ObjectiveTo compare the effect of the physiologic and facilitated labor on the mother and neonate outcomes in the prim gravid women referring to Arash Hospital.Materials And MethodsThis clinical trial study was performed on 200 low risk pregnant women referring to Arash Women’s Hospital in 2012-2013. Mothers were divided into two groups of 100 patients using a simple random sampling method. The first group received the on-pregnancy and physiologic labor training and the second group was nominated for facilitated labor without training. The mother and neonate outcomes in these two delivery methods were then compared.ResultsThe rate of cesarean section in the physiologic group was significantly lower compared with the intervention group (p=0.001). Also in the first stage of labor, VAS was measured to be noticeably lower in the physiologic group in comparison with the intervention group (p=0.001), while the difference of VAS between the two studied groups was found not to be significant in the second stage of labor. In terms of duration of the labor and neonatal Apgar score two groups were not considerably different (p>0.05). Moreover, the laceration rate in the physiologic group was determined to be noticeably higher as compared to the intervention group (p=0.001). The groups were considerably different in terms of the vaginal bleeding and maternal satisfaction (p=0.001).ConclusionThis study revealed the lower rate of cesarean section, abnormal vaginal bleeding and pain score in the physiologic group compared with the facilitated group. Moreover, mothers of the first group were more content with the labor process.Keywords: Physiologic labor, facilitated labor, mother outcomes, neonate outcomes}

مقدمهزایمان قبل از 37 هفته حاملگی، زایمان زودرس نام دارد که یکی از مشکلات مهم مامایی و از عوامل مهم ناتوانی و مرگ و میر نوزادان است. این مطالعه با هدف مقایسه تاثیر دو داروی سولفات منیزیم و نیفیدیپین در درمان زایمان زودرس در بیمارستان علی ابن ابیطالب (ع) زاهدان ازسال1384 تا 1386طراحی و اجرا گردید روش پژوهش: این مطالعه به روش کارآزمایی بالینی و بر روی 110 نفر خانم باردار که برای توقف دردهای زایمانی زودرس بستری شده بودند، انجام شد. بیماران به صورت تصادفی به دو گروه مساوی تقسیم شدند بعد از عدم مهار درد بوسیله استراحت در بستر و مایع درمانی در گروه اول سولفات منیزیم تزریقی و در گروه دوم نیفیدیپین خوراکی تجویز شد. انقباضات رحم به صورت لمسی کنترل شد. آمار توصیفی جهت توصیف داده ها، آزمون تی مستقل جهت مقایسه میانگین ها و از آزمون کای دو جهت مقایسه فراوانی ها بین دو گروه به کمک نرم افزار SPSS.11 استفاده شد.یافته هانتایج نشان داد که بین بیماران از نظر سن حاملگی و پاریته مادر اختلاف آماری معنی داری وجود نداشت. در هر دو گروه 46 نفر (6/83 درصد) بعد از 24 ساعت، دردهای زایمانی مهار شد. همچنین در هر دو گروه از نظر مهار درد های زایمانی بعد از 48 ساعت اختلاف معنی داری را نشان نداد. مجموع عوارض در گروه سولفات منیزیم 5/54 درصد و در گروه نیفیدیپین 22 درصد بود که عوارض در گروه سولفات منیزیم بیشتر بود. نتایج نوزادی در هر دو گروه با هم مشابه بود.نتیجه گیریبا توجه به مساوی بودن نتایج، توقف دردهای زایمانی در گروه سولفات منیزیم و نیفیدیپین بسته به مورد و با در نظر گرفتن سهولت مصرف و عوارض جانبی می توان از هر دو داروی فوق به طور آلترناتیو در اتاق زایمان برای توقف دردهای زایمانی استفاده کرد و با توجه به عوارض جانبی بیشتر در گروه سولفات منیزیم می توان در بیمارانی که فشارخون پایین ندارند از نیفیدیپین برای توقف دردهای زایمانی استفاده کرد.

Preterm labor is one of the major problems in obstetrics, and the greatest cause of morbidity and mortality in the infants. This study was designed to compare the effects of oral Nifedipine and injectable magnesium sulfate for treatment of preterm labor in Ali-ebne-abitaleb hospital in zahdan, from 1384 to 1386.Materials and MethodsThis randomized and clinical trial study was performed on 110 pregnant women who were hospitalized for suppression of preterm labor pain. The participants were divided into two groups, randomly. The pain being not suppressed by bed rest and Fluid therapy, in the first group Magnesium Sulfate infused injection was performed, and the second group were given oral Nifedipine. Uterine contractions were controlled by touching. Descriptive statistics, an independent T-test and Chi-square calculated by SPSS (11) were used for statistical analyses.ResultsThere were no significant statistical differences between groups, regarding the age of pregnancy and mother's parity. The results showed equal labor pains (83.6%) controlled in both groups in the first 24 hours. Also, comparison of labor pains suppression in both groups did not show significant differences after 48 hours. Overall, side effects in magnesium-sulfate-treated group (54.5%) were more than Nifedipine-treated group (22%). Neonatal outcomes were similar in both groups.ConclusionRegarding the equal results of this study, ease of consumption and related side effects depend on the specific case. We can use one of these two drugs for suppression of labor pains, alternatively. Nifedipine can be used in suppression of labor pains instead of magnesium sulfate patients who do not have low blood pressure.Keywords: Preterm labor pain, Magnesium Sulfate, Nifedipine}
