    زمینه و هدف

     پرستاران بخش های مراقبت ویژه به دلیل بار کاری بالا، وظایف محوله خود را اولویت بندی میکنند که می تواند منجر به فراموش شدن برخی مراقبتها شود. از طرفی ثبت دقیق این فعالیتهای مراقبتی بسیار پیچیده و زمان بر بوده و ممکن است بر ارایه مراقبت کامل اثرگذار باشد. لذا مطالعه حاضر با هدف تعیین کیفیت ثبت گزارشات پرستاری و ارتباط آن با مراقبت فراموش شده در پرستاران بخشهای مراقبت ویژه نوزادان شهر تبریز انجام گرفت.

    روش

     این مطالعه توصیفی همبستگی در سال 1401، به روش تمام شماری روی 151 پرستار شاغل در بخش های مراقبت ویژه نوزادان بیمارستان های الزهرا، مردانی آذر و طالقانی تبریز، انجام گرفت. ابزار گردآوری داده ها، پرسشنامه های کیفیت ثبت گزارش پرستاری و مراقبت فراموش شده تابس کالی بود. آنالیز داده ها با آمار توصیفی، آزمونهای همبستگی اسپیرمن و تی در نرم افزار SPSS-26 انجام شد. 

    یافته ها

     میانگین نمره کل کیفیت ثبت گزارشات پرستاران، 8/69±56/19 بود. بیشترین مراقبتهای کاملا فراموش شده، «مشارکت دادن والدین در مراقبت از نوزاد» و «آموزش والدین درباره مراقبت در منزل» بود. تعداد 12 مراقبت با کیفیت ثبت پرستاران ارتباط معنیدار داشتند (0/05> p). از این بین، «رعایت اصول صحیح دارو دادن»، «انجام دستورات دارویی در زمان تعیین شده» و «رعایت احتیاطات کنترل عفونت» بیشترین ضریب همبستگی را دارا بودند (0/001> p).

    نتیجه گیری

     از نظر پرستاران بخشهای مراقبت ویژه نوزادان، کیفیت ثبت های پرستاری بالاتر از سطح متوسط بوده و بیشتر مراقبت ها بطور کامل انجام می شود. در ثبتهای با کیفیت بالاتر، فراموشی مراقبتها کمتر میباشد. با توجه به اینکه در بخش مراقبت ویژه نوزادان جزییات مراقبتی بسیار زیاد است، ثبت دقیق آنها می تواند در کاهش فراموشی مراقبتها مهم باشد.

    Background & aim

    Intensive Care Units nurses prioritize their assigned tasks due to their high workload, which may result in missing some care. On the other hand, accurately documenting these care activities is very complex, time-consuming, and may affect the provision of complete care. Therefore, this study was conducted with the aim of determining the quality of nursing records and its relationship to missed care by Neonatal Intensive Care Units (NICU) Nurses in Tabriz.

    Methods

    This descriptive correlational study was conducted in 2022, using a census study, on all 151 nurses working in the Neonatal Intensive Care Units of Al-Zahra, Mardani, Azar, and Taleghani hospitals in Tabriz. The data collection was performed using the Nursing Report Quality Questionnaire and the Tubbs-Cooley Missed Care Questionnaire. Data analysis was performed using descriptive statistics, Spearman's correlation test, and t-test using SPSS-26.

    Results

    The average score of the total nursing records quality was 56.19±8.69. The most neglected cares were "Parents inclusion in baby’s care" and "Parents education about home management of illness". The number of 12 cares had a significant relationship with the quality of nursing records (p<0.05). Among theme, "Medication administration principles follow", "Medications administration in scheduled time" and "Infection control precautions follow" had the highest correlation coefficient (p<0.001).

    Conclusion

    From the nurses’ perspective in neonatal intensive care units, the quality of nursing records was above the average and most of the cares are provided completely. At higher quality recordings, the missed cares are less likely. Considering that there are many detailed cares in the neonatal intensive care units, recording them accurately can be important in reducing the negligence of care.

    Introduction
    Due to the effect of COVID-19 on nursing care, the methods of communication with patients have changed and ultimately caused nursing care to be postponed or even removed. According to the decrease in COVID-19 patients, the effect of the pandemic has remained in the hospitals, which expressed the necessity of doing this study. 
    Materials and Methods
    350 nurses from four teaching hospitals of the Medical Sciences University of Kurdistan participated in this cross-sectional (descriptive) study using random sampling. Two questionnaires were used: one for missed nursing care and the other for factors associated with missed nursing care. The independent t-test, one-way analysis, and chi-square test were employed in the data analysis using SPSS software version 16. 
    Results
    For missed nursing care in general, the study's score was 40.60. The patient's cooperation and supervision when using the restroom within the first 15 minutes of the request received a score of 2.39, whereas the patient's blood sugar management using a glucometer received a score of 1.24. Additionally, "lack of nursing staff" (score of 3.84) is the most significant instance in examining factors connected with missing nursing care from the perspective of the nurses.
    Conclusion
    Given that the primary cause linked to nursing service gaps is a shortage of nurses, addressing this issue by providing human resources can help.

    Background
    Pediatric nurses are responsible for caring for children and their families; thus, investigating missed care in the pediatric ward is important and helps to improve the quality of care. Accordingly, this study aimed to explain the perceptions of pediatric nurses about missed care.
    Methods
    This study was conducted using content analysis on 15 pediatric nurses selected by purposive sampling. The data were collected through in-depth semi-structured interviews. After obtaining informed consent, interviews were audio-recorded and transcribed verbatim. MAXQDA software (version 10) was used to facilitate coding and thematic analysis.
    Results
    Four main categories emerged from the data analysis, including missed family-centered care, unsafe care, predisposing factors, and adverse outcomes.
    Conclusion
    The findings showed that from the perspective of pediatric nurses, missed care occurs in different dimensions, including psychological support, communication, education, assessment, drug treatment, and infection control, and it can ultimately lead to mother’s dissatisfaction and bring about adverse effects on the child’s recovery besides harming the nurse.

    مقدمه

    مراقبت های پرستاری کودکان متفاوت از مراقبت های پرستاری بزرگسالان است زیرا پرستاران از یک سو مسیولیت مراقبت از کودکان در طیف گسترده ایی از تکامل را بر عهده دارند و از سوی دیگر بایستی از خانواده ی کودک بیمار نیز مراقبت کنند لذا وقوع مراقبت از دست رفته در بخش کودکان می تواند اثرات عمیقی را بر جا بگذارد. مطالعه حاضر با هدف تعیین دیدگاه پرستاران بخش کودکان درباره ی مراقبت پرستاری از دست رفته و عوامل مرتبط با آن در بیمارستان های  شهر یزد در سال 1401 انجام شد.

    روش

    مطالعه ی توصیفی- مقطعی حاضر به شیوه ی تمام شماری بر روی 80 پرستار انجام شد .ابزار جمع آوری داده ها شامل پرسشنامه های دموگرافیک و  مراقبت پرستاری از دست رفته (علل وقوع و انواع) بود. داده های جمع آوری شده در نرم افزاری آماری SPSS نسخه 23 با استفاده از آما توصیفی (میانگین انحراف معیار- فراوانی)تحلیل شد.

    یافته ها

    بیشترین میزان مراقبت از دست رفته بر حسب سه امتیاز بالا از لحاظ میانیگین و انحراف از معیار به ترتیب، "مراقبت از دهان" 29/1± 44/3، " تغیر وضعیت دادن کودک هر 2 ساعت یا طبق دستور" 15/1±  37/3و ارزیابی اثربخشی داروها"19/1 ±   28/3. بررسی علل مراقبت از دست رفته  نشان داد که بر حسب سه امتیاز بالا از لحاظ میانیگین و انحراف از معیار به ترتیب درحیطه ی منابع کاری، "تعداد ناکافی پرستاران" 67/0±   5/3، حیطه ارتباطات " تحویل دادن ناکامل شیفت قبلی یا واحد ارسالی "48/0  ± 77/3 و حیطه  منابع مادی" در دسترس نبودن داروها در صورت نیاز "47/0±   79/3  سه علت اصلی بودند.

    نتیجه گیری

    ضروری است که مدیریت پرستاری در رابطه با اهمیت توجه به مراقبت دهان، استفاده از چک لیست های استاندارد تحویل شیفت و مدیریت نحوه تامین و توزیع داروهای بخش کودکان مداخلاتی را برنامه ریزی نمایند تا گامی در جهت کاهش مراقبت پرستاری از دست رفته  برداشته شود.

    Introduction

    Pediatric nursing care is different from adult nursing care because nurses are responsible for caring for children in a wide range of development and must also care for the sick child's family, so the occurrence of lost care in the department Children can have profound effects.The present study aims to determine the views of pediatric nurses about missed nursing care and factors related to it was conducted in Yazd hospitals in 1401.

    Methods

    The present descriptive-cross-sectional study was carried out on 80 nurses by counting all method. Data were collected through demographic questionnaire  and  missed nursing care questionnaire(factors related and types of) (.Data analysis was done by descriptive statistics (mean -standard deviation - frequency) using SPSS software version 23.

    Results

    The highest amount of missed care in terms of the three points above was respectively related to "oral care" with an average of 3.44+ 1.29,"change position every 2 hours or according to the order with an average of 3.37 ± 1.15 and evaluating the effectiveness of drugs with the average is 3.28 ± 19/1.The investigation of the causes of missed care in three areas showed that in the area of work resources, "insufficient number of nurses" with an average of 3.5± 0.67, the area of communication "incomplete delivery of the previous shift or sent unit" with an average of 3.77±0.48  and the area of resources material "unavailability of drugs when needed" with an average of 3.79 ± 0.47were the three main reasons.

    Conclusion

    It seems necessary that nursing managers  plan interventions regarding the importance of paying attention to oral care, using standard shift delivery checklists and managing the supply and distribution of children's medicine to take a step towards reducing the missed nursing care.

    Background
    The rate of missed care depends on several factors which can affect the quality of care and treatment.
    Aim
    The present study was conducted with aim to investigate the relationship between missed care, work environment and patient safety culture.
    Method
    This cross-sectional study was conducted on 380 nurses working in the teaching hospitals affiliated to Jundishapur University of Medical Sciences in Ahvaz, Iran. Sampling was done through a stratified random sampling method from Jan 2019 to Jul 2020. The data collection tools included a demographic data form, the Missed Nursing Care Questionnaire, the Practice Environment Scale of the Nurse Work Index, and the Hospital Survey on Patient Safety Culture. Data were analyzed using SPSS software (version 25) and one way ANOVA, Pearson correlation and Kolmogorov-Smirnov tests. p<0.05 was considered statistically significant.
    Results
    Three most prevalent missed nursing care were overall patient assessment in each shift (96.1%), administering the drugs up to 30 minutes before or after the scheduled time (94.7%), and measuring the vital signs as ordered (94.1%). The mean scores of missed care were significantly higher among nurses with lower ages (p<0.0001) and less work experience (p<0.0001). A significant inverse correlation was found between missed care and the overall score of patient safety culture (r= -0.22, p<0.0001). Also, a significant correlation was found between missed care and the overall score of the work environment index (r=0.285, p<0.0001).Implications for Practice: Nursing managers can reduce the rate of missed nursing care by improving the working conditions of nurses and educating them about patient safety.

    مقدمه

    مراقبت ازدست رفته، به معنای حذف یا تاخیر در انجام هر جنبه ای از مراقبت موردنیاز بیمار (چه قسمتی از مراقبت و چه کل آن) است. بروز مراقبت ازدست رفته به عنوان یک تهدید مشترک برای کیفیت مراقبت و ایمنی بیمار منجر به عوارض جانبی زیادی برای سلامت بیماران در بخش های اورژانس می شود  این مطالعه به منظور بررسی علل مراقبت های پرستاری ازدست رفته در بخش های اورژانس بیمارستان های منتخب دانشگاه علوم پزشکی تهران انجام شد.

    روش

    این مطالعه از نوع توصیفی مقطعی بود. 146 نفر از پرستاران شاغل در بخش های اورژانس به روش سرشماری در این پژوهش شرکت کردند. معیارهای ورود شامل داشتن مدرک کارشناسی پرستاری و حداقل 6 ماه سابقه کار در بخش اورژانس و رضایت برای شرکت در مطالعه بود. روش جمع آوری اطلاعات خودگزارشی و ابزار جمع آوری اطلاعات دو پرسشنامه اطلاعات دموگرافیک و پرسشنامه مراقبت پرستاری از دست رفته کالیش بود. روایی ابزار به روش روایی محتوا و پایایی آن با روش آلفای کرونباخ با ضریب 743/0 محاسبه شد.  تجزیه و تحلیل داده ها با نرم افزار SPSS16 با استفاده از روش های آمار توصیفی انجام شد.

    یافته ها

    به طور کلی میانگین دلایل مراقبت ازدست رفته 90/2 ±44/0 بود. دلایل مراقبت از دست رفته در حیطه منابع انسانی با میانگین 26/3، منابع مادی با میانگین 82/2 و حیطه ارتباط با میانگین 73/2 به ترتیب بیشترین و کمترین میانگین را کسب کردند.

    بحث و نتیجه گیری

    نتایج مطالعه نشان داد که هر سه حوزه منابع انسانی، منابع مادی و حیطه ارتباط از علل مراقبت ازدست رفته در بخش های اورژانس هستند؛ لذا توصیه می شود جزییات علل مراقبت های پرستاری از دست رفته در هر حیطه مورد توجه مدیران پرستاری قرار بگیرد تا بتوان اقدامات مناسبی را درجهت رفع آن ها انجام داد.

    Background:

     Missed care means elimination or delay of any aspect of patient care (whether part of care or the whole care). Incidence of missed care as a common threat to the quality of care and patient safety leads to many health outcomes for patients in emergency departments. This study was conducted to investigate the causes of missed nursing care in emergency departments of selected hospitals affiliated to Tehran University of Medical Sciences.

    Methods  :

    This cross-sectional and descriptive study was conducted on 146 nurses working in emergency departments of selected hospitals affiliated to Tehran University of Medical Sciences. The samples were selected by census method. Iinclusion criteria included having a bachelorchr('39')s degree in nursing and at least 6 months of experience in the emergency department and consent to participate in the study. The method of data collection was self-report and the data collection tool was two demographic information questionnaires and Kalish Missing Nursing Care Questionnaire. The validity of the instrument was calculated by content validity method and its reliability was calculated by Cronbachchr('39')s alpha method with a coefficient of 0.743. Data analysis was carried out by SPSS-16 software using descriptive statistics.

    Results

    In general, the overall mean score of causes of missed care was 0.44 ± 2.90. The reasons for missed care in the areas of human resources with the mean score of 3.26, material resources with the mean score of 2.82 and communication with the mean score of 2.73 had the highest and lowest mean, respectively.

    Conclusion

    The results of this study showed that causes of missed care are rooted in three areas of human resources, material resources and communication. Therefore, nursing managers are recommended to consider the root causes of missed nursing care in each area of human resources, material resources and communication.

    Background

    Missed nursing care is defined as required care that has been delayed, partially completed, or not completed at all.

    Objectives

    This study aimed to investigate the causes of missed nursing care from the viewpoints of employees in public, private, and social security hospitals.

    Methods

    This descriptive-analytical study was conducted on 453 nurses working in five educational hospitals, four private hospitals, and a social security hospital in Urmia in 2018. Regarding data collection, the “missed care questionnaire” was distributed to nurses by convenience sampling method. The validity of the questionnaire was confirmed by the faculty members of Urmia Azad University, and the reliability of the questionnaire was assessed using Cronbachchr('39')s alpha (0.9). The results were analyzed using descriptive statistics, one-way analysis of variance (p<0.05), and SPSS software version 16.

    Results

    The most common reasons for missed nursing care in educational, private, and social security hospitals were the patientschr('39') emergency conditions (42.9%), the insufficient number of nursing assistants (38.7%), and an unexpected increase in the number of patients (18.5%). Moreover, the causes of missed nursing care in all three areas of communication, financial, and human resources were significantly different between educational, social security, and private hospitals (p=0.0001).

    Conclusion

    Lack of nursing staff, patientschr('39') emergency conditions, and an unexpected increase in the number of patients are the main reasons for missed nursing care in educational, social security, and private hospitals. Therefore, it is recommended to apply appropriate policies to provide nursing staff, manage the admission, transfer, and distribution of patients to the hospital.

    Background

    Missed care is a global phenomenon, which can include many clinical conditions thatthreaten the patients’ safety in all countries and cultures, and also indicates the quality of nursing care.The nursing students’ awareness and understanding of missed nursing care is of great importance. Thecurrent study aims to explore the lived experience of postgraduate nursing students in missed care.

    Methods

    The current qualitative study was performed based on the interpretive phenomenologicalapproach in Tehran, Iran, in February to December 2019. A total of 10 master’s degree nursing studentswere selected through purposive sampling. A total of 10 semi-structured individual interviews wereused to collect the data. The trail version of MAXQDA-10 software was used for coding. All interviewswere recorded and codified, and the main themes were extracted from them using Dicklemann et al.’s(1989) analytical method.

    Results

    Two main themes, five sub-themes, and 31 meaning units were obtained. The main themesincluded: “unfulfilled care” and “living in limbo”.

    Conclusion

    Missed care, as unfulfilled care, is accompanied with living in limbo for nursing students,and this condition is influenced by organizational and personal factors. It seems that managers canprevent missed nursing care by supervising nursing care, reducing the nurses’ workload, creating asense of commitment to work, and enforcing ethical issues among nurses.
