مطالعه ی حاضر با هدف بررسی تاثیر تزریق زیرجلدی متوکلوپرامید، پتیدین و کتامین بر درد بعد از اعمال جراحی اصلاح فتق اینگوینال تحت بیهوشی عمومی و مقایسه ی آن با گروه شاهد انجام شد.
روش هادر این مطالعه ی کارآزمایی بالینی، تعداد 104 نفر از بیماران انتخاب و به طور تصادفی به چهار گروه 26 نفری تقسیم شدند. بیماران گروه 1 معادل 5/0 میلی گرم/کیلوگرم کتامین، بیماران گروه 2 معادل 75/0 میلی گرم/کیلوگرم پتیدین، بیماران گروه 3 معادل 10/0 میلی گرم/کیلوگرم متوکلوپرامید و بیماران گروه شاهد، نرمال سالین به صورت زیرجلدی در محل برش جراحی دریافت کردند. بیماران در 24 ساعت اول بعد از ورود به ریکاوری، از نظر شدت درد بر اساس معیار دیداری سنجش درد (Visual analog scale یا VAS)، متغیرهای همودینامیک، مدت زمان خروج لوله ی تراشه و ریکاوری، عوارض دارویی، مجموع مسکن مصرفی اضافی، زمان اولین نیاز به مسکن اضافی و رضایتمندی مورد بررسی قرار گرفتند.
یافته هامیانگین VAS در بیشتر زمان های بعد از ورود به ریکاوری (050/0 > P) و میانگین مجموع مسکن مصرفی اضافی (007/0 = P) در دو گروه کتامین و پتیدین به طور معنی داری کمتر از دو گروه متوکلوپرامید و شاهد بود. میزان رضایت از بی دردی بعد از عمل در گروه پتیدین به طور معنی داری بیشتر از سایر گروه ها بود (001/0 > P).
نتیجه گیریپتیدین و کتامین، اثر برابری در کاهش شدت درد بعد از عمل دارند، اما رضایتمندی بیماران از پتیدین بیشتر بود. متوکلوپرامید در مقایسه با دو داروی دیگر اثر کمتر و گاهی برابر با دارونما در کنترل درد پس از عمل داشته و همراه با عوارض بیشتری بوده است.

ویروس آنفلوانزا H1N1 به عنوان عامل بیماری زا و تهدید کننده ی حیات انسان مطرح می شود. واکسیناسیون، از راه های موثر برای پیش گیری و مهار بیماری آنفلوانزا است. تولید پروتئین نوترکیب هماگلوتینین در سیستم بیانی باکولوویروس، در بستر سلول یوکاریوت حشره (Sf9) به عنوان یک راهکار موثر پیشنهاد شده است.
روش هاتوالی ژن هماگلوتینین ویروس آنفلوانزا H1N1 از National Center for Biotechnology Information (NCBI) استنتاج و پس از طراحی پرایمر اختصاصی، توالی با استفاده از هضم آنزیمی در پلاسمید pFastBacHTA کلون گردید و برای تولید یک بکمید نوترکیب به سلول DH10Bac انتقال داده شد. پس از استخراج پلاسمید مربوط و تایید صحت کار، به داخل سلول حشره ترانسفکت گردید و بعد از بیان پروتئین توسط سلول Sf9، با روش Sodium dodecyl sulfate-Polyacrylamide gel electrophoresis (SDS-PAGE) و Western blot، حضور پروتئین نوترکیب تایید شد.
یافته هاژن هماگلوتینین با طول 654 جفت باز در پلاسمید pFastBacHTA به کمک دو آنزیم BamHI و Xhol ساب کلون گردید. سلول حشره بعد از دریافت بکمید نوترکیب، پروتئینی به وزن تقریبی 60 کیلودالتون را بیان نمود. پس از استخراج پروتئین، با روش های SDS-PAGE و Western blot تایید انجام گرفت و با روش Lowry، غلظت پروتئین اندازه گیری شد.
نتیجه گیریسیستم بیانی باکولوویروس برای تولید پروتئین هایی با ساختار پیچیده مفید است. به طور کلی، از این طرح می توان به این نتیجه رسید که این پروتئین در سلول حشره به میزان بالایی بیان می شود و به دلیل تشابه سیستم آن با سیستم انسانی، می تواند در آینده به عنوان جایگزین مناسب برای تخم مرغ های جنین دار در زمینه ی واکسیناسیون مورد استفاده قرار گیرد.

عوارض زیادی نظیر عوارض چشمی، به دنبال مصرف ایزوترتینوین سیستمیک که برای درمان آکنه ی شدید استفاده می شود، گزارش شده است. در این مطالعه، یک مورد کنژکتیویت، هوردیولوم و شالازیون دوطرفه که در ارتباط با مصرف ایزوترتینوین خوراکی رخ داده است، گزارش می گردد.
گزارش موردبیمار خانم 20 ساله ای بود که به دلیل آکنه ی شدید التهابی، تحت درمان با ایزوترتینوین خوراکی قرار گرفته و حدود 5/4 ماه پس از شروع درمان دچار کنژکتیویت، هوردیولوم و شالازیون دوطرفه شده بود. دو هفته پس از قطع درمان، علایم چشمی بیمار بهبود یافت، اما بار دیگر، پس از شروع ایزوترتینوین خوارکی (این نوبت با دز کمتر) کنژکتیویت بیمار عود کرد. به همین دلیل، ایزوترتینوین بیمار قطع شد.
نتیجه گیریاز آن جایی که وضعیت چشم بیمار دوطرفه بود، پس از قطع دارو، بیمار بهبود یافت و پس از شروع مجدد، بار دیگر بیماری عود نمود. به احتمال قوی، عوارض چشمی بیمار ثانویه به مصرف ایزوترتینوین است و یک یافته ی تصادفی نمی باشد.

The aim of this study was to investigate the effect of subcutaneous injection of metoclopramide, pethidine, and ketamine on postoperative pain after inguinal hernia repair under general anesthesia compared to control group.
MethodsIn this clinical trial study, 104 patients were selected and randomly divided into four groups of 26 patients. The patients received subcutaneous injection of each drug at the incision site. In the first group, 0.5 mg/kg ketamine, in the second group, 0.75 mg/kg pethidine, in the third group, 0.1 mg/kg metoclopramide, and in the control group, normal saline were administered. We evaluated the pain intensity [by visual analog scale (VAS)], hemodynamic parameters, extubation and recovery time, drug complications, total rescue analgesic consumption, time to first analgesic request, and satisfaction score for the initial 24 hours after entrance to recovery.
FindingsThe mean of VAS at the most of measuring times (P < 0.050) and the mean of total rescue analgesic consumption (P = 0.007) were significantly lower in the ketamine and pethidine groups compared to metoclopramide and control groups. The satisfaction score for postoperative analgesia in pethidine group was significantly higher than the other groups (P < 0.001).
ConclusionPethidine and ketamine have equal effects on postoperative pain; but the patients were more satisfied with pethidine. The effect of metoclopramide on the postoperative pain was lower than the other two drugs; moreover, its effect was equal to the placebo and was associated with more adverse reactions.

The H1N1 influenza virus is a highly pathogenic virus that threatens human life. Vaccination is an effective way of preventing and controlling influenza. Production of recombinant hemagglutinin in the baculovirus expression system, in the insect eukaryotic cell substrate (Sf9), has been suggested as an effective strategy.
MethodsThe H1N1 influenza virus hemagglutinin gene sequence was prepared from National Center for Biotechnology Information (NCBI). After designing a specific primer, the sequence was provided using restriction digestion, cloned into pFastBacHTA plasmid, and transferred to the DH10Bac cell to produce a recombinant bacmid. After extracting the relevant plasmid, it was transfused into the insect cell; and after the expression of the protein by Sf9 cell, the presence of recombinant protein was confirmed using sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) and Western blot methods.
FindingsThe hemagglutinin gene (654 bp) was cloned in pFastBacHTA plasmid using the two enzymes of BamHI and Xhol. Sf9 cell expressed a protein weighing approximately 60 kDa after receiving the recombinant bacmid protein. The extracted protein was identified and confirmed using SDS-PAGE and Western blot methods; and protein concentration was measured by Lowry method.
ConclusionThe baculovirus system is useful for the production of proteins with complex structures. Generally, it can be concluded that this protein is highly expressed in insect cells. Due to the similarity of this system with the human system, it can be a suitable alternative for embryonic eggs in the future, and can be used in vaccination.

Several adverse effects (AEs) such as ocular AEs have been reported following systemic consumption of isotretinoin. Here, we report a case of bilateral chalazion, conjunctivitis, and hordeolum associated with systemic isotretinoin.
Case ReportA 20-year-old girl was treated with oral isotretinoin because of severe inflammatory acne, and developed bilateral chalazion, conjunctivitis, and hordeolum about 4.5 months after initiation of treatment. Two weeks after cessation of oral isotretinoin, patient’s eye condition improved. However, after re-initiation of oral isotretinoin treatment with a reduced dose, her conjunctivitis relapsed; therefore, the treatment was once again terminated.
ConclusionSince the patient’s condition was bilateral, recovered after cessation of therapy; and once again, relapsed after re-initiation. It was most likely a complication of the medication and not a coincidence.
