مقایسه ی همزمان کیفیت تصویر و ریسک سرطان زایی ناشی از تابش در آزمون سی تی اسکن مغز در اسکنرهای مختلفصفحات 1170-1176مقدمه
پارامترهای کیفیت تصویر به دوزتابشی و نوع اسکنر سی تی اسکن وابسته می باشد بنابراین، هدف از انجام این مطالعه، مقایسه ی همزمان کیفیت تصویر و ریسک سرطان زایی ناشی از تابش در آزمون سی تی اسکن مغز در اسکنرهای مختلف بود.
روش هامطالعه ی توصیفی- تحلیلی از نوع مقطعی حاضر بر روی 200 نفر از بیماران که جهت انجام سی تی اسکن مغز به بیمارستان های کاشانی (دستگاه های زیمنس (اسکنر 1) و کانن (اسکنر 2))، الزهرا(س) (جنرال الکتریک (اسکنر 3))، عیسی بن مریم (نیوسافت (اسکنر 4)) شهر اصفهان مراجعه کرده بودند، انجام شد. دوز ارگان ها و دوز موثر بیماران با برنامه محاسباتی Impact dose و ریسک سرطان زایی ناشی از تابش (Risk of exposure-induced death) REID توسط برنامه ی PCXMC به دست آمد. کیفیت تصاویر توسط پارامترهای نویز، نسبت سیگنال به نویز SNR)) و نسبت کنتراست به نویز (CNR) بررسی شد. برای مقایسه ی اسکنرهای مختلف نسبت CNR به REID به عنوان معیار C معرفی شد.
یافته هامیانگین مقادیر REID برای اسکنر 1 تا 4 برای مردان به ترتیب 2/98، 4/46، 5/07 و 4/65 و برای زنان 3/22 ،6/16، 6/53 و 6/26 در هر 100000 نفر به دست آمد. میانگین مقادیر CNR برای اسکنر 1 تا 4 به ترتیب 1/24، 1/05، 1/66 و 2/08 به دست آمد. اسکنر 1 و 2 به ترتیب دارای بالاترین و کمترین معیار C بودند.
نتیجه گیریدوز تابشی و ریسک ناشی از تابش در آزمون سی تی اسکن مغز را باید همواره با در نظر گرفتن پارامترهای کیفیت تصویر مورد ارزیابی قرار داد و اسکنری مناسب تر می باشد که دارای نسبت CNR به REID بالاتری باشد.

انواژیناسیون، یکی از علل شایع شکم حاد در کودکان است. مراکز جراحی اطفال معمولا از انمای باریم برای درمان انواژیناسیون استفاده می کنند. هدف از مطالعه ی حاضر، تعیین ارتباط بین زمان سپری شده از شروع علایم و اندازه ی انواژیناسیون با موفقیت جااندازی با انمای نرمال سالین تحت گایدسونوگرافی در اطفال بود.
روش هاجامعه ی آماری شامل تمامی بیمارانی بود که از ابتدای مهرماه 1398 تا 31 شهریورماه 1399 با تشخیص انواژیناسیون جهت درمان به بیمارستان فاطمی اردبیل مراجعه کرده بودند. 144 بیمار به صورت تمام شماری از سیستم HIS بیمارستان استخراج شدند. برای آنالیز داده ها از آزمون های T-test، Square-Chi و ANOVA و جهت آنالیزهای چند متغیره از آزمون رگرسیون لجستیک استفاده شد. مقادیر P کمتر از 0/05 معنی دار در نظر گرفته شد.
یافته هامیانگین و انحراف معیار سن شرکت کنندگان در مطالعه 7/8 ± 16/8ماه بود. 116 (80/6 درصد) بیمار از 144 بیمار، انمای موفق داشتند که در 4 بیمار (3/4 درصد) علایم برگشت بیماری دیده شد. میزان موفقیت انمای نرمال سالین با سن و جنس و نیز علایم اولیه بیماری ارتباط معنی داری نداشت ولی با مدت زمان سپری شده از شروع علایم و اندازه ی انواژیناسیون ارتباط معنی دار بود.
نتیجه گیریتوجه به اندازه ی انواژیناسیون و تشخیص سریع و صحیح کودکان مبتلا به آن و انمای نرمال سالین به عنوان یک روش درمان موثر در کنار باریم انما برای درمان انواژیناسیون دراطفال می تواند مفید باشد.

آموزش به بیمار و افزایش توان خودمراقبتی در بیمار قلبی اهمیت دارند. هدف از این مطالعه، مقایسه ی تاثیر آموزش خودمراقبتی توسط پرستار و همتا بر توان خودمراقبتی بیماران کاندید آنژیوگرافی بود.
روش هاطرح نیمه تجربی با پیش آزمون- پس آزمون در دو گروه بیمار کاندید آنژیوگرافی در بیمارستان تخصصی قلب شهید مدنی خرم آباد در سال 1401 انجام شد. نمونه گیری در دسترس و حجم نمونه 56 نفر بود. مداخله شامل آموزش خودمراقبتی به بیمار طی دو جلسه ی 30 دقیقه ای قبل و بعد از آنژیوگرافی توسط پرستار یا همتا بود. ابزار پژوهش، پرسش نامه ی توان خودمراقبتی کرنی و فلسچر (1979) بود. داده ها با آزمون Independent sample T-test و Paired T-test، Chi-Square، Fisher's exact test در سطح معنی داری کمتر از 0/05 تحلیل شدند.
یافته هانمونه ها شامل 28 زن (50 درصد) و 28 مرد (50 درصد)، اکثریت متاهل (96/4 درصد)، با تحصیلات زیر دیپلم (75 درصد)، خانه دار و بیکار (48/2 درصد) و با وضعیت درآمد متوسط (42/9 درصد) بودند. میانگین و انحراف معیار توان خودمراقبتی در دو گروه قبل از مداخله 12/56 ± 138/00 و 11/08 ± 133/11 (0/128 = P) و بعد از مداخله 36/26 ± 141/11 و 15/39 ± 139/11 (0/001 = P) بود. دو گروه از نظر اثربخشی آموزش، یکسان بودند (0/216 = P).
نتیجه گیریآموزش توسط پرستار یا همتا، توان خودمراقبتی بیماران کاندید آنژیوگرافی را افزایش می دهد. اثربخشی آموزش خودمراقبتی توسط پرستار یا همتا بر توان خودمراقبتی یکسان است. لذا بنا بر حجم کار و تعداد پرستاران، نسبت پرستار به بیمار و وقت لازم برای مراقبت می توان در کنار آموزش خودمراقبتی پرستار به بیمار، از همتا به عنوان آموزش دهنده استفاده نمود.

The parameters of image quality depend on the radiation dose and the CT scanner type. This study aimed to simultaneously compare the image quality and the risk of carcinogenesis caused by radiation in the CT scan of the brain in different scanners.
MethodsThe present descriptive-analytical cross-sectional study was conducted on 200 patients who had been referred to hospitals of Isfahan city including Kashani (Siemens (scanner 1) and Canon (scanner 2)), Al-Zahra (General Electric (scanner 3)), Isa bin Maryam (Neusoft (Scanner 4)) for brain CT scan. Organ doses and effective doses of patients were obtained with a calculating program, Impact dose, and risk of exposure-induced death (REID) values estimated by the PCXMC program. The image quality was evaluated by examining the parameters of image noise, signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR). To compare different scanners, the CNR to REID ratio was introduced as the C criterion.
FindingsThe average values of REID for scanners 1 to 4 were obtained at 2.98, 4.46, 5.07, and 4.65 per 100,000 men and 3.22, 6.16, 6.53, and 6.26 per 100,000 women, respectively. The average CNR values for scanners 1 to 4 were 1.24, 1.05, 1.66 and 2.08, respectively. Scanners 1 and 2 had the highest and lowest C criteria, respectively.
ConclusionThe results of this study showed that the radiation dose and the risk caused by radiation in the brain CT scan test should always be evaluated considering the image quality parameters, and a scanner with a higher CNR to REID ratio is more suitable.

Intussusception is one of the most common causes of acute abdomen in children. Pediatric surgery centers commonly use barium enemas to treat intussusception. The present study aims to determine the relationship between the time elapsed since the onset of symptoms and the size of intussusception with successful placement with normal saline enema under ultrasound guidance in children.
MethodsThe statistical population included all patients referred to Fatemi Ardabil Hospital for treatment from October 2019 to September 2020 with the diagnosis of intussusception. A total of 144 patients were extracted from the HIS system of the hospital. T-test, Square-Chi, and ANOVA tests were used for data analysis, and the logistic regression test was used for multivariate analysis. The p-value values less than 0.05 were considered significant.
FindingsThe mean and standard deviation of the age of the participants in the study was 16.8 ± 7.8 months. 80.6% of patients out of 144 patients had successful enemas, and 3.4% had symptoms of disease recurrence. The success rate of normal saline enema was not significantly related to age and gender as well as initial symptoms of the disease. However, the time elapsed since the onset of symptoms and the size of intussusception had a significant relationship.
ConclusionAttention to the size of intussusception and quick and correct diagnosis of children with it and normal saline enema as an effective treatment method and, barium enema can be useful for treating intussusception in children.

Patient education and enhancing self-care ability are important for cardiac patients. The study aimed to compare the effect of self-care education by nurses and peers on the self-care ability of Candidate patients for angiography.
MethodsA quasi-experimental design with a pre-post-test was conducted in two groups of candidate patients for angiography in Shahid Madani Heart Hospital of Khorramabad in 2022. Sampling was convenient and the sample size was 56 people. The intervention included self-care education for the patient during two 30-minute sessions before and after angiography by a nurse or a peer. The research tool was Kearney and Flescher's self-care ability questionnaire (1979). Data were analyzed with t, Chi-square, and Fisher's exact tests at a significance level of less than 0.05.
FindingsThe samples included 28 women (50%) and 28 men (50%), married (96.4%), with under-diploma education (75%), housewives or unemployed (48.2%), and with average income status (42.9%). The Mean ± SD of self-care ability in the two groups before intervention was 138.00 ± 12.56 and 133.11 ± 11.08 (P = 0.128), which after the intervention were reached 141.36 ± 11 and 139.11 ± 15.39 (P = 0.001). The two groups were the same in terms of education effectiveness (P = 0.216).
ConclusionSelf-care education by a nurse or peer increases the self-care ability of Candidate patients for angiography. The effectiveness of training by a nurse or a peer on self-care ability is the same. Therefore, depending on workload, the number of nurses, the ratio of nurse to patient, and the time required for caring, a peer can help or replace a nurse to educate the patient.
