صفحات 3-8هدفمقایسه میزان آلودگی مژه ها با گونه های دمودکس در مبتلایان به بلفاریت مزمن با افراد سالم در جمعیت ایرانی.روش پژوهشدر این مطالعه مورد- شاهدی، 203 نفر از مراجعین به بیمارستان های طرفه و لبافی نژاد تهران در سال 1396 (102 نفر از مبتلایان به بلفاریت مزمن و 101 نفر به عنوان گروه شاهد)، به صورت در دسترس انتخاب شدند. اطلاعات مربوط به یافته های مثبت در اسلیت لمپ بیومیکروسکوپی برای هر بیمار ثبت گردید، سپس از هر یک از پلک های بالا و پایین یک مژه از هر نیمه با فورسپس ظریف برداشته شده و به طور جداگانه روی لام قرار داده شد. در صورت مشاهده Cylindrical Dandruff همراه با هر مژه، این مژه به عنوان نمونه انتخاب گردید. یک قطره روغن امرسیون روی نمونه ریخته و لامل روی آن قرار داده شد و با میکروسکوپ نوری با بزرگ نمایی به میزان 40× و 100× از نظر وجود دمودکس در کم تر از 12 ساعت بررسی گردید. میزان آلودگی مژه ها با گونه های دمودکس در دو گروه بررسی و مقایسه شد.یافته هاآلودگی مژه ها با گونه های دمودکس در گروه مبتلایان به بلفاریت مزمن 26/5 درصد و در گروه شاهد 25/7 درصد بود که اختلاف آماری معناداری وجود نداشت (0/05P>). عوامل مرتبط با آلودگی افراد به دمودکس شامل سن بالا، Dandruff Cylindrical و برخی از علایم بالینی مانند خارش و سوزش چشم بودند.نتیجه گیریمیزان آلودگی مژه ها با گونه های دمودکس در مبتلایان به بلفاریت مزمن و افراد سالم تفاوت معناداری نداشت. شیوع آن با افزایش سن بیش تر شد و پوسته های چرب ارتباط معنادار منفی با ابتلا به دمودکس داشتند.کلیدواژگان: بلفاریت مزمن، دمودکس و مژه

صفحات 9-15هدفمقایسه سطح سرمی ویتامینD در بیماران مبتلا به سندرم خشکی چشم با گروه شاهد در بیماران مراجعه کننده به بیمارستان چشم پزشکی الزهراء(س) در سال 1395.روش پژوهشاین مطالعه مورد-شاهدی در سال 1395 روی 80 فرد (40 بیمار با خشکی چشم و 40 فرد سالم به عنوان گروه شاهد) مراجعه کننده به درمانگاه چشم پزشکی بیمارستان الزهرا صورت گرفت. اطلاعات با استفاده از فرم اطلاعاتی جمع آوری شد و توسط نرم افزار SPSS ویرایش 22 مورد تحلیل آماری قرار گرفت.یافته هادر این مطالعه، تعداد 80 فرد مورد بررسی قرار گرفتند. 60 درصد بیماران مرد بودند و میانگین سنی بیماران 11/28±44/50 سال بود. میانگین سطح خونی در گروه مورد معادل ng/dl 11/83±21/18 و در گروه شاهد، ng/dl9/98±20/54 بود و بین دو گروه تفاوت معنی داری وجود نداشت (0/793P=). سطح سرمی ویتامین D با OSDI (که نماد خشکی چشم است) رابطه معکوس و معنی داری داشت (0/48r= و 0/002 P=).نتیجه گیریدر این مطالعه، شدت خشکی چشم بر اساس تست (ocular surface disease index) OSDI رابطه معکوس با سطح سرمی ویتامین D داشت. سطوح پایین تر ویتامین D ممکن است نقش کلیدی در ایجاد علایم خشکی چشم داشته باشد.کلیدواژگان: خشکی چشم و ویتامین D

صفحات 16-25هدفبررسی اثر تزریق استرویید در فضای رتروسپتال اربیت در بیماران مبتلا به افتالموپاتی فعال تیرویید که به استفاده از استرویید سیستمیک مقاوم بودند، یا با قطع استرویید عود کرده و یا دچار عوارض سیستمیک ناشی از استرویید شده بودند.روش پژوهشاین مطالعه آینده نگر روی 31 چشم از 17 بیمار مبتلا به افتالموپاتی فعال تیرویید صورت گرفت که دارای رتبه فعالیت بیماری 3 یا بیش تر و از نظر شدت در مرحله 3 یا بالاتر بودند ولی دچار نوروپاتی فشارنده و کراتوپاتی اکسپوژر شدید نبودند. تمامی بیماران تاریخچه استفاده استرویید سیستمیک و عدم پاسخ مناسب و یا عود بیماری با قطع استرویید و یا عوارض ناشی از مصرف استرویید داشتند. ترکیبی از استرویید طولانی اثر به صورت 20 میلی گرم تریامسینولون و استرویید کوتاه اثر به صورت 4 میلی گرم دگزامتازون به فضای رتروسپتال حدقه در بالا و پایین 4-3 مرتبه به فواصل یک ماه تزریق شد. بیماران به طور دوره ای پس از تزریقات و حداقل به مدت 3 ماه از آخرین تزریق معاینه شدند و در صورت بهبود التهاب، جراحی های ترمیمی در موارد مورد نیاز انجام شد.یافته هامتوسط فعالیت بیماری قبل از تزریق ها (CAS) معادل 1/3±5/2 بود که در پایان چهارمین تزریق به 1/2±1/6 کاهش یافت. رتراکشن پلک بالا در 100 درصد و پلک پایین در 68/2 درصد از چشم ها یک میلی متر یا بیش تر بهبود یافت. لاگ افتالمی در 64/3 درصد از پلک ها یک میلی متر یا بیش تر بهبود یافت. استرابیسم در یکی از 5 بیمار مبتلا برطرف شد و بیش ترین بهبود در حرکت چشم به سمت بالا مشاهده شد. متوسط بهبود بیرون زدگی چشم 1/1±1/2 میلی متر بود. دید بیماران تغییر چشمگیری نداشت. اکیموز پلک ها و خونریزی زیر ملتحمه در 6/5 درصد دفعات تزریق ها مشاهده شد و در 8/8 درصد چشم ها افزایش فشار چشم پس از تزریق اتفاق افتاد که به درمان طبی به خوبی پاسخ داد.نتیجه گیریتزریق استرویید در فضای حدقه می تواند در فرم های فعال افتالموپاتی تیرویید در بیمارانی که به استروییدهای سیستمیک مقاومند و یا با قطع آن، دچار عود می شوند و به عبارتی به استرویید وابسته شده اند و یا دچار عوارض سیستمیک استرویید شده اند به کار رود. میزان فعالیت بیماری، رتراکسیون پلک، لاگ افتالموس، حرکات چشم و پروپتوز با این درمان بهبود می یابند ولی دید تغییری نمی کند.کلیدواژگان: اربیتوپاتی گریوز، التهاب حدقه، بیرون زدگی چشم و تزریق استرویید

صفحات 26-33هدفارزیابی تاثیر برداشتن بخیه بر روی قدرت انکساری و انحنای بافت پیوند بعد از جراحی DALK در بیماران مبتلا به کراتوکونوس.روش پژوهشاین مطالعه گذشته نگر، بر روی 54 چشم مبتلا به کراتوکونوس که تحت عمل جراحی DALK قرار گرفتند، انجام شد. روش بخیه ترکیبی شامل 16 بخیه Running و هشت بخیه مجزا با نخ نایلون 0-10 در زمان انجام پیوند بود. برداشتن انتخابی بخیه های مجزا جهت کاهش آستیگمات بعد از پیوند در گروهی از بیماران انجام شد. در گروه دیگری از بیماران تمام بخیه ها در یک جلسه برداشته شد. تنظیم کشش بخیه های Running در هیچ بیماری صورت نگرفت. نتیجه تاثیر برداشتن بخیه ها بر روی قدرت انکساری کل بیماران و دو زیرگروه ذکر شده مقایسه شد.یافته هامیانگین سن بیماران در زمان انجام پیوند، 6/7±26/6 سال بود. متوسط فاصله زمانی از انجام جراحی تا برداشتن کامل بخیه ها، 5/9±17/4 ماه بود. در مقایسه با مقادیر قبل از برداشتن بخیه، افزایش قابل ملاحظه ای در میزان نزدیک بینی (D 1/73±1/12-، 0/001P=)، آستیگمات انکساری (D 2/47±0/89-، 0/04P=)، میانگین کراتومتری (D 1/64±1/39-، 0/001P=) و آستیگمات کراتومتریک (D 2/43±2/61، 0/001P<) پس از برداشتن کامل بخیه ها رخ داد. تحلیل زیرگروه ها نشان داد که برداشتن انتخابی بخیه های مجزا به اندازه باقی ماندن کلیه بخیه های مجزا و Running موثر است.نتیجه گیریبرداشتن کامل بخیه ها باعث افزایش قابل ملاحظه و غیرقابل پیش بینی قدرت انکساری بعد از جراحی DALK می شود. در زیرگروهی که به طور زودرس تحت برداشتن بخیه های مجزا قبل از برداشتن کامل بخیه ها قرار گرفتند تاثیر مطلوب انجام این کار به طور قابل ملاحظه ای بعد از برداشتن کامل بخیه ها کاهش یافت.کلیدواژگان: آستیگماتیسم، انحنای قرنیه پیوندی، برداشتن بخیه ها، کراتوکونوس و قدرت انکساری

صفحات 34-41هدفمقایسه دو دستگاه متداول Allegretto EX500 (Wavelight Allegretto Wave Eye-Q Laser; Alcon Laboratories, Inc, Fort Worth, TX) و Technolas Teneo 317 (Bausch & Lomb, Rochester, NY) به لحاظ حفظ Asphericity قرنیه و تغییرات ابیراهی های رده بالا (High-Order Aberration) تحت پروفایل بهینه سازی شده در بیماران نزدیک بین آستیگمات.روش پژوهشبیماران نزدیک بین که محدوده نزدیک بینی آن ها بین 8-1 دیوپتر و حداکثر آستیگمات آن ها 5/2 دیوپتر بود، بر اساس معیارهای ورود و خروج انتخاب و به طور تصادفی با یکی از دستگاه های Allegretto EX500 و Technolas Teneo 317 تحت عمل جراحی قرار گرفتند. قبل از عمل معاینه کامل چشمی (حدت بینایی، رفرکشن اسلیت لمپ و فوندوسکوپی) و تصویربرداری شامل Orbscan، Zywave و توپولایزر صورت گرفت. شش ماه پس از عمل نیز معاینات تکرار شد. نتایج با قبل از عمل و بین دو دستگاه مقایسه گردید.یافته هااین مطالعه کارآزمایی بالینی بر روی 97 چشم از 49 بیمار با میانگین سنی 5±28 سال صورت گرفت. شش ماه پس از عمل جراحی، حدت بینایی با بهترین تصحیح بین دو گروه مشابه بود (0/152 P=). میانگین معادل کروی قبل از عمل 1/29±3/43- دیوپتر و 1/29±2/91- دیوپتر (0/047 P=) و شش ماه پس از عمل 0/19±0/14- دیوپتر و 0/23±0/08-دیوپتر (0/173 P=) در گروه های Allegretto و Teneo بود. تفاوت معنی داری بین ابیراهی های قرنیه ای (corneal) رده سوم و چهارم زرنیکه بین دو دستگاه قبل و پس از عمل یافت نشد (0/05P> ). ابیراهی های رده بالای چشمی (ocular) از 0/1±0/3و 0/31±0/12 (0/639(P= به 0/2±0/43 و± 0/18 (828/0 P=) به ترتیب در دستگاه های Allegretto و Teneo افزایش یافت. آسفریسیتی قرنیه در 30 درجه مرکزی قبل از عمل 0/12±0/3- و 0/51±0/38- (0/25 P=) و بعد از عمل 0/28±0/26 و 0/25±0/28 (0/858P= ) در گروه های Allegretto و Teneo بود. این افزایش به ازای یک دیوپتر معادل کروی در گروه Teneo بیش تر بود (0/628P= ).نتیجه گیرینتایج درمان بیماران نزدیک بین آستیگماتیسم بین دو دستگاه Allegretto و Teneo یکسان بود اگرچه تغییرات اسفریسیته در دستگاه Allegretto کم تر است و دستگاه Teneo ابیراهی های رده بالای کمتری را القا می کند.کلیدواژگان: آسفریسیتی، ابیراهی های رده بالا، عمل PRK بهینه سازی شده برای جبهه موج، دستگاه لیزر ویولایت الگرتو و دستگاه لیزر تکنولاس تنئو 317

صفحات 42-49هدفمقایسه ضخامت لنتیکول دهنده گزارش شده توسط بانک چشم با ضخامت لنتیکول اندازه گیری شده،6 ماه پس از عمل DSAEKروش پژوهشاین تحقیق، یک مطالعه مداخله ای گذشته نگر و شامل 30 چشم از 30 بیمار می باشد که تحت عمل DSAEK با بافت قبلا جدا شده توسط بانک چشم، قرار گرفتند. ضخامت لنتیکول قبل از عمل در نواحی مرکزی، اطراف مرکز و محیط بافت پیوندی توسط سیستم انتریور OCT اندازه گیری شد. هم چنین همین اندازه گیری ها، 6 ماه پس از انجام جراحی تکرار گردید. داده های قبل و بعد از عمل جراحی، با یکدیگر مقایسه شدند و ارتباط بین آن ها با استفاده از ضریب تغییرات پیرسون توصیف گردید.یافته هامتوسط سن بافت دهنده بین 10/7±47/7 سال بود. میانگین تراکم سلول های اندوتلیال قبل از جراحی از 4±3016/361 سلول در هر میلی متر مربع به 482±1644 سلول در هر میلی متر مربع کاهش یافت (0/001>P). ضخامت بافت precut قبل از عمل جراحی که توسط بانک چشم گزارش شده بود، به ترتیب در مرکز 24/9±136/8 میکرون، در ناحیه پری سنترال 3/27±149/4 میکرون و در ناحیه محیطی 32/8±198/4 میکرون و همین مقادیر به ترتیب بعد از عمل جراحی 29/8±119/2 میکرون (0/004=P)، 34/6±124/5 میکرون (0/002=P) و 45/4±156/3 میکرون (0/001>P) بود. مقایسه ضخامت بافت پیوند در نواحی مرکزی (0/001=P و 0/63=r) و پری سنترال (0/009=P و 0/5=r) قبل و 6 ماه بعد از جراحی معنی دار بود، در حالی که این ارتباط در ناحیه محیطی قرنیه معنی دارنبود (0/14=P).نتیجه گیریدر مقایسه با داده های قبل از عمل جراحی، ضخامت بافت مرکزی، اطراف مرکز و محیطی، 6 ماه پس از جراحی به ترتیب 10/4، 14/2 و 18/4 درصد کاهش یافت و این نازک شدن بافت پیوندی در محیط بافت در مقایسه با نواحی مرکزی و اطراف مرکز بیش تر بود.کلیدواژگان: ارتباط ضخامت، بانک چشم و ضخامت بافت دهنده

صفحات 50-68رتینوپاتی نوزادان نارس (ROP) علت اصلی نابینایی قابل پیشگیری در کودکان است. این بیماری در نوزادان نارس که عروق شبکیه آن ها کامل نشده، روی می دهد. عدم تکامل عروق شبکیه منجر به ایسکمی و به وجود آمدن عروق جدید می گردد. تشخیص و مداخلات درمانی زودهنگام می تواند مانع نابینایی در بسیاری از این کودکان گردد. بنابراین غربالگری ROP از اهمیت بسیاری برخوردار است. شایع ترین علت ROP، نارسی و کم وزن بودن نوزاد هنگام تولد می باشد، اگرچه دریافت اکسیژن مکمل و تغییرات سطح اکسیژن نیز در پیش رفت بیماری تاثیر دارد. عوامل ژنتیکی ممکن است باعث افزایش خطر بروز ROP شوند. انتقال خون، غلظت گلوکز و هم چنین عفونت های دوران نوزادی می توانند از عوامل خطر برای ROP باشند. در کشورهای توسعه یافته احتمال نابینایی ناشی از ROP کم تر از 10 درصد می باشد ولی در کشورهای در حال توسعه به دلیل محدودیت هایی که در معاینه شبکیه و پی گیری بیماران و تسهیلات NICU وجود دارد، این نسبت به 40 درصد می رسد. به همین دلیل توصیه می شود معیارهای غربالگری در هر کشور بومی سازی گردد. در مطالعه ای که در بیمارستان فارابی تهران در سال 1395 صورت گرفت، نشان داده شد که معیارهای غربالگری مناسب برای نوزادان در ایران gestational age مساوی یا کم تر از 32 هفته و یا وزن هنگام تولد مساوی یا کم تر از 2000 گرم می باشد. این معیار دارای حساسیت تقریبا 100 درصد در تشخیص نوزادان مبتلا به ROP نیازمند به درمان می باشد و هم زمان از معاینات غیرضروری نیز جلوگیری می کند. در این مطالعه مشخص شد که در صورت استفاده از معیارهای غربالگری توصیه شده در آمریکا، 4/8 درصد نوزادان ایرانی مبتلا به ROP که نیازمند درمان هستند، تشخیص داده نمی شوند. اولین معاینه نوزادان نارس با سن کم تر یا مساوی 27 هفته، در هفته 31 صورت می گیرد. نوزادان با سن 32-28 هفته باید در هفته 4 تا 6 بعد از تولد یا در هفته 31 تا 33 postmenstrual (هر کدام که دیرتر بود(، معاینه شوند. براساس تقسیم بندی بین المللی ROP شبکیه از جلو به عقب به سه ناحیه (zone I-III) با مرکزیت عصب بینایی و تغییرات شبکیه بر اساس شدت بیماری به 5 مرحله (5-1(stage تقسیم می گردند. بیماری پلاس (Plus) که با گشاد و پیچ و خم دار شدن عروق پل خلفی مشخص می گردد نیز از علایم شدت بیماری و نیاز به درمان می باشد. بر اساس این تقسیم بندی بین المللی، راهنمای پی گیری و اندیکاسیون درمان تعریف می گردد. تقریبا 90 درصد از ROP در مراحل 1 و 2 پسرفت می کنند. با این حال 17 درصد نوزادانی که ROP تهاجمی خلفی (aggressive posterior ROP) دارند، دچار جداشدگی شبکیه می شوند. درمان استاندارد، لیزر می باشد اما به نظر می رسد در موارد zone I درمان با تزریق بواسیزوماب داخل زجاجیه جای خود را به عنوان درمان اصلی باز کرده است. اگر چه زمان پسرفت بیماری در گروه تزریق، کوتاه تر است اما پی گیری طولانی مدت لازم است چرا که عروق شبکیه ممکن است تا مدتها کامل نشود و عوارض ناخواسته رخ دهد. موارد zone II که با تزریق بواسیزوماب داخل زجاجیه درمان شده اند، عود بیش تری نسبت به لیزر نشان داده اند. آنژیوگرافی فلورسین، در موارد مشکوک به عود، با نشان دادن نشت و افتراق عروق شانت از عروق جدید می تواند کمک کننده باشد. از دیگر درمان ها، ویترکتومی و باکل اسکلرا می باشد که در موارد کشش و کندگی شبکیه اندیکاسیون دارد. در کل، بیماری ROP به دلیل عوارض دیررس نیاز به پی گیری در تمام دوران زتدگی دارند. جدی ترین عوارض در این مرحله جداشدگی شبکیه و گلوکوم زاویه بسته می باشند.کلیدواژگان: تزریق بواسیزوماب داخل ویتره، رتینوپاتی نوزادان نارس، غربالگری و لیزر

صفحات 69-73هدفگزارش یک مورد استئومای سطحی اربیت به صورت دو ندول برجسته قابل لمس.معرفی بیمارخانم 29 ساله ای با شکایت از دو توده قابل لمس در ناحیه اینفرومدیال حاشیه اربیت چشم چپ مراجعه نمود. در لمس، دو توده سفت و ندول مانند کوچک و بدون تندرنس جلوی حاشیه تحتانی داخلی اربیت چپ لمس می شد. در CT اسکن، برجستگی استخوانی اگزوفیتیک کوچک با حدود مشخص روی زائده فرونتال استخوان فک بالایی (ماگزیلاری) وجود داشت. بیمار در زمان اولین مراجعه حاضر به جراحی نشد و بعد از پنج سال با توجه به رشد ضایعه از نظر نمای بالینی و CT اسکن، تحت جراحی قرار گرفت. با برش Modified Linch، دو توده ندول مانند خارج گردید که در بررسی میکروپاتولوژی، استئوما تشخیص داده شد.نتیجه گیریاگرچه استئومای اربیت اغلب از سینوس های پارانازال منشا می گیرد و به صورت یک توده داخل اربیت با یا بدون اجزای سطحی بروز می کند اما می تواند به صورت توده اولیه و سطحی و زیرجلدی بدون اجزای عمقی نیز تظاهر یابد.کلیدواژگان: استئومای چشمی، استئومای سطحی و ندول زیرجلدی

صفحات 74-80* این مقاله ترجمه ای از مجموعه “Focal Point” مربوط به ماه January سال 2019 با عنوان “Pain in the Normal Eye” می باشد. مقدمه انجمن بین المللی مطالعات درد، درد را به عنوان یک حس ناخوشایند و تجربه احساسی مرتبط با آسیب بافتی واقعی یا قریب الوقوع بیان می کند. بیماران، درد (شامل درد چشم) را به صورت های مختلفی تجربه می کنند و احساس درد ممکن است با یافته های فیزیکی نامتناسب باشد. درمان بیماری که با درد چشم مراجعه کرده اما معاینه چشمی کاملا طبیعی دارد، می تواند چالش برانگیز باشد. در این مبحث مروری بر یافته های اخیر مطالعه مربوط به درد، در چشم های به ظاهر طبیعی و استفاده از این نتایج در مراقبت از بیماران خواهیم داشت.کلیدواژگان: درد چشم طبیعی

Pages 3-8PurposeTo determine the prevalence of demodex infestation in patients with chronic blepharitis versus normal subjects in Iranian population.MethodsIn this case-control study, 203 consecutive patients in Torfeh and Labbafinejad Hospitals in Tehran in 2017 were enrolled and the prevalence of demodex infestation in patients with chronic blepharitis (n=102 patients) versus normal population (n=101 subjects) was determined. Demographic data, Clinical signs and symptoms was documented for every subject. Lash sampling was performed by epilating the lashes. Every eyelash has cylindrical dandruff was chosen to epilate .totally four lashes were collected from each patient, two lashes from superior eyelid and two from inferior eyelid. Then the collected lashes were inspected for Demodex based on morphology using a light microscope at the magnification 40 and 100 .The time interval between sampling and observation was less than 12 h in all cases. The main outcome measure was the prevalence of demodex infestation in each study groupResultsThe prevalence of Demodex in total study group is 26.1% and the prevalence in blepharitis group is 26.5% and in control group is 25.7% without statistically significant difference (P > 0.05). Demodex presence was related to older age, cylindrical, and some clinical symptoms such as ocular burning, and itching (P>0.05).ConclusionTotally, according to the obtained results it may be concluded that demodex infestation may not related to chronic blepharitis significantly. Pages 9-15PurposeComparison of serum vitamin D levels in patients with dry eye syndrome and control group in patients referring to ophthalmology clinic Al-Zahra of Hospital in 2016.MethodsThis case-control study was conducted in 2016 on 80 patients with a dry eye syndrome referring to ophthalmology clinic Al-Zahra of Hospital. The data were collected, using information forms and analyzed, using SPSS v.22.ResultsIn this study, 80 patients were evaluated. 60% of the patients were male and the mean age of the patients was 44.5±11.28 years. Mean vitamin D level was 20.86±10.88 ng/dl, which was not statistically different between the case and control groups. There was not a significant relationship between age and gender of the patients with vitamin D level. Vitamin D level had a significant inverse relationship with OSDI (Ocular Surface Disease Index) (r= -0.48, P=0.002).ConclusionIn this study, the severity of eye dryness based on the OSDI test was inversely related to the serum vitamin D level. Low serum vitamin D level may play a role in pathogenesis of dry eye.Keywords: Dry Eye Syndrome, Vitamin D

Pages 16-25PurposeTo evaluate the effect of orbital steroid injection in patients with active thyroid ophthalmopathy resistant to or dependent on systemic steroids, or with complications related to systemic steroids.MethodsThis prospective study was conducted on 31 eyes of 17 patients affected with active thyroid ophthalmopathy and clinical activity score of 3 or more without compressive optic neuropathy or overt exposure keratopathy. All subjects had a history of previous systemic steroid use with steroid resistance or dependence, or had developed complications related to steroids. A combination of steroids including triamcinolone 20 mg and dexamethasone 4 mg was injected in the upper and lower retroseptal orbital spaces 3 or 4 times at 1 month intervals. The patients were examined periodically after the injections and at least 3 months after the last injection; if the disease became quiescent, surgical procedures would be performed as needed.ResultsMean preinjection clinical activity score (CAS) was 5.2±1.3, which was reduced to 1.6±1.2 after the 4th injection (p<0.001). Upper and lower lid retraction improved in 100% and 68.2% of affected eyes for 1 mm or more; lagophthalmos improved significantly in 64.3% of the cases, and strabismus completely resolved in 1 of 5 affected patients and the most significant improvement was observed in supraduction. Mean improvement in proptosis was 1.2±1.1 mm. Visual acuity did not significantly change after the injections. Eyelid ecchymosis and/or subconjunctival hemorrhage was seen in 6.5% of eyes and intraocular pressure rise occurred in 8.8% of eyes, which responded to topical medications.ConclusionOrbital steroid injections can be used for treatment of active thyroid ophthalmopathy when the patient is resistant to or dependent on systemic steroids, or has developed complications of systemic steroids. Pages 26-33PurposeTo evaluate the effect of suture removal on refraction and keratometry measurements after deep anterior lamellar keratoplasty (DALK) performed in patients with keratoconus.MethodsThis retrospective study was conducted on 54 keratoconus-affected eyes that underwent DALK. A combined suturing technique consisting of a 16-bite single running and 8-bite interrupted 10-0 nylon sutures was used at the time of keratoplasty. Selective interrupted suture removal was performed before complete suture removal in a subgroup of patients to reduce post-keratoplasty astigmatism. In another subgroup, all sutures were removed at a single session. The stitching of Running sutures did not occur in any patient. The effect of suture removal on refraction and keratometry readings was investigated in the entire study group, and the two subgroups were compared.ResultsThe mean patient age was 26.6±6.7 years at the time of keratoplasty. The mean interval from surgery to complete suture removal was 17.4±5.9 months. Compared to the pre-suture removal values, there was a significant increase in the magnitudes of spherical equivalent refraction (-1.12±1.73 D; P=0.001), refractive astigmatism (-0.89±2.47 D, P=0.04), mean keratometry (1.39±1.64 D, P=0.001), and keratometric astigmatism (2.61±2.43 D, P<0.001) after complete suture removal. Subgroup analysis revealed that postkeratoplasty astigmatism could not be decreased effectively by the selective removal of interrupted sutures.ConclusionComplete suture removal was followed by an unpredictable and significant increase in post-DALK refraction and keratometry readings. Pages 34-41PurposeTo compare maintaining corneal asphericity and high-order aberration changes in optimized PRK for myopic astigmatism patients, using Allegretto EX500 (WaveLight Allegretto Wave Eye-Q laser devices; Alcon Laboratories, Inc., Fort Worth, TX) and Technolas Teneo 317 (Bausch & Lomb, Rochester, NY).MethodsThe patients with myopic astigmatism, whose range of myopic was between 1 and 8 diopters, and maximum astigmatism of 2.5 diopters, were selected based on inclusion and exclusion criteria and, then, randomly operated with one of the Allegretto EX500 and Technolas Teneo 317 devices. Before the operation, a complete visual examination (visual acuity, refraction, Slit lamp, and Fundoscopy) and imaging including orbscan, zywave, and topolyzer were performed. The examinations were repeated 6 months after surgery. The results were compared before surgery and between the two devices.ResultsThis clinical trial study was performed on 97 eyes of 49 patients with mean age of 28±5 years. Six months after surgery, visual acuity with the best correction was similar between the two groups (P=0.152). The mean spherical equivalent before surgery was -3.43±1.29 and -2.91±1.29 (P=0.047) and 6 months after surgery, -0.14± 0.19 and -0.08±0.23 (P=0.173) in the Allegretto and Teneo groups, respectively. There was no significant difference between the two groups before and after the surgery (P>0.05) between the third and fourth order corneal aberrations. The ocular higher order aberration increased from 0.3±0.1 and 0.31±0.12 (P=0.639) to 0.43±0.2 and 0.180±.42, (P=0.828) in the Allegretto and Teneo, respectively. Corneal asphericity was at 30° before operation -0.3±0.12 and -0.38±0.51 (P=0.25) and after operation, 0.28±0.26 and 0.28±0.25 (P=0.858) in the Allegretto and Teneo groups. This increase was higher per a diopter of spherical equivalent in Teneo group (P= 0.628).ConclusionThe results of the treatment of the two Allegretto and Teneo devices were the same in correction of myopic astigmatism, although the Allegretto device induces less changes in asphericity, and the Teneo device induces less higher-order aberrations.Keywords: Asphericity, High-order Aberration, Optimized PRK, Wavelight Allegretto EX500, Technolas Teneo

Pages 42-49PurposeTo compare donor lenticule thickness reported by the eye bank with lenticule thickness measured at 6 months after Descemet stripping automated endothelial keratoplasty (DSAEK).MethodsThis prospective, interventional case series enrolled 30 eyes of 30 patients, who underwent DSAEK, using precut eye bank tissue. Preoperative thickness measurements of the donor lenticules were taken centrally and at pericentral and peripheral graft areas using an anterior segment optical coherence tomography system. The same measurements were repeated 6 months postoperatively. The preoperative and postoperative values were compared, and the relationships between these measurements were investigated, using Pearson correlation coefficients.ResultsThe mean donor age was 47.7±10.7 years old. The mean preoperative endothelial cell density was 3016.8±361.4 cells/mm2, which decreased to 1644.9±482.0 cells/mm2 postoperatively (P<0.001). The graft thickness of the precut tissue, as reported by the eye bank, was 136.8±24.9 μm at the center, 149.4±27.3 μm at the pericentral area, and 198.4±32.8 μm at the periphery. The corresponding postoperative values were 119.2±29.8 μm (P=0.004), 124.5±34.6 μm (P=0.002), and 156.3±45.4 μm (P<0.001), respectively. The graft thickness measured by the eye bank was significantly associated with that measured in vivo at 6 months at the central (r=0.63; P=0.001) and pericentral (r=0.5; P=0.009) graft area. However, the correlation between preoperative and postoperative peripheral graft thickness was insignificant (P= 0.14).ConclusionCompared to preoperative values, central, pericentral, and peripheral graft thicknesses were reduced by 10.4%, 14.2%, and 18.4%, respectively, 6 months after DSAEK. Pages 50-68Retinopathy of prematurity (ROP) is the main reason of preventable blindness in pediatric age group. In this disease, incomplete development of retinal vessels in a premature neonate cause retinal ischemia and formation of retinal new vessel, which can result in retinal detachment and vision loss. Early diagnosis and intervention can prevent blindness; therefore, timely screening should be highlighted in these infants. The most important risk factors for developing ROP in a premature infant are low birth weight and low gestational age. Other possible predisposing elements are genetic, supplementary oxygen and variations in blood oxygen level, blood transfusion, sepsisو and blood glucose level. In developed countries, chances of ROP causing blindness is less than 10%, whereas this proportion is up to 40% in developing countries due to limitations in resources, neonatal care and NICU facilities, and availability of retinal exam. Therefore, it is recommended that screening guidelines be customized in each country. Based on a research conducted at Farabi eye Hospital in Tehran, 2016, screening guidelines in Iran has been defined as gestational age £32 weeks and gestational weight £2000 gram. Following this guideline, infants need treatment can be identified with a sensitivity of about 100%, while unnecessary exams will be at its lowest rate. Using screening recommendations of American Academy of Pediatrics would miss 8.4% ROP requiring treatment in this cohort. The first retinal examination of premature infants with gestational age of £27 weeks should be at 31st week of postmenstrual age. This time for neonates with gestational age of 28-32 weeks is at 4-6 weeks after birth or postmenstrual age of 31-33 weeks (whichever is later). An international committee of ROP classified retina (with the centration of optic nerve) into 3 zones (I-III) and categorized severity of ROP into 5 stages (1-5). Plus disease, which is defined as vascular dilation and tortuosity in posterior pole, shows severity of disease and it is an indication for treatment. Guidelines for examination follow up and treatment is based on this international classification. 90% of ROP spontaneously regress in stage 1 and 2. Of those with aggressive posterior ROP, approximately 17% progress to retinal detachment. The gold standard of treatment of ROP is laser, which ablates avascular retina and helps control the activity of disease and regression of new vessels. Intravitreal injections of anti VEGF (Bevacizumab) has been recently considered the treatment of choice for treatment of zone I ROP. Although the regression time is shorter in the injection group, these babies require a longer follow-up time for the retina to become fully vascularized and should be monitored for the possible complications may occur in avascular retina. Recurrence showed to be more common in posterior zone II cases. Fluorescein angiography can be helpful in suspicious cases of recurrence due to its potential in differentiating new vessels from shunt vessels. Scleral buckling and vitrectomy are treatment options when there is a retinal detachment. Overall, children with ROP need a long life follow-up for managing the possible complications, of which the most devastating are retinal detachment and glaucoma.Keywords: Intravitreal Anti VEGF Injection, Laser, Screening, Retinopathy of prematurity

Pages 69-73PurposeTo report a case of superficial orbital rim osteoma.Case ReportA 29--year-old woman presented with two small and firm subcutaneous nodule in anterior aspect of the left inferonasal orbital rim. Orbital CT scan showed small well circumscribed exophytic bone density masses on frontal process of maxillary bone. The patient did not undergo surgery at the time of the first referral. After 5 years, she was treated with clinical examination and CT scan according to the growth of the lesion. Complete surgical excision was performed through modified Linch incision. Histopathology evaluation was consistent with ivory type osteoma.ConclusionAlthough the orbital rim osteoma often originates from paranasal sinuses and appears as a mass in the orbit with or without superficial components, it may present as subcutaneous nodules on orbital rim.Keywords: Orbital Osteoma, Sub Cutaneous Nodule, Superficial Osteoma
