صفحه 1مقدمهدیابت نوع یک (T1DM)، یک بیماری خودایمنی است که در آن با واسطه سلول های T، سلول های بتای لوزالمعده بصورت گزینشی تخریب می شوند. نیمی از خطر ایجاد T1DM به وسیله منطقه ژنی HLA بوجود می آید. همچنین، نیمی از خطر بیماری T1DM توسط دیگر ژن های غیر از HLA و احتمالا آنهایی که در کمپلکس میان کنش با آنتی ژن دخیل می باشند، ایجاد شود. محصول ژن CD4 (یکی از مهمترین گیرنده های سطح سلول T که نقش کلیدی در ارائه آنتی ژن دارد) می تواند عاملی از این دست می باشد.روش هاما در این مطالعه با بهره گیری از رویکرد ژن کاندید، ارتباط احتمالی چندشکلی ژن CD4 را با T1DM در جمعیت ایرانی بررسی نموده ایم. چندشکلی ناشی از تکرار پنج نوکلئوتیدی غنی از بازهای پیریمیدینی که در پروموتر ژن مربوطه واقع شده است، مورد مطالعه قرار گرفت. تعداد 92 بیمار T1DM و 108 فرد شاهد سالم با استفاده از شیوه PCR بررسی شدند.یافته هاتجزیه و تحلیل نتایج بدست آمده، ارتباط حمایتی آلل CD4*A3 در مقابل بیماری Pc=0.025 RR=0.159) T1DM؛ (%95 CI:0.036-0.707 و مستعد کنندگی آلل CD4*A5برای بیماری Pc=0.010 RR=7.397) T1DM؛ (%95 CI:1.630-33.414 را نشان داد.نتیجه گیریاین نتایج حاکی از آن است که چند شکلی های خاص ژن CD4 دارای پیوستگی مثبت و منفی با بیماریT1DM در جمعیت ایران می باشد.

صفحه 11مقدمهنفروپاتی دیابتی هنوز هم یکی از شایع ترین علل نارسایی پیشرفته کلیوی محسوب می گردد. یکی از موانع موجود در پیشگیری موثرتر از این عارضه، ماهیت پیچیده و نقش سیالی است که زمینه ژنتیکی افراد دیابتی در زمینه هدایت و هماهنگیOrchestrating)) میان علل شناخته و ناشناخته این عوارض در قالب استعداد و یا مقاومت میزبان از خود نشان می دهند.
بررسی تاثیرات منفرد هر ژن با اولویت ژن هایی که دخالت آنها در ایجاد و یا پیشرفت فنوتیپ مورد مطالعه، توجیه بیولوژیک دارد، تنها رویکرد مطالعاتی است که برای غلبه بر پیچیدگی(Complexity) و تجزیه (Dissection) زیرساخت ژنتیکی بیماری های کمپلکس (مثل نفروپاتی دیابتی)، در دسترس محققین قرار دارد.روش هادر قالب یک «Association Study»، تاثیرات و قابلیت های فنوتیپیک پلی مورفیسم های ژن TGF-β1در کدون های 10 T/C)*869 +) و G/C)*915+) 25 در پیدایش و کنترل شدت نفروپاتی دیابتی در بیماران مبتلا به دیابت نوع یک ((T1DM مورد مطالعه قرار گرفت. میزان فراوانی آلل/ ژنوتیپ حاصله از پلی مورفیسم های ژن مذکور که توسط روش PCR – ARMS تایپ گردید، در دو گروه بیمار (248 نفر:86 DN+ و162 DN−) و کنترل سالم (113 نفر) که همگی «بریتانیایی- قفقازی» بوده اند، بررسی گردید.یافته هاتفاوت معناداری در توزیع فراوانی آلل های پلی مورفیک ژن TGF-β1 بین گروه ها و زیرگروه های افراد مورد مطالعه مشاهده نشد P=NS)).نتیجه گیریاین عدم تفاوت در توزیع آلل ها/ ژنوتیپ ها، می تواند تا حدودی «قابلیت عملکردی» (Functionality) (و البته پتانسیل پروگنوستیک) پلی مورفیسم های مورد مطالعه را در سطح ژنی به عنوان نشانگرهایی «قابل اعتماد»، زیر سوال ببرد. تاکید مجدد برعدم وجود رابطه قطعی و دایمی بین «ژنوتیپ- فنوتیپ» در بیماری های پیچیده، می تواند پیام مطالعه حاضر باشد. البته این نتیجه (عدم پیوستگی) می تواند نتیجه ای «کاذب» نیز فرض گردد و آن حالتی است که به علت «کشنده» بودن نفروپاتی دیابتی، بخشی از بیمارانی که حامل «ژنوتیپ های پرخطر» بوده اند، به علت مرگ از جمعیت مورد مطالعه، حذف قبلی شده اند.

صفحه 21مقدمهسطح استرس اکسیداتیو در بیماران دیابتی بالا است و اکسیداسیون LDL-C ورود و تجمع آن در ماکروفاژها را تسهیل می نماید که شاید از علل افزایش آترواسکلروز و بیماری های قلبی عروقی در این بیماران باشد. از طرف دیگر ترکیبات اکسید شده LDL-C خاصیت ایمونوژنیک داشته و باعث تولید آنتی بادی می شوند. البته اثر این آنتی بادی ها در مهار یا تحریک روند آترواسکلروز به خوبی معلوم نیست. مطالعه حاضر سطح LDL-C اکسید شده و آنتی بادی ضد آن را در بیماران دیابتی و افراد سالم مقایسه می نماید.روش هاجمعیت مورد مطالعه81 بیمار دیابتی نوع دو 40 تا 65 ساله غیر سیگاری بودند که تحت درمان با داروهای پایین آورنده چربی خون و فشار خون نبوده و سابقه بیماری قلبی- عروقی و کلیوی نداشتند. به عنوان گروه شاهد 69 فرد سالم انتخاب شده و از نظر جنس و نمایه توده بدن با گروه مورد هماهنگ شدند. پس از اخذ رضایت برای ورود به مطالعه، اطلاعات دموگرافیک و آنتروپومتریک افراد مورد مطالعه ثبت گردید. در نمونه خون ناشتا قند، کلسترول تام، HDL، تری گلیسرید، نیتروژن اوره، اسید اوریک، الکترولیت ها، هموگلوبین گلیکوزیله (HbA1c)، LDL اکسید شده و آنتی بادی ضد آن اندازه گیری شده و میزان LDL، نسبت LDL اکسید شده به خود LDL و نمایه توده بدن محاسبه گردید.یافته هادر دو گروه شاهد و بیمار، میزان LDL اکسید شده با LDL-C و کلسترول تام متناسب بود. سطح LDL اکسید شده و نسبت LDL اکسید شده به LDL در گروه شاهد بیش از بیماران دیابتی بود. در بیماران دیابتی آنتی بادی ضد LDL اکسیدشده با آن ارتباط معنی دار داشت. سطح LDL اکسید شده با سن و میزان کنترل قند خون ارتباط نداشت.نتیجه گیریسطح LDL اکسید شده در بیماران دیابتی کمتر از افراد سالم و متناسب با آنتی بادی ضد آن است. طبق مطالعات موجود به نظر می آید نوع و شدت اکسیداسیون در بیماران دیابتی به گونه ای است که برداشت آن توسط ماکروفاژ و سایر سلول ها را تسهیل و در نتیجه سطح خونی آن را کاهش می دهد.

صفحه 27مقدمهشیوع دیابت نوع 2 پس از زایمان در میان زنان با تاریخچه دیابت بارداری از 3% تا 65% گزارش شده است. هدف از این مطالعه تعیین عوامل پیشگویی کننده دیابت نوع 2 و اختلال تحمل گلوکز در دوران پس از زایمان در زنان مبتلا به دیابت بارداری بوده است.روش هااین مطالعه به صورت هم گروهی در 2416 زن باردار مراجعه کننده به بیمارستان های تابعه دانشگاه علوم پزشکی تهران انجام شد. جهت غربالگری، آزمون چالش گلوکز (GCT) 50 گرمی یک ساعته با معیار گلوکز بالاتر از 130 mg/dl بود. در موارد اختلال این آزمون، پیگیری با آزمون تحمل گلوکز (GTT) 100 گرم سه ساعته جهت تشخیص دیابت بارداری استفاده شد. 12 - 6 هفته پس از زایمان از بیماران مبتلا به دیابت بارداری، برای انجام آزمون تحمل گلوکز 75 گرمی دو ساعته دعوت به عمل آمد تا از نظر وجود اختلال تحمل گلوکز بررسی شوند. تشخیص دیابت و اختلال تحمل گلوکز بر اساس معیارهای انجمن دیابت آمریکا مطرح شد. از آزمون T دوطرفه و تحلیل واریانس برای مقایسه میانگین مقادیر بدست آمده در گروه های مورد بررسی استفاده شد؛ همچنین جهت مقایسه فراوانی هر یک از عوامل مورد بررسی در گروه های مورد مطالعه، از آزمون مجذور کای استفاده گردید. جهت تعیین عوامل پیشگویی کننده از آنالیز رگرسیون استفاده شد.P کمتر از 05/0 معنی دار تلقی شد.یافته هاافراد مبتلا به دیابت پس از زایمان در مقایسه با افراد سالم اختلاف معنی داری در عواملی مانند فراوانی افراد با سطح قند خون ناشتای بالاتر از 105 mg/dl در بارداری، نیاز به درمان با انسولین طی بارداری و شاخص توده بدن بالاتر از 27 Kg/m2 قبل از بارداری داشتند. با استفاده از مدل رگرسیون تنها نیاز به انسولین و شاخص توده بدن بالاتر از 27 Kg/m2 قبل از بارداری در افراد دیابتی، عوامل مستقل در پیشگویی دیابت نوع 2 بعد از بارداری بودند و در اختلال تحمل کربوهیدرات، سابقه سقط نیز علاوه بر عوامل ذکر شده نقش داشت که این رابطه بعد از تطابق سنی نیز برقرار بود.نتیجه گیریشاخص توده بدنی بالا، نیاز به انسولین طی بارداری، قند خون ناشتای بالا و سابقه سقط، مهمترین عوامل در پیشگویی ابتلا به دیابت و اختلال تحمل کربوهیدرات پس از زایمان هستند.

صفحه 37مقدمهانفارکتوس حاد میوکارد (AMI) مهم ترین علت مرگ و میر در جهان است. این مشکل با بعضی عوامل خطر تشدید می شود. یکی از این عوامل، هیپرگلیسمی ناشی از استرس است. این مطالعه با هدف تعیین فراوانی هیپرگلیسمی استرسی در انفارکتوس حاد میوکارد و رابطه آن با عوارض داخل بیمارستانی AMI انجام شده است.روش هادر 160 بیمار مبتلا به انفارکتوس حاد میوکارد (Q Wave MI) هیپرگلیسمی استرسی بررسی شد. سپس دو گروه نرموگلیسمیک و هیپرگلیسمی استرسی در طی مدت بستری از نظر ایجاد عوارض، نارسایی قلبی، شوک کاردیوژنیک و مرگ و میر پی گیری و مقایسه شدند.یافته هااز 160 بیمار مورد بررسی، 1/63 درصد نرموگلیسمیک، 5/7 درصد هیپرگلیسمی استرسی، 25 درصد به دیابت شناخته شده و 4/4 درصد به دیابت ناشناخته مبتلا بودند. از 18 بیمار هیپرگلیسمی استرسی، 6 بیمار (50 درصد) و از 101 بیمار نرموگلیسمیک، 18 بیمار (8/17 درصد) دچار نارسایی قلبی شدند که اختلاف معنی دار بود P=0.02)، (X2=6.27. شوک کاردیوژنیک در هر دو گروه مشاهده نشد. سه مورد مرگ و میر در گروه بدون دیابت شناخته شده در ساعات اولیه رخ داد (97/2 درصد). چون قند خون هنوز اندازه گیری نشده بود، احتساب این بیماران در گروه نرموگلیسمی یا هیپرگلیسمی استرسی امکان نداشت.نتیجه گیریهیپرگلیسمی استرسی می تواند میزان مرگ و میر، نارسایی قلبی و طول مدت بستری را در بیماران AMI افزایش دهد. در این مطالعه نارسایی قلبی به طور معنی داری با هیپرگلیسمی استرسی ارتباط داشت. تکمیل این بررسی به تعداد نمونه های بیشتر احتیاج دارد.

صفحه 43عوارض پا یکی از مهمترین و هزینه سازترین مشکلات بیماران دیابتی است. قسمت عمده این عوارض بعلت درگیری های عصبی-عروقی حادث می شوند.روش هامطالعه بر روی 142 بیمار دیابتی درمانگاه دیابت بیمارستان دکترشریعتی طی آذرماه 1382 تا شهریور 1383 صورت گرفت. پس از اتمام جمع آوری اطلاعات، با استفاده از پرسشنامه و انجام آزمون های عصبی و عروقی بر مبنای روش های استاندارد، اطلاعات خام بررسی و آنالیز شدند.یافته هابا توجه به حس نکردن مونوفیلامان در بیش از یک نقطه، 9/23 درصد بیماران دارای نوروپاتی بودند. 3/6 درصد برخورد سوزن با پوست را احساس نکردند و 4/13 درصد لرزش ناشی از دیاپازون 128هرتز را احساس ننمودند. در کل براساس مثبت شدن هریک از سه روش قبل، 1/26 درصد دارای نوروپاتی بودند. برطبق معیار میشیگان، 7/42 درصد دارای نوروپاتی بودند. با در نظر گرفتن مجموع علایم و نشانه ها، 61 درصد دارای نوروپاتی بودند. در بررسی ABI، 6/10 درصد دارای اختلال عروقی بوده و مجموعا در 8/8 درصد موارد حداقل یکی از نبض های محیطی در هر دو پا لمس نمی شد. 9/16 درصد موارد از لنگش متناوب شکایت داشتند.نتیجه گیریبه نظر می رسد استفاده از مونوفیلامان، بهترین روش بررسی نوروپاتی در پای بیماران دیابتی است. همچنین شکایت از لنگش متناوب برای تشخیص بیماری عروق محیطی بیماران دیابتی چندان قابل اعتماد نیست و استفاده از شاخص های عروقی برای این منظور توصیه می شود.

صفحه 53مقدمههدف از این مطالعه بررسی نقش لیپیدهای پلاسمایی شامل کلسترول تام (TC)، تری گلیسرید (TG)، LDL-C و HDL-C در برابر شاخص های لیپیدی از جمله TC/HDL-C و non-HDL-C به عنوان متغیرهای پیشگویی کننده رخدادهای قلبی عروقی در کوتاه مدت و در افراد بالای 30 سال بوده است.روش هادر یک مطالعه مورد - شاهد، طی 3 سال پیگیری تعداد 207 واقعه قلبی عروقی (CVD) در شرکت کنندگان مطالعه قند و لیپید تهران (TLGS) ثبت گردید. از این تعداد، 132 نفر که در ابتدای مطالعه هیچ گونه سابقه CVD نداشتند به عنوان گروه مورد و 264 نفر بعد از تعدیل برای سن و جنس به عنوان گروه شاهد انتخاب گردیدند. از تمامی افراد اطلاعات دموگرافیک، بالینی و آزمایشگاهی شامل فشار خون و شاخص های تن سنجی و نیز لیپیدهای سرم و قند ناشتا و 2 ساعته از بانک اطلاعاتی مرحله اول TLGS در دسترس بود. خطر نسبی (RR) مرتبط با هر یک از شاخص های لیپیدی در یک مدل رگرسیون لجستیکی مرحله ای و پس از تعدیل برای سابقه خانوادگی بیماری قلبی زودرس، مصرف سیگار، فشار خون سیستولیک و دیاستولیک، قند ناشتا و 2 ساعته و نسبت دور کمر به دور باسن، محاسبه گردید.یافته هاRR مرتبط با افزایش یک انحراف معیار (SD) در مقدار هر یک از لیپیدهای پیشگویی کننده مستقل CVD در آنالیز چند متغیری به این شرح بودند: RR=1.5[1.2-2.0]) TC و (SD=49 mg/dl، RR=1.5[1.1-2.0]) LDL-C و (SD=38 mg/dl، RR=1.6[1.2-2.1]) non-HDL-C و (SD=49 mg/dl، RR=1.5[1.1-2.0]) TC/HDL-C و (SD=1.8. مقایسه این چهار متغیر مستقل با استفاده از آنالیز منحنی ROC نشان داد که هیچ تفاوت معنی داری میان قدرت پیشگویی آنها برای رخدادهای قلبی عروقی وجود ندارد. در آنالیز چند متغیری ارتباط معنی داری میان TG، HDL-C و LDL/HDL-C و رخدادهای قلبی عروقی مشاهده نگردید.نتیجه گیریاین مطالعه نشان داد که در کوتاه مدت، TC، LDL-C، non-HDL-C و TC/HDL-C قدرت مشابهی در پیشگویی رخدادهای قلبی عروقی دارند. در این میان TC به علت هزینه های پایین تر می تواند انتخاب معقول تری جهت این پیشگویی باشد.

صفحه 65مقدمههموسیستیین یکی از عوامل خطرزای نسبتا جدید مطرح در بیماری های قلبی- عروقی است. می دانیم که اغلب عوامل خطرزای بیماری های قلبی- عروقی و دیابت مشابه می باشند. با توجه به شیوع بالای دیابت در کشور ما و مشخص نبودن تاثیر این عامل در بیماری دیابت، بر آن شدیم تا مطالعه ای در این زمینه انجام دهیم.روش هااین مطالعه در ساکنین 64-25 ساله منطقه 17 شهری تهران و طبق پروتکل MONICA سازمان جهانی بهداشت (WHO) انجام شد. هوموسیستیین بالاتر از 15 میکرومول در لیتر، غیرطبیعی در نظر گرفته شد. طبق معیار انجمن دیابت امریکا (ADA 2004)، گلوکز ناشتای سرم 100 تا 125 میلی گرم در دسی لیتر به عنوان عدم تحمل گلوکز مساوی یا بیشتر از 126 به عنوان دیابت در نظر گرفته شد.یافته ها5/54% کل جمعیت دچار افزایش هموسیستیین بودند. شیوع عدم تحمل گلوکز 5/3% و شیوع دیابت 1/10% بود. شیوع عدم تحمل گلوکز و دیابت در واجدین هموسیستیین طبیعی و غیر طبیعی تفاوت معنی داری نداشت (p=0.365). این تفاوت در هیچ یک از دو جنس نیز معنی دار نبوده است؛ ولی میانگین گلوکز در مبتلایان به اختلال هموسیستیین به طور معنی داری پایین تر بود (p=0.003).نتیجه گیریدر مجموع یافته ها حاکی از ارتباط معکوس سطح گلوکز خون با سطح هوموسیستیین سرم بوده است. از آنجایی که در مورد اثر هموسیستیین به عنوان یک عامل موثر در ابتلا به دیابت همچنان اختلاف نظر وجود دارد؛ نیاز به تحقیقات بیشتری جهت مشخص شدن نقش واقعی هموسیستیین در این بیماری وجود دارد.

صفحه 73مقدمههیپرهموسیستیینمی با اختلال در عملکرد سلول های اندوتلیال عروق ممکن است باعث افزایش فشار خون شود. هدف از این مطالعه تعیین رابطه بین سطح هموسیستیین پلاسما و فشار خون در بیماران دیابتی نوع 2 تازه تشخیص داده شده بود.روش هادر یک مطالعه مقطعی، 46 بیمار دیابتی نوع 2 تازه تشخیص داده شده به روش آسان انتخاب شدند. پس از معاینه بالینی، فشار خون تمامی افراد در دو روز متوالی و در دو نوبت به فاصله نیم ساعت اندازه گیری و میانگین فشار خون ها ثبت شد. نمونه خون جهت آزمایش گلوکز ناشتای پلاسما، HbA1c، هموسیستیین (Hcy)، کراتینین، کلسترول تام، HDL-Cو تری گلیسرید گرفته شد. سپس رابطه بین میانگین غلظت Hcy و فشار خون سیستولی و دیاستولی افراد با استفاده از ضریب همبستگی پیرسون تعیین گردید. بیماران براساس سطح هموسیستیین پلاسما به 3 گروه Hcy<10 μmol/l، 10≤Hcy<15 μmol/l و Hcy≥15 μmol/l تقسیم شدند. سپس میانگین فشار خون سیستولی و دیاستولی بین این گروه ها با استفاده از ANOVA یک طرفه و آزمون Tuckey– HSD مقایسه شد.یافته هامیانگین و انحراف معیار غلظت هموسیستیین 12.2(6.8) μmol/l و میانگین و انحراف معیار فشار خون سیستولی و دیاستولی به ترتیب (1/18)8/128 و (9) 3/82 میلی متر جیوه بود. بین سطح هموسیستیین پلاسما و فشار خون سیستولی P<0.01) و (r=0.39 و دیاستولی P<0.01) و (r=0.44 رابطه معنی دار وجود داشت. میانگین فشار خون سیستولی و دیاستولی بیماران با Hcy≥15 μmol/l به ترتیب (17.7)15.2و (28.5)91.8 میلیمتر جیوه بود که بالاتر از بیماران در دو گروه دیگر بود (P<0.001). بین سطح هموسیستیین پلاسما با FPG، HbA1c، و لیپیدهای سرم رابطه ای وجود نداشت.نتیجه گیریدر بیماران دیابتی نوع 2 تازه تشخیص داده شده، بین سطح هموسیستیین پلاسما و فشار خون سیستولی و دیاستولی رابطه معنی دار وجود دارد. بیماران با هیپرهموسیستینمی فشار خون بالاتری دارند.

صفحه 79دیابت بیماری شایعی بوده و می تواند بسیاری از ارگان های بدن را درگیر کند. یکی از عوارض نادر دیابت، پیومیوزیت می باشد که با درد عضلانی و تورم مشخص می شود. این بیماری در آب و هوای گرمسیری شایع تر است. تشخیص پیومیوزیت مشکل بوده و باید در هر بیمار دیابتی با شکایت پای دردناک به این بیماری فکر کرد. تشخیص افتراقی های آن شامل استئومیلیت، آرتریت سپتیک، هماتوم عضلانی، پارگی عضله و DVT می باشند. در صورت تاخیر در تشخیص، ممکن است سبب ایجاد سندرم کمپارتمان، سپسیس و مرگ شود. پیومیوزیت را می توان به سه مرحله بالینی تقسیم کرد. مرحله اول به صورت درد کرامپی عضلانی، تورم، تب خفیف، لکوسیتوز و سفتی در عضلات درگیر بوده و عضله مانند چوب سفت می شود. مرحله دوم 21-10 روز بعد شروع شده و با تب، تندرنس عضلانی و ادم مشخص می شود و معمولا چرک در عضلات درگیر ایجاد می شود. مرحله سوم با باکتریمی و ظاهر توکسیک مشخص شده ومی تواند منجر به عوارضی مانند شوک سپتیک، اندوکاریت، پنومونی و غیره شود. در این مقاله یک بیمار 53 ساله مبتلا به دیابت که با درد و تورم و سفتی در ناحیه عضلات باسن و ران مراجعه کرده است و برای وی تشخیص پیومیوزیت دیابتی مطرح شد، معرفی و سپس مروری بر مقالات موجود صورت می گیرد.

Page 1BackgroundType 1 diabetes mellitus (T1DM) is an autoimmune disease in which T-cell mediated selective pancreatic β- cell destruction occurs. Half the risk of T1DM development is given by the HLA gene region while the remaining risk is assigned to non-HLA genes, probably those engaged in the formation of antigen interaction complex. The CD4 gene product, which is among the most prominent T-cell surface receptors with a key role in antigen processing, could be regarded as a strong candidate.MethodsWe investigated the possible association of the CD4 gene polymorphism with T1DM using the candidate gene approach. The pyrimidine- rich pentanucleotide repeat polymorphism residing in the promoter region of the CD4 gene was studied. In the present study 92 Iranian T1DM patients and 108 healthy matched control individuals were screened by PCR technique.ResultsThe analysis of our results shows the protective association of CD4*A3 (RR= 0.159, 95% CI: 0.036-0.707; Pc=0.025) and the susceptible role of CD4*A5 (RR= 7.379, 95% CI: 1.630-33.414; Pc=0.010) with T1DM.ConclusionOur results suggest that the certain CD4 alleles are associated either negatively or positively with T1DM in the Iranian population.Keywords: Type 1 diabetes mellitus_Polymorphism_CD4 gene_Autoimmune disease_Iran

Page 11BackgroundDespite substantial progress in the clinical management of diabetes, diabetic nephropathy (DN) still occurs recurrently, implicating diabetes as the major underlying condition leading to the end stage renal disease. One of the main reasons is the influential role of genetic or inherited backgrounds of diabetics that are almost overlooked in daily practice. Owing to be orchestrated by the genetic makeup, cellular and molecular responses are different to similar metabolic disturbances. This in turn defines susceptibility/resistance state of the host to chronic diabetic complications, including DN. Separate analysis of every single gene that may be involved in genetics of a multi-factorial disease (such as DN) is the only available way to dissect the genetic basis of the disease and overcome its complexity. Among different genes accountable for DN, Transforming Growth Factor (TGF)-1 has an exceptional place. TGF-1 has profound impact on cell growth and proliferation, and in particular the regulation of extra cellular matrix deposition, branding it as a "pro-fibrotic" and "hypertrophic" mediator.MethodsBy employing ARMS-PCR technique, the genetic susceptibility to DN was studied in 248 patients with T1DM (86 DN+, 162 DN−) and 113 healthy controls, all from British Caucasian origin. The analysis of two functional TGF-1 gene variations, which change codons 10 (+869*C/T) and 25 (+915*G/C) was carried out.ResultsThere were some differences in alleles/genotypes distribution, but no significant association was apparent in patients as a whole or DN+/DN− subgroups and controls (P=NS).ConclusionThe negative result of this study may be false. As DN is a mortal disability, some fraction of risky genotypes associated with DN may previously be excluded by death. Such under-representation of the risky-genotypes (selective survivor effect) can be avoided by carrying out a prospective study. However, if the non-association result is true, it may question the functionality and reliability of the examined polymorphisms at least in the context of diabetes. Page 21BackgroundOxidized low-density lipoprotein (Ox-LDL), a key factor in the development of atherosclerosis, can cause endothelial dysfunction and augment lipid accumulation within the arterial wall. Increased oxidative stress in diabetes contributes to this process. Ox-LDL is a highly immunogenic molecule and it is not clear whether anti oxidized LDL antibodies (OLAB) are pathogenic or protective in atherosclerosis? The aim of this study was to evaluate Ox-LDL and its antibody in type 2 diabetes and healthy subjects.MethodsAs a case-control study we evaluated 81 type 2 diabetic patients and 69 non-diabetic healthy persons aged 40 to 65 years. Controls were sex and BMI matched with diabetic patients. Patients with history of cigarette smoking, antioxidant or antihyperlipidemic drugs consumption, coronary heart disease, hypertension, and renal impairment were excluded. We measured serum level of Ox-LDL(two monoclonal antibody of Mercodia co.) and OLAB by ELISA. Lipid profile, serum electrolytes, and HbA1c (HPLC) were also determined. Ox-LDL and its antibody were compared between diabetic patients and controls and the correlation with lipid profile, HbA1c and BMI were assessed.ResultsSerum Ox-LDL concentration and Ox-LDL to LDL ratio were distinctively higher in controls (15.7+-6.9 vs. 11.8+-5.6, P < 0.005). Ox-LDL concentrations were correlated with LDL-C (rs=0.36, P<0.0005) and total cholesterol (rs=0.31, P<0.0005) in both groups but not with age and HbA1c. In diabetic patients, Ox-LDL and its antibody were positively correlated (rs=0.26, P<0.05). Obese diabetic patients (BMI > 30) had higher Ox-LDL concentrations in comparison with diabetic patients with BMI less than 30.ConclusionIn diabetic patients Ox-LDL level is lower than non-diabetics and is correlated with its antibodies. Based on previous findings, we suppose that the pattern of LDL oxidation enhances Ox-LDL recognition by macrophage via specific legends. Page 27BackgroundWomen with gestational diabetes mellitus (GDM) have a reported 3% to 65% risk of developing type 2 diabetes. This study aims to identify the factors in GDM patients which can predict the risk of postpartum impaired glucose tolerance (T2DM+IGT) and T2DM.MethodsA cohort study was conducted on 2416 pregnant women referred to five university hospital clinics. The universal screening was performed with a GCT- 50g and those with plasma glucose level 130mg/dl, were diagnosed as GDM if they had an impaired GTT-100g based on Carpenter and Coustan criteria. All pregnancies were followed until delivery. Available GDM patients underwent an OGTT-75gr within 6 to 12 weeks after delivery. Postpartum diabetes mellitus was diagnosed according to ADA criteria.ResultsSuch factors as FBS105 during pregnancy, insulin requirement during pregnancy and BMI≥27kg/m2 before pregnancy were more prevalent in postpartum diabetic patients as compared with normal controls, significantly. Multivariate analysis suggested that gestational requirement for insulin and BMI≥27kg/m2 were the best predictors for developing postpartum diabetes. Also history of abortion, gestational requirement for insulin and BMI≥27kg/m2 were the best predictors for postpartum IGT.ConclusionWomen with GDM have a substantially increased risk of developing postpartum IGT or diabetes. High glucose levels, insulin requirement during pregnancy, history of abortion and BMI≥27kg/m2 are the best predictors for postpartum diabetes and IGT.Keywords: GDM, T2DM, Predictive factors

Page 37BackgroundMyocardial infarction is the most important cause of mortality all over the world. Complications may increase with some risk factors including stress hyperglycemia. The aim of this study was to evaluate the correlation and importance of hyperglycemia in acute intrahospital complications after AMI.MethodsPrevalence of stress hyperglycemia among 160 patients with acute myocardial infarction (Q wave MI) was determined. These patients were divided into normoglycemic and stress hyperglycemic groups. These were compared according to complications such as heart failure, cardiogenic shock and mortality.Results61% were normoglycemic and 7.5% had stress hyperglycemia. 25% had history of diabetes and 4.4% were not aware of diabetes. Among 18 patients with stress hyperglycemia, 6 patient (50%) and among 101 normoglycemic patients 18 (17.8%) were affiliated with cardiac failure that the difference were significant (P=0.02 and χ 2= 6.25). There were no cases of cardiogenic shock in both groups. Three patients died during first hours of admission (2.97%). They hadnt have history of diabetes. We could not assign them to normoglycemic or stress hyperglycemic groups since blood sampling had not been done yet.ConclusionStress hyperglycemia can increase complications of AMI, significantly stress hyperglycemia caused increased incidence of heart failure in this study.Keywords: Stress hyperglycemia, Acute myocardial infarction, complications

Page 43BackgroundFoot complications are among the important problems of diabetic patients. Vascular and neurological involvements are two major causes for such complications.MethodsWe studied 142 diabetic outpatients referred to diabetes clinic of Dr.Shariati Hospital from Dec. 2003 to Sep. 2004. We performed different neurological and vascular tests to assess the diabetic foot and data were analyzed by SPSS software.Results54.9% of the cases reached the symptom score of 5 and upper so they had neuropathy regarding the Michigan Neuropathy Disability Score (MNDS). By analyzing the overall symptom and sign scores, 61% had neuropathy. Regarding MNDS, 42.7% of the subjects were neuropathic. Finally 23.9% were unable to sense at least one point from 12 points of monofilament examination. 16.9% of the patients had the complaint of intermittent claudication, in 8.8% at least one of the peripheral pulses was not palpable, whereas 10.6% had some degree of PAD regarding to ABI and toe pressure.ConclusionUsing a monofilament is the most reliable method for screening of the neuropathy in diabetic patients. On the other hand, relying on symptoms like intermittent claudication and physical examination in order to find peripheral arterial disease in diabetic patients may lead to miss many cases, thus, performing some more precise diagnostic tests, such as ABI and toe pressure are highly recommended and reliable.Keywords: Diabetes Mellitus, Neuropathy, Diabetic foot, Monofilament, Michigan Neuropathy Disability Score, ABI

Page 53BackgroundTo evaluate the role of lipid markers including total cholesterol (TC), LDL-C and HDL-C vs. lipid indices (TC/HDL-C, LDL-C/HDL-C and non-HDL-C) as short term predictors of cardiovascular outcomes in adults over 30 years.MethodsAs a nested case and control study, there were 207 CVD events among participants of Tehran Lipid and Glucose Study (TLGS) documented during 3 years of follow-up. Those cases that were free of CVD at baseline (132 subjects) were matched to 264 controls for age and sex. In all subjects, demographic and clinical data including blood pressure and anthropometric measurements as well as serum lipids, fasting and 2-hour glucose were available from the database of the TLGS. We estimated the relative risk (RR) for each lipid parameter in a multiple stepwise regression model after adjustment for family history of premature CHD, smoking, systolic and diastolic blood pressure, fasting and 2-hour plasma glucose and waist-to-hip ratio.ResultsThe RRs associated with an increase of ≈1 SD of independent lipid predictors in the multivariate model were as follow: total cholesterol (RR=1.6 [1.2-2.0], SD= 1.3 mmol/L), LDL-C (RR=1.5 [1.1-2.0], SD= 1 mmol/L), non-HDL-C (RR=1.6 [1.2-2.1], SD= 1.2 mmol/L) and cholesterol/HDL-C (RR= 1.5 [1.1-2.0], SD= 1.8). The comparing of these four independent variables with ROC curve analysis showed that there was no significant difference in their predictive power for cardiovascular outcome. There was no association between HDL-C, triglyceride and LDL-C/HDL-C and CVD outcome in multivariate analysis.ConclusionThis study showed that TC, LDL-C, non-HDL-C and TC/HDL-C have similar predictive values for short term prediction of CVD outcome. Page 65BackgroundHomocysteine is one of the novel cardiovascular risk factors. We know most of the cardiovascular and diabetes risk factors are accompanied. The aim of this study was to determine the role of homocysteine in diabetes.MethodsThe study was performed in 25-64 year old individuals of the 17th district of Tehran according to WHO MONICA project. Homocysteine levels higher than 15 µmol/l was considered as abnormal. According to ADA 2004 criteria, fasting glucose levels of 100 to 125 mg/dl were considered as impaired glucose tolerance (IGT), while levels higher than 125 mg/dl included diabetes.ResultsPrevalence of hyperhomocysteinemia was 54.5% in the whole population. Prevalence of IGT and diabetes were 3.5% and 10.1%, respectively. There was no difference between prevalence of IGT and diabetes in both hyperhomocysteinemic and normal group (P= 0.365). The difference was also not significant in each genders, but mean glucose levels were significantly lower in people with hyperhomocysteinemia (P= 0.003).ConclusionThis study emphasizes that an inverse relation between glucose and homocysteine levels. Some studies indicate an opposite result and there is controversy on it. More researches may determine the exact role of homocysteine in diabetes.Keywords: Homocysteine, IGT, Glucose, Diabetes

Page 73BackgroundHyperhomocysteinemia may increase blood pressure by inducing endothelial cells dysfunction. The aim of this study was determining the relationship between plasma Homocysteine (Hcy) levels and blood pressure in new cases of type 2 diabetes mellitus.MethodsAs a cross sectional study, 46 new cases of type 2 diabetes mellitus referred to Isfahan Endocrine and Metabolism Research Center were selected by convenient sampling. Basal characteristics of patients were recorded. Systolic and diastolic blood pressure was measured two times with 30 minutes interval on two consequent days. After ten hours of overnight fasting, blood samples were taken to measure fasting plasma glucose, HbA1c, triglyceride, Total cholesterol, HDL-C and creatinine. All data were expressed as mean (SD). Relationship between plasma Homocysteine levels and both systolic and diastolic blood pressure was assessed by Pearson's correlation coefficient. Patients were divided into three groups according to their plasma Homocysteine levels, (Hcy<10mol/L, 10Hcy<15 mol/L and Hcy15mol/L).. Then mean of systolic and diastolic blood pressure between groups were compared by one – way ANOVA and Tuckey-HSD tests.ResultsMean (SD) of plasma Homocysteine levels was 12.2 (6.8) mol/L and of systolic and diastolic blood pressure was 128.8 (18.1) and 82.3(9) mm/Hg, respectively. There was a significant correlation between plasma Homocysteine levels, and both systolic (r=0.39, P<0.01) and diastolic (r=0.46, P<0.01) blood pressures. Mean (SD) of systolic and diastolic blood pressure in patients with plasma Homocysteine level of 15mol/L was 152.5 (17.7) and 91.8 (7.3) mm/Hg, respectively, so blood pressure in those group with hyperhomocysteinemia was higher than other two groups (P<0.001). There was no correlation between Homocysteine levels and FPG, HbA1c and lipid profile.ConclusionThere is a significant correlation between plasma Homocysteine levels and both systolic and diastolic blood pressure in newly diagnosed type 2 diabetics. Page 79Pyomyositis is a pyogenic infection of the skeletal muscle characterized by muscle pain and swelling. The illness is more commonly seen in the tropics. The diagnosis of pyomyositis is difficult due to its vague clinical presentation and poor localizing signs. The differential diagnosis includes osteomyelitis, septic arthritis, muscle hematoma, muscle rupture and deep vein thrombosis. A delay in diagnosis may result in compartment syndrome, sepsis and death. Pyomyositis can be divided into three clinical stages: Stage 1 is characterized by crampy local muscle pain, swelling, low-grade fever and leukocytosis. The muscle may have a "woody" texture on palpation. Stage 2 is characterized by fever, muscle tenderness, and edema. Aspiration of the affected muscle typically yields pus. Stage 3 is characterized by bacteremia and a toxic appearance. Complications such as septic shock, endocarditis, pneumonia and some others may be present. Definite diagnosis should be made by aspiration or surgical drainage of the abscess. We report a 53 years old patient with diabetes who presented with a swelling and painful buttock and thigh, that was diagnosed with pyomyositis.Keywords: Pyomyositis, Abcess, Diabetes
