مقدمه
اوتیسم یک اختلال رشدی- عصبی است که سه حوزه تعامل اجتماعی، ارتباط و الگوهای رفتار کلیشه ای از قبیل بیش فعالی، رفتار تکانشی، آسیب به خود و پرخاشگری را به همراه دارد این پژوهش با هدف مقایسه اثربخشی روش تحلیل رفتار کاربردی و بازی درمانی مبنی بر طبیعت (فلورتایم) برشدت علایم ازمون گارز در کودکان مبتلا به اختلال طیف اوتیسم در سال 1401در قم صورت گرفت.
روش کارمطالعه شامل45 نفر نمونه از کودکان اوتیسم بود و مدل شبه آزمایشی از نوع طرح پیش آزمون-پس آزمون و پیگیری با گروه کنترل بود. جامعه آماری تحقیق شامل کلیه کودکان مبتلابه اوتیسم شهر قم در مرکز اوتیسم گلهای بهشت بودند. ابزار مورد استفاده دارای قسمت های مرتط با اطلاعات دموگرافیک، پرسشنامه گارز2 (شدت اوتیسم) و متغیرهای ارتباط، حرکات کلیشه ای و تعامل اجتماعی کودکان بود. نمونه آماری این پژوهش 45 نفر بودند که به صورت نمونه گیری در دسترس انتخاب و به صورت تصادفی در سه گروه آزمایش روش تحلیل کاربردی 15 نفر ،گروه بازی درمانی فلورتایم 15 نفر، و گروه کنترل 15 نفر قرار داده شدند.گروه های آزمایش درمان با روش تحلیل رفتار کاربردی روش تحلیل رفتار کاربردی و مداخله بازی درمانی فلورتایم در جلسات تعیین شده به گروه آزمودنی 23 جلسه 20 دقیقه ای ارائه شد به گروه کنترل هیچ آموزشی داده نشده است.. آزمون (کلموگروف- اسمیرنف) جهت رعایت پیش فرض نرمال بودن توزیع نمرات در جامعه (آزمون موچلی و آزمون لوین)، تحلیل واریانس اندازه گیری مکرر، آزمون تعقیبی بن فرونی برای مقایسه های زوجی اثر گروه ها و زمان ها بر متغیرهای وابسته، نرم افزار مورد استفاده در این پژوهش برای تجزیه و تحلیل اطلاعات نرم افزار SPSS24 وAMOS24 بود.
یافته هاتفاوت اثر دو شیوه تحلیل رفتار کاربردی و بازی درمانی فلورتایم بر مولفه ها و نمره کل شدت علائم اوتیسم معنادار نیست. براین اساس بین اثربخشی تحلیل رفتار کاربردی و بازی درمانی فلورتایم بر شدت علائم در کودکان مبتلا به اختلال طیف اوتیسم تفاوت معنادار وجود ندارد.(0/001).
نتیجه گیریتحلیل رفتار کاربردی و بازی درمانی فلورتایم در دوره اجرا و دوماه پس از اتمام دوره اجرا منجر به کاهش معنادار در میانگین نمرات مولفه های آزمون گارز در شدت علایم ,و درمان اوتیسم شدند از این دو شیوه آموزشی می توان در جهت مراقبت و آموزش بهتر کودکان اوتیسم استفاده نمود.

This research aimed to compare the effectiveness of applied behavior analysis (ABA) and nature-based play therapy (Fluortime) on the severity of Garz test symptoms in children with autism spectrum disorder in 2011 in Qom.
MethodsThe study included 45 autistic children and a quasi-experimental model of pre-test-post-test design and follow-up with a control group. The statistical population of the research included all children with autism in the city of Qom in Golhay Behesht Autism Center. The tool used had parts related to demographic information, Garez questionnaire 2 (severity of autism) and communication variables, stereotyped movements, and social interaction of children. Sampling was done by the available method and they were randomly divided into three test groups: applied analysis method, 15 people, 15 people in the flortime play therapy group, and 15 people in the control group. Floortime plays therapy intervention was provided to the experimental group in 23 sessions of 20 minutes in the designated sessions. The control group was not given any training. Repeated measurement analysis of variance, and post hoc Ben Feroni test for pairwise comparisons of the effect of groups and times on dependent variables were performed, and the software used in this research for data analysis was SPSS24 and AMOS24.
ResultsThe difference in the effect of the two methods of applied behavior analysis and Floortime play therapy on the components and the total score of the severity of autism symptoms is not significant. Therefore, there is no significant difference between the effectiveness of applied behavior analysis and flortime play therapy on the severity of symptoms in children with autism spectrum disorder. (0.001)
ConclusionFunctional behavior analysis and Floortime play therapy during the implementation period and two months after the implementation period led to a significant decrease in the average scores of Garez test components in the severity of symptoms and autism treatment used.

اختلال طیف اتیسم، اختلالی رشدی عصبی است که ازطریق نقص در تعاملات اجتماعی و الگوهای تکراری و محدود رفتار مشخص میشود. پژوهش حاضر با هدف تعیین اثربخشی مداخله درمانی عصبی-حرکتی آنت بنیل بر رفتار کلیشهای و تعاملات اجتماعی کودکان مبتلا به اختلال طیف اتیسم انجام شد.
روش بررسیروش پژوهش نیمه تجربی با طرح پیشآزمون و پسآزمون همراه با گروه گواه بود. جامعه آماری را تمامی کودکان مبتلا به اختلال اتیسم در دامنه سنی 4تا10 سال تشکیل دادند که در سال 1399 به مرکز اتیسم ماندگار شهرستان خمین مراجعه کردند. از این جامعه آماری، سی کودک مبتلا به اختلال اتیسم و دارای شرایط ورود به پژوهش با روش نمونهگیری هدفمند وارد مطالعه شدند. سپس به صورت تصادفی در دو گروه آزمایش و گواه (هر گروه پانزده نفر) قرار گرفتند. از مقیاس رتبه بندی اتیسم گیلیام (گارز-2) (گیلیام، 1995) برای جمعآوری اطلاعات و به عنوان پیشآزمون و پسآزمون استفاده شد. گروه آزمایش روش آنت بنیل را به مدت هشت جلسه نوددقیقهای دریافت کرد؛ درحالی که برای گروه گواه آموزشی ارائه نشد. دادهها با استفاده از آزمونهای شاپیروویلک و تحلیل کوواریانس چندمتغیری با سطح معناداری 0٫05 تجزیه وتحلیل شدند.
یافته ها:
نتایج نشان داد که پس از حذف اثر پیش آزمون، میانگین نمرات رفتار کلیشه ای (0٫89=η، 0٫001>p) و تعاملات اجتماعی (0٫39=η، 0٫008=p) بین دو گروه آزمایش و گواه در پسآزمون تفاوت معناداری دارد.
نتیجه گیری:
باتوجه به یافتههای پژوهش میتوان گفت، روش درمانی عصبی-حرکتی آنت بنیل در پسآزمون منجربه کاهش رفتار کلیشهای و بهبود تعاملات اجتماعی شده است.

Children and adults with ASD have difficulty with verbal and nonverbal communication, social interactions, and play–related activities. This disorder makes it difficult for them to communicate with others. These deficits tend to present in early childhood and usually lead to impairments in functioning across various settings. Moreover, these deficits have negatively impacted adaptive behavior and functioning. Thus, early diagnosis and intervention are vital for future success within this population. The Autism and Developmental Disabilities Monitoring (ADDM) network reports that approximately 1 in 54 children are diagnosed with ASD. The prevalence of this disorder is higher in boys than girls. Currently, no treatment has been shown for ASD, but several interventions have been developed and studied for use with young children. Anat Baniel method is a neuro–movement therapy. Movement provides information the brain needs to grow and organize itself. This method is an approach based on motor learning. Anat believes that the child's awareness of all body movements and the environment leads to awakening the child's brain and its development. The nine essentials form the core of the neuro–movement approach of the Anat Baniel Method. Each of the nine essentials describes one of the brain's requirements for waking up and doing its job well. These nine essentials include movement with attention, slowness, variation, subtlety, enthusiasm, flexible goals, the learning switch, imagination & dreams, and awareness. The main elements of this intervention method include encouraging the child to play sensory–motor games, accompanying the child to experience each other's movements, increasing awareness of the senses and learning new motor skills, strengthening the child's different senses, and increasing the ability to understand differences better. The purpose of this research was to study the effectiveness of Anat Baniel neuro–motor therapy on stereotyped behavior and social interactions of children with ASD.
MethodsThis research was quasi–experimental with a pretest–posttest design and a control group. The statistical population includes all children with autism aged 4–10 years who were referred to Khomein Mandegar Autism Center, Khomein City, Iran, in 2020. From whom, 30 children with autism disorder were selected by convenience sampling. The inclusion criteria were lacking physical disabilities, having an IQ between 50 and 70 (educable), and having parents' consent. The exclusion criteria were severe mental disabilities and severe physical disabilities.
Then, they were assigned into experimental and control groups. The GARS–2 (Gilliam Autism Rating Scale) test was used to collect information in the pretest and posttest. Gilliam Autism Rating Scale, second edition, is a screening tool for autism spectrum disorder for individuals between the ages of 3 and 22. It was designed to help differentiate those with autism from those with severe behavioral disorders. Coefficients of reliability of the GARS–2 test for the subscales and Autism Index range from 0.80 to 0.90. To conduct the research, in the first session, the GARS–2 test questionnaire was completed as a baseline (pretest), and the informed consent form was completed by the children's parents. After the pretest, the interventions were performed by the occupational therapist of the Center Autism and the first researcher, so the experimental group received the Anat Baniel intervention for eight 90–minute sessions. However, the control group received only routine rehabilitation interventions.ResultsThe ANCOVA data showed a significant difference between the mean of stereotyped behavior (p<0.001, η=0.89) and social interaction (p=0.008, η=0.39) in the experimental and control groups.
ConclusionEvidence was found to support Anat Baniel intervention (ABM) of neuro–motor therapy as an effective tool in reducing stereotypical behaviors and improving social interactions in children diagnosed with ASD. The results of the present study can be used for treatment planning and clinical interventions, and by teaching this treatment to parents and educators of rehabilitation centers and children with autism, a step was taken to improve the performance of these children.

اختلال اتیسم ، اختلالات رشدی عصبی هستند که با مشکلاتی در ارتباط و تعاملات اجتماعی ،رفتارها، علایق و فعالیت های محدود و تکراری مشخص می شود. هدف از پژوهش حاضر مقایسه اثربخشی مداخله درمانی عصبی - حرکتی آنت بنیل و برنامه سان - رایز برتعاملات اجتماعی و رفتار کلیشه ای کودکان مبتلا به اختلال اتیسم است.
مواد و روش ها:
روش پژوهش نیمه تجربی با طرح پیش آزمون - پس آزمون با گروه گواه بود . جامعه آماری شامل تمامی کودکان مبتلا به اختلال اتیسم دردامنه سنی 10-4ساله بودند که در سال 1399 به مراکز اتیسم شهرستان خمین و اراک مراجعه کرده اند. از جامعه آماری مذکور تعداد 30 کودک مبتلا به اختلال اتیسم و دارای شرایط ورود به پژوهش با روش نمونهگیری هدفمند وارد مطالعه شدند سپس بصورت تصادفی دردو گروه آزمایش ویک گروه گواه هر گروه 10 نفر قرار گرفتند. از آزمون گارز -2 (GARS-2) جهت جمع آوری اطلاعات و بعنوان پیش آزمون و پس آزمون استفاده شد.گروه آزمایش یک ، مداخله آنت بنیل را بمدت 8 جلسه 90 دقیقه ای دریافت کردند وگروه آزمایش دو برنامه سان رایز را بمدت 40 ساعت بصورت فشرده دریافت کردند در حالی که گروه گواه آموزشی دریافت نکردند. داده ها با استفاده ازآزمونهای شاپیروویلک وتحلیل کواریانس چند متغیری سطح معناداری 5 درصد مورد تجزیه و تحلیل قرارگرفتند.
یافته ها:
نتابج نشان داد بین میانگین تعامل اجتماعی و رفتار کلیشه ای درگروه های آزمایش و گروه گواه تفاوت معناداری وجود دارد و با توجه به اندازه اثر می توان بیان کرد بیشترین اثر بخشی روش های درمانی بر روی رفتار کلیشه ای بوده است . همچنین یافته ها نشان داد که برنامه سان رایز در بهبود تعاملات اجتماعی کودکان مبتلا به اختلال اوتیسم به طور معنی داری اثر بخشی بیشتری از مداخله عصبی - حرکتی انت بنیل دارد.

Methods and Materials: This research was a semi-experimental study with pre-test-post-test with control group design. The statistical population includes all children with autism in the age of 4-10 years who referred to the autism centers of Khomein in 2021. From the statistical population, the number of 30 children with autism disorder and the conditions of entry into the research were included in the study with the purposeful sampling method, then they were randomly divided into two experimental groups and a control group of 10 people in each group. GARS-2 test (Gilliam Autism Rating Scale) test was used to collect information as a pre-test and post-test. The data were analyzed using Shapiro-Wilk tests and multivariate covariance analysis at a significance level of 0.05.FindingsThe findings showed that there was a significant difference between the mean of stereotyped behavior (p<.001, η=.68) and social interaction (p= .001, η=.90) in the experimental and control groups. Also, the findings showed that the Son-Rise program is significantly more effective in improving the social interactions of children with autism disorder than anat baneil's neuro-motor intervention.
ConclusionsEvidence was found to support Anat Baniel's Method (ABM) of neuro-motor therapy and the Son-Rise program in the post-test have led to the reduction of stereotyped behavior and improvement of social interactions of children with autism disorder and the Son-Rise program has been more effective in improving the social interactions of these children

اختلالهای طیف اوتیسم اختلالاتی رشدیعصبی هستند که بهوسیله نقص در تعاملهای اجتماعی و ارتباطات و رفتارهای تکراری و محدود مشخص میشوند. هدف از انجام این پژوهش بررسی تاثیر تحریک الکتریکی مستقیم مغز بر بهبود رفتار کلیشهای و برقراری ارتباط در کودکان مبتلا به اختلالهای طیف اوتیسم بود.
روشبررسیپژوهش حاضر از نوع کارآزمایی بالینی با طراحی متقاطع بود. در این مطالعه 20 کودک مبتلا به اختلالات طیف اوتیسم مراجعهکننده به مراکز درمانی اعصابوروان استان یزد در سال 1392 و 1393، انتخاب شده و در دوگروه A و B قرار گرفتند. نمرههای رفتار کلیشهای و برقراری ارتباط در پیشآزمون، بعد از تحریک ساختگی، بعد از تحریک واقعی و دوماه پس از درمان مقایسه شد که این دادهها ازطریق آزمون تشخیص اوتیسم گارز (گیلیام، 1994) بهدست آمد. مداخله شامل ده جلسه تحریک الکتریکی مستقیم با جریان یکمیلیآمپر بود. بیماران گروه A در ابتدا پنج جلسه تحریک ساختگی و بیماران گروه B، پنج جلسه تحریک واقعی دریافت کردند. پس از گذشت پنج جلسه درمانهای دو گروه جابهجا شد. تجزیهوتحلیل دادهها در دو سطح توصیفی (میانگین و انحراف معیار) و استنباطی (آزمون آنالیز واریانس با اندازهگیری مکرر و بونفرونی) توسط نرمافزار SPSS نسخه 21 در سطح 95درصد اطمینان انجام شد.
یافتههانتایج این پژوهش نشان داد نمرههای رفتار کلیشهای و برقراری ارتباط بعد از درمان واقعی (0٫041=P برای رفتار کلیشهای و (0٫031=P برای برقراری ارتباط) و در دوره پیگیری (0٫047=P برای رفتار کلیشهای و 0٫038=P برای برقراری ارتباط) درمقایسه با پیشآزمون بهطور معناداری کاهش پیدا کرد.
نتیجهگیریباتوجه به نتایج، تحریک الکتریکی مستقیم از روی جمجمه میتواند در برنامه توانبخشی این بیماران استفاده شود.

Symptoms of this disorder occur before three years old, and the cause of the disease is unknown. This disorder is more common in boys than in girls. The economic, social, lifestyle and parental education level does not play role in the development of autism. The disorder affects the normal development of the brain in the context of social interactions and communication skills. Children and adults with autism, in verbal and non-verbal communication, social interactions and game-related activities have some troubles. The disorder makes it difficult to communicate with others and the outside world for them. In some cases self-harm and aggression are seen. In these people, repetitive movements (touching, jumping) are unusual responses to people, attachment to objects, or resistance to change, and there may be unusual sensations in the five senses (sight, hearing, odor, smell, and taste). The central core of autism is a disorder in communication. One in every 60 to 70 live births in the world is a person with autism. Study results revealed that pregnant women who are obese or diabetic are more likely to having autism child than other mothers. Direct current stimulation of the brain is a technique that is used to stimulate the different area of the brain in neurologic and psychiatric patients. This study was aimed to evaluate the effect of transcranial direct current stimulation (Tdcs) of the brain to the improvement of stereotype behavior and establishment of communication in children who are suffering from autism spectrum disorder and the duration of its effects.
MethodsIn this randomized crossover clinical trial, 20 children choosed with an autism spectrum disorder. They referred to the psychiatric centre in 2013-14 in Yazd province and then divided into two-period treatment groups. Stereotype behavior and communication scores of these children measured in the pre-test. The survey did after artificial and actual stimulation and two months later. Data collected by Gilliam autism rating scale (GARS). Group A patients, received artificial stimulation in 5 sessions, and the patient of group B received actual stimulation by Tdcs. After five sessions, the treatment of the two groups replaced. GARS scale is a norm-referenced instrument that assists teachers and clinicians in identifying and diagnosing autism in individuals between 3 to 22 years and in estimating the severity of the child's disorder. This scale calculated by Hemmatiyan et al. in Iran. In their research to assess the construct validity, the Karz questionnaire used simultaneously, and the correlation coefficients of these two questionnaires were 0.08. Comparing 100 healthy children and adolescents determined diagnostic validity through a differential analysis test. The cut point of 52 tests and the sensitivity and scale feature obtained 99% and 100%, respectively. The reliability of this scale was also estimated using Cronbach's alpha coefficient of 0.89.
ResultThe result illustrate there was improvement in stereotype behaviors (p<0.05) and communication (p<0.05) in children with autism spectrum disorder after this treatment.
ConclusionRegarding to the result of this study it seemed that transcranial direct current stimulation could improvement of stereotype behavior and communication in autistic children.

اختلال طیف اوتیسم، یک اختلال عصبی تحولی است که در سال های اولیه کودکی ظاهر می شود و درمان قطعی برای آن وجود ندارد. روش های مختلفی برای آموزش و توانبخشی این اختلال وجود دارد. بر این اساس پژوهش حاضر با هدف مقایسه اثر تمرین در آب، مکمل یاری ویتامین D، و ترکیب تمرین در آب+مکمل یاری بر رفتارهای کلیشه ای و شاخص توده بدن در پسران مبتلا به اختلال طیف اوتیسم انجام شد.
روشپژوهش آزمایشی حاضر با استفاده از طرح پیش آزمون-پس آزمون با گروه های تصادفی انجام شد. بدین ترتیب 40 کودک مبتلا به اختلال طیف اوتیسم با میانگین سنی 14/2 ± 9/10 سال از موسسه خیریه انجمن اوتیسم گیلان در سال 1397به روش نمونه گیری در دسترس، انتخاب شده و به تصادف در چهار گروه تمرین در آب (10 نفر)، مکمل یاری ویتامین D (10 نفر)، ترکیب تمرین+مکمل (10 نفر)، و گواه (10 نفر) جایدهی شدند. قد، وزن، و شاخص توده بدن کودکان در ابتدا و در پایان پژوهش، اندازه گیری شد. گروه های آزمایشی تمرین در آب و تمرین+مکمل به مدت 10 هفته، هفته ای 2 جلسه 60 دقیقه ای به اجرای برنامه تمرین در آب پرداختند. همچنین گروه های مکمل یاری و تمرین+مکمل به مدت 10 هفته، هفته ای یک بار قرص ویتامین D با دوز 50000 IU مصرف کردند. والدین کودکان مورد مطالعه، قبل از شروع آزمایش و پس از پایان 10 هفته مداخله، خرده مقیاس رفتارهای کلیشه ای مربوط به ویرایش دوم مقیاس درجه بندی اوتیسم گیلیام (گارز-2) را تکمیل کردند. در پایان برای تحلیل داده ها از آزمون تحلیل کوواریانس استفاده شد.
یافته هانتایج تحلیل داده ها نشان داد که هر سه روش تمرین در آب، مکمل یاری ویتامین D، و ترکیب تمرین+مکمل، تاثیر معنی داری بر نمره خرده مقیاس رفتارهای کلیشه ای و شاخص توده بدن در کودکان اوتیسم داشت (0/05>p)، اما روش تمرین+مکمل، اثر بیشتری نسبت به دو روش دیگر نشان داد.
نتیجه گیریبراساس نتایج پژوهش حاضر می توان نتیجه گرفت که شنا می تواند درون داد حسی ثابتی فراهم کند و سطح فعالیت جسمانی کودکان با اختلال طیف اوتیسم را بدون تحمل وزن، افزایش دهد. همچنین، با توجه به کمبود قابل توجه ویتامین D در این کودکان، مکمل یاری ویتامین D می تواند موجب بهبود شاخص توده بدن و کاهش شدت علایم اختلال شود.

There are various methods for training and rehabilitation of this disorder. So, the aim of the present study was to compare the effect of aquatic exercise and vitamin D supplementation on stereotypic behaviors and body mass index (BMI) in boys with ASD.
MethodThe present experimental study was conducted using a pretest-posttest design with random groups. Thus, 40 children with ASD (mean aged 10.9±2.14 years) were selected according to convenience sampling and randomly assigned to four groups of aquatic exercise (n=10), vitamin D supplementation (n=10), aquatic+supplementary (n=10) and control (n=10). Height, weight and BMI of children were measured at the beginning and the end of intervention. Aquatic exercise and aquatic+supplementary experimental groups were received the 60-minute water training program for 10 weeks, 2 sessions per week. The Vitamin D supplements and aquatic-supplementary groups also received vitamin D tablets at a dose of 50,000 IU once daily for 10 weeks.
Parents of autistic children completed the subscale of the Gilliam Autism Rating Scale (GARS-2) subscale before the experiment and after the end of the 10-week intervention. Finally, covariance analysis test was used for data analysis.ResultsThe results of data analysis showed that all three methods had a significant effect on the subscale of stereotypical behaviors and body mass index in autistic children (p<0.05), but the aquatic-supplementary method showed more efficacy than the other two methods.
ConclusionBased on the results of this study, it can be concluded that swimming can provide a stable sensory input and increase the level of physical activity of children with autism spectrum disorder without weight bearing. Also, given the significant deficiency of vitamin D in these children, vitamin D supplementation can improve body mass index and decrease the severity of symptoms.

هدف از این پژوهش مقایسه رفتار کلیشه ای، خود پنداره و جامعه پذیری دانش آموزان مدارس مختلط و غیرمختلط مقطع ابتدایی در منطقه ی شاوور بود.
روش کارروش پژوهش علی مقایسه ای بود. جامعه آماری این پژوهش را دانش آموزان مدارس مختلط و غیرمختلط مقطع ابتدایی در منطقه ی شاوور و نمونه آن را 370 نفر دانش آموز (166 نفر از مدارس مختلط و 204 نفر از مدارس غیرمختلط) که به روش نمونه گیری خوشه ای تصادفی انتخاب شدند، تشکیل می داد. برای جمع آوری داده ها از مقیاس های پرسشنامه سازگاری اجتماعی بل، پرسشنامه نقش جنسیتی بم و پرسشنامه خودپنداره تحصیلی یی یسن چن استفاده شد. به منظور تجزیه و تحلیل داده ها از روش تحلیل واریانس دوطرفه استفاده شد.
نتایجنتایج نشان داد بین کلیشه های جنسیتی، خودپنداره و جامعه پذیری دانش آموزان دختر و پسر تفاوت معنا داری وجود دارد.
نتیجه گیریعلاوه بر این، نتایج جدول نشان می دهد بین کلیشه های جنسیتی، خودپنداره و جامعه پذیری دانش آموزان مدارس مختلط و غیرمختلط تفاوت معنا داری وجود دارد (p<0/001).

MethodsThe research method was causal comparative. The statistical population of this study consisted of students from mixed and non-mixed elementary schools in Shavur district. Would give. The data were collected using Bale's social adjustment questionnaire, Bam's gender role questionnaire and Yi Yen Chen self-concept questionnaire. Two way analysis of variance was used to analyze the data.
ResultsThe results showed that there was a significant difference between gender, self-concept and socialization of male and female students.
ConclusionIn addition, the results of the table show that there is a significant difference between gender, self-concept and socialization of mixed and non-mixed school students (P<0/001).

زمینه و هدفیکی از روش های درمانی نوینی که توجه درمانگران حوزه اختلال اتیسم را به خود جلب کرده است، مداخله های مرتبط با فعالیت های حرکتی و بدنی است. این پژوهش با هدف بررسی تاثیر مهارت های دیداری- حرکتی کپارت بر کاهش رفتارهای کلیشه ای کودکان مبتلا به اختلال طیف اوتیسم انجام شد.روشپژوهش حاضر از نوع مطالعات تک بررسی است. نمونه مورد مطالعه شامل 3 نفر از کودکان مبتلا به اختلال طیف اتیسم در دامنه سنی 5 تا 14 سال شهر تهران در سال 97-1396 بود که با روش نمونه گیری در دسترس انتخاب شدند و در 10 جلسه 45 دقیقه ای مورد مداخله قرار گرفتند. در این پژوهش نخست به وسیله ثبت فاصله زمانی، خط پایه رفتار کلیشه ای مشاهده و ثبت شد و برنامه های مداخله ای ادراک دیداری- حرکتی کپارت ارائه شد و سپس به وسیله ثبت فاصله زمانی تعداد وقوع رفتار کلیشه ای، بلافاصله بعد از برنامه درمانی در دو مرحله مورد مشاهده و ثبت قرار گرفت.یافته هانتایج تحلیل دیداری درون موقعیتی و بین موقعیتی برای داده های آزمودنی میانگین نمرات مربوط به رفتارهای کلیشه ای برای هر 3 آزمودنی به ترتیب به طور معناداری از مرحله خط پایه (8/12، 10، 8/4) به مرحله مداخله (4/3، 2/4، 2/2) کاهش پیدا کرده است. همچنین شاخص PND نشان می دهد که میزان همپوشی بین نقاط خط پایه و مدخله با 100 درصد اطمینان موثر بوده است.نتیجه گیرینتایج نشان می دهد که تمرین های ادراکی حرکتی مبتنی بر دیدگاه کپارت می تواند به عنوان یکی از روش های کاهش رفتار کلیشه ای کودکان اوتیستیک به کار گرفته شود.کلید واژگان: مهارت های دیداری- حرکتی کپارت, رفتار کلیشه ای, اختلال طیف اتیسم}Background and Purpose: Among the new therapeutic methods that have attracted the attention of autistic therapists are the interventions related to motor and physical activities. This study was conducted to investigate the effect of Kephart's visual-motor skills on reducing the stereotypic behaviors of children with autism spectrum disorders.MethodThe present study was a single-case study. The study sample consisted of three 5-14-year-old children with autism spectrum disorder living in Tehran during 2017-2018, selected through convenience sampling. They underwent an intervention of 10 sessions of 45 minutes.
Initially, stereotypic behaviors were observed at the baseline and recorded by interval recording. After that, Kephart's visual-motor perception intervention programs were presented, and then again the occurrence of stereotypic behaviors was immediately observed and recorded by interval recording in two stages.ResultsThe results of in-situ and inter-situ visual analysis of the subjects' data showed that the mean score of stereotypic behaviors for all three subjects decreased significantly from the baseline (4.8, 10, and 12.8) to the intervention phase (2.2, 4.2, and 3.4). Also, PND showed that the overlap of baseline and intervention points was effective with 100 percent confidence.ConclusionResults indicate that the perceptual motor activities based on Kephart approach, can be used as one of the methods to decrease the stereotypic behaviors in children with autism.Keywords: Kephart's visual-motor skills, stereotypic behavior, autism spectrum disorders} -
مقدمهاختلال اتیسم، از شایع ترین اختلالات رشدی در میان کودکان است که در سال های اولیه کودکی به شکل تخریب کیفی در تعامل اجتماعی و الگوهای تکراری و کلیشه ای در رفتار و علایق و فعالیت ها ظهور می کند. الگوهای کلیشه ای و تکراری در رفتار، اختلال در توجه و تمرکز، رشد شناختی و همچنین، آسیب هایی در عملکرد اجتماعی و مهارت های ارتباطی را در این کودکان به دنبال دارد که در فعالیت های روزمره از بازی گرفته تا فعالیتهای تحصیلی کودک، مداخله می کند. این نشان می دهد که رفتارهای کلیشه ای به هر نحو ممکن باید کاهش یابد. از طرفی، برخی محققین دلیل این الگوهای کلیشه ای در این کودکان را نوعی نارسایی حسی در برابر محرکات حسی در نظر می گیرند. بنابراین، مقاله حاضر با هدف مرور منابع به منظور بررسی تاثیر درمان های مبتنی بر پردازش و یکپارچگی حسی بر الگوهای کلیشه ای کودکان مبتلا به اتیسم تدوین شد.مواد و روش هااین مطالعه یک پژوهش مروری بود. برای یافتن مطالعات مرتبط با هدف پژوهش، جستجو در پایگاه های انگلیسی Ot seeker، Google scholar، Scopus و PubMed با استفاده از کلمات کلیدی انگلیسی Sensory processing، Sensory integration autism، Stereotypic behavior و پایگاه فارسی Scientific Information Database (SID) با واژگان کلیدی فارسی اتیسم، یکپارچگی حسی، پردازش حسی و رفتار کلیشه ای انجام گرفت و در نهایت 12 مطالعه مرتبط بر اساس معیارهای ورود انتخاب شد.یافته هامرور منابع نشان داد که رویکرد یکپارچگی حسی در بهبودی تعامل و کاهش حرکات کلیشه ای اتیستیک در کودکان مبتلا به اختلال اتیسم، اثرات مثبت داشته است. نتیجهگیری: به نظر می رسد که این نوع درمان، می تواند در ترکیب با مداخله رفتاری، در کاهش رفتارهای کلیشه ای کودکان مبتلا به اختلال اتیسم موثر باشد.کلید واژگان: اتیسم, یکپارچگی حسی, پردازش حسی, رفتار کلیشه ای}IntroductionAutism disorder is one of the most commonly occurring developmental disorders among children, which appears in early childhood in the form of degradation in the quality of social interaction, and repetitive and stereotypic patterns of behavior, interests, and activities. The stereotypical and repetitive patterns of behavior, make disturbance in attention and concentration, and cognitive development, and also damage to social performance and communication skills in these children, which interfere in daily activities from play to child's educational activities; this suggests that stereotypical behaviors should be minimized in any way. Whereas some scholars consider this stereotypic patterns in this children as a sensory failure to sensory stimulant. The purpose of this article was to review the effect of interventions based on sensory processing and sensory integration on the stereotypical patterns of children with autism.Materials and MethodsThis was a narrative review. To find the studies related to purpose of research, using the keywords autism, sensory integration, sensory processing, and stereotypic behavior electronic, databases including Google Scholar, Scopus, PubMed, Ot seeker, and Scientific Information Database (SID) were searched. Finally, 12 articles were selected based on the inclusion criteria.ResultsReview of the literature demonstrated that sensory integration approach had positive effect on interaction, and reduced autistic mannerism in children with autism disorder.ConclusionIt seems that intervention based on sensory integration in combination with behavioural intervention can be effective in reduction of stereotypic behaviours in children with autism disorderKeywords: Autism, Sensory integration, Sensory processing, Stereotypic behaviour}

هدفاختلال طیف اتیسم با نقص در مهارت های ارتباطی، رفتارهای کلیشه ای و نقص زیاد در تعاملات اجتماعی همراه است. یکی از مشکلات رفتاری که به درجات مختلف در کودکان مبتلا به اختلال طیف اتیسم دیده می شود، رفتارهای کلیشه ای است. پژوهش حاضر با هدف بررسی تاثیر ترکیب درمان یکپارچگی حسی-حرکتی و اصلاح رفتار بر رفتار کلیشه ای کودکان اختلال طیف اتیسم انجام شد.روش بررسیاختلال طیف اتیسم با نقص در مهارت های ارتباطی، رفتارهای کلیشه ای و نقص زیاد در تعاملات اجتماعی همراه است. یکی از مشکلات رفتاری که به درجات مختلف در کودکان مبتلا به اختلال طیف اتیسم دیده می شود، رفتارهای کلیشه ای است. پژوهش حاضر با هدف بررسی تاثیر ترکیب درمان یکپارچگی حسی-حرکتی و اصلاح رفتار بر رفتار کلیشه ای کودکان اختلال طیف اتیسم انجام شد.یافته هانتایج نمودارهای چشمی حاکی از تاثیر ترکیب درمان یکپارچگی حسی-حرکتی و اصلاح رفتار بر رفتار کلیشه ای کودکان اختلال طیف اتیسم بود. نتایج بررسی تغییرات ایجاد شده از طریق اندازه اثر کوهن، حاکی از آن بود که اندازه اثر ترکیب درمان یکپارچگی حسی-حرکتی و اصلاح رفتار، بر رفتار کلیشه ای کودکان اختلال طیف اتیسم در مرحله خط پایه-درمان زیاد و اندازه اثر در مرحله درمان-پیگیری کم و متوسط بود؛ بنابراین می توان گفت که ترکیب درمان یکپارچگی حسی-حرکتی و اصلاح رفتار بر رفتار کلیشه ای کودکان مبتلا به اتیسم موثر بوده است.نتیجه گیریاز نتایج پژوهش حاضر می توان در راستای برنامه ریزی درمانی و مداخلات بالینی، به منظور کاهش و درمان رفتار کلیشه ای کودکان با اختلال اتیسم استفاده کرد.کلید واژگان: یکپارچگی حسی-حرکتی, اصلاح رفتار, رفتار کلیشه ای, اختلال طیف اتیسم}ObjectiveAutism spectrum disorder (ASD) with impairment in communication skills, stereotypes and significant impairment in social interactions. The aim of this study was the effectiveness Combination of sensory integration training and behavior modification to improvement of stereotyped behavior in children with autism spectrum disorder.MethodologyThe research procedure was single subject design (A-B-A with baseline, treatment and follow up steps). The sample group was included 4 subjects that were selected by purposeful sampling method. Autism children boys participated in 20 sessions of combination of sensory integration training and behavior modification. Assessment instrument was Gilliam Autism Rating Scale (GARS). The analysis of the results was performed using the visual inspection and Cohen's d effect size.ResultsThe result of visual inspection indicates that the effectiveness of combination of sensory integration training and behavior modification on improvement of stereotypic behaviors in in ASD children. The result of Cohen's d effect size indicates that is large effect size combination of sensory integration training and behavior modification on stereotypic behaviors in treatment-follow.
