بررسی های اخیر نشان از تحت تاثیر قرار گرفتن عملکرد غده تیرویید در مواجهه با فلزات سنگین دارد. هدف مطالعه ی حاضر، بررسی غلظت فلزات سنگین شیر مادر در روزهای اولیه پس از زایمان و ارتباط آن با هایپرتیروتروپینمی نوزادان است.
مواد و روش هادر این مطالعه ی مورد-شاهد، 82 زوج مادر-نوزاد شامل 21 زوج (دارای نوزاد هایپرتیروتروپینمی) در گروه مورد و 61 زوج (دارای نوزاد سالم) در گروه شاهد در مقطع زمانی 5-3 روز بررسی شدند. غلظت تیروتروپین در نمونه ی خون پاشنه ی پا بیشتر از 5 میلی واحد در لیتر به عنوان هایپرتیروتروپینمی در نظر گرفته شد. در گروه مورد، جهت تایید تشخیص، غلظت تیروتروپین در سرم جداشده از نمونه خون وریدی نیز اندازه گیری شد. در همان مقطع زمانی، نمونه ی شیر از مادران هر دو گروه؛ جهت اندازه گیری غلظت فلزات سنگین شامل؛ جیوه، سرب و کادمیوم اخذ گردید.
یافته هامیانه (دامنه بین چارک) غلظت تیروتروپین نمونه خون پاشنه ی پا در نوزادان سالم و دارای هایپرتیروتروپینمی به ترتیب (1/85-0/40) 0/90 و (8/80-5/70) 6/80 میلی واحد در لیتر بود (0/001<p). این مقدار در نمونه ی وریدی نوزادان دارای هایپرتیروتروپینمی (6/75-5/00) 5/50 میلی واحد در لیتر بود. هیچ یک از فلزات سنگین شیر مادر همبستگی آماری معنی داری با غلظت تیروتروپین نمونه ی خون پاشنه پا در گروه نوزادان سالم و دارای هایپرتیروتروپینمی نشان ندادند. لیکن، بین غلظت سرب شیر مادر با غلظت تیروتروپین نمونه وریدی همبستگی آماری معنی داری وجود داشت (0/478=r، 0/029=P). این همبستگی در مورد غلظت جیوه و کادمیوم شیر مادر مشاهده نشد.
نتیجه گیریطبق یافته های حاضر، غلظت سرب شیر مادر همبستگی معنی داری با عملکرد تیرویید نوزادان دارد. با این حال، جهت تایید نتایج فوق، مطالعات بیشتر با حجم نمونه بالاتر ضروریست.

بسته مداخلات اساسی بیماری های غیرواگیر سازمان جهانی بهداشت (WHO-PEN)، امکان پیشگیری، تشخیص و مدیریت زودهنگام بیماری های قلبی-عروقی، دیابت، بیماری های مزمن تنفسی و سرطان را فراهم می کند. برنامه ایراپن (Ira-PEN)با بومی سازی WHO-PEN برای جمعیت ایرانی طراحی و از سال 1394 به صورت آزمایشی در 4 شهرستان نقده، مراغه، شهرضا و بافت اجرا و سپس در کل کشور اجرایی شد. هدف ما بررسی شیوع عوامل خطر قلبی-عروقی درمردان و زنان مراجعه کننده به مراکز بهداشتی بود.
مواد و روش هادراین مطالعه از داده های سال های 1395و1396 افراد بالای30 سال مراجعه کننده به مراکز در مناطق آزمایشی استفاده شد. خطر10ساله قلبی-عروقی توسط نرم افزار سیب به عنوان یک سیستم اطلاعات یکپارچه سلامت با استفاده از نمودار پیش بینی خطر WHO/ISH تعیین شده است. افراد در چهار گروه خطر: کم (<10%)، متوسط (20-10%)، نسبتا زیاد (30-20%) و زیاد (>30%) طبقه بندی شده اند.
یافته هادربین 160223 نفر (58/1 % زن) شیوع دیابت (فاصله اطمینان 95%) در مردان و زنان به ترتیب 8/5 % (8/8-8/3) و 12/9 % (13/1-12/7) بود که حدود یک سوم موارد آن ها ناشناخته بودند. شیوع فشارخون بالا (فاصله اطمینان 95%) در مردان و زنان به ترتیب 15/6(15/9-15/3) و23/2 (23/5-22/9) بود و فشارخون بالای ناشناخته 40% از فشارخون بالا درمردان و 20% درزنان را شامل می شد. حدود 9% مردان و 15/7 % زنان دچار هایپرکلسترولمی بودند. بیش از 4% مردان و حدود 5% زنان خطر 10% یا بیشتر داشتند.
نتیجه گیریزنان مراجعه کننده به مراکز بیشتر از مردان بودند و به نظر می رسد زنان پرخطر برای کنترل بیماری های خود به این مراکز مراجعه می کنند. با توجه به خطر بالاتر مردان برای ابتلا و مرگ ناشی از بیماری های قلبی-عروقی، تشویق و پی گیری مردان برای کنترل عوامل خطر پیشنهاد می گردد.

دیابت شایع ترین بیماری متابولیک در جهان محسوب می شود و مهار این بیماری بدون تغییر در سبک زندگی، موثر و کارآمد نیست. از این روی، این پژوهش باهدف بررسی تاثیر آموزش برپایه اصلاح سبک زندگی بر میزان قند خون بیماران دیابتی انجام شده است.
مواد و روش هااین پژوهش یک مداخله نیمه تجربی است که در سال 1396 بر روی بیماران دیابتی مراجعه کننده به انجمن دیابت ارومیه انجام شد. 74 بیمار به طور تصادفی در دو گروه مداخله و کنترل (37 نفر در هر گروه) قرار گرفتند. به منظور جمع آوری داده ها، از مشخصات دموگرافیک و چک لیست کنترل قند خون استفاده شد. مداخلات آموزشی طی هشت نشست یک و نیم ساعته و به مدت دو ماه برای گروه مداخله انجام گردید. جهت تجزیه وتحلیل داده ها؛ با استفاده از نرم افزار spss ویرایش 16 آزمون های توصیفی و آزمون تی مستقل و تی زوجی اجرا شد و اختلاف در سطح کمتر از 0/05 معنی دار تلقی گردید.
یافته هامیانگین سنی افراد 10/8±48/9سال بود و اکثریت افراد را زنان تشکیل می دادند (75/6 درصد). میانگین قند خون بیماران پیش از پژوهش 241 میلی گرم در دسی لیتر با انحراف معیار 54/6 بود. هم چنین، میانگین قند خون پیش از آموزش در هر دو گروه تفاوتی نداشت ولی پس از آموزش، میانگین قند خون به طور معنی داری در گروه مداخله کاهش یافت (0/01=p-value).
نتیجه گیریآموزش بر پایه اصلاح سبک زندگی می تواند به طور چشم گیری باعث کاهش میانگین قند خون افراد دیابتی گردد.

دیابت حاملگی (GDM) از شایع ترین اختلالات متابولیک است که برای اولین بار در دوران بارداری تشخیص داده می شود. عفونت پریودنتال نیز التهابی موضعی و مزمن است که می تواند بعنوان یک شروع کننده پاسخ التهاب سیستمیک در نظر گرفته شود و برخی معتقدند که با بروزGDM مرتبط است. با توجه به شواهد ناهمگون در این باره، هدف از مطالعه ی آینده نگر حاضر تعیین ارتباط بین بیماری پریودنتیت و GDM می باشد.
مواد و روش هااین مطالعه به صورت کوهورت در درمانگاه پره ناتال بیمارستان روحانی بابل، در شهریور 1398 لغایت شهریور 1399 انجام شد. وضعیت پریودنشیوم زنان باردار از نظر شاخص های خونریزی در اثر پروبینگ (BOP)، عمق پاکت پریودونتال (PPD)، میزان از دست رفتن چسبندگی (CAL) و شاخص لثه ای Gingival Index (Loe & Silness)، در هفته 12 تا 24 بارداری توسط یک دندان پزشک در همان درمانگاه بررسی و موارد بیماری تشخیص داده شد. سپس در هفته 24 تا 28 بارداری، بر اساس آزمایش تست تحمل گلوکز 75 گرمی، ابتلا به GDM مورد ارزیابی قرارگرفت.
یافته هابروز GDM در زنان مبتلا به پریودنتیت بیشتر بود (0/001<p). میزان خطر نسبی خام ابتلا به دیابت بارداری در زنان مبتلا به پریودنتیت با (0/001P< 95% CI) (21/77-6/54) 11/94 و خطر نسبی تعدیل شده با متغیرهای شاخص توده بدن، دور کمر، تعداد حاملگی و تعداد زایمان با (0/001P< 95% CI) (20/58-6/12) 11/23 بوده است. میانگین GI، PPD، BOP، CAL بطور معنی داری در زنان باردار مبتلا به GDM بیشتر از گروه سالم بود (0/05>P).
نتیجه گیریجهت پیشگیری از بروز دیابت بارداری سیاست تشخیص زودرس و درمان پریودنتیت باید در الویت مراقبت پره ناتال و حتی قبل از باردار شدن قرار گیرد.

با وجود شیوع بالاتر بیماری های کلیوی در جنس ماده، اثرات نیترات بر عملکرد کلیه عمدتا در جنس نر مطالعه شده است. هدف مطالعه حاضر تعیین اثرات تجویز طولانی مدت نیترات سدیم بر شاخص های عملکرد کلیه شامل میزان فیلتراسیون گلومرولی تخمینی (eGFR) و سطح سرمی اوره و کرآتینین در موش صحرایی ماده بالغ سالم می باشد.
مواد و روش هاموش های صحرایی ماده ویستار به 2 گروه (10 سر در هر گروه) تقسیم شدند: گروه شاهد که آب آشامیدنی و گروه نیترات که آب آشامیدنی حاوی 100 میلی گرم در لیتر نیترات سدیم به مدت 9 ماه دریافت کردند. وزن بدن، سطح سرمی اوره و کرآتینین در ابتدا و انتهای مطالعه اندازه گیری و eGFR محاسبه شد. هم چنین سطح متابولیت های اکسید نیتریک (NOx) در سرم و بافت کلیه موش های صحرایی ماده در انتهای مطالعه اندازه گیری شد.
یافته هادر انتهای مطالعه، وزن موش های صحرایی و هم چنین سطح سرمی اوره و کرآتینین در گروه شاهد به طور معنی دار بالاتر و سطح eGFR به طور معنی دار پایین تر از ابتدای مطالعه بود. نه ماه دریافت نیترات سدیم از طریق آب آشامیدنی، مانع از افزایش وزن و هم چنین افزایش سطح سرمی اوره و کرآتینین و کاهش سطح eGFR گردید. هم چنین گروه تیمار شده با نیترات سدیم دارای سطح بالاتر سرمی و کلیوی NOx در انتهای مطالعه بودند.
نتیجه گیریتجویز نیترات منجر به بهبود عملکرد کلیه در موش های صحرایی ماده بالغ سالم گردید. این اثرات مفید می تواند ناشی از افزایش سطح سرمی و بافتی NOx باشد.

مقدمه چاقی با تجمع بیش از حد بافت چربی سفید زیرجلدی و احشایی و هم چنین عدم تعادل بین انرژی دریافتی و مصرفی حاصل می شود. از آنجا که شیوع چاقی در سراسر جهان رو به افزایش است، روش های پیشگیری اهمیت ویژه ای یافته اند. در راهبردهای ضد چاقی جدید، علاوه بر محدود کردن دریافت انرژی و افزایش مصرف آن؛ هدررفت انرژی به شکل گرما یا ترموژنز مورد توجه می باشد. از آنجا که ترموژنز عمدتا در بافت چربی قهوه ای (BAT) رخ می دهد، شناسایی هرچه بیشتر بافت چربی قهوه ای فرصت را برای طراحی مداخله دارویی علیه چاقی، از طریق افزایش مصرف انرژی و گرمازایی، ایجاد می کند. شناخت مسیرهای سلولی مولکولی و عوامل تنظیمی، از جمله RNA های غیرکدکننده ی درگیر در فرایند ترموژنز در بافت چربی قهوه ای، راه کار کلیدی برای کاربردهای درمانی علیه چاقی و بیماری های مرتبط با چاقی محسوب می شود. RNA های غیرکدکننده از جمله miRNA ها و lncRNA ها تنظیم کننده های مهم برای فرایندهای زیست شناختی هستند. miRNA ها یک گروه از RNA های کوچک غیرکدکننده هستند که در تنظیم بیان ژن نقش کلیدی ایفا می کنند. مطالعات اخیر نشان داده اند که RNA های غیرکدکننده ی اختصاصی BAT در تنظیم عملکرد آدیپوژنز قهوه ای، قهوه ای شدن بافت چربی سفید (مثل آدیپوژنز بژ) و ترموژنز قهوه ای نقش کلیدی دارند. در این مطالعه به بررسی miRNA ها و lncRNA های مرتبط با ژن های درگیر در آدیپوژنز قهوه ای و ترموژنز پرداخته شده است. این دسته از miRNA و lncRNA ها ممکن است در آینده به عنوان نشان گرهای زیستی و اهداف درمانی برای پیشگیری و درمان چاقی و بیماری های مرتبط با چاقی مورد استفاده قرار گیرند.

کمبود ویتامین D به عنوان یکی از مسایل شایع سلامتی در جهان مطرح شده است. در ایران تخمین زده می شود که بیش از نیمی از افراد دچار کمبود ویتامین D باشند. شواهد موجود نشان دهنده این است که سطح کافی ویتامین D، نه تنها از طریق کنترل کلسیم و فسفر سبب سلامت عملکرد سیستم اسکلتی می شود؛ بلکه نقش مهمی را در تنظیم فعالیت ارگان های دیگر بازی می کند. متاسفانه عدم توافق و سوالات بدون پاسخ زیادی در خصوص چگونگی تشخیص، نحوه بررسی، روش درمان و حتی مناسب ترین سطح ویتامین D وجود دارد. در این مقاله مروری ما جدیدترین شواهد در خصوص نحوه بررسی و درمان کمبود ویتامین D را با تاکید بر مطالعات بر روی ایرانیان، مورد بررسی قرار داده ایم. هم چنین؛ در مورد نحوه رسیدن به سطح مناسب ویتامین D و چگونگی نگه داری آن بحث و بررسی نموده ایم. در نتیجه ی این بررسی مشخص شد که با توجه به شیوع بالای کمبود ویتامین D در ایران، لازم است در افراد در معرض خطر سطح سرمی ویتامین D اندازه گیری شود. در صورت وجود کمبود ویتامین D، شروع درمان با دوز بارگذاری و سپس ادامه آن با دوز نگه دارنده توصیه می شود. در مواردی که علی رغم مصرف ویتامین D سطح آن افزایش نیابد، بهتر است بررسی از نظر سندروم های سوءجذب انجام گیرد. هم چنین بر اساس این بررسی؛ توصیه می شود در جمعیت عمومی از کلسی تریول برای درمان استفاده نشود.

Recent studies show that exposure to heavy metals can affect thyroid function. This study aimed to assess heavy metals in breast milk during the first few days postpartum and their relationships with hyperthyrotropinemia in neonates.
Materials and MethodsIn this case-control study, 82 mother-infant pairs, consisting of 21 pairs (neonates with hyperthyrotropinemia) in the case group and 61 pairs (healthy neonates) in the control group, were assessed at 3-5 days postpartum. Thyrotropin concentrations >5 mIU/L in the heel blood sample were considered as hyperthyrotropinemia. In the case group, thyrotropin concentration was also measured in the venous blood sample for confirmation. At the same time, to assess mercury (Hg), lead (Pb), and cadmium (Cd) concentrations, a breast milk sample was collected from each mother.
ResultsThe median (interquartile range, IQR) thyrotropin concentrations in the heel blood sample of healthy neonates and neonates with hyperthyrotropinemia were 0.90 (0.40-1.85) and 6.80 (5.70-8.80) mIU/L, respectively (P<0.001). This value was 5.50 (5.00-6.75) mIU/L in the venous sample of neonates with hyperthyrotropinemia. None of the heavy metals in breast milk showed a significant correlation with thyrotropin concentration in the heel blood sample. However, there was a significant correlation between the concentration of Pb in breast milk and thyrotropin level in the venous blood sample (r=0.478, P=0.029). No correlations were observed between breast milk Hg and Cd concentrations and venous thyrotropin level.
ConclusionThe present findings indicate that the concentration of Pb in breast milk correlates with neonates' thyroid function. However, further studies with larger sample sizes are needed to confirm our results.

The World Health Organization Package of Essential Noncommunicable Disease Interventions (WHO-PEN) enables the prevention, early detection, and management of cardiovascular disease, diabetes, chronic respiratory disease, and cancer. Ira-PEN is the adapted WHO-PEN program for the Iranian population, which started in 2015 as a pilot program in four cities (Naghadeh, Maragheh, Shahreza, and Baft). Then, it was implemented throughout the country. We aimed to investigate the prevalence of cardiovascular risk factors in men and women referred to the health centers.
Materials and MethodsThis study used data for people over 30 years old in the pilot areas in 2016 and 2017. The 10-year cardiovascular risk was determined with SIB software, as an integrated health information system, using the WHO/ISH risk chart. Individuals were classified into four risk categories: low (<10%), medium (10-20%), relatively high (20-30%), and high (>30%).
ResultsAmong 160,223 subjects (58.1% female), the prevalence (95%CI) of diabetes in men and women was 8.5% (8.3-8.8) and 12.9% (12.7-13.1), respectively, and about one-third of diabetes cases were unknown. The prevalence of hypertension in men and women was 15.6 (15.3-15.9) and 23.2 (22.9-23.5), respectively. The unknown hypertension cases included 40% and 20% of men and women, respectively. Around 9% of men and 15.7% of women had hypercholesterolemia. More than 4% of men and around 5% of women had a risk of ≥10%.
ConclusionWomen are referred to the health centers more than men, and it seems that high-risk women refer to these centers for their diseases. Considering the high risk of morbidity and mortality due to cardiovascular diseases in men, we recommend encouraging and following men to control their risk factors.

Diabetes is the most common metabolic disease globally, and its control is not effective without changing the lifestyle. Therefore, this study aimed to investigate the effect of lifestyle modification education on the blood sugar of diabetic patients.
Materials and MethodsThis quasi-experimental interventional study was performed in 2017 among diabetic patients referred to the Urmia Diabetes Association. We randomly divided 74 patients into the intervention and control groups (37 patients in each group). Demographic characteristic forms and blood sugar control checklists were used to collect information. Educational interventions were performed in eight sessions of 1.5 hours during two months for the intervention group. Descriptive statistics, independent t-test, and paired t-test were used to analyze the data. SPSS software version 16 was used for data analysis, and the difference was considered significant at a level of less than 0.05.
ResultsThe mean age of the subjects was 48.9±10.8, and the majority (75.6%) were women. Before the study, the mean blood sugar level of patients was 241 mg/dl with a standard deviation of 54.6 mg/dl. The mean blood sugar level was not different between the groups before the intervention but, after the intervention, it was significantly lower in the intervention group (p-value=0.01).
ConclusionLifestyle modification education can significantly reduce the average blood sugar of people with diabetes.

Gestational diabetes mellitus (GDM) is one of the most common metabolic disorders firstly diagnosed in pregnancy. Periodontal infection is also a localized and chronic inflammation that can trigger the systemic inflammatory response, and some believe it is associated with GDM. There is insufficient evidence to support a link between periodontal disease and GDM. This prospective study aimed to determine the relationship between periodontal disease and GDM.
Materials and MethodsThis cohort study was performed in the Prenatal Clinic of Rouhani Hospital in Babol from September 2018 to September 2019. The periodontal status of pregnant women in terms of bleeding on probing (BOP), periodontal pocket depth (PPD), clinical attachment loss (CAL), and gingival index (Loe & Silness) at 12 to 24 weeks of gestation by one dentist was examined in the same clinic to diagnose periodontitis cases. Then, at 24 to 28 weeks of gestation, GDM was evaluated based on the 75 g glucose tolerance test.
ResultsThe incidence of GDM was higher in women with periodontitis (P<0.001). The raw relative risk of GDM was 11.94 (CI 95%: 6.21-54.77, P<0.001). The relative risk, adjusted for body mass index, waist circumference, number of pregnancies, and number of deliveries, was 11.23 (CI 95%: 6.20-12.58, P<0.001). The mean GI, CAL, BOP, and PPD were higher in pregnant women with GDM than in the healthy group (P<0.05).
ConclusionTo prevent GDM, the early detection and treatment of periodontitis should be a priority in prenatal care, even before pregnancy.

Despite the higher prevalence of kidney dysfunction in females, the effects of nitrate have mainly been studied in males. This study aimed to determine the effects of long-term sodium nitrate administration on kidney function in normal adult female rats by assessing estimated Glomerular Filtration Rate (eGFR), serum urea, and serum creatinine.
Materials and MethodsFemale Wistar rats were divided into two groups (n = 10 per group). The control group received tap water, and the nitrate group received tap water containing 100 mg/L sodium nitrate for nine months. Bodyweight, serum urea, and serum creatinine were measured at the beginning and end of the study, and eGFR was calculated. The nitric oxide metabolites (NOx) levels were also measured at the end of the study in serum and kidney tissue.
ResultsThe body weight, serum urea, and creatinine levels of rats in the control group were significantly higher, and eGFR was significantly lower at the end of the study compared to the beginning of the study. Nitrate administration for nine months prevented increases in body weight, serum urea, and creatinine and decreases in eGFR. Nitrate-treated female rats also had higher NOx levels in serum and kidney tissue.
ConclusionNitrate administration improved kidney function in female normal adult rats. These beneficial effects may be associated with increased serum and kidney levels of NOx.

Obesity is caused by excessive accumulation of subcutaneous and visceral white adipose tissue and an imbalance between energy intake and consumption. Because the prevalence of obesity is increasing worldwide, prevention methods have become particularly important. In new anti-obesity strategies, in addition to limiting energy intake and increasing its consumption, energy dissipation in the form of heat or thermogenesis is considered. Because thermogenesis occurs primarily in brown adipose tissue (BAT), identifying BAT provides an opportunity to design an anti-obesity drug intervention by increasing energy expenditure and thermogenesis. Understanding the molecular and cellular pathways and regulatory factors, including non-coding RNAs involved in the thermogenesis process in BAT, is a crucial strategy for therapeutic applications against obesity and obesity-related diseases. Non-coding RNAs, including miRNAs and lncRNAs, are essential regulators in biological processes. Besides, miRNAs are a group of small non-coding RNAs with vital regulatory roles in regulating gene expression. Recent studies have also shown that BAT-specific non-coding RNAs play a key role in regulating the function of brown adipogenesis, browning of white adipose tissue (such as beige adipogenesis), and brown thermogenesis. In this study, miRNAs and lncRNAs related to genes involved in brown adipose tissue and thermogenesis were investigated. These miRNAs and lncRNAs may be used in the future as biomarkers and therapeutic targets for the prevention and treatment of obesity and obesity-related diseases.

Vitamin D deficiency is a prevalent health problem all over the world. It is estimated that more than half of the Iranians are vitamin D deficient. Evidence has shown that adequate vitamin D levels are required for skeletal health through calcium and phosphorus homeostasis and play an essential regulatory role in extra-skeletal organs. However, there are several controversies and unanswered questions regarding diagnosis, evaluation, treatment, and even optimal serum levels of vitamin D. In this review study, we will discuss current evidence regarding the evaluation and treatment of vitamin D deficiency, emphasizing studies in the Iranian population. We also discuss how to achieve and maintain the optimum serum levels of vitamin D. This study concluded that according to the high prevalence of vitamin D deficiency in Iran, it is necessary to measure vitamin D serum levels in high-risk individuals. In the face of vitamin D deficiency, the initiation of treatment with a loading dose of vitamin D is recommended, followed by maintenance doses. Besides, malabsorption syndromes should be considered in people without increased serum levels despite the regular consumption of vitamin D. Moreover, calcitriol should not be used for the treatment of vitamin D deficiency in the general population.
