    پلاکتها نقش مهمی در پاتوژنز انسداد عروق کرونری دارند. سندرم های حاد کرونری با آسیب آندوتلیال، پارگی پلاک آترواسکلروتیک، فعال شدن پلاکت ها، تشکیل ترومبوز و درنهایت انسداد عروق همراه هستند. پلاکت ها با اتصال به ساختار زیر آندوتلیوم فعال می شوند و گرانولهای حاوی ترومبوکسان A2، سروتونین و عوامل منقبض کننده عروق و تجمع پلاکتی رها می کنند. افزایش فعالیت پلاکتی با افزایش شدت بیماری ایسکمیک قلبی همراه بوده است. نشان داده شده که بیماران با ایسکمی قلبی که شمارش پلاکتی بالاتری دارند مستعد مرگ و میر بیشتری هستند. مطالعه شفاهی و همکارانش در سال 2005 نشان داد که تغییرات شمارش پلاکتی در افراد مبتلا به سندرم کرونری حاد معنی دار است؛ به گونه ای که در این افراد تعداد پلاکت ها به علت مصرف شدن در ناحیه ضایعه کاهش می یابند. به عبارتی بیماران با سندرم کرونری حاد شمارش پلاکت کمتری در مقایسه با آنژین پایدار و افراد نرمال داشته اند. مطالعه زانگ و همکاران در سال 2015 نشان داد که شمارش پلاکتی در بیماران آنژین ناپایدار و انفارکتوس میوکارد در مقایسه با بیماران مبتلا به آنژین مزمن پایدار و افراد مبتلا به درد قفسه سینه غیر قلبی بیشتر کاهش می یابد. نویسندگان این نامه در بررسی 180 بیمار با میانگین سنی 11/15 ± 57/55 سال (86 – 20) به مقایسه تعداد پلاکت در سه گروه مبتلا به درد قفسه سینه غیر قلبی (60 نفر) ، آنژین قلبی پایدار (60 نفر) و آنژین قلبی ناپایدار یا انفارکتوس میوکارد (60 نفر) پرداختند. میانگین تعداد پلاکت در سه گروه مذکور به ترتیب 92/62 ± 73/258، 88/65 ± 95/238 و 82/47 ± 78/218 هزار در میکرولیتر بود (001/0 = p). همانطور که پیداست در این مقایسه نیز تعداد پلاکت در گروه با آنژین ناپایدار به طور معنی داری پایین تر بوده است. اما نکته اینجاست که در همه این موارد پلاکت در محدوده نرمال (400 – 150 هزار در میکرولیتر) قرار دارد و شاید در نظر گفتن تعداد پلاکت به تنهایی نتواند در افتراق این گروه از بیماران کمک کنند باشد. به نظر می رسد شاید اندازه گیری مواد رها شده از پلاکت به جای توجه به تعداد پلاکت درافتراق سه وضعیت مذکور بیشتر کمک کننده باشد. با این حال اظهار نظر در مورد ارتباط میزان و الگوی افت پلاکتی در وضعیت های متفاوت سندرم کروناری نیاز به مطالعات دقیق تر و با حجم نمونه بالاتر دارد.

    : پرسنل اورژانس پیش بیمارستانی روزانه در خط مقدم پاسخ اورژانس استرس های فراوانی را تجربه می کنند. شرایط بحرانی و تلاش برای نجات بیماران،عملیات را بر تکنسین های فوریت پزشکی دشوار می نماید. بار استرس در این گروه شغلی مرتبط با حوادث بحرانی، مشکلات عملکردی و عاطفی است که حاصل میزان مواجهه در شدت تنش زا بودن عامل استرس زا است. بنابراین این مطالعه با هدف بررسی بار استرس مرتبط با عملیات اورژانس در بین تکنسین های فوریت پزشکی ایران انجام گرفت.
    در این مطالعه توصیفی- همبستگی 320 تکنسین شاغل در پایگاه های اورژانس پیش بیمارستانی شرکت نمودند. برای جمع آوری داده ها از پرسشنامه ی بررسی بار استرس که حاصل میزان شدت در میزان مواجهه عوامل استرس زا بود استفاده شد. داده ها با استفاده از نرم افزار آماری SPSS و آمار توصیفی و استنباطی مورد تجزیه و تحلیل قرار گرفت.
    در این مطالعه مهمترین عامل بار استرس عوامل محیطی شغلی و عوامل مربوط به بیمار گزارش شدند. میانگین و انحراف معیار بار استرس 61/3±70/8 بود. کار با بیماران ترومایی، توقعات بیش از حد بیمار و همرا هان آن و رانندگی در شرایط بحرانی و احیاء بیماران، بیشترین بار استرس را بر پرسنل داشتند. در این مطالعه نمره میانگین شدت عوامل استرس زا بالاتر از نمره میانگین میزان مواجهه با عوامل استرس زا بود و بین میزان بار استرس با تعداد ماموریت، مدرک تحصیلی، پایگاه های اورژانس و وضعیت استخدام ارتباط معنی داری دیده شد.

    : از آنجا که پرسنل اورژانس پیش بیمارستانی اولین ارایه دهندگان خدمات بهداشتی و درمانی در مواقع بحران هستند، این پژوهش با هدف تعیین میزان آگاهی پرسنل اورژانس پیش بیمارستانی از مدیریت بحران و عوامل مرتبط با آن در کلیه پایگاه های اورژانس سطح استان چهار محال و بختیاری انجام گردید.
    این مطالعه به صورت توصیفی مقطعی انجام شد. نمونه گیری به صورت تمام شماری انجام گردید. ابزار جمع آوری اطلاعات یک پرسشنامه محقق ساخته بود که حاوی سوالاتی در مورد ویژگی های دموگرافیک و میزان آگاهی پرسنل اورژانس پیش بیمارستانی از مدیریت بحران بود که میزان آگاهی را در چهار حیطه آموزشی، مدیریتی، حرفه ای و امنیتی بررسی می نمود. در هر حیطه چهار سوال در نظر گرفته شد که با مقیاس لیکرت و توسط پنج گزینه اصلا، کم، متوسط، زیاد و خیلی زیاد میزان آگاهی پرسنل پیش بیمارستانی را می سنجید. اعتبار پرسشنامه به روش اعتبار محتوا و پایایی آن از طریق آلفای کرونباخ برابر 88% ارزیابی شد. جهت تجزیه و تحلیل داده ها از نرم افزار SPSS-13 و روش های آمار توصیفی استفاده شد.

    استروک بخش قابل توجهی از علل مراجعات به بخش اورژانس را شامل می شود. این عارضه در صورت تشخیص سریع و مداخله درمانی مناسب می تواند با پیامد مطلوبی همراه باشد، ولی در صورت تاخیر در درمان با هزینه های فراوانی برای فرد، خانواده وی و همچنین سیستم بهداشت جامعه همراه خواهد بود. بنابراین تشخیص به موقع قدم اول و اساسی در برخورد با این بیماران در بخش اورژانس محسوب می شود و بعد از آن انتخاب روش درمانی مناسب در الویت خواهد بود. امروزه روش های درمانی مختلفی در برخورد با این بیماران، از اقدامات حمایتی تا ترومبولیتیک تراپی و مداخلات اندوواسکولار، در دسترس می باشند و یکی از سوالات اصلی در درمان سکته مغزی حاد در بخش اورژانس این است که کدام بیمار برای دریافت کدام درمان مناسب است؟ مداخلات اندوواسکولار روش درمانی نسبتا جدیدی است که می تواند لخته را از داخل عروق مغزی بیرون آورده و در مطالعات مختلف نشان داده شده است که یکی از موثرترین مداخلات درمانی در برخورد با سکته مغزی می باشد. مطالعات مختلفی در حمایت از فواید آن در سالهای اخیر منتشر شده اند که باعث تغییر گایدلاین های درمانی بیماران دچار استروک در امریکا و اروپا شده و نقش مداخلات اندوواسکولار پررنگ تر گردیده است. البته این مداخله درمان، روشی تهاجمی به شمار می رود که می تواند با عوارضی نیز همراه باشد و بنابراین برای دستیابی به حداکثر موفقیت در درمان با این روش باید تلاش شود تا بیمارانی که با احتمال بیشتری از این مداخله سود می برند به این منظور انتخاب شوند. گفته می شود اگر دلیل سکته مغزی انسداد یک رگ بزرگ باشد بهترین کاندید برای اینترونشن و درمان آندوواسکولار می باشد. در این شرایط مهمترین وظیفه پزشک بخش اورژانس، شناسایی عروق مسدود شده است. مهمترین روش تشخیصی که گایدلاینهای موجود در این زمینه پیشنهاد می دهند انجام سی تی آنژیوگرافی می باشد که وضعیت عروق بزرگ را از ابتدای شریان آئورت تا کوچکترین عروق داخل مغز نشان می دهد. بعضی از مراکز درمانی این اقدام را فقط برای موارد با احتمال سکته های وسیع مغزی انجام می دهند ولی پیشنهاد می شود که هم سی تی اسکن مغزی و هم سی تی آنژیوگرافی برای همه موارد استروک حتی موارد خفیف نیز انجام گیرد؛ چرا که ممکن است این بیماران در معرض بروز یک سکته وسیع قریب الوقوع باشند. در بعضی از مراکز تکنیک CT Perfusion را به دوگانه سی تی اسکن مغزی و سی تی آنژیوگرافی (CTA) اضافه می کنند. متاسفانه این تکنیک تقریبا در اغلب مراکز کشور ایران در دسترس نیست. ولی در مراکز پیشرفته دنیا در همان مقطعی که سی تی اسکن مغزی انجام می شود قابل انجام بوده و می تواند وضعیت دقیقی از حجم درگیری بافت مغزی و حتی برگشت پذیری بافت درگیر در استروک را نشان دهد. عروق مناسب برای ترومبکتومی شامل شاخه شریان مغزی میانی (MCA) کاروتید اینتراکرانیال، خصوصا قسمت افقی آن (M1) و در بعضی موارد بعد از دو شاخه شدن کاروتید (M2) می باشد. همچنین با پیشرفت های صورت گرفته، ترومبکتومی کاروتید اکستراکرانیال نیز امروزه قابل انجام است. ولی تقریبا هیچگاه شریان های مغزی قدامی (ACA) و شریان مغزی خلفی (PCA) در شرایط فعلی کاندید اینترونشن نیستند. مطالعات گسترده ای شامل IMS-3، MR-CLEN، ESCAPE، EXTEND-IA، SWIFTPRIME و PREVASCAT با هدف انتخاب عروق مناسب و کاندیداهای مداخله اندوواسکولار انجام گرفته اند که تقریبا هیچ یک امکان انجام ترومبکتومی شریان بازیلر را در مطالعه مربوطه بررسی نکرده اند. ولی مطالعه ای تحت عنوانBASICS در این زمین انجام گرفته است که در مجموع حتی زمان انجام ترومبکتومی برای شریان بازیلر و پروگنوز آن حتی بهتر از سایر عروق گزارش کرده است و بیشتر نورولوژیست های اینترونشنیست و استروک نسبت به انجام آن دید مثبتی پیدا کرده اند. در مجموع مطالعات فوق حدود 1700 بیمار تحت ترومبکتومی اورژانس قرار گرفتند که میزان موفقیت بدون انجام CTA حدود 40% و در صورت آگاهی از رگ درگیر در CTA میزان موفقیت حدود 70% گزارش شده است. در خصوص تحلیل شرایطی که امکانCTA وجود ندارد باید یادآوری شود که بعد از فراهم شدن امکان ترومبولیتیک تراپی، استفاده از ترومبکتومی حتی بدون اطلاع از وضعیت عروق درگیر با تکنیکی مانند CTA میزان موفقیت درمان بیماران دچار استروک از حدود 23% به حداقل40% افزایش یافته است. بنابراین به نظر می رسد که فراهم آوری امکانات انجام CTA قبل از ترومبکتومی اورژانسی باید حداقل در تمام بخش های اورژانس بیمارستانهای مرجع کشور مورد حمایت جدی قرار گیرد. همچنین از امکانات فعلی موجود نیز در کشور مثل دستگاه سی تی اسکن و بخشهای نورولوژی و آنژیوگرافی و متخصصین مربوطه به شکل مطلوبی بهره برداری شود. چنانچه بتوان دستگاه سی تی اسکن را به شکلی برنامه ریزی نمود که همزمان با انجام سی تی اسکن مغزی، همزمان سی تی آنژیوگرافی را هم انجام دهد، در نتیجه انتخاب بیمار کاندید انجام ترومبکتومی به نحوی مناسب تر انجام خواهد گرفت و با این تمهیدات ممکن است میزان بهبودی بیمار دچار سکته مغزی و در نتیجه رهایی از یک فلج و مشکل طولانی و ناتوان کننده را به 70% ارتقا داد.

    : با توجه به تفاوت در زیرساخت های سیستم بهداشت و درمان کشورمان با کشور های پیشرفته، نیاز است که آموزش دستیاری طب اورژانس به گونه ای جهت دهی گردد که ضمن پاسخ دهی به نیازهای درمانی جامعه، اهداف آموزشی مشخص شده برای دستیاران این رشته را نیز با راهنمایی و کمک اعضای هیئت علمی پوشش دهد. شاید اولین قدم در این راه بررسی وضعیت موجود و نظرسنجی از دستیاران و اعضای هیئت علمی فعال در رشته تخصصی طب اورژانس باشد. بنابراین، مطالعه حاضر با هدف بررسی عوامل موثر بر کیفیت آموزش دستیاری در بخش اورژانس طراحی و اجرا گردید.
    ابتدا گروهی متشکل از 5 عضو هیئت علمی باتجربه نظرات خود را در مورد عوامل موثر بر آموزش دستیاری در قالب مصاحبه بیان کردند که منتج به طراحی پرسشنامه ای با 27 سرفصل گردید که در نهایت و بعد از بررسی روایی و پایایی، پرسشنامه ای شامل 2 3 آیتم تهیه شد. این آیتم ها در سه گروه عوامل فردی، عوامل محیطی و عوامل مربوط به بیماران طبقه بندی شدند. نمونه گیری به روش سرشماری انجام شد و کلیه دستیاران و اعضای هیئت علمی گروه طب اورژانس دانشگاه علوم پزشکی تهران که در محدوده زمانی اجرای این مطالعه در این رشته تخصصی فعالیت داشتند قابلیت ورود به مطالعه را داشتند. نظر سنجی کمی با استفاده از مقیاس لیکرت 5 امتیازی انجام شد. سپس داده ها مورد تحلیل آماری قرار گرفت تا میزان توافق در هر مورد در گروه دستیاران و اعضای هیئت علمی بررسی شود.

    : علی رغم اینکه بعضی پزشکان اصرار به انجام گرافی گردن برای تمام بیماران ترومایی دارند، برخی دیگر انجام آن را برای بیماران علامت دار توصیه می کنند. لذا مطالعه حاضر با هدف ارزیابی ویژگی های غربالگری گرافی ساده گردن در تعیین ضایعات تروماتیک گردن طراحی شده است.
    این مطالعه دقت تشخیصی بر روی مصدومان مراجعه کننده به واحد ترومای بخش اورژانس بیمارستان های وابسته به دانشگاه علوم پزشکی شهید بهشتی (امام حسین و شهدای تجریش) ، تهران، ایران طی فروردین تا اسفند سال 1394 انجام شد. نتایج: مجموعا 180 بیمار با میانگین سنی 32/11 ± 30/32 (68-16) سال مورد بررسی قرار گرفتند (7/76 درصد مرد). بیشتر بیماران (3/73 درصد) در رده سنی 16 تا 40 سال بودند و شایعترین مکانیسم آسیب تصادف با وسایل نقلیه موتوری (3/83 درصد) بود. شکستگی مهره هفتم گردنی شایعترین شکستگی مشاهده شده در گرافی (2/7 درصد) و سی تی اسکن گردن (2/17 درصد) بود. بر اساس یافته های گرافی و سی تی اسکن گردن به ترتیب 54 (0/30 درصد) و 139 (2/77 درصد) بیمار دارای حداقل یک یافته پاتولوژیک (شکستگی یا دررفتگی) در ستون فقرات گردنی بودند. سطح زیر منحنی راک، حساسیت، ویژگی، ارزش اخباری مثبت و منفی و درستنمایی مثبت و منفی گرافی ساده در تشخیص ضایعات تروماتیک گردن به ترتیب و با حاشیه اطمینان 95 درصد معادل (664/0 – 472/0) 568/0، (64/41-48/25) 09/33، (90/62-63/64) 48/80، (62/94-32/72) 18/85، (91/34-95/18) 16/26، (00/11-00/3) 75/5 و (26/3-43/2) 81/2 بود.

    : روشی برای بستن زخم ایده آل است که سریع، راحت، ایمن، ارزان، بی درد با حداقل عفونت و از همه مهمتر کمترین تاثیر در زیبایی بیمار باشد. لذا این مطالعه با هدف بررسی پیامد ترمیم با دو روش چسب مایع بافتی و بخیه در زخم های تروماتیک اطفال طراحی شد.
    p به عنوان سطح معنی داری در نظر گرفته شد.
    p) در موارد استفاده از چسب بالاتر بود. از جهت پیامد شش ماهه مرتبط با زیبایی بین دو گروه تفاوت معنی داری دیده نشد (463/0 = p). این در حالی است که میزان التهاب، درد و میزان بسته شدن زخم در روز چهارم بعد از ترمیم در گروه چسب از شرایط بهتری برخوردار بود. نتایج آنالیز زیر گروهی دو گروه مورد مطالعه بر اساس عمق نشان داد که اختلاف بین دو گروه از جهت درد (756/0 = p) و التهاب (422/0 = p) روز چهارم در موارد با عمق پایین (2 تا 4 میلی متر) معنی دار نبود. بر اساس آنالیز چند متغیره تنها فاکتور موثر مستقل در پیامد بهتر چهار روزه بیماران عمق زخم (004/0 = p) بود و در این مدل نوع ترمیم از مدل نهایی حذف گردید.

    : با توجه به اهمیت نقش و عملکرد اورژانس پیش بیمارستانی در سلامت جامعه و نیز لزوم بررسی مستمر عملکرد آن مخصوصا در شاخص های تاثیرگذار در فرآیند خدمات رسانی به بیماران، پژوهش حاضر با هدف مقایسه شاخص های زمانی ارائه خدمات اورژانس پیش بیمارستانی در دو بازه زمانی 6 ماهه و به فاصله یک سال در سطح استان زنجان انجام گرفت. روش کار: این پژوهش یک مطالعه مقطعی گذشته نگر بود. از بین کلیه تماس هایی که در شش ماهه دوم سال های 1392 و 1393 به مراکز اورژانس پیش بیمارستانی استان زنجان ثبت گردیده بود تعداد 742 تماس که منجر به انتقال بیماران به مراکز درمانی شده بود، به صورت تصادفی انتخاب و اطلاعات مورد نیاز شامل شاخص های زمانی و جمعیت شناختی از فرم های اورژانس 115 که برای هر ماموریت توسط تکنسین فوریت های پزشکی کامل می گردد، استخراج شد.
    از مجموع 742 بیمار منتقل شده توسط اورژانس پیش بیمارستانی در شش ماهه دوم سال های 1392 و 93 تعداد 452 نفر مرد و 290 نفر زن بودند. میانگین سنی بیماران 28 سال بود. بیشترین تماس ها با اورژانس پیش بیمارستانی به ترتیب مربوط به ترومای ناشی از حوادث و سوانح (345 مورد) و داخلی (114 مورد) بود. از نظر شاخص های زمانی به ترتیب میانگین زمان 02/0 دقیقه (زمان تاخیر) ، 8 دقیقه (زمان پاسخگویی) ، 10 دقیقه (زمان حضور در صحنه) ، 20 دقیقه (مجموع سه فاصله زمانی) ، 27 دقیقه (زمان انتقال به بیمارستان) ، 94 دقیقه (زمان کل ماموریت) به دست آمد. تنها در مورد دو شاخص زمانی انتقال به بیمارستان (001/0p= و 3-t=) و زمان کل ماموریت (009/0p= و 2-t=) تفاوت معناداری در شش ماهه دوم سال های 92 و 93 وجود داشت و در خصوص سایر شاخص های زمانی تفاوت معناداری مشاهده نشد.

    : طرح تحول نظام سلامت از سال 1393، مشتمل بر 7 برنامه در کشور ما آغاز شده است. از آنجایی که بهبود وضعیت بخش های اورژانس نیز جزو اهداف این طرح بوده است در این مطالعه به بررسی تاثیر طرح تحول نظام سلامت بر شاخص های عملکردی بخش اورژانس در بیمارستانهای دانشگاه های علوم پزشکی تهران و ایران پرداختیم.
    پژوهش مقطعی حاضر در کلیه بیمارستان های دانشگاه های علوم پزشکی تهران و ایران انجام شد. 6 شاخص عملکردی اورژانس به صورت ماهانه و در دو بازه زمانی 12 ماهه قبل و بعد از اجرای طرح از طریق مراجعه به واحد آمار بیمارستان ها جمع آوری گردید. داده ها با نرم افزار SPSS نسخه 22 و با استفاده از آزمون تی زوجی و نیز مدل سری زمانی منقطع (ITS) آنالیز شدند.
    p). به طوری که سطح دو شاخص مذکور بعد از اجرای طرح تحول نظام سلامت افزایش ولی روند این دو شاخص کاهش داشته اند.

    : قسمت عمده ای از زندگی روزانه هر فرد صرف اشتغال به کار می شود و فعالیت کاری از نظر سلامت روانی شخص، امری مهم به حساب می آید. همواره عوامل متعددی به عنوان منابع فشار های روانی (استرس) در محیط کار وجود دارند و با توجه به اهمیت مشکل فرسودگی شغلی و عواقب آن مطالعه حاضر با هدف بررسی شیوع و علل فرسودگی شغلی در دستیاران طب اورژانس در بیمارستان های دانشگاه ایران انجام شد.
    این مطالعه مقطعی در بیمارستان حضرت رسول وابسته به دانشگاه علوم پزشکی ایران در طی سالهای 1393 تا 1396 انجام گرفت. نمونه گیری به شیوه سرشماری انجام شد و تمامی دستیاران طب اورژانس دانشگاه علوم پزشکی ایران در طی مدت انجام این در صورت تمایل و داشتن حداقل 6 ماه سابقه کار در بخش اورژانس وارد مطالعه شدند. خوانا نبودن و یا مخدوش بودن پرسشنامه تکمیل شده به عنوان معیار خروج از مطالعه لحاظ گردید. اطلاعات پس از جمع آوری توسط نرم افزار SPSS-16 تحت روش های آماری مورد تجزیه و تحلیل قرار گرفت.
    در کل 83 دستیار طب اورژانس با میانگین سنی 4/3 ± 0/33 سال در مطالعه شرکت کردند که 45 نفر (2/54 درصد) زن بودند. از این تعداد 60 نفر (3/72 درصد) به رشته خود ابراز علاقه نمودند. متوسط میزان فرسودگی شغلی دستیاران با میانگین 13/19 ± 32/90 (محدوده بین 124- 26) نمره بود. .

    : مهارت در انتخاب کلار مناسب و بستن صحیح آن جهت جلوگیری از آسیب‏های عصبی-عروقی در بیماران مبتلا به ترومای سر و گردن از اهمیت بالایی برخوردار است. لذا مطالعه حاضر با هدف ارزیابی آگاهی و عملکرد کارورزان در استفاده از کلار گردنی برای بیماران ترومایی طراحی و اجرا شده است.
    در این مطالعه مقطعی آگاهی و عملکرد کارورزان پزشکی ورودی سال 1388 دانشگاه علوم پزشکی گیلان طی زمستان سال 1395 با استفاده از پرسشنامه ای که روایی و پایایی آن تایید شده بود، مورد سنجش و ارزیابی قرار گرفتند.
    80 کارورز با میانگین سنی 02/1 ± 93/25 سال مورد مطالعه قرار گرفتند (2/76 درصد زن). میانگین نمره آگاهی شرکت کنندگان برابر با 7/2 ± 5/11 (3 تا 19) بود. همچنین میانگین نمره عملکرد آنها برابر با 3/2 ± 5/16 (10 تا 20) بود. بیشترین میزان ناآگاهی در مورد فاکتور های پیش بینی کننده میزان آسیب گردن و نخاع (8/98 درصد) ، میزان اهمیت آسیب (0/95 درصد) ، حداکثر مدت مجاز استفاده از کلار گردنی (3/91 درصد) و اقدامات ناصحیح (3/91 درصد) بود. همچنین بیشترین میزان نقص عملکرد نیز به ترتیب مربوط به هم سطح کردن مئاتوس شنوایی خارجی با شانه بعد از بستن کلار گردنی در کودکان (8/98 درصد) ، گذاشتن پد محافظ پشت قفسه سینه بعد از بستن کلار در کودکان (0/95 درصد) و استفاده از حداقل 3 نفر هنگام بستن کلار و بی حرکتی ستون فقرات قبل از بستن کلار (0/65 درصد) بود. نمره عملکرد و آگاهی ارتباط معکوس داشتند (225/0-=r، 023/0=p).

    درد احساس و تجربه حسی ناخوشایند می باشد که در نتیجه یک آسیب بالقوه و بالفعل بوجود می آید و کنترل آن به قدری مهم است که انجمن درد آمریکا برای تاکید بیشتر بر درد و کنترل آن، از درد بعنوان پنجمین علامت حیاتی نامبرده است. تقریبا همه انسانها تجربه دردهای متوسط تا شدید را داشته اند و بیشتر اوقات به دنبال انجام پروسیجرهای جراحی، شرایط پزشکی و یا تروما، این تجربه اتفاق افتاده است. وقتی درد و رنج در یک موقعیت بالینی مشخص می گردد، دنبال کردن و رفع آن به عنوان یک هدف و وظیفه بر عهده پرسنل سیستم بهداشتی درمانی می باشد. از این رو درد و درمان آن یک اولویت درمانی است. درد بر تمام افراد با هر سن، جنس، نژاد و طبقه اجتماعی و اقتصادی تاثیرگذار است و درد درمان نشده می تواند بر سیستم های مختلف بدن اثر منفی گذاشته و منجر به عوارض فراوانی گردد. افزایش سطح سرمی کورتیزول، نوراپی نفرین و رنین آنژویوتانسیون، افزایش قند خون، کاهش انسولین، بروز اضطراب، اثرات قلبی مثل ایسکیمی، دیس ریتمی، افزایش نبض، افزایش بار قلبی، افزایش مصرف اکسیژن، افزایش انعقادپذیری خون، افزایش فشار خون بخصوص در بیماران با صدمات سر، کاهش اکسیژن خون، افزایش حجم مایعات بدن، کاهش پتاسیم خون، خستگی، گرفتگی عضلانی، کاهش عملکرد سیستم ایمنی، کاهش قدرت ترمیم زخم و غیره، از جمله عوارض نامطلوب درد می باشند. همچنین عدم درمان دردهای حاد نیز منجر به تحمل دردهای غیرضروری، افزایش طول مدت بستری، افزایش هزینه های پزشکی و احتمال پیشرفت و تبدیل شدن درد حاد به درد مزمن می شود. چنانچه بار اقتصادی عدم درمان درد در آمریکا حدود 100 میلیارد دلار در سال برآورد شده است. درد شکایت بسیار شایعی بوده و بخش قابل توجهی از علل تماس بیماران با سیستم اورژانس پیش بیمارستانی را شامل شده و همچنین در مصدومان و بیماران ترومایی معمولا وجود دارد. نتایج مطالعات نشان می دهد که بین 20 الی 31 درصد بیماران منتقل شده با آمبولانس دارای درد متوسط و شدید بوده اند. همچنین درد با شیوع 52 الی 86 درصد در اورژانس ها یکی از موضوعات اصلی بهداشتی درمانی کشورهای مختلف می باشد. پرسنل اورژانس پیش بیمارستانی نیز اولین اعضای کادر درمانی هستند که در بلایا و سوانح با این بیماران مواجه شده و بیشتر در معرض درد و رنج بیماران قرار دارند. علیرغم رشد کمی پایگاه های فوریتهای پزشکی کشور ایران به بیش از 1900 پایگاه در سراسر کشور، مشاهدات پژوهشگر و شواهد و دستورالعمل های موجود در اورژانس پیش بیمارستانی 115 ایران، نشان دهنده عدم بکارگیری کافی داروهای ضد درد، علی الخصوص ضد دردهای اپیوئیدی به عنوان راهکاری برای کاهش درد بیماران و مصدومان در آمبولانس های فوریتهای پزشکی 115 می باشد. برخی مطالعات ذکر می کنند نزدیک به یک سوم بیماران پیش بیمارستانی و بیش از 80 درصد بیماران دارای شکستگی درد متوسط تا شدید را گزارش می کنند و از طرفی تاثیر مثبت کاربرد مسکن ها در مناطقی که بکار رفته به وضوح بر اکثریت بیماران نشان داده شده است. به نظر می آید که کاربرد داروهای ضد درد در فوریتهای پیش بیمارستانی مبحث بسیار مهمی می باشد و محققان بیان می کنند که مدیریت مناسب درد یک اصل مهم در اقدامات پیش بیمارستانی و خدمت رسانی به بیماران می باشد و مسکن های مخدر نظیر مورفین و فنتالین جایگاه ویژه ای در دستورالعمل های اورژانس پیش بیمارستانی برخی کشورهای پیشرفته در فوریتهای پزشکی بعنوان درمان دردهای حاد ترومایی و غیرترومایی و به ویژه درد قفسه سینه مربوط به بیماری های قلبی دارند. با نگاهی به مطالب پیشگفت در مورد اهمیت درد و درمان آن به خصوص اهمیت درمان درد در تروما و بیماری های ایسکمیک قلبی، شیوع بالای درد در ماموریت های پیش بیمارستانی، شدت درد و نوع ماموریت های پیش بیمارستانی که اکثرا با درد همراه می باشند، هزینه های بالای عدم درمان درد و نیاز مبرم به استفاده از مسکن در تروما و بیماری های ایسکمیک قلبی، آیا زمان آن نرسیده است که فکری به حال درمان موثر و علمی تر درد در فوریت های پزشکی گردد؟ آیا بهتر نیست در کنار این همه امکانات که بر بالین بیمار مهیا کرده ایم، چند نوع داروی ضد درد مسکن و مخدر حداقل برای بیماری های ایسکمیک قلبی و یا ترومای شدید نیز در نظر گرفته شود؟ هرچند نتایج مطالعه ای که در خوزستان انجام شد نشان داد که پرسنل فوریتهای پزشکی 115 از نظر اطلاعات و محاسبات دارویی بسیار ضعیف می باشند. بعضا مطالعاتی نیز بیان کرده اند که ممکن است دوزاژ دارویی و کاربرد داروهای مخدر درست انجام نگردد و یا بعضا ممکن است از داروهای مخدر سوء استفاده شود، اما همه اینها دلیل قانع کننده ای برای عدم کاربرد این داروها نمی باشد و می توان با ارایه راهکارهای بهتر نظیر آموزش بیشتر و بهتر پرسنل فوریتهای پزشکی، ایجاد پروتکل های آفلاین و آنلاین و برنامه های مثمر ثمر در جهت جلوگیری از سوء استفاده از داروهای مسکن بطور موثری از این دارو در فوریتهای پزشکی 115 استفاده نمود. از این رو مجدد متذکر می شویم که این اولویت درمانی نباید به هیچ دلیل نادیده گرفته شود.

    ترومای سر یکی از علل شناخته شده تشنج است و حدود 10% بیماران با ترومای شدید و متوسط سر دچار تشنج می شوند. پیشگیری و کنترل تشنج پس از ضربه به سر جهت محدود کردن آسیب های ثانویه مغزی بسیار حائز اهمیت است. تشنج های پس از تروما برحسب زمان وقوع آسیب به انواع تشنج های اولیه، تشنج های زودرس و تشنج های دیررس تقسیم می شوند. تشنج های اولیه در کم تر از 24 ساعت پس از ضربه اتفاق می افتند در حالی که تشنج های زودرس در کم تر از یک هفته و تشنج های دیررس در بیش از یک هفته بعد از تروما روی می دهند. فنی توئین یک داروی ضد تشنج است که به طور گسترده برای پیشگیری از تشنج در بیماران ترومایی تجویز می گردد. این دارو حتی در غلظت های درمانی نیز دارای فارموکینیتیک غیر خطی بوده و تجویز آن نیازمند مانیتورینگ قلبی، پایش مداوم سطح پلاسمایی دارو و تنظیم دوز است. بررسی ها نشان داده اند که فنی توئین از تشنج های اولیه جلوگیری می کند اما در کاهش تشنج های دیررس موثر نمی باشد. از طرفی در اکثر موارد به دلیل تغییرات فارماکوکینتیکی در بیماران با ترومای سر، غلظت خونی فنی توئین خارج از محدوده درمانی بوده و تعیین سطح سرمی فنی توئین همیشه مقدور نمی باشد. لذا یافتن درمان های جایگزین جهت پیشگیری از بروز تشنج در این بیماران ضروری است. سدیم والپروات یک داروی ضد تشنج است که اغلب همراه با داروهای دیگر برای درمان انواع مختلف صرع به کار می رود. این دارو تخلیه های الکتریکی را در مغز کاهش می دهد. از سدیم والپروات در پیشگیری از حملات تشنج نیز استفاده می شود. عوارض جانبی سدیم والپرات نادر و معمولا کم اهمیت هستند. نویسندگان این نامه 70 بیمار مراجعه کننده به بخش اورژانس با ترومای شدید سر (سطح هوشیاری کمتر یا مساوی 8 در مقیاس کمای گلاسکو) و با میانگین سنی 81/14 ± 41/36 سال (64 – 18) را به صورت تصادفی به دو گروه 35 نفری تقسیم کردند و به یک گروه فنی تویین و به گروه دیگر سدیم والپروات به عنوان داروی پیشگیری از تشنج دادند و آنها را به مدت 6 ساعت تحت نظر قرار گرفتند (4/91 درصد مرد). دو گروه از نظر ویژگی های زمینه ای با یکدیگر تفاوت معنی داری نداشتند. نتایج این مطالعه نشان داد که از پنج نفری که در طی شش ساعت اول دچار تشنج شدند هر 5 نفر در گروه دریافت کننده فنی تویین قرار داشتند. به نظر می رسد قبل از اظهار نظر نهایی در خصوص مذکور، نیاز به انجام مطالعاتی با حجم نمونه و دوره پیگیری مناسب برای مقایسه اثرات و عوارض دو داروی مذکور در پیشگیری از تشنج ناشی از تروما داریم.

    : بر اساس پروتکل انجمن قلب آمریکا، دمای هدف بین 32 تا 36 درجه سانتی گراد برای حداقل 24 ساعت به صورت مداوم، باید برای همه موارد بازگشت خود بخودی جریان خون به دنبال احیاء قلبی ریوی موفق در افراد بزرگسال دچار کاهش هوشیاری واضح اعمال گردد. لذا مطالعه حاضر با هدف ارزیابی موانع و امکان سنجی اجرای این پروتکل برای بیماران بخش اورژانس طراحی شده است.
    این مطالعه یک پژوهش کیفی بود که شامل دو فاز می گردید. در فاز اول با مراجعه حضوری به متخصصین در گیر با این بیماران اقدام به تکمیل پرسشنامه ای استاندارد در خصوص موانع اجرای پروتکل از دیدگاه آنان گردید. در فاز دوم اقدام به اجرای پروتکل هیپوترمی بر روی نمونه ای از بیماران واجد شرایط در بخش اورژانس شد. هدف از این فاز کشف مشکلات القاء این پروتکل در فاز عملی بود. یافته ها با استفاده از آماره های توصیفی گزارش شدند.
    در مجموع 100 پرسشنامه تکمیل شد. نداشتن تجربه یا نیاز به اطلاعات بیشتر با 58 درصد، محدودیت تجهیزات و وسایل مورد نیاز با 46 درصد و نداشتن پروتکل کشوری با 24 درصد به ترتیب مهم ترین موانع اجرای پروتکل از دیدگاه متخصصین بودند. همچنین از دست رفتن زمان طلائی (14/57 درصد) ، اجازه ندادن اتند کشیک (57/28 درصد) و عدم رضایت خانواده (28/14 درصد) از مهمترین موانع درفاز اجرایی بودند.

    : داشتن اطلاعات کافی در زمینه آگاهی و نگرش افراد جهت هرگونه برنامه ریزی و مداخلات آموزشی مناسب با هدف فراگیری کمکهای اولیه در جامعه ضروری می باشد. مطالعه حاضر در سال 1394جهت بررسی دانش و نگرش نیروهای خدماتی شهر ساری درباره کمکهای اولیه و در چهار گروه شغلی پلیس، رانندگان تاکسی، رانندگان اتوبوس و آتش نشانان انجام شد.
    این مطالعه از نوع مقطعی بوده و جامعه پژوهش شامل رانندگان تاکسی و اتوبوس شهری، پلیس و آتش نشانان شهر ساری در سال 1394 بود. نمونه گیری بصورت تصادفی ساده طبقه بندی شده و با حجم نمونه 500 نفر انجام شد. دانش و نگرش مشارکت کنندگان با استفاده از پرسشنامه محقق ساخته و با نرم افزار SPSS ویرایش 22 مورد تجزیه تحلیل قرار گرفت.
    تعداد 500 نفر با میانگین سنی51/. 8 ±32/40 (حداقل 28 و حداکثر 60) سال که همگی مرد بودند در این مطالعه شرکت کردند. افراد شرکت کننده در مطالعه دارای میانگین بعد خانوار 8/13 ± 1/4 بودند. میانگین نمره نگرش 8/51 ± 2/11 بود. بیشترین فراوانی نمره کسب شده در نگرش برابر با 12 بود و بیشترین امتیاز نگرشی نیز مربوط به سازمان آتش نشانی و خدمات ایمنی با میانگین امتیاز 8/17 و کمترین امتیاز نیز مربوط به رانندگان تاکسی سازمان تاکسیرانی با میانگین امتیاز 2/11 از حداکثر نمره 20 بود. متوسط نمره آگاهی بدست آمده 7/42 ± 2/7 بود. بالاترین میانگین آگاهی مربوط به آتش نشانی با امتیاز 6/11 و کمترین آن مربوط به تاکسی رانی با امتیاز 1/7 از حداکثر نمره 20 بود.

    درد، به عنوان یک مکانیسم پیچیده نوروفیزیولوژیک و نوروسایکولوژیک، یکی از شایع ترین تجارب مشترک بیماران در اورژانس پیش بیمارستانی می باشد. هر چند آمار دقیق شیوع درد در اورژانس پیش بیمارستانی همچون اورژانس های بیمارستانی در دست نیست، اما در کشورهای توسعه یافته، علیرغم ناسازگاری نتایج مطالعات متعدد، شواهد حاکی از شیوع بالای درد حاد در اورژانس پیش بیمارستانی بین 20 تا 53 درصد است. این در حالی است که متاسفانه در ایران آماری در رابطه با این موضوع موجود نیست. عوارض فیزیولوژیک (تاثیر بر سیستم قلب و عروق، تنفس، اندوکرین و…) و سایکولوژیک (اضطراب، خشم، پرخاشگری و…) ناشی از درد حاد کنترل نشده، اثرات سوء بسیاری بر پیامدهای بالینی بیماران داخلی و تروما داشته و هزینه های مالی مستقیم و غیرمستقیم کلانی را بر منابع محدود نظام های مراقبت سلامت تحمیل می نماید. لذا، امروزه مدیریت موثر درد با استفاده از روش های مختلف دارویی و غیر دارویی در صحنه و حین انتقال در اورژانس پیش بیمارستانی، به عنوان یک شاخص عملکردی بالقوه کلیدی به ضرورتی غیرقابل انکار مبدل گردیده است. مرور گسترده متون نیز نشان داد که اخیرا پیشرفت های چشمگیری در بهره گیری از تجهیزات و داروهای ضد درد در اورژانس پیش بیمارستانی بسیاری از کشورهای توسعه یافته حاصل شده و داروهای ضد درد اپیوئیدی (مورفین سولفات، فنتانیل و کتامین) ؛ داروهای ضد التهاب غیر استروئیدی (کتورولاک و ایبوپروفن) ؛ آپوتل و گاز نیتریک اکسید برای تسکین درد بیماران در لیست داروهای اورژانس پیش بیمارستانی قرار گرفته که استفاده از این داروها بر حسب شرح وظایف و مدارج علمی تحصیلی پرسنل پیش بیمارستانی، با هماهنگی پزشک مشاور و یا با بهره گیری از پروتکل های بالینی آف-لاین در این عرصه میسر گردیده است. همچنین اکثر پژوهش های اخیر در این حوزه بر عدم کفایت مدیریت درد پیش بیماستانی و نیز مقایسه و ترکیب داروهای مختلف ضد درد جهت افزایش کارایی و اثربخشی و بهبود کیفیت متمرکز می باشد.

    پارگی آنوریسم آئورت شکمی عارضه ای با میزان مورتالیتی بالا در مراجعه کنندگان به بخش اورژانس می باشد. علائم شایع آن شامل درد شکم، پشت و پهلوها بوده و هیپوتانسیون و سنکوپ از سایر تظاهرات آن می باشد. ولی گاها این بیماران با تظاهرات نامعمول مراجعه می کنند که با توجه به این مهم، در این گزارش به معرفی بیماری با تظاهرات آتیپیک پرداختیم. تشخیص اولیه این عارضه بر مبنای ظن بالینی بالا و انجام سونوگرافی شکمی بر بالین است. استاندارد تشخیصی سی تی اسکن شکمی در بیماران پایدار از نظر علائم حیاتی است. درمان انجام جراحی باز یا اندوواسکولار است و تاخیر در درمان با مرگ و میر بسیار بالایی همراه خواهد بود.

    مقدمه
    مدیریت بحران نقش مهمی در کسب آمادگی بیمارستان ها برای پاسخ به بلایا ایفا می کند. بنابراین افزایش دانش و بهبود نگرش تیم مدیریت بحران در ارتقای آمادگی بیمارستانها در مرحله پاسخ گویی به شرایط بحرانی و حوادث واقعی، یک ضرورت اجتناب ناپذیر است. از آنجایی که بر اساس مطالعات انجام شده ضعف دانش و عدم نگرش مطلوب از مشکلات اصلی مدیریت حوادث در ایران می باشد، آموزش مدیریت بحران به روش تمرین شبیه سازی شده به عنوان یک راهکار در نظر قرار گرفته است، لذا هدف از انجام این پژوهش تعیین تاثیر آموزش مدیریت بحران در حوادث ترافیکی پرتلفات با روش تمرین شبیه سازی شده (مانور) بر دانش و نگرش کارکنان پرستاری بیمارستان بود.
    روش کار
    این مطالعه تجربی مداخله ای از شهریور تا دی ماه سال 1396 در مرکز آموزشی درمانی شهید مطهری مرودشت انجام شد. جامعه پژوهش گروه پرستاری بیمارستان و نمونه برابر جامعه پرسنل بخش اورژانس بیمارستان بود (100نفر). داده ها با مقیاس نگرش در بحران، پرسشنامه اطلاعات آمادگی اورژانس و آزمون دانش تریاژ حوادث قبل و دو هفته بعد از مداخله جمع آوری شدند. مداخله شامل کارگاه آموزشی یک روزه برای کل جامعه پژوهش و سپس انجام مانور رو میزی و مانور حادثه جاده ای شبیه سازی شده برای گروه آزمایش بود. تجزیه و تحلیل داده ها با استفاده از SPSS 22 و آزمون تی زوجی و آزمون آنکوا انجام شد.
    یافته ها
    میانگین نمره اطلاعات آمادگی اورژانس بعد از مداخله بین دو گروه آزمایش (47/25 69/166) و کنترل (24/39± 14/136) تفاوت آماری معنی داری داشت (0001/0 >p). همچنین نمره کلی نگرش نسبت به مدیریت بحران پس از مداخله در گروه آزمایش (10/1± 44/21) و کنترل (57/1± 92/20) تفاوت معناداری داشت (02/0=p).
    نتیجه گیری
    یافته ها نشان داد که آموزش به روش تمرین شبیه سازی و کارگاه آموزشی بهتر از کارگاه آموزشی به تنهایی توانست دانش و نگرش نسبت به مدیریت بحران را بهبود بخشد. بنابراین، تمرین شبیه سازی برنامه های آموزشی جهت بهبود دانش و نگرش مدیریت بحران توصیه می شود.

    حفظ تندرستی برای داشتن یک شغل رضایت بخش و کارا لازم و ضروری است. فعالیت 24 ساعته و اشتغال در تمامی روزهای هفته موجب می شود پزشکان شاغل در بخش های اورژانس به دلیل تغییر در ساعات کار و فعالیت شیفتی دچار عوارض مرتبط با بهم ریختگی وضعیت فیزیولوژیک بدن شوند. ثابت شده که کار شیفتی منجر به کاهش کیفیت خواب، خستگی، اختلال وضعیت روحی، اختلال در روابط اجتماعی و خانوادگی و بروز اشکال در تامین نیازهای ذهنی افراد می شود. با افزایش خستگی روحی-جسمی به تدریج پزشکان احساس Depersonalization، سرخوردگی و پوچی می کنند. دید منفی نسبت به بیمار و درمان وی پیدا کرده و رفته رفته در صورت تداوم خستگی، رابطه بیمار-پزشک خدشه دار می شود. پایه و اساس راهکار درمانی در موارد بروز خستگی، بر ایجاد تعادل در مدل زندگی و توجه به خود و سلامتی استوار است. در جهت ارتقا بهداشت و سلامت جسم-روان در یک سیستم سلامت مجموعه ای از افراد دخیل هستند که هریک به تنهایی نقش بسزایی در این امر مهم ایفا می کنند.

    مقدمه
    بخش اورژانس به عنوان یکی از مهمترین بخش های بیمارستان، وظیفه فراهم نمودن مراقبتهای درمانی فوری جهت کاستن مرگ و میر و معلولیت در بیماران را عهده دار می باشد. دستیابی به این امر با مدیریت و ارزیابی فعالیت های اورژانس از طریق مستند سازی کامل، دقیق و به موقع اطلاعات بر اساس اصول استاندارد میسر می باشد. لذا این پژوهش با هدف تعیین میزان رعایت استانداردهای مستند سازی در فرم های ماموریت بیماران در اورژانس پیش بیمارستانی طراحی شده تا با بررسی میزان فرم های ماموریت بتوان به مشکلات موجود پی برد و جهت رفع آنها اقدام به برنامه ریزی نمود.
    روش کار
    این پژوهش یک مطالعه مقطعی است که در سال 1396 بر روی فرم های ماموریت پیش بیمارستانی صورت گرفت. جهت جمع آوری اطلاعات از چک لیستی که شامل 24 سوال در دو بخش بود، استفاده شد. بخش اول شامل11 سوال جهت تعیین میزان رعایت استانداردهای مستندسازی مشخصات بیماران و بخش دوم شامل 14 سوال به منظور تعیین میزان رعایت استاندارهای مستندسازی در فرم های ماموریت بود. بر اساس سیستم تقسیم بندی و امتیازدهی لیکرت پاسخ هر گزینه شامل سفید (امتیاز یک) ، ناخوانا (امتیاز دو) ، ناقص (امتیاز سه) و کامل (امتیاز چهار) بود. بنابراین حداقل و حداکثر امتیاز قابل کسب در قسمت مستندسازی صحیح مشخصات بیماران (44-11) و در قسمت گزارشات ماموریت (56-14) تعیین گردید. در نهایت داده های مطالعه با نرم افزار SPSS نسخه 18 و با استفاده از آزمون های آماری مربع کای و دقیق فیشر جهت مقایسه داده های کیفی و students t-test جهت مقایسه داده های کمی مورد تجزیه و تحلیل قرار گرفت.
    یافته ها
    در این مطالعه 250 فرم ماموریت پیش بیمارستانی مورد بررسی قرار گرفت. میانگین امتیاز کسب شده در قسمت بررسی کیفیت تکمیل دستورات پزشکی 0/15±4/22 (حداقل 15 و حداکثر 34) بود. میانگین امتیاز کلی کسب شده برای مستندسازی گزارشات فرم های ماموریت برابر 8/4±0/32 (حداقل 28 و حداکثر 46) بود. امتیاز کل فرم های ماموریت اورژانس 115 به جهت رعایت استانداردهای مستندسازی 2/5 ±7/50 (حداقل 45 و حداکثر 74) بود.
    نتیجه گیری
    بر اساس یافته های مطالعه حاضر وضعیت و کیفیت رعایت استانداردهای مستندسازی فرم های ماموریت در واحد اورژانس 115 مورد مطالعه در سطح مطلوبی قرار نداشت.

    طوفان تیروئیدی که به نام کریز تیروتوکسیک نیز شناخته می شود، یک وضعیت هایپرمتابولیک حاد و شدید است که به علت افزایش تولید و آزادشدن هورمونهای تیروئید یا افزایش حساسیت بافتهای محیطی به هورمونهای تیروئید و در نتیجه افزایش خطرناک سرعت متابولیسم در تمام سیستمهای بدن، رخ می دهد. اغلب موارد طوفان تیروئیدی در اثر بروز یک عامل شروع کننده در حضور بیماری زمینه ای تیروئید بروز می کند. شک بالینی قوی و توانایی تشخیص سریع موارد قریب الوقوع طوفان تیروئیدی در بیمار بسیار حیاتی است. در موارد شک بالینی به این عارضه، فراهم کردن امکان تشخیص سریع و ارایه درمان مناسب موجب بهبود پیش آگهی بیماری خواهد شد.

    مقدمه
    محیط پر تنش بخش اورژانس کارکنان این بخش را در معرض خطرات ناشی از تهاجمات و تعارضات فیزیکی و کلامی قرار میدهد که می تواند علاوه بر امنیت و رضایت شغلی کارکنان بر روند و کیفیت ارائه خدمات و مراقبت ها به بیماران نیز موثر باشد.
    روش کار
    در این مطالعه مقطعی توصیفی از طریق پرسشنامه کلیه پرسنل شاغل در بخش اورژانس بیمارستان های امام خمینی (ره) و گلستان اهواز نسبت به خشونتهای کلامی و فیزیکی در شش ماهه اول سال 1396مورد پرسش قرار گرفتند. سپس داده ها جمع آوری و با استفاده از نرم افزارSPSS نسخه 20 آنالیز گردید.
    یافته ها
    در بخش های اورژانس مورد مطالعه 132 نفر پرسنل در رده های مختلف مشغول به کار بودند و 100 درصد ایشان در این مطالعه شرکت نمودند که 58 درصد ایشان مرد بودند. میانگین سن شرکت کنندگان در مطالعه حاضر 8/5 ± 7/31 و بین 21 تا 56 سال متغیر بود. از این بین 1/78 درصد حداقل یک بار مورد خشونت کلامی قرار گرفته بودند که میانگین تعداد خشونت کلامی در شش ماهه اول سال 1396، 8/34 بار بود. در مجموع 4/17 درصد پرسنل اورژانس با میانگین 31/7 بار در طی شش ماه مورد خشونت فیزیکی قرار گرفته بودند. همچنین 17 درصد پرسنل بخش اورژانس حین فعالیت در اورژانس احساس امنیت بالا، 61 درصد احساس امنیت متوسط و 12درصد احساس امنیت پایینی داشتند و 10 درصد هم اصلا احساس امنیت نمی کردند.
    نتیجه گیری
    با توجه به نتایج این مطالعه میزان بروز خشونت در بخش اورژانس بیمارستان های امام خمینی (ره) و گلستان اهواز بالاست و این خشونت بیشتر از طرف همراهان بوده است. این امر موجب نگرانی و دغدغه پرسنل اورژانس شده است.

    Platelets play an important role in pathogenesis of coronary artery obstruction. Acute coronary syndromes are associated with endothelial injuries, atherosclerotic plaque rupture, platelet activation, thrombosis formation and finally artery obstruction. Platelets are activated by attachment to the subendothelial structure and release granules containing A2 Thromboxane, serotonin, vasoconstrictors and platelet aggregation factors.
    Increased platelet activity has been associated with higher severity of ischemic cardiac disease. It has been shown that patients with cardiac ischemia who have a higher platelet count are more at risk of mortality. A study by Shefaee et al. in 2005 showed that platelet count change in those with acute coronary syndrome is significant; in these individuals, platelets decrease at the site of lesion due to being used up. In other words, patients with acute coronary syndrome have a lower platelet count compared to those with stable angina and normal people. Zhang et al. study in 2015 showed that platelet count in patients with unstable angina and myocardial infarction decreases more compared to patients with chronic stable angina and those with non-cardiac chest pain. The writers of this letter studied 180 patients with the mean age of 55.57 ± 15.11 years (20-86) and compared the number of platelets in 3 groups of non-cardiac chest pain (60 cases), stable cardiac angina (60 cases) and unstable cardiac angina or myocardial infarction (60 cases). Mean platelet count in the 3 mentioned groups was 258.73 ± 62.92, 238.95 ± 65.88 and 218.78 ± 47.82 thousand/µl, respectively (p = 0.001). As you can see, the number of platelets in the group with unstable angina was significantly lower in this comparison, too. However, the important point is that in all these cases platelet count is in the normal range (150-400 thousand/µl) and perhaps considering the number of platelets alone cannot help in differentiation of this group of patients. It seems that measuring substances released from platelet instead of considering platelet count can be more helpful in differentiating the 3 mentioned conditions. However, reaching a decision regarding the correlation of rate and pattern of platelet decrease with various states of coronary syndrome needs more accurate studies with bigger sample sizes.

    Introduction
    The staff of pre-hospital emergency medical service experience many stresses daily in the frontline of emergency responses. Critical conditions and trying to save patients’ lives make operation difficult for emergency medical technicians. The burden of stress in this job group is related with critical incidents, and functional and emotional problems that are the result of the rate of exposure and the intensity of the stressor.
    Methods
    In this descriptive-correlation study, 320 technicians working in pre-hospital emergency bases participated. To gather data, stress burden evaluation questionnaire, which was the result of rate of exposure to stressors, was used. Data were analyzed using SPSS statistical software and descriptive and inferential statistics.
    Results
    In the present study, the most important causes of burden of stress were reported to be environmental occupational factors and patient-related factors. Mean and standard deviation of stress burden was 8.70 ± 3.61. Working with trauma patients, unrealistic expectations of the patient or their relatives, and driving in critical conditions and resuscitation of patients inflicted the highest burden of stress upon the staff. In this study, the mean score of stressor intensity was higher than the mean score of exposure to stressors and a significant correlation was seen between the rate of stress burden with number of operations, educational degree, emergency bases, and status of employment.
    Conclusion
    Proper identification of stressors in terms of prevalence and intensity, leads to better understanding of these factors in pre-hospital emergency and can be of great help in prevention and control of stress burden among emergency medical service staff.

    Introduction
    Since emergency medical service (EMS) staff members are the first providers of health care at times of crisis, this study was done with the aim of determining the knowledge of EMS staff on crisis management and its related factors in all emergency bases located throughout Chaharmahal and Bakhtiari province, Iran.
    Methods
    This study was done as a descriptive cross-sectional one. Sampling was done via convenience sampling. The tool used for data gathering was a questionnaire prepared by a researcher that included questions regarding demographic characteristics and knowledge of EMS staff on crisis management, which evaluated their knowledge in 4 categories of educational, management related, professional, and safety related. 4 questions were asked in each category that assessed the knowledge of EMS staff on Likert scale using 5 choices of none, low, moderate, high, and very high. Validity of the questionnaire was evaluated using the content validity method and its reliability was calculated to be 88% via Cronbach’s alpha. To analyze the data, SPSS 13 software and descriptive statistics methods were applied.
    Results
    Data analysis showed that 100% of the participants in this study were male and their mean age was 27.52 ± 6.47 years. Knowledge of the EMS staff on crisis management was very high in 2.2 % of the participants; high in 10.2%; moderate in 48.4%; and low in 39.2%.     Stroke makes up a significant part of causes for emergency department (ED) visits. This complication can be accompanied by a desirable outcome in case of rapid diagnosis and proper treatment intervention. However, in case of delay in treatment, it will bring about many costs for the individual, his or her family and the health care system. Therefore, timely diagnosis is the first and major step in managing these patients in ED and after that, choosing the proper treatment method is the priority. Currently, various treatment methods are available for managing these patients, from palliative care to thrombolytic therapy and endovascular interventions and one of the major questions in treatment of acute brain stroke in ED is which patient is suitable for receiving which treatment? Endovascular intervention is a relatively new treatment method that can extract the blood clot from the brain vessels and various studies have shown that it is one of the most effective treatment interventions in management of brain stroke. Various studies have been published in favor of its benefits in recent years, which have resulted in changes in treatment guidelines of stroke patients in America and Europe and highlighting the role of endovascular interventions in them. However, this interventional treatment is considered an invasive method that can be accompanied by side effects and thus, for reaching maximum success in this method, efforts should be made to select patients who have a higher probably to benefit from this intervention. It has been said that if obstruction of a big vessel is the reason for brain stroke, the patient is the best candidate for intervention and endovascular treatment. In this situation, the most important duty of the ED physician is to identify the obstructed arteries. The most important diagnostic method that existing guidelines suggest in this regard is performing computed tomography (CT) angiography that shows the status of big arteries from the beginning of Aorta to the smallest arteries inside the brain. Some treatment centers do this only for cases with the probability of extensive brain stroke, but it is suggested to perform both brain CT scan and CT angiography for all stroke cases including mild cases; because these patients might be at risk of an upcoming extensive stroke. Some centers add CT perfusion technique to the brain CT scan and CT angiography (CTA) duo. Unfortunately, this technique is not available in most centers in Iran. However, in the advanced centers around the world it can be performed at the same time as brain CT scan and can show an accurate picture regarding the extent of the brain tissue involved and even the reversibility of the tissue involved in the stroke. Proper arteries for thrombectomy are the middle cerebral artery (MCA) branch of intracranial carotid, especially its horizontal part (M1) and in some cases after branching of the carotid (M2). In addition, with the advances made, extracranial carotid thrombectomy can also be done today. But anterior and posterior cerebral arteries (ACA and PCA) are almost never intervention candidates. Extensive studies including IMS-3, MR-CLEN, ESCAPE, EXTEND-IA, SWIFTPRIME, and PREVASCAT have been done with the aim of selecting the proper vessels and candidates of endovascular interventions, almost none of which have evaluated the possibility of performing thrombectomy of basilar artery. However, a study called BASICS has been done in this regard that has even reported the time of thrombectomy for basilar artery and its prognosis even better than the other arteries and most interventionist and stroke neurologists have gained a positive view to performing it. Overall, in the above-mentioned studies about 1700 patients underwent emergency thrombectomy and the success rate without performing CTA was about 40% and if aware of the artery involved via CTA, the success rate has been reported to be about 70%. Regarding processing the situations in which CTA cannot be performed it should be noted that after enabling thrombolytic therapy, using thrombectomy even without knowing the status of the arteries involved with techniques such as CTA, the success rate of treating patients with stroke increased from about 23% to at least 40%. Therefore, it seems that preparing the equipment for performing CTA before emergency thrombectomy should be seriously supported, at least in major hospitals of Iran. In addition, the presently available equipment such as CT scan device and neurology and angiography departments and the specialists in these fields should be employed effectively. If the CT scan device can be programmed in a way that when performing brain CT scan, CT angiography can also be performed simultaneously, the candidate patients for undergoing thrombectomy can be selected more properly and by using these solutions the rate of improvement in patients with brain stroke and thus, being relieved from paralysis and long-term problems might increase to 70%.

    Introduction
    Considering the differences between the infrastructures of healthcare systems in Iran and advanced countries, there is a need for directing the education of emergency medicine residents in a way that not only meets the treatment needs of the society, but can also cover the determined educational goals for the residents of this specialty with the guidance and help of the faculty members. The first steps might be evaluating the present status and surveying the residents and faculty members who are active in emergency medicine specialty. Therefore, the present study was designed and performed with the aim of evaluating the factors affecting the quality of resident education in emergency department (ED).
    Methods
    Initially, a group that consisted of 5 experienced faculty members expressed their opinions on the factors affecting the quality of resident education in an interview, which resulted in the design of a questionnaire with 27 topics that led to preparation of a 23-item questionnaire after validity and reliability evaluation. These items were classified in 3 groups of personal factors, environmental factors, and patient-related factors. Consecutive sampling was done and all the residents and faculty members of emergency medicine in Tehran University of Medical Sciences who were active in this specialty during the study period were eligible to participate in the study. A quantitative survey was done using 5-point Likert scale. Then the data were statistically analyzed to evaluate the agreement rate of the residents and faculty members in each item.
    Results
    In total, 57 residents with the mean age of 32.75 ± 6.12 years and 23 faculty members with the mean age of 39.65 ± 5.54 years participated in the study and filled out the questionnaires. Mean scores of the 3 categories of evaluated factors, namely personal, environmental, and patient-related factors from the viewpoint of all participants were 1.17 ± 0.12, 1.09 ± 0.15, and 1.52 ± 0.22, respectively. The mean scores calculated for the 3 studied categories were not significantly different between the residents and faculty members of emergency medicine who participated in the study (p > 0.05). In the opinion of faculty members, decreasing the number of shifts does not lead to improvement in the quality of resident’s training. However, they believed that 12-hour shifts, clinical rounds in ED and the residents teaching to each other are effective in improvement of the quality of their education and the opinion of residents and faculty members were significantly different in these cases (p
    Conclusion
    Faculty members and residents of emergency medicine do not share the same opinion on working hours, and the number of clinical shifts and their effect on resident training. The faculty members believed that 12-hour shifts provide more opportunities for education compared to 8-hour shifts and reducing the number of clinical shifts would decrease the quality of education.

    Introduction
    Although some physicians insist on performing plain radiography for all trauma patients, some others recommend performing it for symptomatic ones. Therefore, the present study has been designed with the aim of evaluating the screening performance characteristics of plain radiography in identifying traumatic neck injuries.
    Methods
    The present diagnostic accuracy study was performed on injured patients presenting to the trauma unit of emergency departments of hospitals affiliated with Shahid Beheshti University of Medical Sciences (Imam Hossein and Shohadaye Tajrish), Tehran, Iran, during March 2015 to March 2016.
    Results
    180 patients with the mean age of 32.30 ± 11.32 (16-68) years were evaluated (76.7% male). Most patients (73.3%) were in the 16-40 years age range and the most common mechanism of injury was motor vehicle collision (83.3%). Fracture of the 7th cervical vertebrae was the most prevalent fracture seen in radiography (7.2%) and computed tomography (CT) scan (17.2%). Based on the findings of cervical radiography and CT scan, respectively, 54 (30.0%) and 139 (77.2%) of the patients had at least 1 pathological finding (fracture or dislocation) in their cervical spine. Area under the ROC curve, sensitivity, specificity, positive and negative predictive value, and positive and negative likelihood ratio of plain radiography in detection of traumatic cervical injuries and with 95% confidence interval were 0.568 (0.472-0.664), 33.09 (25.48-41.64), 80.48 (64.63-62.90), 85.18 (72.32-94.62), 26.16 (18.95-34.91), 5.75 (3.00-11.00) and 2.81 (2.43-3.26), respectively.
    Conclusion
    Accuracy of plain radiography in detection of traumatic cervical injuries is very low. This is due to the very low sensitivity of plain cervical radiography in detection of fractures and dislocations of the cervical spine. Therefore, it seems that plain radiography cannot be considered as a proper tool for ruling out cervical spine injuries following head and neck trauma.

    Introduction
    An ideal method for wound closure is one that is rapid, easy, safe, inexpensive, and pain-free with minimal infection and most importantly has minimum effect on the cosmetic issues. Therefore, this study was designed with the aim of evaluating the outcome of wound repair with liquid tissue adhesive agent and suture in pediatric traumatic wound repair.
    Methods
    In this case-control study, 1 to 14 year-old children admitted to emergency department following traumatic injuries in need of wound closure via either suture or liquid tissue adhesive, were compared regarding speed of performance, rate of satisfaction in patient and the parents, rate of inflammation, pain and rate of wound closure on the 4th day as well as beauty results in the 6th month after wound repair.     Based on the results of the present study, liquid tissue adhesive was superior to suture regarding speed of repair performance and satisfaction of the patients and the parents. Liquid tissue adhesive was accompanied by less inflammation and pain and higher probability of complete wound closure on the 4th day after repair. Regarding beauty, no significant difference was seen between the 2 mentioned methods.

    Introduction
    Considering the importance of the role and function of emergency medical services (EMS) in a society’s health and the need for continuously evaluating its function, especially in indices affecting the process of giving service to the patients, the present study was done with the aim of comparing time indices of providing services in EMS during two 6-month periods 1 year apart throughout Zanjan province, Iran.
    Methods
    This study was a retrospective cross-sectional study. Out of all the calls recorded in EMS centers of Zanjan province during October to March 2013 and 2014, 742 calls that had resulted in the patient being transferred to a hospital were randomly selected and the required data including time indices and population indices were extracted from EMS forms that are filled by a medical emergency technician for each mission.
    Results
    Among the total of 742 patients transferred by EMS during October to March 2013 and 2014, 452 were male and 290 were female. Mean age of the patients was 28 years. Most calls to EMS were related to trauma due to accident (345 cases) and internal (114 cases), respectively. Regarding time indices, mean times observed, respectively, were 0.02 minute (delay time), 8 minutes (response time), 10 minutes (time to reach the scene), 20 minutes (total of the 3 mentioned times), 27 minutes (time to reach hospital), 94 minutes (total mission time). Only 2 time indices, namely time to reach hospital (t=3 and p=0.001) and total mission time (t=2 and p=0.009), were significantly different between the studied periods and no significant difference was detected regarding other time indices.
    Conclusion
    Based on the findings of this study, time indices of EMS were desirable and at the same level as national and international indices. Meanwhile, differences in time indices were only significant regarding time to reach hospital and total mission time and these indices were lower in 2013 compared to 2014.

    Introduction
    The health sector evolution (HSE) plan has been implemented since 2014 and includes seven programs. Since one of the goals of this plan was to improve the condition of emergency departments, in this study we evaluated the effect of HSE plan on performance indices of emergency department in hospitals affiliated with Tehran and Iran Universities of Medical Sciences, Tehran, Iran.
    Methods
    The present cross-sectional study was conducted in all Hospitals affiliated with Tehran and Iran Universities of Medical Sciences. Six performance indices of emergency department were collected monthly in two 12-month periods before and after implementation of the HSE plan by referring to the hospitals’ statistics unit. Data were analyzed via SPSS 22 using paired T-test and Interrupted Time Series (ITS) model.
    Results
    The results of ITS showed that implementation of HSE plan had no significant effect on the rate and trend of indices such as percentage of patients who left the emergency department within 12 hours and percentage of patients discharged against medical advice (p > 0.05). On the other hand, HSE plan implementation affected the rate and trend of indices such as percentage of patients who were decided upon within 6 hours and the average turnaround time for emergency tests significantly (p
    Conclusion
    Implementation of HSE plan has resulted in improved percentage of patients who were decided upon within 6 hours and increased average turnaround time for emergency tests. In addition, although it has significantly increased the trend of successful CPR percentage, it has decreased the rate of successful CPR.

    Introduction
    A major part of the daily life of each individual is spent on working and work activity is an important matter from the viewpoint of mental health. There are always many factors in the work-place that act as sources of mental pressure (stress). Considering the importance of job burnout as a problem and its consequences, this study was carried out with the aim of assessing the prevalence and causes of job burnout among emergency medicine residents in hospitals affiliated with Iran university of Medical Sciences.
    Methods
    This cross-sectional study was performed in Hazrate Rasou Hospital, which is affiliated to Iran University of Medical Sciences, from 2014 to 2017. Census method was used for including participants and during this time, all the emergency medicine residents of Iran University of Medical Sciences were included if they gave consent and had at least 6 months working experience in the emergency department. Illegibility of or damage to the questionnaire were considered as exclusion criteria. After gathering, data underwent statistical analysis via SPSS16 software.
    Results
    Overall, 83% of emergency medicine residents with the mean age of 33.0 ± 3.4 years participated in the study, 45 (54.2%) of which were female. 60 (72.3%) of the participants expressed an interest in their field. Mean job burnout score of the residents was 90.32 ± 19.13 (range: 26 – 124) points.
    Conclusion
    Mean job burnout score among the studied residents was in the desirable range and increase in age was the most important cause of rise in their job burnout index.

    Introduction
    Having the skill for choosing the proper collar and correctly fastening it is very important for preventing neurovascular injuries in patients with head and neck trauma. Therefore, the present study was designed and carried out with the aim of evaluating the knowledge and practice of medical interns regarding use of neck collar for trauma patients.
    Methods
    In this cross-sectional study, the knowledge and practice of medical interns who were admitted to the medical internship program of Guilan University of Medical Sciences in 2009 was assessed in winter of 2017 using a questionnaire, the validity and reliability of which had been approved.
    Results
    80 medical interns with the mean age of 25.93 ± 1.02 years were studied (76.2% female). Mean knowledge score of the participants was 11.5 ± 2.7 (3 to 19). In addition, their mean practice score was 16.5 ± 2.3 (10 to 20). The highest rate of lack of knowledge was observed regarding factors predicting the rate of damage to neck and spinal cord (98.8%), the importance of injury (95.0%), the maximum time allowed for using neck collar (91.3%) and incorrect measures (91.3%). In addition, the highest rate of error in practice also belonged to leveling external auditory meatus with shoulder after fastening the neck collar in children (98.8%), putting a protective pad behind the rib cage after fastening the collar in children (95%) and employing at least 3 people when fastening the collar and maintaining the immobility of the vertebra before fastening the collar (65.0%), respectively. The score of practice and knowledge had a reversed correlation (p = 0.023, r = -0.225).
    Conclusion
    Based on the findings, the rate of knowledge regarding neck collar usage in more than 70% of the medical interns in the present study was in the intermediate to poor range. In addition, the practice of 60% of them was in the intermediate to poor range. There was no direct correlation between the knowledge and practice of the students in this regard.

    Pain is an unpleasant feeling and experience that is caused by a potential or practical injury and its control is so important that the American Pain Society has introduced pain as the 5th vital sign for emphasizing the importance of pain and controlling it. Almost all humans have experienced moderate to severe pain and most of the time the experience has happened following surgical procedures, medical conditions or trauma. When pain manifests in a clinical condition, health care staff are responsible for following it and relieving it as a goal and responsibility. Pain affects all individuals regardless of age, sex, race, and socioeconomic status and untreated pain can negatively affect various systems of the body and lead to many consequences. Increase in serum level of cortisol, nor epinephrine, and renin angiotension, hyperglycemia, decrease in insulin, manifestation of anxiety, cardiac effects such as ischemia, dysrhythmia, increase in pulse, increase in cardiac load, increased oxygen uptake, increased blood coagulation potential, hypertension especially in patients with head injuries, decreased blood oxygen level, increased body fluid volume, drop in blood potassium, fatigue, muscle cramps, decreased immune system function, decreased ability for wound healing and etc. are among the undesirable side effects of pain. In addition, not treating acute pains also leads to bearing unnecessary pain, elongated duration of hospitalization, increased medical costs and probability of acute pain progressing and developing into chronic pain. The financial burden of not treating pain in America has been estimated to be about 100 billion dollars per year. Pain is a common complaint and makes up a considerable portion of reasons that patients call pre-hospital emergency services and is usually present in the injured and trauma patients. Study results showed that 20% to 31% of the patients presenting via ambulances had moderate and severe pain. Additionally, with 52% to 86% prevalence in emergencies, pain is a key topic in health care of various countries. Pre-hospital emergency staff members are the first in line among health care staff that face patients at times of disaster and accident and are more exposed to the patients’ pain. Despite the quantitative growth of pre-hospital emergency bases to 1900 bases throughout Iran, observations of researchers and evidence and instructions of emergency medical services (EMS) of Iran are indicative of not using enough analgesic drugs, especially opioid analgesic drugs, as a means to relieve patients’ pain in EMS ambulances. Some studies have mentioned that about one third of the pre-hospital patients and more than 80% of patients with bone fractures report moderate to severe pain and on the other hand the positive effect of using pain killers in the regions that used them have been shown to be obvious in most patients. It seems that using analgesics in EMS is a very important topic and researchers express that proper pain management is an important principle in pre-hospital care and opioid analgesics such as morphine and fentanyl have a special place in the instructions of EMS as treatments for acute trauma and non-trauma pains, especially cardiac chest pains, in some advanced countries regarding emergency care. Considering the afore-mentioned points concerning the importance of pain and its treatment, especially the importance of treating pain in traumatic and ischemic cardiac diseases, high prevalence of pain in pre-hospital missions, severity and type of pre-hospital missions, which are mostly associated with pain, high costs of not treating pain and the obvious need for using pain killers in trauma and ischemic cardiac diseases, isn’t it time to introduce a more effective and scientific approach in EMS? Isn’t it better to add a few types of analgesics and opioid drugs to the equipment provided at patients’ bedside, at least for ischemic cardiac diseases and severe trauma? Although the result of a study carried out in Khuzestan province showed that EMS staff members were weak regarding drug information and calculations and other studies have also expressed that drug dosage and use of opioid drugs might not be done correctly or opioid drugs might be abused in some cases, none are convincing reasons for not using these drugs. These problems could be solved by more and better training for EMS staff, creating offline and online protocols and useful programs for preventing abuse of analgesic drugs and these drugs could be effectively used in EMS. Therefore, we again note that this treatment priority should not be overlooked for any reason.

    Head trauma is a known cause of seizure and about 10% of patients with severe or moderate head trauma are affected with seizure. Preventing and controlling seizure after head trauma is of great importance for minimizing secondary brain injuries. Depending on their time of occurrence, post-trauma seizures are divided into 3 types, namely immediate seizures, early seizures, and late seizures. Immediate seizures occur within less than 24 hours after the trauma, while early seizures happen in less than a week and late seizures happen more than a week after trauma. Phenytoin is an antiepileptic drug that is widely prescribed for preventing seizure in trauma patients. This drug has nonlinear pharmacokinetics even in therapeutic doses and its prescription requires cardiac monitoring, continuous monitoring of the drug’s plasma levels and dose adjustment. Studies have shown that phenytoin prevents early seizures but is not effective in decreasing late seizures. On the other hand, in most cases, due to pharmacokinetic changes in patients with head trauma, blood concentration of phenytoin is outside the therapeutic range and determining its serum levels cannot always be done. Therefore, finding replacement treatments for preventing seizure in these patients is necessary. Sodium valproate is an antiepileptic drug that is usually used along with other drugs for treating different types of epilepsy. This drug decreases electric discharges in the brain. Sodium valproate is also used for prevention of seizure attacks. The side effects of this drug are rare and mostly unimportant. The authors of this letter randomly divided 70 patients presenting to emergency department with severe head trauma (consciousness level of 8 or less on Glasgow coma scale) with the mean age of 36.41 ± 14.81 years (18 – 64) into 2 groups of 35 and prescribed phenytoin for 1 group and sodium valproate for the other as antiepileptic drugs and monitored them for 6 hours (91.4% male). The 2 groups were not significantly different regarding baseline characteristics. The results of this study showed that out of the 5 patients who had seizure during the first 6 hours, all 5 were in the group receiving phenytoin. It seems that before reaching a conclusion in this regard, studies with larger sample sizes and longer follow-up periods are required for comparing the efficiency and side effects of the mentioned drugs in prevention of seizure due to trauma.

    Introduction
    Based on the protocol of American Heart Association (AHA), target temperature of 32°C to 36°C for at least 24 hours should be provided, continuously, for all cases of spontaneous return of blood flow following successful cardiopulmonary resuscitation (CPR) in adult patients with evident altered level of consciousness. Therefore, the present study was done to evaluate the obstacles and assess the possibility of performing this protocol for patients in emergency department.
    Methods
    This study was a qualitative research that consisted of 2 phases. In the first phase, by meeting the specialists involved with these patients, a standard questionnaire was filled regarding the obstacles to performing this protocol from their viewpoint. In the second phase, hypothermia protocol was done on a sample of eligible patients in emergency department. The aim of this phase was to find the problems of doing this protocol in real practice. The findings were reported using descriptive statistics.
    Results
    A total of 100 questionnaires were filled. Lack of experience or needing more information with 58%, limited facilities and equipment with 46% and lack of a national protocol with 24%, respectively, were the most important obstacles from the specialists’ point of view. In addition, losing golden time (57.14%), the in charge attend not giving permission (28.57%), and the relatives not giving consent (14.28%) were among the most important obstacles in the executive phase.
    Conclusion
    Based on the findings of the present study, it seems that training the physicians involved with these patients regarding induction of hypothermia with the primary equipment present in emergency departments, writing inter-department protocols and preparing national protocols in this regard can help in spreading this treatment method.

    Introduction
    Having sufficient data regarding the knowledge and attitude of individuals towards any planning and appropriate educational interventions with the aim of learning first aid in society is essential. The present study was performed in 2015 for evaluating knowledge and attitude of service providing personnel of Sari, Iran, regarding first aid in 4 occupational groups of police, taxi driver, bus driver, and firefighter.
    Methods
    This study is a cross-sectional one and the study population consists of city taxi and bus drivers, police and firefighters of Sari, Iran, in 2015. Sampling was done via simple random method and with a sample size of 500 individuals. Knowledge and attitude of the participants was evaluated using a questionnaire designed by the researchers and via SPSS software version 22.
    Results
    A total of 500 individuals with an average age of 40.32 ± 8.51 (at least 28 and a maximum of 60) years, all men, participated in this study. Participants of the study had a mean family dimension of 4.1 ± 13.8. Mean attitude score was 11.2 ± 51.8. The highest frequency of obtained score in attitude belonged to 12 and the highest attitude score belonged to firefighting and safety services organization with the mean score of 17.8 and the lowest score belonged to taxi drivers of taxi driving organization with the mean score of 11.2 from the maximum possible score of 20. Mean knowledge score obtained was 7.2 ± 42.7. The highest mean of knowledge belonged to firefighters with the mean score of 11.6 and the lowest score belonged to taxi drivers with the mean score of 7.1 from the maximum possible score of 20.
    Conclusion
    The results of the study showed that level of knowledge and attitude of those providing service to people was not proper regarding first aids and trainings in this regard is not desirable. To improve it, there is a need for schedule planning, retraining classes and trainings at the beginning and during service with concentration on improving the skills.

However, unfortunately, there are no analgesics or similar products in prehospital emergency services in most provinces of our country and only Ketorolac, which is only effective in management of mild to moderate pain, is available for pain management quite limitedly and in just a handful of regions.
    Obviously, the most important outcome of lack of analgesics for managing acute pain in urban and intercity prehospital emergency services, which are the first line in medical emergencies, accidents and disasters, is infliction of suffering, illness, distress and destructive psychophysiological responses of pain on patients from the occurrence of the accident to hospital arrival, and depriving them from their right for reducing and controlling pain, which is incompatible with patient's rights charter in our country and will lead to patients being dissatisfaction with and distrusting prehospital emergency service. In addition, daily exposure to patients suffering from uncontrolled acute pain and incapability to relieve their pain despite having the necessary clinical competency causes a high level of moral distress to prehospital care providers and has negative effects on their morale and performance.
    A comparison of the existing pain management proceedings in prehospital emergency service of developed countries with Iran indicates a lack of standardized scientific approach in the field of prehospital pain management in our country, which results in the current unfavorable condition. Though strong evidence exist regarding the necessity of early treatment of pain, which is not only ethically correct, but is also of primary importance in controlling both its metabolic repercussions and improvement of patient outcomes, and despite the extensive use of analgesics in developed countries' advanced prehospital care, unfortunately, misconceptions like fear of possible adverse events tied to analgesic use, possibility of masking clinical findings (signs and symptoms of illness) following pain relief, probable abuse of analgesic medications specially opioids, and etc. have prevented consideration of analgesics for common use in our prehospital emergency service, to date.
    Working in prehospital emergency service and providing service in real accident scenes and observing the present problems in this field made the authors address this neglected issue by explaining the status quo and weighing up the opportunities and threats ahead. We hope that the officials of Iran Disaster and Emergency Management Center (DEMC), Ministry of Health and others involved pay special attention to this matter and take effective steps towards removing organizational and legal obstacles that lie in the way of using analgesics in our prehospital emergency service by taking proper actions such as development, ratification, review and dissemination of evidence-based clinical practice guidelines and off-line protocols for prehospital pain management and training highly competent advanced prehospital providers (paramedics).

    Abdominal aortic aneurysm is a condition with a high mortality rate among those presenting to the emergency department. Its common symptoms include abdominal, back, and flank pain and hypotension and syncope are among its other manifestations. However, sometimes these patients present with uncommon manifestations. Considering this important point, we have introduced a patient with atypical manifestations in the present report. Initial diagnosis of this condition is based on high clinical suspicion and performing bedside abdominal ultrasonography. Diagnostic standard is abdominal computed tomography (CT) scan in patients with stable vital signs. The treatment is performing endovascular or open surgery and delay in treatment is accompanied with a high rate of mortality.

    Introduction
    Crisis management plays an important role in preparing hospitals for responding to disasters. Therefore, improvement of knowledge and attitude of the crisis management team is an unavoidable necessity for increasing the preparedness of the hospitals in the response phase to critical conditions and real disasters. Since, based on previous studies, lack of knowledge and proper attitude is one of the main problems of disaster management in Iran, teaching crisis management using simulated training method has been considered as a solution. Therefore, the aim of this study was determining the effect of teaching crisis management in traffic accidents with a high rate of mortality using simulated training (maneuver) method on the knowledge and attitude of nursing staff of Hospital.
    Methods
    The present experimental interventional study was performed from September 2017 to January 2018, in Shahid Motahari Hospital, Marvdasht. The study population consisted of the nursing staff of the hospital and the sample included all of the emergency department nurses of the hospital (100 nurses). Data were gathered with the attitude scale in crisis, emergency preparation questionnaire and triage knowledge test of accidents before and 2 weeks after intervention. Intervention included a one-day workshop training for all of the study population and then performing table maneuver and stimulated road accident maneuver for the test group. Analysis of data was done using SPSS 22 and descriptive statistics, paired t-test and ANCOVA test.
    Results
    Mean score of emergency preparation knowledge showed a statistically significant difference between the 2 groups of test (166.69 ±25.47) and control (136.14±39.24) after intervention (p<0.0001). In addition, the overall score of attitude towards crisis management after intervention was significantly different between test (21.44±1.10) and control (20.92±1.57) groups (p=0.02).
    Conclusion
    The results showed that training via simulation method and workshop could improve knowledge and attitude towards crisis management better than educational workshop alone. Therefore, simulation training of educational programs is suggested for improvement of knowledge and attitude regarding crisis management.

    Maintaining health is a requirement for having a satisfying job and being efficient. 24-hour activity and working every day of the week leads physicians who work in emergency departments to facing the effects of imbalance in physiological status of the body due to changes in working shifts and working hours. It has been proved that working in shifts leads to decreased quality of sleep, fatigue, mental state disorders, dysfunction in social and family relationships, and problems in providing the mental needs of individuals. With increase in mental and physical tiredness, physicians gradually develop feelings of depersonalization, frustration and emptiness. They develop a negative view towards the patients and their treatment and if the fatigue continues the patient-physician relationship will suffer. The basis of the solution in cases of fatigue is providing a balance in life and looking after oneself and personal health. To improve sanity and mental-physical health in a healthcare system, an array of people are involved, each of whom plays an important role in this major issue.

    Introduction
    As one of the most important departments in the hospital, emergency department is responsible for providing rapid treatment care for reducing mortality and disability among patients. Achieving this aim is possible with managing and evaluating the activities in this department via complete, accurate, and timely documentation of data based on standard principles. Therefore, the present study has been designed with the aim of determining the extent to which documentation standards are adhered to in mission forms of patients in pre-hospital emergencies. By evaluating the mission forms, existing problems can be identified and plans can be made to relieve them.
    Methods
    The present study is a cross-sectional one performed on pre-hospital mission forms from March 2017 to March 2018. To gather data, a checklist consisting of 24 questions in 2 parts was used. The first part consisted of 11 questions to determine the extent to which standards of patient characteristics documentation were adhered to and the second part consisted of 14 questions for determining the rate of adherence to documentation standards in mission forms. Based on Likert scale, each question was rated as no answer (1 point), illegible (2 points), incomplete (3 points) and complete (4 points). Therefore, the minimum and maximum obtainable points were considered (11-44) in the section regarding correct documentation of patient characteristics and (14-56) in the mission reports part. Finally, data were analyzed via SPSS statistical software version 18 using chi square and Fisher’s exact tests for comparing qualitative data and student’s t-test for comparing qualitative data.
    Results
    In this study, 250 pre-hospital mission forms were evaluated. Mean score obtained in evaluating the quality of documenting medical orders was 22.4±15.0 (minimum 15 and maximum 34). Mean total score obtained for documentation of mission form reports was 32.0±4.8 (minimum 28 and maximum 46). The overall score of emergency medical services (EMS) mission forms regarding adhering to the standards of documentation was 50.7±5.2 (minimum 45 and maximum 74).
    Conclusion
    Based on the results of the present study, the state and quality of adhering to documentation standards in mission forms of the studied EMS unit was not at a desirable level.

    Thyroid storm also known as thyrotoxic crisis, is an acute and severe hypermetabolic condition that occurs due to increase in synthesis and release of thyroid hormones or increased sensitivity of peripheral tissues to thyroid hormones due to a dangerous increase in metabolism speed in all the systems of the body. Most cases of thyroid storm manifest due to appearance of an initiating factor in the presence of an underlying thyroid disease. Strong clinical suspicion and ability to rapidly diagnose imminent thyroid storm cases in patients are vital. In cases of clinical suspicion to this condition, preparing the grounds for rapid diagnosis and providing proper treatment leads to improvement in prognosis of the patient.

    Introduction
    The tense environment of the emergency department (ED) exposes the staff of this department to risks resulting from physical and verbal confrontations and attacks, which can affect not only the staff’s job satisfaction and safety but also the process and quality of care and service provided for the patients.
    Methods
    In this descriptive cross-sectional study, using a questionnaire, all the staff members working in the EDs of Imam Khomeni and Golestan Hospitals, Ahvaz, Iran, were questioned regarding encountering verbal and physical violence from March to September 2017. Data were then gathered and analyzed via SPSS software version 20.
    Results
    In the studied EDs, 132 staff were employed in different categories and 100% of them participated in the study, of whom 58% were male. The mean age of the participants in the study was 31.7 ± 5.8 (rang from 21 to 56) years old. Among them, 78.1% of the studied individuals had faced verbal violence at least once and the mean incidence rate of verbal violence in the studied 6 months was 34.8 times in each month. 17.4% of the ED staff had faced physical violence with the mean incidence rate of 7.31% times per month in the studied 6 months. 17% of ED staff members reported feeling highly safe, 61% felt moderately safe, and 12% felt a low level of safety, and 10% did not feel safe at all.
    Conclusion
    Considering the results of the present study, the rate of violence incidence is high in the EDs of Imam Khomeni and Golestan Hospitals, Ahvaz, Iran, and this has raised concerns among the staff members of these EDs. In addition, in this study, violence has been mostly on the part of patients’ companions.
