    This preliminary investigation explored additive manufacturing to fabricate cobalt-chromium onlay restorations without the use of digital design. Extracted molars were prepared for foursurface onlays followed by the conventional approach for the fabrication of provisionals. The provisionals were digitized with an intraoral scanner, and stereolithography (STL) files were fabricated with additive manufacturing in cobalt-chromium, utilizing selective laser melting (SLM). Onlays were bonded to the corresponding tooth. Restorations were polished after cementation and assessed with photography, radiography, and a clinical post-cementation checklist. Cementation was unremarkable; marginal adaption and surface finish were generally acceptable. A simple, efficient, and inexpensive alternative workflow for the fabrication of indirect restorations without using the digital design is proposed.

    Background

     Direct pulp capping is a method designed to preserve the exposed dental pulp. Due to good biological, physical, and mechanical properties, new versions of calcium silicate-based materials have been developed as pulp capping materials. The present study aimed to evaluate the cytotoxic effects of four calcium silicate-based pulp capping materials, of which the Bio-C Repair Íon+ is still in an experimental phase.

    Methods

     Biodentine, MTA Repair HP, Bio-C Repair, and Bio-C Repair Íon+ cements were dispensed in a metallic matrix to produce 125-mm3 specimens, which were immersed in Dulbecco’s Modified Eagle Medium (DMEM) to obtain extracts. NIH 3T3 cells were cultured and exposed to the extracts for 24 hours and seven days. Cell viability was assessed by the methyl tetrazolium test (MTT). The mean values for the experimental and control groups (without treatment) were compared by analysis of variance (ANOVA) and post hoc Tukey tests, considering a significance level of 5%.

    Results

    All the tested materials demonstrated a reduction in cell viability (P<0.05). According to ISO 10993-5: 2009 (E), Bio-C Repair Íon+ exhibited mild and moderate cytotoxicity in the 24- hour and 7-day analyses, respectively. Bio-C Repair and Biodentine showed mild cytotoxicity, and MTA Repair HP exhibited moderate cytotoxicity at both intervals.

    Conclusion

    The highest cell viability was demonstrated by Biodentine, MTA, and Repair HP, in descending order. Bio-C Repair and Bio-C Repair Íon+ showed moderate cytotoxicity, similar to MTA Repair HP in the 7-day analysis.

    Background

    The present study aimed to compare the efficacy of sonic irrigation and conventional syringe irrigation (CSI) in terms of curcumin (CUR) and triple antibiotic paste (TAP) removal from a standardized groove artificially created in root canals.

    Methods

     The root canals of 72 anterior maxillary teeth were prepared using the Reciproc system to size R50. The teeth were split longitudinally, and a standardized groove was created in the apical region of one root half. The standardized grooves were filled with CUR or TAP with the exclusion of six teeth that served as the negative control group, and then the root halves were reassembled. The teeth were divided into two subgroups according to the irrigation protocols used: sonic activation with EndoActivator (EA) or CSI (n=15). After the removal of the medicament, the residual medicament was assessed under a stereomicroscope. Kruskal-Wallis and Mann-Whitney U tests were used for statistical analyses (P = 0.05).

    Results

    The EA sonic activation method was significantly more efficient in removing CUR medicament from the root canals. Considering the medicament types, more CUR than TAP was removed from the root canals using both CSI and the EA (sonic activation) system (P<0.05).

    Conclusion

     As compared with CSI, the EA was not significantly more efficient in removing TAP, but it was significantly more effective than CSI in removing CUR.

    Background

    This retrospective study aimed to investigate the effect of various dental and maxillary sinus variables on maxillary sinus mucosal thickness (MT). The variables included periodontal bone loss (PBL), periapical status, dental restorations of posterior maxillary teeth, and the distance from the root apices to the sinus mucosa.

    Methods

     Cone-beam computed tomography (CBCT) images of the maxillary sinuses (n=600) in 300 patients were examined. The sinus MT and the distance of the roots from maxillary sinuses were measured. Apical lesions of the roots, PBL, and situations of adjacent teeth were recorded. The relationships between these conditions and MT and characterization of MT were evaluated. The Kruskal–Wallis H test was used to compare groups due to the non-normal distribution of the data. The relationship between categorical variables was analyzed using chi-squared test.

    Results

     There was a significant correlation between maxillary sinus MT and periapical lesions, PBL, and restorations (P<0.05). MT increased as the apical lesions of premolar teeth enlarged (P<0.05, P=0.022). MT increased in cases of mild PBL of molar teeth (P=0.041).

    Conclusion

     In this retrospective study, the MT significantly increased in patients with periapical lesions, inadequate endodontic treatment, increased PBL, and inadequate dental restorations.

    Background

     This study aimed to compare the cyclic fatigue resistance of ProTaper Next, Hyflex CM, 2Shape, and TF-Adaptive nickel-titanium endodontic file systems with various alloy properties and production methods and investigate the fractured cross-sectional surface of files due to cyclic fatigue by scanning electron microscopy (SEM).

    Methods

     A total of 120 instruments were used (n=30). For standardization, #25/.06 apical diameter and taper angle were selected for each file system. The experiment of files was subjected to a static cyclic fatigue model. The time for files’ failure was recorded with a digital chronometer and multiplied by the rotation speed to calculate the number of cycles. Kolmogorov-Smirnov, one-way ANOVA, and post hoc Bonferroni analysis were used for statistical analysis. Statistical significance was set at P<0.05.

    Results

    The number of cycles for the failure of files was compared between the groups, and significant differences were found (P<0.05). The number of cycles for instrument failure was recorded from the highest to the lowest as follows: Hyflex CM, TF-Adaptive, ProTaper Next, and 2Shape.

    Conclusion

     The files were fractured at different average numbers of cycles in an artificial canal in all the groups. The Hyflex CM demonstrated better cyclic fatigue resistance than TF Adaptive, ProTaper Next, and 2Shape file systems. Factors such as production patterns, alloy properties, and the phase in which the files were produced might affect the lifespan of file systems.

    Background

     Ferritin is a positive acute phase protein (APP) in inflammation and chronic infections, including chronic periodontitis. Two key factors that can regulate ferritin expression are iron and pro-inflammatory cytokines. Serum ferritin levels increase after menopause, affecting women’s health. This study aimed to evaluate serum ferritin levels in postmenopausal women upon undertaking non-surgical periodontal treatment.

    Methods

     In this cross-sectional study, blood samples of 38 postmenopausal women with chronic periodontitis were collected before any treatment. The serum ferritin levels and periodontal parameters, probing depth (PD), clinical attachment level (CAL), and gingival index (GI) were recorded at baseline and three months after non-surgical periodontal therapy. Wilcoxon test was used to compare serum ferritin levels before and after treatment. T-test was used for comparison of periodontal parameters, with a P value of ≤0.05 considered significant.

    Results

     A decrease was observed in the serum ferritin level (from 108.55 mcg/L to 98.28 mcg/L) after treatment compared to baseline (P<0.001). Also, significant improvements in periodontal parameters were observed compared to the baseline (P<0.001).

    Conclusion

    Based on the results, it can be concluded that non-surgical periodontal treatment significantly reduces serum ferritin levels in postmenopausal women with chronic periodontitis.

    Background

     The present study aimed to evaluate the sealing ability of three different calcium silicate-based materials in furcation perforations.

    Methods

     Seventy-six human mandibular molar teeth were selected. Perforations were created in the center of the pulp chamber floor. The experimental teeth were randomly divided into three groups (n=22). Perforations were repaired with MTA Angelus, Endocem MTA, or EndoSequence BioCeramic Root Repair Material Fast Set Putty (BC-RRM Putty). Microleakage of the different repair materials to be tested was measured by computerized fluid filtration method at 24- and 72-hour intervals.

    Results

     For each time interval, no statistically significant difference was observed between the groups. For Endocem MTA and BC-RRM Putty groups, the difference between the leakage values measured at both periods was not statistically significant (P>0.05). However, there was a significant difference for the MTA Angelus group (P<0.05).

    Conclusion

     All the calcium silicate-based materials used in the present study showed similar performance in repairing furcation perforations at 24- and 72-hour intervals.

    Background

     The present study aimed to evaluate the influence of the veneering technique on the tensile stress distribution and survival of full-ceramic fixed dental prostheses (FDPs).

    Methods

     A three-dimensional model of an FDP was modeled on a second premolar and a second molar with a pontic between them for finite element analysis (FEA). The groups were divided according to the veneering technique: conventional stratification, rapid layer, and CAD-on techniques. A mesh control test determined the number of elements and nodes. The materials’ properties were attributed to each solid component with isotropic, homogeneous, and linear elastic behavior. For the in vitro fatigue test (n=30), the FDPs were cemented on dentin analog abutments and submitted to 2×106 mechanical cycles (100 N at 3 Hz).

    Results

     Maximum principal stress showed that the connector between the pontic and the second molar concentrated higher stresses, regardless of the techniques: Rapid layer (6 MPa)> CAD-on (5.5 MPa)> conventional stratification (4 MPa). The conventional stratification technique concentrated high stresses at the interface between the framework and veneering ceramic (2 MPa), followed by the rapid layer (1.8 MPa) and CAD-on (1.5 MPa) techniques. The crowns fabricated using the rapid layer and CAD-on techniques exhibited a 100% survival rate, while the conventional stratification group had 0% survival.

    Conclusion

    Even with similar stress distribution between the veneering techniques, the conventional stratification technique was more prone to failure under fatigue due to higher defects incorporated than CAD-on and rapid layer techniques.

    Background

     Ultrasonic scaling generates aerosols and splatters contaminated with microorganisms, increasing the risk of disease transmission in the dental office. The present study aimed to evaluate the effectiveness of extraoral suction (EOS) units in aerosol and splatter reduction during ultrasonic scaling.

    Methods

     Ultrasonic scaling was conducted on a dental manikin headset to simulate a scaling procedure. Water containing Lactobacillus acidophilus at a concentration of 107 colony-forming units per milliliter and 1% fluorescein solution was used as the water supply of the scaler. The scaling procedure was conducted with a high-volume evacuator (HVE) or the combination of HVE and an EOS unit. de Man–Rogosa–Sharpe agar plates were placed at different distances surrounding the dental chair. Filter papers were placed at various positions surrounding the oral cavity and on areas of the body.

    Results

     Bioaerosols were detected at every sampling site and could travel as far as 150 cm from the oral cavity. The combination of HVE and EOS significantly reduced the total number of bacterial colonies in the air (P<0.001). Dissemination of the stain was in the range of 20 cm from the oral cavity. The maximum contaminated surface area was at the 4 o’clock position from the oral cavity. The combination of EOS and HVE significantly reduced the contaminated area (P<0.05). The stain was also found on the wrists, chest, abdomen, and lap of the operator and assistant. The lap was the most contaminated area of the body.

    Conclusion

     EOS was effective in reducing the bioaerosols and splatters generated during ultrasonic scaling.

    Background

     Continuing assessment of the quality of evaluation programs promotes the quality of exams and ensures learners’ accurate evaluation. This study aimed to examine the quality of the competency-based evaluation program for dentistry based on the context, input, process, and product (CIPP) model.

    Methods

     In a mixed-methods study (quantitative-qualitative), dentistry students’ evaluation program using competency-based assessment methods was examined by dentistry professors based on the CIPP model and via a reliable and valid researcher-made questionnaire. The questionnaire had three sections on demographic information, evaluation questions, and open-ended questions. Descriptive statistics (mean and SD) were used to analyze the questionnaire items. Open-ended questions were analyzed by content analysis, and the quantitative part was analyzed using SPSS.

    Results

    Twenty-five faculty members from the departments of orthodontics and dental prosthesis completed the questionnaires. The overall level of faculty members’ satisfaction with the new evaluation program was above average (54±17.02). They had the highest degree of satisfaction with output indices and the lowest degree with input indices. The analysis of the open-ended questions yielded two general categories of “providing the human and physical infrastructure” and “spiritual support and encouragement of educational innovation.”

    Conclusion

    The competency-based evaluation program needs the support of managers and planners. The faculty should provide the infrastructure for the implementation of these methods. By meeting the requirements, the professors will be motivated to implement these methods, and the paradigm can shift from traditional to novel evaluation methods.

    Background

     Interfacial failures at the cement‒restoration interface highlights the importance of effective surface treatment with no adverse effect on the zirconia’s mechanical properties. This study aimed to determine the effect of different surface treatments on dental graded zirconia’s surface roughness and certain mechanical properties.

    Methods

      Forty sintered zirconia specimens were randomly divided into four groups (n=10): control (no surface treatment), sandblasting (SA), grinding with diamond bur (GB), and Er,Cr:YSGG laser (LS). Following surface treatment, the surface roughness and surface topography of the specimens were examined. X-ray diffraction (XRD) was conducted. In addition, the biaxial flexural strengths of specimens were evaluated. The data were analyzed using one-way analysis of variance (ANOVA) and post hoc Tukey tests; the Pearson correlation coefficient was calculated between either volumetric percentage of monoclinic phase or roughness and flexural strength of specimens (α=0.05).

    Results

     The GB group exhibited significantly greater surface roughness compared to the other groups (P<0.005). The LS and control groups exhibited a significantly lower volumetric percentage of the monoclinic phase (P<0.001) than the GB and SA treatments. The SA group exhibited significantly higher flexural strength than the control (P=0.02) and GB groups (P<0.01). Furthermore, the Weibull analysis for the LS showed higher reliability for the flexural strength than other treatments.

    Conclusion

     Er,Cr:YSGG laser treatment, with the lowest extent of phase transformation and reliable flexural strength, can be a promising choice for surface treatment of zirconia.

    Head and neck cancers are highly prevalent worldwide. Most of these lesions are diagnosed in the advanced stages of the disease. Thus, they do not often have a good long-term prognosis. Like other cancer types, head and neck cancers are managed by surgery, radiotherapy, and chemotherapy. Despite significant advances in the treatment of oral squamous cell carcinoma (OSCC), physicians encounter several challenges in the course of treatment. Various mechanisms mediate the clinical responses of a certain cancer to medications. Thus, efficient treatment planning requires adequate knowledge about the genes involved in drug resistance and the evaluation of the frequency percentage of resistance. Several studies have evaluated the causes and frequency percentages of 5-fluorouracil (5-FU) and cisplatin resistance. In this systematic review, all the relevant articles published until November 30, 2019, were retrieved from the Scopus, Embase, Medline, ISI, Web of Science, and Cochrane databases using certain MeSH and EMTTree keywords. A total of 2164 articles were retrieved of which, 18 were included in the review since they had reported the frequency percentages of drug resistance. Of all, 10 articles had evaluated cisplatin (1317 samples). A meta-analysis of the results revealed a frequency of 33% for cisplatin resistance. Eight studies had evaluated 5-FU (476 samples). A meta-analysis of the results revealed a frequency of 40.2 % for 5-FU resistance. Overcoming cisplatin resistance or 5-FU resistance can significantly enhance recovery in advanced HNSCC. Attempts should be made to eliminate the cause and use multi-drug regimens to increase the success rate of treatment.
