سندرم تحریک بیش از حد تخمدان، یک بیماری یاتروژنیک است که با تحریک تخمدان در روشهای کمک باروری و سایر درمانهای ناباروری مرتبط است. این مطالعه با هدف مقایسهی اثربخشی ستروتاید و کابرگولین در پیشگیری و درمان سندرم تحریک بیش از حد تخمدان انجام شد.
روشهادر این مطالعهی نیمه تجربی که در سال 1398 در بیمارستان شهید بهشتی اصفهان انجام شد. 30 زن نابارور تحت درمان با روش کمک باروری خارج رحمی، به طور تصادفی به دو گروه تقسیم شدند. گروه اول با ستروتاید و گروه دوم با کابرگولین تحت درمان قرار گرفتند. سن، شاخص تودهی بدنی، دز کل گنادوتروپین تجویز شده، تعداد تخمکها و سطح استرادیول، مدت زمان بستری در بیمارستان و شدت علایم تحریک بیش از حد تخمدان در دو گروه تعیین و مقایسه گردید.
یافتههافراوانی بروز سندرم تحریک بیش از حد تخمدان در هر گروه 2 مورد (33/13 درصد) بود. همچنین، میزان گنادوتروپین تزریقی، تعداد تخمکهای برداشت شده، سطح استرادیول، مدت بستری و میزان پاراسنتز بین دو گروه اختلاف معنیداری نداشت (050/0 < P).
نتیجهگیریطبق نتایج مطالعهی حاضر، تاثیر دو داروی کابرگولین و ستروتاید در پیشگیری از بروز و شدت سندرم تحریک بیش از حد تخمدان، مدت بستری و میزان نیاز به پاراسنتز مشابه است و ارجحیتی در استفاده از هر یک از داروهای پیشگفته وجود ندارد.

کبد چرب غیر الکلی، از آسیبهای مزمن کبدی است که شیوع آن در جهان رو به افزایش است. از این رو، هدف از انجام مطالعهی حاضر، بررسی اثرات تمرین ورزشی به همراه مصرف مکمل سیلیمارین بر آنزیمهای کبدی و وزن Ratهای نر مدل کبد چرب بود.
روشهادر این پژوهش، 40 سر Rat نر بالغ نژاد Wistar با میانگین وزنی 3 ± 159 گرم به طور تصادفی به 5 گروه (8 = n) شامل رژیم غذایی معمولی/سالین، رژیم غذایی پر چرب/سالین، رژیم غذایی پرچرب/مکمل، رژیم غذایی پرچرب/تمرین/سالین، رژیم غذایی پرچرب/تمرین/مکمل تقسیم شدند. مدلسازی کبد چرب با رژیم غذایی پر چرب که حاوی 45 درصد انرژی از کربوهیدرات، 41 درصد از چربی و 14 درصد از پروتئین بود، با دز روزانهی 10 گرم به ازای 100 گرم وزن بدن انجام شد. گروه شاهد، رژیم غذایی معمولی داشتند. گروههای تمرینی به مدت هشت هفته (5 جلسه در هفته، هر جلسه 30 دقیقه) به تمرین تناوبی در شدتی نزدیک به VO2max پرداختند. مکمل سیلیمارین و سالین به مدت 2 هفته روزانه 140 میلیگرم/کیلوگرم وزن بدن گاواژ شد. 72 ساعت بعد از اتمام تمرین، نمونهبرداری از بافت کبد جهت ارزیابی آنزیمهای کبدی انجام گرفت. دادهها با روش One-way ANOVA و آزمون تعقیبی Tukey تجزیه و تحلیل شدند (050/0 > P).
یافتههاآنزیمهای Alanine transaminase (ALT)، Aspartate aminotransferase (AST) و Alkaline phosphatase (ALP) کبدی در گروههای تمرین تناوبی و تمرین تناوبی/مکمل سیلیمارین کاهش معنیداری نسبت به گروه شاهد داشتند، اما تفاوت معنیداری در گروههای مکمل سیلیمارین نسبت به گروه شاهد دیده نشد.
نتیجهگیریاستفادهی همزمان از مکمل سیلیمارین در کنار تمرینات ورزشی به ویژه از نوع تناوبی و با شدت زیاد میتواند باعث تعدیل پاسخهای التهابی و اکسایشی شود و روش مناسبی برای درمان بیماران مبتلا به کبد چرب غیر الکلی باشد.

با توجه به اهمیت کنترل عود مجدد بیماری سرطان کولورکتال و تاثیر پروفایل لیپیدی سرم و نوع چربی غشای تومور بر میزان پاسخ به درمان و بقا و عدم وجود مطالعهی بالینی در مورد اثر مکمل یاری اسید لینولییک کونژوگه (Conjugated linoleic acids یا CLA) در این بیماران، مطالعهی حاضر تاثیر مکمل یاری CLA بر الگوی اسیدهای چرب و پروفایل لیپیدی سرم و نشانگر تومور (Carcinoembryonic antigen یا CEA) در بیماران مبتلا به سرطان رکتال تحت درمان با کمورادیوتراپی قبل از عمل جراحی مورد بررسی قرار گرفت.
روشهادر این کارآزمایی بالینی، 31 بیمار مبتلا به سرطان رکتال تحت درمان با کمورادیوتراپی به طور تصادفی به دو گروه مورد یا مکمل (15 = n) و شاهد یا دارونما (16 = n) تقسیم شدند. گروه مکمل، روزانه 3 گرم اسید لینولییک کونژوگه و گروه دیگر، دارونما به مدت 6 هفته دریافت کردند. قبل و بعد از مداخله، اسیدهای چرب، مقادیر تریگلیسیرید، کلسترول، Low-density lipoprotein-cholesterol (LDL-C)، High-density lipoprotein-cholesterol (HDL-C) و CEA سرم ارزیابی شدند.
یافتههامکمل یاری با CLA سبب افزایش درصد ایزومرهای CLA در مقایسه با دارونما شد (35/0 = P). همچنین، مقادیر آلفا اسید لینولنیک در گروه مکمل به طور معنیداری افزایش (01/0 = P) و مجموع اسیدهای چرب امگا6 و نسبت امگا6 به امگا3 کاهش یافت؛ در حالی که در گروه دارونما این مقادیر افزایش یافتند. مکمل یاری با CLA در مقایسه با دارونما تاثیر معنیداری بر پروفایل لیپیدی سرم نداشت (05/0 < P).
نتیجهگیریبا توجه به اثرات مثبت مکمل CLA بر مقادیر اسیدهای چرب امگا3 و این که مطالعهی حاضر، اولین مطالعهی بالینی در مورد تاثیر مکمل یاری CLA در این بیماران بود. برای تعمیم یافتههای مطالعهی حاضر، انجام مطالعات با حجم نمونهی بیشتر ضروری به نظر میرسد.

Ovarian hyperstimulation syndrome (OHSS) is an iatrogenic disease that is related to ovarian stimulation by assisted reproduction technology and other infertility treatments. The aim of this study was to determine the effect of GnRH antagonist and cabergoline (Dostinx) on the treatment of ovarian hyperstimulation syndrome in women exposed to this syndrome.
MethodsIn this quasi-experimental study conducted in Shahid Beheshti hospital in Isfahan, Iran, in 2020, 30 infertile women treated with in vitro fertilization (IVF) were randomly divided into two groups; the first group was treated with steroids and the second group with cabergoline. Age, body mass index, total dose of prescribed gonadotropin, number of oocytes and estradiol level, length of hospital stay, and severity of ovarian hyperstimulation symptoms were determined and compared between the group.
ResultsThe frequency of ovarian hyperstimulation syndrome in each group was 2 cases (13.33%). Besides, the amount of injected gonadotropin, number of eggs removed, estradiol level, length of hospital stay and paracentesis were not significantly different between the two groups (P > 0.050 for all).
ConclusionAccording to the results of the present study, the effect of cabergoline and cetrotide in preventing the occurrence and severity of ovarian hyperstimulation syndrome, length of hospital staying, and the need for paracentesis are similar, and there is no preference in using any of these drugs.

Non-alcoholic fatty liver disease includes a spectrum of clinical syndromes from early steatosis to liver cirrhosis. Therefore, the aim of the present study was to investigate the effects of exercise training with silymarin supplementation on liver enzymes and weight of male fatty liver rats.
MethodsIn this study, 40 adult male Wistar rats weighing 159 ± 3 g were randomly divided into 5 equal groups of normal diet/saline, high-fat diet/saline, high-fat diet/silymarin supplement, high-fat diet/training/ saline, and high-fat diet/training/supplement. Modeling of fatty liver with high-fat diet containing 45% energy from carbohydrates, 41% fat, and 14% protein was performed at a daily dose of 10 g per 100 g of body weight; the control group had a normal diet. The training groups practiced intermittently for 8 weeks (5 sessions per week, each session lasting 30 minutes) at intensity close to VO2max. Silymarin and saline supplementation was gavaged as 140 mg daily per kg body weight for 2 weeks. Liver tissue sampling was performed 72 hours after exercise to evaluate liver enzymes. Data were analyzed using one-way analysis of variance and Tukey post hoc test (P <0.05).
FindingLiver alanine transaminase (ALT), aspartate aminotransferase (AST), and alkaline phosphatase (ALP) enzymes significantly reduced in the periodic training and periodic exercise/silymarin supplement groups compared to the control group; but no significant difference was observed in silymarin supplement groups compared to the control group.
ConclusionUse of silymarin supplement with exercise, especially intermittent and high-intensity exercise, can modulate inflammatory and oxidative responses, and can be a suitable method to treat patients with non-alcoholic fatty liver disease.

Due to the importance of controlling recurrence of colorectal cancer, the effect of serum lipid profile and the fatty acid composition of cancer cell membranes on the response to treatment and survival, and the lack of clinical study on the effect of conjugated linoleic acid supplementation (CLA) in these patients, this study investigated the effect of CLA supplementation on serum fatty acid pattern, lipid profiles, and carcinoembryonic antigen (CEA) tumor marker in patients with rectal cancer treated with preoperative chemotherapy.
MethodsIn this randomized double-blind placebo-controlled pilot study, 31 patients with rectal cancer undergoing chemoradiotherapy assigned into the CLA group (n = 15), receiving 3 g CLA/d, and placebo group (n = 16) receiving placebo capsules for 6 weeks. Before and after intervention, serum fatty acid pattern, triglyceride, cholesterol, low-density lipoprotein-cholesterol (LDL-C), high-density lipoprotein-cholesterol (HDL-C), and CEA were evaluated.
FindingsIn placebo group, CLA supplementation increased the percentage of serum CLA (P = 0.35). CLA supplementation increased alpha linolenic acid (P = 0.01) significantly, and decreased the total omega-6 fatty acids and the omega-6 to omega-3 ratio. As compare with placebo, CLA supplementation had no significant effect on lipid profiles (P > 0.05).
ConclusionConsidering the positive effects of CLA on omega-3 fatty acids, and to our knowledge, this is the first study that assess the effect of CLA supplementation; further studies with larger sample size are needed to generalize findings of this study.
