Effect of local anesthetic Bupivacaine at reliefing postoperative pain after caesarean section

Abstract:
Background

Post operative pain is one of the most important causes of patient's fear.according to International Pain Association pain is unpleasure sense, and an emotional experience with tissue truma.In spite of knowldge in analgesic drugs pharmacology and physiology of pain and also great development in the ways of pain control,yet the most patient complain from post operative pain and the essential problem is how to control the post operative pain in surgery wards.The impact in unfairble effect of pain physiology lead to long hospitalization and more cost.usually opoid drugs and NSAIDS are used to relifing pain in surgry wards. According to side effects that occasionally lead to patient death.This study has been accomplished to evaluate effects of local anesthetic bupivacaine to reliefi post caesarean section pain.

Material and Methods

In This study 60 nuligravida patients scheduled for elective cesarean under general anesthesia, aged 18-40 years, without medical problem, were investigated in a randomized clinical trail,in Moatazedi hospital.patients were randomaly allocated between two groups to recive 0.3 ml/kg bupivacaine %0.5 (study group) after clousurre of fascia the subcutaneous tissues were infiltrated and control groups has no infiltration bupivacaine. pain score and analgesic consumption were comparied between groups during first 24 hours .The data were analysed by ch-squar, manwhitny and T-Test.

Finding

This study demonstrated that the mean of pain intensity was 9.83 ± 2.21 in study group and 12.3 ± 1.44 in cotrotral group. that there was statistically significant differences between the groups (P=0.001).also the mean of analgesic consumption was 0.8 ± 0.76 in study group and 1.86±0.68 in control group that there was significant differences between groups (P=0.001). The mean of the cost in study group was 48068 ± 446.60 Rials and in control group 1151.33 ± 402.5 that there was statistically significant differences between the groups (P=0.001).

Conclusions

This study demonstrate that subcutaneous wound infiltration with bupivacaine%0.5,inspite of a great deal of costing is an easy technic and has no need special instruments or skills, and in comparison with other ways of control post operative pain, has the few side effect, because no one of patient had special problem during the study. Where as, there is not possibiling of using more opiod ( the potent analgesic). perhaps this way is better and practical for replaceing, and provide comfortness and pain's relief at the first hours after the operation.
