقی مانده و کمبود پروتامین ممکن است با مصرف الکل در اسپرم افزایش نیابد. با این حال، باعث افزایش درصد اسپرم با DNA قطعه قطعه شده و آپوپتوز در اسپرم می شود. آسیب نخاعی به عنوان یک مشکل پزشکی، می تواند پارامترهای اسپرم را تضعیف کرده و باعث آسیب قابل توجه DNA دراسپرم در این مردان شود. همچنین عفونت می تواند تولید ROS را افزایش دهد و، کاهش ظرفیت آنتی اکسیدانی و قطعه قطعه شدن DNA اسپرم و یا تولید آنتی ژن که منجر به اختلالات اسپرم و قطعه قطعه شدن DNA می گردد را را القا کند. در حالی که ROS با افزایش سن افزایش می یابد و استرس اکسیداتیو ممکن است مسیول آسیب DNA اسپرم وابسته به سن مردان مسن تر نسبت به جوان ترها باشد. بررسی کیفیت کروماتین اسپرم درموارد سرطان بیضه و بیماران مبتلا به لنفوم هوچکین قبل از شیمی درمانی شیوع بالای آسیب به DNA و تراکم کم درکروماتین اسپرم در زمان تشخیص را نشان داده است. در سیکل شیمی درمانی با عوامل ژنوتوکسیک در بیماران مبتلا به سرطان، افزایش آسیب DNA اسپرم بعد از درمان نشان نیزداده شد. بطور کلی فاکتورهایی که باعث تغییر توزیع پروتئین در کروماتین اسپرم می گردد می توانند در دوران جنینی و یا بزرگسالی بر سلولهای زایای اسپرم ساز تاثیر بگذارند که ناباروری را در بیماران القا کند.

شواهد اندکی در مورد خاصیت آنتی اکسیدانی ویتامین D وجود دارد. مطالعات اخیر پیشنهاد می کنند که استرس اکسایشی می تواند در پاتوفیزیولوژی PCOS ایفای نقش کند، اما ارتباط ویتامین D و استرس اکسایشی در PCOS هنوز شناخته شده نیست.
هدفهدف اصلی مطالعه کنونی ارزیابی همبستگی بین سطح ویتامین D سرم با شاخص های استرس اکسایشی در گروه PCOS و مقایسه آن با گروه شاهد است.
موارد و روش ها60 زن مبتلا به PCOS (20 تا 40 سال) و 90 زن سالم به عنوان گروه شاهددر این مطالعه موردی- شاهدی شرکت نمودند. سطح سرمی ناشتا 25-هیدروکسی ویتامین D، قند خون،انسولین،کلسیم، مالون دی آلدهید (MDA)، پروتئین کربونیل (PC) و همچنینمدل همیوستاز ارزیابی مقاومت به انسولین (HOMA-IR) و نسبت گلوکز ناشتا به انسولین (FGIR) محاسبه گردید.
نتایجمیانگین ویتامین D سرم در گروه PCOS (17/4±76/10) از گروه شاهد (26/6±07/12) پایین تر بود، ولی این تفاوت معنادار نبود (125/0=p). میانگین انسولین ناشتا، HOMA-IRوMDA به طور معناداری در گروه PCOS بالا بود، در حالیکه سطح PC بین دو گروه تفاوت معناداری نداشت (156/0=p). و همبستگی معناداری بین 25- هیدروکسی ویتامین D سرم و شاخص های استرس اکسایشی (PC، MDA) مشاهده نشد.
نتیجه گیرینتایج این مطالعه حاکی از عدم تفاوت معنادار سطح سرمی ویتامین Dبین بیماران PCOS و گروه شاهد همسان بود و نیز همبستگی بین سطح سرمی ویتامین D و شاخص های استرس اکسایشی در دو گروه مشاهده نشد.

هدف از این مطالعه مقایسه اثربخشی آگونیست هورمون آزاد کننده گنادوتروپین (GnRH) و گنادوتروپین جفتی انسانی (HCG) بر بلوغ نهایی تخمک و میزان حاملگی درچرخه های تلقیح داخل رحمی (IUI) بود.
موارد و روش هادر این کارآزمایی بالینی تصادفی شده، 110 زن نابارور که برای درمان IUI انتخاب شدند وارد مطالعه شدند. تحریک تخمک گذاری انجام شد. گروه یک 0.1 میلی گرم آگونیست GnRH به عنوان عامل تحریکی و گروه دوم نیز 10،000 واحد HCG دریافت کردند. سطح سرمی E2، LH، FSH در 12 و 36 ساعت پس از تزریق اندازه گیری شد.
نتایجفوران LH در تمام بیماران تشخیص داده شد. سطح LH در 12 و 36 ساعت پس ازتحریک نهایی تخمک گذاری در گروه یک بالاتر بود و زودتر از گروه دوم از خون شسته شد (00/0=p). میزان بارداری در گروه یک بالاتر بود، اما تفاوت آماری معنی دار نبود (9/26% در مقابل 8/20%، 46/0=p). همچنین بروز سندرم تحریک بیش از حد تخمدان در دو گروه متفاوت نبود (11/0=p). اختلاف معنی داری از نظر سطح سرمی استرادیول در 36 ساعت بعد از تحریک نهایی تخمک گذاری وجود داشت (000/0=p).
نتیجه گیریاثر آگونیست هورمون آزاد کننده گنادوتروپین در افزایش LH درونی برای آزاد سازی تخمک و بلوغ نهایی فولیکولی کافی است. میزان بارداری و بروزسندرم تحریک بیش از حد تخمدان بین گروه ها متفاوت نبود. ما استفاده از آگونیست های GnRH رابه عنوان یک گزینه جایگزین به جای HCG در چرخه های تلقیح داخل رحمی پیشنهاد می کنیم.

تریکومونال واژینالیس و نیسریا گونوره آ از جمله شایعترین عوامل غیر ویروسی عفونت های مقاربتی در جهان اند. اطلاعاتی همه گیری شناسی از عفونت های تناسلی در زنان استان قم در دسترس نیست. قم مقصد هزاران مهاجر است که سالانه از کشورهای مختلف به این استان سفر می کنند.
هدفهمه گیری شناسی عوامل باکتریال و تک یاخته ای عفونت های تناسلی در زنان مراجعه کننده به بیمارستان مرکزی زنان و زایمان قم مورد بررسی قرار گرفته است.
موارد و روش هااز ترشحات دستگاه تناسلی زنان داوطلب مراجعه کننده به کلینیک مرکزی زنان استان قم نمونه برداری انجام شد و سواب ها برای انجام آزمایش های مرسوم میکروبی، برای LightCycler Real-time PCR مبتنی بر شناسایی ژن proA (تشخیص نیسریا) و برای PCR مبتنی بر شناسایی ناحیه ITS (تشخیص تریکوموناس) استفاده شد.
نتایجاز میان 420 داوطلب زن که وارد مطالعه شدند 277 نفر (9/65٪) علایم و نشانه های بیماری داشتند که از جمله شامل 3/38٪ ترشحات بدبو، 9/37٪ مقاربت دردناک و 8/54٪ درد زیر شکم می شود. در مجموع 2 ایزوله استرپتوکوک آگالاکتیه تشخیص داده شد. همچنین، 5 نمونه (2/1٪) در کشت تایر مارتین و 17 نمونه (1/4٪) در real-time PCR بعنوان نیسریا گونوره آ تشخیص داده شدند. جمعا 54 نمونه (9/12٪) در گسترش مرطوب، 64 نمونه (2/15٪) در کشت دورسه و 81 نمونه (3/19٪) در ITS-PCR بعنوان تریکوموناس واژینالیس تشخیص داده شدند. تعداد 5 نمونه عفونت مخلوط تریکوموناس و نیسریا مشاهده شد. خطر ابتلا به تریکومونیازیس در زنان با سابقه وزن کم تولد (29/43 OR، 007/0>p)، سقط (84/91 OR، 000/0>p) و پارگی کیسه آب (75/21 OR، 009/0>p) افزایش داشت. احتمال مشاهده نوتروفیل ها (34/43 OR، 007/0>p) در گسترش ترشحات واژن در عفونت با تریکوموناس واژینالیس افزایش داشت.
نتیجه گیریشیوع قابل توجه عفونت تریکومونیازیس و گونوره آ بر ضرورت تشخیص صحیح و مراقبت موثر برای پیشگیری از عوارض شدید باروری در زنان تاکید می کند.

بهبود سلامت زنان از طریق استفاده از روش های پیشگیری از بارداری جدید، استراتژی کلیدی برای جلوگیری از بارداری ناخواسته و عوارض آن است. با این حال، شواهد محدودی در مورد استفاده از روش های مدرن ضد بارداری در منطقه مورد مطالعه وجود دارد.
هدفاین مطالعه عوامل موثر بر استفاده از روش های مدرن پیشگیری از بارداری در زنان در سنین باروری در مناطق روستایی منطقه دمیبا را در سال 2015 شناسایی کرد.
موارد و روش هامطالعه مقطعی در جامعه در سال 2015 در منطقه دمیبا انجام شد. روش انتخاب چند مرحله ای برای انتخاب 616 شرکت کننده در این مطالعه بود. مدل رگرسیون لجستیک دو متغیره و چند متغیره برای شناسایی عوامل مرتبط با استفاده از پیشگیری از بارداری مدرن مورد استفاده قرار گرفت. نسبت شانس با فاصله اطمینان 95٪ (CI) برای تعیین سطح معنی داری محاسبه شد.
نتایجاستفاده از روش های مدرن پیش گیری از بارداری 7/31٪ CI 95٪ (3 / 28-35 / 3) بود. سن (نسبت شانس تنظیم شده (AOR): 1.94 (95٪ CI: 1.170-3.216)]، زنان دارای شوهر تحصیلی [AOR: 0.28 (95٪ CI: 0.617-0.166-0.666)، وضعیت زناشویی [AOR: 2.81، (95٪ CI: 1.344-5.855)] و اعلان های همسر درباره مسایل مربوط به برنامه ریزی خانواده [AOR: 2.58 (95٪ CI: (1.276-5.202)] عوامل مرتبط با استفاده از روش های پیشگیری از بارداری مدرن بودند.
نتیجه گیریاستفاده از روش های ضد بارداری مدرن کم بود. ارایه فرصت های آموزشی، ایجاد آگاهی در مورد روش های پیشگیری از بارداری و مشاوره موثر، می تواند استفاده از روش های پیشگیری از بارداری مدرن را افزایش دهد.

سندرم تخمدان پلی کیستیک (PCOS) یا سندرم استین-لونتال یک بیماری شایع پیچیده در زنان است. عوامل ژنتیکی و محیطی همراه با PCOS و عدم توازن هورمونی در این بیماران می تواند در روند تولید مثل، سوخت و ساز(متابولیک) بدن و ویژگی های روانی، اختلال ایجاد نماید. مطالعات انسانی و تجربی نشان می دهد که فعالیت سیستم سمپاتیک در تخمدان ممکن است ناهماهنگ (بیش فعالی) شود. عوامل متعددی در تنظیم تخمک گذاری و روند لوتیینه شدن (luteinization) دخالت می نمایند. ترشح ناکارآمد IL-1 و یا سیتوکین های مرتبط دیگر ممکن است سبب تخمک گذاری و لوتیینه شدن غیر طبیعی گردند. نوروهورمونهای استرس (نورآدرنالین و کورتیزول) از اجزای یکپارچه و تنظیم کننده های اصلی برای پاسخ های ایمنی متفاوت، بشمار می آیند.
هدفهدف ما از این مطالعه 1) بررسی الگوی سایتوکین های مورد نظر در زنان مبتلا به تخمدان پلی کیستیک 2) توضیح مکالمه متقابل بین دو سیستم بزرگ سمپاتیک و ایمنی و 3) سیگنالینگ یا پیام رسانی روانی اینترلوکین ها در سندرم تخمدان پلی کیستیک می باشد.
موارد و روش هادر این مطالعه 171 نفر به دو گروه PCOS و کنترل تقسیم شدند. غلظت سایتوکین های سرمی شامل: IL-17A، IL-1α، IL-1β و TNFα در دو گروه به روش الیزا اندازه گیری شد.
نتایجسطوح سرمی(0/001>IL-1 α (p و (0/017=IL-1β (p در گروه PCOS بالاتر از گروه کنترل بود. اما سطح سرمی IL-17A در گروه PCO بطور معنی دار پایین تر از گروه کنترل بود (0/001˂p).
نتیجه گیریالتهاب مزمن خفیف در زنان مبتلا به تخمدان پلی کیستیک می تواند در نتیجه تغییر در الگوی سایتوکین ها باشد. اما کاهش معنی دار IL-17A در زنان مبتلا به تخمدان پلی کیستیک می تواند در نتیجه لوکالیزه یا تمرکززایی تنظیم IL-17A توسط نوروهورمون های استرس در این بیماران باشد. این نتایج تایید دیگری بر التهاب مزمن بودن سندرمتخمدان پلی کیستیک است.

Here, we collected the papers of the session entitled: Stem Cells, Good manufacturing practice, and tissue engineering, that presented and discussed at this meeting by the international and national experts of the overlaps of the fields of stem cells and reproductive medicine, and the translation of these efforts towards practical application in regenerative medicine.

The sperm DNA damage may occur in testis, genital ducts, and also after ejaculation. Mechanisms altering chromatin remodeling are abortive apoptosis and oxidative stress resulting from reactive oxygen species. Three classifications of intratesticular, post-testicular, and external factors have been correlated with increased levels of sperm DNA damage which can affect the potential of fertility. Alcohol consumption may not increase the rate of sperm residual histones and protamine deficiency; however, it causes an increase in the percentage of spermatozoa with DNA fragmentation and apoptosis. In a medical problem as spinal cord injury, poor semen parameters and sperm DNA damage were reported. Infection induces reactive oxygen species production, decreases the total antioxidant capacity and sperm DNA fragmentation or antigen production that lead to sperm dysfunctions and DNA fragmentation. While reactive oxygen species generation increases with age, oxidative stress may be responsible for the age-dependent sperm DNA damage. The exposing of reproductive organs in older men to oxidative stress for a long time may produce more DNA-damaged spermatozoa than youngers. Examining the sperm chromatin quality in testicular cancer and Hodgkins lymphoma patients prior to chemotherapy demonstrated the high incidence of DNA damage and low compaction in spermatozoa at the time of diagnosis. In chemotherapy cycles with genotoxic agents in cancer patients, an increase in sperm DNA damage was shown after treatment. In overall, those factors occurring during the prenatal or the adult life alter the distribution of proteins associated with sperm chromatin induce changes in germ cells which can be detected in infertile patients.

There is little evidence about antioxidant properties of vitamin D. Recent studies suggest that oxidative stress may play a major role in the pathophysiology of polycystic ovary syndrome (PCOS), but the association of vitamin D with oxidative stress is still not known in PCOS.
ObjectiveThe goal of the present study was to evaluate the correlation between serum 25-hydroxy vitamin D and oxidative stress markers in PCOS group compared to control group.
Materials And Methods60 PCOS women (20-40 yr old) and 90 healthy women as control group were participated in this case-control study. Fasting serum level of 25-hydroxy vitamin D 25(OH) D, glucose, insulin, calcium, malondialdehyde (MDA), protein carbonyl (PC), also homeostasis model assessment for insulin resistance (HOMA-IR) and fasting glucose to insulin ratio (FGIR) were measured.
ResultsIt was found that the mean of serum 25(OH)D was lower in the PCOS group (10.76±4.17) than in the control group (12.07±6.26) but this difference was not statistically significant (p=0.125). Fasting insulin, HOMA-IR and MDA were significantly higher in the PCOS patients as compared to the controls, whereas PC level did not differ for the two groups (p=0.156). No significant correlations were found between 25(OH) D levels and oxidative stress markers (MDA and PC).
ConclusionThe findings indicated no significant differences in the serum 25(OH) D levels between the PCOS patients and the matched controls. Also, no correlation was found between the serum vitamin D levels and oxidative stress markers in both groups.

The purpose of triggering in ovulation induction is to induce the final maturation of oocytes and their release from the ovary for fertilization.
ObjectiveThe aim of the present study was to compare the effectiveness of gonadotropin-releasing hormone (GnRH) agonist and human chorionic gonadotropin (HCG) on the final maturation of oocytes and pregnancy rates in intrauterine insemination (IUI) cycles.
Materials And MethodsIn this randomized clinical trial, 110 infertile women who were selected for IUI entered the study. Ovulation induction was performed. Group I received 0.1 mg GnRH agonist as triggering and group II received 10,000 IU of HCG. The serum Estradiol (E2), Luteinizing Hormone (LH), and Follicle-Stimulating Hormone (FSH) levels were measured at 12 and 36 hr after injection.
ResultsLH surge was detected in all patients. LH levels at 12 and 36 hr after triggering was higher in Group I and it washed out earlier than group II (p=0.00). The pregnancy rate was higher in Group I, but the difference was not statistically significant (26.9% vs. 20.8%, respectively p=0.46). Also, the incidence of ovarian hyperstimulation syndrome was not different between the two groups (p=0.11). There was a significant difference regarding the estradiol levels at 36 hours after triggering (p=0.00).
ConclusionEffects of GnRH on endogenous LH surge is sufficient for oocyte releasing and final follicular maturation. Pregnancy rates and ovarian hyperstimulation syndrome incidence were not different between the groups. We suggest that GnRH agonists might be used as an alternative option instead of HCG in IUI cycles.

Trichomonas vaginalis (T.vaginalis) and Neisseria gonorrhoeae (N.gonorrhoeae) are two most common non-viral sexually transmitted infections in the world. No data are available regarding the epidemiology of genital infections in women of Qom, central Iran.
ObjectiveEpidemiological investigation of sexually transmitted infections in genital specimens of women referred to the referral gynecology hospital in Qom, central Iran.
Materials And MethodsGenital swab specimens were collected from women volunteers and used for identification of bacterial and protozoal infections by conventional microbial diagnostics, porA pseudo gene LightCycler® real-time PCR (for N.gonorrhoeae) and ITS-PCR (for T.vaginalis).
ResultsOf 420 volunteers, 277 (65.9%) had genital signs/symptoms, including 38.3% malodorous discharge, 37.9% dyspareunia, and 54.8% abdominal pain. Totally, 2 isolates of Streptococcus agalactiae were identified. Five specimens (1.2%) in Thayer-Martin culture and 17 (4.1%) in real-time PCR were identified as N.gonorrhoeae. Fifty-four specimens (12.9%) in wet mount, 64 (15.2%) in Dorsets culture, and 81 (19.3%) in ITS-PCR showed positive results for T.vaginalis. Five mixed infections of T.vaginalis N.gonorrhoeae were found. The risk of T.vaginalis infection was increased in women with low-birth-weight (p=0.00; OR=43.29), history of abortion (p=0.00; OR=91.84), and premature rupture of membranes (PROM) (p=0.00; OR=21.75). The probability of finding nuclear leukocytes (p=0.00; OR=43.34) in vaginal smear was higher in T.vaginalis infection.
ConclusionThe significant prevalence of trichomoniasis and gonorrhea emphasizes the need for accurate diagnosis and effective surveillance to prevent serious reproductive complications in women.

Improving womens health through modern contraceptive methods utilization is the key strategy to prevent unwanted pregnancy and its complication. However, there was limited evidence on utilization of modern contraceptive methods in the study area.
ObjectiveThis study identified factors affecting utilization of modern contraceptive methods among women at reproductive age group in rural areas of Dembia district, 2015.
Materials And MethodsCommunity based cross sectional study was conducted in 2015 in Dembia District. Multi-stage sampling technique was used to select a total of 616 study participants. Bivariate and multivariable logistic regression model was fitted to identify factors associated with modern contraceptive utilization. Odds ratio with 95% confidence interval (CI) was computed to determine the level of significance.
ResultsModern contraceptive methods utilization was found to be 31.7% 95%CI (28.0-35.3). Age [Adjusted odds ratio (AOR): 1.94, (95%CI: 1.170-3.216)], women who has educated husband [AOR: 0.28, (95%CI: 0.117-0.666)], Marital status [AOR: 2.81, (95%CI: 1.344-5.855)] and Spousal announcement about family planning issues [AOR: 2.58, (95%CI: (1.276-5.202)] were factors associated with modern contraceptive methods utilization.
ConclusionModern contraceptive methods utilization was found to be low. Providing educational opportunities, creating awareness about contraception and effective counseling would increase modern contraceptive methods utilization.
